Introducing molecular nanolayers (MNLs) is attractive for enhancing the stability of, and inducing unusual properties at, inorganic thin film interfaces. Although organic molecules anchored to inorganic surfaces have been studied extensively, property enhancement mechanisms underpinned by molecular assemblies at inorganic thin film interfaces are yet to be revealed and understood. Here, ab initio molecular dynamics simulations of tensile strain of Au/MNL/Au thin film nanosandwich models provide insights into molecularly induced strain hardening and toughening. Au/MNL/Au nanosandwiches support up to ≈30% higher stresses and exhibit up to ≈140% higher toughness than pure Au slab models. Both hardening and toughening are governed by molecular length and terminal chemistry in the MNL. Strong Au/MNL interface bonding and greater molecular length promote defect creation in Au, which results in strain hardening. Accommodation of increasing post-hardening strains in the MNL mitigates the stress increase in the Au slabs, delays interface fracture, and contributes to toughening. Remarkably, toughening correlates with equilibrium interface strain, which could be used as a proxy for efficiently identifying promising inorganic/MNL combinations that provide toughening. Why do transnational actors choose to campaign on specific issues, and why do they launch campaigns when they do? In this article, we theorize the membership, focus, timing and strategies used in transnational advocacy campaigns as a function of long-standing professional networks between NGOs and individual professional campaigners. Unlike previous scholarship that highlights the role of powerful 'gatekeeper' organizations whose central position within transnational issue-networks allows them to promote or block specific issues, we draw on recent work in organizational sociology to bring into focus a wider transnational community of individuals and organizations whose competition for professional growth and 'issue-control' is crucial in defining the transnational advocacy agenda. In doing so, we qualify existing notions of agenda-setting and gatekeeping in International Relations (IR) scholarship. To illustrate our theory we use a longitudinal network analysis approach, alongside extensive interviews and analysis of primary non-governmental organization (NGO) sources. The article explores media depictions of industrial labour in Italy, with a special focus on visual, film and television portrayals, spanning from the 1960s to the first two decades of the twenty-first century. Rather than delving into an analysis of labour processes, the primary objective of the article is to scrutinise the gendered representations of work and whether and how the representation of work, including all professions, has played a pivotal role in shaping narratives about Italian society and its inherent contradictions. In this context, the article also emphasises the significance of what remains unrepresented, highlighting the absence of work as equally consequential as its presence. Of particular importance within this exploration is the examination of women's work, a realm less frequently depicted than that of men. The article dedicates specific attention to unravelling the nuances of women's role in the workforce, recognising their portrayal as a key element in understanding broader narratives about Italian society and its complexities. Understanding species differences in sensitivity to toxicants is a critical issue in ecotoxicology. We recently established that double‐crested cormorant (DCCO) embryos are more sensitive than Japanese quail (JQ) to the developmental effects of ethinylestradiol (EE2). We explored how this difference in sensitivity between species is reflected at a transcriptomic level. The EE2 was dissolved in dimethyl sulfoxide and injected into the air cell of eggs prior to incubation at nominal concentrations of 0, 3.33, and 33.3 µg/g egg weight. At midincubation (JQ 9 days; DCCO 16 days), livers were collected from five embryos/treatment group for RNA sequencing. Data were processed and analyzed using EcoOmicsAnalyst and ExpressAnalyst. The EE2 exposure dysregulated 238 and 1,987 genes in JQ and DCCO, respectively, with 78 genes in common between the two species. These included classic biomarkers of estrogen exposure such as vitellogenin and apovitellenin. We also report DCCO‐specific dysregulation of Phase I/II enzyme‐coding genes and species‐specific transcriptional ontogeny of vitellogenin‐2. Twelve Kyoto Encyclopedia of Genes and Genomes pathways and two EcoToxModules were dysregulated in common in both species including the peroxisome proliferator‐activated receptor (PPAR) signaling pathway and fatty acid metabolism. Similar to previously reported differences at the organismal level, DCCO were more responsive to EE2 exposure than JQ at the gene expression level. Our description of differences in transcriptional responses to EE2 in early life stage birds may contribute to a better understanding of the molecular basis for species differences. Introduction: Status asthmaticus (SA) and near-fatal asthma (NFA) are life-threatening conditions that continue to present a management challenge for physicians. Extracorporeal Membrane Oxygenation (ECMO) has been employed as a last resort in treating these patients. Case presentation: We described six patients who were admitted to the ICU for NFA and received ECMO treatment at a high-complexity institution in Cali, Colombia, between 2015 and 2019. All patients are registered in the ELSO registry. Baseline patient characteristics, arterial blood gases (ABG), ventilatory parameters, and complications were collected as specified in the ELSO registry form. Efficacy was analyzed in terms of the improvement in respiratory acidosis, the number of ventilator-free days (VFD), and a reduction in mechanical power (MP). MP, which refers to the energy associated with the mechanical forces involved in breathing and the functioning of the respiratory system, was calculated using a mathematical formula. Safety was evaluated based on the incidence of complications. After 12 hours of ECMO, we achieved a correction of respiratory acidosis, a significant decrease in all ventilatory parameters, and a reduction in MP ranging from 52.8% to 89%. There was one mortality. Among the five surviving patients, all except one, who required a tracheostomy, had a high VFD score, with a mode of 26 days, demonstrating a reduction in ventilation time. Conclusion: Further randomized controlled trials are needed to fully understand the efficacy and safety profiles of ECMO in SA/NFA. MP is being widely used to achieve safer ventilation, and although more data is required, it appears to be a promising option for evaluating the risk of developing VILI and the success of the therapy. This article theorizes path-dependent changes in the institutional architecture of the nuclear nonproliferation regime complex; it analyses the effects of different regime-complex structures on institutional contestation and policy adjustment. I first offer a general theory of how the preexisting institutional structures of international regime complexes (IRCs) facilitate and constrain subsequent institutional developments in ways that make IRCs prone to endogenous, path-dependent change. Next, I illustrate how strategies of regime shifting and rival regime creation in the nuclear nonproliferation complex have triggered path-dependent 'reactive sequencing', resulting in growing institutional fragmentation. To illustrate endogenous dynamics of IRC evolution, I examine the nuclear nonproliferation complex at three 'critical junctures': The mid-1970s, the end of the Cold War, and the early-2000s. During each period, exogenous proliferation shocks interacted with pre-existing institutional structures to produce specific patterns of contestation which set in motion a reactive sequence of growing institutional fragmentation. My argument has relevance for global economic governance broadly and for the growing IPE literature which explores reactive sequencing and institutional decay in global governance institutions. De Schutter's conclusions stand in contrast to Bellamy and Kröger's project, according to which the EU would become a fairer institutional arrangement if it allowed for differentiated integration: Differentiated integration (DI) describes an institutional mechanism whereby some member states opt out or are excluded from certain common EU policies for sovereignty or capacity reasons. This should of course not be very surprising given the centrality of "solidarity talk" in both public debates about European integration, and in the EU's official documents (e.g. Articles 2 and 3 of the Treaty of European Union that mention solidarity). Unlike state-building in medieval Europe, America, and Africa, where a combination of security threats and economic incentives led to a swift consolidation of central authority, post-war Europe has lacked an essential ingredient of successful state-building: an existential threat. The result, argue Keleman and McNamara, has been a 'gradual, uneven, and dysfunctional' integration process, stopping short of statehood. Ironically, their comparative historical explanation for the EU's shortcomings is strangely 'ahistorical', failing to consider the specific world-historical time at which the EU was born. The EU emerged at a time when (a) existing nation-states were relatively solidly formed, and (b) the territorial state is increasingly anachronistic as a means of amassing and projecting power. In a globalized world, providing for citizens' security and welfare demands global alliance-building more than coercive taxation to build large standing armies. War-deprivation is therefore not what explains the EU's limited statehood. As security threats loom on the EU's borders, concentration of fiscal capacity and coercive power in Brussels remains unlikely. We classify rank zero 5d SCFTs geometrically engineered from M-theory on quasi-homogeneous compound Du Val isolated threefold singularities. For all such theories, we characterize the Higgs Branch, by computing the dimension, the continuous and discrete symmetry groups, as well as more refined details such as the charges of the hypermultiplets under these groups. We derive these data by means of a gauge-theoretic method, that we have recently introduced, based on establishing a correspondence between an adjoint Higgs field and the M-theory geometry. As a byproduct, this further allows us to construct several T-brane backgrounds, that yield inequivalent 5d spectra but are associated with the same geometry. [ABSTRACT FROM AUTHOR]

We combine experiments and ab initio density functional theory calculations to investigate the evolution in structural and mechanical properties of TaAlN coatings as a function of the annealing temperature T. Formation of coherent cubic TaN- and AlN-rich nanometer-size domains, occurring during the initial stage of thermally induced phase separation within cubic NaCl-type (B1) TaAlN solid solutions, yields a monotonic increase in hardness from 29 GPa (as deposited coatings) up to a maximum of 35 GPa (+17%) reached after annealing at 1000 °C. Further thermal treatment at T > 1000 °C leads to the transformation of metastable cubic domains into stable hexagonal TaNx and wurtzite AlN phases, thus resulting in hardness reductions. A comparison of our results with those reported in the literature reveals that TaAlN coatings are at least as hard while considerably less stiff (lower elastic moduli) than TiAlN coatings, thus indicating a substantial increase in toughness achieved upon replacing Ti with Ta in the host lattice. Present findings suggest that cubic TaAlN solid solutions are promising candidates for applications in protective coatings possessing both high-temperature hardness and toughness. Objective: Congenital defects/diseases Background: Long arm (q) deletion syndrome of chromosome 18 is a congenital chromosomal disorder. The specialist dental management of patients with 18q deletion is a challenge, as these individuals fall into the category of patients with special needs. The aim of this work was to describe the surgical and dental management in hospital of a patient with long arm deletion syndrome of chromosome 18 (18q). Case Report: An 8-year-old patient with deletion syndrome of chromosome 18 (18q) was referred to the Department of Dentistry and Oral Surgery. The patient presented dental pain and difficult feeding. The examination of the oral cavity revealed a destructive carious lesion of the lower right second deciduous molar and the need to perform a frenectomy due to the short lingual frenulum, which limited the movements of the tongue. Given the complex management of the patient, it was necessary to carry out surgical procedures in the operating room. Frenectomies of the lower labial and lingual frenulum were carried out with the aid of an electric scalpel with an ultra-sharp microdissection needle. At 2-month follow-up, the patient presented with good extraction site healing and satisfactory lingual mobility, along with improvements of speech and feeding. Conclusions: Dental involvement in patients with deletion syndrome of the long arm of chromosome 18 is poorly documented in the literature. The hospital regimen appears to be the criterion standard for the management of the patient with long arm deletion syndrome of chromosome 18. [ABSTRACT FROM AUTHOR]

