La symptomatologie cognitive est un enjeu majeur en santé mentale, elle reste pourtant trop peu prise en compte dans la pratique clinique. Cet article vise à fournir une vision globale de la littérature actuelle sur le fonctionnement cognitif des personnes vivant avec un trouble bipolaire. Il a également pour objectif de mettre en lumière les interventions possibles, notamment au sein des Centres Experts du réseau FondaMental. Nous avons réalisé une revue de la littérature pour explorer différents aspects liés aux troubles cognitifs dans les troubles bipolaires, tels que les troubles cognitifs traits, les variations au cours de la maladie, et l'évaluation et le traitement de ces troubles. Des troubles cognitifs au sein de différentes sphères sont présents dans les troubles bipolaires. Ils sont retrouvés dans toutes les phases et sous-types de la pathologie, avec une augmentation de l'intensité des troubles en phase aiguë. On observe cependant une hétérogénéité interindividuelle. La société internationale pour les troubles bipolaires a élaboré des recommandations, telles que la systématisation d'un bilan de dépistage permettant d'orienter vers des consultations spécialisées en neuropsychologie, ou encore des évaluations régulières facilitant la détection précoce des processus neurodégénératifs. Les Centres Experts dans les troubles bipolaires peuvent proposer ces évaluations, des actions de prévention, de la psychoéducation ou des interventions adaptées aux troubles cognitifs, telles que la remédiation cognitive. Nous présentons enfin le programme de remédiation cognitive écologique ECo, qui se base sur des exercices créés spécifiquement pour permettre le transfert des stratégies en vie quotidienne pour les personnes vivant avec un trouble de l'humeur. L'association d'évaluations régulières, de mesures préventives et curatives permettrait de prendre en charge la symptomatologie cognitive et d'accompagner les personnes vivant avec un trouble bipolaire dans leur parcours de rétablissement. Cognitive impairments represent a major issue in mental health that remains understudied in clinical practice. This article aims to provide an overview of the literature on cognitive functioning in bipolar disorders. Additionally, its purpose is to highlight potential interventions that promote recovery, particularly within the bipolar "Centers of Expertise" of the Fondation FondaMental network. We conducted a literature review to explore various aspects related to cognitive issues in bipolar disorders, such as trait-related cognitive impairments, variations throughout the course of the illness, and the assessment and treatment of these cognitive deficits. In bipolar disorders, many cognitive disorders may present themselves in different ways; the literature identifies disorders of attention, psychomotor speed, executive functions, memory, emotional and social cognition, and metacognition. These cognitive disorders are present in all phases of the disease with heterogeneity between patients. This heterogeneity does not depend on whether patients have type I or type II bipolar disorders; nor does it depend on depressive, manic, or euthymic phases, although cognitive symptomatology is more intense in the acute phases. Bipolar disorders require treatment, although some treatments can have an influence on cognition, notably antipsychotics, lithium, or more general polypharmacy. The International Society for Bipolar Disorders (ISBD) Targeting Cognition Task Force developed consensual recommendations for clinicians regarding cognitive interventions in bipolar disorders. The recommendations include an objective and subjective assessment of cognition for all patients, regardless of whether partial or complete remission is achieved. Caregivers who are not neuropsychology specialists may, after a short training period, use screening tools such as the "Screen for Cognitive Impairment in Psychiatry" (SCIP) and "Cognitive Complaints in Bipolar Disorders Rating Assessment" (COBRA), and must refer patients for a more complete evaluation if any difficulties emerge. There is also an interest in regularly assessing cognition, in connection with the possibility of neurodegeneration. Indeed, there are several theories about the development of cognitive impairments in bipolar disorders in the literature. One theory suggests that neurodevelopmental factors could influence the occurrence of these disorders. On the contrary, the "neuroprogression model" postulates that allostatic load may disrupt cognitive functioning as part of a longer term degenerative process. The FondaMental network Centers of Expertise provide educational programs as well as interventions adapted to several aspects of psychiatric diseases, notably cognitive impairment. Patients can be provided with a complete cognitive assessment, and then directed towards appropriate therapies. The centers offer, for example, cognitive remediation for bipolar patients, with the