Abstract: The concept of comprehensive gerontological assessment is a foundation of modern geriatrics. Our focus was to try to clarify the underlying concepts, assess the level of evidence and clarify the issues still under debate. The concept implies the definition of an interdisciplinary process for a multidimensional assessment in order to produce a coordinated plan. The central notion is that the systematization of this multidimensionality and interdisciplinarity needs the establishment of dedicated process (meeting tools, clinical information system, etc.). Following dimensions should be covered: health, social, economic, environmental and psychological. Any assessment process that could lead to forgetting one of its dimensions cannot be viewed as a comprehensive gerontological assessment. The level of evidence is higher in hospital acute inpatient unit but it is still low in all other areas of health care but the scattered data in the literature argues for qualitative benefits (improved quality of care or quality of life). The questions that remain are numerous including the choice of strategy for initial evaluation (maximum versus minimum; from the outset by many professionals versus graduated based on the minimum initial evaluation), the choice of tool, the optimal location, the required intensity of monitoring and the ideal target population. We studied the factors influencing the choice of admission to Geriatrics units, instead of other acute hospital units after an emergency visit. We report the results from a cohort of 1283 randomly selected patients aged >75 years hospitalized in emergency and representative of the French University hospital system. All patients underwent geriatric assessment. Baseline characteristics of patients admitted to Geriatrics and other units were compared. A center effect influencing the use of Geriatrics units during emergencies was also investigated. Admission to a Geriatrics unit during the acute care episode occurred in 499 cases (40.3%). By multivariate analysis, 4 factors were related to admission to a Geriatrics unit: cognitive disorder: odds ratio (OR)=1.79 (1.38–2.32) (95% confidence interval=95% CI); “failure to thrive” syndrome OR=1.54 (1.01–2.35), depression: OR=1.42 (1.12–1.83) or loss of Activities of Daily Living (ADL): OR=1.35 (1.04–1.75). The emergency volume of the hospital was inversely related to the use of Geriatrics units, with high variation that could be explained by other unstudied factors. In the French University Emergency Healthcare system, the “geriatrics patient” is defined by the existence of cognitive disorder, psychological symptoms or installed loss of autonomy. Nevertheless, considerable nation-wide variation was observed underlining the need to clarify and reinforce this discipline in the emergency healthcare system. [Copyright &y& Elsevier]

Abstract: Purpose: We have little information on the geriatric characteristics of elderly patients visiting the emergency departments (ED) in France. In order to develop an adapted prevention of the arrival of some elderly patients, the determinants of their arrival to the ED deserves to be better known. Methods: A one-day cross-sectional study was conducted in French ED. A standardized questionnaire was used for each patient over 80 years (Pts), specifying the sociodemographic characteristics, the circumstances of visit to and care received in the ED, and the orientation of the patients after consulting the ED. Results: On a sample of 1298 Pts, health event leading to ED started at home in 63.8% of cases. When the patients initiated themselves the recourse to health care (RHC), they called less often a general practitioner (61.9% of cases) than when the RHC was triggered by their family (69.6%, p =0.01). When a health care professional initiated the RHC, it was a GP in more than 80% of cases. Return to residence was more frequent when the patient triggered the RHC (34.5% versus 22.9% for the family and 16.0% for the professional health care, p <0,001). Conclusion: The actor of the decision of arrival to the ED has an impact in the RHC, in resources utilisation, and on the patient''s orientation after coming in ED. The results of this study may help to design strategies aiming at avoiding unnecessary ED consultations of elderly persons. [Copyright &y& Elsevier]