Cancer patients need multimodal therapies to treat their disease increasingly. In particular, drug treatment, as chemotherapy, immunotherapy, or various associations between them are commonly used to increase efficacy. However, the use of drugs predisposes a percentage of patients to develop toxicity in multiple organs and systems. Principle chemotherapy drugs mechanism of action is cell replication inhibition, rapidly proliferating cells especially. Immunotherapy is another tumor therapy strategy based on antitumor immunity activation trough agents as CTLA4 inhibitors (ipilimumab) or PD-1/PD-L1 inhibitors as nivolumab. If, on the one hand, all these agents inhibit tumor growth, on the other, they can cause various degrees toxicity in several organs, due to their specific mechanism of action. Particularly interesting are bowel toxicity, which can be clinically heterogeneous (pain, nausea, diarrhea, enterocolitis, pneumocolitis), up to severe consequences, such as ischemia, a rare occurrence. However, this event can occur both in vessels that supply intestine and in submucosa microvessels. We report drug-related intestinal vascular damage main characteristics, showing the radiological aspect of these alterations. Interpretation of imaging in oncologic patients has become progressively more complicated in the context of "target therapy" and thanks to the increasing number and types of therapies provided. Radiologists should know this variety of antiangiogenic treatments and immunotherapy regimens first because they can determine atypical features of tumor response and then also because of their eventual bowel toxicity. We study the dynamics of M-theory on isolated non-toric Calabi-Yau threefold singularities of type (Aj, Al) and (Ak, Dn), engineering five-dimensional rank-zero SCFTs. Our approach consists in mapping these backgrounds to type IIA string theory with D6 branes at angles and O6− planes, computing the five-dimensional open string modes located at the brane intersections. This permits us to characterize the Higgs Branches of these theories as algebraic varieties, determine the flavour and gauge group and inspect subtleties related to T-branes. We propose a new way to compute the genus zero Gopakumar-Vafa invariants for two families of non-toric non-compact Calabi-Yau threefolds that admit simple flops: Reid's Pagodas, and Laufer's examples. We exploit the duality between M-theory on these threefolds, and IIA string theory with D6-branes and O6-planes. From this perspective, the GV invariants are detected as five-dimensional open string zero modes. We propose a definition for genus zero GV invariants for threefolds that do not admit small crepant resolutions. Case: We present here a case of chronic pelvic separation 22 years after twin natural delivery in a 49-year-old woman surgically treated with anterior and posterior stabilization. The functional and radiological recovery after a 4-year follow-up was extremely good. Conclusion: Postpartum pubic symphysis diastasis is a rare but dreaded complication of natural delivery. Nonoperative treatment is still considered the gold standard. However, when pain persists despite nonoperative treatment and when a pelvic radiograph reveals a pubic diastasis greater than 2 cm, surgery is recommended. Background: Nonsteroidal anti-inflammatory drug-exacerbated respiratory disease (NERD) might benefit from aspirin desensitization (AD) as an alternative treatment to standard care. However, there is conflicting evidence regarding its role in bronchial symptoms and asthma exacerbations. Objective: To analyze the clinical effects of AD in terms of lung function, systemic and inhaled steroid use, the frequency of acute asthma exacerbations, and adverse effects in patients with NERD and asthma. Methodology: We identified randomized clinical trials (RCTs) from PubMed, EMBASE, SCOPUS, and EBSCO. We also searched the RCT references for additional studies. Studies comparing AD to placebo in patients with a previous history of pulmonary symptoms triggered by ASA or other NSAIDs or with a positive provocation test to ASA were included. Primary results: Five studies with 210 participants with NERD were included in this review. The study duration ranged from 3 to 6 months. Overall, the risk of bias across the included RCTs was low. We identified 3 studies evaluating lung function, 2 of which reported a significant improvement in FEV1 in the AD group after 6 months, while the other reported no difference among the treatments. Due to high heterogeneity, we did not pool the results. The remaining primary outcomes were reported only in a single study each, hindering their interpretation. Secondary outcomes revealed reduced symptom and medication scores in patients with AD. Conclusions: Due to the small number of studies included in this systematic review, conclusions should be made with caution. AD shows a trend towards improving lung function (FEV1) following 6 months of treatment, although no conclusions can be made regarding the use of corticosteroids or the frequency of acute exacerbations. AD appears to reduce both symptom and medication scores. Additional RCTs are needed to fully assess the efficacy of AD in reducing bronchial symptoms in patients with NERD. [ABSTRACT FROM AUTHOR]

This article discusses the collective action problems associated with different kinds of joint external bordering and highlights specific aspects of the European Union which I argue make it particularly ineffective at supplying (many) forms of external bordering. Applying this framework to the EU's Common Security and Defence Policy, European Immigration Policy, and European Neighbourhood Policy, I explain why each of these areas is subject to chronic under-provision. I conclude that a 'Fortress Europe' is unlikely to materialize, giving rise instead to national re-bordering as a reaction to growing pressures on the Union's internal and external borders. This article addresses the puzzle of why, and under what conditions, international organisations cease to exist. International Relations literature offers rich explanations for the creation, design and effectiveness of international institutions and their organisational embodiments, international organizations (IOs), but surprisingly little effort has gone into studying the dynamics of IO termination. Yet if we want to understand the conditions under which international organisations endure, we must also explain why they frequently fail to do so. The article formulates and tests a theory of 'IO death' using a combination of population-wide statistical analysis and detailed historical case studies. My analysis is based on an original dataset covering the period 1815–2016. I find that exogenous shocks are a leading proximate cause of IO terminations since 1815 and that organisations that are newly created, have small memberships, and/or lack centralised structures are most likely to succumb. Surveillance is the only pragmatic approach to improve treatment of hepatocellular carcinoma (HCC) owing to the fact that it allows detection of the tumor at an early and better curable stage. International liver societies recommend surveillance with biannual abdominal ultrasound (US) for patients with cirrhosis of any etiology because of their high risk of developing HCC. This strategy is considered cost-effective, as surveillance requires an articulated and costly set of interventions, including linkage to care of patients with an early detected tumor. However, as transition to HCC is increasingly being observed in noncirrhotic patients, the majority of which does not reach the threshold of cost effectiveness for screening. The European and Japanese liver societies elected to confine recommendations for HCC screening to noncirrhotic patients with advanced fibrosis due to hepatitis C or hepatitis B only. These latter recommendations, however, are challenged by the increasing number of patients with viral hepatitis in whom HCC risk has been attenuated but not eradicated by successful antiviral