goal of personal recovery. Cognitive remediation is a psychosocial rehabilitation treatment which consists of training cognitive functions via exercises aimed at developing problem-solving strategies that can be transferred to daily life. This treatment should lead to an improvement in patients' quality of life and autonomy. Worldwide, there is a lack of recognized programs for people with mood disorders, including bipolar disorders. However, some studies report that cognitive remediation therapy leads to cognitive and functional improvement, despite heterogeneity in the results of these studies. Some authors of these studies recommend developing individualized and integrative treatments centered around the patients' personal and professional projects, and targeting different psychological processes that mediate functional improvement. An ecological cognitive remediation program, called "Eco", was recently created in France for patients with mood disorders, including bipolar disorders. This program uses paper and manipulable tools for cognitive remediation exercises that have been created specifically to support the transfer of cognitive skills into people's daily lives. It is composed of a psychoeducational module that: develops the patient's metacognitive knowledge; reduces self-stigmatization; and increases intrinsic motivation towards therapy. The program includes three cognitive training modules (on attention, memory, and executive functions), with sub-modules for specific functions, with exercises to be done in a guided session as well as occasionally at home. The order of presentation of the modules, as well as the number of sessions per sub-module, are adapted to each patient. Exercises are composed of ten levels of difficulty, and they allow patients to discover, develop, and then generalize new cognitive strategies. A final functional training module is used in conjunction with cognitive training, in order to help the transfer of the cognitive strategies developed to complex situations in daily life. The combination of frequent assessments, preventive measures, and treatments should make it possible to manage cognitive symptoms and to support people living with a bipolar disorder in their recovery journey. Background: Progressive cognitive deficits are the main clinical symptom of Alzheimer's disease; however, the precise recording of cognitive deficits and assessment of their progression pose major problems in patient care and early interventions. Objective: Which problems for care and early intervention result from the current practice of cognitive assessment of patients with memory problems and which opportunities arise from the use of mobile apps? Material and methods: Evaluation of current care structures, discussion of basic work, expert recommendations and current developments. Results: The current practice of the pencil and paper-based diagnostics of cognitive deficits, which is temporally and spatially bound to a clinical environment, constrains the feasibility, validity and reliability of cognitive assessment and the quantification of progression. This limits the meaningful use of further diagnostic measures, such as magnetic resonance imaging (MRI) and cerebrospinal fluid (CSF) analyses. Recent progress in mobile app-based technologies, illustrated here with the example of the neotiv app, can help to overcome these problems. Conclusion: Mobile app-based technologies can help to improve the cognitive assessment of patients with the main symptom of memory complaints. They can reduce overuse and underuse of diagnostic and therapeutic pathways and enable a targeted and meaningful use of advanced diagnostics. In this paper the author considers Descartes' place in current thinking about the mind-body dilemma. The premise here is that in the history of ideas, the questions posed can be as significant as the answers acquired. Descartes' paramount question was 'How do we determine certainty?' and his pursuit of an answer led to cogito ergo sum. His discovery simultaneously raised the question whether mind is separate from or unified with the body. Some who currently hold that brain and subjectivity are unified contend that the philosopher 'split' mind from body and refer to 'Descartes' error'. This paper puts forward that Descartes' detractors fail to recognise Descartes' contribution to Western thought, which was to introduce the Enlightenment and to give a place to human subjectivity. Added to this, evidence from Descartes' correspondence with Princess Elisabeth of Bohemia supports the conclusion that Descartes did in fact believe in the unity of mind and body although he could not reconcile this rationally with the certainty from personal experience that they were separate substances. In this Descartes was engaged in just the same dilemma as that of current thinkers and researchers, a conflict which still is yet to be resolved. [ABSTRACT FROM AUTHOR]

