The integration of a composite of porous medium with phase change material (PCM) offers significant advantages in thermal management systems, enhancing heat transfer efficiency and addressing various thermal regulation challenges. This approach utilizes the PCM's latent heat absorption and the enhanced thermal conductivity provided by the porous medium, resulting in optimized system performance. Its applicability spans across electronics cooling and building insulation systems. However, predicting the thermal behavior of this composite material is challenging, necessitating computational tools to anticipate its response under different conditions and evaluate its influence on cooling strategies. The objective of this study is to create a computational tool specifically tailored to evaluate constitutive parameters of this composite material, thereby providing a comprehensive description of its thermal behavior. To achieve this goal, the multiscale homogenization principle is employed to assess the composite's effective thermophysical material properties using the representative volume element approach. The repeating unit cell of the aluminum lattice is incorporated into the PCM to define a representative volume element. The finite element method (FEM) is utilized to solve the three-dimensional homogenization problem, yielding an orthotropic effective thermal conductivity due to the inherent symmetry of the repeating material cell. Moreover, the study leverages the apparent heat capacity method to effectively manage the phase transitions within the PCM domain, utilizing smooth and temperature-dependent functions to accurately describe the thermophysical properties of the PCM. Integrating the composite into battery pack thermal management, this study thoroughly examines thermal dynamics by comparing outcomes with and without PCM integration. The transient thermal problem is accurately tackled using the FEM, employing the evaluated effective constitutive parameters of the homogenized composite to minimize computational effort. The results indicate a notable decline in the highest temperatures of the battery pack, leading to a reduction of about 14 °C at the specific moment when the phase change material fully transitions into its liquid form. The obtained results emphasize the effectiveness and practical feasibility of the proposed thermal management strategy. The modeling approach presented provides a robust tool with significant efficiency in reducing computational time for analyzing the thermal behavior of large models, as the utilization of the homogenization technique notably decreases the computational time. Background: Cerebrovascular diseases (CVAs) have several risk factors that are categorized as modifiable and nonmodifiable. Periodontal diseases (PD) have a modifiable role in causing CVA, where several studies suggested direct or indirect correlations with systemic diseases. This study aims to summarize, evaluate and analyze all the evidence available in literature, to reach a better understanding of the relation between periodontitis, gingivitis, tooth loss and CVA. Methods: PubMed, Cochrane, Scopus and Web of Science databases were searched for all meta-analyses assessing the effect of PD on CVA in accordance with Joanna Briggs Institute guidance for umbrella reviews in March 2024. Assessment of Multiple Systematic Reviews (AMSTAR) was used for quality assessment. Pooled analysis was performed to assess the effect of periodontitis, gingivitis and tooth loss on CVA, depending on the availability of data using Review Manager Version 5.2.11. Results: Seven of the identified meta-analyses were of high quality, and they were distributed in different countries. Periodontitis was generally associated with a significant increase in CVA risk (OR = 2.32, 95% CI: 1.70, 3.17, p < 0.00001 and RR = 1.22, 95% CI: 1.15–1.29, p < 0.00001), same as tooth loss, but to a lower degree (0.78). However, the effect of gingivitis was insignificant in terms of CVA (RR = 1.32, 95% CI: 1.12–1.56, p = 0.0008). Conclusions: This study confirms the effect of periodontitis and tooth loss on CVA, disregarding any significant role for gingivitis. Introduction: The effect of antiplatelet and anticoagulant medications on the outcomes of sinus floor augmentation remains unclear. Methods: This retrospective cohort study analyzed data from electronic medical records of consecutive patients undergoing sinus floor augmentation at a single medical center. Patients were categorized into three categories: patients under antiplatelet medications, patients under anticoagulation medications, and healthy individuals. Data collected included tobacco smoking, residual alveolar bone height, timing of implant placement, materials used, vertical bone gain, early implant failure (EIF), and complications such as Schneiderian membrane perforation and postoperative bleeding. Multivariable analysis was performed to assess risk factors for EIF. Statistical significance was considered below 5%. Results: Among 110 patients with 305 implants, EIF occurred in 10% of patients and 4.65% of implants. No significant difference in postoperative bleeding or EIF was found between study groups. Univariate and multivariable analyses highlighted tobacco smoking (odds ratio [OR] = 7.92), lower residual alveolar ridge height (OR = 0.81), and staged implant placement (OR = 4.64) as significant EIF risk factors in this cohort. Conclusions: Anticoagulant and antiplatelet therapies do not significantly elevate the risk of EIF or postoperative bleeding following sinus floor augmentation. Tobacco smoking, residual alveolar ridge height and staged sinus floor augmentation were risk factors for EIF in patients using antiplatelet or anticoagulation medications undergoing sinus floor augmentation. The gender gap in physics in Australian secondary schools remains a significant issue. This study aims to address this problem by examining how physics teachers perceive girls' participation in physics, and how these perceptions influence their approach to gender issues and their potential impact on girls' physics identity. The study uses a multi-case qualitative research design and applies a physics identity framework from a gender perspective. Data was collected from seven physics teachers from three Australian states. The findings indicate a connection between teachers' perceptions of the gender gap in physics and their prioritisation of addressing this issue in their teaching. The data reveals that teachers with feminist beliefs actively tackle gender issues and challenge norms that hinder girls' participation. On the other hand, teachers with beliefs aligned with mainstream gender roles tend to maintain the status quo without making evident pedagogical adjustments to specifically support girls' physics identity. The findings emphasise the necessity for ongoing professional development for physics teachers, enabling them to counter their unconscious gender biases and promote physics to girls. Wear parameters identified by linear friction tester (LFT) experiments overestimate the wear mass loss when applied on a laboratory abrasion & skid tester 100 (LAT100) simulation. On the other hand, the identification of wear parameters directly from the LAT100 experiment can be challenging since variables such as the contact area and the slip velocity are not measured experimentally and need to be assumed. To improve the identification process, numerical calibration is used as a more suitable approach. This approach relies on meta-models as a target function for