Complex regional pain syndrome (CRPS) is one of the common complications in stroke patients. Sympathetic dysfunction is usually considered underlying pathology. The evidence for contrast bath (CB) used in the treatment of CRPS is limited. The aim of the study is to investigate the possible effects of CB on the sympathetic activity in poststroke CRPS. Stroke patients with CRPS (CRPS group) and without CRPS (control group) were included in the study. Age, gender, duration of stroke, aetiology, plegic side, dominant hand, spasticity level, Brunnstrom stages, and comorbidities were recorded. CB was performed for one session to the plegic side of the CRPS and control groups. Sympathetic skin responses (SSR) was recorded from the plegic and healthy hands of participants at pre-CB and post-CB. Each group consisted of 20 participants. No significant difference was detected in stroke duration, aetiology, and Brunnstrom stages between groups. Elbow spasticity level was significantly higher in CRPS group (p =.034). SSR amplitudes of plegic hands were significantly decreased after CB in CRPS and control groups (p =.0002, p =.013 respectively). Also, CB reduced the SSR amplitude of healty side in CRPS group (p =.003). There was no statistically significant change in SSR latency or percentage changes of SSR amplitude and latency after CB in both groups. CB leads to a significant reduction in sympathetic activity of plegic side of stroke patients. We aimed to evaluate the relation between the severity of carpal tunnel syndrome and metabolic syndrome. One hundred and fifty patients who had a clinical and electrophysiologically confirmed diagnosis of carpal tunnel syndrome (CTS), were included in this study. The patients were divided into two groups (with or without metabolic syndrome) according to the criteria of National Cholesterol Education Program. Eighty one (73.5 %) of the patients with CTS had metabolic syndrome. The patients with metabolic syndrome the severity of CTS was found 22.2 % had mild CTS, 56.8 % had moderate CTS, and 21 % had severe CTS. The patients without metabolic syndrome the severity of CTS was found 44.9 % had mild CTS, 40.6 % had moderate CTS, and 14.5 % had severe CTS. The severity of CTS between both groups was found to be statistically significant ( p = 0.0009). While a correlation was found between the severity of CTS and high level of LDL with the presence of metabolic syndrome (correlation coefficient 0.209). In conclusion metabolic syndrome that appears to be a risk factor for CTS. At the same time, the presence of metabolic syndrome increases the severity of the disease. [ABSTRACT FROM AUTHOR]

Antibodies to cyclic citrullinated peptides (anti-CCP) are highly specific for the diagnosis of rheumatoid arthritis (RA) with a marginal increased prediction of the disease. In this study, we aimed to investigate the relation of the presence of anti-CCP with clinical manifestations and disease activity in a cohort of RA patients. A total of 61 RA patients were included in this study. Data of disease-related parameters such as duration of disease, medications, degree of pain (visual analog scale, VAS), disease activity score 28 (DAS-28) and health assessment questionnaire (HAQ) were recorded. Laboratory workup included erythrocyte sedimentation rate (ESR), plasma C-reactive protein (CRP), rheumatoid factor (RF), anti-CCP, complete blood count and anemia parameters. Anti-CCP positivity was associated with higher scores of DAS-28, longer duration of morning stiffness, serum RF positivity and low levels of serum ferritin, while it was not associated with disease duration, VAS, HAQ, ESR, CRP and hemoglobin. Background/aim: β1-selective beta-blockers (BBs) are sympatholytic agents, and discerning their effects on bone health would be of great