Neuropsychiatric symptoms (NS), such as depression, apathy, hallucinations, etc., as well as changes in creativity in older age, are telltale marks in the development of such neurodegenerative diseases as Alzheimer's disease, frontotemporal lobar degeneration, Lewy body disease, and Parkinson's disease. Both NS and creativity are important non-cognitive aspects of behavior that changes as a result of neurodegeneration, similar to the development of cognitive deficit. NS can occur in the preclinical stages of neurodegeneration, as well as in the clinical stages (mild cognitive impairment or dementia). However, in clinical routine, their presence is underestimated, or patients with neurodegeneration are often considered to have psychiatric disorders, which may negatively affect their treatment. Scientific research is increasingly devoted to the analysis of the non-cognitive aspects of neurodegenerative diseases, resulting in the newly defined concept of Mild Behavioral Impairment (MBI). In this paper, a review of the most common NS and changes in creativity as a result of neurodegeneration with a special focus on Alzheimer's disease, frontotemporal lobar degeneration, Lewy body disease, and Parkinson's disease is presented. Obstructive sleep apnea (OSA) is characterised by repetitive cessation or reduction of airflow due to upper airway obstructions. These respiratory events lead to chronic sleep fragmentation and intermittent hypoxemia. Several studies have shown that OSA is associated with daytime sleepiness and cognitive dysfunctions, characterized by impairments of attention, episodic memory, working memory, and executive functions. This paper reviews the cognitive profile of adults with OSA and discusses the relative role of altered sleep and hypoxemia in the aetiology of these cognitive deficits. Markers of cognitive dysfunctions such as those measured with waking electroencephalography and neuroimaging are also presented. The effects of continuous positive airway pressure (CPAP) on cognitive functioning and the possibility of permanent brain damage associated with OSA are also discussed. Finally, this paper reviews the evidence suggesting that OSA is a risk factor for developing mild cognitive impairment and dementia in the aging population and stresses the importance of its early diagnosis and treatment. Objectives: The paper summarizes neuropsychological monitoring of employees exposed to TCDD during mass industrial intoxication in years 1965 - 1968 in an herbicides producing factory. Sample and methods: About 350 persons were exposed, the signs of poisoning expressed themselves in about 80 workers. 13 men in the years 1996 and 2001, 14 men in 2004, and 10 men in 2008 were examined. 1. Basic cognitive performance (WAIS-R), 2. Memory, attention, executive functions (WMS, HRNB, WCST, and others), 3. Self-assessment of examined persons, and 4.Symptom validity (TOMM, Retest Consistency Index - RCI and the performance pattern in Digit span and Vocabulary subtests) were evaluated. Statistical analysis: Methods of descriptive statistics and Spearman's rank correlation were used. Results: Even forty years after the intoxication, the level of TCDD in fat was still 100 times higher in the group of examined men than in the general population. During the whole period of monitoring, there was the evidence of neuropsychological impairment, first of all in the executive functions, in the examined group. Significant correlations of TCDD levels and neuropsychological achievements were found. Study limitation: The small scope of the sample limits the possibility of the generalization of results. The paper focuses on neuropsychological studies of perception laterality executed in Slovakia. Above all, adaptations and variants of methods for investigating auditory, visual, and haptic laterality are described, possibilities of their application in neuropsychological diagnostics are discussed. The paper informs about the battery and its development, including recent projects of screening batteries (Horwitz et al., 2008; Vanderlice-Barr et al., 2008). It sums up the discussions about psychometric features of HRNB, about relation of fixed and flexible batteries, and quite flexible test selection, about rightfulness of court expert examinations in the USA. According to the author, the objections of some critics that the HRNB battery is outclassed by the development do not stand. HRNB is being developed due to numerous studies, the norms and interpretations are precised, new procedures originate in its framework. The sample consisting of more than 520 persons was examined by the Czech translation of the method, with useful application among others in schizophrenia (comparison of ill and their healthy twins, relation of neuropsychological tests to monitoring eye-movements and to density of white cortex), in epilepsy (pharmacotherapy and compensation), and in persons with chronic intoxication by toxic substances (toluene, mercury, dioxin).