minimization. The meta-models are generated using a sample of wear parameters applied to an LAT100 finite element modeling (FEM) to calculate the corresponding wear mass.. In this model, a transport velocity is defined for the rolling simulation, and an arbitrary lagrangian–eulerian (ALE) adaptive meshing approach is adopted for the wear modeling. For the wear model, a combination of archard's wear model and schallamach's abrasion law is used. This model contains a pair of wear parameters to be identified. The ALE adaptive meshing technique moves the nodes independently of the material. Since the mesh topology remains the same, failure of the simulation occurs if the wear volume loss exceeds that of the element. Meta-models are created to extend wear modeling beyond this failure. Once the meta-models are created, they are used as a target function for the minimization algorithm. The minimization algorithm aims to find the optimal wear parameters by minimizing the difference between experimentally observed and numerically produced wear mass loss. The minimization algorithm inputs a set of wear parameters into the meta-models which in turn yield a prediction of the wear mass loss. The process is carried out until an optimum parameter set is identified. Such an approach has a lower accuracy if the parameters are identified directly from the experiment using assumptions regarding the contact shear stress and the sliding velocity. Chronic wounds present a substantial healthcare obstacle, marked by an extended healing period that can persist for weeks, months, or even years. Typically, they do not progress through the usual phases of healing, which include hemostasis, inflammation, proliferation, and remodeling, within the expected timeframe. Therefore, to address the socioeconomic burden in taking care of chronic wounds, hydrogel-based therapeutic materials have been proposed. Hydrogels are hydrophilic polymer networks with a 3D structure which allows them to become skin substitutes for chronic wounds. Knowing that peptides are abundant in the human body and possess distinct biological functionality, activity, and selectivity, their adaptability as peptide-based hydrogels to individual therapeutic requirements has made them a significant potential biomaterial for the treatment of chronic wounds. Peptide-based hydrogels possess excellent physicochemical and mechanical characteristics such as biodegradability and swelling, and suitable rheological properties as well great biocompatibility. Moreover, they interact with cells, promoting adhesion, migration, and proliferation. The article looks at civil society as an entity that often reflects a country's contextual realities. More specifically, it focuses on socio-political structures, showing how weak institutions, consociational principles, as well as formal and informal elite dynamics in Lebanon have created a perfect atmosphere for sectarian leaders to expand their clientelistic networks into civil society. By creating their own service-providing sectarian organizations; indirectly pushing civic organizations to become players in the system; and infiltrating interest groups that may pose a threat, sectarian leaders have been able to neutralize the impact of civil society in the country. Background: Obesity is a significant health concern in Lebanon, contributing to various chronic diseases. Uncoupling Protein 2 (UCP2) plays a critical role in regulating metabolism, and genetic polymorphisms in the UCP2 gene, such as the 45-bp insertion (I)/deletion (D) polymorphism, may influence susceptibility to obesity. This study aims to assess the association between the UCP2 45-bp I/D polymorphism and obesity in a sample of the North Lebanese population. DNA was extracted from peripheral blood of 123 subjects that were categorized into "non-obese" and "obese" groups, with further stratified analysis based on age and physical activity levels. UCP2 45-bp I/D variants were screened using polymerase chain reaction (PCR). Results: The genotype frequencies of the UCP2 45-bp I/D polymorphism were as follows: D/D (46.3%), I/D (46.3%), and I/I (7.4%), with a minor allele frequency of 0.30. Statistical analyses, including Chi-squared tests and logistic regression, were conducted to explore the association between the UCP2 polymorphism and obesity. After adjusting for body mass index confounders and stratifying by physical activity and age, no significant differences were found in genotypic distributions: Stratum 1 (S1: irregular exercise, age < 40, P = 0.157), Stratum 2 (S2: irregular exercise, age ≥ 40, P = 0.192), and Stratum 3 (S3: regular exercise, age < 40, P = 0.160). Additionally, there were no significant differences in allelic distributions across these strata (S1: P = 0.514; S2: P = 0.869; S3: P = 0.681). Gender was also not associated with the UCP2 45-bp I/D polymorphism. Conclusions: This study found no significant association between the UCP2 45-bp I/D polymorphism and obesity in the North Lebanese population, suggesting that this genetic variation may not play a major role in obesity risk. Quality is receiving increasing attention and application, especially in the healthcare sector. This focus has encouraged leaders and boards of organizations to cultivate and promote a culture of quality. Given that the primary focus of healthcare organizations is on patients, their safety, the well-being of employees, and patient satisfaction, this culture must be centered on continuous improvement and the adoption of the latest methodologies. Therefore, the study topic on the impact of quality culture, through its three dimensions, on continuous improvement was carefully selected. Adopting a descriptive-analytical approach, the research focused on healthcare workers in North Lebanon. The study gathered both qualitative and quantitative data. A closed questionnaire, based on a five-point Likert scale, was distributed to a random sample of 300 employees from private hospitals across diverse roles. The collected data were subsequently analyzed using Microsoft Excel and SPSS software (v.25). The study’s findings indicate a significant and robust relationship between each aspect of quality culture and the process of continuous improvement. This highlights to healthcare organizations the necessity of maintaining continuous improvement as an integral part of the organizational culture among employees at all levels. In current toxicological research, 2D cell cultures and animal models are well- accepted and commonly employed methods. However, these approaches have many drawbacks and are distant from the actual environment in human. To embrace this, great efforts have been made to provide alternative methods for non-animal skin models in toxicology studies with the need for more mechanistically informative methods. This review focuses on the current state of knowledge regarding the in vitro 3D skin model methods, with different functional states that correspond to the sustainability in the field of toxicology testing. We discuss existing toxicology testing methods using in vitro 3D skin models which provide a better understanding of the testing requirements that are needed. The challenges and future landscape in using the in vitro 3D skin models in toxicology testing are also discussed. Background/objectives: Gestational Weight Gain (GWG) impacts maternal and fetal health; deviations from optimal ranges pose health risks. Maternal lifestyle before and during pregnancy strongly influences GWG. This study explores factors linked to inadequate GWG, focusing on Mediterranean