Nearshore waters are utilized by elasmobranchs in various ways, including foraging, reproduction, and migration. Multiple elasmobranch species have been previously documented in the nearshore waters of North Carolina, USA, which has a biogeographic break at Cape Hatteras on the Atlantic coast. However, comprehensive understanding of the elasmobranch community in this region is still lacking. Monthly year-round trawling conducted along two ocean transects (near Cape Lookout and Masonboro Inlet in 5 to 18 m depth) in Onslow Bay, North Carolina provided the opportunity to examine the dynamics and seasonal patterns of this community using a multivariate approach, including permutational multivariate analysis of variance and nonparametric BIO-ENV analysis. From November 2004 to April 2008, 21,149 elasmobranchs comprised of 20 species were caught, dominated by spiny dogfish (Squalus acanthias) and clearnose skate (Rostroraja eglanteria). All species exhibited seasonal variation in abundance, but several key species contributed the most to seasonal differences in species composition within each transect. Spiny dogfish was most abundant in the winter at both locations, comprised mainly of mature females. Although clearnose skate was caught in all seasons, the species was most abundant during the spring and fall. Atlantic sharpnose (Rhizoprionodon terraenovae) was one of the most abundant species in the summer, and two distinct size cohorts were documented. Temperature appeared to be the main abiotic factor driving the community assemblage. The extensive year-round sampling provided the ability to better understand the dramatic seasonal variation in species composition and provides new information on the relative abundance of several understudied elasmobranch species that may be of significant ecological importance. Our results underscore the importance of inner continental shelf waters as important elasmobranch habitat and provide baseline data to examine for future shifts in timing and community structure at the northern portion of the biogeographic break at Cape Hatteras. Background: The Parkinson Associated Risk Syndrome (PARS) study was designed to evaluate whether screening with olfactory testing and dopamine transporter (DAT) imaging could identify participants at risk for developing Parkinson's disease (PD).Objective: Hyposmia on a single test has been associated with increased risk of PD, but, taken alone, lacks specificity. We evaluated whether repeating olfactory testing improves the diagnostic characteristics of this screening approach.Methods: Participants completed up to 10 years of clinical and imaging evaluations in the PARS cohort. Olfaction was assessed with the University of Pennsylvania Smell Identification Test at baseline and on average 1.4 years later. Multiple logistic regression and Cox proportional hazards regression were used to estimate the hazard of development of clinical PD or abnormal DAT imaging.Results: Of 186 participants who were initially hyposmic, 28% reverted to normosmia on repeat testing (reverters). No initially normosmic subjects and only 2% of reverters developed DAT imaging progression or clinical PD, compared to 29% of subjects with persistent hyposmia who developed abnormal DAT and 20% who developed clinical PD. The relative risk of clinical conversion to PD was 8.3 (95% CI:0.92-75.2, p = 0.06) and of abnormal DAT scan was 12.5 (2.4-156.2, p = 0.005) for persistent hyposmia, compared to reversion.Conclusions: Persistent hyposmia on serial olfactory testing significantly increases the risk of developing clinical PD and abnormal DAT imaging, compared to hyposmia on a single test. Given the increasing recognition that neurodegeneration begins decades before the appearance of motor symptoms of Parkinson disease (PD), recent attention has turned to methods of preclinical or prodromal diagnosis. Accurate preclinical diagnosis of individuals at high risk of developing manifest motor PD can improve clinical counseling as well as provide an enriched cohort for studies of possible disease-modifying therapies. In this review article, the authors synthesize the myriad clinical, radiographic, and biochemical signatures of preclinical PD, with an emphasis on biomarkers that may provide accurate population screening for the disease. As individual biomarkers have relatively lowsensitivity and specificity, any population-based approach to preclinical diagnosis will likely combine multiple biomarkers to improve both negative and positive predictive value. «2» In his target article, Michel Bitbol presents the radical metaphysical nature of the Merleau-Ponty-inspired neurophenomenology project as Varela wished it - as a challenge to the metaphysics of "science as usual." «5» A key point Bitbol makes is that this hard problem was not intended by Varela to be solved by a one-sided "naturalization of phenomenology" but also, and more strongly, a "phenomenologization of nature" (§18). So again, while Varela's neurophenomenological project was inspired by Merleau-Ponty, Bitbol shows that Merleau-Ponty's full ontology, or intra-ontology, is best presented front and center, as necessary, if we are to complete and extend the very research program Varela originally intended. Without this necessary ontological foundation, which Bitbol is here recommending, neurophenomenology risks the continual default back into the same psychophysics of Gustav Fechner and Wilhelm Wundt at the dawn of modern naturalistic psychology. Recent shifts in the