Individualized funding of disability support services has implications for people's choices about when to share their home. This paper examines how people with disabilities made choices about who to live with and the factors influencing these choices. This paper discusses data from interviews with 30 people with mostly intellectual disabilities using individualized support services, 21 interviews with family members, four interviews with service managers, and a focus group with five support workers. The data come from a large evaluation of individualized housing support programs in New South Wales, Australia. Only some people had the opportunity to choose whether to share and with whom. Their choices were constrained by the range of housing options and their limited experience of them, even when they had support to make choices about shared housing or living alone. In some cases, the choices reflected a conceptualization of people with disabilities as different to other citizens in their rights and expectations about their social arrangements. The results have implications for information sharing, housing stock, and the need to challenge the positioning of people with disabilities relative to other people regarding choices about where and with whom to live. Many people preferred not to live alone, so as to improve their economic and social circumstances, and their choice and control. The choices about shared housing that many people and their supporters made were constrained by their limited experience of housing options or their familiarity with the range of choices made by other people with disabilities. Being able to draw on the material, social, and information resources of family made a big difference to their housing choices. It raises questions for policy implementation about whether individualized support may lock some people into shared housing arrangements by failing to include housing costs in the individual package. Take-home naloxone (THN) is provided to non-medically trained people to reverse potential opioid overdoses. There is an increasing range of effective intramuscular (IM) and intranasal (IN) naloxone devices and this paper explores the types preferred by people who use opioids, using consumer behaviour literature to interpret the findings. Data derive from two unconnected qualitative studies involving audio-recorded semi-structured interviews. Study 1 was conducted in the United States (n = 21 users of non-medical/illicit opioids). Study 2 was conducted in Australia (n = 42 users of non-medical/illicit or prescribed opioids). Most participants preferred IN naloxone. Preferences were based on the ease, speed, safety and comfort of each device and underpinned by accounts of overdose revivals as being very rushed and frightening situations. Preferences related to complex interactions between the naloxone device ('product'); the knowledge, skills, experience and attitudes of the lay responder ('consumer'), and when, where and how naloxone was to be used ('usage situation'). THN programs should offer choice of device when possible and nasal naloxone if resources permit. Asking people which devices they prefer and why and treating them as valued consumers of naloxone products can generate insights that improve future naloxone technology and increase THN uptake and usage. Children and young people in rural Australia experience disadvantage compared with metropolitan counterparts, with low educational attainment and disengagement from schooling being linked to poorer health outcomes. Schools are an existing contact point between individuals and health services. However, these health services are often overburdened and have limited scope to address broader social issues and teaching staff are focused on achieving curriculum outcomes. Embedding social workers within schools may provide an avenue for supporting students, yet social workers are rarely present in New South Wales public schools. This paper describes a study to determine teachers' understanding of the socioeconomic issues faced by school students, and the role of the social work profession in addressing such issues as part of the early stages of establishing a Social Work in Schools (SWiS) project. Eighteen semistructured interviews were undertaken with teachers across four rural Australian schools. Participants identified their awareness of potential risk factors that could lead to educational disengagement, reflected on their own limitations in dealing with these broader health presentations given their teaching focus, and provided insights into their understanding of the potential role of social workers. Rural and remote school children in Australia are at risk of reduced educational attainment and disengagement due to low socioeconomic status (SES), geographical location, parental education and employment, and family dysfunction. Social workers should advocate for the role they can play within rural school communities to assist in ameliorating the impact of socioeconomic factors on a student's capacity to engage in learning. Effective working relationships are those which are characterised by close personal contact, even friendship-like in nature, contained within professional boundaries. However, such personal ways of working remain a contentious aspect of professional relationships. Indeed, workers operating in this way commonly experience disapproval from colleagues. Guidance regarding how workers build personalised professional relationships while operating in a disapproving environment is limited. This is even more pronounced for building relationships with families where child neglect is an issue. This paper draws on a