Purpose: The purpose of this study was to investigate the changes of optical coherence tomography angiography (OCTA) parameters in diabetic retinopathy (DR) using an updated software with 3D projection artifact removal. Methods: In this cross-sectional observational study, 192 eyes of 111 patients with diabetes mellitus (DM) and 55 eyes of 34 age-matched healthy subjects were included. Diabetic patients were divided into three subgroups: without DR, with mild non-proliferative DR, and with moderate-to-severe non-proliferative DR. All eyes underwent dilated fundoscopy along with 3x3mm and 6x6mm OCTA image acquisition. Vessel density (VD), retinal thickness and foveal avascular zone (FAZ) parameters were analyzed. Correlation analyses between OCTA parameters and DR severity were also performed. Results: There was a statistically significant difference in all OCTA parameters among groups, except for superficial foveal VD in 6x6mm scan and whole image retinal thickness in both 3x3mm and 6x6mm scans, while 3x3mm scan parameters were found to be diagnostically superior to the corresponding ones of 6x6mm scan. As the DR stage progressed, the mean VD values decreased. FD-300, which is the VD of a 300-μm width annulus surrounding FAZ, demonstrated the strongest inverse correlation with DR severity (r = −0.590/rs = −0.562, p <.001) and showed the highest area under the ROC curve (AUROC = 0.833 ± 0.030, p <.001) in scan 3 × 3. Conclusion: OCTA shows progressive decrease of VD parameters with increasing DR severity. Foveal VD, FAZ area, and perimeter are not very useful indexes due to the high interindividual variability of FAZ size. Purpose: To compare the hypotensive effect of travoprost 0.004%/timolol 0.5% fixed combination (TTFC) to the concomitant use of travoprost and timolol 0.1% gel formulation (Trav + Geltim). Materials and methods: Thirty-three patients (62 eyes) were enrolled and divided into two groups. Patients in group 1 (31 eyes) received the TTFC and patients in group 2 (31 eyes) received the concomitant treatment with Trav + Geltim. Patients on previous antiglaucoma treatment discontinued their drops for 2–4 weeks before starting their new treatment (TTFC or Trav + Geltim). The drops were instilled in the evening in group 1 and in group 2, the prostaglandin was installed in the evening, and timolol in the morning. IOP was measured at 1 and 3 months after the initiation of treatment at four time points during the day (09:00, 12:00, 15:00, and 18:00). Results: Both groups showed significant IOP reduction from baseline at all time points at 1 and 3 months. When the two groups were compared, group 2 showed slightly better hypotensive effect that reached statistical significance only at the 18:00 time point at both 1 and 3 months. Conclusion: Both the TTFC and the concomitant use of the travoprost/timolol gel showed similar hypotensive effect with the latter being slightly more potent in reducing the IOP. Laser-assisted refractive procedures have become very popular in the last two decades. As a result, a “generation” of patients with altered corneal properties is emerging. These patients will require both cataract extraction and glaucoma follow-up in the future. Since the glaucoma examination largely depends on the corneal properties, the reshaped postrefractive surgery cornea poses a challenge in the diagnosis, follow-up, and management of the glaucomatous patient. In order to overcome this problem, every patient who is planned to undergo corneal refractive surgery must have a thorough glaucoma examination in order for the ophthalmologist to be able to monitor their patients for possible glaucoma development and/or progression. Some examinations such as tonometry are largely affected by the corneal properties, while others such as the evaluation of the structures of the posterior pole remain unaffected. However, the new imaging modalities of the anterior segment in combination with the most recent advances in tonometry can accurately assess the risk for glaucoma and the need for treatment. This report describes a case of a newly diagnosed 49-year-old HIV patient, who presented with decreased visual acuity and retinal lesions characterized by ischemia at the level of the deep retinal capillary plexus, documented with optical coherence tomography (OCT), OCT angiography, fluorescein angiography, and visual fields testing. These lesions closely resembled the morphologic and clinical characteristics of late paracentral acute middle maculopathy. The presence of these lesions suggests that HIV microangiopathy can potentially affect both superficial and deep retinal capillary plexuses. [ABSTRACT FROM AUTHOR]

Purpose: To study the effects of maximum dynamic physical exercise on retrobulbar blood flow and ocular perfusion pressure (OPP). Methods: Thirty male subjects undergoing routine periodic medical evaluation were included. All participants underwent cardiac stress test according to a standard protocol. Before the test, the intraocular pressure was measured and colour Doppler imaging was performed with a 7.5 MHz linear probe, to record peak systolic velocity (PSV), end diastolic velocity and resistivity index at the ophthalmic artery (OA), central retinal artery (CRA) and nasal and temporal branches of short posterior ciliary arteries (SPCA). The same measurements were repeated 1 and 30 min after the test. Results: OPP and PSV at the OA were significantly higher at the 1-min interval, compared with the pretest scores ( P = 0.01, in both cases), whereas the respective differences on the 30-min interval were statistically not significant. On the contrary, PSV at the CRA and SPCA were not significantly changed on the same intervals. Differences between pretest and post-test scores for end diastolic velocity and resistivity index were statistically not significant for all examined vessels. Conclusions: Maximal physical exercise increases OPP and blood flow at the OA without affecting blood flow at the CRA and SPCA, implying that auto-regulative mechanisms are active in both retinal and choroidal circulations. Purpose: To evaluate the 24-hour intraocular pressure (IOP)–lowering effect of latanoprost and the dorzolamide/timolol fixed combination (DTFC) after 2 and 6 months of treatment. Design: Randomized, prospective, crossover comparison. Participants: Thirty-nine patients had primary open-angle glaucoma, and 14 patients had ocular hypertension. Methods: After a 6-week washout period, patients were randomized to either 6 months of treatment with the DTFC twice daily or latanoprost every evening and then crossed over to the opposite treatment for an additional 6 months. Main Outcome Measure: Mean 24-hour IOP after 2 and 6 months of treatment. Results: Fifty-three patients had an average 24-hour baseline IOP of 25.2±2.3 mmHg. After 6 months of treatment, 24-hour IOPs were 18.1±1.9 mmHg for the DTFC and 18.3±1.9 mmHg for latanoprost. Compared with 2 months of therapy, at 6 months the DTFC showed no significant change in mean 24-hour IOP, whereas latanoprost demonstrated a reduction of 0.3 mmHg (P = 0.01). The DTFC had more burning (P<0.001) and bitter taste (P = 0.01), whereas the latanoprost had more hypertrichosis (P = 0.02). Conclusions: After 6 months of therapy, the DTFC and latanoprost have clinically similar 24-hour IOP-lowering efficacies. [Copyright &y& Elsevier]