geographic distribution of marine species have been linked to shifts in preferred thermal habitats. These shifts in distribution have already posed challenges for living marine resource management, and there is a strong need for projections of how species might be impacted by future changes in ocean temperatures during the 21st century. We modeled thermal habitat for 686 marine species in the Atlantic and Pacific oceans using long-term ecological survey data from the North American continental shelves. These habitat models were coupled to output from sixteen general circulation models that were run under high (RCP 8.5) and low (RCP 2.6) future greenhouse gas emission scenarios over the 21st century to produce 32 possible future outcomes for each species. The models generally agreed on the magnitude and direction of future shifts for some species (448 or 429 under RCP 8.5 and RCP 2.6, respectively), but strongly disagreed for other species (116 or 120 respectively). This allowed us to identify species with more or less robust predictions. Future shifts in species distributions were generally poleward and followed the coastline, but also varied among regions and species. Species from the U.S. and Canadian west coast including the Gulf of Alaska had the highest projected magnitude shifts in distribution, and many species shifted more than 1000 km under the high greenhouse gas emissions scenario. Following a strong mitigation scenario consistent with the Paris Agreement would likely produce substantially smaller shifts and less disruption to marine management efforts. Species richness has long been used as an indicator of ecosystem functioning and health. Global richness is declining, but it is unclear whether sub-global trends differ. Regional trends are especially understudied, with most focused on island regions where richness is strongly impacted by novel colonisations. We addressed this knowledge gap by testing for multi-decade trends in species richness in nine open marine regions around North America (197 region-years) while accounting for imperfect observations and grounding our findings in species-level range dynamics. We found positive richness trends in eight of nine regions, four of which were statistically significant. Species' range sizes generally contracted pre-extinction and expanded post-colonisation, but the ranges of transient species expanded over the long-term, slowly increasing their regional retention and driving increasing richness. These results provide more evidence that sub-global richness trends are stable or increasing, and highlight the utility of range size for understanding richness dynamics. [ABSTRACT FROM AUTHOR]

Even species within the same assemblage have varied responses to climate change, and there is a poor understanding for why some taxa are more sensitive to climate than others. In addition, multiple mechanisms can drive species' responses, and responses may be specific to certain life stages or times of year. To test how marine species respond to climate variability, we analyzed 73 diverse taxa off the southeast US coast in 26 years of scientific trawl survey data and determined how changes in distribution and biomass relate to temperature. We found that winter temperatures were particularly useful for explaining interannual variation in species' distribution and biomass, although the direction and magnitude of the response varied among species from strongly negative, to little response, to strongly positive. Across species, the response to winter temperature varied greatly, with much of this variation being explained by thermal preference. A separate analysis of annual commercial fishery landings revealed that winter temperatures may also impact several important fisheries in the southeast United States. Based on the life stages of the species surveyed, winter temperature appears to act through overwinter mortality of juveniles or as a cue for migration timing. We predict that this assemblage will be responsive to projected increases in temperature and that winter temperature may be broadly important for species relationships with climate on a global scale. Drug-induced parkinsonism ( DIP) is common, and the motor symptoms can be indistinguishable from Parkinson's disease ( PD). When symptoms persist after drug withdrawal, this may represent 'unmasking' of underlying PD. We previously reported that hyposmia, a common nonmotor feature of PD, was associated with persistent DIP. Here, we report on a series of 33 consecutive patients who underwent dopamine transporter imaging to evaluate DIP. We examined the clinical correlates of underlying dopaminergic denervation by comparing subjects with normal and abnormal scans. Imaging was abnormal in 7 of 33 (21%) cases. Motor features were similar in patients with normal and abnormal scans. Olfactory testing was available for 30 subjects and was concordant with imaging in 27 of 30 (odds ratio = 63; 95% confidence interval: 4.8-820; P = 0.002). Little is known about the behavioral responses of fishes at low temperatures. Of particular interest are predator–prey interactions because feeding at low temperature is necessary for the overwinter survival of many species. This experiment examined how low temperatures affect behavioral interactions between bluefish ( Pomatomus saltatrix L.) and two sizes of bay anchovy ( Anchoa mitchilli V.) prey. Temperature had an effect on multiple responses of predator–prey encounters including the approach distance of bluefish towards