The present study investigates vibrational behavior of a porous conical hollow rotor. Inner and outer rotor surface, sensor, and actuator strips made of piezoceramic are modeled and positioned, which are symmetrically distributed in the circumferential direction. Kinetic and potential energies were computed considering the effects of Coriolis and centrifugal force. Modeling of the system has been carried out assuming classical theory and using Lagrange approach for simple-supported boundary conditions. Also, the relations of piezoceramic and three types of porosity attend in equilibrium equations. The discretization of kinetic and potential energies using displacement field which is defined as double mixed series based on independent mode shape and time function leads to obtaining the equations in form of ordinary differential. To calculate the natural frequencies, the equations were converted to a special form of state space model. The correctness of the study is examined by comparing it with similar study results. The most consequential parameters affecting the forward and backward natural frequency, such as type and porosity coefficient, size, number, and thickness of piezoceramic, and the rotor angular velocity, are researched. This investigation analyzes the effects of internal static fluid on the free vibrational behavior of joined shells made of carbon nanotube (CNT) fiber‐reinforced composite. Various types of functionally graded CNTs composite with temperature dependence are considered. Classic shell theory and Love's assumptions combined with Hamilton's principle are used to obtain motion equations. Bernoulli equation, velocity potential, and impermeability condition are employed to the coupling effects of fluid pressure with motion equations of shells. Continuity conditions in the joint point of conical and cylindrical sections are supposed to couple equations of two parts. The generalized differential quadrature (GDQ) method is employed to solve equations for various boundary conditions. Experimental modal tests on a sample made by the 3-D printer and prior others' research are used to validate the accuracy of results. Consequently, parametric studies are expressed, such as longitudinal and circumferential mode numbers, semi-vertex angle, distributions of CNTs, CNTs fraction volumes, fluid density, shell thickness, and operating temperature. This research analyzes the influence of temperature changes on the vibration of single-walled carbon nanotubes (SW-CNTs) composite joined conical-cylindrical shells. The governing dynamic equations of temperature-dependent CNTs with initial thermomechanical stresses are established using the Love shell assumptions and classical shell theory. The initial thermomechanical stresses are derived from the linear membrane approach method. Two possibilities are assumed for the calculation of temperature change: a uniform temperature distribution and steady-state heat transfer by conduction through the thickness of the shell. The initial thermomechanical stresses are determined using the linear membrane approach. The generalized differential quadrature (GDQ) method is used to solve the equations after combining it with continuity conditions between the conical part and the cylindrical part and various boundary conditions. After validating the natural frequency and the different types of temperature distribution with the studies of other researchers, the effects of semi vortex of the cone, the volume fraction, and the type of distribution on the temperature rise are given as the results. 5. Reliability Estimation of Low-Voltage Power Distribution Systems.

Many accidents always happen in the electricity distribution sector, which requires the use of control strategies in this field. Therefore, this study presented a risk assessment method based on a fuzzy Bayesian network (FBN) and William Fine method in low-voltage power distribution systems to reduce the structural uncertainties of the studies and examined the reliability of these systems to achieve proper performance in emergencies. The first step is to develop a fault tree (FT) and event tree (ET) in bow tie (BT) format. In this study, fuzzy theory and expert judgment were used to qualitatively analyze the root events affecting the system failure and determine the relevant probabilities. Then, deductive and inductive reasoning was done after determining the likelihood of basic events (BEs) and establishing the Bayesian network (BN). Then, the final risk was estimated using William Fine's method and fuzzy logic. The results of a case study in a low-voltage power distribution system showed that there were 36 BEs and 20 intermediate events (IEs) for system failure, and the reliability of the system was obtained as 0.96654476 and 0.96654476 based on the FT and FBN, respectively. According to the results of BE25 (no installation of the earth) and BE18 (contact of metal objects with the live wire), the most critical events were based on FBN. The results showed that FBN simulation and its combination with static methods such as BT and William Fine provide a suitable method for the accurate identification of distribution systems. 6. The effect of Mediterranean diet on nutritional status, muscle mass and strength, and inflammatory factors in patients with colorectal cancer-induced cachexia: study protocol for a randomized clinical trial.

- Abstract
Background: Current dietary strategies to manage cancer cachexia and the relevant outcomes did not provide a comprehensive solution. This study will evaluate the effect of a Mediterranean diet on inflammatory markers, nutritional status, muscle mass, and strength among patients with cancer cachexia (CC). Methods: This will be a randomized clinical trial involving men and women diagnosed with localized or advanced colorectal cancer-induced cachexia. In total, 40 patients with CC will be recruited based on inclusion criteria and then these patients will be randomly allocated to receive either a Mediterranean diet (n = 20) or only routine nutritional advice (n = 20) for 8 weeks. The primary outcome will be nutritional status, muscle mass and strength, and serum concentrations of inflammatory markers including interleukin-6 (IL-6), tumor necrosis factor-alpha (TNF-α), and high-sensitive C-reactive protein (hs-CRP). Moreover, we will consider serum albumin and total protein levels, complete blood count (CBC), and quality of life as the secondary outcomes. All outcomes will be measured at the beginning and end (the eighth week) of the study. We will assess participants’ adherence to the prescribed diets by using a 1-day food record in the second, fourth, sixth, and eighth weeks of the study. Discussion: Along with adequate calorie and protein intake in cancer cachexia, reducing inflammatory cytokines might be a useful strategy for maintaining nutritional status and body composition. Mediterranean diet has been shown to have anti-inflammatory properties, and by its components, it might help patients with cachexia to have a better nutritional status and quality of life. In this article, two high step-down bridgeless power factor correction rectifiers based on the switched inductor network (SIN) are introduced. The proposed rectifiers employ the SIN to provide high step-down voltage gain with a higher duty cycle than the competitors. They also offer higher efficiency, lower current stress, and total peak switching device powers. A thorough and straightforward design algorithm in the discontinuous conduction mode is provided that ensures a unity power factor and a low total harmonic distortion with a simple control scheme. As a demonstration of the superior performance of the proposed rectifiers, a 300-W high-gain sepic rectifier setup with 48Vdc output voltage from a 230Vrms/50Hz source is built in the laboratory. • Presenting a collaborative model for the operation of interconnected microgrids. In the proposed model, the microgrids create a big coalition for centralized management and loss reduction. After the coalition forming, the profit of the cooperation is divided among MGs by Shapley value. • The impacts of the proposed model on loss reduction and emission reduction were considered. Also, the performance of the proposed model is compared with the decentralized framework. • Providing a sensitivity analysis on electric vehicles to determine their efficiency on the operation of interconnected microgrids. Microgrids are known as a local energy system that changes the central power plants toward distributed and renewable energy resources. The microgrids provide low-cost, more efficient, and clean energy for the energy systems. Therefore, this paper presents a techno-sustainable-economic framework for the operation scheduling of multi-microgrid systems that are integrated with renewable energy sources, dispatchable resources, and energy storage systems. The proposed model presents a multi-objective optimization model that attempts to simultaneously minimize the total cost, power losses, and carbon emissions by cooperation of each microgrid with other neighboring energy systems. In the cooperative framework, the microgrids form a big coalition to reduce the operation costs by their local energy system and energy trading with neighbor microgrids. Also, microgrids try to minimize the power losses by local energy trading. The Genetic algorithm is used to model the multi-objective cooperative scheduling of microgrids and presents a fair cost allocation to consider the impact of each microgrid in the coalition. The proposed model is tested on the IEEE 33-bus test system and simulation results demonstrate that the proposed model reduces the power losses and carbon emissions by 68.67 % and 19.65 % compared to the decentralized framework because of local energy sharing between microgrids. 9. SOX2 and Bcl-2 as a Novel Prognostic Value in Hepatocellular Carcinoma Progression.