Objective: To evaluate 24-hour intraocular pressure (IOP) efficacy of latanoprost versus travoprost, each given every evening, in exfoliative glaucoma patients. Design: Prospective, observer-masked, crossover comparison. Participants: Forty patients with exfoliation glaucoma. Methods: Patients with a pressure of >24 mmHg were randomized to latanoprost or travoprost for an 8-week treatment period after a 6-week medicine-free period. Patients were then switched to the opposite treatment for the second period. At untreated baseline and at the end of each treatment period the IOP was measured at 6 am, 10 am, 2 pm, 6 pm, 10 pm, and 2 am. Main Outcome Measure: Diurnal IOP. Results: The mean 24-hour IOP was 25.1±2.5 mmHg at baseline, 17.8±2.1 mmHg on latanoprost, and 17.3±2.2 mmHg on travoprost (P = 0.001). Individual time points were similar between treatments, except at 6 pm when travoprost provided lower IOP (16.7±2.6 vs 17.9±2.5 mmHg, P<0.001). Adverse events showed more conjunctival hyperemia with travoprost (n = 15) than latanoprost (n = 6; P = 0.03). Conclusions: Latanoprost and travoprost both significantly reduce the 24-hour IOP from baseline in exfoliative glaucoma, but travoprost may demonstrate a greater hypotensive efficacy in the late afternoon. : PurposeTo prospectively compare the influence of fornix-based and limbus-based conjunctival flaps on the final outcome and complications of 2-site phacotrabeculectomy with mitomycin-C in fellow eyes of patients with bilateral open-angle glaucoma (OAG).: SettingGlaucoma Unit, Department of Ophthalmology, University of Crete, Crete, Greece.: MethodsTwenty-two patients with bilateral primary OAG and 8 patients with bilateral exfoliative glaucoma had 2-site phacotrabeculectomy in both eyes. Eyes were randomly assigned to the fornix-based flap or limbus-based flap group by the use of random tables.: ResultsThe intraocular pressure (IOP) decreased significantly in both groups (P < .01); however, there was no statistically significant difference between the groups in the amount of IOP decrease or the number of postoperative antiglaucoma medications after a 1-year follow-up. Faster improvement in visual acuity was observed in the fornix-based group during the first week. The mean time of surgery was 3.5 minutes less in the fornix-based group. An increased incidence of fibrin exudation, pupillary membrane formation, and capsule opacification was found in eyes with exfoliative glaucoma. The early bleb leakage was 3 times more frequent in the fornix-based group.: ConclusionThe type of conjunctival flap in a 2-site phacotrabeculectomy did not seem to influence the final outcome. The main advantage of the fornix-based conjunctival flap is the shorter surgical time and the relatively faster improvement in vision postoperatively. Purpose: Emerging evidence suggests that choroidal microcirculation and microstructural changes after verteporfin photodynamic therapy (vPDT) for chronic central serous chorioretinopathy (CSC) can be shown in detail using OCT-Angiography (OCT-A). The use of OCT-A for the examination of choriocapillaris (CC) has attracted significant attention as the technique offers potential explanations for the effects of vPDT on choroidal tissue. Methods: A meticulous literature search was performed in the PubMed database without restriction on year of publication until June 2021. The reference list of all electronically retrieved articles was carefully reviewed for potentially relevant articles that had not been identified. Results: We identified and reviewed 11 studies reporting a comprehensive update on microvasculature and morphologic changes of the CC layer as seen on OCT-A in chronic CSC. The reviewed articles extensively analyze both the qualitative and quantitative characteristics of the CC flow pattern after applying vPDT safety-enhanced protocols. The changes in the CC plexus indicate the potential of beneficial or deleterious treatment effect on choroidal tissue remodeling. The reviewed series have revealed variability of flow pattern, vessel density, and perfusion of the CC over time. Conclusion: The CC plexus alterations during the post-vPDT period in chronic CSC may imply the treatment effect on choroidal tissue, indicating the potential of anatomical or functional recovery over time. A letter to the editor is presented related to the relationship between visual ability and visual fields.

Purpose: To examine central corneal mechanical sensitivity (CCMS) after the instillation of latanoprost, travoprost, and bimatoprost. Design: Nonrandomized interventional study. Methods: Latanoprost was used in 24 patients (43 eyes), travoprost in 21 patients (42 eyes), and bimatoprost in 15 patients (28 eyes). CCMS was examined with the Cochet-Bonnet esthesiometer before the instillation of prostaglandin analogs and then 5 and 30 minutes later. Examination of Schirmer and breakup time (BUT) tests were also performed. Thirty healthy subjects were examined as a control group. Results: CCMS was significantly reduced at the 5-minute interval for latanoprost (P = .03), travoprost (P = .04), and bimatoprost (P = .04). Differences between the three analogs were statistically not significant. CCMS changes significantly correlated with Schirmer and BUT test scores. Conclusions: The correlation between CCMS reduction and BUT and Schirmer test scores implies that the use of artificial tears may be considered if prostaglandin analogs are administered. Purpose: The aim of this study was to evaluate and compare the changes in Intraocular Pressure (IOP) and other ocular parameters: the Anterior Chamber Angle (ACA), Anterior Chamber Volume (ACV), and Anterior Chamber Depth (ACD) during phacoemulsification surgery in Greek patients with normotensive eyes and those with well-controlled Open-Angle Glaucoma (OAG). Additionally, parameters such as the Corneal Thickness (CCT), Axial Length (AL), Central Macular Thickness (CMT), and Retinal Nerve Fibre Layer (RNFL) were also examined. Patients and Methods: This was a prospective observational case–control study that included 50 phakic eyes, 25 normotensive (Group 1), and 25 with OAG: 15 Primary Open-Angle Glaucoma (POAG) and 10 Exfoliation Glaucoma (EXG) (Group 2). Ophthalmic assessment included IOP measurements, ocular biometry, and anterior and posterior segment optical coherence tomography evaluation of the aforementioned ocular parameters, prior and 6 months after phacoemulsification surgery. Results: At the 6 months post-operative review, a greater IOP reduction was recorded in eyes with OAG, in comparison to normotensive ones (5.3mmHg and 1.6 mmHg respectively). In addition, a significant but similar increase was recorded in the values of the ACA, ACV, and ACD of both groups between the pre- and the post-op period. Furthermore, the CCT and AL values remained unaltered. Finally, there was a non-statistically significant change in the mean CMT and the mean average RNFL of both groups. Conclusion: Eyes with OAG tend to undergo a greater reduction in IOP post-phacoemulsification surgery, in comparison to normotensive eyes. This reduction may not be solely attributed to ocular anatomical changes after phacoemulsification surgery but may also be due to the remodeling of the trabecular meshwork and the ciliary body. Introduction: The aim of this study was to evaluate the correlation between several ocular parameters (intraocular pressure [IOP], corneal biomechanical properties) and the visual field (VF) mean deviation (VF MD) in eyes with open-angle glaucoma (OAG). Methods: We conducted a cross-sectional, observational study in which we measured the IOP with Goldmann applanation tonometry, the central corneal thickness (CCT), and the corneal parameters obtained from the Ocular Response Analyzer® (ORA®) and the Corvis® ST non-contact tonometer, in newly diagnosed and treatment-naïve eyes with OAG, to investigate whether there was any correlation between these ocular parameters and the VF MD. Results: A total of 51 eyes were analyzed. A statistically significant correlation was found only between the VF MD and corneal hysteresis (CH) (P = 0.003, r2 = 0.16) and CCT (P = 0.03, r2 = 0.08). Purpose: The aim of this study was to evaluate the characteristics of corneal endothelial cells and