prey, attack and escape speeds, and prey handling time. The reaction distance of prey was important in determining the outcome of an attack; anchovy reacting at a greater distance from an attacking bluefish escaped more often. However, temperature did not have an effect on either reaction distance or bluefish capture success. The influence of prey size depended on how capture success was defined. Bluefish ability at catching prey was not affected by anchovy size, but larger prey were ingested less frequently due to a greater incidence of prey being dropped in trials with large anchovy. Further, bluefish had greater difficulty handling and ingesting prey at lower temperatures, especially for larger prey. At the lowest temperature treatment small anchovy were readily consumed, but no attacks were made on larger prey. This shows that bluefish modify prey size-selectivity behavior based on temperature, which probably results from a perceived inability to handle and ingest large prey at low temperatures. Abstract: Background: Drug-induced Parkinsonism is common, causes significant morbidity, and can be clinically indistinguishable from idiopathic Parkinson's disease. Additionally, drug-induced Parkinsonism may, in some cases, represent “unmasking” of incipient Parkinson's disease. Clinical features or tests that distinguish degenerative from pharmacologic Parkinsonism are needed. Methods: We performed a retrospective case-control study of 97 drug-induced Parkinsonism subjects and 97 age-matched patients with Parkinson's disease. We compared the frequency of subjective motor and non-motor complaints, objective motor findings (Unified Parkinson's Disease Rating Scale Part III) and, where available, objective olfactory tests. We also performed a nested case-control study wherein we compared these same features between drug-induced Parkinsonism patients based on whether or not they recovered after changing the offending agent. Results: Non-motor symptoms including constipation and sexual dysfunction were more common in Parkinson's disease than in drug-induced Parkinsonism. While total motor scores were similar between groups, Postural Instability-Gait Difficulty scores were also higher in Parkinson's disease. Features that were significantly different or showed a trend towards significance in both comparisons included subjective loss of facial expression, dream-enactment behavior, autonomic complaints and Postural Instability-Gait Difficulty scores. Hyposmia was more common in Parkinson's disease and was strongly predictive of persistent drug-induced Parkinsonism after therapy change (odds ratio 30.3, 95% confidence interval: 1.5–500, p = 0.03). Conclusions: A constellation of motor and non-motor features may differentiate unmasked Parkinson's disease from drug-induced Parkinsonism. In particular, olfactory testing may offer a simple and inexpensive method to help predict outcomes in drug-induced Parkinsonism and, potentially, identify a cohort of pre-motor Parkinson's disease. Drug-induced Parkinsonism (DIP) secondary to antipsychotics and other dopamine antagonists is common and can be clinically indistinguishable from idiopathic Parkinson's disease (PD). Making the correct diagnosis is essential as it has important implications both for management of the underlying psychiatric condition and potentially lifelong therapy with antiparkinsonian agents. Additionally, because Parkinsonism does not always resolve with withdrawal of the offending agent or can recur years later, DIP may sometimes represent unmasking of incipient PD. The problem is increasing in scope as antipsychotic drugs are prescribed for a widening variety of indications, and understanding the factors that distinguish pharmacologic from degenerative Parkinsonism represents a significant unmet need. Length distributions of juvenile bluefish (Pomatomus saltatrix) are bimodal, consisting of spring- and summer-spawned fish. Research during the 1990s from the northeastern United States suggested that the summer cohort contributes little to the adult population and that overwinter mortality may limit their survival. We examined length distributions of juvenile bluefish from before and after winter and found that the mean length of the summer cohort increased during winter. Based on a winter-growth experiment and temperatures from the two winters examined, changes in mean length were due to size-selective mortality for one year class and growth for the other. Despite evidence for winter mortality, summer-spawned bluefish were commonly caught at age 1. We reexamined the relative contribution of each cohort to the adult population using archived scales from North Carolina fisheries. Cohort origin of adults was determined by back-calculating length at age 1. One-third of adults consisted of summer-spawned fish, contrasting with previous research from the northeast. The differences in relative cohort abundance between the northeastern and southeastern United States arise from sized-based migration in age-1 and age-2 bluefish (the dominant age groups sampled in these studies) and the size-selective gear and sampling season in which they were collected. The role of genetic factors in cognitive decline associated with Parkinson's disease (PD) is unclear. We examined whether variations in apolipoprotein E (APOE), microtubule-associated protein tau (MAPT), or catechol- O-methytransferase (COMT) genotypes are associated with cognitive decline in PD. We performed a prospective cohort study of 212 patients with