Sex-determining region Y-box 2 (SOX2) is a stem cell transcription factor and a major regulator of self-renewal and pluripotency of cancer stem cells (CSCs). In many types of cancer, SOX2 is dysregulated due to overexpression associated with tumor progression and low survival rate. Many HCC cases encounter recurrence and metastasis which might be due to CSCs and also apoptosis. Since little is known about the expression pattern of SOX2 and apoptotic genes in HCC, we aimed to determine the prognostic significance of SOX2, Bax, and Bcl-2 in clinicopathological features, tumor progression, and survival rate of the HCC patients. The expression of SOX2, Bax, and Bcl-2 were evaluated using qRT-PCR in 53 formalin-fixed, paraffin-embedded tissues (FFPE) of patients and 44 controls. Correlation of these genes was analyzed with clinicopathological features and tumor progression. The correlationship between SOX2 expression and ALBI grade as prognostic indicators were calculated. Survival rates were determined by Kaplan-Meier survival curves. SOX2 and Bcl-2 were remarkably overexpressed in HCC patients compared to controls (p = 0.04 and p = 0.003, respectively). A significant association was found for both SOX2 and Bcl-2 overexpression with TNM staging (p = 0.02, p = 0.04) and tumor grading (p = 0.01, p = 0.003), respectively. A significant correlation was observed: patients with SOX2 overexpression had a lower 5-year overall survival rate (p = 0.04); however, there was no significant association between Bcl-2 and survival (p = 0.5). Collectively, overexpression of SOX2 and Bcl-2, alone or combined, may be a potential marker to evaluate prognosis and response to HCC treatment. [ABSTRACT FROM AUTHOR]

Structural and physiological changes in sperm and semen parameters reduce fertility in diabetic patients. Securigera Securidaca (S. Securidaca) seed is a herbal medicine with hypoglycemic, antioxidant, and anti-hypertensive effects. The question now is whether this herbal medicine improves fertility in diabetic males. The study aimed to evaluate the effects of hydroalcoholic extract of S. Securidaca seeds (HESS), glibenclamide and a combination of both on fertility in hyperglycemic rats by comparing histological and some biochemical changes in testicular tissue and sperm parameters. The treatment protocol included administration of three doses of HESS and one dose of glibenclamide, as well as treatment with both in diabetic Wistar diabetic rats and comparison of the results with untrated groups. The quality of the testicular tissue as well as histometric parameters and spermatogenesis indices were evaluated during histopathological examination. Epididymal sperm analysis including sperm motility, viability, abnormalities, maturity, and chromatin structure were studied. The effect of HESS on the expression of LDH and FGF21 genes and tissue levels of glycogen, lactate, and total antioxidant capacity in testicular tissue was investigated and compared with glibenclamide. HESS improved sperm parameters in diabetic rats but showed little restorative effect on damaged testicular tissue. In this regard, glibenclamide was more effective than the highest dose of HESS and its combination with HESS enhanced its effectiveness so that histological tissue characteristics and sperm parameters were were comparable to those of healthy rats. The expression level of testicular FGF21 gene increased in diabetic rats, which intensified after treatment with HESS as well as glibenclamide. The combination of HESS and glibenclamide restored the expression level of testicular LDH gene, as well as tissue storage of glycogen, lactate and LDH activity, and serum testosterone to the levels near healthy control. S. Securidaca seeds can be considered as an effective supplement in combination with hypoglycemic drugs to prevent infertility complications in diabetes. [ABSTRACT FROM AUTHOR]

To investigate the effect of different levels of bioplex manganese along with probiotics and multi‐enzymes on the performance and immune system of broilers, 640 one‐day‐old male chicks of the Ross 308 strain were reared and the data analysed in a 4 × 2 × 2 factorial experiment with four levels of bioplex manganese (0, 60, 72 and 84 mg per kg of diet), two levels of Parsilact probiotic (0 and 200 mg per kg of diet) and two levels of Combo multi‐enzyme (0 and 1,000 mg per kg of diet) in a completely randomized design with 16 experimental treatments, 4 replicates and 10 chickens per replicate during a period of 42 days. The results showed that the performance of the broiler chickens in the diets containing 72 and 84 mg bioplex manganese along with probiotics and multi‐enzymes had the greatest difference compared to the control (p <.05). Compared to the control with 0 mg/kg manganese; the bursa of Fabricius weight was greater in chickens fed diets containing additional manganese (p <.05). The concentration of antibodies produced against Newcastle disease virus, as well as the concentrations of IgG, IgM and total immunoglobulins produced against SRBC, were highest in the group fed a diet containing 84 mg manganese along with probiotics and multi‐enzymes (p <.05). The results show combining additional manganese with probiotics and multi‐enzymes in chicken diets leads to better performance as well as a stronger immune system of chickens. Background: Atherosclerosis is the leading cause of death in patients with diabetes. We aimed to evaluate the association between ankle-brachial index (ABI) and toe-brachial index (TBI) with carotid intima-media thickness (CIMT) in patients with type 2 diabetes (T2DM). Methods: This cross sectional study included 296 patients with T2DM who met the American Diabetes Association criteria for the assessment of peripheral arterial atherosclerosis. The ABI ≤ 0.9 and TBI ≤ 0.7 were considered as abnormal values. Linear and logistic regression analyses were performed to evaluate the association between TBI/ABI and CIMT. Results: Right CIMT was significantly greater in the low TBI group (p = 0.03) while, left CIMT did not show a significant difference. Each 0.1-unit decrease in TBI value was independently associated with 0.017 mm increase in the right CIMT (β ± SE; -0.017 ± 0.005, p = 0.002) and with odds of the presence of increased CIMT [odds ratio and 95% confidence interval: 1.21 (1.02, 1.44)] after adjustment with all traditional risk factors. There was not any significant association between ABI and increased CIMT. Conclusions: Beyond a suitable tool for the diagnosis of peripheral artery disease in patients with T2DM, TBI can be applied for prediction of subclinical carotid atherosclerosis. Purpose: Brain metastases (BM) often leave residual tumors despite having visible margins, which increases the risk of local tumor recurrence and can impact overall patient survival rates. Fluorescence-guided surgery (FGS) utilizing sodium fluorescein (FL) has been reported as an effective technique in recent studies. This study aimed to evaluate the efficacy of FL FGS in improving the extent of resection of brain metastases and its impact on overall survival. Methods: We conducted a systematic search following the Preferred Reporting Items for Systematic Reviews and Meta-Analyses. Our primary focus was on gross total resection (GTR). Additionally, we extracted survival data and evaluated the risk of bias using a modified version of the Joanna Briggs Institute critical appraisal tool. Results: The study comprised 970 patients with brain metastases through eight different studies. The study found that patients who underwent FL-guided resection had a significantly higher rate of GTR (OR: 2.02, 95% CI: 1.14–3.56, p = 0.0156, I2 = 41.5%). Additionally, the study concluded that FL-guided resection is associated with better overall survival rates (HR: 0.61, 95%CI: 0.47 0.80, p = 0.0003, I2 = 41.5%). Conclusion: Our research suggests that the use of FL is associated with a higher rate of GTR and improved overall patient survival. None of the studies we reviewed reported significant complications associated with the use of FL in patients. [ABSTRACT FROM AUTHOR]

15. Posterior Reversible Encephalopathy Syndrome after Allogeneic Stem Cell Transplantation in Pediatric Patients with Fanconi Anemia, a Prospective Study.