Abstract: Background: Inappropriateness of hospital use occurs when a gap between the patient''s needs and the level of care delivered exists. Taking into account the improvement of number of acute geriatric care, it appears relevant to study the rate and causes of inappropriate hospital use in this context. Methods: All patients in two services of acute geriatrics were included: medical and socioeconomic data were collected, the appropriateness of each day of their hospitalization was evaluated using the French version of the Appropriateness Evaluation Protocol and the inappropriate days'' Causes Analysis Protocol. Risk factors of having at least one inappropriate day occurring during the stay were searched using relevant statistical tests. A logistic regression model assessed influence of independent variables on the risk of inappropriateness. Results: Only the existence of cognitive impairment and the department where the hospitalization takes place were found to be risk factors of inappropriateness. The ranking of inappropriateness according to the causes is the same in the two services, yet with statistically different rates, in particular for causes related to waiting for admission in subacute or long-term care institutional network and for a service provided outside the hospital where the patient was admitted. In the two departments, over 25% of the inappropriate days were related to a patient''s or his family''s choice. Conclusion: Access to subacute or long-term care institution is the first cause of inappropriate hospital use in the two departments. The importance of the rate of inappropriate days related to a choice of the patient or his family was probably a Geriatric specificity. Furthermore, in view of reducing the inappropriate hospital use, attention should be particularly paid on patients with cognitive impairment. [Copyright &y& Elsevier]

Introduction: The fragmentation of the French health and services system has a negative impact on the quality of the services offered because dependant community-dwelling older persons receive services from a mean of three interveners, and 25% of the most dependent ones perceive assistance from at least 6 interveners. In 2006, the Ministry of Health funded an experimentation of implantation of an evidence-based model of integration based on case management: PRISMA. Aims and objective: An implementation evaluative research was associated with the experimentation undertaken by an international (France, Quebec) and multidisciplinary (medical doctors, sociologists, economists) group of research. Methods: This evaluation is carried out continuously all along the implementation process. It is essentially based on research-action framework. A multi-case qualitative analysis is performed on three sites contrasted in regard of the health and services local organisation: rural, urban and megalopolitan. Results: Initial implementation seems to be low during the first 18 months. First objective signs of integration after 12 months were the emergence of a coordination process on a strategic level, the adoption of a unique assessment tools by all the partners and the project of constituting a multi-professional team of case managers. Introduction: The PRISMA implementation research has used an innovative tool: the penetration rate of integrated model in the system. The experimentation in France of the PRISMA adopts and adapts this methodology. Aims and objective: This measurement relates to the functions of the 6 tools and mechanisms of the model: coordination boards, single entry point, case-management, single assessment instrument, individualised services plan and information system. In contrast with Quebec, no public policy in France incites to move towards this model of integration. In addition, in the French system the construction of coordination boards is complex because there is as much as ten supervision authorities and about fifteen organisation managers involved in each experimental site. Methods of adaptation of the tool in France: The measurement was not only the absence, partial existence or existence of the 6 components but takes into account the appropriation process for each one, the administrative time and the new definition of thresholds to reach as compared to the model. Results: This research tool presents two advantages. The measurement scale enables also the partners to appropriate the function of each model component and more precisely the global model of the integration. Two cases of invasive aspergillosis (IA) in immunocompetent patients with a fulminant fatal outcome are reported. Both patients were elderly and had a history of chronic lung disease treated with prolonged inhaled corticosteroids and a short course of systemic corticosteroids. They presented with dyspnea and fever, their respiratory function deteriorated rapidly, and they died 7 days after admission. Aspergillus fumigatus was cultured from respiratory samples. IA was confirmed in one case by necropsy that showed diffuse bilateral necrotizing pneumonitis and myocarditis. In the other case, IA diagnosis was established by thoracic CT scan plus detection of Aspergillus antigen in two blood samples. These two cases demonstrate that short-term corticosteroid therapy in immunocompetent patients with underlying chronic lung conditions is a risk factor for IA, and that its evolution can be fulminant. La dénutrition est une pathologie fréquente