a clinical diagnosis of PD. The primary outcome was change in Mattis Dementia Rating Scale version 2 score. Linear mixed-effects models and survival analysis were used to test for associations between genotypes and change in cognitive function over time. The ε4 allele of APOE was associated with more rapid decline (loss of 2.9; 95% confidence interval [CI]: 1.7-4.1) of more points per year; P < 0.001) in total score and an increased risk of a ≥10 point drop during the follow-up period (hazard ratio, 2.8; 95% CI: 1.4-5.4; P = 0.003). MAPT haplotype and COMT genotype were associated with measures of memory and attention, respectively, over the entire follow-up period, but not with the overall rate of cognitive decline. These results confirm and extend previously described genetic associations with cognitive decline in PD and imply that individual genes may exert effects on specific cognitive domains or at different disease stages. Carrying at least one APOE ε4 allele is associated with more rapid cognitive decline in PD, supporting the idea of a component of shared etiology between PD dementia and Alzheimer's disease. Hyposmia, psychiatric disorders, and cognitive problems are common nonmotor manifestations in Parkinson's disease, but how they are related remains unclear. We investigated the relationship between olfactory dysfunction and neuropsychiatric manifestations and performed a cross-sectional study of 248 patients at two movement disorders clinics at academic medical centers. Psychiatric measures were the Geriatric Depression Scale-15, Inventory of Depressive Symptomatology, State Anxiety Inventory, Apathy Scale, and Parkinson's Psychosis Rating Scale. Cognitive measures were the Mini-Mental State Examination, Hopkins Verbal Learning Test-Revised, Digit Span, Tower of London-Drexel, and the Stroop Color Word Test. Olfaction was tested with the University of Pennsylvania Smell Identification Test. There was no significant association between olfaction and mood measures, but psychotic symptoms were more common in patients with olfaction scores below the median (30% vs. 12%; P < 0.001). Worse olfaction was associated with poorer memory (Hopkins Verbal Learning Test-Revised delayed recall items: mean [standard deviation], 6.2 [3.2] vs. 8.4 [2.8]; P < 0.001) and executive performance (Tower of London total moves, 52 [38] vs. 34 [21]; P < 0.001). Odor-identification score was a significant predictor of abnormal performance on these cognitive tests after adjustment for age, sex, and disease characteristics in logistic regression models. The relationship between hyposmia, psychosis, and specific cognitive impairments may reflect the anatomic distribution of Lewy pathology and suggests that olfactory dysfunction could be a biomarker of additional extranigral disease. Yoga, the ancient inter-religious thread running through all Indian Spirituality, shares a remarkable congruence with twentieth-century phenomenology. But this conjuncture is not based on a common aspiration of "transcendence from the world," as argued by previous comparisons. Instead, by applying the more advanced Existential Phenomenology of Maurice Merleau-Ponty to the more indigenous Tantric stream of yoga, it will be shown that this congruence occurs in just the opposite direction of immersion into the very "flesh of the world"—the lived human body as homology of the cosmos. Yoga may offer phenomenology a much-needed somatic contemplative praxis, as much as phenomenology may offer yoga the basis for an appropriate theoretical articulation. [ABSTRACT FROM AUTHOR]

The article "The Return of Phenomenology to European Psychology" examines the historical connection between philosophy and psychology. In the 19th century, experimental psychology and phenomenology developed as different approaches to the study of human consciousness. However, the psychologism debate led to the institutional separation of philosophy and psychology. In the second half of the 20th century, there were attempts to conceptualize and systematize phenomenological psychology. Various centers of phenomenological psychology were identified, including Utrecht, Heidelberg, Copenhagen, Leuven, and Pittsburgh. The article focuses on the return of phenomenology to European psychology. It is noted that the Copenhagen School is the only European tradition that continues to exist to this day. In the United States, there has been a continuous discourse on the possibility of phenomenological work in psychology since the end of the last century. In Europe, phenomenological traditions could only be preserved in a rudimentary form in philosophy. However, prominent phenomenological researchers in psychology worked in Switzerland in the second half of the 20th century. In North America, phenomenological psychology has proven to be more resilient. There are differences between the various approaches, both in terms of institutional differences and methodological differences. Seminal ecological theories, island biogeography and the single large or several small (SLOSS) reserve debate, examine whether large contiguous habitats conserve biodiversity better than multiple smaller patches. Today, delineating the ecological effects of habitat area versus configuration in a fragmentation context remains difficult, and often confounds efforts to understand proximate and ultimate drivers of community change in response to habitat alteration. We examined how the major components of fragmentation, habitat division versus area loss, independently