Simple Summary: Pleural mesothelioma is a neoplastic disease originating from mesothelium and is commonly associated with asbestos exposure. Despite the progress made, mortality is still high both because of late diagnosis and treatment resistance. With this review, we want to reassess all aspects of this disease from pathogenesis to diagnosis, and the most recent proposed treatment in order to inspire further research to cover unmet needs. Pleural mesothelioma is an aggressive disease with diffuse nature, low median survival, and prolonged latency presenting difficulty in prognosis, diagnosis, and treatment. Here, we review all these aspects to underline the progress being made in its investigation and to emphasize how much work remains to be carried out to improve prognosis and treatment.