chez les résidents d'Établissement pour personnes âgées dépendantes (EHPAD) et est associée à un risque accru de chutes, d'hospitalisations et de décès. Le Score d'évaluation facile des ingesta (SEFI®) est une méthode d'évaluation semi-quantitative de la prise alimentaire et du risque de dénutrition. Évaluer : (i) la reproductibilité des portions alimentaires consommées au moment du déjeuner avec le SEFI® par un évaluateur sur place et par le SEFI assisté par photographie (SEFI®-AP) (principal) ; ii) si le SEFI® pouvait prédire la survenue de chutes, d'hospitalisations et de décès ; iii) le caractère appétissant des plats par la réalisation de questionnaires complétés par les résidents, les infirmier(e)s, les aide-soignant(e)s et les familles des résidents. Il s'agit d'une étude prospective, non interventionnelle, menée dans 4 EHPAD françaises pendant 2 mois. Le SEFI® a été déterminé pour chaque résident inclus, au début de l'étude (M0), à 1 mois (M1) et à 2 mois (M2) par un évaluateur sur place. Des photographies de l'assiette du plat principal ont été prises avant et après consommation du repas du midi, puis analysées par 2 évaluateurs externes qui ont déterminé le SEFI®-AP. Chaque évaluateur a déterminé un SEFI® entre 0 (pas de consommation du plat) et 10 (plat consommé en entier). Des coefficients de corrélation intraclasse ont été calculés entre SEFI® et SEFI®-AP. Des aires sous courbes (AUC), sensibilité, spécificité, valeurs prédictives positive (VPP) et négative (VPN) ont été calculées afin de déterminer le risque de chute, d'hospitalisation et de décès. Des questionnaires relatifs à l'appréciation des plats ont été complétés par des résidents, des aide-soignant(e)s, des infirmier(e)s et des familles de résidents. Au total, 79 résidents d'EHPAD ont été inclus et analysés. À M0, M1 et M2, les coefficients de corrélation intraclasse sont compris entre 0,91 et 0,96. À M2, 7 résidents ont été hospitalisés, 20 chutes ont été rapportées, mais aucun décès. Le SEFI®-AP à M0 n'était pas corrélé au risque de chute (AUC = 0,56) ni au risque d'hospitalisation (AUC = 0,61) à M2. SEFI®-AP ≥ 8 à M0 avait des sensibilités, spécificités, VPP et VPN de 75 %, 46 %, 32 % et 84 %, respectivement, sur la survenue de chute. SEFI®-AP ≤ 9 à M0 avait des sensibilités, spécificités, VPP et VPN de 71 %, 53 %, 13 % et 95 %, respectivement, sur la survenue d'une hospitalisation. Le caractère appétissant était jugé comme mauvais pour 58 %, 47 %, 42 % des aide-soignant(e)s, infirmier(e)s et familles, respectivement. Les portions étaient considérées comme trop copieuses par 16 % des résidents. L'excellente reproductibilité entre SEFI® et SEFI-AP® montre que l'utilisation de la photographie est une alternative valide pour déterminer les ingesta par le SEFI®. L'évaluation des prises alimentaires pourrait donc être externalisée afin de pallier le manque de disponibilité du personnel soignant sur place. De nouvelles études prospectives avec une puissance suffisante sont nécessaires afin de déterminer si le SEFI® pourrait prédire la survenue de chutes, d'hospitalisations ou de décès. Résumé Contexte La conciliation médicamenteuse (CM) est un processus formalisé qui prend en compte, lors d’une nouvelle prescription, tous les médicaments pris et à prendre par le patient. Dans cette étude, une fiche de conciliation de sortie (FCS) a été réalisée puis remise au patient à sa sortie. Elle contient le traitement de sortie et les modifications effectuées par rapport à l’ordonnance d’admission, avec leurs justifications. L’objectif de cette étude est d’évaluer la perception des médecins généralistes vis-à-vis de cette fiche afin d’y apporter des améliorations. Matériels et méthodes Il s’agit d’une étude prospective observationnelle. Des entretiens individuels semi-directifs ont été réalisés auprès de médecins généralistes ayant reçu une FCS suite à l’hospitalisation d’un de leurs patients. Les réponses ont été regroupées par thèmes et ont fait l’objet d’une analyse descriptive. Résultats D’octobre 2015 à juillet 2016, 33 FCS ont été réalisées. Parmi les 33 médecins généralistes interrogés, 16 ont revu un patient avec sa FCS. Cette dernière a été très appréciée par les médecins généralistes et permet de fluidifier le parcours de soin. Toutefois, cette étude a mis en évidence des difficultés de transmission de cette fiche, liées notamment au manque d’information du médecin et du patient ainsi qu’au format papier qui paraît inadapté. La principale amélioration proposée est la transmission de cette fiche en temps réel par messagerie électronique. Conclusion L’avis des médecins interrogés est en faveur de l’usage de la FCS. Certains points restent néanmoins à améliorer, notamment une meilleure information du médecin et du patient ainsi que la transmission par messagerie électronique sécurisée. Background Medication reconciliation (MR) is a systematic and