influence faunal communities using landscapes constructed from artificial seagrass at scales relevant for juvenile estuarine nekton. We deployed 25 unique, 234‐m2 landscapes designed along orthogonal axes: habitat percent cover (i.e., area) and fragmentation per se (i.e., patchiness) to examine their effects on faunal density, community composition, and probability of bait‐assay consumption. Faunal sampling occurred in both artificial seagrass and interspaced sandflat matrix. We also examined whether larval‐settler density drove faunal density patterns across landscapes. Further, we assessed the relative importance of landscape‐scale parameters versus fine‐scale complexity–canopy height and epiphyte biomass–in determining faunal densities. We most consistently observed increasing epibenthic fish and macroinvertebrate density with increasing seagrass percent cover. Fragmentation per se only negatively affected epibenthic faunal density within the matrix at low seagrass coverage. Bait consumption increased with seagrass cover, suggesting larger habitats are relative foraging hotspots. Alternatively, benthopelagic fish density was unaffected by habitat parameters, reflecting lower seagrass reliance, or increased matrix tolerance. Community compositions did not vary across landscapes, suggesting that abundant species used landscapes indiscriminately. Finally, the relative importance of habitat parameters shifted across faunal guilds and life stages. Landscape percent cover most affected epibenthic faunal density, but not benthopelagic fish density, and neither pattern was related to settler density. Further, only fine‐scale complexity influenced settler densities. Collectively, our results indicate habitat area is a primary, positive driver of faunal densities and generalist consumption, and therefore should be prioritized in seagrass conservation. However, sampling across spatial scales and habitat types revealed nuances in habitat use patterns among faunal guilds and life stages that were not solely area‐dependent, illustrating that a variety of landscape configurations support essential nursery functions. Abstract: Olfactory impairment is a common early non-motor manifestation in Parkinson''s disease that has garnered interest as a clinical biomarker for early “pre-motor” diagnosis and prediction of associated clinical phenotypes. Whether olfactory impairment correlates well with motor symptoms is not yet clear, and recent interest has focused on the relationship between hyposmia and other non-motor symptoms. In this paper, we will review emerging evidence for a relationship between hyposmia and neuropsychiatric manifestations, discussing the potential pathophysiology together with challenges and opportunities for future research. The article discusses the author's views on the article "Spiritual Relationships As an Analytical Instrument in Psychotherapy With Religious Patients," by Agneta Schreurs. According to the author, it is hard to disagree with the proposal of Schreurs to transfer a model for understanding human relationship into a model for understanding relation between humans and their deities. But, he believes that the worldview to achieve the goals of therapy is not always compatible with philosophical truth.

This essay strives to bring together the institutional history of phenomenological psychology within the American academy from the middle of the 20th century to the current moment. Although phenomenological psychology has always been a dynamically international and interdisciplinary movement, the scope of this essay is limited to the different ways in which this new field expressed itself in certain psychology departments and educational institutions across the United States. As global climate change and variability drive shifts in species' distributions, ecological communities are being reorganized. One approach to understand community change in response to climate change has been to characterize communities by a collective thermal preference, or community temperature index (CTI), and then to compare changes in CTI with changes in temperature. However, important questions remain about whether and how responsive communities are to changes in their local thermal environments. We used CTI to analyze changes in 160 marine assemblages (fish and invertebrates) across the rapidly‐changing Northeast U.S. Continental Shelf Large Marine Ecosystem and calculated expected community change based on historical relationships between species presence and temperature from a separate training dataset. We then compared interannual and long‐term temperature changes with expected community responses and observed community responses over both temporal scales. For these marine communities, we found that community composition as well as composition changes through time could be explained by species associations with bottom temperature. Individual species had non‐linear responses to changes in temperature, and these nonlinearities scaled up to a nonlinear relationship between CTI and temperature. On average, CTI increased by 0.36°C (95% CI: 0.34–0.38°C) for every 1°C increase in bottom temperature, but the relationship between CTI and temperature also depended on community composition. In addition, communities responded more strongly to interannual variation than to long‐term trends in temperature. We recommend that future research into climate‐driven community change accounts for nonlinear responses and examines ecological responses across a range