In recent years, a growing acknowledgement of the importance and originality of Sabina Spielrein's theoretical and clinical work has taken place. However, few studies have been specifically devoted to a thorough analysis of her theory, so a satisfactory understanding of her thinking has not yet been fully achieved. The objective of this article is to contribute to the understanding of Spielrein's theory of language, based on the analysis of her two main papers on the subject, "The Origin of the Child's Words 'Papa' and 'Mama'" (1922) and "Some Analogies between the Thought of Children, Aphasia, and the Subconscious Thought" (1923). Reference is also made to some hypotheses formulated in her first works in order to clarify the assumptions of the theory of language she advances in the early 1920s. It is argued that Spielrein developed an original psychoanalytic perspective on language. She brought about an innovative contribution to the understanding of the origins and functioning of both language and thought, articulating linguistic and neurological knowledge with psychoanalytic hypotheses and her own observations. This paper seeks firstly to grasp both conceptually and historically the different phenomenologies that are captured by the term 'Unconscious Phantasy'. The term is shown to refer to a number of distinct though overlapping conceptual domains. These include: phantasy as scene, phantasy as representation of drive, phantasy as representation of wish as its fulfilment, phantasy as split off activity of the mind functioning under the aegis of the pleasure principle; phantasy as representation of the minds own activities (which Wollheim calls' the way 'the mind represents its own activities to itself''). Lastly unconscious phantasy is understood as being the basic foundation of all mental life, including drives, impulses, all anxiety situations and defences. Having mapped out this territory through following the development of the concept in the work of Freud and Klein, the author draws on the work of the philosopher Richard Wollheim who, the author contends, has made a fundamental contribution to our conceptual understanding of unconscious phantasy. In the last section of the paper, the author draws a distinction between what he terms 'objects' (namely psychic objects) and what he terms 'facts'. It is suggested that this distinction, though implicit in much of our work, benefits from being made explicit and that in so doing an important dimension of analytic work is illuminated. We aim to help the patient to discover what he is like, to understand the ways in which he conceives and misconceives himself, to unravel the fact-ness of himself and his world from its 'object qualities', to differentiate between unconscious phantasy and reality. That there is a lack of consensus as to how to decide between competing, at times even contradictory theories, and about how to integrate divergent concepts and theories is well known. In view of this situation, the IPA Committee on Conceptual Integration (2009-2013) developed a method for comparing the different versions of any given concept, together with the underlying theories and fundamental assumptions on which they are based. Only when situated in the same frame of reference do similarities and differences begin to appear in a methodically comprehensible and reproducible form. After having studied the concept of enactment followed by the publication of a paper in this Journal in 2013, we proceeded to analyze the concept of unconscious phantasy while at the same time continuing to improve our method. Unconscious phantasy counts among the central concepts in psychoanalysis. We identified a wide range of definitions along with their various theoretical backgrounds. Our primary concern in the present paper addresses the dimensional analysis of the semantic space occupied by the various conceptualizations. By way of deconstructing the concepts we endeavoured to establish the extent to which the integration of the different conceptualizations of unconscious phantasy might be achieved. The paper explores the formation of psychic elements from an epistemological point of view, drawing on the work of Bion to examine a clinical case of autistoid perversion. Distinguishing the qualification of psychic elements from the realization of pre-conceptions, the paper argues that psychical elements are constituted through a mutually shared experience of presence, and so they should be understood in a paradoxical way - through being-O and transformations into K . These ideas are explored via a clinical case concerning a patient with an autistoid-perverse organization. The patient had been denied any bodily contact with her parents during her first year of life due to an infection; in later life she exhibited an autistoid coprophilic perversion. During the course of her treatment, as it became possible to break down the autistoid organization, the nameless contents surfaced in a mutually shared experience of presence. The analyst was able to hold on to their meaning, which was unavailable to the patient. The absent analyst, however, turned into the mother who 'put the child down' and was experienced by the patient as a suicidal threat. In being-O , the analyst was able to endure the paradox of being the one who 'put her down' in order not to put her down; the paradox of being-O functioned as a container for the destructive objectal dimension of the state of 'being put down'. After the Isaacs' seminal work on the nature and function of unconscious phantasy (1948), several authors (mostly in the British Society) have reflected on the topic and tried to extend the concept of fantasy. In this paper I shall examine the contributions of Winnicott, Gaddini, Joseph and Anne Marie Sandler that aim at