central corneal thickness (CCT) in patients with diabetes mellitus (DM), comparing them with those of healthy subjects (controls) and to determine potential factors affecting the corneal parameters in patients with DM. Methods: Participants in this study were 72 patients with DM and 88 healthy controls. Diabetic patients were further classified into groups depending on the severity of diabetic retinopathy (no retinopathy, mild, moderate, severe non-proliferative diabetic retinopathy, and proliferative diabetic retinopathy). All participants underwent non-contact specular microscopy to evaluate corneal endothelium parameters and CCT, while factors affecting endothelial cell density and CCT in patients with DM were also analyzed. Results: Patients with DM presented significantly decreased endothelial cell density compared to controls (2,297.9 ± 311.3 and 2,518.3 ± 243.7 cells/mm2, respectively; p < 0.001), while the two groups did not differ significantly in any other measured corneal parameter. In the diabetic group, the multivariate analysis showed a significant association between decreased endothelial cell density and increased HbA1c (p < 0.001), longer DM duration (p = 0.003), and more severe diabetic retinopathy status (p = 0.008). Conclusion: DM seems to affect the corneal endothelium, since endothelial cell density was decreased in the diabetic group, while duration of disease, HbA1c levels, and severity of retinopathy were significantly associated with changes in endothelial cell density and should be taken into account. To evaluate peripapillary retinal nerve fiber layer (RNFL) changes in patients with diabetes mellitus (DM) and compare them with those of normal population. In addition, this study aims to determine potential factors, affecting RNFL changes in patients with DM. Participants in this study were 107 patients (211 eyes) with DM and 100 healthy controls (200 eyes). Diabetic patients were further classified into four groups depending on severity of diabetic retinopathy (no retinopathy, mild, moderate, severe nonproliferative diabetic retinopathy, and proliferative diabetic retinopathy, while presence of macular edema was also assessed. All participants underwent spectral-domain optical coherence tomography (SD-OCT) to measure RNFL thickness, while demographic and clinical characteristics of the participants were also recorded. Patients with DM with or without DR presented significantly decreased peripapillary RNFL thickness in all quadrants. In the diabetic group, the multivariate analysis showed a significant association between decreased average RNFL thickness and increased HbA1c (p < 0.001), longer DM duration (p = 0.007), and more severe diabetic retinopathy status (p = 0.016), while presence of DME, age, gender, hypertension, and hyperlipidemia were not found to be associated with RNFL thickness decrease. Diabetes mellitus seems to affect RNFL thickness, suggesting that early neurodegenerative changes may occur, potentially before microvascular alterations. Since duration of disease, DR severity, and HbA1c levels were associated with RNFL thinning, modifying glycemic control seems to be important to potentially prevent the development of DM complications. To analyze the recovery of the subbasal nerve plexus in corneas treated with laser in situ keratomileusis (LASIK) at least 10 years prior compared with nonoperated corneas. Clínica Novovisión, Madrid, Spain. Prospective nonrandomized observational study. Eyes that had LASIK surgery at least 10 years before (LASIK group) were compared with nonoperated healthy eyes (control group). The subbasal nerve plexus morphology (main nerves density, nerve length, nerve branches density, grade of nerve tortuosity, and reflectivity) and the dendritic cell density from the confocal images obtained with the Heidelberg Retina Tomograph II were analyzed. The study comprised 47 eyes (27 eyes in the LASIK group and 20 eyes in the control group). The values were significantly lower in the LASIK group compared with the control group in main nerves density (9.15 nerves/mm2 ± 3.46 [SD] versus 11.75 ± 2.86 nerves/mm2; P =.009) and nerve branches density (2.96 ± 1.99 nerves/mm2 versus 5.35 ± 3.05 nerves/mm2; P =.002). No significant differences were found in nerve length, grade of nerve tortuosity, dendritic cell density, or subbasal nerve plexus reflectivity between the two groups. The outcomes suggested that the subbasal corneal nerve plexus did not completely recover its preoperative pattern at least 10 years after LASIK. Both main nerves density and nerve branches density continued to be significantly lower compared with virgin corneas after a mean 13-year follow-up, although other parameters of nerve morphology (nerve length, tortuosity, and reflectivity) returned to normal levels. Purpose To develop an algorithm for the Fourier analysis of posterior corneal videokeratographic data and to evaluate the derived parameters in the diagnosis of Subclinical Keratoconus ( SKC) and Keratoconus ( KC). Methods This was a cross-sectional, observational study that took place in the Eye Institute of Thrace, Democritus University, Greece. Eighty eyes formed the KC group, 55 eyes formed the SKC group while 50 normal eyes populated the control group. A self-developed algorithm in visual basic for Microsoft Excel performed a Fourier series harmonic analysis for the posterior corneal sagittal curvature data. The algorithm decomposed the obtained curvatures into a spherical component, regular astigmatism, asymmetry and higher order irregularities for averaged central 4 mm and for each individual ring separately (1, 2, 3 and 4 mm). The obtained values were evaluated for their diagnostic capacity using receiver operating curves ( ROC). Logistic regression was attempted for the identification of a combined diagnostic model. Results Significant differences were detected in regular astigmatism, asymmetry and higher order irregularities among groups. For the SKC group, the parameters with high diagnostic ability ( AUC > 90%) were the higher order irregularities, the asymmetry and the regular astigmatism, mainly in the corneal periphery. Higher predictive accuracy was identified using diagnostic models that combined the asymmetry, regular astigmatism and higher order irregularities in averaged 3and 4 mm area ( AUC: 98.4%, Sensitivity: 91.7% and Specificity:100%). Conclusions Fourier decomposition of posterior Keratometric data provides parameters with high accuracy in differentiating SKC from normal corneas and should be included in the prompt diagnosis of KC. Purpose To evaluate the contribution of Fourier analysis of videokeratographic data in the diagnosis of subclinical keratoconus and keratoconus. Setting Eye Institute of Thrace, Democritus University, Alexandroupolis, Greece. Design Observational case series. Methods The following Pentacam-derived parameters, resulting from Fourier decomposition of keratometric data, were evaluated for their diagnostic capacity using receiver operating curves: spherical component and eccentricity, maximum decentration, regular astigmatism in the center and in the periphery, mean astigmatism, irregularities, regular astigmatism in the center plus the irregularities, and total astigmatism. Logistic regression was performed to identify a combined diagnostic model. Results The study comprised 80 keratoconus eyes, 55 eyes diagnosed with subclinical keratoconus, and 50 normal eyes. Significant differences were detected in spherical eccentricity, maximum decentration, irregularities, regular astigmatism in the center and in the periphery, regular astigmatism in the center plus the irregularities, mean astigmatism, and total astigmatism parameters between the groups. Almost all parameters had high diagnostic ability in both study groups (area under the