Deep learning solutions in big data applications can benefit cloud centres and can also lead to network communication overhead. Typically, data collected from traffic are sent to the traffic management centre for analysis. However, this process can worsen the network route to the traffic management centre. A two-tier mechanism has been developed to address this issue, which performs vehicle speed estimation and traffic congestion detection for efficient traffic management. The real-time traffic video data are captured and the video frames are initially processed through a foreground extraction process, which extracts the temporarily stopped vehicles on the road by removing background pixels from the frames. The video frames are then wrapped in an up-down view to remove the influence of the observation angle. The traffic congestion is then detected accurately based on the traffic characteristics using the proposed Ensemble Random Forest-based Gradient Optimization (ERF-GO) algorithm. The generalization error occurs when learning complex features on frames is minimized using a gradient-based optimization (GO) algorithm. Finally, the learned information on traffic conditions is forwarded to the cloud and edge computing environments based on network connection speed. The efficiency of the proposed ERF-GO is investigated in terms of performance metrics, namely root mean square error, speed detection error, execution time, computational cost, accuracy, latency, workload balance, precision, recall, f-measure, and congestion detection error rate. The analytic result displays that the proposed ERF-GO algorithm attains a greater accuracy rate of about 98.65% in detecting traffic congestion which is comparably higher than state-of-the-art methods. This study evaluated the impact of the Salisbury Protocol for Assessment of Cauda Equina Syndrome (SPACES) on the waiting time for MRI in patients presenting with suspected Cauda Equina Syndrome (sCES) within a UK district general hospital. All consecutive patients undergoing an MRI scan in our hospital, for sCES, over a 12 month period, prior to and following the introduction of SPACES, were identified. Patient's gender, age, MRI diagnosis, time from MRI request to imaging and outcome were recorded. In the year prior to the introduction of SPACES, 66 patients underwent MRI for sCES, out of which 10.6% had cauda equina compression (CEC), 63.5% had other spinal pathology and 25% had a normal scan. In the year after introduction of SPACES, 160 patients underwent MRI for sCES out of which 6.2% had CEC, 70.7% had other spinal pathology and 23% had a normal scan. Despite the referrals for sCES increasing by more than 2-fold following the introduction of SPACES, the median time from MRI request to scan decreased from 9.1 to 4.2 hours (p = 0.106, Mann-Whitney-U) and the number of patients transferred to the regional hub hospital decreased from 7 to 3. Implementation of SPACES for patients with sCES resulted in a substantial reduction in waiting time for MRI and decreased the number of transfers to the regional hub hospital. Stable carbon isotope analysis of plant remains is a promising tool for researchers studying palaeoclimate and past agricultural systems. The potential of the technique is clear: it offers a direct measure of the water conditions in which plants grew. In this paper, we assess how reliably stable carbon isotope discrimination can be used to infer water conditions, through the analysis of present-day crop plants grown at multiple locations across the Mediterranean and south-west Asia. The key findings are that: (1) ∆¹³C, as expected, provides an indication of water conditions, (2) even for plants grown in similar conditions there is variation in ∆¹³C and (3) ∆¹³C may reflect crop water status for a period beginning well before the grain filling period. A new framework is presented which increases the robustness with which ∆¹³C values of plant remains can be interpreted in terms of the water conditions in which ancient crops grew. Do American troops help or hinder economic growth in other countries? We consider a newly constructed dataset of the deployment of U.S. troops over the years 1950–2000 and discover a positive relationship between deployed troops and host country economic growth, which is robust to multiple control variables. Each tenfold increase in U.S. troops is associated with a one–third percentage point increase in average host country annual growth. We explore three possible causal explanations: a Keynesian aggregate demand boost; the diffusion of institutions; and security. Arid and semi-arid environments are characterized by extreme fluctuations in temperature and low rainfall, which present significant challenges to the animals inhabiting these areas. However, the presence of burrows may allow animals to avoid climatic extremes and predators and may act as valuable foraging sites. We assessed the microhabitat conditions (maximum and minimum temperature, relative humidity and seed abundance) of aardvark (Orycteropus afer) burrows in relation to paired non-burrow areas at three sites in South Africa. We also describe the extent to which they are used as resources by other vertebrates. Maximum temperatures were significantly lower and minimum temperatures and relative humidity values were significantly higher inside the burrows than outside. The concentration of seeds inside the burrows and at the paired non-burrow sites was similar. Twenty-seven vertebrate species (21 mammals, two birds, three reptiles and one amphibian) were recorded making use of the burrows and it is likely that these species accrue benefits (e.g. a buffered microclimate) from burrow use. However, our sampling was biased towards mammals and nocturnal species. Consequently, we recommend further work to establish the overall reliance of vertebrate taxa on aardvark burrows in arid and semi-arid environments. [ABSTRACT FROM AUTHOR]