broadening this psychoanalytic concept. The authors that I have considered share a focus mostly on the early stages of child development. Both Winnicott and Gaddini belong to a line of thinking that explores the vicissitudes of the primary emotional development of the infantile self (in the mother-infant relationship) involving the earliest processes of holding and bodily and kinaesthetic fantasy that form the bodily integrity of the person. The Sandlers focused mostly on the concept of the past unconscious understood as a place of primitive vicissitudes with a deficit in figuration where the process of repression is missing. The present unconscious phantasy (that is located in the here and now) has the function of rendering the past unconscious phantasy partly accessible; otherwise it would remain unknowable. The author discusses the traditional interpretations of the principal dramatis personae in Verdi's Rigoletto , suggesting that the opera expresses the composer's unconscious but highly perceptive and intuitive exploration of: (i) paranoid and perverse father-daughter oedipal dynamics as enacted between Rigoletto and Gilda; (ii) a folie-à-deux sado-masochistic relationship between Rigoletto and the Duke; (iii) the avoidance of conscious guilt and responsibility for Rigoletto's part in the tragedy through the rejection of insight achieved via massive projection, an addiction to mania, and perversion. In this respect Rigoletto is compared with King Lear. Some technical aspects of Verdi's compositional style employed to portray the contrasting characters of the protagonists are analysed. The paper also touches on Verdi's own possible unconscious (as well as conscious) investment in this particular opera, noting similarities between Verdi's portrait of the tragic, hunchback jester and his own self-depiction. The problems posed in understanding and working through the patient's layers of self-criticism are challenging for both patient and analyst. In particular, this paper explores some countertransference phenomena related to underlying grandiosity embedded in self-criticism. For patients who are self-critical, analyzing grandiose elements may create further grounds for self-reproach or open up new modes of self-experience and freedom. The paper tries to focus on how the analyst's experience of the patient's self-criticism often shifts over the course of analytic work. It is important for the analyst to not be crippled by a fear of considering the relevance of underlying grandiosity in relation to self-reproach. Understanding this dimension of self-reproach can help elucidate why it is so durable and refractory to interpretation. The patient has a stake in holding on to this self-punishment because it perpetuates self-regulatory fantasies. These fantasies sometimes relate to the feeling that the patient will be more successful or better loved by holding on to aspects of self-reproach. In this paper the author discusses a specific type of dreams encountered in her clinical experience, which in her view provide an opportunity of reconstructing the traumatic emotional events of the patient’s past. In 1900, Freud described a category of dreams – which he called ‘biographical dreams’– that reflect historical infantile experience without the typical defensive function. Many authors agree that some traumatic dreams perform a function of recovery and working through. Bion contributed to the amplification of dream theory by linking it to the theory of thought and emphasizing the element of communication in dreams as well as their defensive aspect. The central hypothesis of this paper is that the predominant aspect of such dreams is the communication of an experience which the dreamer has in the dream but does not understand. It is often possible to reconstruct, and to help the patient to comprehend and make sense of, the emotional truth of the patient’s internal world, which stems from past emotional experience with primary objects. The author includes some clinical examples and references to various psychoanalytic and neuroscientific conceptions of trauma and memory. Translations of summary The concept of unconscious phantasy has played - and still does play - a central role in psychoanalytic thinking. The author discusses the various forms by which unconscious phantasies manifest themselves in the analytic session as they are lived out and enacted in the transference relationship. This paper also aims at expanding the kleinian theory of symbol formation by exploring the impact that emotional aspects connected to early 'raw', 'pre-symbolic' phantasies have in the analysis and how their corporeal elements interlock with the signifying process. The author follows the expressive forms of primitive unconscious phantasies as they appear in a psychoanalytic session and proposes that the emotional effect that can be experienced in the communication between patient and analyst depends in great measure on 'semiotic' aspects linked to primitive phantasies that are felt and lived out in embodied ways. Rather than a move from unconscious phantasies that typify symbolic equations to those showing proper symbolization, these can coexist and simultaneously find their way to what is communicated to the analyst. This paper describes three early developmental modes of handling the individual's psycho-genetic heritage. The first one, which characterizes normal development, is called living with the shadow of one's heritage. The shadow (of history, of life and death) is a natural counterpart of the self. The second mode, which accounts for more disturbed patients, is called living under the shadows of heritage. This type is characterized by an unconscious phantasy of the person being haunted by persecutory and vindictive ghosts instead of benign ancestors. The