curve >90%). Among individual parameters, those with the highest predictive accuracy were the regular astigmatism in the center plus the irregularities (subclinical keratoconus 97.6%, keratoconus 98.8%) and the maximum decentration (subclinical keratoconus 91.4%, keratoconus 98.5%). Sufficient predictive accuracy (subclinical keratoconus 99.4, keratoconus 100%) was identified in a diagnostic model that combined the regular astigmatism in the center plus the irregularities and the maximum decentration. Conclusion Fourier decomposition of keratometric data provided parameters with high accuracy in differentiating corneas with subclinical keratoconus from normal corneas and should be included to allow prompt diagnosis of keratoconus. Purpose: To assess the potential impact of corneal crosslinking treatment (365 nm ultraviolet (UV)A irradiation with riboflavin) on the material properties of Intracorneal Rings Segments (ICRS). Materials and methods: Material properties were studied using FT-IR spectroscopy and UV-Vis spectroscopy. Rings were examined: (1) after installation of riboflavin solution, (2) after irradiation with UV-A light and (3) after instillation of riboflavin solution followed by instillation and coeval irradiation. The experiments followed the standard corneal cross-linking (CXL) protocol of corneal crosslinking treatment. Results: After instillation of riboflavin solution, a permanent intense yellow staining of the samples was observed. UV-Vis spectroscopy confirmed that a certain amount of riboflavin solution was absorbed into the samples after CXL procedure. FT-IR spectroscopic analysis showed alterations in the spectra of ICRS mainly at the 2800-3200 cm−1 spectral region [modification in band intensities of CH2 (2925 cm−1) and CH3 (2950 cm−1)]. Conclusions: Our results suggest crosslinking reaction in ICRS material. This should be taken into consideration prior to any CXL treatment of post ICRS-implanted cornea. [ABSTRACT FROM AUTHOR]

Purpose To compare the effect of monovision correction and multifocal intraocular lens (IOL) implantation on patient satisfaction, spectacle dependence, visual acuity, and dysphotopsia in cataract patients. Setting University Hospital of Alexandroupolis, Democritus University of Thrace, Alexandroupolis, Greece. Design Prospective randomized trial. Methods Patients with a diagnosis of senile cataract with stage 2 nuclear opalescence were randomly assigned to 2 groups: monovision and multifocal IOL implantation. Uncorrected (UDVA) and corrected (CDVA) distance visual acuity, Visual Function Index-14 (VF-14) scores, and spectacle dependence were assessed prior to surgery and 6 months postoperatively. Results The monovision group comprised 38 patients and the multifocal IOL implantation group, 37 patients. Both techniques provided excellent refractive outcomes in UDVA and VF-14 scores (all P < .01). No significant intergroup differences were detected in VF-14 scores at the final postoperative examination. The monovision group patients presented significantly more spectacle dependence for near vision but less glare. Conclusions Monovision and multifocal IOL implantation provided excellent refractive outcomes for distance vision. Multifocal IOL insertion was associated with less dependence on glasses overall but significantly more dysphotopsia. Introduction Infectious keratitis following penetrating keratoplasty is a common postoperative complication. Intensive topical and systemic treatments do not always prevent the risk of graft failure. In this report we demonstrate the beneficial anti-microbial effect of corneal collagen cross-linking in a late-onset, sight-threatening, corneal graft ulcer. Case presentation A 57-year old Caucasian man underwent penetrating keratoplasty in his left eye, due to corneal bullosa after cataract extraction surgery. Twelve months after the penetrating keratoplasty, he visited our emergency service complaining of burning and foreign body sensation in his operated eye. Slit-lamp examination revealed a central, round-shaped ulcer of the graft. Due to poor response to the intensive topical antibiotic therapy, corneal collagen cross-linking was applied 3 days after admission, in an attempt to control the infection. Cultures indicated the predominance of methicillin-resistant Staphylococcus aureus infection. Five days after corneal collagen cross-linking treatment, the epithelium was completely reepithelized, while the transparency of the transplanted cornea was gradually restored within the 12-month follow-up period. No relapses occurred. Conclusion Corneal collagen cross-linking seems to be a safe and effective therapeutic alternative in resistant cases of infectious keratitis following penetrating keratoplasty. Purpose: To evaluate the diagnostic capacity of the keratoconus match index (KMI) and keratoconus match probability (KMP) classification from a dynamic bidirectional applanation device (Ocular Response Analyzer) in eyes with pellucid marginal degeneration (PMD). Setting: Department of Ophthalmology, University Clinics Saarland, Homburg, Germany. Design: Cross-sectional study. Methods: Pellucid marginal degeneration eligibility was confirmed by inferior peripheral thinning, corneal protrusion, and irregular astigmatism. The KMI and KMP parameters in PMD eyes (study group) were compared with those in normal eyes (control group). The KMI's overall predictive accuracy was assessed operating characteristic (ROC) curves. The relationship between KMI and corneal hysteresis (CH), the corneal resistance factor (CRF), and a series of Scheimpflug camera indices was evaluated with Spearman analysis (r). Results: The mean KMI in the study group (40 eyes) and control group (40 eyes) was 0.34 ± 0.43 (SD) and 0.95 ± 0.30, respectively (P<.001). The KMI correlated significantly with the CH, CRF, and most Scheimpflug camera indices. Regression analysis indicated that the index of height decentration (r = −0.877, P<.001) was the primary determinant of the KMI. Moreover, the KMP index identified 50.0%, 29.16%, and 20.83% of PMD eyes as ectatic, suspect for ectasia, and normal, respectively. The ROC curve analysis of the KMI parameter indicated a predictive accuracy of 94.8% (cutoff point 0.626; sensitivity 85.71%; specificity 90.1%). Conclusions: The KMI seems to be a promising diagnostic index for PMD. In contrast, the KMP index identified a significant percentage of topographically defined PMD eyes as normal, limiting its diagnostic value in PMD. Degenerative ocular conditions, such as age-related macular degeneration, diabetic retinopathy, retinal vein occlusions, and myopic degeneration, have become a major public health problem and a leading cause of blindness in developed countries. Anti-vascular endothelial growth factor (VEGF) drugs seem to be an effective and safe treatment for these conditions. Ranibizumab, a humanized monoclonal antibody antigen-binding fragment, which inhibits all biologically active isoforms of VEGF-A, is still the gold standard treatment for the majority of these pathological entities. In this review, we present the results of the most important clinical trials concerning the efficacy and safety of ranibizumab for the treatment of degenerative ocular conditions. Lacrimal outflow can be compromised by anatomical obstructions or stenoses (nonfunctional epiphora) or by defective lacrimal "pump" function (functional epiphora). Although classic imaging modalities, such as X-ray dacryocystography, computed tomography, or magnetic resonance imaging can effectively evaluate the former, their success is much less in the evaluation of the latter. This is largely due to the fact that forced diagnostic injection of fluid into the canalicular system can overcome partial obstruction sites. On the other hand, lacrimal scintigraphy