importance. This study aimed to investigate the influence of β1-selective BBs on bone mineral density (BMD) and fracture risk. Materials and methods: This study included postmenopausal women who used β1-selective BBs (BB group) and control group. Sociodemographic characteristics, BMD and previous fragility fractures were recorded. Additionally, the 10-year probability of a major osteoporotic and hip fracture was calculated using the fracture risk assessment tool (FRAX). Results: A total of 60 participants were included in the study. L1-4 and L2-4 BMD values were significantly higher in BB group than control group (P = 0.015 and P = 0.025, respectively). Moreover, T-scores of lumbar and femur total were significantly higher in the BB group. Two patients in BB and 6 patients in control group had previous fragility fracture. No statistically significant intergroup difference was noted regarding FRAX. Conclusion: Based on our results, β1-selective BB usage was associated with higher BMD at the lumbar region in postmenopausal women. [ABSTRACT FROM AUTHOR]

Amaç: İnsan vücudunda pürin nükleotidlerinin kataboliznıa ürünü olarak endojen üretilen ve aynı zamandan hayvaasal gıdalar ile fruktozdan da egzojen yolla aldığımız ürik asitin anti-oksidan ve proinflamatuar etkileri bulunmaktadır (1). Ürik asit (ÜA) düzeyindeki değişiklikler birçok hastalıkta gözlenirken os-teoporoz hastalarında da ürik asitin kemik mineral yoğunluğu üzerine olumlu etkileri bildirilmiştir (2). Bu çalışmanın amacı ankilozan spondilit (AS) hastalarda serum ÜA düzeylerinin, an-kilozan spondilit hastalarının klinik durumlarına ve kemik mineral yoğunluğu üzerine etkilerini araştırmaktır. Yöntem: İstanbul Fizik Tedavi ve Rehabilitasyon Eğitim Araştırma I Iastanesi'ndc AS tanısı ile başvurmuş hastalar bu çalışma için değerlendirildi. Renal, kardiyovasküler ve karaciğer fonksiyon bozukluğu olan, geçirilmiş kanser hikayesi olan, hormonal ve me-tabolik rahatsızlığı ile psoriyatik artirit ..vs olan hastalar çalışma dışı bırakıldı.Hastaların yaş, cinsiyet, boy,kilo, beden kitle indeksi (BKİ), AS süresi, sigara-alkol tüketimleri, düzenli egzersiz yapma dununu, eski kırık hikayesi, periferik artirit, sabah tutukluğu, modifiye sehober, göğüs ekspansiyonu, tragus duvar, parmak zemin mesafesi ve BASDAI, BASFI VE BASMI skorları kaydedildi. Ayrıca serum ÜA, biyokimya, sedim ve C-reaktif protein (CRP) değerleri ile lomber ve femur kemik mineral yoğunlukları (KMY) hastaların dosyalarından kaydedildi. Hastalar serum ÜA sevilerine göre yüksek ÜA grubu ve normal ÜA grubu olarak ikiye ayrıldı. İki grubun çalışma verileri karşılaştırıldı. Bulgular: Çalışmaya dahil edilen otuz sekiz hastanın yaş ortalaması 39± 11.4 yıl ve hastalık süresi 9.6±7.8 yıldı. Hastaların cinsiyet dağılımı dokuz (%23.7) kadın ve yirmi dokuz (%76.3) erkek şeklindeydi. Hastaların serum ÜA düzeyleri 5.54±1.31 ıııg/dL. On hastada serum ÜA seviyesi yüksekken, 28 hastada normal sınırlardaydı. Her iki grup arasında yaş, cinsiyet, hastalık süresi, sedim, CRP, sigara-alkol tüketim miktarları, düzenli egzersiz yapma durumu, önceki kırık hikayesi, Modifiye Scho-ber, tragus-duvar, göğüs ekspaasiyonu, BASDAİ, BASFİ ve BASiYIİ açısından anlamlı farklılık saptanmadı. Sahalı tutukluğu varlığı yüksek ÜA grubunda anlamlı şekilde daha azdı. Yüksek ÜA grubunda Ll^f ve L2-4 KMY değerleri istatistiksel anlamlı şekilde fazlaydı (p<0.05). Sonuç: Bu çalışma pilot çalışma olmakla birlikte, elde edilen verilere göre AS'li hastalarda serum ÜA seviyesindeki yükseklik, lomber KMY artışı ile ilişkili olabilir. [ABSTRACT FROM AUTHOR]