third mode, which might be encountered in severely disturbed patients, is being the shadow. This mode, called Chimerism, describes a confused organism which may turn against itself as parts of it are experienced as alien. On the unconscious level this signifies a heritage which cannot be experienced or mentalized as such. Rather, it is a complete chaos with moments where the hardly existent self is experienced as a bizarre object made up of non-combining, welded parts. These three modes will be examined with the help of material drawn from two analyses: of an autistic boy and of an adult patient who was persecuted by an unspeakable, horrific ancestral past. The author views paper, The nature and function of phantasy , as making an important contribution to the development of a radically revised psychoanalytic theory of thinking. Perhaps Isaacs's most important contribution is the notion that phantasy is the process that creates meaning, and that phantasy is the form in which all meanings - including feelings, defense 'mechanisms,' impulses, bodily experiences, and so on - exist in unconscious mental life. The author discusses both explicit formulations offered by Isaacs as well as his own extensions of her ideas. The latter include (1) the idea that phantasying generates not only unconscious psychic content, but also constitutes the entirety of unconscious thinking; (2) the notion that transference is a form of phantasying that serves as a way of thinking for the first time (in relation to the analyst) emotional events that occurred in the past, but were too disturbing to be experienced at the time they occurred and (3) a principal aim and function of phantasy is that of fulfilling the human need to get to know and understand the truth of one's experience. The author concludes by discussing the relationship between Isaacs's concept of phantasy and Bion's concepts of alpha function and the human need for the truth, as well as the differences between Fairbairn's and Isaacs's conceptions of the nature of unconscious internal object relationships. Interpretation is at the center of psychoanalytic activity. However, interpretation is always challenged by that which is beyond our grasp, the 'dark matter' of our mind, what Bion describes as ' O'. O is one of the most central and difficult concepts in Bion's thought. In this paper, I explain the enigmatic nature of O as a high-dimensional mental space and point to the price one should pay for substituting the pre-symbolic lexicon of the emotion-laden and high-dimensional unconscious for a low-dimensional symbolic representation. This price is reification - objectifying lived experience and draining it of vitality and complexity. In order to address the difficulty of approaching O through symbolization, I introduce the term 'Penultimate Interpretation'- a form of interpretation that seeks 'loopholes' through which the analyst and the analysand may reciprocally save themselves from the curse of reification. One of the problems in dealing with terrorism is that we have virtually no access to individual terrorists; only their actions are visible. The founders of the Italian terrorist group, the Red Brigades, on the other hand, have written about their experiences and have exhaustively explained their motivations. The author’s premise is that these autobiographies and her interviews with several of the group’s members give us access to the unconscious processes involved in the formation and operation of the group. After terrorist attacks, it is natural to ask whether the terrorists’ capacity for collective violence is an indication of personal pathology. This paper argues that the relevant pathology in the terrorist enterprise is not that of the individual but that of the group. Relying on the theories of groups of Freud (1921), Bion (1961), Anzieu (1984) and Kaes (2007), the author argues that psychoanalytic theory is essential to understanding the motivations and actions of violent groups which otherwise remain obscure. Although the discussion has been confined to one terrorist group, the author hopes that it can also be useful for understanding the unconscious dynamics of other groups structured around an ideology which mandates the destruction of human life. There has been a recent surge of interest in a type of thinking seen in some patients most of the time, and all patients some of the time. In the past it was simply called regressive thinking, but attempts to be more specific have led it to be called by various names, like ‘pre-symbolic’, ‘pre-conceptual’ and ‘pre-operational’. What these labels attempt to capture is that the patient’s thinking, at these times, is without representations and closer to actions. As a clinical phenomena I prefer to use Loewald’s term, ‘action–language’, that is, where words become attempts to bore, seduce, anger, etc. It is different from when words are used to communicate internal states via free association. A case is made for action–language being the primary method by which the unconscious speaks to us in psychoanalytic treatment. This paper explores some of the reasons for this type of thinking, along with the clinical methods to find the thinker where there appears to be none. Distinctions will be made as to the use of the process versus the content, on the goal of interventions as bringing insight versus the capacity for insightfulness, and the model of treatment as leading to transformations in thinking, rather than lifting of repressions. In this paper the author suggests that understanding the roots of the subjective sense of time can throw light on the disturbances in psychic time which are found in particular in the more severe pathologies. She introduces the argument that the roots of the development of the sense of time rest