An international meeting of experts on Cardiovascular Imaging by Ultrasound was held in Aachen from 26-27 April, 1991. It provided new and interesting insights into what has already been achieved in ultrasound-based cardiovascular diagnosis and therapy and what will be introduced in clinical practice in the near future. Since the introduction of ultrasound in clinical practice in 1984 there has been no other physical principle that has added and will continue to add so much to clinical diagnosis and therapy. Echocardiography, once established as a non-invasive diagnostic tool, is increasingly becoming an invasive technique for cardiovascular imaging. This book contains the edited contributions from 38 scientists and engineers from all over the world who presented the most up-to-date findings on 2-dimensional echocardiography, different Doppler modalities, contrast and stress echocardiography and the different modalities of transesophageal echocardiography, including mono-, bi- and multiplane TEE, as well as pulsed and CW-Doppler application via TEE. Exciting and promising developments are discussed in the field of intravascular ultrasound, tissue characterization, ultrasound ablation, ultrasound-based 3-dimensional reconstruction of the heart, and high frequency Doppler analysis.

Today, coronary artery disease is one of the major causes of mortality and morbidity in the Western World. In the last decade many major diagnostic and therapeutic advances have been made, considerably furthering our potential in the management of coronary artery disease. At the same time, a new generation of cardiac tools has appeared. The field which has, perhaps, undergone the most important technological innovations is echocardiography. Nowadays, in fact, the world of ultrasounds ofters the cardiologist a wide range of technical applicatons: two-dimensional real-time imaging, intra-and extra-cardiac Doppler flow measurements, real-time imaging of cardiac struc­ ture and flow by 2D color Doppler, high resolution cardiac imaging by transesophageal echocardiography, tissue characterization by analysis of ultrasound wave characteristics, information on myocardial perfusion by con­ trast echocardiography, etc. Thanks to these technical improvements and to its consequent increased potentiality, echocardiography now plays an impor­ tant and irreplaceable role in the management of all cardiac diseases. In the field of coronary artery disease, echocardiography can reliably be used not only in the acute phases of the disease to derive useful functional and prog­ nostic information but also as a stress diagnostic procedure (thanks to new stress modalities and the continuing improvement of reviewing digital sys­ tems) for the diagnosis of coronary artery disease and for the evaluation of various therapeutic interventions. Furthermore, other promising applications of ultrasounds in this disease are currently being investigated: tissue charac­ coronary artery anatomy terization, myocardial contrast echocardiography, and flow evaluation by specially-designed ultrasoundcatheters.

This monograph represents an attempt to collect the methods developed within the past decade in which echocardiography was being used to guide or to assess the results of some cardiovascular interventions or to overcome the imaging limitations of transthoracic approaches. I have entitled this book Echocardio­ graphy in Cardiac Interventions, although I am aware that this title is not entirely applicable to all the included methods. In these interventive procedures echocar­ diography preserves its noninvasive nature, and by combining it with invasive procedures, they become more accurate, safer and less invasive. The book is divided into 8 sections and 42 chapters written by many authorities in the field. When such comprehensive contributions from many authors are used, some overlapping cannot be avoided. Almost every effort in the care of patients with heart disease begins with some description of disordered physiologic performance or mor­ phologic anatomy. Since the early work of Edler and Hertz in 1954, echocardiographic methods have grown in importance and reliability for the diagnosis of many cardiac disorders. The placement of a maneuverable transducer on the tip of a modified endoscope is the result of relatively recent technologic advances. The transesophageal approach is now a reality for obtaining new information from ultra-sonic images of a beating human heart. Since images obtained by transesophageal echo cardiography are uniformly of excellent quality, it extends the diagnostic potential of echocardiography to the patient who is difficult to image from the conventional chest wall approach. More importantly, transesophageal echocardiography provides a means to acquire useful information in new situations, such as the operating room. When this imaging modality is brought to patients undergoing surgical procedures, surgeons and anesthesiologists have a ready means for assessing cardiac performance during anesthesia, directing various surgical approaches and immediately evaluating the results of surgical repair. Transesophageal echocardiographic techniques represent a major vi vii advance in the care of patients with cardiovascular disease. Never before has there been a means to acquire such important information about the heart during an operative intervention. New questions are being asked and new answers are at hand.

Present day cardiology is in great need of non invasive, non toxic, and inexpensive devices which permit the delineation and visualization of normal and abnormal intracardiac structures, the calculation of intra­ cardiac volumes and study of contractility of the cardiac muscle. All of these may become within our reach if the principles outlined in this book and the preliminary clinical experience can be validated in general cardiac practise. Why a book on contrast echocardiography? Over the past dozen years enough experience has accumulated to warrant a more extensive treatment of this method. Furthermore, there are new developments that suggest increased clinical utility for contrast echocardiography in the future. This book aims to summarize the'state of the art'for those interested in echocardiography - presumably mainly cardiologists, but here and there those of a more technical bent will find useful information as well. Some 25 years ago, the coronary care unit concentrated high technology and the acutely ill patients who might benefit from it in a single, recognizable space. Since then, that space and its technical equipment have changed, as has part of its population. Acute ischemia, silent and manifest, occurs within and outside of the coronary care unit as pain, arrhythmia, or pump failure. Its detection and treatment require the utilization of many diagnostic techniques and skills, not the least of which is two-dimensional Doppler echocardio­ graphy, which is gaining importance. Future developments, in tandem with computer technology, may add to this importance by enabling tissue identification, spatial representation, and Doppler flow mapping.
