Ectopic adrenal tissue is a rare finding often encountered incidentally during histopathological examinations. The most common site is the genitourinary tract and pelvis, and more frequently in males than females. Ectopic adrenal tissue is primarily found in neonates and is extremely rare in adults. Although it is an unexpected entity, pathologists should be aware of it because it may be confused with metastasis of clear cell renal carcinoma. To the best of our knowledge, only three cases of ectopic adrenal tissue in the mesosalpinx of the fallopian tube have been reported in the medical literature, making this case the fourth one. The ovarian bursa is a small peritoneal cavity enclosed by the mesovarium and mesosalpinx, which surrounds the ovaries and oviductal infundibulum in mammals. The ovarian bursa is considered as the structure facilitating the transport of ovulated oocytes into the oviduct. Our previous study revealed reduced oocyte pick‐up function in the oviduct of lupus‐prone MRL/MpJ‐Faslpr/lpr mouse, suggesting the possibility of an escape of ovulated oocytes into the peritoneal cavity, despite the presence of an almost complete ovarian bursa in the mouse. In this study, we revealed anatomical and histological characteristics of the ovarian bursa in C57BL/6 N, MRL/MpJ, and MRL/MpJ‐Faslpr/lpr mice. All strains had the foramen of ovarian bursa (FOB), with a size of approximately 0.04 to 0.12 cm2, surrounded by the ligament of ovarian bursa (LOB), which is part of the mesosalpinx. The LOB was partially lined with the cuboidal mesothelial cells and consisted of a thick smooth muscle layer in all strains. In 6‐month‐old MRL/MpJ‐Faslpr/lpr mice, in which the systemic autoimmune abnormality deteriorated and oocyte pick‐up function was impaired, the size of the FOB tended to be larger than that of other strains. Additionally, in MRL/MpJ‐Faslpr/lpr mice at 6 months of age, there was infiltration by numerous immune cells in the mesosalpinx suspending the isthmus; however, the LOB prevented severe inflammation and showed deposition of collagen fibers. These results not only indicate that the FOB is a common structure within mice, but also imply the physiological function of the LOB and its role in maintaining the microenvironment around the ovary, as well as regulating healthy reproduction. This type of hernia is very rare, accounting for 7% of all internal hernias and for 0.9% of all small bowel obstructions (SBO).[1],[2] The pathogenesis of the defects on the mesosalpinx is unknown; it can be congenital or secondary to surgery, pelvic inflammatory disease, or delivery trauma. Objective: To describe the laparoscopic transection of restrictive bands of the mesosalpinx as a useful adjunct to the topical application of prostaglandin E2 to treat mares with suspected uterine tubal blockage. Methods: A standard left flank laparoscopic approach was made to the abdomen using three laparoscopic portals. If restrictive bands of the mesosalpinx were observed traversing the uterine tube perpendicularly, they were carefully transected and 1 mg of prostaglandin E2 was then applied to the external surface of the uterine tube. Skin incisions were closed with surgical staples and the procedure was repeated on the right uterine tube. Results: Nine Thoroughbred mares suspected of uterine tubal blockage were treated. The treated mares had been barren for 1.8 years on average (range: 1–5 years). The overall postoperative conception rate in treated mares was 89% (8/9 mares). The mean number of mated oestrus cycles before pregnancy in the eight mares that conceived was 1.9 ± 1.6. These mares had been bred on average 6.2 ± 1.9 cycles without becoming pregnant prior to surgery. Conclusion: Transection of restrictive bands of the mesosalpinx is easily performed as an adjunctive procedure to laparoscopic‐guided application of prostaglandin E2 to the uterine tube. The procedure does not appear to have any detrimental effects on fertility and may improve fertility in a particular subset of mares with complicated uterine tubal disease. [ABSTRACT FROM AUTHOR]

Background: Serum anti-Müllerian hormone (AMH) concentration has been used to assess ovarian reserve in patients with endometriosis, especially when endometrioma surgery is involved. Previously, we reported that decreased serum AMH levels after cystectomy for endometriomas can recover to preoperative levels in some cases. In this present study, we assessed the sequential changes in serum AMH levels before and after cystectomy in terms of the state of the mesosalpinx prior to surgery. Methods: The retrospective cohort study recruited 53 patients from a series of prospective studies conducted from 2009 to 2015. All patients underwent laparoscopic cystectomy for endometriomas. If either mesosalpinx was involved in the endometrioma or adnexal adhesion before cystectomy, the case was defined as 'involved mesosalpinx' (n = 14). If both mesosalpinx remained anatomically correct, the case was classified as 'intact mesosalpinx' (n = 39). Blood samples were obtained from the patients 2 weeks before surgery, and at 1 month and 1 year after surgery to assess serum AMH levels. Results: The serum AMH levels (the involved group vs. the intact group) were 1.92 vs. 0.98 (P = 0.552) preoperatively, 0.59 vs. 1.99 (P = 0.049) at 1 month postoperatively, and 0.48 vs. 2.37 ng/mL (P = 0.007) at 1 year postoperatively. The involved mesosalpinx group showed a further decrease in serum AMH levels at 1 year postoperatively, while serum AMH levels in the intact mesosalpinx group tended to recover. Conclusion: These results suggest that pre-existing mesosalpinx disturbance, in combination with adhesiolysis, may be involved in the medium- and long-term decrease in ovarian reserve after endometrioma surgery.
