Objective: The purposes of our study were to determine the relationship between coronavirus disease (COVID) pneumonia and mortality and immunization status of patients and to present our first extracorporeal membrane oxygenation (ECMO) experiences by retrospectively evaluating pregnant women. Materials and Methods: The research was conducted by screening the files of 37 pregnant/postpartum COVID-2019 cases monitored and treated in the intensive care unit between March 1, 2020 and December 1, 2021. The patients' ages, systemic comorbidities, vaccination details, and clinical and laboratory features were recorded and analyzed. The patients were divided into two groups as survivors (group 1; n=17) and exitus ones (group 2; n=20); and the results were compared statistically. Results: Of 37 patients, 17 (45.9%) survived (group 1) and 20 (54.1%) died (group 2) with a median of 31 gestational weeks and 9 days length of stay in the ICU. ICU admission time [which day of polymerase chain reaction (PCR)+] and cesarean time (which day of PCR+) were 8 days. Nine (24%) patients received ECMO and mechanical ventilation, with 6 (66.6%) exitus and 3 (33.3%) survivors who were discharged from the hospital without sequelae. Of 37 pregnant/postpartum patients, 36 were unvaccinated. Conclusion: Vaccination should be given priority in pregnant women, and ECMO may be effective in the recovery of oxygenation in pregnant COVID-19 patients. [ABSTRACT FROM AUTHOR]

• Feature ranking with proposed semi-supervised score improves the performance of predictive clustering trees. • Multi-objective approaches enhance semi-supervised feature ranking process. • Ensembles of totally randomized trees present an effective tool on dissimilarity calculations. This study focuses on semi-supervised feature ranking (FR) applications for multi-target regression problems (MTR). As MTRs require prediction of several targets, we use a learning model that includes target interrelations via multi-objective trees. In processing the features for a semi-supervised learning model, transformation or scaling operations are usually required. To resolve this issue, we create a dissimilarity matrix via totally randomized trees to process the unsupervised information. Besides, we treat the split score function as a vector to make it suitable for considering each criterion regardless of their scales. We propose a semi-supervised FR scheme embedded to multi-objective trees that takes into account target and feature contributions simultaneously. Proposed FR score is compared with the state-of-the-art multi-target FR strategies via statistical analyses. Experimental studies show that proposed score significantly improves the performance of a recent tree-based and competitive multi-target learning model, i.e. predictive clustering trees. Objectives: Crimean‐Congo haemorrhagic fever (CCHF) is a zoonotic viral infection which is an important public health problem in Turkey. CCHF causes fever and bleeding and can lead to severe health outcomes. The study aims to report a case of a male patient with severe CCHF, hemophagocytic lymphohistiocytosis (HLH) treated with steroids and portal vein thrombosis. Case Report: A 37‐year‐old man was admitted to the emergency department with complaints of high fever, headache, myalgia and diarrhoea. The patient travelled to the endemic region of Turkey. In laboratory findings, thrombocytopenia, abnormal liver function tests and elevated coagulation parameters were observed. Real‐time polymerase chain reaction assay was used for diagnosis of CCHF. Hypofibrinogenemia, hypertriglyceridemia, elevated ferritin and d‐dimer levels were observed in the clinical follow‐up. Prednisolone treatment was performed due to considered the diagnosis of HLH. Portal vein thrombosis was detected on abdominal computed tomography scan. He was successfully treated with ribavirin, corticosteroids, anticoagulant and supportive therapy. Conclusion: The clinical presentation of CCHF can range from self‐limiting flu‐like to severe symptoms possibly fatal. Acute portal vein embolism is a rare complication that has not been reported before to our knowledge. Corticosteroids may be a life‐saving treatment for CCHF patients presenting with HLH. In this study, we analyze the price