mimicks "physiological" lacrimal outflow, being performed under pressure gradients present in everyday life. This is why it is considered more suitable for the study of functional epiphora. Furthermore, quantitative lacrimal scintigraphy (with time-activity curves) enables the accurate measurement of lacrimal clearance from the conjunctival fornices and may be used to study the physiology of the lacrimal "pump." Data obtained from the scintigraphic study of lacrimal outflow may be used to design more effective procedures in the management of functional and nonfunctional epiphora. This is a review article, based on a literature search with emphasis on recent publications and on those supporting interdisciplinary cooperation between ophthalmology and nuclear medicine. Background The aim was to assess the intra-session, inter-session and inter-observer repeatability of curvature and aberrometric measurements of Pentacam-derived posterior corneal surface in normal control ( CG), keratoconic ( KCG) and after corneal collagen cross-linked ( CXLG) groups. Methods This was a prospective, clinic-based, observational study that was conducted at the Eye Institute of Thrace ( EIT) in Alexandroupolis, Greece. Sixty-two normal eyes formed the control group, 33 were in the keratoconic group, while 34 eyes, which already had received corneal collagen cross-linking formed the CXLG. Curvature and aberrometric measurements were obtained using the Pentacam by two operators within each of the two sessions. Precision (Pre), repeatability ( COR) and the intra-class correlation coefficient ( ICC) were calculated for evaluating intra-session and inter-session repeatability. Bland-Altman analysis was used for assessing inter-observer repeatability. Results Simulated keratometric parameters K1 and K2, asphericity at the central 8 mm zone (Q8) and central steepest radius of best fit toric ellipsoid reference body (Rs), achieved an acceptable level of accuracy. Aberrometric parameters were repeatable, reliable and reproducible only in terms of the root mean square ( RMS) errors (coma, coma-like and the RMS of higher order aberrations or HOA RMS). For both observers, best precision and lowest range of agreement ( ROA) was found for HOA RMS parameters in all groups (less than 0.27 and less than 0.29 for the control group, less than 0.95 and less than 0.72 for the keratoconic group and less than 0.67 and 0.53 for the collagen cross-linked group). Conclusion The Pentacam system provides precise measurements of corneal posterior keratometry, reference body radius and asphericity for large diameter analysis. Although Zernike coefficients for individuals presented poor repeatability, the coma, coma-like and HOA RMS seem to be accurate indices in characterisation of back corneal aberrations. Purpose: To evaluate the effect of tafluprost on the central corneal thickness (CCT) in patients with primary open-angle glaucoma (POAG). Methods: This was a prospective study and included 100 eyes of 54 patients with POAG. All patients received tafluprost 0.0015% (Saflutan®) once daily in one or both eyes. CCT was measured by using noncontact pachymetry prior to the treatment and after 6 and 12 months. Results: Mean CCT of all treated eyes ( n = 100) was 547.79 ± 29.48 μm at baseline, 535.61 ± 26.54 μm after 6 months and 533.55 ± 26.45 μm after 12 months (Student's t test, p < 0.0001). Ninety-three percent of all treated eyes showed a decrease of CCT. CCT reduction was more significant within the first six months of the treatment period (Student's t test, p < 0.0001). After 12 months, a CCT reduction >25 μm occurred in 5% of all treated eyes. There was a significant positive correlation between the magnitude of corneal thinning and the initial CCT (Pearson, r = 0.49, p < 0.0001) but not between the magnitude of corneal thinning and intraocular pressure (IOP) reduction (Pearson, r = 0.145, p = 0.15). Discussion: Long-term use of tafluprost may decrease the CCT in patients with POAG. Purpose of the study was to develop and assess a novel controlled drug delivery system of latanoprost acid (LA). Poly(lactide)/Monomethoxy-poly(ethyleneglycol) (PLA-PEG) nanoparticles (NPs) were prepared using an emulsification-solvent evaporation technique. NPs were characterized in vitro according to their size, ζ-potential, drug entrapment efficiency and LA release. LA-loaded NPs (equivalent to 8.5 μg LA) were administered into the subconjunctival space of normotensive rabbits (group A). A free LA solution of the same drug content was subconjunctivally injected in a second rabbit group (group B), while blank NPs were administered in a third group (group C). A group of untreated animals (group D) served as the control. Intraocular pressure (IOP) was monitored for 8 consecutive days, using the Tono-pen XL. Aqueous humor (AH) levels of LA were evaluated for 6 days post-administration, by means of HPLC. Mean nanoparticle size was 80 nm. The drug entrapment efficiency was 18.3%. NPs sustained the release of LA over several days in vitro. Non-significant differences in baseline IOP were found between groups (p = 0.22). LA-loaded NPs exerted a significant hypotensive effect on group A, while IOP values remained significantly lower compared to the rest groups, throughout the study (p = 0.04). LA AH concentrations in group B continuously decreased with time, while LA levels in group A steadily increased. On day 6, LA levels were higher in group A compared to group B (344 ± 73.5 ng/ml and 228 ± 41.01 ng/ml, respectively). No adverse effects were observed. In conclusion, after subconjunctival administration, the LA-loaded NPs provided sustained LA delivery in vivo. They appear to be a promising system for the controlled subconjunctival delivery of LA. Purpose: To evaluate the diagnostic capacity of the Ocular Response Analyser's keratoconus match index (KMI) and keratoconus match probability (KMP) classification in a sample of keratoconus (KC) patients. Methods: Keratoconus match index and KMP from 114 KC eyes, randomly selected from 114 patients with bilateral keratoconus (KCG), were compared with the corresponding ones from 109 normal eyes (CG). Keratoconus match index's predictive accuracy was assessed by receiver operating curves (ROC). Keratoconus match probability level of agreement was evaluated at the different KC stages of the Amsler-Krumeich classification. Correlations were estimated with topographic keratoconus classification (TKC), keratoconus index (KI), index of surface variance (ISV), vertical asymmetry (IVA), height asymmetry (IHA), height decentration (IHD), minimal radius (Rmin), central corneal thickness (CCT), thinnest corneal thickness (TCT) mean keratometry (Km) and intraocular pressure (IOPg). Results: Mean KMI in KCG and CG was 0.20 ± 0.38 and 0.98 ± 0.25, respectively (p < 0.01). Significant KMI differences (p < 0.01) were detected in different KC groups [range: 0.62 ± 0.38 (KC 1), −0.62 ± 0.04 (KC 4)]. Significant correlation was detected between KC staging and KMI ( r = −0.56, p < 0.0001). Keratoconus match probability identified 22.03% of the CG eyes as suspect. Moreover, KMP identified 7.01% and 23.68% of the KCG eyes as normal and suspect, respectively. Receiver operating curves analysis for KMI parameter indicated a predictive accuracy of 97.7% (cut-off point: 0.512, sensitivity: 91.18%, specificity: 94.34%). Conclusions: Keratoconus match index seems to be a reliable index in keratoconus diagnosis and staging. Background/Aims: To compare the impact of liquefaction and torsional IP cataract extraction methods on endothelial cell count (ECC), central corneal edema, and central corneal sensitivity (CCS) in a sample of cataract patients. Methods: The liquefaction (LG) and torsional IP (TG) group consisted of 47 and 48 grade 2 cataract eyes, respectively. Uncorrected