Bushclumps are scattered islands of thicket-like vegetation within a matrix of more open vegetation. We investigated the role of bushclumps as refugia for small mammals, and examined the effect of a limited number of abiotic and biotic factors on their richness, diversity and abundance. Small mammals were surveyed using Sherman small mammal traps at two sites in the Eastern Cape Province, South Africa (Mountain Zebra National Park and Kwandwe Private Game Reserve). Soil hardness and seed abundance, inside and outside bushclumps, were determined. Trap success was significantly higher inside bushclumps than in areas outside, and species diversity and the abundance of small mammals were greater within bushclumps compared to outside. Bushclumps also had significantly softer soil and a higher concentration of seeds. We conclude that bushclumps provide a concentrated source of food and protection from predators for small mammals, and are thus used significantly more than adjacent open areas. In recent years there has been an increasing demand for the design, development and use of questionnaires that can assess patients' experiences of health and illness or their health-related quality of life (HRQoL). The application of these questionnaires in research and clinical practice provides information from the patient's perspective on the ways in which symptoms and treatments can affect their daily well-being and functioning. The aim of this paper is to report on the ways in which information on the HRQoL of women with polycystic ovary syndrome (PCOS) can be collected. With particular reference to PCOS-associated infertility, this paper will discuss the types of questionnaires that are currently available. It will discuss the benefits and limitations of the disease-specific PCOS questionnaire, and the other generic and condition-specific tools that have been used in the existing PCOS literature. It is generally recommended that, in cases of difficulty in removing a ventricular catheter during a shunt revision, it is best left alone to avoid intraventricular haemorrhage. Retained ventricular catheters (RVCs) are usually safe, although in the presence of ventriculitis they may become colonized by organisms and become a source of persistent or recurrent infection. The authors present a case of persistent and intractable ventriculitis due to an old retained ventricular catheter. A 23-year-old female, who had a RVC and a functioning shunt, was admitted for a suspected blocked shunt. At surgery the shunt was found to be infected and external drainage was instituted. Over the next 4 months, she developed intractable and persistent staphylococcal ventriculitis, despite undergoing 10 further surgical procedures, and appropriate intravenous and intrathecal antibiotic therapy. She responded rapidly only after surgical removal of the old RVC via a craniotomy. The staphylococcus cultured from the RVC had an identical antibiogram to the organism responsible for the intractable ventriculitis. This case emphasizes the point that, although RVC are generally considered safe, removal becomes imperative in the presence of concurrent CSF infection that fails to respond quickly to intrathecal antibiotic therapy. Dimethylsulphide (DMS) has been implicated in climate change as a possible negative feedback to global warming, and several Models have been developed that simulate the production of DMS in the marine environment. The focus of this study is to improve the nitrogen based Gabric Model, using field data collected during the Southern Hemisphere First Marine Aerosol Characterisation Experiment (ACE-1) in the Southern Ocean in 1995. Two Model Runs (Series A and B) were carried out with six simulations of varying biotic and abiotic inputs applied over the voyage transect (41–48°S), reflecting Model default values or field values from the experiment. The abiotic inputs were time-step, dissolved dimethylsulphoniopropionate (DMSP) and DMS, and the biotic nitrogen inputs were from phytoplankton, bacteria, zooflagellates, large protozoa, micro and mesozooplankton. The focus of the abiotic assessment was nutrient (nitrate) uptake and dissolved DMSP and DMS output. Model output of the biotic compartments was assessed for congruence with predicted ecological patterns of succession. Despite a limited data set the study provides a good insight into the utility of the Model, which functioned as a heuristic