of temporal and geographical scales. Species with short life spans frequently show a close relationship between population abundance and environmental variation making these organisms potential indicator species of climatic variability. White (Penaeus setiferus), brown (P. aztecus), and pink (P. duorarum) penaeid shrimp typically have an annual life history and are of enormous ecological, cultural, and economic value to the southeastern United States and Gulf of Mexico. Within North Carolina, all three species rely on the Pamlico Sound, a large estuarine system that straddles Cape Hatteras, one of the most significant climate and biogeographic breaks in the world, as a nursery area. These characteristics make penaeid species within the Pamlico Sound a critical species-habitat complex for assessing climate impacts on fisheries. However, a comprehensive analysis of the influence of the environmental conditions that influence penaeid shrimp populations has been lacking in North Carolina. In this study, we used more than 30 years of data from two fishery-independent trawl surveys in the Pamlico Sound to examine the spatial distribution and abundance of adult brown, white, and pink shrimp and the environmental drivers associated with adult shrimp abundance and juvenile brown shrimp recruitment using numerical models. Brown shrimp recruitment models demonstrate that years with higher temperature, salinity, offshore windstress, and North Atlantic Oscillation phase predict increased abundance of juveniles. Additionally, models predicting adult brown, white, and pink shrimp abundance illustrate the importance of winter temperatures, windstress, salinity, the North Atlantic Oscillation index, and the abundance of spawning adult populations from the previous year on shrimp abundance. Our findings show a high degree of variability in shrimp abundance is explained by climate and environmental variation and indicate the importance of understanding these relationships in order to predict the impact of climate variability within ecosystems and develop climate-based adaptive management strategies for marine populations. Achieving long‐term retention of pop‐up satellite archival tags (PSATs) has proven difficult for all fishes but is particularly challenging for small migrant species due to the relatively large size of tags. In this study, the authors tested the latest and smallest PSAT model on the market, the mark‐report satellite tag (mrPAT), and developed a simple, cost‐effective method of tag attachment on sheepshead Archosargus probatocephalus (Walbaum 1792), a small marine fish. During laboratory trials, the method of tag attachment used in this study outperformed the existing methods with two c. 40 cm fish retaining their tags for 3 months (the duration of the laboratory study). During field deployments, data were successfully obtained for 17 of the 25 tagged fish [37–50 cm fork length (FL)]. Of these, 14 tags (82%) remained on the fish until the pre‐programmed release date resulting in tag retention times of up to 172 days (mean: 140 days). The investigation represents the first extensive study into the feasibility of PSATs for monitoring fishes in this size range. The authors demonstrate that their method of attachment and this latest PSAT model are feasible for c. 5‐month deployments on fishes that are relatively small (c. 45 cm FL). These results with A. probatocephalus represent a potentially significant advance in PSAT methodology for fishes of this size. Purpose: To determine whether dysautonomia can stratify individuals with other prodromal markers of Parkinson's disease (PD) for risk of phenoconversion and functional decline, which may help identify subpopulations appropriate for experimental studies. Methods: Data were obtained from Parkinson's Progression Markers Initiative. Cohorts without PD but with at-risk features were included (hyposmia and/or rapid-eye-movement-sleep behavior disorder, LRRK2 gene mutation, GBA gene mutation). Dysautonomia measures included Scales-for-Outcomes-in-Parkinson's-Disease Autonomic (SCOPA-AUT), seven SCOPA-AUT subscales, and cardiovascular dysfunction (supine hypertension, low pulse pressure, neurogenic orthostatic hypotension). Outcome measures were phenoconversion and Schwab-and-England Activities-of-Daily-Living (SE-ADL) ≤ 70, which indicates functional dependence. Cox proportional-hazards regression was used to evaluate survival to phenoconversion/SE-ADL ≤ 70 for each dysautonomia measure. If a significant association was identified, a likelihood-ratio test was employed to evaluate whether a significant interaction existed between the measure and cohort. If so, regression analysis was repeated stratified by cohort. Results: Median follow-up was 30 months. On multivariable analysis, gastrointestinal and female sexual dysfunction subscales were associated with increased risk of phenoconversion, while the cardiovascular subscale and neurogenic orthostatic hypotension were associated with increased risk of SE-ADL ≤ 70; respective hazard ratios (95% confidence intervals) were 1.13 (1.01–1.27), 3.26 (1.39–7.61), 1.87 (1.16–2.99), 5.45 (1.40–21.25). Only the association between the cardiovascular subscale and SE-ADL ≤ 70 was modified by cohort. Conclusions: Symptoms of gastrointestinal and female sexual dysfunction predict phenoconversion in individuals with other risk markers for PD, while signs and symptoms of cardiovascular dysfunction may be associated with functional decline. ABSTRACT Background There is