and best spectacle-corrected visual acuity, ECC, central corneal thickness (CCT), and CCS were measured 1 day prior to surgery, 10 days, 1 and 3 months postoperatively. Results: Significant difficulties in the nucleus fragmentation were encountered in 2 LG eyes, and they were excluded from the study. Both techniques provided excellent refractive outcomes (LG, p < 0.001; TG, p = 0.02). Nonsignificant differences were detected in CCT values at the final postoperative examination; however, TG patients presented higher CCT on the first postoperative day (p = 0.04). Both groups presented comparable significant reductions in ECC (LG, p < 0.001; TG, p < 0.001) and CCS (LG, p = 0.02; TG, p = 0.02). Conclusion: Both methods provide excellent refractive outcomes, with comparable impact on ECC and CCS. Purpose: To assess the efficacy of the Allegretto Wave and the wavefront-optimized ablation profile (WFO) in non-anterior astigmatism correction, in both LASIK and photorefractive keratectomy (PRK) treatments. Methods: Seventy-four refractive surgery candidates were recruited prospectively in a non-randomized trial. Only one eye from each candidate was randomly enrolled in the study. Of them, 40 eyes underwent LASIK treatment (LG group), while 34 eyes underwent PRK treatment (PG group). Preoperatively, the ocular residual astigmatism (ORA) was calculated for each eye, according to which each astigmatism fault was characterized as primarily anterior or non-anterior. Twenty LG eyes and 16 PG eyes presented primarily anterior astigmatism (LG-A and PG-A subgroups, respectively), while 20 LG eyes and 18 PG eyes demonstrated primarily non-anterior astigmatism (LG-NA and PG-NA subgroups, respectively). Postoperatively, vector analysis of astigmatism correction was conducted. The following indexes were calculated: (i) correction index (CI), (ii) difference vector (DV) and (iii) index of success (IOS). Results: Preoperatively, mean differences between manifest and topographic astigmatisms for the LG and the PG subgroups were significant (p:0.006 and p < 0.001, respectively), while postoperatively, aforementioned differences were non-significant (p:0.18 and p:0.09, respectively). Regarding vector analysis in the LG group, mean CI, IOS and DV were 1.39 ± 1.26, 0.37 ± 1.06 and 0.30 ± 0.51, respectively. Differences in CI, IOS and DV between LG-A and LG-NA subgroups were non-significant. Regarding vector analysis in the PG group, mean CI, IOS and DV were 1.22 ± 0.33, 0.47 ± 0.46 and 0.27 ± 0.25, respectively. Differences in CI, IOS and DV between PG-A and PG-NA subgroups were non-significant. Conclusions: Our results suggest that the Allegretto Wave and WFO profile seem to be equally effective in both anterior and non-anterior astigmatism correction, regardless of treatment type. Purpose: To determine the variability in Scheimpflug image–derived posterior elevation measurements in eyes with keratoconus or corneal collagen crosslinking (CXL). Setting: Eye Institute of Thrace, Alexandroupolis, Greece. Design: Prospective observational study. Methods: Two operators in 2 sessions obtained posterior elevation measurements using the Pentacam Scheimpflug system. Variability in measurements was assessed using Bland-Altman analysis and intraclass correlation coefficients (ICCs). Measurements were associated with a series of Scheimpflug-derived keratoconus indices using the Pearson correlation and multivariate linear-regression analysis. Results: Mean interobserver differences for the central elevation point (e1) and the highest elevation point (HEP) in the keratoconus group were −0.9 μm (95% limits of agreement [LoA], 5.7 to −7.5 μm) and 1.7 μm (95% LoA, 26 to −22.6 μm), respectively. In the CXL group, the mean differences were 0.2 μm (95% LoA, 6.0 to −5.5 μm) and −6.1 μm (95% LoA, 16.8 to −29 μm), respectively. The ICC values in the keratoconus group were satisfactory for most elevation points; the ICC values were unsatisfactory for a series of points in the lower corneal hemisphere. The ICC values for points in both hemispheres were unsatisfactory in the CXL group; HEP had adequate reproducibility. The e1 and HEP parameters correlated significantly with most Scheimpflug-derived indices (P<.05) in the keratoconus group but not in the CXL group. Regression analysis indicated that the central keratoconus index was the primary determinant of the variability in keratoconus eyes. Conclusions: Scheimpflug-derived posterior elevation measurements had acceptable reproducibility in keratoconus eyes and post-CXL eyes. Background: To evaluate corneal hysteresis (CH) and corneal resistance factor (CRF) in keratoconic (KC) eyes before and after corneal collagen cross-linking (CXL). Furthermore, to determine potential correlations with a series of corneal and demographic factors. Methods: The study consisted of 50 KC eyes that underwent CXL. CH and CRF were measured by the ocular response analyzer (ORA). Correlations were attempted with uncorrected visual acuity (UVA), best spectacle-corrected visual acuity (BSCVA), central corneal thickness (CCT), mean keratometry (Km), astigmatism (Astig.), residual astigmatism, age, and gender. Fifty non-KC eyes served as controls. Results: CH and CRF (mean ± SD) for non-KC eyes were 10.1 ± 1.9 mmHg and 9.7 ± 2.4 mmHg respectively, while for KC eyes preoperatively they were 8.2 ± 1.4 mmHg ( p = 0.007) and 7.4 ± 2.3 mmHg ( p = 0.01) respectively. Non-significant differences were detected between preoperative and postoperative CH and CRF measurements in KC eyes ( p = 0.518 and p = 0.479 respectively). Significant correlations were found between ORA parameters and BSCVA, CCT, Km, Astig. and residual astigmatism. Conclusions: ORA parameters demonstrate significant differences between KC and non-KC eyes. Both CH and CRF present significant correlations with visual acuity and corneal parameters. A bstract Background: To evaluate the level of agreement between intended and measured ablation of the wavefront optimize profile corrected by the Wellington nomogram in myopic spherocylindrical corrections and assess its impact on refractive outcomes. Design: Prospective, university-institute setting. Participants: Eighty-six participants (86 eyes for photorefractive keratectomy group) and 86 participants (86 eyes for laser in situ keratomileusis group) recruited in a consecutive-if -eligible basis. Methods: Differences between intended and measured ablation were evaluated with Scheimpflug camera. Refractive outcomes were evaluated by means of postoperative spherical equivalent, postoperative defocus equivalent, contrast sensitivity, correction index, difference vector and index of success. Main Outcome Measures: Correlation of visual outcomes with intended ablation. Results: Both groups demonstrated significant over-ablations (16.7 ± 8.5, P < 0.001 & 11.8 ± 18.5, P < 0.001, respectively). Intended ablation was the primary determinant of the measured difference ( r-square 0.769 & 0.765, respectively). Photorefractive keratectomy corrections over 100 µ had significant impact on postoperative spherical equivalent, postoperative defocus equivalent, correction index, difference vector and index of success ( P = 0.044, P = 0.05, P = 0.019, P = 0.016, P = 0.006, respectively), but laser in situ keratomileusis corrections over 100 µ had significant impact only on postoperative defocus equivalent, difference vector and index of success ( P = 0.04, P = 0.05). Conclusions: The results suggest that the wavefront optimize profile seems to over-ablate corneal tissue both in photorefractive keratectomy and laser in situ keratomileusis. Purpose: To assess flap-thickness predictability with a pendular microkeratome (130 μm head). Setting: Eye Institute of Thrace, Democritus University of Thrace, Alexandroupolis, Greece. Design: Clinical trials. Methods: The study comprised 263 eyes (132 patients). Laser in situ keratomileusis was performed using the 130 μm head of the Carriazo pendular microkeratome; right eyes were treated first. Ultrasound pachymetry and topography were used for central corneal thickness (CCT) and keratometry (K) measurements. Evaluation included flap thickness, flap diameter, and flap shape. Results: The mean flap thickness was 125 μm ± 22 (SD) (range 74 to 187 μm) in right eyes and 112 ± 21 μm (range 61 to 190 μm) in left eyes. Flap thickness was significantly correlated with preoperative CCT (r = 0.271; P<.001) but not with K values or the manifest refraction spherical equivalent (P>.15). Right eyes had thicker flaps than left eyes (P<.001); both were significantly below the 130 μm head thickness (mean flap thickness 119.2 ± 22.8 μm; P<.001). The mean achieved flap diameter was 9.2 mm using the 9.0 ring and 9.8 mm using the 10.0 mm ring. Flap-thickness stabilization and convergence between right eyes and left eyes occurred after 100 consecutive flap cuts. Conclusions: Flap-thickness predictability was influenced by preoperative CCT only. All cuts were significantly thinner than the head thickness regardless of the suction ring size. Second surgical eyes had thinner flaps, possibly from blade deterioration from the first cut. Approximately 100 flaps were required as a learning curve. Purpose: To determine the direct costs of therapy over 5 years of a European monotherapy cohort begun on a prostaglandin (PTG) versus timolol in patients with primary open-angle glaucoma or ocular hypertension. Methods: A retrospective, multicenter, active-controlled, observational study. Data were abstracted for European patients treated as initial monotherapy in 1996 or afterward, with 5 years of available records. Results: This study included 271 patients (166 on a PTG and 105 on timolol at baseline). The average cost/month/patient over 5 years was $45.47±12.61 for PTG and $31.50±15.47 for timolol ( P<0.001, based on German prices). After 5 years, although there was no difference in number of glaucoma medicines prescribed between groups (1.0 PTGs and 1.1 timolol, P=0.41), the timolol group demonstrated a higher intraocular pressure (17.7±2.9 vs. 16.5±3.0 mm Hg, P<0.001), more medication changes ( P=0.01), greater incidence of glaucomatous progression ( P=0.04), and less patients persistent on original monotherapy ( P<0.001) than the PTG cohort. Conclusions: Patients originally on timolol monotherapy have a lower cost of care over 5 years than those started on a PTG. However, timolol patients during follow-up may demonstrate a higher intraocular pressure, more progression, more medication changes, and lower persistency of the original monotherapy. Background: To evaluate and compare the recorded IOP values of the Pascal dynamic contour tonometer (PDCT) and the Goldmann applanation tonometer (GAT) after photorefractive keratectomy (PRK) and laser in situ keratomileusis (LASIK). Methods: Three groups of 84, 182 and 43 patients each were treated by PRK for myopia, LASIK for myopia and LASIK for hyperopia respectively. Intraocular pressure (IOP) measurements were performed in all eyes 1 day before and 1, 3, 6 and 12 months after treatment with PDCT and GAT. Ultrasound pachymetry was performed in all eyes preoperatively and at 1st month postoperatively. Results: Preoperatively and postoperatively, GAT readings were lower than PDCT in all groups (all p < 0.05). Postoperatively in the PRK group, compared to the preoperative value, the mean differences of IOP recorded with GAT at 1st, 3rd, 6th and 12th month were −1.4 mmHg, −1.7 mmHg, −1.7 mmHg and −1.9 mmHg respectively (all p < 0.05). In the myopic LASIK group; the corresponding values with GAT were −3.6 mmHg, −3.6 mmHg, −3.6 mmHg and −3.5 mmHg (all p < 0.05), while in the hyperopic LASIK group the corresponding values were −1.1 mmHg, −0.7 mmHg, −1.1 mmHg and −0.9 mmHg (all p < 0.05). The mean IOP difference (GAT-PDCT) for myopic PRK, myopic LASIK and hyperopic LASIK were respectively −3.8 mmHg, −4.1 mmHg and −1.5 mmHg at the 12-month follow-up. No statistically significant changes were found for any group with the PDCT. Conclusions: GAT-determined IOP values were significant lower at all time-points after hyperopic LASIK, as well as myopic PRK or LASIK. The Pascal tonometry values remained unaffected for all groups. Introduction: Corneal collagen cross-linking is a rather new technique that uses riboflavin and ultraviolet A light for collagen fiber stabilization in keratoconus corneas. Other than reversible side effects, the preliminary results of corneal collagen cross-linking studies suggest that it is a rather safe technique. In this report, we demonstrate a case of corneal melting after corneal collagen cross-linking for keratoconus corneas associated with an acute inflammatory response.Case Presentation: A 23-year-old Caucasian man with keratoconus cornea stage 1 to 2 underwent uneventful corneal collagen cross-linking treatment according to the Dresden protocol. The next day the patient had intense photophobia, watering and redness of the eye, and his visual acuity was limited to counting fingers. Slit lamp biomicroscopy revealed severe corneal haze accompanied by non-specific endothelial precipitates following an acute inflammatory response. Mild inflammation could be detected in the anterior chamber. Moreover, the re-epithelialization process could barely be detected. His corneal state gradually deteriorated, resulting in descemetocele and finally perforation.Conclusion: In this report, we present a case of a patient with corneal melting after standard corneal collagen cross-linking treatment for keratoconus corneas following an acute inflammatory response. Despite modifying postoperative treatment, elaboration of all apparent associated causes by the treating physicians and undergoing extensive laboratory testing, the patient developed descemetocele, which led to perforation. To evaluate the impact of photorefractive keratectomy (PRK) and laser in situ keratomileusis (LASIK), on retinal nerve fiber layer (RNFL) measurements, by means of scanning laser polarimetry (SLP) GDx VCC (with variable corneal compensation). Two groups of 119 myopic patients each were treated by PRK and LASIK respectively; one eye of each patient was randomly selected for the study. RNFL measurements (around the entire calculation circle) was performed in all eyes 1 day before and 1, 3, 6 and 12 months after treatment by using SLP GDx VCC. Corneal compensation was performed in all eyes preoperatively, and in every examination step postoperatively, in order to neutralize the new corneal birefringence. No statistical differences were found between the PRK and LASIK groups, in all preoperative and postoperative RNFL measurements. Only the p values for CCT and mean K-readings preoperatively were statistically significant. PRK and LASIK do not seem to influence the RNFL measurements at 1st, 3rd, 6th and 12th postoperative months when measured with SLP GDx VCC. Purpose: The aim of this study was to evaluate the efficacy and safety in ocular hypertensive or open-angle glaucoma patients changed to a latanoprost/timolol fixed combination (LTFC) from previous therapy. Methods: We employed a retrospective, multicenter, active-controlled study evaluating patients who had a previous therapy substituted with LTFC and had a 3-month follow-up. Results: In the 168 patients included in the trial, LTFC reduced the intraocular pressure (IOP) after switching from previous therapies: timolol (22.8 ± 3.5 to 19.0 ± 3.9 mmHg, N = 49, p ± 0.001), latanoprost (21.2 ± 3.8 to 18.3 ± 2.5 mmHg, N = 54, p ± 0.001), and a dorzolamide/timolol fixed combination (20.9 ± 2.4 to 20.0 ± 2.7 mmHg, N= 32, p ± 0.03). In switching from a latanoprost and timolol unfixed