dynamics of carbon certificates that are traded under the European Union's Emissions Trading System (EU-ETS). With the aim of investigating the joint relations among carbon, electricity, and fuel prices, we model historical prices using several methods and incorporating structural changes, such as econometric time series, regime switching, and multivariate vector autoregression models. We compare the results of the structural model with the results of traditional Markov switching and autoregressive models with breaks and present performance analysis based on the mean average percentage error, root mean squared error, and coefficient of determination. According to these performance tests, models with regimes outperform the approaches where breaks are defined using ex ante dummy variables. Moreover, we conclude that among regime switching models, univariate models are better than multivariate counterparts for modeling carbon price series for the analysis of both in-sample and out-of-samples. Amaç: Hellp sendromu karaciğer enzimlerinde artış, tromositopeni, hemolitik anemiyle seyreden, ağır preeklampsili gebeliklerin %10- 20’sinde görülebilen, maternal ve perinatal mortaliteye sebep olabilen ciddi bir hastalıktır. Hastalarda karaciğer enfarktı, hematomu, batın içi kanama, intrakraniyal kanama, plevral effüzyon, renal yetmezlik ve nadir de olsa perikardiyal tamponad tablosu gelişebilmektedir. Biz 26 haftalık gebeliği şiddetli preeklampsi sebebiyle ölü doğumla sonuçlanan, Hellp sendromuyla yoğun bakım ünitesinde takibimiz sırasında perikardiyal tamponad tanısı koyduğumuz hastayı sunmayı amaçladık. Olgu: Mide bulantısı, kusma, baş ağrısı, el ve ayaklarda ödem, vajinal kanama şikayeti olan 24 yaşında 26 haftalık gebe şiddetli preeklampsiye bağlı olarak eks fetus abortu sonrası Hellp sendromu tanısıyla yoğun bakım ünitemize alındı (Hb: 8,2 hct: 25,1 Plt: 39 bin AST: 352 ALT: 436 fibrinojen: 340 mg/dL). Sıvı kısıtlaması, force diürez uygulanan hastanın tansiyon regülasyonu sağlandı, trombosit defisiti replase edildi. Yatışının 6. saatinde batıcı göğüs ağrısı tarifleyen hastanın akciğer grafisinde KTİ >0,5, bilateral kostadiafragmatik sinüslerinde silinme, EKG’de sinüs taşikardisi, elektriksel alternans saptanması üzerine acil kardiyoloji konsültasyonu istendi. Ekokardiyografide perikardiyal tamponad tespit edildi, anjiyo ünitesine alınıp perkütan perikardiyosentez uygulandı ve 950 cc seröz mayi drene edildi. Light kriterleri açısından incelendi. Eksüda vasfındaki sıvının mikrobiyolojik örneklemesinde üreme olmadı. Hastanın 48 saat sonra yapılan kontrol ekokardiyografisinde effüzyon saptanmadı. Transaminazları gerileyen, trombositopeni tablosu düzelen hasta yatışının 3. günü kadın doğum kliniğine devredildi. Ancak serviste 2 gün sonra ani gelişen hipotansiyon sebebiyle ekokardiyografik incelemesi yapılan hastada perikadial effüzyonun arttığı, tamponad tablosunun tekrarladığı görüldü. Hastada perikardiyosentez sırasında kardiyak arrest gelişti. Resusitasyona yanıt alınamayan hasta eks oldu. Sonuç: Perikardiyak tamponad şiddetli preeklampsi ile nadiren birliktelik gösteren, yüksek mortalite ile seyreden bir durumdur. Ani gelişen göğüs ağrısı, dispne, takipne, hipotansiyon durumunda ayırıcı tanıda mutlaka düşünülmelidir. Akciğer grafisinde kardiyomegali, EKG’de sinüs taşikardisi ve alternan ritm tanıyı desteklemekte TTE ile tanı kesinlestirilmektedir. Şiddetli preeklamside tedavi doğum eyleminin gerçekleşmesi olsa da, postoperatif dönemde yakın takip gereklidir. Kardiyak tamponad doğumdan sonra da tekrarlayabilir ve maternal kayba sebep olabilir. • Multi-criteria approaches improve the performance of semi-supervised decision trees. • Scale inconsistencies can be effectively handled via proximity measures. • Using target relations enhances predictive performance of semi-supervised decision trees. Scale inconsistency is a widely encountered issue in multi-output learning problems. Specifically, target sets with multiple real valued or a mixture of categorical and real valued variables require addressing the scale differences to obtain predictive models with sufficiently good performance. Data transformation techniques are often employed to solve that problem. However, these operations are susceptible to different shortcomings