To explore issues related to the effects of disasters on children and youth and lessons learned from experiences during previous disasters, the virtual workshop From Hurricane Katrina to Paradise Wildfires, Exploring Themes in Disaster Human Services was convened on July 22 and 23, 2020, by the National Academies of Sciences, Engineering, and Medicine. The workshop was designed to focus on families engaged with federal, state or local supportive programs prior to disasters. Additional areas of focus were the coordination of disaster response efforts and the transition to reestablishing routine service delivery programs post-disaster by human services, social services, and public health agencies at the state, local, tribal, and territorial levels. The workshop was also intended to provide a platform for highlighting promising practices, ongoing challenges, and potential opportunities for coordinated delivery and restoration of social and human services programs. This publication summarizes the presentations and discussion of the workshop.

Those who make and implement policies for children and families are seriously hampered by several features of the federal statistical system: categorical fragmentation, sampling strategies that follow adults and families rather than children, and lack of longitudinal data on children. The workshop summarized in this report was organized as part of a study sponsored by the Substance Abuse and Mental Health Services Administration (SAMHSA) and the Office of the Assistant Secretary for Planning and Evaluation of the U.S. Department of Health and Human Services, with the goal of assisting SAMHSA in its responsibilities of expanding the collection of behavioral health data in several areas. The workshop brought together experts in child mental health, psychiatric epidemiology and survey methods to facilitate discussion of the most suitable measures and mechanisms for producing estimates of serious emotional disturbance in children, which are necessary to enable the distribution of block grants that support state-level mental health services for children. The report discusses existing measures and data on mental disorders and functional impairment, challenges associated with collecting these data in large-scale population-based studies, as well as study design and estimation options.

Good healthcare is about more than making a diagnosis and giving the correct medical treatment - the experience of those going through the system is hugely important. As a result, the use of patient stories or “narratives” in learning is gathering significant support and interest. This is a collection of such narratives from children and from the parents of children with a range of long-term or complex conditions. They would be useful for medical students and instructors, but also for anyone working for children with chronic and complex conditions, including nurses and other allied health professionals, as well as psychology students. The Patient Protection and Affordable Care Act (ACA), which was signed into law in 2010, has several provisions that could greatly improve the behavioral health of children and adolescents in the United States. It requires that many insurance plans cover mental health and substance use disorder services, rehabilitative services to help support people with behavioral health challenges, and preventive services like behavioral assessments for children and depression screening for adults. These and other provisions provide an opportunity to confront the many behavioral health challenges facing youth in America. To explore how the ACA and other aspects of health care reform can support innovations to improve children's behavioral health and sustain those innovations over time, the Forum on Promoting Children's Cognitive, Affective, and Behavioral Health held a workshop on April 1-2, 2015. The workshop explicitly addressed the behavioral health needs of all children, including those with special health needs. It also took a two-generation approach, looking at the programs and services that support not only children but also parents and families. This report summarizes the presentations and discussions of this workshop.

Increasing public investments in health care services for low-income and special needs children and adolescents in the United States have raised questions about whether these efforts improve their health outcomes. Yet it is difficult to assess the general health status and health care quality for younger populations, especially those at risk of poor health outcomes, because the United States has no national information system that can provide timely, comprehensive, and reliable indicators in these areas for children and adolescents. Without such a system in place, it is difficult to know whether and how selected health care initiatives and programs contribute to children's health status. Child and Adolescent Health and Health Care Quality identifies key advances in the development of pediatric health and health care quality measures, examines the capacity of existing federal data sets to support these measures, and considers related research activities focused on the development of new measures to address current gaps. Socioeconomic conditions are known to be major determinants of health at all stages of life, from pregnancy through childhood and adulthood.'Life-course epidemiology'has added a further dimension to the understanding of the social determinants of health by showing an association between early-life socioeconomic conditions and adult health-related behaviors, morbidity, and mortality. Sensitive and critical periods of development, such as the prenatal period and early childhood, present significant opportunities to influence lifelong health. Yet simply intervening in the health system is insufficient to influence health early in the life course. Community-level approaches to affect key determinants of health are also critical. Many of these issues were raised in the 1995 National Academies book, Children's Health, the Nation's Wealth. The present volume builds upon this earlier book with presentations and examples from the field. Focusing on Children's Health describes the evidence linking early childhood life conditions and adult health; discusses the contribution of the early life course to observed racial and ethnic disparities in health; and highlights successful models that engage both community factors and health care to affect life course development.

The National Children's Study (NCS) is planned to be the largest long-term study of environmental and genetic effects on children's health ever conducted in the United States. It proposes to examine the effects of environmental influences on the health and development of approximately 100,000 children across the United States, following them from before birth until age 21. By archiving all of the data collected, the NCS is intended to provide a valuable resource for analyses conducted many years into the future. This book evaluates the research plan for the NCS, by assessing the scientific rigor of the study and the extent to which it is being carried out with methods, measures, and collection of data and specimens to maximize the scientific yield of the study. The book concludes that if the NCS is conducted as proposed, the database derived from the study should be valuable for investigating hypotheses described in the research plan as well as additional hypotheses that will evolve. Nevertheless, there are important weaknesses and shortcomings in the research plan that diminish the study's expected value below what it might be.

Before a separate Department of Medical Humanities was formed, the editors of this volume were faculty members of the Department of Pediatrics at our medical school. Colleagues daily spoke of the moral and social problems of children's health care. Our offices were near the examining rooms where children had their bone-marrow procedures done. Since this is a painful test, we often heard them cry. The hospital floor where the sickest children stayed was also nearby. The physicians, nurses, and social workers believed that children's health care needs were not being met and that more could and should be done. Fewer resources are available for a child than for an adult with a comparable illness, they said. These experiences prompted us to prepare this volume and to ask whether children do get their fair share of the health care dollar. Since the question'What kind of health care do we owe to our children?'is complex, responses should be rooted in many disciplines. These include philosophy, law, public policy and, of course, the health professions. Representing all of these disciplines, contributors to this volume reflect on moral and social issues in children's health care. The last hundred years have brought great changes in health care tor children. The specialty of pediatrics developed during this period, and with it, a new group of advocates for children's health care. Women's suffrage gave a political boost to the recognition of children's special health needs.