study of perceptions of eight parent–family worker relationship cases in New South Wales, Australia. The study utilised qualitative research methods to analyse and compare participants' perceptions of the experiences and meaning of working relationships with families where child neglect is an issue. The paper will explore how the workers' use of friendship-like characteristics to build highly personalised, yet professional and ethical, relationships created discord with some colleagues, including accusations of unprofessional and unethical practice. Drawing on these workers' experiences, along with literature that challenges traditional ways of working with clients, it will be argued that dominant ideas about ethical and professional practice hinder the development of effective relationships with clients, create unnecessary stress for workers, and should be challenged. Infant well-being is intrinsically linked to maternal physical and emotional well-being. Internationally health services have implemented policies to identify women at risk of mental health problems and developed effective care pathways. The aim of this paper is to describe how perinatal and infant mental health clinicians perceive their role and the attachment-based interventions they use in their work. The study comes from a larger mixed methods study, which examined two specialist perinatal and infant mental health services in New South Wales (Australia). Two hundred and forty-four medical records were reviewed, and six perinatal and infant mental health clinicians participated in in-depth semi-structured interviews. Data were analysed by content and thematic analysis. One overarching theme, modelling a secure base and three supporting themes, enhancing reflective capacity, enhancing emotional regulation and enhancing empathy emerged from the analysis. These findings demonstrate how perinatal and infant mental health clinicians use attachment theory to inform practice by modelling "holding" and being a secure-base for women. They also provide a clearer understanding of perinatal mental health practice and can be used to inform educational programs for multidisciplinary mental health professionals particularly those working with women and infants. The relationship of parents and infants is ruptured at birth in the context of urgent issues such as congenital heart defects. The infants referred to in 10 semi-structured interviews had undergone cardiac surgery in a children's hospital in Australia and were participating in a new programme known as Heart Beads instigated by the cardiac ward nurses. Within the biomedical context infants are often alienated from their parents and treated as isolated bodies. The paper discusses the social implications of an ill or deformed child and the role of narrative in addressing social demands and the problems faced by parents in infant intensive care contexts. It is argued that theorising the infant within a relational framework allows agency to be attributed to the infant and parents while also recognising the importance of the intimate social world and interactional context that it is expressed within. The paper draws on Nick Crossley's relational sociology to highlight the importance of the relational embodiment that encompasses infants within a social world preceding them and enveloping them. The narrative is developed by parents in order to facilitate the social expression and integration of their child and thus plays a particularly social role in the context of infant surgery, rather than serving to adjust the individual to new circumstances. Exploring the role of narrative in this social relational sense is important for bringing the mother and the family back into the context and survival of the infant both physically and socially. This paper presents findings from a larger, qualitative study that explored the potential of a school-based teacher community of practice as a model for a transformative form of teacher professional development. This paper reports on initial findings from a grounded theory exploration of the factors that facilitated and constrained the evolution of a community of practice among a team of middle years' teachers in a small central school in rural New South Wales, Australia. This paper reports on a small-scale, qualitative study on children's perspectives about their participation in decision-making processes regarding supervised contact. The paper begins with an overview of the study and a summary of findings in relation to four key research questions framed around the idea of children having a say, that is, children's views and perspectives of their participation in family law decision-making processes. These key questions include: What are children's experiences of having a say? What are children's understandings of having a say? Did children want a say in the decision for them to have supervised contact? How did having (or not having) a say feel? Discussion focuses on what importance children place on having a say in family law matters, a finding that is contrasted with children's experiences of marginalisation and exclusion from decision-making processes and of ambivalence and reluctance sometimes expressed around having a say. Children's idea of having a say as taking place in and through particular forms of dialogue and conversation, thus enabling the recognition of children and respect for what they have to say, are also explored. In this paper we investigate the problems experienced by single African women of refugee background being resettled in Wagga Wagga (Wagga), New South Wales, Australia. Wagga is one of the regional centres that the Australian government