comprehensive review of all medication a patient is taking. In this study, a discharge medication sheet (DMS) is given to patients upon discharge: it contains discharge prescription and any changes made to admission prescription with justifications. The aim of this study is to explore general practitioners’ (GP) perceptions of this DMS in order to suggest improvements. Methods In this prospective observational study, individual semi-directed interviews were conducted with GPs who received a DMS following the hospitalization of one of their patients. Answers were grouped by topic and subjected to descriptive analysis. Results Between October 2015 and July 2016, 33 DMS were completed. Among the 33 GPs, 16 had seen their patients with their DMS and agreed to be interviewed. The DMS was very appreciated and improved care pathway. However, this study highlights transmission difficulties for this sheet, attributed in particular to a lack of information of practitioners and patients and to the paper format, which appears to be inadequate. The main suggested improvement is real-time transmission of the DMS via email. Conclusion Practitioners’ opinion is in favor of the use of a DMS. Certain specific points need to be improved, such as better information of practitioners and patients, and transmission of the DMS via a secure email system. [ABSTRACT FROM AUTHOR]

La dénutrition est fréquente chez les personnes de 70 ans et plus résidant en EHPAD et affecte leur pronostic. Identifier l'insuffisance de prise alimentaire (PA) en EHPAD est un moyen de dépistage de la dénutrition. Le Score d'évaluation facile des ingesta (SEFI®) a été développé. Les objectifs étaient d'évaluer la reproductibilité dans le temps d'une évaluation ponctuelle de la PA par le SEFI® (principal) et la performance du SEFI® pour diagnostiquer la dénutrition en EHPAD (secondaire). Les portions alimentaires consommées au déjeuner et dîner ont été mesurées tous les jours 5 jours/7 par SEFI® assisté par photographie chez 73 résidents d'une seule EHPAD durant 4 semaines, soit 20 jours. Le SEFI® est un score notant les portions consommées de 0 (rien n'est consommé) à 10 (tout est consommé) en chiffre entier. Un SEFI® < 7 définit une insuffisance de PA. Le SEFI® a été établi rétrospectivement à distance du suivi alimentaire à partir des photographies des plateaux repas avant et après la PA. Une évaluation nutritionnelle complète (incluant poids et perte de poids, indice de masse corporelle (IMC), circonférence du mollet (CM), et les mesures de la composition corporelle par bioimpédancemétrie et de la fonction musculaire) à la recherche d'une dénutrition et d'une sarcopénie a été réalisée selon les recommandations HAS 2021. Vingt-sept patients (38,6 %) avaient une insuffisance de PA au 3e jour (j3) du suivi. L'IMC (24,2 ± 5,0 vs 27,4 ± 5,4, p = 0,02) et la CM (31,1 ± 4,3 vs 34,5 ± 5,1 cm, p = 0,005) étaient significativement plus bas chez les résidents avec un SEFI® < 7 à j3. Les médianes des SEFI® des déjeuners (p = 0,11) ou des moyennes des déjeuner-dîner (p = 0,15) ne variaient pas de manière significative au cours du temps. Le SEFI® moyennes des déjeuner-dîner à j3 permettait de prédire le diagnostic d'insuffisance de PA des jours 4 à 20 avec une sensibilité de 78 % (intervalle de confiance à 95 % (IC95 %), 62–94), une spécificité de 30 % (17–44), une valeur prédictive positive de 41 % (28–55), et une valeur prédictive négative de 68 % (48–89). Selon les méthodes utilisées, entre 25 et 33 patients (36–48 %) avaient une dénutrition et entre 16 et 25 patients (24–37 %), une sarcopénie. La performance diagnostique du SEFI® < 7 à j3 pour le diagnostic de dénutrition en mesurant la masse musculaire avec la CM était correcte : AUC = 0,71 (IC95 %, 0,58–0,83). La PA des résidents d'EHPAD est stable sur un mois. Les performances du SEFI® d'un jour donné pour prédire l'insuffisance de prise PA au cours du mois suivant et pour le diagnostic de la dénutrition sont correctes. Les patients âgés peuvent présenter des effets indésirables liés aux médicaments (EIM) prescrits au cours de l'hospitalisation, qui peuvent entraîner des passages aux urgences et des réhospitalisations non programmées. Malgré le fait que la conciliation médicamenteuse (CM) a démontré son efficacité dans la réduction des erreurs de prescriptions causant des EIM, son efficacité dans la réduction de la consommation de soins est controversée. L'objectif principal de cette étude était d'évaluer l'efficacité de la conciliation médicamenteuse de sortie (CMS) sur les réhospitalisations ou passages aux urgences liés aux EIM pour les patients âgés de plus de 65 ans. Il s'agit d'une étude prospective, observationnelle et multicentrique comparant deux groupes de patients: ceux ayant bénéficié d'une conciliation médicamenteuse à l'entrée (CME) uniquement et ceux ayant bénéficié d'un processus complet de CM, c'est-à-dire une