• Individuals with Fanconi anemia have a higher incidence of posterior reversible encephalopathy syndrome (PRES). • Cyclosporine level is not correlated with PRES occurrence. • PRES can present with a nonposterior distribution. • PRES is nonreversible in all cases. Posterior reversible encephalopathy syndrome (PRES) is one of the most common neurologic complications following hematopoietic stem cell transplantation (HSCT). We aimed to evaluate the incidence, clinical, and imaging features of PRES in pediatric patients with Fanconi anemia (FA) following HSCT. This prospective study included all post-HSCT patients with underlying FA disease between 2014 and 2017. Brain computed tomography scan and magnetic resonance imaging (MRI) were performed in all individuals who developed neurologic symptoms. PRES was diagnosed based on clinic-radiological evidence. Follow-up MRI was performed in all patients with PRES within two months. Forty-one patients with FA (28 males; mean age, 8.19 ± 3.25 years) were enrolled. Out of 15 patients with acute neurologic symptoms, PRES was diagnosed in 9 individuals (21.95% of the total cohort). The occurrence of PRES was significantly higher in patients who had a donor with a 1-locus mismatch (P =.02). Donor relation, stem cell source, and graft-versus-host disease grade did not have any significant association with the development of PRES. MRI showed asymmetric vasogenic edema in 5 patients, an overt infarct in 1 patient, and foci of microhemorrhages in 3 patients, 1 of whom developed a hemorrhagic infarct. This patient died shortly, and persistent microhemorrhages were noted in the other 2 patients. Our findings demonstrate a greater risk of developing PRES after HSCT in patients with FA compared with those with other diseases (21.95% versus 1% to 10%), and in contrast to its term, it might be irreversible and has adverse effects on HSCT outcomes. The increased vascular and endothelial fragility in FA may contribute to the higher frequency of PRES in these individuals. [ABSTRACT FROM AUTHOR]

Key Clinical Message: Placenta previa, accompanied by placenta percreta, which involves invasion of the bladder, presents a significant risk of excessive bleeding during and after delivery. This case highlights that prophylactic embolization, conservative surgery, and careful monitoring offer an effective approach to avoid hysterectomy in cases of placenta percreta with adjacent organ involvement. Placenta previa complicated by placenta percreta is associated with a high risk of massive intra and post‐partum hemorrhage. We present a case of a 35‐year‐old woman (G2 P1) who was referred to the Akbar‐Abadi hospital at 13 weeks of gestation. Color Doppler ultrasound indicated complete placenta previa‐percreta with bladder invasion. After induction of fetal demise, bilateral uterine and bladder artery endovascular embolization was conducted for the patient. After 48 h, under ultrasound guidance, surgical resection of residual percreta tissue was conducted as much as possible. Eight weeks later, a follow‐up sonography showed the minimum residual placenta tissue and she regained menstrual cycles after 2 months. This case indicated that the combination of prophylactic embolization, conservative surgical management with placenta left in situ, and follow‐up with serial color Doppler monitoring, is an optimum method to avoid hysterectomy in placenta percreta patient with adjacent organ invasion. Transformer neural networks have revolutionized natural language processing by effectively addressing the vanishing gradient problem. This study focuses on applying Transformer models to time-series forecasting and customizing them for a simultaneous multistep-ahead prediction model in surrogate model predictive control (MPC). The proposed method showcases improved control performance and computational efficiency compared to LSTM-based MPC and one-step-ahead prediction models using both LSTM and Transformer networks. The study introduces three key contributions: (1) a new MPC system based on a Transformer time-series architecture, (2) a training method enabling multistep-ahead prediction for time-series machine learning models, and (3) validation of the enhanced time performance of multistep-ahead Transformer MPC compared to one-step-ahead LSTM networks. Case studies demonstrate a significant fifteen-fold improvement in computational speed compared to one-step-ahead LSTM, although this improvement may vary depending on MPC factors like the lookback window and prediction horizon. • Transformer and LSTM models compared in Model Predictive Control. • New multistep Transformer architecture proposed for MPC. • Transformer MPC outperforms LSTM-based MPC with 15x speedup. • Practical implementation on fluidized-bed gold ore roaster. The hedgehog tongue is a tactile and taste organ which carries out various functions. Detailed functional and morphological studies are required to clearly define the relationship of the hedgehog tongue with taste, food palatability, mastication and swallowing of food, as well as the production of sounds. The aim of this study was to determine the relationship between the morphological characteristics of the European hedgehog tongue and the lifestyle of this animal, as well as to compare findings with the results of studies on other vertebrates. Gross and micro-anatomical light and scanning electron microscopy studies revealed that the hedgehog tongue could be divided in three areas, namely the apex, body and root. A keratinized stratified squamous epithelium, which was smooth on the ventral surface but bore four types of papillae on the dorsal surface, lined the tongue. Three types of these papillae were found to have gustatory functions and to express their activity in close relation with the salivary glands. These simple conical filiform papillae were situated caudally and distributed one after the other without a break. The dome-shaped fungiform papillae on the apex, with the highest distribution rate on the apex edge, were small, but those on the body and root were large. The three circular vallate papillae were arranged in a triangular shape. The foliate papillae with a few tiny projections, found in a shallow furrow, were situated between the root and the body. Most of the nerve fibers observed in different sections of the tongue tissue were of the unmyelinated type, confirming that the main task of the hedgehog tongue was its gustatory function. 19. Adjuvant high-dose-rate brachytherapy in the management of oral cavity cancers: 5 years of experience in Iran.

Purpose: Brachytherapy is a cost-effective method for the management of oral cavity cancers in low to middle income countries. We aimed to evaluate the clinical outcomes of high-dose-rate interstitial brachytherapy (HDR-IBT) in patients with oral cavity cancer. Material and methods: From 2009 to 2013, 78 patients (49 combined external beam radiotherapy [EBRT] plus IBT and 29 IBT monotherapy) with oral cavity cancers had been treated in our center. Slightly more than half the patients were male, and the median age was 54 years. The treatment was planned based on the Paris system. The main outcomes were disease-free and overall survival. Results: The median follow-up duration was 36.5 months (range, 1.17-54.23). The actuarial four-year overall and disease-free survival rates were 83% and 65%, respectively. The local and locoregional control was achieved among 89.74% and 87.17% of patients, respectively. None of the factors including tumor size, node status, gender, and radiation modality (IBT alone vs. IBT + EBRT) had a significant statistical correlation to the local control rate. All the patients tolerated the planned treatment in the IBT alone group. Late complications included a case of trismus and three cases of catheter insertion site fibrosis. Conclusions: HDR-IBT as a monotherapy or in combination with EBRT is an appropriate option for the management of oral cavity squamous cell carcinomas, and supports the improvement in treatment outcomes and toxicity profiles in adjuvant settings. Migration of Ascaris lumbricoides through the papilla of Vater in humans, and entry into the biliary tree, is well‐recognised. Ascaris suum and Toxocara vitulorum have been recovered from the liver of swine and buffalo. We necropsied a Persian Kurdish filly at age 6 months, weighing ∼100 kg. Death evidently was caused by oleander (Nerium oleander) intoxication. An 8‐cm adult male Parascaris was found at the lobar‐left hepatic bile duct junction. We suggest that the nematode entered anteriorly into the hepatic tree, via the duodenum, major duodenal papilla, bile duct, left hepatic duct and finally the lobar duct. Considering the brief 4‐h elapsed time between death and necropsy, and the 18‐cm distance from the major duodenal papilla to the location of the parasite, we conclude that entry into the biliary tree likely occurred ante‐mortem. We advise consideration of Parascaris infection in differential diagnosis of equine hepatic and pancreatic dysfunction. [ABSTRACT FROM AUTHOR]