Diet (MD) adherence and specific food consumption. Subjects/methods: 178 pregnant women were enrolled at Fondazione IRCCS Policlinico San Matteo (Pavia) during pre-hospital care before birth meeting inclusion/exclusion criteria. Sociodemographic data, pre-pregnancy BMI, GWG, MD adherence, physical activity (PA) levels, and smoking habits were retrospectively collected. Validated questionnaires adapted for the target group, assessed MD adherence and PA level. Participants were classified into adequate (AGWG) and inadequate GWG groups following IOM guidelines. Results: Among 200 pregnant women (aged 30–36), 37.1% experienced low GWG and 24.1% excessive GWG. Our study revealed a significant association between inadequate GWG and educational level (P = 0.011); pre-pregnancy BMI (P = 0.005); MD adherence (P = 0.008), and daily average consumption of vegetables (P < 0.001). Our results also showed that a lower risk of EGWG vs. AGWG was associated with daily average consumption of vegetables (RRR = 0.279, P = 0.004), while a higher risk of EGWG vs. AGWG was associated with high daily meat product consumption (> 1.5 portions/day) (RRR = 7.83, P = 0.03). Conclusion. These findings emphasize the importance of promoting lifestyle changes before and during pregnancy to tackle the increasing incidence of inadequate GWG and improve the health outcomes of both mother and child. An important hurdle in medical diagnostics is the high-quality and interpretable classification of audio signals. In this study, we present an image-based representation of infant crying audio files to predict abnormal infant cries using a vision transformer and also show significant improvements in the performance and interpretability of this computer-aided tool. The use of advanced feature extraction techniques such as Gammatone Frequency Cepstral Coefficients (GFCCs) resulted in a classification accuracy of 96.33%. For other features (spectrogram and mel-spectrogram), the performance was very similar, with an accuracy of 93.17% for the spectrogram and 94.83% accuracy for the mel-spectrogram. We used our vision transformer (ViT) model, which is less complex but more effective than the proposed audio spectrogram transformer (AST). We incorporated explainable AI (XAI) techniques such as Layer-wise Relevance Propagation (LRP), Local Interpretable Model-agnostic Explanations (LIME), and attention mechanisms to ensure transparency and reliability in decision-making, which helped us understand the why of model predictions. The accuracy of detection was higher than previously reported and the results were easy to interpret, demonstrating that this work can potentially serve as a new benchmark for audio classification tasks, especially in medical diagnostics, and providing better prospects for an imminent future of trustworthy AI-based healthcare solutions. Aficamten is a cardiac myosin inhibitor that mitigates left ventricular outflow gradients in obstructive hypertrophic cardiomyopathy (oHCM). The clinical efficacy of aficamten across multiple outcome domains in oHCM has not been fully defined. This responder analysis from the SEQUOIA-HCM (Phase 3 Trial to Evaluate the Efficacy and Safety of Aficamten Compared to Placebo in Adults With Symptomatic oHCM) trial characterizes the clinical impact of aficamten. Patients who were symptomatic of oHCM were randomized to aficamten (n = 142) or placebo (n = 140) daily for 24 weeks. Outcomes assessed included the proportion of patients with complete hemodynamic response (rest and Valsalva gradient <30 mm Hg and <50 mm Hg, respectively), relief in limiting symptoms (≥1 improvement in NYHA functional class and/or ≥10-point change in Kansas City Cardiomyopathy Questionnaire–Clinical Summary Score), enhanced exercise capacity (≥1.5 mL/kg/min change in peak oxygen uptake), and ≥50% reduction in N-terminal pro–B-type natriuretic peptide. Eligibility for septal reduction therapy was also evaluated. At 24 weeks, patients treated with aficamten vs placebo showed significant improvement in limiting symptoms (71% vs 42%), were more likely to have complete hemodynamic response (68% vs 7%), demonstrated enhanced exercise capacity (47% vs 24%), and showed a decrease ≥50% in N-terminal pro–B-type natriuretic peptide (84% vs 8%) (P ≤ 0.002 for all). An improvement in ≥1 of these outcome measures was achieved in 97% of patients treated with aficamten (vs 59% placebo), including 23% on aficamten who achieved all 4 outcomes compared with none in placebo. Among 32 patients receiving aficamten and 29 patients receiving placebo who were eligible for septal reduction therapy, 28 (88%) from the aficamten group were no longer eligible at 24 weeks compared with 15 (52%) from the placebo group (P = 0.002). Treatment with aficamten was associated with substantial improvements across a broad range of clinically relevant efficacy measures. Standard-of-care (SoC) medications for the treatment of obstructive hypertrophic cardiomyopathy (oHCM) are recommended as first-line therapy despite the lack of evidence from controlled clinical trials and well known off-target side effects. We describe the impact of SoC therapy downtitration and withdrawal in patients already receiving aficamten in FOREST-HCM (Follow-Up, Open-Label, Research Evaluation of Sustained Treatment with Aficamten in Hypertrophic Cardiomyopathy; NCT04848506). Patients receiving SoC therapy (beta-blocker, nondihydropyridine calcium-channel blocker, and/or disopyramide) were eligible for protocol-guided SoC downtitration and withdrawal at the discretion of the investigator and after achieving a stable dose of aficamten for ≥4 weeks. Successful SoC withdrawal was defined as at least a 50% dose-reduction in ≥1 medication. Adverse events (AEs) were prospectively evaluated 1 to 2 weeks after any SoC withdrawal. Of 145 patients with oHCM who were followed for at least 24 weeks (mean age 60.5 ± 13.2 years; 44.8% female; 42% NYHA functional class III), 136 (93.8%) were receiving ≥1 SoC therapy; of those, 64 (47%) had an attempt at withdrawal, with 59 (92.2%) successful. Thirty-eight (64.4%) patients completely discontinued ≥1 medication, and 27 (45.8%) achieved aficamten monotherapy with 2 later restarting a SoC medication. There were no significant differences in baseline characteristics on day 1 in FOREST-HCM in those with a SoC-withdrawal vs no-withdrawal attempt. In patients who underwent successful SoC therapy withdrawal, NYHA functional class improved by ≥1 class in 79.2% from baseline, Kansas City Cardiomyopathy Questionnaire–Clinical Summary Score improved to 83.0 ± 15.8 points, and resting and Valsalva left ventricular outflow tract gradient improved to 14.3 ± 10.9 and 32.9 ± 21.4 mm Hg, respectively. N-terminal pro–B-type natriuretic peptide levels improved to a median of 220.0 pg/mL (Q1-Q3: 102.0-554.0.0 pg/mL) and high-sensitivity troponin I improved to a median of 6.0 ng/L (Q1-Q3:3.5-10.7 ng/L). Downtitration and withdrawal of SoC therapy did not impact these results (all P values for change were >0.05), and these changes were similar in patients who did not undergo SoC therapy withdrawal. There were no serious AEs attributed to SoC withdrawal and treatment emergent AEs were similar between groups. In FOREST-HCM, one-half of the patients with oHCM attempted downtitration and withdrawal of SoC medications while receiving aficamten treatment, with infrequent instances of resumption of SoC. Obstructive hypertrophic cardiomyopathy (oHCM) is characterized by left ventricular (LV) hypertrophy, LV outflow tract obstruction, and left atrial dilation, which can