has earmarked for resettlement of people arriving under the refugee resettlement, better known as "humanitarian", programs. In this exploratory qualitative research we have focused on the challenges women encountered in accessing services and the stigma of single parenthood within their cultures. Individual interviews were conducted with 10 women of varying ages and African nationalities. The research found that the key concerns were a lack of knowledge about available services, few specific and culturally relevant African services, feelings of isolation, and African community attitudes towards single women. This paper illuminates two under-investigated, and often intersecting, frontline services accessed by unemployed people. It reports on the narratives of 32 Australian service providers working in emergency relief or employment services during 2012, including perceptions of clients' presenting issues and problems, and rationale for intervention. Workers tended to use reductionist and individualistic terms to describe complex client issues suggesting self-motivation and personal responsibility were key to gaining employment or alleviating poverty. Clients were often viewed as having the power to change their circumstances, while workers appeared unaware of their positional power and drew from dominant conservative discourse about welfarism and unemployment to fortify their compliance requirements. Background: Sexually transmitted infections (STIs) are prevalent throughout the world with the rate of these infections increasing on a daily basis. STI acquisition has the ability to cause personal adversity and elicit feelings of stigma and shame. Aim: The aim of this paper is to report on the sources of support young women who acquired STIs drew on to overcome their associated adversity. Methods: This study utilised a feminist qualitative methodology. Findings: Findings revealed that the women drew on both personal and anonymous sources of support. Conclusion: Nurses and other health-care professionals need to be equipped with knowledge and strategies to promote personal wellbeing and minimise the adversity felt among people having acquired these infections. This study examines experiences of living with chronic illness for haemodialysis patients. In order to understand these experiences the paper takes an existential-phenomenological approach. Interviews of seven participants (five males, two females) were collected in a haemodialysis clinic. Based upon the participants experiences three core themes emerged: (1) the challenges of living with chronic renal failure; (2) body changes and embodiment; (3) their illness experience and social relationships. The findings suggest that the illness experience of chronic renal failure is an on-going struggle to attain a sense of control. We suggest that where a sense of control is limited this can create a sense of powerlessness. Further, the illness experience was not solely restricted to the individual, but also affected wider social relationships. It is only through taking into account the context of patients experience of illness that clinicians/nurses can meaningfully draw on all aspects of evidence to reach integrated clinical judgement. This paper explores the effects of mental policy changes and the curtailment of mental health nursing education on the realities of working as a mental health nurse in rural and remote locations in New South Wales, Australia. Using the twin lenses of mental health nursing and the sociology of work and social change, the experiences of mental health nurses are explored and set in the context of the evolution of the mental health nurse into non-specialist mental health worker. This paper focuses on embodied geographies of alcohol to rethink the weekend. We explore the weekend as it is produced via the mutually constitutive relationships between time, space and bodies. Drawing on qualitative research undertaken in Bega, New South Wales, Australia with 23 young women, we offer a critical analysis of how alcohol shapes the bodily, social, spatial and temporal boundaries that interpose knowing the weekend. We illustrate how alcohol mediates the felt and performative dimension of the embodied geographies that configure the weekend. We argue that young women’s bodies and spaces may be understood as sites where the pleasures and pain of alcohol may rupture or make resilient bodily, spatial, social or temporal boundaries through which the weekend makes sense. Australia is rapidly becoming home to an increasing number of Polynesians. While the Maori/Pacific Islander 'economic migrant' may appear fit and healthy, statistically the incidence of diabetes, hypertension and cardiovascular disease is extremely high. When this profile is coupled with family reunion migration of older relatives who are living longer, it represents an escalation of current and future outlay for Australian health services. Qualitative semi-structured interviews were conducted with 67 Polynesian migrants in regard to their perceptions of health and illness, and their experience of health services in Sydney and the Hunter region of New South Wales. Three key respondents, all Polynesian nurses practising in Australia, were also interviewed. The findings indicate a concentration of disadvantage concerning educational achievement, skill levels and health literacy that reinforce the process whereby social disadvantage impacts on health outcomes. This study also reveals a nexus of issues around cultural behaviours and poverty that contribute to the incidence of obesity-related illness and limit compliance with recommended preventative measures and treatments. This paper