1. Resectable pancreas adenocarcinoma: the longer the wait time, the longer the oncologic survival?

To the Editor, We read with great interests the article by Vasilyeva et al.[1] In this retrospective observational study, after excluding patients with borderline resectable or metastatic pancreas adenocarcinoma (PAC) based on preoperative imaging, 144 patients with resectable PAC were divided into four wait-time groups (<4, 4-8, 8-12, and >12 weeks). It is totally believed that the longer the wait time to treat, the more advanced the cancer progression, especially for those extremely aggressive cancers like PAC.[2] Moreover, from our experience, considerable proportion of originally resectable PACs would become unresectable if having a delay of >12 weeks. Surprisingly, in this cohort of lucky patients who still had the chance of curative-intent surgery after a varying wait time, >70% (33/47) of the patients in >12 weeks group underwent curative pancreaticoduodenectomy, while only <30% underwent palliative bypass surgery.