be associated with progressive heart failure, atrial fibrillation, and stroke. Aficamten is a next-in-class cardiac myosin inhibitor that reduces outflow tract obstruction by modulating cardiac contractility, with the potential to reverse pathological remodeling and, in turn, reduce cardiovascular events. This study sought to investigate the effect of aficamten on cardiac remodeling compared with placebo using cardiovascular magnetic resonance (CMR) and its association with key clinical endpoints in the SEQUOIA-HCM (Safety, Efficacy, and Quantitative Understanding of Obstruction Impact of Aficamten in HCM) CMR substudy. SEQUOIA-HCM was a phase 3 double-blind, placebo-controlled trial for adults with symptomatic oHCM who were randomized 1:1 to 24 weeks of aficamten (dose range: 5-20 mg) or placebo. Eligible participants were offered enrollment in the CMR substudy with studies performed at baseline and week 24. Image analysis was performed in a blinded fashion by a core laboratory. Of the 282 randomized patients, 57 (20%) participated in the substudy, and of those, 50 (88%) completed both baseline and week 24 CMR. Baseline characteristics of the CMR cohort were similar to the overall study population. Of these 50 patients, 21 received aficamten and 29 received placebo. Relative to placebo, patients receiving aficamten demonstrated significant reductions (Δ least-squares mean) in LV mass index (−15 g/m2; 95% CI: −25 to −6 g/m2; P = 0.001), maximal LV wall thickness (−2.1 mm; 95% CI: −3.1 to −1.1 mm; P < 0.001), left atrial volume index (−13 mL/m2; 95% CI: −19 to −7 mL/m2; P < 0.001), native T1 relaxation time (−37 ms; 95% CI: −69 to −5 ms; P = 0.026), indexed extracellular volume fraction (−3.9 g/m2; 95% CI: −7.0 to −0.9 g/m2; P = 0.014), and indexed myocyte mass (−14 g/m2; 95% CI: −23 to −4 g/m2; P = 0.004), while there were no significant changes in LV chamber volumes, LV replacement fibrosis (late gadolinium enhancement mass −0.7 g; 95% CI: −2.9 to 1.6 g; P = 0.54), or extracellular volume (0.7%; 95% CI: −2.2% to 3.6%; P = 0.61). The CMR substudy of SEQUOIA-HCM demonstrated that treatment with aficamten relative to placebo for 24 weeks resulted in favorable cardiac remodeling. Introduction: Several studies investigated the implication of the gut microbiota (GM) in the formation and progression of aneurysms, suggesting a key role for GM metabolites in aneurysmal pathogenesis and prognosis. Aim: This systematic review aims to collect key findings concerning the impact of gut bacterial compositions, GM-related metabolites, probiotics administration, and inflammatory markers in aneurysmal development and rupture. Methods: A PubMed, Medline, Embase, and Web of Science database search was conducted in accordance with PRISMA guidelines for systematic reviews, targeting all studies assessing the GM's role in aneurysms till 2023. Results: Data from 19 out of 292 non-duplicated studies were included. Based on the published literature, aneurysmal incidents in several locations were accompanied by an alteration in specific intestinal bacteria that may affect the prognosis of the aneurysm. The gut dysbiosis was also accompanied by modifications in the metabolic pathways. Hence, the administration of specific probiotics showed a significant implication in reversing the GM-related changes that were affecting the aneurysm, leading to a decrease in its severity, a better prognosis, and even serving as a prophylactic approach. Conclusions: The outcomes of this review highlight the role of GM in the pathogenesis of aneurysms, assessing some involved mechanistic pathways such as gut dysbiosis, inflammation, and the alteration of gut-derived metabolites levels, which orient new research on developing therapeutic strategies. Neutron activated concrete reference material for measurement of residual radioactivity by gamma-ray spectrometry has been prepared. The main radionuclides were 60Co, 152Eu, 54Mn, 154Eu and 134Cs. One-way ANOVA homogeneity test has shown that the samples are homogeneous. The activity concentrations of 54Mn, 59Fe, 154Eu and 134Cs were relatively low; only 60Co activity concentration was found to be higher therefor it was considered as the main target isotope. Maize is widely growth in arid and semi-arid region where, drought is common and a limiting factor for crop production. Potassium plays a key role in enhancing plant growth under drought condition. The objective of this study is to determine the effect of K fertilization with and without NP on maize growth grown in sandy loam soil under adequate and deficit irrigation conditions. The following treatments were investigated in pot experiment: (1) control with no fertilizer application (C); (2) 128 kg N + 328 kg P2O5 ha-1 (NPK0); (3) 128 kg N + 328 kg P2O5 ha-1 + 152.5 kg K2O ha1 (NPK1); (4) 128 kg N + 328 kg P2O5 ha-1 + 305 kg K2O ha-1 (NPK2); and 128 kg N + 328 kg P2O5 ha-1 + 457.5 kg K2O ha-1 (NPK3). Treatments were investigated under adequate and deficit soil moisture content. Each pot filled with 3.5 kg air-dry soil and seeded with maize and pots were watered according to the treatments. The results indicated that plant growth and nutrient uptake were significantly reduced under water stress condition. The application of NP increased plant growth and nutrient uptake and further were increased with K application. K application also enhanced plant tolerance to deficit soil moisture condition. In addition, K enhanced nutrient uptake and leaf chlorophyll content. Based on the results, it can be concluded that application of NP for maize was not adequate to achieve the highest plant growth, unless it is combined with K application. Since Western technology platforms began colonizing online spaces, they have neglected Majority World populations, including those in MENA/SWANA countries like Lebanon. This neglect exacerbates harassment and violence against marginalized individuals, reinforcing systemic harms in Lebanon's cisheteropatriarchal and sectarian society. Our study draws on 14 in-depth interviews with women and queer journalists and activists in Lebanon who have faced online harassment and violence. Utilizing Sarah Ahmed's feminist notion of complaint, we explore the flaws in platforms' reporting mechanisms and document what we term 'techno-alienation': a state where those marginalized by these platforms feel abandoned and unable to envision an inclusive digital future. Starting from full-dimensional models of solute transport, we derive and analyze multi-dimensional models of time-dependent convection, diffusion, and exchange in and around pulsating vascular and perivascular networks. These models are widely applicable for modelling transport in vascularized tissue, brain perivascular spaces, vascular plants and similar environments. We show the existence and uniqueness of solutions to both the full- and the multi-dimensional equations under suitable assumptions on the domain velocity. Moreover, we quantify the associated modelling errors by establishing a-priori estimates in evolving Bochner spaces. In particular, we show that the modelling error decreases with the characteristic vessel diameter and thus vanishes for infinitely slender vessels. Aims: The aim of this study was to report safety and efficacy of aficamten in patients with non‐obstructive hypertrophic cardiomyopathy (nHCM) over 36 weeks in the ongoing FOREST‐HCM trial. Methods and results: Patients were started on aficamten 5 mg daily, with doses adjusted in 5‐mg increments (5–20 mg) at ≥2‐week intervals according to site‐read left ventricular ejection fraction (LVEF). Aficamten dose was increased if LVEF ≥55%, maintained if LVEF 50–54%, decreased if LVEF 40–<50%, and temporarily interrupted if LVEF <40%. Safety and efficacy were assessed over 36 weeks. Overall, 34 patients were enrolled (mean age 57.2 ± 15.3 years, 62% female, 41% in New York Heart Association [NYHA] class III). Over 36 weeks, 82.3% achieved 15–20 mg daily dose and there was a modest reduction in LVEF by −4.3% ± 5.2 from 70% ± 6.1 (p < 0.0001). At Week 36, NYHA class improved by ≥1 class in 27 (79.4%) patients. Mean Kansas City Cardiomyopathy Questionnaire clinical summary score improved by 13.8 ± 12.5 points relative to baseline. Median (interquartile range) levels of N‐terminal pro‐B‐type natriuretic peptide were significantly improved from baseline (−665.5 pg/ml [−1244.0, −232.0]; p < 0.0001), while high‐sensitivity cardiac troponin I was unchanged (−2.7 ng/L [−11.3, 1.6]; p = 0.25). There were no drug discontinuations due to adverse events. LVEF <50% occurred in 2 (5.9%) patients, one following pulmonary vein isolation and one associated with atrial fibrillation. Spinal cord injury (SCI) is a serious condition accompanied by severe adverse events that affect several aspects of the patient's life, such as motor, sensory, and functional impairment. Despite its severe consequences, definitive treatment for these injuries is still missing. Therefore, researchers have focused on developing treatment strategies aimed at ensuring full recovery post-SCI. Accordingly, attention has been drawn toward cellular therapy using mesenchymal stem cells. Considering their wide availability, decreased immunogenicity, wide expansion capacity, and impressive effectiveness in many therapeutic approaches, adipose-derived stem cell (ADSC) injections in SCI cases have been investigated and showed promising results. In this review, SCI pathophysiology and ADSC transplantation benefits are discussed independently, together with SCI animal models and adipose stem cell preparation and application techniques. The mechanisms of healing in an SCI post-ADSC injection, the outcomes of this therapeutic approach, and current clinical trials are also deliberated, in addition to the challenges and future perspectives, aiming to encourage further research in this field. This scoping review evaluates the applications, methodologies, and effectiveness of 3D-printed bioinks in chronic wound healing, focusing on bioinks incorporating living cells to facilitate wound closure and tissue regeneration. Relevant studies were identified through comprehensive searches in databases, including PubMed, Scopus, and Web of Science databases, following strict inclusion criteria. These studies employ various 3D bioprinting techniques, predominantly extrusion-based, to create bioinks from natural or synthetic polymers. These bioinks are designed to support cell viability, promote angiogenesis, and provide structural integrity to the wound site. Despite these promising results, further research is necessary to optimize bioink formulations and printing parameters for clinical application. Overall, 3D-printed bioinks offer a transformative approach to chronic wound care, providing tailored and efficient solutions. Many governments maintain lists of terrorist groups, imposing sanctions on designated organizations. However, the logic behind designation remains unclear. Furthermore, most studies focus on Western countries. This paper develops arguments for why attack attributes, group attributes, and policy diffusion might explain proscription. Empirically, we examine hundreds of militant organizations to see which are listed by the European Union, India, Pakistan, Russia, the United Kingdom, or the United States. Recommendations are provided to address these challenges and ethical issues, along with examples of existing regulations and strategies for implementing AI governance frameworks within organizations. This paper highlights innovation, regulation, and ethical issues in relation to AI within the banking and finance sectors. Analyzes the previous literature, and suggests strategies for AI governance framework implementation and future research directions. Innovation in the applications of AI integrates with fintech, such as preventing financial crimes, credit risk assessment, customer service, and investment management. These applications improve decision making and enhance the customer experience, particularly in banks. Existing AI regulations and guidelines include those from Hong Kong SAR, the United States, China, the United Kingdom, the European Union, and Singapore. Challenges include data privacy and security, bias and fairness, accountability and transparency, and the skill gap. Therefore, implementing an AI governance framework requires rules and guidelines to address these issues. This paper makes recommendations for policymakers and suggests practical implications in reference to the ASEAN guidelines for AI development at the national and regional levels. Background: Despite advancements in family planning (FP) services, several barriers persist in the Occupied Palestinian territory (oPt), blocking women's access to suitable, high-quality and equitable FP services. The aim of this study was to understand how healthcare providers perceive their abilities, barriers and opportunities in providing good quality FP services. Furthermore, it seeks to explore knowledge and training regarding FP among healthcare providers engaged in providing FP services. Methods: A qualitative study was undertaken from August to September 2022 in seven Primary Health Care (PHC) clinics distributed in three governorates and operating under the Palestinian Ministry of Health (MoH) in the West Bank. Semi-structured, in-depth face-to-face interviews were conducted with 13 health providers (Physicians, midwives and nurses), using an interview guide in Arabic language. Transcripts were subsequently analyzed using the six phases of reflexive thematic analysis. Results: FP services face various challenges, including shortages in resources such as staff, supplies, infrastructures and FP methods. Midwives possess significant potentials to offer accessible, high-quality, efficient and equitable FP services, yet, their capacities remain underutilized, representing a missed opportunity for a country like Palestine. The study provided a current overview of FP services while illustrating the need for quality FP services and the need for an updated continuous education and training, updated standardized guidelines and protocols and supportive supervision are needed across all levels of healthcare providers. Finally, providers reported a wide range of structural barriers to FP services. Conclusions: It is crucial to meticulously address both community-related and health system factors to enhance the fulfillment of FP needs and reduce unintended and closely spaced pregnancies. Policymakers should invest in the development of laws and regulations regarding FP services, promoting a comprehensive and holistic approach to FP services. We propose and analyze discontinuous Galerkin (dG) approximations to 3D - 1D coupled systems which model diffusion in a 3D domain containing a small inclusion reduced to its 1D centerline. Convergence to weak solutions of a steady state problem is established via deriving a posteriori error estimates and bounds on residuals defined with suitable lift operators. For the time-dependent problem, a backward Euler dG formulation is also presented and analyzed. Further, we propose a dG method for networks embedded in 3D domains, which