includes a discussion of the socio-political context of health delivery to Maori and Pacific Islander migrants and how the contraction of services under neoliberal 'belt tightening' undermines potential capacity to treat culturally and linguistically diverse (CALD) populations effectively. As Australian scholarship has traditionally concentrated on Melanesia, there is very little research on Polynesians who are one of our burgeoning migrant groups. The grandparent-as-parent role is a growing social issue both within Australia and internationally. This paper explores the experience of grief as reported by grandparents who are raising their grandchildren. The data presented are part of a larger qualitative study that investigated the lived experience of 34 grandparents who have taken on the full-time care of grandchildren when their own children have been unable to parent them. In-depth interviews conducted with the grandparent participants and analysed through a three-layer narrative analysis reveal the paradoxical dimension of grandparents' experience as they navigate the complex issues that characterise their predominantly unanticipated role. While all of the grandparents referred to the benefits, satisfaction, and joy of taking on the parenting of grandchildren, their narratives were deeply imbued with experiences of loss and grief. The discussion analyses this experience in relation to grief theory and posits that the complex and disenfranchised nature of grandparents' grief means it often goes unacknowledged, including in the policies, programs, and services developed to support grandparents-as-parents. Encouraging participation and agency are fundamental elements of social work practice using a community development approach that emphasises community identification of needs and strengths, and community involvement in social change. Ensuring children have a voice is also a cornerstone of social work practice. This paper reports on a project being undertaken in two social housing neighbourhoods in Sydney that began with an exploration of community members' perceptions of community and priorities for change. Initial engagement undertaken with adults living in the two estates indicated that lack of support for children and the behaviour of young people were pressing community concerns. This prompted researchers to revisit the community development research design in order to actively seek out the perspectives of children and young people. Unsurprisingly, the children and young people provided a layer of additional and alternative knowledge about community life and a raft of suggested strategies to improve community wellbeing. We argue that the position of children and young people as community members in community development discourse and practice requires further attention. If community development is posed as an alternative to ''top down'' approaches to work in communities, then including children and young people (and other marginalised age groups) in ''bottom up'' initiatives is crucial. Currently many nursing faculties and schools employ high numbers of sessional teachers to meet the demands of teaching. Sessional teachers are a source for future continuing academic staff; however, there is little exploration on the experiences of sessional teachers as they transition into the full-time nurse academic role. A qualitative study of 14 registered nurse participants used a story-telling approach to explore the experiences of sessional teachers as they transitioned into full-time and continuing academic roles. Findings revealed that participants had only a very limited understanding of the requirements of the academic role when appointed to it. Thematic analysis revealed two major themes. These were: 'Uncertainty: Dealing with role expectation' and 'Mitigating lack of confidence'. The implications of this paper contributes to and enhances knowledge of the transition experiences of sessional teachers and provides new evidence to suggest that adequate support processes are essential for sessional staff transitioning into permanent, full-time academic positions. Proportionately few problem gamblers seek formal help for gambling problems. However, research into gambling help-seeking behaviour is limited, especially qualitative studies capturing experiences of recovery. This paper conducted narrative analysis of in-depth interviews relating the help-seeking behaviour of 10 recovering problem gamblers. A common temporal sequence moved from self-help, professional and nonprofessional help, then returned to self-help, in a journey that emphasised the importance of their sharing of narratives. Key themes in the plot structures were: self-loathing and loss of identity; fear of failure, of the loss of the gambling experience, and of being judged; negotiation of control, being in control, and needing to be in control; changing based on insight, cognitive behavioural interventions, or integrative interventions; and finally, the shared narrative. The findings highlight the need for policy makers and practitioners to acknowledge the power of narrative and to open the door to a broader community awareness of problem gambling. Much of the medical literature on patient compliance explores why patients fail to follow practitioners' instructions. This paper explores perspectives on 'compliance' expressed in online discussion groups for kidney patients. Discourse analysis was used to investigate how contributors conceptualise knowledge and the role of the renal patient. In the discussion groups, patients were sometimes seen to have better understanding of the details of their illness than some practitioners, a focus born of