rather than predictive tool. In simulation 1 (Series A) where the only field value was nitrate, all latitudes from 41–48°S concurred with the ecological succession predicted by the Model authors and the successional pattern predicted by other researchers, with a double phytoplankton peak indicating remineralisation of nitrogen via the microbial loop. In many simulations the Model produced lower values of dissolved DMS than were measured, and production of DMS in the Model appears constrained. However, in simulation 5 (Series A) DMS model outputs were closest to the mean dissolved DMS levels reported on RV Discoverer . In this simulation, field values were used for phytoplankton, nitrate, dissolved DMSP and DMS, with bacterial abundance and micro and mesozooplankton increased over their Gabric default values. Also, the phytoplankton double peak occurred earlier, as did the peaks in bacteria, zooflagellates, and large protozoa. Simulations that deviated more significantly from the predicted successional patterns were characterised by single peaks in phytoplankton growth and delayed bacterial growth. Series C simulations at latitude 43°S, in an attempt to reduce phytoplankton predation by bacteria, increased DMS output reasonably successfully. However, significant recalibration of the Model is recommended in conjunction with field studies to gather vital background biological data  –  particularly in the areas of nutrient limitation, phytoplankton speciation, and the cellular content of the DMS precursor compound, DMSP. Despite early successes in oral drug delivery of small molecules such as antibiotics, cephalosporins and vitamins, little success has been achieved with more recent pharmaceuticals such as peptides and proteins. This lack of success is primarily due to the impenetrable nature of the gastro-intestinal epithelial cell layers. Despite this, many bacteria, viruses and toxins are readily taken up from the intestine and thereby gain entry into the circulation. This review focuses on the use of various surface molecules from these organisms as well as toxin binding subunits as carriers for oral delivery of other molecules. It also describes the subversion of the natural uptake mechanisms for various vitamins and iron to allow for oral delivery of pharmaceuticals. These mechanisms provide exciting solutions to overcome the problems in oral delivery of peptides and proteins, which has been the nemesis of pharmaceutical scientists for many decades. The effects on population development of two-spotted spider mite Tetranychus urticae by sprays of hop β-acids, a repellent and antifeedant of T. urticae, together with an inoculative release of the predatory mite Phytoseiulus persimilis were compared on tall and dwarf hops. The combination of the semiochemical with the predator was more effective than either factor singly on both hop types. The highest numbers of spider mites and their eggs were recorded on untreated plots. The sprays of β-acids slowed the rate of increase of the pest and contributed more to the effect of the combined treatment early in the season when the predator was establishing itself on the crop. Psychological issues are common in patients with chronic disabling diseases such as asthma. National guidance recommends that specialist asthma clinic attendees should complete questionnaires screening for psychological comorbidities. However, completing these in addition to asthma specific questionnaires can be burdensome. In order to investigate whether anxiety and depression questionnaires can be used in a targeted manner, this study investigates the correlation between the respective scores. We hypothesize that there is correlation between asthma-specific and anxiety/depression questionnaire scores. Three-hundred individuals with poorly controlled asthma attending a specialist clinic were asked to complete Asthma Control Questionnaire (ACQ), mini-Asthma Quality of Life Questionnaire (mini-AQLQ), Generalized Anxiety Disorder (GAD-7) and Patient Health Questionnaires (PHQ-9). A Pearson correlation coefficient was generated and area under ROC and confusion matrices were used to assess correlation. 49% and 47% of patients completing the GAD-7 or PHQ-9 questionnaires, respectively had scores above the screening level for co-morbid anxiety and depression. Additional questionnaires were often incomplete (GAD-7 = 150 and PHQ-9 = 140). GAD-7 and PHQ-9 correlated with ACQ and mini-AQLQ (all p = <0.001, r values 0.53–0.65). Asthma-specific questionnaire scores were predictive of GAD-7 and PHQ-9 scores (AUCs of 0.78 to 0.84). 