Fractures of the scapula are usually associated with high-energy trauma and its management has deeply changed in the last decade. The unfavorable scapular anatomy, combined with the complexity of the approaches for fracture fixation, make the treatment challenging, even for experienced surgeons. Furthermore, the literature is controversial regarding surgical indications and rationale for treatment. This book is designed to address and discuss critical aspects of decision making for the management of scapular fractures, including anatomy, epidemiology, surgical indications, rehabilitation, treatment, and outcomes. It also presents several challenging cases with adequate preoperative planning, emphasizing the different approaches, fixation strategies, soft tissue handling, and postoperative care. Floating shoulder and the association between scapular fractures and clavicle fractures are also addressed, as well as a hot topic and emerging issue: the periprosthetic scapular fracture following reverse shoulder arthroplasty. Intensive Care Medicine (or Critical Care, the terms are used interchangeably) is an evolving specialty both within the UK and worldwide. It has recently been established as a UK speciality in it's own right, and is at the centre of the modern acute hospital, responsible for managing the sickest and most complex patients.There is a growing cadre of critical care specialists in the UK, underpinned by a large number of doctors in training within the specialty. Management of patients with severe traumatic injury is provided by intensive care specialists, often in conjunction with a range of other professionals such as surgeons and interventional radiologists. The management of these patients, who have competing complex injuries can be challenging. Traumatic injury is recognised as a significant cause of preventable mortality and such patients are now clustered within Major Trauma Centres across the UK. The Defence Medical Services of the UK have spent the last 10 years managing patients with very severe traumatic injuries, first in Iraq and most recently in Afghanistan. The lessons learnt from this experience has filtered through to the NHS, resulting in many changes to established practice. Whilst several books have been published based on this experience, none have focused on the intensive care management of such patients, which represents a vital link in the chain of survival from injury to recovery.

In response to persistent donor organ shortages, organs from marginal donors, such as expanded criteria donors (ECD) and donation after cardiac death (DCD) donors, are now accepted and have been successfully transplanted, reducing the waiting times for transplantation. Especially in Japan, transplantation of DCD kidneys has a relatively long history because of the difficulty or lack of national consensus in accepting brain death, which has made it possible to accumulate considerable clinical experience. Thus, the current organ shortage has stimulated interest in the use of marginal donors for transplantation. On the other hand, however, it is known that these organs have a high rate of delayed graft function and a more complicated postoperative course. These drawbacks have created the greatest clinical challenge in transplantation to date because of the current shortage and limitations of donors using ECD and DCD.This book, prepared by distinguished authorities in their fields, is intended for clinicians and researchers. It highlights the use of marginal donors as a comparatively novel source of transplantation organs and provides a thorough overview of marginal donors from their historical origins to recent clinical applications, including the state-of-the-art science of organ/donor management, procurement, and preservation.