such as changing the statistical properties of the data and increase the computational burden. Scale differences also pose problem in semi-supervised learning (SSL) models as they require processing of unsupervised information where distance measures are commonly employed. Classical distance metrics can be criticized as they lose efficiency when variables exhibit type or scale differences, too. Besides, in higher dimensions distance metrics cause problems due to loss of discriminative power. This paper introduces alternative semi-supervised tree-based strategies that are robust to scale differences both in terms of feature and target variables. We propose use of a scale-invariant proximity measure by means of tree-based ensembles to preserve the original characteristics of the data. We update classical tree derivation procedure to a multi-criteria form to resolve scale inconsistencies. We define proximity based clustering indicators and extend the supervised model with unsupervised criteria. • Multi-objective approaches change the learning trajectory for the better. • Using targets as additional inputs improves the generalization power of the classifiers. • The order of adding targets is important in chaining. • Exploiting target relations augments the learning process. Multitask (multi-target or multi-output) learning (MTL) deals with simultaneous prediction of several outputs. MTL approaches rely on the optimization of a joint score function over the targets. However, defining a joint score in global models is problematic when the target scales are different. To address such problems, single target (i.e. local) learning strategies are commonly employed. Here we propose alternative tree-based learning strategies to handle the issue with target scaling in global models, and to identify the learning order for chaining operations in local models. In the first proposal, the problems with target scaling are resolved using alternative splitting strategies which consider the learning tasks in a multi-objective optimization framework. The second proposal deals with the problem of ordering in the chaining strategies. We introduce an alternative estimation strategy, minimum error chain policy, that gradually expands the input space using the estimations that approximate to true characteristics of outputs, namely out-of-bag estimations in tree-based ensemble framework. Our experiments on benchmark datasets illustrate the success of the proposed multitask extension of trees compared to the decision trees with de facto design especially for datasets with large number of targets. In dealing with sharp changes in electricity prices, contract planning is considered as a vital risk management tool for stakeholders in deregulated power markets. In this paper, dynamics of spot prices in Turkish electricity market are analyzed, and predictive performance of several models are compared, i.e., time series models and regime-switching models. Different models for derivative pricing are proposed, and alternative portfolio optimization problems using mean-variance optimization and conditional value at risk (CVaR) are solved. Expected payoff and risk structure for different hedging strategies for a hypothetical electricity company with a given demand are analyzed. Experimental studies show that regime-switching models are able to capture electricity characteristics better than their standard counterparts. In addition, evaluations with various risk management models demonstrate that those models are highly competent in providing an effective risk control practice for electricity markets. Giriş: Nicolau sendromu, intramusküler enjeksiyonun yapıldığı bölgede deri altındaki dokuların iskemik nekrozu ile karakterize bir sendromdur. Diklofenak enjeksiyonu sonrası gluteal bölgede nekroz gelişen ve derin doku kültüründe C. perfringens üreyen iki olgunun sunulması amaçlanmıştır. Olgu 1: Otuz yaşında erkek hasta, Koronavirüs hastalığı-2019 (COVID19) enfeksiyonu tanısıyla, yaygın vücut ağrısı nedeniyle acile başvurmuş. Hastanın sol gluteal bölgesine diklofenak enjeksiyonu yapılmış. Enjeksiyondan üç gün sonra gluteal bölgede şiddetli ağrı ve şişlik nedeniyle hasta yeniden acil servise başvurmuş. Yapılan muayenesinde gluteal bölgede 15x15 cm boyutunda