21. A Quasi-Z-Source Inverter with a Network Capacitor-Inductor.

- Abstract
In this research, a new Z-source inverter inspired by the quasi-inductor-switched Z-source inverter is presented. The new inverter has an acceptable boost factor, and gain voltage compared to the similar Z-source inverter. Also, in this new topology, the capacitors’ voltage stress has been decreased compared to similar inverters. In this converter, a capacitor is replaced with the side diodes of the quasi-Z-source inverter. The input current in this inverter is continuous and it is a low figure that at the moment of switching off about 54 A (
V in = 120 V). The high inrush current damages the converter components. The operation of this inverter has been checked in different modes and the voltage and current equations of all elements have been calculated. The new converter can be extended to n-cascade, and it makes to achieve a higher boost factor with a low-duty cycle. The ingredients used in the new topology are fewer than those of the switched-inductor Z-source inverter. The analysis and extraction of all voltage stress such as capacitors, diodes, switches, all currents such as inductors, dc-link average, inrush current, voltage gain and boost factor of the new topology have been done and compared with other structures. Background and Objectives: The non-surgical treatment outcome of gastroesophageal junction (GEJ) adenocarcinoma remains to be defined. We aimed to assess the outcomes of definitive chemoradiation (CRT) of GEJ tumors. Methods: This retrospective cohort study was entirely carried out in the radiation oncology ward of Cancer Institute of Iran. We included patients with adenocarcinoma histology whose tumors had involved the gastro-esophageal junction and underwent chemoradiotherapy without surgery. In the final analysis, we evaluated 50 patients with non-metastatic adenocarcinoma of GEJ (Siewert's type I and II) from 2008 to 2017. The primary outcome was overall survival; secondary outcomes were progression-free survival and local and distal metastasis. Results: The reasons for not undergoing surgery in order of frequency from highest to lowest were patient refusal or medical unfitness for surgery, tumor unresectability or progress at the time of operation and incident metastasis in pre-op restaging, and. The 1-year, 2-year, and 3-year overall survival rates were 53%, 26%, and 12%, respectively. The 1-year, 2-year, and 3-year progression-free survival rates were 44%, 18%, and 10%, respectively. In the multivariate analysis, the only independent predictor of survival was a distant failure (P=0.031). Conclusion: Although the outcomes of non-surgical treatments are disappointing in GEJ adenocarcinomas, a few patients may experience long-term survival using definitive CRT. Neuroendocrine carcinoma usually originates from lung. Few data exist in the literature regarding neuroendocrine carcinoma of the tongue. Patient data including history, surgical procedure, histology, and radiology investigations were collected and summarized. A 40-year-old woman was referred after partial glossectomy. Squamous mucosa with neoplasm and cells with round nuclei and light cytoplasm was reported in the tongue biopsy. Immunohistochemistry (IHC) staining was positive for cytokeratin, neuron specific enolase, synaptophysin and chromogranin and negative for leukocyte common antigen. This case showed a high proliferative activity (Ki-67 labeling index were 60%). These IHC findings were in favor of poorly differentiated neuroendocrine carcinoma. After surgery, she received chemotherapy and chemoradiation. The diagnosis of neuroendocrine tumors in the present case is based on immunohistochemical markers and cellular shapes. Postoperative chemoradiotherapy is a critical element of therapy for head and neck high-grade neuroendocrine carcinomas, our patient received this treatment after surgery. [ABSTRACT FROM AUTHOR]

The morphology of the major and minor glands in the Persian squirrel (Sciurus anomalus) is crucial for understanding this mammal's biological and ecological relationships. This study aimed to compare the macro- and microanatomies of the major and minor salivary glands in Persian squirrels. Five euthanized adult Persian squirrels were used for this investigation. The macroscopic and microscopic evaluations were carried out after dissection, and tissue samples were fixed and slides prepared. Afterward, specific staining methods were used for the histochemical study. The results of this investigation indicated that Persian squirrels, as pets and wild rodents, have several minor salivary glands, including caudal buccal, lingual, palatine, and palatoglossal salivary glands. The rostral buccal region and labial region of the Persian squirrel were sectioned serially, but there were no salivary secretory units in these regions. The parotid gland was completely acinar serous, the submandibular salivary gland was branched tubuloacinar seromucous, and the sublingual gland was tubuloacinar seromucous. In addition, in this research, the two parts of the sublingual gland, monostomatice, and polystomatic parts, were completely determined. The striated, intercalated, interlobar, and excretory ducts composed the ductal system of the major salivary glands in Persian squirrels. The interlobar septa between the lobules contained blood vessels and interlobar ducts. The myoepithelial cells were seen around the secretory units of all major salivary glands. In conclusion, it can be noticed that the major and minor salivary glands of the Persian squirrel had been scattered in the oral cavity, such as the palatine, tongue, and buccal regions, and the secretory units of these glands were different from other rodents due to the difference in nutrition of the Persian squirrel. Key Clinical Message: Adenoid cystic carcinoma (ACC) is an uncommon malignancy of head and neck. Although the cornerstone of treatment is surgery, concurrent chemoradiotherapy (CRT) might be used as an effective treatment for unresectable tumors. Herein we report a case of massive ACC of base of tongue with durable complete response to definitive CRT. Adenoid cystic carcinoma (ACC) is a rare tumor accounting for 1% of all head and neck cancers. The best treatment option is complete surgical resection with or without adjuvant radiotherapy. When surgical resection is not feasible, definitive radiotherapy with or without concurrent chemotherapy can be considered. Herein we report a non‐smoker 72‐year‐old woman presented with throat discomfort and sensation of a lump. Evaluation revealed an unresectable adenoid cystic carcinoma of the base of tongue in whom complete clinical response was achieved after definitive concurrent chemoradiation. Although the cornerstone of treatment is complete surgical resection, this case report indicates that concurrent chemoradiotherapy might result in complete clinical response and could be used as a definitive treatment in selected ACC tumors. 26. Evaluation of Ventilation Methods and Cardiorespiratory Outcomes in Children with Foreign Body Aspiration Undergoing Rigid Bronchoscopy Within 2008 - 2019.