therapy. In this set of patients, entry criteria of surveillance need to be refined in the light of the suboptimal diagnostic accuracy of non invasive tests that are employed to identify the ideal candidates for surveillance. An unexpected early increase in incidence, recurrence and clinical aggressiveness of hepatocellular carcinoma (HCC) has been reported (and refuted) in patients with HCV-related cirrhosis following direct-acting antiviral (DAA) treatment. To address this controversy, we performed a prospective multicenter study on consecutively enrolled cirrhotic patients, with or without a history of HCC, undergoing DAA therapy. A total of 1,161 HCC-free cirrhotics (group 1) and 124 cirrhotics who had received a curative treatment for an HCC (group 2) were enrolled. Clinical features, including presence of undefined/non-malignant liver nodules (UNMNs), were analyzed with respect to HCC incidence and recurrence. During a median study time of 17 months in group 1 and 16 months in group 2, de novo HCC developed in 48 patients (yearly incidence 3.1/100 patient-years, 75% BCLC 0-A) and recurred in 40 (mean yearly incidence 29.9/100 patient-years, 83% BCLC 0-A). A peak of HCC instant incidence was observed at 4.2 months in group 1 patients with UNMNs, and at 7.7 months in group 2. By multivariable Cox regression models, UNMNs (hazard ratio [HR] 3.11; 95% CI 1.47–6.57: p = 0.003), ascites detected any time before enrolment (HR 3.04; 95% CI 1.23–7.51; p = 0.02), and alpha-fetoprotein log-value (HR 1.90; 95% CI 1.05–3.44; p = 0.03) were the variables independently associated with the incidence of de novo HCC, while history of alcohol abuse (HR 2.10; 95% CI 1.08–4.09; p = 0.03) and history of recurrence of HCC (HR 2.87; 95% CI 1.35–6.09; p = 0.006) were associated with HCC recurrence. An early high incidence of both de novo HCC, in patients with UNMNs, and recurrent HCC was observed in DAA-treated patients; this was not accompanied by increased tumor aggressiveness. This prospective study focuses on the risk of developing de novo or recurrent hepatocellular carcinoma (HCC) after direct-acting antiviral (DAA) treatment in patients with hepatitis C-related cirrhosis. We found that DAA treatment was associated with an early high HCC incidence in patients with undefined or non-malignant nodules, as well as in those with a history of complete response to HCC treatment. Whether this is related to the presence of clinically undetectable nests of cancer cells or to precancerous lesions that may progress to overt HCC upon DAA treatment remains unanswered. No evidence of increased clinical aggressiveness was reported in de novo or recurrent HCC. • The study focuses on the risk of developing de novo or recurrent HCC after DAA treatment in patients with HCV cirrhosis. • An early peak of HCC incidence was observed in DAA-treated patients. • Undefined non-malignant nodules, ascites and AFP were independently associated with the incidence of de novo HCC. • History of alcohol abuse and history of previous HCC recurrence were associated with HCC recurrence. • No evidence of increased tumour aggressiveness was reported in de novo or recurrent HCC. Gillian Brock's compelling and richly textured new book aims to set out a human-rights-based framework for thinking about justice in migration. There is much to celebrate in these chapters, not least Brock's masterful effort at weaving together her basic justificatory framework with real-world political concerns. In this article, I query the focus she places on self-determination in setting out the basic normative argument elaborated in Chapters 2, 3, and 9. In particular, I will wonder whether she gives the collective self-determination of a people anything more than instrumental value, and so whether she is able to distance herself from so-called proponents of "open borders". The contemporary global order is widely said to be in crisis. But despite a rapidly proliferating literature on the subject, there is little clarity or consensus about wherein the 'crisis' consist, or what precisely is under threat. We offer a restricted characterization of the post-war global order based on its fundamental substantive and procedural ordering principles: sovereign inter-state relations and a relatively open global economy, characterized by practices of inclusive, rule-bound multilateralism. We argue that only if one of more of these foundational principles are systematically violated, can we speak of a demise of the order. To this end, we consider the extent to which each of these basic principles is currently endangered. We conclude that what we are witnessing is not the collapse of the current world order, but rather its transformation and adaptation into a broader, more flexible and multifaceted system of global governance – a change within the order rather than of the order. Purpose: To assess the efficacy of interferon (IFN) alpha-2a in the treatment of post-uveitic refractory macular edema (ME).Methods: Retrospective cohort of patients with post-uveitic refractory ME, who received subcutaneous IFN alpha-2a injections for at least 3 months. Baseline central macular thickness (CMT) and best-corrected visual acuity (BCVA) were compared with those at follow-up visits up to 12 months.Results: Thirty-seven patients were included. Treatment duration (median [interquartile range]) was 14[8-24] months with a follow-up of 17[10-38] months. CMT (mean [standard deviation]) decreased from 438[140] to 335[119] μm after 1 month (p < 0.0001) and remained significantly lower up to 12 months (286[98] μm, p = 0.001). BCVA (0.48[0.33] logMAR at baseline) improved by 0.26[0.33] logMAR (p = 0.001) at 12 months. There were 14 recurrences. Seven patients had treatment side effects, without serious adverse events.Conclusions: IFN alpha-2a was effective, safe, and well tolerated in treating post-uveitic refractory ME. In the field of regenerative medicine, acrylated epoxidized vegetable oils are emerging as a promising avenue of exploration. The aim of this study is to evaluate the degradability of two formulations of acrylated epoxidized soybean oil (AESO): pure AESO and AESO diluted with soybean oil (SO) for potential bioprintability applications. The comprehensive investigation of these two polymeric formulations included optimization of polymerization conditions, confirmation of cytocompatibility, and, most importantly, the study of their degradability. The results reveal that AESO, used as a biomaterial for biomedical applications, undergoes a distinctive degradation process, combining both enzymatic and oxidative degradation (AESO/SO samples lose 29.45% of their weight after 60 days). This phenomenon is the result of a complex interplay of factors, including the chemical composition and physical characteristics of the polymer, the unique tissue environment in which it is implanted, and the duration of implantation. Advanced age-related macular degeneration (AAMD) is a complex sight-threating disease of public health significance. Micro RNAs (miRNAs) have been proposed as biomarkers for AAMD. The presence of certain single nucleotide polymorphisms (SNPs) may influence the explanatory