is, up to jump terms, locally mass conservative on bifurcation points. Introduction: The present study reports the first clinical experience with subcostal uniportal VATS (suVATS) compared with the conventional lateral uniportal VATS (luVATS) approach. Patients and Methods: All patients who underwent suVATS between January 2019 and April 2020 were included. Patients who had undergone luVATS for similar indications were included as the control group. The data were prospectively and retrospectively analysed. Results: The suVATS group included 38 patients with a mean age of 61 (30–83) years. The luVATS group included 33 patients (mean age, 69 years; range: 46–89 years). An intercostal block was performed intraoperatively in the luVATS group. Local infiltration under anaesthesia was performed around the incision in the suVATS group. The duration of the surgery was significantly longer in the suVATS group. However, the chest tube treatment and hospital stay duration were significantly shorter in the suVATS group. The routinely recorded Visual Analogue Scale scores on the first post-operative day and the day of discharge were significantly lower in the suVATS group. Conclusion: Subcostal uniportal VATS enables a shorter drainage treatment duration and hospital stay and significantly reduces post-operative pain. Thus, a faster patient recovery can be achieved. The agricultural fumigant pesticide aluminum phosphide (AlP) is cardiotoxic. Water causes AlP to emit phosphine gas, a cardiac toxin that affects heart function and causes cardiogenic shock. AlP poisoning's high fatality rate is due to cardiotoxicity. This study examines how resveratrol reduces oxidative stress, mitochondrial activity, and apoptosis in human cardiac myocyte (HCM) cells. After determining the optimal doses of resveratrol using the MTT test, HCM cells were subjected to a 24-h treatment of resveratrol following exposure to AlP (2.36 μM). The levels of reactive oxygen species (ROS), superoxide dismutase (SOD) activity, mitochondrial swelling, mitochondrial cytochrome c release, and mitochondrial membrane potential (MMP) in HCM cells were investigated. Also, the expression of Bax and Bcl-2, caspace-3 activity, and apoptosis were assessed. The present investigation revealed that AlP substantially increased the level of ROS and decreased SOD activation, which were significantly modulated by resveratrol in a dose-dependent manner. Moreover, AlP induced an elevation of mitochondrial swelling, cytochrome c release, and MMP collapse. Co-administration of resveratrol significantly reduced above mitochondrial markers. AlP also significantly upregulated BAX and downregulated Bcl-2 expression, elevated caspace-3 activity, and apoptosis. Resveratrol co-administration was able to meaningfully modulate the mentioned parameters and finally reduce apoptosis. In conclusion, resveratrol, via its pleotropic properties, significantly demonstrated cytoprotective effects on HCM cytotoxicity induced by AlP. [ABSTRACT FROM AUTHOR]

Even a transient period of hearing loss during the developmental critical period can induce long-lasting deficits in temporal and spectral perception. These perceptual deficits correlate with speech perception in humans. In gerbils, these hearing loss-induced perceptual deficits are correlated with a reduction of both ionotropic GABAA and metabotropic GABAB receptor-mediated synaptic inhibition in auditory cortex, but most research on critical period plasticity has focused on GABAA receptors. Therefore, we developed viral vectors to express proteins that would upregulate gerbil postsynaptic inhibitory receptor subunits (GABAA, Gabra1; GABAB, Gabbr1b) in pyramidal neurons, and an enzyme that mediates GABA synthesis (GAD65) presynaptically in parvalbumin-expressing interneurons. A transient period of developmental hearing loss during the auditory critical period significantly impaired perceptual performance on two auditory tasks: amplitude modulation depth detection and spectral modulation depth detection. We then tested the capacity of each vector to restore perceptual performance on these auditory tasks. While both GABA receptor vectors increased the amplitude of cortical inhibitory postsynaptic potentials, only viral expression of postsynaptic GABAB receptors improved perceptual thresholds to control levels. Similarly, presynaptic GAD65 expression improved perceptual performance on spectral modulation detection. These findings suggest that recovering performance on auditory perceptual tasks depends on GABAB receptor-dependent transmission at the auditory cortex parvalbumin to pyramidal synapse and point to potential therapeutic targets for developmental sensory disorders. In this study, an innovative hybrid matrix based on recycled expanded polystyrene (EPS), dissolved in gasoline, and polyester resin (PR) is developed. The aim is to improve the physical and mechanical properties of the dissolved EPS, used previously by the authors as a matrix in composite materials. A rate of 20, 30, 40, 50, and 60 wt% of PR were added in the EPS/PR hybrid matrix. Two processes were used for drying the hybrid matrix, namely ambient temperature (ATDP) and thermal drying process (ThDP) at 60C°. Pycnometer and three-point bending measurements with a characterization by scanning electron microscopy (SEM) were carried out in order to determine: the bulk density, the flexural modulus, and the maximum stress and also to observe the EPS/PR morphology. The results showed that the addition of PR increases the physical properties of the EPS/PR hybrid matrix in both drying processes with a drastic improvement of 3 to 6 times in the mechanical properties. Using ThDP at 60℃ makes the matrix lighter and more resistant even at 20 wt% of PR than using ATDP. Otherwise, using ThDP can increase the density from 7 to 16% and improve the tensile strength from 11 to 64% of the dissolved EPS. It was concluded that the elaboration of the innovative hybrid matrix EPS/PR with ThDP at 60℃ improves the physical and mechanical properties of the EPS. BACKGROUND One of the major determinants of exercise intolerance and limiting symptoms among patients with obstructive hypertrophic cardiomyopathy (HCM) is an elevated intracardiac pressure resulting from left ventricular outflow tract obstruction. Aficamten is an oral selective cardiac myosin inhibitor that reduces left ventricular outflow tract gradients by mitigating cardiac hypercontractility. METHODS In this phase 3, double-blind trial, we randomly assigned adults with symptomatic obstructive HCM to receive aficamten (starting dose, 5 mg; maximum dose, 20 mg) or placebo for 24 weeks, with dose adjustment based on echocardiography results. The primary end point was the change from baseline to week 24 in the peak oxygen uptake as assessed by cardiopulmonary exercise testing. The 10 prespecified secondary end points (tested hierarchically) were change in the Kansas City Cardiomyopathy Questionnaire clinical summary score (KCCQ-CSS), improvement in the New York Heart Association (NYHA) functional class, change in the pressure gradient after the Valsalva maneuver, occurrence of a gradient of less than 30 mm Hg after the Valsalva maneuver, and duration of eligibility for septal reduction therapy (all assessed at week 24); change in the KCCQ-CSS, improvement in the NYHA functional class, change in the pressure gradient after the Valsalva maneuver, and occurrence of a gradient of less than 30 mm Hg after the Valsalva maneuver (all assessed at week 12); and change in the total workload as assessed by cardiopulmonary exercise