Background: Foreign body aspiration is a common accident with life-threatening outcomes. Rigid bronchoscopy is considered an effective tool in the diagnosis and treatment of this complication; however, there is still no agreement on the preferred ventilation method for the treatment of patients with foreign body aspiration. Objectives: This study aimed to compare the cardiovascular and respiratory outcomes followed by ventilation methods in patients with foreign body aspiration undergoing rigid bronchoscopy. Methods: In this cross-sectional study, the information was recorded from 90 patients' files, including age, gender, aspirated foreignbody, interval from aspiration to referral, signs, andsymptoms, comorbidity, bronchoscopic results, ventilationmethod during the procedure, occurrence of respiratory complications, heart rate, blood pressure, arterial oxygen saturation, length of hospital stay, and mortality of patients. The data were analyzed by SPSS software (version 22). A p-value less than 0.05 was considered statistically significant. Results: Out of 90 reviewed cases, 57.67% were male, and the mean age was 2.06-1.25 years. The most common aspirated foreign body was peanuts (38.89%). The mostcommonclinical signs were cough (81.11%) and wheezing (51.11%). The mostcommonsymptoms were wheezing (54.44%) and decreasing respiratory sounds (46.67%). Bronchoscopy was associated with foreign body extraction in 65.6% of the patients. The relationship between the ventilation method with blood pressure changes, length of hospital stay, cardiopulmonary complications, and oxygen saturation was not significant; however, the cases with spontaneous breathing referred at shorter intervals from aspiration and experienced fewer heart rate changes. Conclusions: The results of the present study showed no preference between spontaneous and controlled ventilation during rigid bronchoscopy in patients with foreign body aspiration. Undesired effects of synthetic antidiabetic agents have made researchers to seek for safer and healthier resources. With this aspect, herbal materials have attracted substantial research interest and are being extensively investigated. Considering that herb-drug interactions can be a double-edged sword presenting both risks and benefits, investigation of such interactions is greatly in demand. to investigate possible beneficial effects of hydroalcoholic extract of Securigera Securidaca seed (HESS) on antioxidant capacity, fibroblast growth factor 21 (FGF21) and insulin resistance in Streptozotocin (STZ)-induced diabetic rats, alone and in combination with glibenclamide. Forty male Wistar rats were randomly divided in to eight equal groups including healthy and diabetic controls and six treated groups with a various doses of HESS alone and in combination with glibenclamide, for 35 consecutive days. Serum samples were taken and analyzed for biochemical profile, HOMA indexes, FGF21, oxidative/nitrosative stress and inflammatory biomarkers as compared with the controls. Moreover, total phenolic and flavonoid contents of herbal extract were assessed. The herbal extract was found to be rich in flavonoid and phenolic components. Both of glibenclamide and the HESS decreased glucose and insulin resistance, as well as increased body weight and insulin sensitivity. Moreover, the extract could mitigate oxidative/nitrosative stress and inflammation dose-dependently, however, the standard drug was less effective than HESS. Induction of diabetes increased FGF21 levels and both of the treatments could reduce its contents, however, glibenclamide was more effective than HESS. The results clearly show that there is no contradiction between HESS and glibenclamide. Moreover, the herbal extract could augment antioxidant and anti-inflammatory properties of the standard drug. Background and purpose: Radiotherapy (RT) is an essential treatment modality against cancer and becoming even more in demand due to the anticipated increase in cancer incidence. Due to the rapid development of RT technologies amid financial challenges, we aimed to assess the available RT facilities and the issues with achieving health equity based on current equipment compared to the previous reports from Iran. Materials and methods: A survey arranged by the Iran Cancer Institute's Radiation Oncology Research Center (RORC) was sent to all of the country's radiotherapy centers in 2022. Four components were retrieved: the reimbursement type, equipment, human resources, and patient load. To calculate the radiotherapy utilization rate (RUR), the Lancet Commission was used. The findings were compared with the previous national data. Results: Seventy-six active radiotherapy centers with 123 Linear accelerators (LINACs) were identified. The centers have been directed in three ways. 10 (20 LINACs), 36 (50 LINACs), and 30 centers (53 LINACs) were charity-, private-, and public-based, respectively. Four provinces had no centers. There was no active intraoperative radiotherapy machine despite its availability in 4 centers. One orthovoltage X-ray machine was active and 14 brachytherapy devices were treating patients. There were 344, 252, and 419 active radiation oncologists, medical physicists, and radiation therapy technologists, respectively. The ratio of LINAC and radiation oncologists to one million populations was 1.68 and 4.10, respectively. Since 2017, 35±5 radiation oncology residents have been trained each year. Conclusion: There has been a notable growth in RT facilities since the previous reports and Iran's situation is currently acceptable among LMICs. The Z‐source inverter was introduced to overcome shortcomings of voltage source inverter and current source inverter. In this paper, the concept of one switched‐inductor Z‐source inverter is extended to switched‐capacitor inductor ZSI with an impedance network. In the proposed inverter, capacitors are used instead of the side diodes of the one switched inductor Z‐source inverter. The proposed inverter has a higher boost factor compared to One SL‐ZSI and quasi‐ZSI. Also, it has lower voltage stress compared to similar inverters. The proposed inverter can be extended to n cases in cascade form. The components used in the proposed topology are lower than the SL‐ZSI. The analysis, derivation of boost factor, capacitor voltages, diode voltages, and switch voltages stress of the proposed inverter topology are carried out and are compared with the similar inverters. Two new power factor correction (PFC) rectifiers with the voltage buck-boost capability are proposed in this article, called as Ferdowsi rectifiers (FRs). The proposed rectifiers employ a reduced number of semiconductors compared to the competitors. Also, since the reverse recovery problem of antiparallel diodes of transistor switches is avoided, then MOSFETs can be readily utilized, which lets much higher switching frequencies and lower size of passive components. In addition, two parallel capacitors provide a path for the input current during the discharge mode of the buck-boost inductors to improve the current quality. The discontinuous conduction mode operation of the proposed rectifiers guarantees a high quality input current with a low total harmonic distortion. All aforementioned features offer a highly efficient operation and at the same time a purely sinusoidal current drawn from the ac source compared to the previous well-developed competitors. Introduction: Seeds of Securigera securidaca (L.) Degen & Dorfl are rich in flavonoids and phenolic acids which have potent biological effects. The current study was undertaken to evaluate the effects of hydroalcoholic extract of S. securidaca seeds (HESS) alone, and in combination with a standard drug, glibenclamide (GB) on paraoxonase1 (PON1) activity, lipid profile and peroxidation, and cardiovascular risk indices in streptozotocin (STZ) induced diabetic rats. Methods: Forty-eight male Wistar rats were randomly divided into eight equal groups and orally treated with various doses of HESS (100, 200, 400 mg/kg) alone and in combination with GB (5 mg/kg) for 35 consecutive days. After blood sampling, lipid profile including triglyceride (TG), cholesterol, high, low and very low-density lipoprotein-cholesterol (HDL-C, LDL-C, and VLDL-C), as well as serum PON1 activity, were assessed. Malondialdehyde (MDA), tumor necrosis factor-alpha (TNF-a), and high-sensitivity C-reactive protein (hs-CRP) levels were also measured. Several indices of cardiovascular risk and the correlation between PON1 activity and these indices were calculated based on the obtained results from the lipid profile. Results: Induction of diabetes could dramatically alter all of the parameters mentioned above, and the lower dose of HESS (100 mg/kg) was not effective in restoring the parameters. However, the higher doses (200 and 400 mg/kg) alone and in combination with GB could significantly improve lipid profile, restore PON1 activity, and decrease cardiovascular risk indices, MDA, as well. However, neither HESS nor GB could significantly reduce TNF-a and hs-CRP. A significant negative correlation also was detected between PON1 activity and cardiovascular risk indices. Conclusion: Conclusively, HESS can be considered as a potent antihyperlipidemic agent with remarkable cardioprotective effects and can potentiate the antidiabetic effects of GB. Hepatitis B virus (HBV), along with Hepatitis C virus chronic infection, represents a major risk factor for hepatocellular carcinoma (HCC) development. However, molecular mechanisms involved in the development of HCC are not yet completely understood. Recent studies have indicated that mutations in CTNNB1 gene encoding for β-catenin protein lead to aberrant activation of the Wnt/ β-catenin pathway. The mutations in turn activate several downstream genes, including c-Myc, promoting the neoplastic process. The present study evaluated the mutational profile of the CTNNB1 gene and expression levels of CTNNB1 and c-Myc genes in HBV-related HCC, as well as in cirrhotic and control tissues. Mutational analysis of the β-catenin gene and HBV genotyping were conducted by direct sequencing. Expression of β-catenin and c-Myc genes was assessed using real-time PCR. Among the HCC cases, 18.1% showed missense point mutation in exon 3 of CTNNB1, more frequently in codons 32, 33, 38 and 45. The frequency of mutation in the hotspots of exon 3 was significantly higher in non-viral HCCs (29.4%) rather than HBV-related cases (12.7%, P = 0.021). The expression of β-catenin and c-Myc genes was found upregulated in cirrhotic tissues in association with HBV infection. Mutations at both phosphorylation and neighboring sites were associated with increased activity of the Wnt pathway. The results demonstrated that mutated β-catenin caused activation of the Wnt pathway, but the rate of CTNNB1 gene mutations was not related to HBV infection. HBV factors may deregulate the Wnt pathway by causing epigenetic alterations in the HBV-related HCC. [ABSTRACT FROM AUTHOR]