on a primitive sense of time she calls ‘reverberation time’. By this notion she refers to the particular quality of the earliest ‘back and forth’ internalized exchange with the mother in which the auditory dimension plays a significant part. Referring to a wide range of literature and clinical examples, the author thus suggests that the subjective sense of time is created by the reverberation between mother and infant. Disturbances in this area will be reflected in the pathological ‘arresting’ of time which is observed in the different pathologies and, in particular, around the negotiation of the depressive position and the oedipal situation. Extending this argument, the author goes on to suggest that it is the internalization of this experience of ‘reverberation’ which lies at the heart of the experience of dreaming; she considers that dreaming understood as an internal dialogue points both to its roots in the relationship to the maternal object and to its fundamental role in psychic life. The author concludes that ‘reverberation time’ is also the building block of a psychoanalysis, leading to ‘unfreezing’ psychic time and enabling the reconnection of ‘here and now’ with ‘there and then’ in a flexible way which promotes open possibilities, and that this takes place via the analyst’s reverie, or time of reverberation. In this paper the author discusses some characteristics of a psychoanalytic text on the basis of two pages of Freud’s essay, Delusions and dreams in Jensen’s ‘Gradiva’ ( Freud, 1906 ), on the concept of the return of the repressed. Analysis of the text shows that the four references (Horace, Rops, Rousseau, and a clinical vignette) occurring in it present unexpected connections both with each other and with the phenomenon they illustrate. There thus emerges a hidden scenario that reveals a concealed level of the text, relating to the maternal imago. Particular attention is devoted to the importance of the figurative apparatus and images (examples in the form of narrations and visual images, metaphors, and similes) that accompany the metapsychological and conceptual construction of Freud’s text. Representation in visual form is necessary for the description and construction of the psyche and for conferring life on its conceptual formulations. However, metapsychological definition also reveals a phantasy dimension underlying the text. In addition, the author shows how certain textual constraints limit the intrinsic intuitive and arbitrary nature of interpretation. Finally, the complexity of the psychoanalytic text (with its various planes and levels) is emphasized, as well as the network of possible connections fundamental to the work of interpretation. A diagram illustrates the spatio-temporal aspects of the interpretive process, as defined by the interaction between conceptual factors and specific flights of the imagination which also have to do with unconscious affects, whether in the text, the author, or the reader. In this paper the author discusses two categories of patients which differ in terms of the impact they have in the countertransference. On the one hand, there are patients who create an empty space in the analyst's mind. The response they provoke is a kind of depressive feeling that remains after they leave. The patient may bring dreams and associations, but they do not reverberate in the analyst's mind. The experience is of dryness, a dearth of memory, which mayat timesleave the analyst with a sense of exclusion from the patient's internal world. At the other extreme, there are patients who fill the consulting room. They do that with their words, dreams and associations but also with their emotions and their actions. The experience is that the analyst is over-included in the patient's world. They have dreams that directly refer to the analyst and the analyst feels consistently involved in the patient's analysis. The pathway through which the analyst can understand both these types of patients is via the countertransference or, to put it another way, the analyst's passion. In 'Analysis terminable and interminable' Freud suggested that the bedrock of any analysis is the repudiation of femininity. The author believes this statement may be viewed as lying at the crossroads of the discussion about the limits of the theoretical and clinical psychoanalytic formulations which she refers to. In this paper, the author explores the phenomenon of not being able to dream (as opposed to not being able to remember one's dreams) from three different vantage points. First, from the point of view of psychoanalytic theory, he discusses Bion's idea that the work of dreaming creates the conscious and unconscious mind (and not the other way around). A person who cannot dream is unable to generate differentiable conscious and unconscious experience and, consequently, lives in a psychic state in which he is unable to differentiate waking from sleeping, dreaming from perceiving. The author then approaches the problem of the inability to dream from the perspective achieved by a literary work. He discusses a Borges fiction that creates, in a singularly artful way, the experience of not being able to dream. Finally, the author utilises the vantage point of a detailed account of a clinical experience to explore what it means not to be able to dream. He describes an initial state characterised by the patient's proliferation of unutilisable 'psychic noise' which, over a period of years, led to the analyst's experiencing 'reverie-deprivation' and brief periods of countertransference psychosis. Two analytic sessions are presented and discussed in which psychological work was done that contributed to an enhanced capacity on the part of both patient and analyst for genuine dreaming - both in sleep and in analytic reverie states.