inhabiting their situation and therefore having more at stake. Some patients set themselves the task of supervising their own care, double checking the work of practitioners. This did not threaten biomedical knowledge, but these counter-discourses challenge professional boundaries by making it the prerogative of patients to obtain biomedical knowledge. I explore issues of surveillance and frustration, and the potential usefulness of patients' overseeing their own safety, while flagging the potential plight of less 'empowered' or motivated patients. Early parenting residential units provide a child and family health support and education service for parents experiencing parenting difficulties. An ongoing concern of nursing staff and management is whether the parenting knowledge and skills gained are translated into sustainable parenting practices after discharge. This paper explores the response to a post discharge telephone interview about parents' experience of nursing care during their residential stay and their parenting experience since discharge. A descriptive qualitative approach identified four themes in the parents' responses: greater confidence, greater knowledge about their babies, changing expectations of parenting and their infants, and sustainability of parenting skills. Health screening and early detection of cancer results in significantly better health outcomes and lower mortality. However barriers to such screening are multiple and complex. This paper specifically addresses barriers to women's health screening for socially disadvantaged women in an economically and service disadvantaged area. In this qualitative study, women's healthcare workers and consumers of women's health screening were interviewed and data related to issues for women who had special needs were analysed. Findings indicate there is a lack of access to appropriate services for socially disadvantaged women which affects their screening uptake rates. This study also highlights the difficulties socially disadvantaged women encountered when they were able to access these services which also influenced their decisions regarding subsequent health screening. Implications for nurses and other healthcare professionals are manifold and include advocating for greater access to services and more sensitive care in the delivery of health screening services for socially disadvantaged women. Aim: Lack of engagement in parenting support programs by young parents has been identified previously in the literature. This paper reports an innovative strategy utilizing appreciative inquiry (AI) to involve service providers and young parents in identifying strategies to engage young parents in services and programs. Methods: A one-day workshop was held involving service/program staff and young mothers in three phases of AI: discovery, dreaming, and delivery. A variety of activities were used to gather data from participants: Small group discussion, creative 'art work' development, and notes. Results: Results identified strategies that would promote and sustain young parents' engagement in services/programs, such as supportive non-judgmental attitude of staff, having a 'one stop shop' for services, and meeting childcare and educational needs of parents. Conclusion: Through AI, factors were identified that promote and sustain engagement of young parents into parenting support programs, and the need to engage fathers was also recognized. This paper reports on an evaluation of an educational initiative that seeks to improve the diabetes health outcomes of a vulnerable group, Indigenous Australians residing in remote and rural New South Wales. In this context seven Aboriginal Health Workers (AHWs) and ten registered nurses (RNs) undertook an accredited Australian Diabetes Educators Association (ADEA) course. The aims of this study were to identify the beliefs, attitudes and experiences of this group concerning specialist diabetes training, strategies already used by managers and those that could be used to help consolidate the diabetes expertise of AHWs and RNs. The fi ndings indicate specialist diabetes training and constructive support is required if AHWs and RNs are to develop from a novice to an expert. The transition process from student to Registered Nurse has been recognised as an important yet challenging time for newly graduated nurses. Knowledge about this experience from the nurse's perspective, particularly in a rural setting, is limited. This paper reports the findings of a qualitative study of the experiences of newly graduated nurses working in a rural acute care facility in New South Wales. The study examined, from the perspective of the new nurse, the orientation and support which can help to facilitate the transition from student to registered nurse. Four themes emerged which were being supported, being challenged, reflections on being a new graduate, and reflections on a rural new graduate program. These findings contribute to what is know about the transition of new graduates in a rural facility and have implications for program improvements, specifically within the rural acute care environment. With advances in medical technology, more people are surviving major trauma experiences. Those with a traumatic brain injury are one such group who may survive for several decades with significant health consequences including physical, cognitive, and psychosocial sequelae. Traumatic brain injury sufferers are typically young males with a premorbid history of risk taking. Consequently, the differential needs of female traumatic brain injury survivors are less likely to be acknowledged in the literature or by allied health workers. A