CME et une CMS. Entre juin et novembre 2019, tous les patients hospitalisés dans l'un des 12 services participants (médecine interne, gériatrie, pneumologie, soins de suite et de réadaptation) répartis dans cinq hôpitaux, âgés d'au moins 65 ans et ayant bénéficié d'une CME étaient inclus. Les données cliniques et les médicaments des patients étaient recueillis via le dossier médical, l'interrogatoire du patient et/ou des proches et des auto-questionnaires. A la sortie d'hospitalisation, les patients étaient divisés en deux groupes selon qu'ils avaient pu avoir une CMS ou non puis ils étaient recontactés par téléphone à 7 et 30 jours. S'ils avaient été réhospitalisés et/ou admis aux urgences, les dossiers médicaux étaient revus par un comité d'adjudication en aveugle du groupe du patient pour déterminer si cela avait été causé par un EIM ou non. Le critère de jugement principal était le nombre de patients décédés, réhospitalisés ou admis aux urgences du fait d'un EIM dans les 30 jours suivant la sortie d'hospitalisation. Les critères secondaires comportaient le vécu de sa sortie d'hospitalisation par le patient, sa connaissance de ses traitements et la transmission d'informations aux professionnels de ville. Au total, 3557 patients ont été hospitalisés pendant la période de l'étude, 441 ont été inclus et 64 exclus: 156 avaient reçu une CME uniquement et 221 une CME et une CMS. Les patients ayant reçu une CM complète n'étaient pas moins réhospitalisés ou décédés à 30 jours du fait d'un EIM (OR = 1.6 [0,7 to 3,6]) ou quelle qu'en soit la cause (p = 0,96) comparativement au groupe ayant reçu uniquement la CME. Cependant, la CMS améliorait significativement le vécu de la sortie d'hospitalisation du patient et la transmission d'informations à son pharmacien d'officine. Malgré cela, la connaissance de ses traitements par le patient restait insuffisante. Cette étude n'a pas démontré d'efficacité de la CMS pour réduire les consommations de soins 30 jours après la sortie d'hospitalisation mais elle a amélioré significativement le vécu du patient à la sortie d'hospitalisation. Cependant, sa connaissance de ses traitements restait insuffisante. Une meilleure implication des professionnels de santé de ville et une transmission d'informations plus efficace entre l'hôpital et la ville paraissent nécessaires pour démontrer un potentiel impact clinique de la CM. Résumé: Propos: La gestion de cas est une pratique professionnelle récente en France. Les gestionnaires de cas ont pour mission d’évaluer les besoins de personnes âgées dont la situation à domicile est jugée particulièrement complexe et de coordonner les services nécessaires. Un des éléments de complexité, fréquent, réside dans le refus des services nécessaires. L’objectif de cette étude est de confronter le discours des gestionnaires de cas et celui des usagers sur la question du refus de soins, afin de mieux comprendre sa signification, les attitudes des professionnels et les enjeux éthiques sous-jacents. Méthode: Deux chercheurs ont coopéré pour cette recherche qualitative : l’un, anthropologue, a interrogé 19 personnes suivies en gestion de cas et 11 de leurs proches. L’autre, gériatre et chercheur en éthique médicale, a réalisé quatre groupes de discussion ciblés rassemblant au total 18 gestionnaires de cas. Résultats: Le refus de soin traduit souvent une volonté de la personne de préserver son identité, fragilisée par la maladie. Elle cherche ainsi à reprendre le contrôle de sa vie. Face à cette attitude, les gestionnaires de cas tentent de sécuriser la personne en établissant une relation personnelle, respectueuse de ses choix, quitte à différer certains soins. Le refus de soins peut aussi révéler un désir de disparaître, qui confronte le professionnel à ses propres limites. Conclusion: Reconnaître une personne âgée en refus de soin comme un individu unique capable de faire des choix sécurise son identité et peut lui permettre d’accepter certaines aides. Objective: To describe intensive care unit referral decisions by emergency room physicians in patients aged > or =80 yrs.Design: Prospective, observational cohort study of patients aged > or =80 yrs who were triaged in the emergency room, using a list of intensive care unit admission criteria selected by emergency physicians among 76 preliminary criteria adapted from the 1999 Society of Critical Care Medicine guidelines. The Delphi method was used to select the criteria.Setting: Fifteen French hospitals.Patients: A total of 2646 patients aged > or =80 yrs with at least one criterion.Interventions: None.Measurements and Main Results: In the Delphi process, level of agreement was assessed as follows: when all answers fell within a single interval (7-9 = definite admission criteria; 4-6 = equivocal admission criteria or 1-3 = inappropriate admission), agreement was strong; when answers spanned two intervals, agreement was