The 5th International Symposium on Artificial Heart and Assist Devices was held in Tokyo on January 26 - 27, 1995, bringing together leading researchers and specialists from all over the world. The proceedings of the symposium presents the newest ideas and approaches in the field, and will be of special interest and relevance to all who are concerned with artificial organs, cardiovascular surgery, organ transplantation, biomaterials, and related disciplines. In Japan, cadaveric donor liver transplantation is not common even though cadaveric organ transplantation was legally established in 1998. In contrast, the number of living donor liver transplantations is increasing, with more than 1700 cases at 43 Japanese institutes by November 2001. Indications for and have become living donor liver transplantation are widening in Japan similar to those for cadaveric donor liver transplantation in the United States and Europe. At the same time, split liver transplantation from cadaveric donors shares some technical aspects with living donor liver transplantation. Remarkable progress has been reported recently, and thus it was an auspicious time to hold a symposium on'Current issues in liver/small bowel transplantation'in Japan. We were honored to hold a very fruitful symposium sponsored by the Keio University Medical Science Fund and to bring together top-rank transplant surgeons from Japan and other countries. It was a productive and rewarding time for all participants. We were able to share our experience through excellent presentations followed by active discussions and insightful com ments. At the symposium, we focused on current issues in liver transplanta tion such as widening indications for viral hepatitis and malignant tumors. We also discussed technical aspects and physiological problems in split/iiving donor liver transplant, novel strategies in immunosuppression, and the current status and future prospects in small bowel transplantation. This book contains the papers from all the distinguished guest speakers, focusing on the topics discussed at the symposium.

A symposium,'Perioperative Management of Pacemaker Patients', was held in May 1990 at the Karl-Franzens-Universitat, Landeskrankenhaus Graz, Austria. The purpose of the symposium, organized by the Departments of Anesthesiology and Surgery, was to discuss the current status of permanent pacemaker technology, emerging developments in this field, legal issues, indications and methods for tempor ary perioperative pacing, and the potential for pacemaker malfunction and adverse patient-pacemaker interactions in perioperative and other medical settings. It was hoped that participants would come to some consensus concerning recommendations for satisfactory perioperative management of pacemaker patients. This symposium was probably the fIrst occasion ever that brought together representatives of the pacemaker industry, implanting physicians, and physicians responsible for the management of pacemaker patients in perioperative and other hospital settings to discuss these important matters. Certainly, it was recognized by all who attended that there existed at the time little in the way of substantial knowledge of how to best manage pacemaker patients or patients with possible indications for temporary pacing in'unique'hospital settings especially with exposure to electroma gnetic and other potential interference, but in the absence of professional persons with direct knowledge of pacemakers and related devices. Much new information was presented and useful ideas exchanged, so that three symposium participants were of the opinion that papers presented at this meeting should be published as a collective work.

The book was envisioned by bedside nurses caring for transplant and cellular therapy patients as a way to teach novice nurses and health care colleagues about the care required for this complex patient population. The nurse authors recruited an oncology nurse practitioner, transplant physician, nurse scientist and expert in medical humanities and health studies to join the project team. The dedication of the team and the willingness of the patients to contribute to the project, has led to a unique case study approach focused on the lived experience of patients and care partner(s) during the transplant/cellular therapy journey. The case studies are unique in that they encompass a qualitative narrative developed using Interpretive Phenomenological Analysis (IPA) and the inclusion of the individual's actual medical and nursing care from their health record. At every step along the trajectory of the illness, the medications and care received are described, interspersed with the voice of the patient and care partner(s) sharing their experiences. The chapters follow the individual patients and their care partner(s) through all phases of their illness. The interviews were conducted, and case studies written by the bedside clinical nurses. Visual art and prose created by patients and care partner(s)s are included in the chapters. The book also includes an introduction written by the nurse authors, a methods chapter on the inclusion of IPA in a case study, and poignant epilogue. The premise of nursing support for survivorship is woven throughout the book, highlighting how giving back aids in recovery identity.This book, full of beautiful artworks and poems, is the work of clinical nurses devoted to improving care for their patients by sharing the patient stories with others. It introduces a unique approach to the case study which could be replicated and applied to any diagnosis.

When Joseph Murray performed the first successful living kidney donor transplant in 1954, he thought this would be a temporary stopgap. Today, we are no closer to the goal of adequate organ supply without living donors--if anything, the supply-demand ratio is worse. While most research on the ethics of organ transplantation focuses on how to allocate organs as a scarce medical resource, the ethical treatment of organ donors themselves has been relatively neglected. In The Living Organ Donor as Patient: Theory and Practice, Lainie Friedman Ross and J. Richard Thistlethwaite, Jr. argue that living donor organ transplantation can be ethical provided that we treat living solid organ donors as patients in their own right. Ross and Thistlethwaite develop a five-principle framework to examine some of the attempts to increase living donation. It uses the three principles of the Belmont Report: respect for persons, beneficence, and justice modified to organ transplantation, as well as the principles of vulnerability and special relationships creating special obligations. Their approach requires that the transplant community fully embrace current and prospective living organ donors as patients to whom we have special obligations. Only when living organ donors are regarded as patients in their own right and have a living donor advocate team dedicated to their well-being can the moral limits of living solid organ donation be realized and living donors be given the full respect and care they deserve.

Bile duct injuries (BDI) are considered the most serious surgical complication associated with cholecystectomy. According to different reports, its incidence has remained constant over the years, ranging from 0.1% to 0.9%. BDI is associated with a greater risk of perioperative morbidity and mortality, a reduction in the quality of life, and a decrease in long-term survival. Also, this complication is a concern to surgeons since its progression is uncertain and may lead to demands on professional responsibility ('malpractice') and emotional and physical consequences ('second victim').Given that injuries in a high percentage of patients are initially unsuspected, the postoperative recovery may be prolonged, and the possibility of a successful repair reduced. Several surgical, endoscopic, and percutaneous procedures may be necessary to manage the lesions and to treat coexisting complications. BDI patients often undergo several repair attempts before successful resolution. This affects their quality of life and has a high psychological, physical, and mental impact due to the prolonged, complex, and unexpected nature of the injury. This currently represents a frequent problem in specialized referral centers or hepatobilopancreatic surgery units, where patients arrive with the sequelae of previous inadequate treatments. Usually, these patients require more elaborate procedures such as reoperations, liver resections and liver transplantation. The prevention, early diagnosis, and adequate treatment in the first approach is of crucial importance to ensure good long-term results. For the surgeon of antiquity the liver has been an organ of mystery – and danger. Attempts to repair its wounds or remove tumors were fraught with hemorrhage and often a fatal outcome. Most forays were those to remove easily accessible tumors on the liver edge, but bleeding was a feared consequence still and surgeons wielded a plucky fortitude to take on even those. Not until the mid-20th Century were surgeons able to safely excise neoplasms that lay deep within the liver substance. Jean-Louis Lortat-Jacob achieved notoriety in his famous Paris hepatectomy of 1951 but he was not the first. That distinction may have belonged to German Professor Walther Wendel in 1910 or to Japanese surgeon Ichio Honjo who reported his operation in 1950, but in Japanese. It was not picked up by the Western surgical community until 1955. Names such as Hugo Rex, James Cantlie, Jean-Louis Lortat-Jacob, Tôn Thất Tùng, Jacques Hepp, Claude Couinaud, Henri Bismuth, Thomas Starzl, Roy Calne, and a host of others highlight the extraordinary curiosity, tenacity, and skill of those surgeons who broached unknown territory to master understanding and techniques of manipulation, resection, and transplantation that were formerly considered unapproachable by the surgical world.