This paper introduces a novel single-phase buck–boost noninverting variable-frequency ac–ac converter that offers higher efficiency compared to the competitors. This converter utilizes a lower number of semiconductors. A simple and flexible switching strategy is also proposed, which generates the desired output waveform avoiding unnecessary high-frequency switching operation of semiconductor devices. A high reliable operation due to the elimination of the input source shoot-through risk, an inherent commutation capability that mitigates the voltage spikes across the semiconductors, a lower semiconductors rating requirement, an improved input current waveform quality, and a smaller required input filter inductor are the main advantages of the proposed converter. Thus, the proposed converter can be successfully applied to many industrial applications such as medium-frequency transformer isolation for traction and wind turbine converters, ac–dc high-voltage conversion based on Cockcroft–Walton circuit and induction heating systems. Introduction: Nonalcoholic fatty liver disease (NAFLD) frequently coexists with type 2 diabetes mellitus (T2DM) and synergistically contributes to the development of atherosclerosis. Flow-mediated dilation (FMD) is a commonly used noninvasive test for assessing endothelial function. The main objective of this study was to explore FMD in patients with T2DM with and without NAFLD. Methods: In this cross-sectional study, conducted on people with T2DM, NAFLD was defined as controlled attenuation parameter (CAP) score > 302 dB/m. Endothelial dysfunction was detected when arterial FMD of brachial artery was equal or less than 0.7%. Regression analyses were applied to assess factors associated with impaired FMD. Result: A total of 147 patients (72 with NAFLD and 75 without NAFLD) were included in the final analysis. Patients with NAFLD were more likely to develop FMD ≤ 7% (77.8% vs. 58.7%, P = 0.01). In multivariate analysis, NAFLD (OR = 2.581, 95% CI (1.18–5.62), P = 0.017) and hypertension (HTN) (OR = 3.114, 95% CI (1.31–7.35), P = 0.010) were associated with an increased risk of impaired FMD. However, female sex was associated with a decreased risk of impaired FMD (OR = 0.371, 95% CI (0.15–0.87), P = 0.024). Conclusion: NAFLD is associated with endothelial dysfunction in people with T2DM. This risk is comparable with the risk imposed by HTN, highlighting the importance of screening and management of NAFLD in these patients. [ABSTRACT FROM AUTHOR]

Introduction: Type 2 diabetes mellitus (T2DM) is a significant risk factor for the development of critical limb ischemia (CLI), the most advanced stage of peripheral arterial disease. The concurrent existence of T2DM and CLI often leads to adverse outcomes, namely limb amputation. Objective: To identify biomarkers for improving the screening of CLI in high-risk people with T2DM. Methods: We investigated metabolome profiles in serum samples of 113 T2DM people with CLI (n = 23, G2) and without CLI (n = 45, G0: no lower limb stenosis (LLS) and n = 45, G1: LLS < 50%), using hydrogen nuclear magnetic resonance (1H NMR) approach. Principle component analysis (PCA) and partial least squares-discriminant analysis (PLS-DA) were used to analyze 1H NMR data. Results: Twenty potential metabolites that could discriminate people with T2DM and CLI (G2) from non-CLI patients without LLS (G0) were determined in serum samples. The correct percent of classification for the PLS-DA model for the test set samples was 85% (n = 20) and 100% (n = 5) for G0 and G2 groups, respectively. Non-CLI patients with LLS < 50% (G1) were projected on the PCA abstract space built using 20 discriminatory metabolites. Eleven people with T2DM and LLS < 50% were prospectively followed, and their ankle-brachial index (ABI) was measured after 4 years. A promising agreement existed between the PCA model's predictions and those obtained by ABI values. Conclusion: The findings suggest that confirmation of blood potential metabolic biomarkers as a complement to ABI for screening of CLI in a large group of high-risk people with T2DM is needed. Background: One of the most important fundamental issues in mental health is patients' satisfaction with the beauty of their teeth and their compatibility with complete dentures. Therefore, this study investigated the role of early maladaptive schemas and body image in the level of satisfaction with complete dentures among patients referred to the dental clinic of Islamic Azad University of Tabriz, Iran, in 2021-2022. Methods: The research was a descriptive correlational study. This study's statistical population included all candidates for complete dentures referring to the dental clinic of Islamic Azad University of Tabriz in 2021-22, from among which, 168 people were selected through convenience sampling method. The research tools included the Young Schema Questionnaire-Short Form (YSQ-SF) (1988), Body Image Concern Inventory (BICI) Littleton et al. (2005), and Dental Satisfaction Questionnaire (DSQ) by Davies and Ware (1982). The collected data were analyzed using multiple regression tests in SPSS software. Results: The results showed a relationship between the components of early maladaptive schemas [failure (B = -0.225), defectiveness/shame (B = -0.279), social isolation/alienation (B = 0.278), abandonment/instability (B = 0.331), subjugation (B = -0.264), and dependence/incompetence (B = 0.246)] and the level of satisfaction with complete dentures among the patients (P < 0.05). The results also showed a significant relationship between the components of body image and the level of satisfaction with complete dentures (P < 0.05). Conclusion: It can be concluded that early maladaptive schemas and body image playa role in the satisfaction level of patients referred to the dental clinic of Islamic Azad University of Tabriz. Continuous welded rail (CWR) tracks have particular advantages over common tracks with jointed rails such as increased ride comfort, reduced noise and vibration and decreased maintenance costs due to the removal of joints in rail connections. Alternatively, some complications associated with CWR tracks, for instance increased lateral forces, are the main reason of track buckling and its subsequent lateral deformation. These problems are usually more severe in curved tracks. In order to overcome the large lateral forces caused by temperature deviations of CWR tracks which results in railway vehicle instability, the ballasted track lateral resistance should be improved. Among the various methods proposed in this area, no specific study has been carried out on the effect of geogrid reinforcement on ballasted track lateral resistance. Thus, the present research was allocated to investigating the effect of geogrid on the lateral resistances of both single tie and track panel via laboratory and field tests. In this regard, at the first stage, the ballast layer was reinforced with various number of geogrid layers, the effect of which was investigated by conducting the single tie push test (STPT) in the lab environment to assess the optimum number of geogrid layers and their installation levels along the ballast layer thickness. Afterwards, a test track was executed in the field including various sections which were reinforced in the same way as the lab tests. Consequently, many STPTs and track panel displacement tests (TPDTs) were accomplished. As a result, the STPTs in the lab and field confirmed more than 31% and 42% increase in single tie lateral resistance for ballast layers reinforced respectively with one and two geogrid layers, while these values were reached to 29% and 40% in the case of TPDT. Introduction: Nigella sativa (N. sativa), one of the most commonly used medicinal herbs with antioxidant properties, increases blood insulin levels and lowers fasting blood sugar. Nuclear Erythroid Factor-Related Factor 2 (Nrf2) and Fibroblast Growth Factor 21 (FGF21) are two antioxidant factors that are increased by oxidative stress and hyperglycemia. The present study investigated how hydroalcoholic extract of N. sativa seed (HENS) increases blood insulin levels, taking into account changes in antioxidant factors and expression of insulin transcription factors. Materials and methods: Two groups of male diabetic wistar rats were treated orally with HESN at doses of 200 and 400 mg/kg-body weight for one month. Fasting blood sugar (FBS) and insulin were measured using standard kits by photometric and ELISA methods, respectively. The expression levels of the Nrf2, FGF21 and β-Klotho genes as well as the insulin gene-stimulating transcription factors of MafA and PDX-1 were evaluated using real-time PCR. Oxidative stress was assessed by assessing serum total oxidation status (TOS), malondialdehyde (MDA), and total antioxidant capacity (TAC). Results: HSEN showed a significant reducing effect on FBS and oxidative biomarkers and an increasing effect on serum insulin levels in treated diabetic rats compared to untreated diabetics (P < 0.05). The elevated levels of NRF2 and FGF21 in the liver and pancreas of the diabetic control group were significantly reduced after treatment with both HESN doses (P < 0.05). Following the ameliorative effects of HENS on pancreatic tissue and the reduction of oxidative stress, the expression level of MafA and PDX1 genes approached the level of these factors in healthy rats (P < 0.05). Conclusion: This study showed the therapeutic effects of HENS on diabetic pancreas by reducing oxidative stress and tissue damage, modifying the expression levels of PDX-1 and MafA genes, and regulating insulin secretion and blood glucose levels. Aim: In the present study, we evaluated treatment tolerance and side effects of 6 days a week accelerated radiation therapy using concomitant boost methods with chemotherapy in locally advanced head and neck cancer. Materials and Methods: Thirty patients suffered locally advanced head and neck malignancies were included into this clinical trial. The patients were scheduled for accelerated radiotherapy with total dose of 70 Gy 6 days a week (5 days radiotherapy and 1-day concomitant boost radiotherapy) for 5 weeks and also concurrently for chemotherapy with cisplatin and also celecoxib. Results: The average age of the patients was 51.47 ± 11.49 years. The incidence of acute mucositis at the end of the 1st week was 33.3% that was gradually increased until the end of the 5th week (93.3%) and then had a ecreasing trend within the 6th week (70.0%). The incidence of acute dysphagia was estimated 23.3% at the end of the 1st week and reached 60% at completion of treatment. Conclusion: Scheduling a treatment approach with 6 days a week, accelerated radiation therapy using concomitant boost methods with chemotherapy, and celecoxib leads to significant reducing the incidence of complications in the final weeks of therapy in patients with locally advanced head and neck cancer. In this study, a new systematic approach is proposed to design the fuzzy controller for a class of Takagi–Sugeno fuzzy‐partial differential equation (TS fuzzy‐PDE) systems which describe the non‐linear distributed parameter system formulated by first‐order semi‐linear hyperbolic PDEs. In this study, non‐quadratic Lyapunov function is utilised and some slack matrices are introduced to derive stability conditions in terms of linear matrix inequalities (LMIs). The proposed approach has three main features. First, stability conditions are not derived in the form of spatial differential LMI. Second, conservativeness of LMI conditions is reduced. Third, there is no restriction on the form of semi‐linear hyperbolic PDE systems and therefore more semi‐linear systems classes can be stabilised. Purpose: Post-surgery radiation can reduce the risk of loco-regional relapse in high-intermediate-risk endometrial cancer. High-dose-rate vaginal cuff brachytherapy (HDR-BRT) is an acceptable method of radiation in majority of endometrial cancer cases. Although 2D planning is frequently used for treatment based on physical examination without any imaging, measurement of the dose received by organs-at-risk (OARs) is not possible. Therefore, the present study was the first to compare dose delivered to target and OARs in 2D vs. 3D planning in patients treated with cobalt-60 source. Material and methods: In this study, organs including vagina wall, bladder, rectum, and sigmoid were contoured on computed tomography (CT) scan images of 37 endometrial cancer patients, and doses delivered to organs were recorded. Statistics, such as D90, D99, V100, V150, V200, D0.1cc, D1cc, and D2cc were determined. Results: D90 and D99 were lower in 3D treatment planning in comparison with 2D. Although V100 was more in 3D planning, V150 and V200 were less. Analyzing D0.1cc, D1cc, and D2cc of OARs revealed that doses given to rectum, sigmoid, and bladder were less in 3D planning compared to 2D. Conclusions: Comparison of 2D and 3D planning results showed that 3D planning could deliver an appropriate dose to the target while sparing more OARs. [ABSTRACT FROM AUTHOR]