compelling evidence from over 60 epidemiological studies that smoking significantly reduces the risk of Parkinson's disease (PD). In general, those who currently smoke cigarettes, as well as those with a past history of such smoking, have a reduced risk of PD compared to those who have never smoked. Recently it has been suggested that a cardinal nonmotor sensory symptom of PD, olfactory dysfunction, may be less severe in PD patients who smoke than in PD patients who do not, in contrast to the negative effect of smoking on olfaction described in the general population. Methods We evaluated University of Pennsylvania Smell Identification Test (UPSIT) scores from 323 PD patients and 323 controls closely matched individually on age, sex, and smoking history (never, past, or current). Results Patients exhibited much lower UPSIT scores than did the controls ( P < 0.0001). The relative decline in dysfunction of the current PD smokers was less than that of the never- and past-PD smokers (respective Ps = 0.0005 and 0.0019). Female PD patients outperformed their male counterparts by a larger margin than did the female controls (3.66 vs. 1.07 UPSIT points; respective Ps < 0.0001 and 0.06). Age-related declines in UPSIT scores were generally present ( P < 0.0001). No association between the olfactory measure and smoking dose, as indexed by pack-years, was evident. Conclusions PD patients who currently smoke do not exhibit the smoking-related decline in olfaction observed in non-PD control subjects who currently smoke. Motor features of Parkinson's disease (PD) are heterogeneous and well-studied; non-tremor features of postural instability and gait dysfunction (PIGD) have been linked to worse outcomes and Alzheimer's disease (AD) co-pathology. However, these features are understudied in Lewy body dementias (LBD). Here we perform retrospective analysis of a unique cohort of LBD (n = 30) with Unified Parkinson's Disease Rating Scale (UPDRS) data collected at baseline in proximity to cerebrospinal fluid collection to test the hypothesis that LBD patients with a positive AD biomarker profile (LBD + AD = 13) would have higher PIGD burden compared with LBD patients without AD biomarker positivity (LBD-AD = 17). We find novel evidence for selective impairment of PIGD burden in LBD + AD vs LBD-AD (OR = 1.95, 95%CI = 1.02-3.70, p = 0.04) and a direct association of increasing CSF tau/Aβ1-42 ratio with increasing PIGD disability in the total cohort (β = 0.23, SE = 0.08, p = 0.01). A "no-knowledge" measurement of an open quantum system yields no information about any system observable; it only returns noise input from the environment. Surprisingly, performing such a no-knowledge measurement can be advantageous. We prove that a system undergoing no-knowledge monitoring has reversible noise, which can be canceled by directly feeding back the measurement signal. We show how no-knowledge feedback control can be used to cancel decoherence in an arbitrary quantum system coupled to a Markovian reservoir that is being monitored. Since no-knowledge feedback does not depend on the system state or Hamiltonian, such decoherence cancellation is guaranteed to be general and robust, and can operate in conjunction with any other quantum control protocol. Background: A composite measure that assesses both cognitive and functional abilities in Parkinson's disease (PD) would be useful for diagnosing mild cognitive impairment (MCI) and PD dementia (PDD) and as an outcome measure in randomized controlled trials. The Clinical Dementia Rating Scale Sum of Boxes (CDR-SOB) was designed to assess both cognition and basic-instrumental activities of daily living in Alzheimer's disease but has not yet been validated in PD. Objective: To validate the CDR-SOB as a composite cognitive-functional measure for PD patients, as well as to assess its sensitivity to change. Methods: The CDR-SOB and a comprehensive cognitive and functional battery was administered to 101 PD patients at baseline (39 normal cognition [NC], 41 MCI and 21 PDD by expert consensus panel), and re-administered to 64 patients after 1-2 years follow-up (32 NC and 32 cognitive impairment [CI] at baseline). Results: Cross-sectionally, CDR-SOB and domain scores were correlated with corresponding neuropsychological or functional measures and were significantly different between cognitive subgroups both at baseline and at follow-up. In addition, CDR-SOB ROC curves distinguished between normal cognition and dementia with high sensitivity, but did not distinguish well between NC and MCI. Longitudinal changes in the CDR-SOB and domain scores were not significant and were inconsistent in predicting change in commonly-used cognitive and functional tests. Conclusion: The CDR-SOB detects dementia-level cognitive impairment in PD but may not be appropriate for predicting longitudinal combined cognitive-functional changes in patients without significant cognitive impairment at baseline. Background: Cognitive impairment (CI) is one of the most feared and debilitating complications of PD. No therapy has been shown to slow or prevent CI in PD. Objective: To determine associations between modifiable comorbidities, including cardiovascular disease risk factors, mood disorders, and sleep characteristics, and rate of cognitive decline in Parkinson's disease (PD). Methods: Data from the Parkinson's Progression Markers Initiative (PPMI) cohort was queried for baseline cardiovascular disease risk factors, mood disorders, and sleep characteristics. Linear mixed‐ effects models (LME) were used to examine the association between baseline factors and change in cognition, evaluated by the Montreal Cognitive Assessment (MoCA) over time. Baseline comorbidities found to affect MoCA decline were assessed for an association with focal cognitive domains using LME. Results: Higher Body Mass Index (BMI) (β = −0.009, P = 0.039), State Trait Anxiety Inventory (STAI) (β = −0.005, P < 0.001), Geriatric Depression Scale (GDS) (β = −0.034, P < 0.001), Epworth Sleepiness Scale (ESS) (β = −0.017, P = 0.003), and REM Sleep Behavior Disorder Screening Questionnaire (RBDSQ) (β = −0.037, P < 0.001) were associated with faster rates of MoCA decline. Using established cut‐offs for clinically significant symptoms, being overweight, or the presence of depression, excessive day time sleepiness (EDS), and possible REM sleep behavior disorder (pRBD), were all associated with faster rate of cognitive decline. Conclusion: Several modifiable baseline comorbidities are associated with faster rate of CI over time in patients with PD. Objectives: To investigate the impact of Alzheimer's disease (AD) co‐pathology on an in vivo structural measure of neurodegeneration in Lewy body disorders (LBD). Methods: We studied 72 LBD patients (Parkinson disease (PD) = 2, PD‐MCI = 25, PD with dementia = 10, dementia with Lewy bodies = 35) with either CSF analysis or neuropathological examination and structural MRI during life. The cohort was divided into those harboring significant AD co‐pathology, either at autopsy (intermediate/high AD neuropathologic change) or with CSF signature indicating AD co‐pathology (t‐tau/Aβ1‐42 > 0.3) (LBD+AD, N = 19), and those without AD co‐pathology (LBD−AD, N = 53). We also included a reference group of 25 patients with CSF biomarker‐confirmed amnestic AD. We investigated differences in MRI cortical thickness estimates between groups, and in the 21 autopsied LBD patients (LBD−AD = 14, LBD+AD = 7), directly tested the association between antemortem MRI and post‐mortem burdens of tau, Aβ, and alpha‐synuclein using digital histopathology in five representative neocortical regions. Results: The LBD+AD group was characterized by cortical thinning in anterior/medial and lateral temporal regions (P < 0.05 FWE‐corrected) relative to LBD−AD. In LBD+AD, cortical thinning was most pronounced in temporal neocortex, whereas the AD reference group showed atrophy that equally encompassed temporal, parietal and frontal neocortex. In autopsied LBD, we found an inverse correlation with cortical thickness and post‐mortem tau pathology, while cortical thickness was not significantly associated with Aβ or alpha‐synuclein pathology. Interpretation: LBD+AD is characterized by temporal neocortical thinning on MRI, and cortical thinning directly correlated with post‐mortem histopathologic burden of tau, suggesting that tau pathology influences the pattern of neurodegeneration in LBD. Prolonged status epilepticus (SE) can be refractory to conventional interventions, with high rates of subsequent morbidity and mortality. A high fat, low protein, low carbohydrate ketogenic diet (KD) has been used successfully to treat intractable epilepsy. However, its possible role in prolonged SE has not been well described. We report successful use of the KD in two adult patients with prolonged nonconvulsive SE (NCSE) refractory to multiple other interventions. Our observations suggest induction of ketosis may be a novel strategy to safely and effectively treat status in adults even after weeks to months of refractory seizures. Although there are few data regarding the use of the ketogenic diet in the treatment of adult epilepsy syndromes, it may be an option for the treatment of adults with refractory, prolonged SE. Objective: The objective of this study was to determine the frequency and impact of subjective cognitive complaint (SCC) in Parkinson's disease (PD) patients with normal cognition.Methods: Patients with PD with expert consensus-determined normal cognition at baseline were asked a single question regarding the presence of SCC. Baseline (N = 153) and longitudinal (up to 4 follow-up visits during a 5-year period; N = 121) between-group differences in patients with PD with (+SCC) and without (-SCC) cognitive complaint were examined, including cognitive test performance and self-rated and informant-rated functional abilities.Results: A total of 81 (53%) participants reported a cognitive complaint. There were no between-group differences in global cognition at baseline. Longitudinally, the +SCC group declined more than the -SCC group on global cognition (Mattis Dementia Rating Scale-2 total score, F1,431 = 5.71, P = 0.02), processing speed (Symbol Digit Modalities Test, F1,425 = 7.52, P = 0.006), and executive function (Trail Making Test Part B, F1,419 = 4.48, P = 0.04), although the results were not significant after correction for multiple testing. In addition, the +SCC group was more likely to progress to a diagnosis of cognitive impairment over time (hazard ratio = 2.61, P = 0.02). The +SCC group also demonstrated significantly lower self-reported and knowledgeable informant-reported cognition-related functional abilities at baseline, and declined more on an assessment of global functional abilities longitudinally.Conclusions: Patients with PD with normal cognition, but with SCC, report poorer cognition-specific functional abilities, and are more likely to be diagnosed with cognitive impairment and experience global functional ability decline long term.