fair; and when answers spanned all three intervals, agreement was poor. Of the 76 preliminary criteria, two were removed; 44 were selected as definite intensive care unit admission criteria; and 30 were selected as equivocal intensive care unit admission criteria. Of the 1426 patients meeting definite admission criteria, 441 (30.9%) were referred for intensive care unit admission and 231 of 441 (52.4%) were admitted to the intensive care unit. Of the 1041 patients with equivocal admission criteria, 181 (17.3%) were referred for intensive care unit admission; and, of these, 79 (43.6%) were admitted to the intensive care unit. Factors associated independently with no intensive care unit referral were age odds ratio [OR], 1.04; 95% confidence interval [CI], 1.04-1.07), active cancer (OR, 1.61; 95% CI, 1.09-1.38), unknown hospitalization status (OR, 1.53; 95% CI, 1.11-2.11), unknown living arrangements (OR, 1.69; 95% CI, 1.19-2.42), regular psychotropic medications (OR, 1.42; 95% CI, 1.10-1.81), low severity at referral (OR, 0.60; 95% CI, 0.53-0.68), low activity in daily living score (OR, 0.93; 95% CI, 0.88-0.99).Conclusions: Emergency and intensive care unit physicians were extremely reluctant to consider intensive care unit admission of patients aged > or =80 yrs, despite the presence of criteria indicating that intensive care unit admission was certainly or possibly appropriate. OBJECTIVE: To identify factors predictive of rapid cognitive decline (RCD) among elderly subjects aged 75 or over suffering from dementia. METHODS: The analysis concerned 250 patients drawn from the 'Sujet Agé Fragile--Evaluation et Suivi' (SAFES) cohort, presenting a dementia syndrome at inclusion and followed-up for at least 1 year. RCD was defined as the loss of at least 3 points on the Mini-Mental State Examination (MMSE) in the follow-up period of 12 months. All patients underwent a standardised geriatric evaluation. Logistic regression was used to identify factors predictive of RCD. RESULTS: In the study sample, 84 patients (33.6%) presented RCD. The factors identified in multivariate analysis as predictive of RCD were: high level of education (OR = 7.8, 95% CI = [1.9-31.2], p = 0.004), risk of depression (OR = 1.8, 95% CI = [1.02-3.18], p = 0.048, and the initial MMSE score (OR = 1.1, 95% CI = [1.0-1.2], p = 0.002). Among subjects with a main caregiver (n = 177), the predictive factors were malnutrition or risk thereof (OR = 4.2, 95% CI = [1.3-14.1], p = 0.02), risk of a fall (OR = 2.6, 95% CI = [1.1-6.1], p = 0.03, caregiver burden (OR = 2.6, 95% CI = [1.1-6.4], p = 0.04) and initial MMSE score (OR = 1.1, 95% CI = [1.0-1.3], p = 0.004). CONCLUSIONS: As soon as dementia is diagnosed in elderly subjects, information should be collected about the subject's socioeconomic status, nutritional status, risk of falling, mood state, and caregiver burden. BACKGROUND: In France, there is evidence to suggest that 50% of elderly individuals are prescribed psychotropic medications. However, it is known that use of these agents increases the risk of falls, fractures and delirium in older people. OBJECTIVE: To study the consumption of 'potentially inappropriate medication' (PIM) among patients aged >/=75 years, paying particular attention to psychotropic drugs and the factors influencing the use of 'potentially inappropriate psychotropics' (PIPs). METHOD: This was a cross-sectional analysis of a prospective multicentre cohort of 1306 hospitalized French patients aged >/=75 years (the SAFEs [Sujet Agé Fragile: Evaluation et suivi (Frail Elderly Subjects: Evaluation and follow-up)] cohort). The present analysis involved the 1176 patients for whom there was information on the usual treatments being taken in the 2 weeks before hospitalization. The drugs were coded according to the Anatomical Therapeutic Chemical classification; the Beers list as updated in 2003 defined which medications were considered PIPs. Standardized geriatric assessment variables were recorded on inclusion in the study. Logistic regression analysis was performed to identify factors linked to use of psychotropics and PIPs. RESULTS: The mean number of drugs taken was 5.7 +/- 2.9 per patient. Twenty-eight percent of patients took at least one PIM. The number of patients who had taken at least one psychotropic drug in the 2 weeks before hospitalization (mean 1.6 +/- 0.9 psychotropics per patient) was 589 (50.1%). More than half of both the 510 patients with a depressive syndrome and the 543 patients affected by dementia were treated with psychotropics. Multivariate analysis showed that prescription of psychotropics was linked to the presence of a dementia syndrome (odds ratio [OR] = 1.4; 95% CI 1.1, 1.9; p = 0.03), the presence of a depressive syndrome (OR = 1.7; 95% CI 1.3, 2.1; p < 0.001), living in an institution (OR = 2.2; 95% CI 1.5, 3.4; p < 0.001), use of more than five drugs (OR = 3.2; 