Rectal cancer is one of the most prevalent cancers in the world. In many countries, the current standard of care is long-course chemoradiation (CRT), followed by total mesorectal excision. Some efforts have been made by intensifying radiation or chemotherapy components of the neoadjuvant therapy to further decrease the local recurrence and augment surgery's feasibility and improve the oncological outcomes. This paper reviews recent intensified neoadjuvant interventions in locally advanced rectal cancer (LARC) in terms of efficacy and treatment-related toxicity. Many maneuvers have been made so far to improve the oncological outcomes of rectal cancer with intensified neoadjuvant longcourse CRT. Some of these approaches seem compelling and deserve further study, while some have just increased the treatment-related toxicities without evident benefits. Those endeavors with greater pathological complete response than the standard of care may make us await the long-term results on survival rates and chronic treatment-related toxicity. After introduction of neoadjuvant CRT for LARC there have been many efforts to improve its outcomes. Here, this study gathered most of these efforts that intensified the neoadjuvant therapy with some being promising and some being futile. [ABSTRACT FROM AUTHOR]

Introduction: To evaluate the efficacy of empagliflozin compared to pioglitazone in patients with non-alcoholic fatty liver disease (NAFLD) and type 2 diabetes (T2DM). Methods: In this prospective randomized, double-blind, placebo-controlled trial, we assigned 106 patients with NAFLD and T2DM to receive empagliflozin 10 mg (n = 35), pioglitazone 30 mg (n = 34), or placebo (n = 37) for 24 weeks. Liver fat content and liver stiffness were measured using fibroscans. Body composition assessment was performed by dual-energy x-ray absorptiometry (DEXA) scans. The primary end point was change from baseline in liver steatosis, using the controlled attenuation parameter (CAP) score. Results: A borderline significant decrease in CAP score was observed with empagliflozin compared to placebo, mean difference: − 29.6 dB/m (− 39.5 to − 19.6) versus − 16.4 dB/m (− 25.0 to − 7.8), respectively; p = 0.05. Using multivariate analysis, we observed a significant reduction in the placebo-corrected change in liver stiffness measurement (LSM) with empagliflozin compared to pioglitazone: − 0.77 kPa (− 1.45, − 0.09), p = 0.02, versus 0.01 kPa (95% CI − 0.70, 0.71, p = 0.98), p for comparison = 0.03. Changes in serum aspartate aminotransferase (AST) and alanine aminotransferase (ALT), fasting insulin, homeostatic model assessment for insulin resistance (HOMA-IR), HOMA2-IR, fibrosis-4 index (FIB4 index), NAFLD fibrosis score, aspartate aminotransferase to platelet ratio index (APRI), android/gynecoid ratio (A/G ratio), and skeletal muscle index (SMI) were comparable between the two treatment groups, while significant reductions of the body weight and visceral fat area were observed only in the empagliflozin group (p < 0.001 and p = 0.01, respectively) and both were increased in the placebo and pioglitazone groups. There were no serious adverse events in either group. Conclusion: Treatment for 24 weeks with empagliflozin, in contrast to pioglitazone, was associated with improvement of liver steatosis and fibrosis in patients with NAFLD and T2DM. Purpose: Colorectal cancer is becoming an increasing concern in the middle-aged population of Iran. This study aimed to compare the preliminary results of short-course and long-course neoadjuvant chemoradiotherapy treatment for rectal cancer patients. Materials and Methods: In this clinical trial we recruited patients with rectal adenocarcinoma located from 5 cm to 15 cm above the anal verge. Patients in group I (short-course) received three-dimensional conformational radiotherapy with a dose of 25 Gy/5 fractions in 1 week plus concurrent XELOX regimen (capecitabine 625 mg/m² from day 1-5 twice daily and oxaliplatin 50 mg/m² on day 1 once daily). Patients in group II (long-course) received a total dose of 50-50.4 Gy/25-28 fractions for 5 to 5.5 weeks plus capecitabine 825 mg/m² twice daily. Both groups underwent consolidation chemotherapy followed by delayed surgery at least 8 weeks after radiotherapy completion. The pathological response was assessed with tumor regression grade. Results: In this preliminary report on complications and pathological response, 66 patients were randomized into two study groups. Mean duration of radiotherapy in the group II (long-course) was 5 ± 1 days (range, 5 to 8 days) and 38 ± 6 days (range, 30 to 58 days). The median follow-up was 18 months. Pathological complete response was achieved in 32.3% and 23.1% of patients in the shortcourse and long-course groups, respectively (p = 0.558). Overall, acute grade 3 or higher treatment- related toxicities occurred in 24.2% and 22.2% of patients in group I and II, respectively (p = 0.551). No acute grade 4 or 5 adverse events were observed in either group except one grade 4 hematologic toxicity that was seen in group II. Within one month of surgery, no significant difference was seen regarding grade =3 postoperative complications (p = 0.333). Conclusion: For patients with rectal cancer located at least 5 cm above the anal verge, short-course radiotherapy with concurrent and consolidation chemotherapy and delayed surgery is not different in terms of acute toxicity, postoperative morbidity, complete resection, and pathological response compared to long-course chemoradiotherapy. We aimed to compare the sonographic measurement of median nerve cross-section area (CSA) in patients with Amyotrophic Lateral Sclerosis (ALS) and healthy individuals. The effect of duration of the disease on correlations between paraclinical findings and ALS functional rating scale (ALSFRS) were secondarily aimed to be evaluated. The cross-sectional study was approved by the Ethical Committee of Iran University of Medical Sciences and conducted between January 2017 and December 2018. We evaluated the median nerve surface area by means of sonography in 35 ALS patients and 35 healthy controls. Compound muscle action potential (CMAP) amplitudes during nerve conduction study and ALSFRS were recorded by the same trained specialist. Data were analyzed using SPSS software version 18. We did not find a significant difference between CSA in ALS patients and the normal population (P>0.05). Comparing to normal individuals, the mean CMAP decreased significantly in ALS patients (6.6±3.07 mV versus 10.25±2.2 mV, P<0.001). ALSFRS correlated with both CSA of the median nerve at the wrist (P:<0.001, r:0.78) and the CMAP (P:<0.001, r:0.74) that were confirmed by regression models designed to consider the effect of disease duration on these correlations. CSA was not different between ALS patients and the normal population, but CMAP decreased in ALS patients. ALSFRS correlated with both CSA and CMAP of the median nerve. [ABSTRACT FROM AUTHOR]