combination, the pressure changed from 18.3 ± 3.8 to 18.9v 3.0 mmHg (N = 33, p ± 0.38). LTFC was persistent in 131 (78%) patients within the 3-month treatment period. The most common adverse event with LTFC was conjunctival hyperemia, which occurred in patients not previously treated with latanoprost therapy (N = 16, 10%). Conclusions: LTFC generally provides reduced IOP and limited side effects when substituted for other common glaucoma therapies, while providing similar pressure when switched from its own individual components. Purpose: To determine the efficacy of brimonidine tartrate 0.2% drops given 2 times a day in reducing intraocular pressure (IOP) spikes during the first 24 hours after phacoemulsification cataract surgery.Setting: Department of Ophthalmology, General Hospital of Patras Agios Andreas, Patras, Greece.Methods: In this prospective double-blind placebo-controlled study, 1 eye of 40 consecutive normotensive cataract patients having small-incision cataract surgery was randomized into 1 of 2 treatment arms. Twenty patients received a placebo (artificial tears) and 20 patients received brimonidine tartrate 0.2% drops 2 times a day the day before and the day of surgery. Diurnal IOP variation was the primary efficacy variable; IOP was measured at baseline, before surgery, and 4, 6, 12, and 24 hours postoperatively.Results: The placebo group had higher IOPs at every time point after surgery. Peak elevation of IOP occurred 6 hours after surgery. The mean IOP in the placebo group (27.71 mm Hg ± 3.75 [SD]) was statistically significantly higher than in the brimonidine group (21.45 ± 1.32 mm Hg) (P<.001). A major IOP rise (≥20 mm Hg above baseline IOP) occurred in 1 patient (5%) in the placebo group who required emergency hypotensive therapy. Twenty-four hours after surgery, 11 eyes (55%) in the brimonidine group and 4 eyes (20%) in the placebo group had an IOP lower than baseline.Conclusion: Prophylactic treatment with brimonidine tartrate 0.2% 2 times a day for 2 days was effective in reducing IOP peaks throughout the first 24 hours after phacoemulsification surgery. The present study aimed to investigate the prevalence of glaucomatous optic neuropathy in patients with obstructive sleep apnea/hypopnea syndrome (OSAHS). In total, 83 subjects (45 cases of severe OSAHS and 38 controls) underwent polysomnographic assessment and were evaluated for the severity of the disease using the Apnea-Hypopnea index. A detailed ophthalmologic exam was then performed, including measurement of the intraocular pressure (IOP) with a Goldmann applanation tonometer and Pascal dynamic contour tonometer (DCT), recording of the ocular pulse amplitude measured by the Pascal DCT, standard automated perimetry and retinal nerve fiber layer (RNFL) and ganglion cell complex (GCC) thickness evaluation using optical coherence tomography. Ocular blood flow was assessed using color Doppler imaging (CDI) and ophthalmic artery indices were evaluated, including peak systolic blood velocity, end diastolic blood velocity and resistivity index (RI). There was a significant difference in the mean IOP between controls (11.03±3.85 mmHg) and cases of severe OSAHS (18.06±3.39 mmHg) when the IOP was measured by DCT (P<0.0001), but not with the Goldmann applanation tonometer (IOP, 13.97±2.85 mmHg for controls and 14.89±3.21 mmHg for cases of severe OSAHS; P=0.0877). Significant negative correlations were observed between RNFL thickness and RI (P=0.0011) in cases of severe OSAHS, as well as between GCC thickness and RI (P<0.008) in all subjects. Furthermore, a negative correlation between RI and RNFL thickness in severe cases of OSAHS suggested a hemodynamically induced vulnerability of RNFL in OSAHS. The correlation between RI and GCC thickness in all subjects suggested that impaired perfusion, more prominent in OSAHS, leads to structural changes. Therefore, cases of severe OSAHS should be monitored for changes in RNFL and GCC thickness, as well as CDI findings. Background: The purpose of this study is to examine the repeatability of Pentacam-derived anterior chamber volume measurements, and assess the level of agreement of anterior chamber volume measurements obtained with the Visante and the Pentacam imaging devices. Methods: Anterior chamber volume was calculated with the Pentacam device and the Visante using the Wang formula. Results were analysed by means of the Wilcoxon signed rank test, Bland-Altman analysis, Cronbach's alpha test and intraclass correlation coefficient. P-values <0.01 were considered statistically significant. Results: One randomly selected eye from each of 20 persons was analysed. Spherical equivalent ranged between −2.50 and +1.50 D, and their age ranged from 25 to 79 years (mean ± SD: 54.4 ± 19.2 years). Mean anterior chamber volume measured with Pentacam was 171.1 ± 39.6 µL (minimum: 103.0 µL, maximum: 236.0 µL), and mean anterior chamber volume measured by Visante was 171.4 ± 42.4 µL (minimum: 96.4 µL, maximum: 245.5 µL). There was no statistically significant difference between Pentacam and Visante measurements of the anterior chamber volume ( P = 0.691, Wilcoxon signed rank test). Cronbach's alpha and intraclass correlation coefficient for Pentacam were 0.998 and 0.991, respectively. Conclusions: Anterior chamber volume measurements obtained with the Visante using the Wang formula were in good agreement with the corresponding ones obtained with Pentacam. To describe a case of conjunctival hyperaemia and proptosis of the right eye.Three-dimensional computed tomography (CT) was performed to reveal the size and position of a lesion of osseous density extending into the right orbit. The lesion was then excised using a right paranasal transcutaneous approach.The pathology report suggested ethmoidal osteoma. The postoperative course was uncomplicated and the lesion did not recur during a 5-year follow-up period. Repeated postoperative ophthalmic examinations revealed preservation of visual acuity and visual fields postoperatively.Diagnosis is based on imaging studies, especially CT and three-dimensional CT scans. A three-dimensional CT scan is critical in understanding the actual dimensions of the tumour and its relations with other structures. Surgical removal is indicated in cases with orbital matrix compression and displacement. A transcutaneous paranasal approach allows for increased exposure of affected structures. Introduction: Infectious keratitis following penetrating keratoplasty is a common postoperative complication. Intensive topical and systemic treatments do not always prevent the risk of graft failure. In this report we demonstrate the beneficial anti-microbial effect of corneal collagen cross-linking in a late-onset, sight-threatening, corneal graft ulcer.Case Presentation: A 57-year old Caucasian man underwent penetrating keratoplasty in his left eye, due to corneal bullosa after cataract extraction surgery. Twelve months after the penetrating keratoplasty, he visited our emergency service complaining of burning and foreign body sensation in his operated eye. Slit-lamp examination revealed a central, round-shaped ulcer of the graft. Due to poor response to the intensive topical antibiotic therapy, corneal collagen cross-linking was applied 3 days after admission, in an attempt to control the infection. Cultures indicated the predominance of methicillin-resistant Staphylococcus aureus infection. Five days after corneal collagen cross-linking treatment, the epithelium was completely re-epithelized, while the transparency of the transplanted cornea was gradually restored within the 12-month follow-up period. No relapses occurred.Conclusion: Corneal collagen cross-linking seems to be a safe and effective therapeutic alternative in resistant cases of infectious keratitis following penetrating keratoplasty.