value of these biomarkers. Here we present findings from an integrated approach used to determine whether AAMD-associated SNPs have the capacity to influence miRNA-mRNA pairing and, if so, to what extent such pairing may be manifested in a discrete AAMD transcriptome. Using a panel of 8854 SNPs associated with AAMD at p-values ⩽5.0E-7 from a cohort of >30,000 elderly people, we identified SNPs in miRNA target-encoding constituents of: (1) regulator of complement activation (RCA) genes (rs390679, CFHR1, p ⩽ 2.14E-214 rs12140421, CFHR3, p⩽4.63E-29); (2) genes of major histocompatibility complex (MHC) loci (rs4151672, CFB, p⩽8.91E-41 rs115404146, HLA-C, p⩽6.32E-12 rs1055821, HLA-B, p⩽1.93E-9 rs1063355, HLA-DQB1, p⩽6.82E-14); and (3) genes of the 10q26 AAMD locus (rs1045216, PLEKHA1, p⩽4.17E-142 rs2672603, ARMS2, p⩽7.14E-46). We used these findings with existing data on AAMD-related retinal miRNA and transcript profiles for the purpose of making inferences on SNP-mRNA-miRNA-AAMD relationships. Four of 12 miRNAs significantly elevated in AAMD retina (hsa-miR-155-5p, hsa-let-7a-5p, hsa-let-7b-5p hsa-let- 7d-5p) also showed strong pairing capacity (TarBase 7.1 context+ + score < -0.2, miRanda 3.3 pairing score>150) with miRNA target transcripts encoded by AAMDassociated SNPs resident in HLA-DQB1 (rs1063355, hsamiR-155-5p) and TGFBR1 (rs868, hsa-let-7). Three of the 12 miRNAs overexpressed in AAMD retina are inducible by NFkB and have high affinity targets in the complement factor H (CFH) mRNA 3' UTR. We used ENSEMBL to identify polymorphic regions in the CFH mRNA 3' UTR with the capacity to disrupt miRNA-mRNA pairing. Two variants (rs766666504 and rs459598) existed in DNA sequence encoding the seed region of hsa-miR-146a-5p in the CFH mRNA 3' UTR - as this miRNA is also elevated in both vitreous and serum of people with AAMD, it shows great value as a biomarker. Our findings suggest that knowledge on the nature of DNA sequence variation may increase the explanatory power of miRNA biomarkers in genetically diverse populations, while yielding information with which to develop: (1) mechanistic tests on processes implicated in AMD pathogenesis; and, (2) site-specific small molecules (synthetic mimetics or anti-miRNAs) with preventive or therapeutic efficacy for AAMD. Few data are available on the impact of COVID-19 vaccination on CD4 counts and HIV-RNA in persons living with HIV (PLWH). We present the data of 235 PLWH who were vaccinated with BNT162b2 in March 2021–February 2022 at the "Cotugno" hospital in Naples. PLWH treated at the "Cotugno" hospital, who were vaccinated at the hospital vaccination center, without prior COVID-19 and for whom immunological/virological data were available in the last 12 months and in the 6 months after vaccination were included. Antispike Ab were available for 187 and 64 PLWH after the second and third doses: PLWH with antispikes >33 binding antibodies units (BAU)/mL increased from 91% to 98%. Antinucleocapsid Ab performed in 147 and 56 patients identified 19 (13%) asymptomatic/paucisymptomatic COVID-19 infections after the second dose and an additional 15 (27%) after the third dose. Immunological/virological data were collected before vaccination (T0), after the second dose (T1), and after the third dose (T2). The absolute number of CD4 increased after the third dose (median 663, 657, and 707 at T0, T1, and T2; p < 0.000 T0 vs. T2). The proportion of patients with HIV-RNA <50 copies/mL increases significantly after the second dose (73%; 85.7%; 87.7%; p < 0.000 T0 vs. T2). The presence of COVID-19 asymptomatic/paucisymptomatic infections (demonstrated by the presence of antinucleocapsid Ab) significantly increases SARS-CoV-2 antispike Ab after second dose, but not after third dose. Asymptomatic/paucisymptomatic COVID-19 infections do not have influence on CD4 cell number and HIV-RNA level. Similarly, the presence of not-controlled HIV-RNA (HIV-RNA >50 copies/mL) does not influence antispike Ab response. According to our data, the response to SARS-CoV2 vaccination is effective in people living with HIV. Primary care providers (PCPs) have few resources to manage their overweight/obese patients. The purpose of study was to determine if technological resources, such as smartphone apps, may be useful for weight management counseling. PCPs were surveyed about their current use of resources for obesity management and whether smartphone apps would be helpful. Seventy-four PCPs completed the survey. Only 15% currently referred patients to smartphone apps at least sometimes or more often. When asked about features of apps, 66% were not aware of apps with interactive healthy eating games, yet if aware, 45% reported they would refer patients. Providers reported a greater likelihood of being more effective using an app compared with their current ability to manage overweight/obesity, 3.19 versus 2.85, P <.01. The majority of pediatric providers surveyed do not provide technological resources for their overweight/obese patients; yet, they appear interested in using smartphone apps as a resource. Supervised classification is one of the most powerful techniques to analyze data, when a-priori information is available on the membership of data samples to classes. Since the labeling process can be both expensive and time-consuming, it is interesting to investigate semi-supervised algorithms that can produce classification models taking advantage of unlabeled samples. In this paper we propose LapReGEC, a novel technique that introduces a Laplacian regularization term in a generalized eigenvalue classifier. As a result, we produce models that are both accurate and parsimonious in terms of needed labeled data. Background: Next Generation Sequencing (NGS) experiments produce millions of short sequences that, mapped to a reference genome, provide biological insights at genomic, transcriptomic and epigenomic level. Typically the amount of reads that correctly maps to the reference genome ranges between 70% and 90%, leaving in some cases a consistent fraction of unmapped sequences. This 'misalignment' can be ascribed to low quality bases or sequence differences between the sample reads and the reference genome. Investigating the source of the unmapped reads is definitely important to better assess the quality of the whole experiment and to check for possible downstream or upstream 'contamination' from exogenous nucleic acids. Results: Here we propose DecontaMiner, a tool to unravel the presence of contaminating sequences among the unmapped reads. It uses a subtraction approach to identify bacteria, fungi and viruses genome contamination. DecontaMiner generates several output files to track all the processed reads, and to provide a complete report of their characteristics. The good quality matches on microorganism genomes are counted and compared among samples. DecontaMiner builds an offline HTML page containing summary statistics and plots. The latter are obtained using the state-of-the-art D3 javascript libraries. DecontaMiner has been mainly used to detect contamination in human RNA-Seq data. Conclusions: DecontaMiner is a tool designed and developed to investigate the presence of