Differentiation of the psychiatric diagnosis in the clinical praxis sometimes could be difficult; particulary in cases of kindred/similar diagnoses like schizophrenia, schizoaffective disorder or personality disorder cluster A (or cluster I) - paranoid, schizoid and schizotipal. Relationship between schizophrenia and personality disorder cluster I is not quite clear; according some authors, descendants of schizophrenic patients could suffer from personality disorder cluster I; which suggest on possible genetic correlation between those diagnoses. As a case report we going to present a female single patient, 32 years of age, no kids, living with her mother, disapointed in herself after unsucessfull second year on faculty of low which she left. After that, she closed up, she also had problems with nutrition, showing signs of anorexic and bulimic behavior. After 10–12 years of living in social retraction, not working, without social contacts, she „decided to start from the beginning” after being on intensive vitamine therapy, and asked for the psychiatric therapy for the first time in her life, with motivation for working. In conclusion, in this study we are going to present a critical view of both diagnosis traying to understand similarity and differences, as well as identify common points between them and make guidelines for practical approach to the question. Creativity is a divine madness, a gift from the gods (Plato)…No great genius was without a mixture of insanity (Aristotle/Seneca)…The only difference between me and a madman is that I am not mad (Slvador Dali)…Imagination is more important than knowledge (Albert Einstein)… Creativity is a complex multidimensional concept still no defined and no cleared until the end. It is a capability of creating original ideas or products of wide social use; a mental function apears independent of complex functions grouped in the concept of intelligence even though in close correlations with them. Many genious people through history, philosophers, artists, scientists suffered from some psychiatric disease, mostly bipolar affective disease, shizotipal personality disorder, psychoses, dependencies, suicidal behavior, as well as psychiatric comorbidity. Recently, there are many theories explaining the connect points between creativity and psychopathology such as personality as a bridge betwenn two entities, cultural and historical roots, theory of chaos, intuition, spirityality, perception of normal and deviant etc. In this study we are going to present review of literature on this issue. In conclusion, recent attitude on this issue is that creativity (geniusness) and psuchopathology have the same roots in the common source. Introduction: While in eating disorders such as anorexia nervosa, the comorbidity of pathological personality traits is very common, and accordingly the personality traits of borderline personality disorder is considered very frequent and coexisting. The treatment of anorexia nervosa is based primarily on the psychotherapy and work on pathological personality traits even more than the treatment of the syndrome and the consequences of malnutrition itself. That achieves a longer-term and more reliable solution than symptomatic treatment of anorexia nervosa which usually does not bring satisfactory results. On the other hand, in patients with obesity, pathological personality traits, especially those of borderline personality disorder, are still very rarely associated, since obesity is usually not even considered a disorder, but a variation in the population. Objectives: The aim of this paper is to investigate the pathological personality traits of borderline personality disorder in people with obesity. Methods: Investigating relevant scientific and professional literature from the field of personality pathology and eating disorders. Results: When obesity is related to impulse control disorder in the sense of emotional eating under increased stress according to today's relevant literature, it can definitely be related to personality traits of borderline personality disorder, i.e. the presence of elements of borderline personality organization and prementalization models. Such an inability to deal with negative emotions such as increased anxiety or rejection sensitivity, which results in overeating and the related feeling of shame that overwhelms the person, regardless of whether he/she/they has any of the certain forms of compulsive behaviour afterwards, can be related to impulsive behaviour and the "all or nothing" way of thoughts. This is also confirmed by cases when certain people have a history of both one and the other disorder. Thus, some people have, for example, malnutrition in adolescence as part of anorexia nervosa, only to have problems with obesity after some time with a healthy body mass. Conclusions: Since pathological personality traits in people with anorexia nervosa and obesity give indications of common characteristics in the form of borderline personality disorder traits, i.e. borderline personality organization and prementalization models in both disorders, future research will certainly shed light on the connection between these eating disorders. Anorexia nervosa is a psychiatric disorder with a prevalence of circa five percent in young women population, with an increased tendency of frequent occurrence over the past twenty years. Although the diagnosis is relatively simple and straightforward, the treatment of these patients is often time-consuming and challenging for clinicians, as the occurrence of malignant and persistent forms of the condition is becoming ever more common. Resistant clinical diagnoses usually arise through comorbidity with other psychiatric disorders. Counted among them, first of all, are personality disorders. A.N. is treated by means of using various groups of psychopharmacological drugs in combination with psychotherapeutic treatment, with intention to establish a favorable therapeutic relationship and stability of the patient. Psychotherapeutic part of the treatment gains in importance when the condition occurs in comorbidity with personality disorders. Supportive treatment measures, namely aggressive rehydration, caloric values compensation via probes and and catheters are often an integral part of the treatment, and become particularly important when faced with treating malignant forms of the condition. Presented case will show that the malignant forms of Anorexia Nervosa can only be understood through insight into the psychological foundations of personality and frequently accompanying disorders on Axis II (DSM-IV). The female patient in question displays a base borderline personality structure with dominant narcissistic tendencies which prompted the development of a malignant form of the condition and thus made the treatment significantly more difficult. Only early intervention with emphasis on the psychotherapy treatment can ensure a better long-term prognosis in such cases. [Copyright &y& Elsevier]

Introduction: Obesity is one of the leading problems of today's society. According to WHO, 650 million people worldwide are obese, which is 13% of total population (in Croatia 21.5%). There are various psychodynamic theories that interpret the psychological aspects of obesity. Objectives: The aim of this paper is to present psychodynamic and contemporary psychiatric concepts that explain the interrelated phenomena presenting in obese patients. Methods: The review of the literature included the investigation of the existing studies in the field of modern psychiatry, as well as previous knowledge in the field of psychodynamics. Results: Obesity is associated with the emptiness of not recognizing one's own emotions from hunger, and the need for constant replacement. The everyday life of the obese is filled with shame, an uncomfortable perception that is so intense that can be unbearable. The emptiness and shame which overwhelm and create discomfort cannot be fulfilled by constant food intake and are associated with pathological narcissism (grandiose or vulnerable), which in turn is associated with more regressive behaviour. Thus, obesity may sometimes be associated with addictive behaviours, and a cognition that a bad pattern of rewarding behaviour through food has been adopted in parallel with poor self-control. Conclusions: Relationship between psychodynamic phenomena and obesity is complex and multidimensional. Further research is needed in order to ameliorate our understanding of these connections.