large quantitative and qualitative study of people with traumatic brain injury was conducted in rural areas of New South Wales, Australia in 2004–2005. The qualitative component of the study included in-depth interviews with 11 women and 21 men. This paper focuses on the interviews with the 11 women who live in two rural areas of New South Wales. These data illustrate the psychosocial consequences of traumatic brain injury for women. They reveal that women are particularly vulnerable to isolation, disempowerment, and in extreme cases, abuse, and are less likely to have a carer. The need for gender to be a factor in ongoing social work and allied health treatment and support of traumatic brain injury sufferers and their carers is evident from this research. Objectives: Although Chinese-Australian women are at higher risk of developing breast cancer after migration to Australia, information on their experience is limited. This paper explores Chinese- Australian women's perceptions of the meaning and experience of a breast cancer diagnosis, treatment and coping mechanism. Methods: Three focus groups were conducted with 23 Chinese-Australian women diagnosed with breast cancer in their native language (Cantonese or Mandarin). Following transcription and translation, interview data was analysed by content analysis. Results: Culturally specific values, beliefs and language barriers played a significant role in shaping the women's breast cancer experiences and their response to the diagnosis. Of note these women found the experience isolating and distressing, factors that were compounded by the lack of culturally sensitive resources and information. Despite almost two decades of reform under Australia's National Mental Health Strategy, the life circumstances of many people with mental illness seem little improved. While lack of rehabilitation, housing and community support services have been blamed for policy shortfalls, there is also concern that mental health services may impede rather than facilitate recovery from mental illness. To explore this particular concern, this paper reports data from a project which evaluated a group mentorship programme for new graduate nurses working in an Australian public mental health service. Prominent among the problems raised in mentorship group discussions were: the arduous nature of mental health work; the uncaring attitudes and practices of many veteran nursing staff; and the maltreatment and neglect of service users. These participants characterised mental health facilities as tough security-minded places, where staff act more as risk-managers rather than therapists, and all service users are treated as if they might be dangerous. The concept and definition of pastoral care in aged care remains ambiguous. This paper reports on the defining characteristics and meaning of pastoral care from the perspective of older recipients, their family members and pastoral care workers. Using a qualitative descriptive approach semistructured in-depth interviews were conducted with 18 pastoral care workers and 11 older people. Transcribed data were analysed using NVivo software and coded for emerging themes. The defining characteristics of pastoral care that emerged from analysis of transcribed interviews were: a trusting relationship, spiritual support, emotional support and practical support. Findings also portray the role of the pastoral care worker as spiritual guide, confidante, and emotional and practical supporter acting within a trusting relationship. The use of call centre operations in public and private sectors is a notable development in the changing structure and nature of employment worldwide. This trend is clearly evident in the South Pacific with the establishment of a call centre as the official point of entry for the statutory child protection systems in both New Zealand and New South Wales. As sites of professional practice however call centres are a controversial development. This paper discusses the practice findings of a qualitative research project which explored the particularity of intake social work (ISW) with 14 social workers employed at the Child Youth and Families' (CYF) National Contact Centre (NCC) in New Zealand. Findings were established through semi structured interviews and thematic analysis. In a "new" protected-area management paradigm which requires close working relationships with a range of stakeholders, it is important that national parks managers recognise the individual interests of each group with whom they are expected to collaborate. A substantial body of research has investigated non-commercial collaborative activities among natural resource managers, tourism organisations and community groups. However, little academic attention has been paid to commercial tourism collaborations, particularly public-private partnerships (PPPs) with tourism operators. PPPs are often more contentious and require careful management to ensure sustainability and political acceptance. This paper reports on qualitative research, exploring the quality and nature of commercial lease arrangements between the New South Wales National Parks and Wildlife Service (NPWS) and a number of their private on-site tourism providers. In-depth interviews were conducted with fourteen key stakeholders involved in these partnerships. Based on an emergent approach to analysis, five main themes emerged which define the nature and quality of relationships between NPWS and tourism operators. Ultimately, it is argued that in commercial PPPs, protected-area managers need to move beyond viewing tourism operators as mere "lessees", seeing them instead as valued and long-lasting "partners".