contaminating sequences in unmapped NGS data. IEC 60156 and IEC 60897 were standardized with the purpose of evaluating the breakdown voltage of liquid dielectrics both under AC and impulse voltages. AC breakdown voltage determination is a simple method but only able to provide information on the presence of contaminants. The limit of this test is the large scatter in the results and, for this reason, this IEC standard has been recently revised. Impulse breakdown voltage determination allows discrimination of different dielectric liquids, also in terms of their chemical composition. This test has only limited application because of the dimensions of the high voltage generator. The present paper shows a comparison among breakdown voltages of various mineral oils and natural esters under both AC and impulse voltages. Introduction: Acetabulum periprosthetic fractures are rare, but are increasing, due to increase in high-energy trauma and a decrease in mortality index. Reconstruction of an acetabular fracture, in the presence of hip arthroplasty can be very complex and represents a real challenge for orthopedic surgeon. Aim of this multicentric study is to classify periprosthetic acetabulum fractures and to propose a treatment algorithm.Materials and Methods: 24 cases of acetabular periprosthetic fracture were treated surgically from 01.01.2010 to 31.04.2017 in three different hospitals; 4 males and 20 females, average age 76 years (range 56-90 years).Treatment: 4 cases treated conservatively, 8 cases ORIF, 2 cases treated with acetabular ring and screws, 9 cases ORIF and acetabular cup revision, 1 implant removal without revision.Results: All cases were reviewed at minimum 12 months follow-up. In 22 cases, there was no need for new surgical procedures and radiographically all implants appeared stable and with good bone integration. In a case of a type 1b fracture, a dislocation of prosthetic implant was observed after 3 months. In a case prosthesis was explanted and it was not possible to perform a revision.Discussion and Conclusions: Fracture classification systems must facilitate communication between surgeons and encourage documentation and research. However, they should also have prognostic value, so from them should come directly a treatment algorithm. In our experience, most important factors as prognostic and therapeutic predictors were: implant stability and timing of fracture: intraoperative or postoperative. In postoperative fractures CT is mandatory to evaluate cup mobilization and fracture patterns. Our classification proposal is simple and easy to remember for daily use. States’ capacity for using modern information and communication technology to inflict grave harm on enemies has been amply demonstrated in recent years, with many countries reporting large-scale cyberattacks against their military defense systems, water supply, and other critical infrastructure. Currently, no agreed-upon international rules or norms exist to govern international conflict in cyberspace. Many governments prefer to keep it that way. They argue that difficulties of verifiability and challenges posed by rapid technological change rule out agreement on an international cyber convention. Instead, they prefer to rely on informal cooperation and strategic deterrence to limit direct conflict. In this article, I seek to rebut some of the main objections to seeking an international convention on the use of cyber weapons. While there are significant obstacles to achieving effective arms control in the cyber domain, historical experience from other areas of international arms control suggests that none of these obstacles are insurmountable. Furthermore, while most critics of cyberarms control assume that cyberspace favors offensive strategies, closer inspection reveals the dominance of cyber-defensive strategies. Abstract: Background: Management of hepatocellular carcinoma (HCC) is framed within standardized protocols released by Scientific Societies, whose applicability and efficacy in field practice need refining. Aim: We evaluated the applicability and effectiveness of guidelines for the treatment of HCC of the American Association for the Study of the Liver (AASLD). Methods: 370 consecutive cirrhotic patients with de novo HCC in different stages, 253 BCLC A, 66 BCLC B, 51 BCLC C received treatment through a multidisciplinary team (MDT) decision and were followed until death or end of follow‐up. Results: Treatment was adherent to AASLD recommendations in 205 (81%) BCLC A patients, 36 (54%) BCLC B, and 27 (53%) BCLC C. Radiological complete response was achieved in 165 (45%) patients after the first‐line treatment, in 22 (19%) after a second‐line and in 9 (23%) after a third‐line treatment. Adherence to AASLD recommendation allowed a lower yearly mean mortality rate in BCLC A patients compared with other treatment (5.0% vs 10.4% P = .004), whereas upward treatment stage migration compared with the standard of care was associated with reduced yearly mortality in BCLC B (8.6% vs 20.7%, P = .029) and BCLC C (42.6% vs 59.0%, P = .04) patients. Conclusions: HCC multimodality treatment including other than first‐line therapy is common in clinical practice and impact on the achievement of complete response. Density-functional ab initio molecular dynamics (AIMD) simulations are carried out to determine Cu adatom and admolecule transport properties as a function of temperature, as well as atomistic processes leading to formation of Cu/TiN(0 0 1) islands at 350 K. At very low temperatures T  ≤ 200 K, Cu adatoms (Cu ad ) migrate among favored fourfold-hollow surface sites by passing across atop-Ti metastable positions. For increasing temperatures, however, Cu ad transport becomes progressively more isotropic, and switches continuously from normal- to super-diffusive with mean-square displacement dependencies on time that alternate between linear and exponential. Despite that, the Cu ad diffusivity D can be expressed by a fairly Arrhenius-like behavior D ( T ) = 8.26(×2 ±1 ) × 10 −4  cm 2  s −1 exp[(−0.04 ± 0.01 eV)/( k B T )] over the entire investigated temperature range (100 ≤  T  ≤ 1000 K). AIMD simulations also reveal that the condensation of Cu adatoms into Cu x>1 adspecies is kinetically hindered by long-range (>5.5 Å) adatom/adatom repulsion. During Cu island nucleation, all Cu atoms occupy atop-N positions indicating favored Cu(0 0 1)/TiN(0 0 1) epitaxial growth. Nevertheless, Cu agglomerates formed by five, or more, atoms tend to arrange in 3D structures, which maximize intracluster bonds while minimizing film/substrate interactions. Gastritis is a widely spread inflammatory disease, mostly caused by Helicobacter pylori infection. Release of IL-8 by the stomach epithelium is a hallmark of gastritis and contributes to the amplification of the inflammatory state. Pharmacological modulation of IL-8 release is a strategy to relieve gastric inflammation and prevent more severe clinical outcomes. In search of nutraceuticals with potential anti-gastritis properties we used a bio-guided approach based on IL-8 secretion by gastric cells to characterize extracts from the fruits of different chestnut varieties. We