kızarıklık ve sertlik saptanarak yatışı yapılmış. Hastanın laboratuvar testlerinde lökosit sayısı: 19.900 mm3, CRP: 84 mg/l, prokalsitonin: 7,4 ng/ml olarak belirlendi. Pelvis tomografisinde sol gluteal bölgede gluteus maksimus ve medius kaslarında asimetrik kalınlaşma, amfizematöz hava dansiteleri ve komşu deri altı yağlı dokularda sıvı, ödem ve amfizem bulguları saptandı (Şekil 1). Hastaya meropenem, vankomisin ve klindamisin tedavisi ampirik olarak başlandı. Hasta opere edildi ve sonrasında yoğun bakım ünitesine (YBÜ) alındı. Ancak YBÜ yatışının üçüncü gününde hasta kaybedildi. Hastanın daha sonra çıkan kültür sonucunda meropenem, vankomisin, piperasilin-tazobaktam ve penisiline duyarlı, klindamisine dirençli C. perfringens ürediği öğrenildi. Olgu 2: Seksen beş yaşında erkek hasta, genel durum bozukluğu, halsizlik, kol ve bacakta ağrı şikayeti ile acil servise başvurmuştu. Metastatik prostat Ca tanısı olan hastanın iki ay önce düşme sonrası kollarda ve bacakta şiddetli ağrı şikayeti olmuştu. Bu nedenle de evde hastaya tekrarlayan diklofenak uygulamaları yapılmıştı. Hastanın fizik muayenesinde sol gluteal bölgede çok sayıda enjeksiyon giriş yerleri, ekimoz, ele gelen sertlik ve krepitasyonu belirlendi. Laboratuvar testlerinde lökosit sayısı: 4370 mm3, CRP: 360 mg/l, prokalsitonin: 73,3 ng/ml, olarak saptandı. Pelvis tomografisinde kemik yapılarda litik-sklerotik lezyonlar ve sol gluteal bölgede derin kas planları arasında serbest gaz dansiteleri belirlendi. Hasta ortopedi tarafından opere edildi. Hastaya ampirik meropenem ve linezolid tedavisi başlandı. Hastanın operasyon sırasında alınan kültüründe C. perfiringens ürediği öğrenildi. Tedavisi piperasilin tazobaktam ve klindamisin olarak değiştirildi. Antibiyogram sonucu piperasilin tazobaktam, penisilin ve klindamisin duyarlı olarak geldi. Hasta YBÜ’deki yatışının dokuzuncu gününde olup, halen yatışı devam etmektedir. Sonuç: Diklofenak sonrası Nicolau sendromu nadir görülen bir komplikasyondur. Nekrotik dokuların debritmanı, derin dokudan alınan materyalin anaerob besiyerine de ekiminin yapılması, kültür antibiyogram sonucuna göre tedavi yaklaşımının belirlenmesi ve hastanın yakın takibi gereklidir. Giriş: Stenotrophomonas maltophilia (S. maltophilia) özellikle komorbiditeleri fazla olan hastaları etkileyen, yüksek mortalite ve morbiditeye neden olabilen bir patojendir. Birçok antibiyotiğe dirençli olması nedeniyle tedavide zorluklar yaşanmaktadır. Bu çalışmada S. maltophilia üreyen hastaların klinik ve laboratuvar özelliklerinin irdelenmesi amaçlanmıştır. Gereç ve Yöntem: Çalışmamız Şehit Prof. Dr. İlhan Varank Eğitim ve Araştırma Hastanesi ve Feriha Öz Acil Pandemi Hastanesi’nde 1 Ocak 2020-1 Ocak 2022 tarihleri arasında yatan hastalarda yapıldı. S. maltophilia enfeksiyonu tanısı alan 72 hastanın özellikleri retrospektif olarak irdelendi. Bulgular: S. maltophilia üreyen hastaların %55’i erkek olup, yaş ortalaması 63,85±16,66 idi. Hastaların %98’inin yoğun bakım ünitesi yatışı olmuştu. COVID-19 tanısı ile takip edilen hastalar, tüm hastaların %68,3’ü idi. Komorbiditeleri irdelendiğinde; %50’sinde HT, %32,6’sında DM, %28,3’ünde KAH, %15,2’sinde KOAH tanısı bulunmaktaydı. Hastaların %78’inde mekanik ventilasyon, %23,3’ünde total parenteral nütrisyon kullanımı, %15’inde cerrahi girişim öyküsü vardı. Hastaların 87,5’inde bakteriyemi, %4,16’sında ventilatör ilişkili pnömoni, %4,16’sında intraabdominal enfeksiyon, %2,7’sinde hastane kaynaklı pnömoni etkeni olarak izole edilmişti. Laboratuvar verileri değerlendirildiğinde, ortalama lökosit sayısı 12561±72/mm³, trombosit sayısı 221271±16/mm³, C-reaktif protein 109,19±108,86 mg/l, kreatinin 1,18±0,99 mg/dl, prokalsitonin 1,18±1,93 ng/ml, ferritin 1466.92±3146.69 ml/ng, D-dimer 4,17±4,66 ng/ml, LDH 521±343 U/l idi. Antibiyogram sonucunda suşların %88’inde trimethoprim-sulfametoksazole duyarlı olduğu belirlendi. Ortalama yatış süresi 27,64±25,60 gün idi. Hastaların %25’i mortal seyretti. Bu hastaların %43,1’inde aynı anda başka etkenlere bağlı enfeksiyonları vardı. Sonuç: S. maltophilia özellikle komorbiditesi fazla olan kritik hastalarda etken olabilmektedir. Mortal seyirli olabilen bu enfeksiyonlardan hastaları koruyabilmek için enfeksiyon kontrol ve önlemlerine eksiksiz uyulması gerekmektedir. [ABSTRACT FROM AUTHOR]
