Objective: Onuf's nucleus is an anatomical structure essential in the regulation of urogenital functions. Lumbosacral pathologies may cause changes in urogenital circulation due to Onuf's nucleus injury; however, there is limited evidence corroborating the relationship between spinal cord injury and urethral artery changes. Materials and Methods: We used 23 sexually mature male rabbits--5 rabbits in the control group (GI), 5 rabbits in the sham group (GII), and 13 rabbits in the experimental group (GIII; received autologous blood transfusion into the T12--L1 subarachnoid space to induce subarachnoid hemorrhage (SAH). The GIII underwent a S1-3 laminectomy after 2 weeks and was decapitated. Histologic specimens were prepared to examine changes in Onuf's nucleus, pudendal ganglion, and urethral arteries. The density of damaged neurons and vasospasm index (VSI) in the urethral artery were evaluated. Results: The mean density of damaged neurons (n/mm³) in Onuf's nucleus and pudendal ganglia (S3) and the mean VSI of the 3 groups were as follows--GI: 6 ± 2 per mm³, 12 ± 4 per mm³, and 1.63 ± 0.25, respectively; GII: 27 ± 6 per mm³, 221 ± 62 per mm³, and 1.97 ± 0.36, respectively; GIII: 154 ± 41 per mm³, 1890 ± 541 per mm³, and 3.04 ± 0.95 (P < .05 each for GI/GII, GI/GIII, and GII/GIII). Neuronal damage criteria, such as cytoplasmic condensation and cytoplasmic halo formation, were more prominent in GIII. Conclusion: SAH can lead to ischemia of the Onuf's nucleus--pudendal nerve structures due to urethral artery spasm, resulting in urogenital complications. [ABSTRACT FROM AUTHOR]

Background/aim: Chronic subdural hematoma is the most frequently operated on intracranial hemorrhage worldwide. Although surgical results are satisfactory, recurrence remains an important problem. In this study, it was aimed to evaluate patients who underwent craniotomy with limited membranectomy (CwLM) in terms of recurrence rate and other complications. Materials and methods: Electronic records of 291 consecutive patients who were operated on at our clinic (Atatürk University Yakutiye Research Hospital) between 2015 and 2020 were retrospectively reviewed. Their radiological images at the time of admission, clinical status, any early postoperative complications, and clinical status at the 6-month follow-up were all evaluated. Results: According to the results of the study, postoperative mortality and morbidity of patients who underwent CwLM were similar to those with minimally invasive methods, and it reduced recurrence, especially in laminar and separated subtypes. Conclusion: It is our belief that CwLM will be an appropriate treatment approach in suitable patients (radiologically detected) in chronic subdural hematoma, where recurrence is an important problem. In the literature, only 11 Enterococcus gallinarum group meningitis has been reported so far. The Enterococcus gallinarum group was shown for the first time in a pediatric patient presenting with meningitis after bowel perforation, a complication of a ventriculoperitoneal shunt. A 30-month-old male patient presented with vomiting and fever, with the ventriculoperitoneal shunt distal catheter protruding from the anal orifice. The patient was diagnosed with intestinal perforation and meningitis. Enterococcus gallinarum group bacterial yield in cerebrospinal fluid culture. A total of 6 weeks of intravenous antibiotic treatment was given in the hospital. After the treatment, the patient was re-ventriculoperitoneal shunt and was discharged. Among the shunt complications, meningitis with intestinal perforation is rare. It should be kept in mind that meningitis in such patients may be caused by very rare microbiological factors such as Enterococcus gallinarum group. Antibiotherapy should be given according to the culture result. Then planning should be made for a permanent shunt. [ABSTRACT FROM AUTHOR]

Background and Objectives: This article aims to investigate the subcortical microanatomy of the supramarginal gyrus (SMG) and angular gyrus (AnG) using a microfiber dissection technique and diffusion tensor imaging (DTI)/fiber tractography (FT). The cortical and subcortical structures of this region are highly functional, and their lesions often present clinically. For this reason, the possibility of post-surgical deficits is high. We focused on the supramarginal gyrus and the angular gyrus and reviewed their anatomy from a topographic, functional and surgical point of view, and aimed to raise awareness especially for neurosurgeons. Methods: Four previously frozen, formalin-fixed human brains were examined under the operating microscope using the fiber dissection technique. Four hemispheres were dissected from medial to lateral under the surgical microscope. Brain magnetic resonance imaging (MRI) of 20 healthy adults was examined. Pre-central and post-central gyrus were preserved to achieve topographic dominance in dissections of brain specimens. Each stage was photographed. Tractographic brain magnetic resonance imaging of 10 healthy adults was examined radiologically. Focusing on the supramarginal and angular gyrus, the white matter fibers passing under this region and their intersection areas were examined. These two methods were compared anatomically from the lateral view and radiologically from the sagittal view. Results: SMG and AnG were determined in brain specimens. The pre-central and post-central gyrus were topographically preserved. The superior and medial temporal gyrus, and inferior and superior parietal areas were decorticated from lateral to medial. U fibers, superior longitudinal fasciculus II (SLF II), superior longitudinal fasciculus III (SLF III), arcuat fasciculus (AF) and middle longitudinal fasciculus (MdLF) fiber groups were shown and subcortical fiber structures belonging to these regions were visualized by the DTI/FT method. The subcortical fiber groups under the SMG and the AnG were observed anatomically and radiologically to have a dense and complex structure. Conclusions: Due to the importance of the subcortical connections of SMG and AnG on speech function, tumoral lesions and surgeries of this region are of particular importance. The anatomical architecture of the complex subcortical structure, which is located on the projection of the SMG and AnG areas, was shown with a DTI/FT examination under a topographic dominance, preserving the pre-central and post-central gyrus. In this study, the importance of the anatomical localization, connections and functions of the supramarginal and angular gyrus was examined. Traumas are the most important cause of mortality in the pediatric population. Bleeding is an important complication, especially in traumatic brain injuries with coagulation problem addition. Low-velocity penetrating brain injuries may be caused by sewing needles, nails, and knives. There are few studies in the literature for this injury type. Hydatid cyst of the posterior cranial fossa is rarely seen. We report the case of a 13-year-old girl with extradural hydatid cyst of the posterior cranial fossa presenting with the symptoms of high intracranial pressure. Diagnosis was made by radiological investigations and confirmed during surgery and by histological examination. Cranial hydatid cyst must be considered especially in children presenting with the symptoms of raised intracranial pressure and living in endemic areas. [ABSTRACT FROM AUTHOR]