testing at week 24. RESULTS A total of 282 patients underwent randomization: 142 to the aficamten group and 140 to the placebo group. The mean age was 59.1 years, 59.2% were men, the baseline mean resting left ventricular outflow tract gradient was 55.1 mm Hg, and the baseline mean left ventricular ejection fraction was 74.8%. At 24 weeks, the mean change in the peak oxygen uptake was 1.8 ml per kilogram per minute (95% confidence interval [CI], 1.2 to 2.3) in the aficamten group and 0.0 ml per kilogram per minute (95% CI, -0.5 to 0.5) in the placebo group (least-squares mean between-group difference, 1.7 ml per kilogram per minute; 95% CI, 1.0 to 2.4; P<0.001). The results for all 10 secondary end points were significantly improved with aficamten as compared with placebo. The incidence of adverse events appeared to be similar in the two groups. CONCLUSIONS Among patients with symptomatic obstructive HCM, treatment with aficamten resulted in a significantly greater improvement in peak oxygen uptake than placebo. The rapid deployment of electric vehicles (EVs) in Jordan requires massive efforts to prepare the infrastructures that serve the transportation sector. The lack of EV charging stations is the major obstacle that faces EV drivers. Utilizing renewable energy in EV charging stations contributes to the spread of these stations. Renewable energy technologies are environmentally friendly as they mitigate greenhouse gases that cause the global warming phenomenon. In this paper, an EV charging station, based on a PV-biodiesel-battery hybrid system, is investigated. The importance of this article is to discuss the hybrid system of PV and waste vegetable oil (WVO) along with storage that applies the maximum reliability supply, having an environmentally friendly supply and achieving the lowest energy cost of charging. This station is designed based on renewable and WVO utilization. In fact, WVO, coming from restaurants, is exploited to produce electricity by a diesel generator through direct burning after converting it into biodiesel. The capacity of each station is 14 cars/day with a medium-speed charger of 7 kW. The system is simulated and optimized using iHOGA software where multiobjective optimization is applied to achieve the minimum net present cost (NPC) and CO2 emissions considering three cases, PV-diesel, PV-biodiesel, and PV-WVO, all with battery-hybrid system. These values for the EV charging station that uses the PV-biodiesel-battery hybrid system are 624408 € and 15.4 tons/year, respectively. The corresponding values are 781473 € and 15.14 tons/year and 615310 € and 18.84 tons/year for the systems that work with diesel and WVO, respectively, and energy cost is achieved in best solution to be 0.11 Euro/kWh. Abstract\nPoints of InterestTrying to contribute in promoting social justice, equity and equality in inclusive society, this study explored intersectoral challenges and negative feelings experienced by Palestinians with disability during COVID-19 pandemic in besieged and conflict-stricken Gaza, Palestine. It involved two focus group discussions and six individual interviews that included 45 participants, mainly university students with different disabilities and intersectoral stakeholders representing 15 governmental and non-governmental institutions concerned with policies and practices in education, health and socio-economic sectors. The thematic analysis of the results revealed 22 sectoral challenges and also 16 interrelated negative feelings and emotions caused by these challenges. These findings were discussed in light of pre-existing contextual realities that Palestinians with disability were living through tripled burden contextual realities: COVID-19 pandemic, prolonged siege and 11-day aggression on Gaza. A numerical modeling methodology based on the finite element method is utilized in this investigation to predict the impact of the PCM characteristics on the system cooling performance and the solar module output power. The thermal analysis considers the transient nature and the nonlinearity of the problem. The boundary conditions include the cyclic variation with time of solar irradiance and ambient temperature in a hot climatic region, with peak temperatures close to 50 °C. Under these circumstances and owing to the various influencing parameters, choosing an appropriate PCM is a challenging task. By allowing the PCM characteristics to vary continuously over the applicable domains and not just by giving specified discrete values, a systematic approach is adopted in this work. This technique is applied to explore the relationships between system input parameters, as defined by the PCM properties, and output variables, as described by cell temperature and module efficiency. Due to the harsh boundary conditions, the obtained results demonstrated that PCM with high thermal conductivity is required to achieve efficient thermal regulation. The enhanced material characteristics can only be provided by composite PCMs with graphite or metal additives. The resulting optimized design variables of the suitable PCM, melting temperature, thermal conductivity, and thickness, can reduce the cell temperature to 57 °C (20% reduction). The corresponding efficiency and power output are raised by around 8%. The power output was found to increase from 125 W (reference case) to 135 W for the optimized case. The results suggest that using a passive cooling system based on PCM is a viable method for enhancing the power output of a solar PV module. The created coupled model predicts the thermal behavior of the PV module and significantly facilitates the PCM selection procedure to ensure its applicability. [ABSTRACT FROM AUTHOR]

This paper compares four methods of combining n independent tests. The methods are Fisher, logistic, sum of p-values and inverse normal. It is assumed that n independent test statistics {T(i), i = 1, ..., n} are available to combine the n independent tests. The four methods are compared, as n → ∞, via exact Bahadur slope under the assumption that the test statistics follow Conditional Laplace Distribution T(i)|ξi ~ L(τ ξi, 1), ξi ∈ [a,∞), a ≥ 0 where ξ1, ξ2, ... are distributed according to the distribution function (DF) Hξ. This cross-sectional study aimed to report all neuroimaging findings suggestive of raised intracranial pressure in children with pseudotumor cerebri syndrome (PTCS), before and after re-review by two neuroradiologists. We included 48 children aged <18 years diagnosed with PTCS between 2016 and 2021. Clinical and radiological data were obtained from their medical files. Two neuroradiologists independently re-reviewed all neuroimages, and the average of their assessments was compared with the initial neuroimaging reports; an additional review was done to analyze inter- and intraclass correlation. The initial neuroimaging reports showed under-reporting of findings, with only 26 of 48 (54.1%) patients identified with abnormal reports. After revision, the proportion of the reported findings increased to 44 of 48 (91.6%). Distention of the perioptic space was the most commonly reported finding after revision (36.5 of 48; 76%). Flattening of the posterior globe and empty sella were initially under-reported but improved after revision. Moreover, several findings suggestive of increased intracranial pressure not mandated by Friedman criteria were identified, such as narrowing of the Meckel cave, posterior displacement of the pituitary stalk, and narrowing of the cavernous sinus. Analysis of associations between neuroimaging findings and demographic