During an organism’s evolution, functional adaptations help species to become better suited to their ecological niches. From the morphological aspect, these adaptations are reflected in the anatomical specializations of different organs. Specializations of the lingual organ is a critical adaptation of birds, such as the white-headed duck (Oxyura leucocephala), that enables their nutritional requirements to be met. For optimal use of the available food resources, the white-headed duck utilizes three methods of food collection, namely pecking, grazing and filter-feeding. Since this species is classified as endangered, we conducted the present study on two carcasses of the white-headed duck (death due to natural causes) employing routine histological methods, light microscopy and scanning electron microscopy. Our results show that the tongue of this bird shares some similarities and some differences with the tongue of other members of the family Anatidae. The results confirm that it is better adapted to the filter-feeding method rather than to other types of food intake. This adaptation is reflected by anatomical specializations of its lingual structures, including the stair-like outline shape, bi-sectional lingual body, a deep median sulcus, lateral conical papillae, mucus secreting glands, lack of serous secreting glands, cartilaginous skeleton and the triangular fibromuscular structure of the lingual body. The so-called triangular structure and cartilaginous skeleton are the major structures involved in the lingual motions during the filter-feeding method. The presence of the triangular structure and its connection with the cartilaginous skeleton and lingual mucosa have not previously been reported in any species of birds. [ABSTRACT FROM AUTHOR]

Introduction: Avascular necrosis (AVN) of the femoral head is an increasingly common cause of musculoskeletal disability with unknown etiology. Traumatic and non-traumatic factors can be effective in its occurrence. About 50% of cases are bilateral. The underlying treatment is prosthetic replacement surgical procedure. Case Presentation: We report a case of bilateral AVN of femoral heads who was a candidate for prosthetic replacement surgery but improved significantly, using laser acupuncture. The patient was a 55-year-old woman with a diagnosis of bilateral osteonecrosis of the femoral head for five years. As the patient declined the surgical option, laser acupuncture was started for pain control. Regarding the signs of bilateral femoral head recovery (according to the MRI scan criteria), a total of 3 courses of laser acupuncture (each course was 20 sessions) with 2 months intervals was performed. Ten acupuncture points: Li4, Li11, St36, SP6, LIV3, GB4, GB5, GB6, GB13, GB14, GB20, GB30, GB31, GB34, were irradiated bilaterally using red 650 nm laser, 100 mW, 1505 Hz frequency, duty cycle 50 3 J/point and near infrared 810 nm laser, 100 mW, 1705 Hz frequency, duty cycle 50 3 J/point, for 2 minutes. The patient received 3 laser therapy courses. During the first-course, laser therapy was done every other day to reduce pain. For the second and third courses, according to pain decrease, therapy was done every week. Results: The results of the MRI scans and x-ray studies show progressive regeneration of the right femoral head from VI to B II and of the left from V to C II (based on the standard table of Pennsylvania). Conclusion: It seems that this procedure may be mentioned in future research projects, especially in cases with high risks of surgery. [ABSTRACT FROM AUTHOR]

Purpose: This study aimed to assess complications and outcomes of a new approach, that is, combining short course radiotherapy (SRT), concurrent and consolidative chemotherapies, and delayed surgery. Materials and Methods: In this single arm phase II prospective clinical trial, patients with T3-4 or N+ M0 rectal adenocarcinoma were enrolled. Patients who received induction chemotherapy or previous pelvic radiotherapy were excluded. Study protocol consisted of three-dimensional conformal SRT (25 Gy in 5 fractions in 1 week) with concurrent and consolidation chemotherapies including capecitabine and oxaliplatin. Total mesorectal excision was done at least 8 weeks after the last fraction of radiotherapy. Primary outcome was complete pathologic response and secondary outcomes were treatment related complications. Results: Thirty-three patients completed the planned preoperative chemoradiation and 26 of them underwent surgery (24 low anterior resection and 2 abdominoperineal resection). Acute proctitis grades 2 and 3 were seen in 11 (33.3%) and 7 (21.2%) patients, respectively. There were no grades 3 and 4 subacute hematologic and non-hematologic (genitourinary and peripheral neuropathy) toxicities and perioperative morbidities such as anastomose leakage. Grade 2 or higher late toxicities were observed among 29.6% of the patients. Complete pathologic response was achieved in 8 (30.8%) patients who underwent surgery. The 3-year overall survival and local control rates were 65% and 94%, respectively. Conclusion: This study showed that SRT combined with concurrent and consolidation chemotherapies followed by delayed surgery is not only feasible and tolerable without significant toxicity but also, associated with promising complete pathologic response rates. Background Clinical trials investigating the effects of addition of oxaliplatin to neoadjuvant radiochemotherapy in locally advanced rectal cancers (LARCs) have brought controversial results for pathologic complete response as an endpoint. This randomized clinical trial investigated downstaging as a short-term surrogate for progression-free survival (PFS). Methods Patients with magnetic resonance imaging (MRI) defined T3, T4 or N+ histologically proven adenocarcinoma of rectum within 15 cm from anal verge were randomly assigned to receive 50-50.4 Gy external beam radiation in 25-28 fractions and concurrent capecitabine 825 mg/m2 twice daily 5 days a week with or without oxaliplatin 60 mg/m2 weekly as neoadjuvant radiochemotherapy (Capox and Cap group, respectively). T downstage was defined as at least one stage regression in pathologic report after surgery comparing to MRI image before the preoperative treatment. Adverse effects of treatment were recorded on a weekly basis according to National Cancer Institute Common Toxicity Criteria, version 4. Results Sixty-three patients were randomly assigned to Cap ( n = 31) and Capox ( n = 32) groups. There was no grade 4 toxicity. The only grade 3 toxicity that occurred more in Capox group was diarrhea (22% vs 0%; P = 0.006). Histopathologic stage of 52 patients (27 patients in Cap and 25 patients in Capox groups) was compared to their preoperative stage defined by MRI. There was a greater rate of T downstage in Capox group (59% vs 42%; P = 0.037). Eleven patients in Capox group (34%) achieved pathologic complete response, comparing to four in Cap group (13%); P = 0.072. Conclusion The addition of oxalipatin to neoadjuvant radiochemotherapy in LARC led to higher rate of tumor downstaging. Longer follow-up is needed to evaluate PFS. [ABSTRACT FROM AUTHOR]