found that the ability to inhibit IL-8 secretion correlated with the amount of proanthocyanidins and was associated to the not edible parts of chestnut in all the tested varieties. We also found that the anti-inflammatory activity is preserved upon mild thermal treatment and after in vitro simulated gastric digestion. By combining a robust bio-guided approach with a comprehensive analysis of the tannin fraction of chestnut extracts, we provide evidence for the potential use of chestnut-based nutraceuticals in human gastritis. The bioactive components of chestnut fruits inhibit IL-8 secretion by impairing NF-κB signaling and by other mechanisms, thus opening new applications of proanthocyanidins for inflammation-based diseases. Hard refractory transition-metal nitrides possess unique combinations of outstanding mechanical and physical properties, but are typically brittle. Recent experimental results demonstrated that single-crystal NaCl-structure (B1) V 0.5 Mo 0.5 N pseudobinary solid solutions are both hard (∼20 GPa) and ductile; that is, they exhibit toughness, which is unusual for ceramics. However, key atomic-scale mechanisms underlying this inherent toughness are unknown. Here, I carry out density-functional ab initio molecular dynamics (AIMD) simulations at room temperature to identify atomistic processes and associated changes in the electronic structure which control strength, plasticity, and fracture in V 0.5 Mo 0.5 N, as well as reference B1 TiN, subject to <001> and <110> tensile deformation. AIMD simulations reveal that V 0.5 Mo 0.5 N is considerably tougher than TiN owing to its ability to ( i ) isotropically redistribute mechanical stresses within the elastic regime, ( ii ) dissipate the accumulated strain energy by activating local structural transformations beyond the yield point. In direct contrast, TiN breaks in brittle manner when applied stresses reach its tensile strength. Charge transfer maps show that the adaptive mechanical response of V 0.5 Mo 0.5 N originates from highly populated d - d metallic-states, which allow for counterbalancing the destabilization induced via tensile deformation by enabling formation of new chemical bonds. The high ionic character and electron-localization in TiN precludes the possibility of modifying bonding geometries to accommodate the accumulated stresses, thus suddenly causing material's fracture for relatively low strain values. Transition-metal (TM) carbides are an important class of hard, protective coating materials; however, their brittleness often limits potential applications. We use density functional theory to investigate the possibility of improving ductility by forming pseudobinary cubic M 1 M 2 C alloys, for which M 1 = Ti or V and M 2 = W or Mo. The alloying elements are chosen based on previous results showing improved ductility of the corresponding pseudobinary nitride alloys with respect to their parent compounds. While commonly-used empirical criteria do not indicate enhanced ductility in the carbide alloys, calculated stress/strain curves along known slip systems, supported by electronic structure analyses, indicate ductile behavior for VMoC. As VMoC layers are sheared along the 1 1 ¯ 0 direction on {111} planes, the stress initially increases linearly up to a yield point where the accumulated stress is partially dissipated. With further increase in strain, the stress increases again until fracture occurs. A similar mechanical behavior is observed for the corresponding TM nitride VMoN, known to be a ductile ceramic material [1]. This study tested whether parental knowledge of the American Academy of Pediatrics’ (AAP) recommendations on juice limits for children is associated with decreased consumption of juice and sugar-sweetened beverages (SSBs) among parents and children. Fifty-two parents with children 2 to 12 years old in a resident continuity clinic in East Harlem, New York, completed a survey asking about children’s and parent’s practice and quantitative consumption of juice and SSBs as well as parental knowledge of the AAP recommendations on juice limits. Parent’s total daily consumption of juice and SSBs (P < .01), parent’s score on the test of AAP guidelines (P = .04), and parent’s post–high school education (P = .01) were associated with children’s juice and SSB consumption in a multivariable linear regression model. Children’s consumption of juice and SSBs is positively associated with parental consumption of juice and SSBs and negatively associated with parental formal education and knowledge of the AAP recommendations on juice limits. The involvement of brain-derived neurotrophic factor (BDNF) in different central nervous system (CNS) diseases suggests that this neurotrophin may represent an interesting and reliable therapeutic target. Accordingly, the search for new compounds, also from natural sources, able to modulate BDNF has been increasingly explored. The present review considers the literature on the effects of botanicals on BDNF. Botanicals considered were Bacopa monnieri (L.) Pennell, Coffea arabica L., Crocus sativus L., Eleutherococcus senticosus Maxim., Camellia sinensis (L.) Kuntze (green tea), Ginkgo biloba L., Hypericum perforatum L., Olea europaea L. (olive oil), Panax ginseng C.A. Meyer, Rhodiola rosea L., Salvia miltiorrhiza Bunge, Vitis vinifera L., Withania somnifera (L.) Dunal, and Perilla frutescens (L.) Britton. The effect of the active principles responsible for the efficacy of the extracts is reviewed and discussed as well. The high number of articles published (more than one hundred manuscripts for 14 botanicals) supports the growing interest in the use of natural products as BDNF modulators. The studies reported strengthen the hypothesis that botanicals may be considered useful modulators of BDNF in CNS diseases, without high side effects. Cultured primary human keratinocytes are frequently employed for studies of immunological and inflammatory responses; however, interpretation of experimental data may be complicated by donor to donor variability, the relatively short culture lifetime, and variations between passages. To standardize the in vitro studies on keratinocytes, we investigated the use of HaCaT cells, a long-lived, spontaneously immortalized human keratinocyte line which is able to differentiate in vitro, as a suitable model to follow the release of inflammatory and repair mediators in response to TNFα or IL-1β. Different treatment conditions (presence or absence of serum) and differentiation stimuli (increase in cell density as a function of time in culture and elevation of extracellular calcium) were considered. ELISA and Multiplex measurement technologies were used to monitor the production of cytokines and chemokines. Taken together, the results highlight that Ca2+ concentration in the medium, cell density, and presence of serum influences at different levels the release of proinflammatory mediators by HaCaT cells. Moreover, HaCaT cells maintained in low Ca2+ medium and 80% confluent are similar to normal keratinocytes in terms of cytokine production suggesting that HaCaT cells may be a useful model to investigate anti-inflammatory interventions/therapies on