95% CI 2.5, 4.2; p < 0.001) and Charlson's co-morbidity score >1 (OR = 0.6; 95% CI 0.5, 0.8; p = 0.001). Nineteen percent of all psychotropics prescribed were PIPs. Of these PIPs, 66.5% were anxiolytics, 28.4% were antidepressants and 5.1% were antipsychotics. Use of PIPs in the multivariate analysis was associated only with consumption of more than five drugs (OR = 1.7; 95% CI 1.1, 2.5; p = 0.01). CONCLUSION : PIM use is common among hospitalized older adults in France. The most important determinant of risk of receiving a psychotropic medication or a PIP was the number of drugs being taken. The elderly, who have multiple co-morbidities, complex chronic conditions and are usually receiving polypharmacy, are at increased risk for adverse drug events. These adverse events are often linked to problems that could be preventable such as delirium, depression and falls. Regular review of prescriptions would help optimize prescription of psychotropics in the elderly. The Beers list is a good tool for evaluating PIMs but is too restrictive with respect to psychotropics; in the latter respect, the list could usefully be widened. [ABSTRACT FROM AUTHOR]

OBJECTIVES: To identify early markers of prolonged hospital stays in older people in acute hospitals. DESIGN: A prospective, multicenter study. SETTING: Nine hospitals in France. PARTICIPANTS: One thousand three hundred six patients aged 75 and older were hospitalized through an emergency department (Sujet Agé Fragile: Evaluation et suivi (SAFEs)--Frail Elderly Subjects: Evaluation and follow-up). MEASUREMENTS: Data used in a logistic regression were obtained through a gerontological evaluation of inpatients, conducted in the first week of hospitalization. The center effect was considered in two models as a random and fixed effect. Two limits were used to define a prolonged hospital stay. The first was fixed at 30 days. The second was adjusted for Diagnosis Related Groups according to the French classification (f-DRG). RESULTS: Nine hundred eight of the 1,306 hospital stays that made up the cohort were analyzed. Two centers (n=298) were excluded because of a large volume of missing f-DRGs. Two-thirds of subjects in the cohort analyzed were women (64%), with a mean age of 84. One hundred thirty-eight stays (15%) lasted more than 30 days; 46 (5%) were prolonged beyond the f-DRG-adjusted limit. No sociodemographic variables seemed to influence the length of stay, regardless of the limit used. For the 30-day limit, only cognitive impairment (odds ratio (OR)=2.2, 95% confidence interval (CI)=1.2-4.0) was identified as a marker for prolongation. f-DRG adjustment revealed other clinical markers. Walking difficulties (OR=2.6, 95% CI=1.2-16.7), fall risk (OR=2.5, 95% CI=1.7-5.3), cognitive impairment (OR=7.1, 95% CI=2.3-49.9), and malnutrition risk (OR=2.5, 95% CI=1.7-19.6) were found to be early markers for prolonged stays, although dependence level and its evolution, estimated using the Katz activity of daily living (ADL) index, were not identified as risk factors. CONCLUSION: When the generally recognized parameters of frailty are taken into account, a set of simple items (walking difficulties, risk of fall, risk of malnutrition, and cognitive impairment) enables a predictive approach to the length of stay of elderly patients hospitalized under emergency circumstances. Introduction La conciliation médicamenteuse consiste à établir une liste précise de l’ensemble des traitements pris par le patient lors de son hospitalisation. Elle est désormais recommandée par la HAS et vise à améliorer la continuité des soins. En outre, une étude récente réalisée auprès de médecins généralistes a mis en évidence des défauts importants de transmission d’informations entre les secteurs hospitalier et ambulatoire, notamment concernant l’ordonnance de sortie. Actuellement, la réalisation d’une fiche de conciliation médicamenteuse à la sortie du patient est effectuée dans notre service de médecine interne et est remise au patient à l’intention de son médecin traitant. Néanmoins, la perception du médecin traitant vis-à-vis de cette fiche n’a pas été évaluée et serait intéressante afin d’en déterminer l’intérêt et d’y apporter des améliorations. L’objectif de cette étude est d’évaluer la perception des médecins généralistes vis-à-vis de la fiche de conciliation de sortie effectuée lors de l’hospitalisation de leurs patients. Matériels et méthodes Il s’agit d’une étude qualitative prospective. Des d’entretiens individuels semi-directifs étaient réalisés auprès de médecins traitants ayant reçu une fiche de conciliation suite à l’hospitalisation de l’un de leurs patients. Cette fiche comportait l’ensemble des traitements prescrits à la sortie du patient ainsi qu’une courte note expliquant les raisons des modifications des traitements. Les différentes réponses ont été regroupées par thèmes et ont fait l’objet