75% of patients with a mini-AQLQ score of <3 met the threshold GAD-7 and PHQ-9 score of 10. ACQ and mini-AQLQ correlate with GAD-7 and PHQ-9 amongst specialist asthma clinic attendees. A mini-AQLQ >3 suggests patients are unlikely to have anxiety or depression. Some preliminary investigations are described of speckle fields produced from light propagation in a multimode optical fibre exposed to low frequency (100 Hz) acoustic vibrations. Measurements are presented that have been made with a photodiode detector and a CCD array, the results obtained with the latter being analysed using space domain chromatic techniques. Optimum operating conditions are considered for providing good sensitivity and repeatability for detecting low frequency vibration signals (not precise parameter measurements) with an ultimate objective of monitoring fault development from the vibration of a mechanical plant. Streptococcus intermedius is increasingly being recognised as an aetiological agent in central nervous system infections. Primary ventriculitis caused by this organism has not been reported so far. We present a case of primary ventriculitis, which resulted in adhesions and multiloculated hydrocephalus, necessitating numerous surgical procedures to control it. No predisposing factor(s) could be identified. Although the organism could not be cultured from CSF, as he was already on antibiotic treatment, it could, however, be identified by 16S rDNA polymerase chain reaction on the CSF sample. It appears important to recognise this condition and to treat it aggressively to prevent complications such as adhesions and multiloculated hydrocephalus. [ABSTRACT FROM AUTHOR]

Waste management is an increasingly important problem in many developed countries and much effort is currently being expended into identifying novel uses for waste products. We consider the effects of substituting proportions of earthworm-digested composted green waste (vermicompost) into commercial peat based growing media on the germination, growth, and productivity of Helianthus annuus, Cosmos bipinnatus and Eschscholzia californica. The percentage germination, time to flowering, seed production, overall heights and biomass were measured for all three species in treatments of commercial peat based growing media where the growth medium was substituted with vermicompost at rates of 0%, 20%, 40%, 60% and 100%. In addition, the total numbers of inflorescences was recorded in multiple flowering species. Results for Helianthus suggest that while vermicompost additions induced small species specific growth responses in germination rate, growth rate, overall height and total biomass, none were statistically significant. For Cosmos there was a significant increase in early plant growth rate and in the final flower numbers at substitution rates of 20% and 40% vermicompost. The total biomass and numbers of flowers produced by Eschscholzia was decreased significantly by substitution of 20% and 40% vermicompost. We conclude that the plant growth enhancement properties of this vermicompost seem more species specific than previously reported. Further, in order to preserve consumer confidence in waste-derived horticulture products, vermicompost marketing strategies should acknowledge these findings. [ABSTRACT FROM AUTHOR]

Our aim was to examine, using microsatellite (ms) markers, the contribution of the telomeric part of the HLA region to rheumatoid arthritis (RA) predisposition in the Spanish population. We have looked at the distribution of DQB1, DRBI and five ms loci (D6S1014, D6S273, D6STNFa, MIB and C1-2-5) within the HLA region in 147 Spanish RA patients and 202 control subjects. A total of 19 conserved ms configurations were observed, twelve of them in linkage disequilibrium with particular DQB1-DRB1 haplotypes. Interestingly, haplotype c1 (DQB1*0201-DRB1*0301-D6S1014*143-D6S273*139-D6STNFa*99-MIB*350-C1-2-5*196) was significantly associated with RA predisposition. As part of this haplotype, the MIB*350 allele was found to be a risk factor independently of the RA-predisposing haplotypes. The present results along with data from others prove the existence of a second predisposing locus located inside the MHC region, and suggest that might be located within the TNFa-HLA-B region. Non-adherence to medical regimens is a critical threat to the health of HIV-infected individuals. Patients who do not adhere to routine medical care cannot fully benefit from the increasingly efficacious treatments available to them. Consistent attendance at medical appointments plays a central role in both prolonging life and enhancing quality of life for persons living with HIV/AIDS. By identifying why many patients do not reliably attend medical services, interventions can be undertaken to improve appointment-keeping. The primary purpose of the present study was to identify factors predictive of HIV-related medical appointment attendance. One-hundred-and-forty-four outpatients in a public hospital ambulatory care HIV clinic were followed for seven months. Demographic, medical care and psychosocial factors were measured in order to prospectively predict the percentage of missed clinic appointments by persons with HIV disease. Greater outpatient appointment non-attendance was associated with younger age, minority status, less severe illness and lower perceived social support. Treatment duration, provider consistency, hopelessness and religious coping did not emerge as significant predictors of outpatient appointment-keeping in this sample. Objective: Turner syndrome (TS) is associated with hypogonadism, osteoporosis and fractures. We investigated the prevalence and risk factors for low bone density and fractures in a TS cohort.Methods: We included 76 TS patients (median age 