skin diseases. Background Reducing meat consumption is often advised; however, inadvertent nutritional deficiencies during pregnancy may result in residual neurodevelopmental harms to offspring. This study assessed possible effects of maternal diets in pregnancy on adverse substance use among adolescent offspring. Methods Pregnant women and their 13-year-old offspring taking part in a prospective birth cohort study, the Avon Longitudinal Study of Parents and Children (ALSPAC), provided Food Frequency Questionnaire data from which dietary patterns were derived using principal components analysis. Multivariable logistic regression models including potential confounders evaluated adverse alcohol, cannabis, and tobacco use of the children at 15 years of age. Results Lower maternal meat consumption was associated with greater problematic substance use among 15-year-old offspring in dose-response patterns. Comparing never to daily meat consumption after adjustment, risks were greater for all categories of problem substance use: alcohol, odds ratio OR = 1.75, 95% CI = (1.23, 2.56), p < 0.001; tobacco use OR = 1.85, 95% CI = (1.28, 2.63), p < 0.001; and cannabis OR = 2.70, 95% CI = (1.89, 4.00), p < 0.001. Given the likelihood of residual confounding, potential causality was evaluated using stratification for maternal allelic variants that impact biological activity of cobalamin (vitamin B12) and iron. Lower meat consumption disproportionally increased the risks of offspring substance misuse among mothers with optimally functional (homozygous) variants (rs1801198) of the gene transcobalamin 2 gene ( TCN2) which encodes the vitamin B12 transport protein transcobalamin 2 implicating a causal role for cobalamin deficits. Functional maternal variants in iron metabolism were unrelated to the adverse substance use. Risks potentially attributable to cobalamin deficits during pregnancy include adverse adolescent alcohol, cannabis, and tobacco use (14, 37, and 23, respectively). Conclusions Lower prenatal meat consumption was associated with increased risks of adolescent substance misuse. Objectives: This study compared the number of children enrolled in Medicaid in rural and urban areas of South Carolina with an overweight/obesity diagnosis and the mean rates of office visits with overweight/obesity diagnosed.Methods: Medicaid claims data from 2012 for children in three South Carolina counties, categorized as urban, rural high resource, and rural low resource, were used to identify those who had been diagnosed as being overweight/obese during any encounter. Logistic and Poisson regressions were performed to predict whether overweight/obese children in each county would receive an overweight/obesity visit diagnosis and to calculate the mean rate of total office visits with an overweight/obesity diagnosis in each county.Results: A total of 1233 children enrolled in Medicaid were diagnosed as being overweight/obese at any encounter in the designated counties. Well visits with overweight/obesity diagnosed varied significantly, with 42.6%, 28%, and 11% in urban, rural high-resource counties, and rural low-resource counties, respectively (P < 0.01). In the logistic regression rural high-resource children (adjusted odds ratio 0.58, 95% confidence interval 0.38-0.88) and rural low-resource children (adjusted odds ratio 0.16, 95% confidence interval 0.09-0.28) were less likely than urban children to be diagnosed as being overweight/obese at a well visit. All of the children had a low number of total office visits with overweight/obesity diagnosed. When comparing the counties, urban children (1.22 visits per year) had more visits than rural low-resource children (0.75 visits per year, P < 0.01) and rural high-resource children (0.89 visits per year, P < 0.01).Conclusions: Overweight/obesity is underdiagnosed in rural children enrolled in Medicaid in South Carolina, which affects the number of children who receive help to manage their weight. ABSTRACT The contract-based design formalism supports compositional design and verification, and generalizes many other languages where components are defined in terms of their assumptions and guarantees. Most languages and tools for contract-based design provide constructs to define, instantiate, and connect contracts, but fall short in capturing families of potential architectures in a flexible way. This article presents a Contract-Based Specification Language for Platforms ( CSL4P). A platform comprises a set of contract types and a set of constraints called rules. Contract types can be instantiated and connected to form platform instances. While the meaning of composition is predefined in most languages, composition rules are used in CSL4P to provide a finer control on the semantics of interconnections. In addition, the separation of contract types from rules allows users of CSL4P to define different platforms out of the same set of components. Background: One of the most challenging issue in the variant calling process is handling the resulting data, and filtering the genes retaining only the ones strictly related to the topic of interest. Several tools permit to gather annotations at different levels of complexity for the detected genes and to group them according to the pathways and/or processes they belong to. However, it might be a time consuming and frustrating task. This is partly due to the size of the file, that might contain many thousands of genes, and to the search of associated variants that requires a gene-by-gene investigation and annotation approach. As a consequence, the initial gene list is often reduced exploiting the knowledge of variants effect, novelty and genotype, with the potential risk of losing meaningful pieces of information. Results: Here we present Var2GO, a new web-based tool to support the annotation and filtering of variants and genes coming from variant calling of high-throughput sequencing data. Var2GO permits to upload either the unprocessed Variant Calling Format file or a table containing the annotated variants. The raw data undergo a preliminary step of variants annotation, using the SnpEff tool, and are converted to a table format. The table is then uploaded into an on the fly generated database. Genes associated to the variants are automatically annotated with the corresponding Gene Ontology terms covering the three GO domains. Using the web interface it is then possible to filter and extract, from the whole list, genes having annotations in the domain of interest, by simply specifying filtering parameters and one or more keywords. The relevance of this tool is demonstrated on exome sequencing data. Conclusions: Var2GO is a novel tool that implements a topic-based approach, expressly designed to help biologists in narrowing the search of relevant genes coming from variant calling analysis. Its main purpose is to support non-bioinformaticians in handling and processing raw variant calling data through an intuitive web interface. Furthermore, Var2GO offers a complete pipeline that, starting from the raw VCF file, allows to annotate both variants and associated genes and supports the extraction of relevant biological knowledge.