d’une analyse descriptive. Résultats D’octobre 2015 à juillet 2016, 33 fiches de conciliation de sortie ont été réalisées. Parmi les 33 médecins généralistes contactés, 16 ont revu le patient avec la fiche de conciliation et ont accepté un entretien. La majorité des médecins interrogés soulignent un manque d’information quant aux modifications de traitement effectuées pendant l’hospitalisation. Ils ont apprécié la fiche de conciliation car elle est disponible dès la sortie du patient, justifie les modifications réalisées et permet de diminuer le risque d’erreur médicamenteuse. Ils ont également noté l’intérêt de ce document pour que le patient comprenne les modifications réalisées. Certains médecins ont repéré des erreurs, d’autres ont suggéré des modifications aussi bien sur la forme que sur le contenu. Par ailleurs, un nombre important de médecins généralistes n’avaient pas eu accès à cette fiche car ils n’en connaissaient pas l’existence. Enfin, un accès immédiat à cette fiche dès la sortie, par le patient et via une messagerie électronique sécurisée est souhaité. Discussion/conclusion Cette étude a permis de démontrer l’intérêt de la réalisation d’une fiche de conciliation médicamenteuse à la sortie d’hospitalisation du patient pour améliorer sa prise en charge et de proposer des pistes pour l’optimiser. Une transmission de cette fiche au médecin traitant par messagerie sécurisée, en y associant un résumé d’hospitalisation, est actuellement en cours dans notre service. L’évaluation de cette pratique, à la fois en termes de réduction des erreurs médicamenteuses mais également en termes d’économies de santé, fera l’objet d’une étude ultérieure. Introduction et but de l’étude Nous assurons les consultations diététiques à l’Hôpital de Jour Gériatrique (HDJ), interface de l’hôpital et de la médecine de ville. Les patients présentent des problèmes médico-psycho-sociaux souvent intriqués. Nous savons que pour limiter les risques et le développement d’une sarcopénie, un bon état nutritionnel associé à une activité physique optimale sont préconisés. Nous souhaitons savoir si les patients dépistés sarcopéniques à l’HDJ sont accompagnés et conseillés de manière adaptée. Matériel et méthodes La circonférence du mollet (CM) est mesurée systématiquement pour renseigner le Mini Nutritional Assessment. Nous repérons les patients sarcopéniques sur le critère CM < 31 (Y. Rolland et al, 2003). Nous menons une étude exploratoire sur une période de 4 mois pour savoir si tous les patients repérés sarcopéniques ont bénéficié d’une consultation diététique et d’un bilan kinésithérapique. L’existence d’une éventuelle dénutrition a été mise en parallèle (selon les critères HAS : IMC, albuminémie, MNA) Résultats et Analyse statistique Nous avons considérés 87 patients. Les 28 patients avec un CM < 31 ont tous bénéficié d’une consultation diététique mais seulement 8 ont eu un bilan kinésithérapique. Quinze sont considérés comme dénutris. Ces résultats seront discutés en fonction des recommandations et des critères HAS ainsi que des missions de l’HDJ. Conclusion Pour améliorer l’accompagnement des patients, la prise en charge pluridisciplinaire est nécessaire. Il est convenu qu’une sensibilisation aux risques liés à la sarcopénie sera faite auprès des gériatres. Aussi, la sarcopénie sera signalée explicitement dans les comptes rendus adressés aux médecins afin qu’ils délivrent des conseils ciblés. Dans le suivi des patients sarcopéniques les évaluations diététique et kinésithérapique seront systématiquement prescrites. Purpose: Recent work on diffusion of innovation insists on the importance of the processual dimensions of an innovation's adoption. Context: In the course of a pilot-project aiming to implement three service integration networks for people losing their autonomy, we performed an analysis of the prior conceptions the various actors held of two founding principals: integration and coordination. This research aims to identify the prior conceptions held by the actors and to elucidate the adjustments they do to sustain the adoption of the innovation. Data sources: We conducted 32 interviews with the various key actors pertaining, as well to the strategic, tactical and operational levels of the three different sites. Case description: A content analysis of the conflicts of conception and adjustments elaborated by the adoptants during the course of the interviews was conducted. Conclusion: We notice differences in regards to the operational conceptions of the actors about the founding principles of the innovation. The capacity of working out compromises between these different conceptions is considered as predictive of the adoption of the innovation. Discussion: The adjustments they have elaborated favour a progressive adoption of the innovation and found the next step of the process of diffusion of innovation, which is local appropriation.