28.5 years) attending a tertiary hospital between 1998 and 2015 who underwent dual-energy X-ray absorptiometry. Spine and femoral neck (FN) areal bone mineral density (aBMD) were compared with those of a control group. To adjust for smaller bone size, bone mineral apparent density (BMAD) was calculated.Results: Primary amenorrhea was common (83%) in the TS cohort; the median age of pubertal induction was 15 years (range 11-30 years), and non-continuous estrogen therapy (ET) recorded in 40%. Almost one-third of TS patients reported fractures. TS patients had lower median spinal aBMD (1.026 g/cm2 vs. 1.221 g/cm2) and BMAD (0.156 g/cm3 vs. 0.161 g/cm3) than controls, and lower median FN aBMD (0.850 g/cm2 vs. 1.026 g/cm2) (all p < 0.01). More women with TS had spinal Z-score < -2.0 compared to controls (26.0% vs. 3.6%, p = 0.001). Spine and FN aBMD, BMAD and Z-scores were inversely associated with age commencing ET or years of estrogen deficiency.Conclusions: Delay in ET commencement was an independent risk factor for the lower bone density observed in women with TS. Background Although low back pain and obesity are major health issues for women, our understanding of the relationship between these conditions is limited. This study aimed to investigate the relationship between occupational activities and low back pain and disability in obese and non-obese, middle-aged females. Methods Eighty-nine obese and 56 non-obese participants were recruited for a community-based study of musculoskeletal health. Low back pain intensity and disability were examined using the Chronic Pain Grade Questionnaire and participants were asked about their involvement in occupational activities. Results More manual activity and heavy lifting, bending or squatting were found to be associated with low back pain intensity in obese females (odds ratio (OR) 1.83, 95% confidence interval (CI) 1.14-2.94; OR 3.02, 95% CI 1.24-7.37, respectively), but not in non-obese females (OR 0.83, 95% CI 0.42-1.63; OR 0.81, 95% CI 0.25-2.6, respectively), after adjusting for age and recreational activity. Similarly, there were also relationships between performance of more manual activity and heavy lifting, bending or squatting and low back disability in the obese (OR 1.68, 95% CI 1.07-2.63; OR 2.79, 95% CI 1.21-6.46, respectively), but not in the non-obese (OR 0.88, 95% CI 0.36-2.13; OR 1.78, 95% CI 0.39-8.22, respectively). Conclusions Obese females who perform predominately manual activity or heavy lifting, bending or squatting at work have high levels of low back pain and disability, independent of their recreational activity. This was not the case for non-obese, female workers. The purpose of this research was to provide an overview of the current state of intergenerational programming in one Canadian city. One-hundred and seven individuals in charge of programming in youth and seniors' organizations completed a telephone survey about intergenerational programs. More than one-third of organizations surveyed reported offering intergenerational programs, and the majority of these perceived their program to be successful. Participants added that there were no current overlaps in services, though a variety of gaps in and barriers to intergenerational programming were identified. There was also significant interest in and ideas for offering a diverse range of programs in the future. It was concluded that while interest in offering intergenerational programs was high, multiple barriers currently exist to offering such programs. [ABSTRACT FROM AUTHOR]

We present results showing enhancement of terahertz (THZ) emission from indium arsenide (InAs) which demonstrate that at 170K and magnetic fields (B) up to B = 8T, increased visible to terahertz conversion efficiencies are possible. With an average total power of 1μW at B = 0T and 12μW at B = 8T, we have observed an order of magnitude increase in total conversion efficiency with no visible saturation. The InAs sample was placed within an optical cryostat and illuminated by Ti:Sapphire laser pulses with duration 100fs. The terahertz powers were recorded using a calibrated Hecooled bolometer. Free space electro-optic sampling (EOS) measurements confirmed the coherent nature of this radiation over the field and temperature range investigated. The optical cryostat could produce magnetic fields of B = 8T at temperatures from 3 to 300K. We therefore present a comprehensive study of the terahertz generation as a function of both these variables and a brief comparison with an alternative material system, InSb. [ABSTRACT FROM AUTHOR]

This report is concerned with a sampling excavation of Bronze Age, Roman and late Saxon/early medieval settlement traces near the church at Wraysbury, Berkshire. The most extensive evidence is for an agricultural settlement of the late ninth to twelfth centuries A.D. based on a series of ditched enclosures and trackways. The settlement moved location during the eleventh century. The environmental evidence is particularly important. A large faunal assemblage including extensive fish remains exists for the late Saxon and medieval phases as well as an unusual collection of charred plant remains.