and clinical characteristics yielded no statistically significant results. The inter- and intraclass correlation results demonstrated a significant agreement between raters and within each rater's assessment (P < 0.05). This study highlights the impact of image revision in enhancing PTCS diagnosis. An edge-of-field subsurface drainage discharge system removes excess subsurface water from agricultural fields to improve the conditions of the soil system making them favorable for plant growth. Conversely, edge-of-fields can increase the nitrate (NO3 -) offsite movement to surface water bodies, which ultimately impacts water quality. In St. Johns, MI. Two on-farm large areas were initiated in 2013 for conservation (NT) and conventional till (CT) practices, of which two microscale fields, an area of 4 acres for each, were delineated in 2019 for NT and CT practices under a corn/soybean crop rotation. NT and CT fields were monitored and evaluated through full water years (WYs) 2019-2022. The objective was to use an innovative approach in coupling edge-of-field with soil health assessments to learn how improved soil health functions are influencing nitrate leaching and loads. NT practice enhanced SOM, other physical properties, and soil health parameters, promoting soil health functions compared to CT practice under conditions of adequate precipitation and favorable temperatures. Additionally, it improved the soil's ability to retain and resist nitrate loads in the 2020 water year, with reductions of 9.6 and 18.3 kg/ha, respectively. In 2021, WY nitrate loads reported nearly an equal value in NT vs. CT (4.8 and 4.4 kg/ha, respectively). Factors of uneven weather conditions and drought, variable agronomic practices, and residual nitrogen after corn, rendered soil conditions unfavorable to retain nitrate and induced adverse effects by increasing the nitrate leaching and loads in NT vs CT in 2022 WY (8.1 and 5.8 kg/h, respectively). Therefore, the limited evaluation period (2019-2022 WYs) presented significant challenges and revealed no affirmative evidence that the improved soil health functions under NT reduced the nitrate leaching loads in all WYs of this research. Early detection of infant pathologies by non-invasive means is a critical aspect of pediatric healthcare. Audio analysis of infant crying has emerged as a promising method to identify various health conditions without direct medical intervention. In this study, we present a cutting-edge machine learning model that employs audio spectrograms and transformer-based algorithms to classify infant crying into distinct pathological categories. Our innovative model bypasses the extensive preprocessing typically associated with audio data by exploiting the self-attention mechanisms of the transformer, thereby preserving the integrity of the audio's diagnostic features. When benchmarked against established machine learning and deep learning models, our approach demonstrated a remarkable 98.69% accuracy, 98.73% precision, 98.71% recall, and an F1 score of 98.71%, surpassing the performance of both traditional machine learning and convolutional neural network models. This research not only provides a novel diagnostic tool that is scalable and efficient but also opens avenues for improving pediatric care through early and accurate detection of pathologies. Background: Lambda-cyhalothrin (LCT) belongs to pyrethroid insecticides, the use of which has increased for pest control. It is essential to study the effects of LCT on the DNA of living organisms to prevent its mutagenic and carcinogenic properties. Currently, there is a lack of information on the effects of LCT on humans. This study examined the cytotoxicity and genotoxicity of LCT insecticides on human dental pulp stem cells (DPSCs). Methods: We examined the cytotoxicity of LCT at serial concentrations of 0.5, 1, 2.5, 5, 10, 25, and 50 μM using an MTT assay. Four concentrations of LCT at 0.5, 1, 25, and 50 μM were selected from the cytotoxicity curve and subjected to a comet assay to assess genotoxicity. Results: The results of the MTT assay showed that LCT inhibited cell proliferation at 1 μM concentration of the 5% formulation, while the other concentrations of LCT at 0.5, 2.5, 5, 10, 25, and 50 μM increased cell proliferation rates by 10, 1, 4, 20, 59, and 76%, respectively. The results of the comet assay provided evidence that the LCT insecticide induced a statistically significant increase in DNA damage in DPSCs at all tested concentrations compared to those of the negative controls (P>0.05). Conclusion: The LCT insecticide was genotoxic to DPSCs but was not cytotoxic at the tested concentrations, except at 1 μM. Instead, it increased cell proliferation. This suggests that LCT may function through an additional mechanism that mimics that of estrogen and may potentially become a candidate as a xenoestrogen. [ABSTRACT FROM AUTHOR]

In-flight medical emergencies (IMEs) are common during commercial airline flights. These events occur once every 100 to 1000 flights with pressurization corresponding to an altitude of 5000 to 8000 feet during the flight, low oxygen partial pressures and low humidity. This study was designed to evaluate the knowledge, confidence and attitudes of medical students in Saudi Arabia with regards to managing in-flight medical emergencies. A cross-sectional study targeting all medical students in Saudi Arabia was conducted using an online questionnaire. In total, we analyzed 378 medical students and interns; only 18% possessed an adequate perception of knowledge relating to IMEs; 36.8% felt assured in their IME response, 36.5% believed they could provide competent care, and only 34.5% considered their medical education was adequate for the treatment of IMEs. Our analyses showed that medical students in Saudi Arabia have inadequate confidence and knowledge in managing IMEs. Consequently, there is a significant gap in current medical school curricula in the Kingdom of Saudi Arabia that could be filled by implementing focused training on the management of IMEs. Background: Alumina particles from the grit blasting of Ti-alloy stems are suspected to contribute to aseptic loosening. An alumina-reduced stem surface was hypothesized to improve osseointegration and show comparable short-term outcomes to those of a standard stem. Methods: In this prospective, double-blind, randomized trial, 26 standard (STD) and 27 experimental new technology (NT) stems were implanted. The latter were additionally treated by acid etching and ice blasting to remove alumina particles from the grit-blasting process. Follow-up occurred at 12 and 24 months. Bone mineral density (BMD) around the stem was measured by a dual-energy x-ray absorptiometry device (DEXA). Radiographs were reviewed for alterations. Clinical scoring comprised the Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC) and the Harris Hip Score (HHS). Survival rates were calculated up to 50 months. Results: Lower mean BMD and more severe cortical hypertrophies were found in the NT group. At 12 months, radiolucent lines were observed mostly in the metaphyseal zone for both groups, with a progression tendency in the NT group at 24 months. At 12 months, pain scores and the WOMAC total and physical activity scores were significantly lower in the NT group, without any differences thereafter. The number of NT stem revisions amounted to 6 (24%) and 11 (41%) at 24 and 50 months, respectively. Conclusion: In the NT group, unexpected catastrophic failure rates of 41% caused by early aseptic loosening were noted within 50 months.