Objective: Vitamin D deficiency can cause pain and disability in many diseases. Subacromial impingement syndrome (SAIS) is a common reason for shoulder pain. In this study, we aimed to assess the relationship between vitamin D and the clinical features in patients with SAIS. Methods: Patients with SAIS were evaluated, and age, gender, affected side, disease duration, 25-OH vitamin D, and other laboratory tests were recorded from patients' files. Also, patients' data on shoulder visual analog scale (VAS), shoulder range of motions (ROMs), and scores of Disabilities of the Arm, Shoulder, and Hand (DASH) were obtained. The patients were grouped according to their 25-OH vitamin D levels. (Group 1: <10 ng/mL, Group 2: 10-20 ng/mL, and Group 3: >20 ng/mL). Demographic and clinical features were compared between groups, and the relation of vitamin D with VAS, DASH scores, and ROMs were investigated. Results: Eighty patients with SAIS were included in the study. The mean age of patients was 51.99±11.14 years. There were 35 female and 25 male patients. The mean disease duration was 6.32 months. VAS was statistically higher in Group 1 compared to Group 3. Also, shoulder pain was negatively correlated with 25-OH vitamin D level. There was no statistically significant relation between the 25-OH vitamin D level and DASH score or the 25-OH vitamin D level and shoulder ROMs. Conclusion: Severe deficiency of vitamin D increases shoulder pain in patients with SAIS. Vitamin D deficiency should be taken into consideration in patients with severe shoulder pain in SAIS. [ABSTRACT FROM AUTHOR]

Amaç: Anldlozan spondilit (AS) tedavisinde kullanılan anti-TNF ajanlann önemli yan etkilerinden biri de santral sinir sisteminde demiyelinizan süreçlere neden olmasıdır. Santral sinir sistemi de-miyelinizsyonu ile ilgili birçok çalışma varken biyolojik ajanlann uzun dönemde periferik sinir sistemi üzerine etkilerini araştıran sınırlı sayıda çalışma bulunmaktadır. Bu çalışmanın amacı uzun dönem anti-TNF ajanlarının kullanımına bağlı periferik sinirlerde etkilenme olup olmadığını araştırmaktır. Yöntem: İstanbul Fizik Tedavi ve Rehabilitasyon Eğitim Araştırma Hastanesinde AS tanısı ile taldpli olan hastalar bu çalışına kapsamında değerlendirildi. Hastalar almakta oldukları tedavilerine göre konvansiyonel grup ve anti-TNF grubu olarak ikiye ayrıldı. Anti TNF grubuna dahil edilen hastalarda beş yıl ve üzeri an-ti TNF kullanma şartı arandı. Hastaların yaş, cinsiyet, boy, kilo, hastalık süresi, dominant kolu, BASDAI, BASFI VE BASıVU skorları kaydedildi. Tüm hastalara her iki üst ekstremitede median-ul-nar motor ve duyu iletim çalışmaları ile her iki alt ekstremi te tibi-al- peroneal motor ve sural sinir duyu iletim çalışmaları yapıldı. Ayrıca nıedian, ulnar ve tibial sinir F-yamt çalışmaları yapıldı. Bulgular: Çalışmaya dahil edilen on altı hastanın yaş ortalaması 41.9±9.9 yıldı. Hastaların ikisi (% 12.5) kadın ve ondördü(%87.5) erkekti. Her iki grubun yaş, dnisyet, boy, kilo ve hastalık süresi açısından fark yoktu. Elektrofizyolojik değerlendirmede dominant kol median sinir duyu amplitüdü konvansiyonel gruba göre anlamlı şekilde düşüktü (p=0.0339). Dominant ve non dominan kol ulnar ve median F yamdarı ile tibial Fyanıtlan anti TNF grubunda uzamıştı (p<0.05). Tibial ve peroneal sinir motor iletim çalışmasında distal lataaslar ile sural sinir duyu iletim çalışmasında iletim hızı anti-TNF grubunda yavaşlamıştı. Anti-TNF grubunda ilaç kullanım süresi ile elektrofizyolojik veriler arasında anlamlı bir korelasyon gözlenmedi. Sonuç: Uzun dönem anti-TNF kullananımı ile pcriferik sinir tutulumu arasındaki ilişkiyi değerlendiren bu pilot çalışmanın sonucunda anti-TNF grubunda konvansiyonel gruba göre F yanıtlarında uzama ve sural sinir iletim hızlarında yavaşlama gözlendi.F yanıtları ve sural sinir duyu iletim hızı her ne kadar konvaasiyonel gruba göre uzamış olsa da normal referaas sınırlar içinde olduğu da gözlenmiştir.Hasta sayısı arttırıldığında elde edilecek sonuçlar daha kesin sonuçlara ulaşmamızı sağlayacaktır.
