Intelligent Transportation Systems (ITS) are gaining momentum due to the advantages it possesses in congestion-free traffic, reduced probability of accidents, and economical transit thus reducing the transit time, saving human life, and helping the growth of the economy. Autonomous vehicles (AV) are an important part of ITS as these are the actual actuators of the ITS. In AVs, the perception system is particularly important as this provides vital information to the motion and planning system. Any error in perception of the environment by the AV will lead to the failure of the entire ITS. This article provides a thorough review of the recent technological advancements in perception systems. In this work, we have considered peer-reviewed journals and conference proceedings on AV from 2019 onwards. The primary focus is on motion prediction models, object detection, localization, sensor data fusion, sensors, autonomous driving (AD), communication technology and AI. This article also discusses the various challenges faced by perception systems in AD and how communication technology and Artificial Intelligence (AI) can help the perception system to overcome the existing challenges. Background: Despite the many benefits of exclusive breastfeeding to infants and mothers, only 33% of Jamaican infants are exclusively breastfed up to the recommend six months. This study was conducted to identify factors affecting mothers' feeding choices focusing on barriers to exclusive breastfeeding of infants six weeks to less than six months old. Methods: A qualitative study consisting of four focus group discussion sessions was conducted among 22 mothers attending postnatal clinics in western Jamaica from May to August 2016. The transcripts were coded by three independent coders and content analysis conducted to generate themes. Results: Four themes were identified namely, perceived advantages of breastfeeding centered mainly on the benefits of breastfeeding for the infant and mother, perceived barriers of breastfeeding highlighting physical pain and fatigue, supplementing culturally acceptable complementary foods and herbal remedies, and cultural norms including perception of how breastfeeding affects a woman's body, societal sources of breastfeeding information, satiation of infants, and family and other support. Mothers overwhelmingly agreed that breastfeeding was inexpensive, allowed them to bond with their infants and was good for the overall health and intellectual development of the infants. They identified painful nipples, engorged breasts, lack of sleep, physical exhaustion and pressure to return to work as barriers to breastfeeding. Mothers named a number of complementary foods, such as pumpkin, carrots, potato, banana, and chocho (Chayote), that were culturally accepted for feeding infants in Jamaica and discussed herbs that were considered to aid in infants' nutrition and overall health. Other cultural factors that were noted to influence exclusive breastfeeding were mothers feeling that breastfeeding would help their bodies, especially their bellies, go back to their pre-maternity figure, sources of breastfeeding information in the society including the internet, belief that breast milk alone does not satisfy babies, and family and other support. Conclusion: Mothers in this study identified unique challenges to exclusive breastfeeding that if addressed, would help to increase exclusive breastfeeding so that the World Health Organization's exclusive breastfeeding recommendations can be achieved. Context: Increasing emphasis on patient-centered care has led to highlighted importance of shared decision making, which better aligns medical decisions with patient care preferences. Effective shared decision making in metastatic breast cancer (MBC) treatment requires prognostic understanding, without which patients may receive treatment inconsistent with personal preferences. Objectives: To assess MBC patient and provider perspectives on the role of prognostic information in treatment decision making. Methods: We conducted semi-structured interviews with MBC patients and community oncologists and separate focus groups involving lay navigators, nurses, and academic oncologists. Qualitative analysis utilized a content analysis approach that included a constant comparative method to generate themes. Results: Of 20 interviewed patients with MBC, 30% were African American. Academic oncologists were mostly women (60%), community oncologists were all Caucasian, and nurses were all women and 28% African American. Lay navigators were all African American and predominately women (86%). Five emergent themes were identified. (1) Most patients wanted prognostic information but differed in when they wanted to have this conversation, (2) Emotional distress and discomfort was a critical reason for not discussing prognosis, (3) Religious beliefs shaped preferences for prognostic information, (4) Health care professionals differed on prognostic information delivery timing, and (5) Providers acknowledged that an individualized approach taking into account patient values and preferences would be beneficial. Conclusion: Most MBC patients wanted prognostic information, yet varied in when they wanted this information. In its 2003 report, the Institute of Medicine concluded that bias against racial minorities may affect communication or care offered. However, to the authors' knowledge, the role of bias within the context of recruitment of racial and ethnic minorities to cancer clinical trials has not been explored to date. Therefore, the authors assessed the experiences of clinical and research personnel related to factors influencing the recruitment of racial and ethnic minorities for cancer clinical trials. Methods: A total of 91 qualitative interviews were conducted at 5 US cancer centers among 4 stakeholder groups: 1) cancer center leaders; 2) principal investigators; 3) referring clinicians; and 4) research staff. Data analysis was conducted using a content analysis approach to generate themes from the transcribed interviews. Results: Five prominent themes emerged: 1) recruitment interactions with potential minority participants were perceived to be challenging; 2) potential minority participants were not perceived to be ideal study candidates; 3) a combination of clinic‐level barriers and negative perceptions of minority study participants led to providers withholding clinical trial opportunities from potential minority participants; 4) when clinical trial recruitment practices were tailored to minority patients, addressing research misconceptions to build trust was a common strategy; 5) for some respondents, race was perceived as irrelevant when screening and recruiting potential minority participants for clinical trials. Conclusions: Not only did some respondents view racial and ethnic minorities as less promising participants, some respondents reported withholding trial opportunities from minorities based on these perceptions. Some providers endorsed using tailored recruitment strategies whereas others eschewed race as a factor in trial recruitment. The presence of bias and stereotyping among clinical and research professionals recruiting for cancer clinical trials should be considered when designing interventions to increase minority enrollment. Bias may impede the willingness of some clinical and research professionals to offer clinical trial enrollment to minority patients. Because a color‐blind approach may not be effective either, the authors suggest that a tailored recruitment strategy may increase minority participation in cancer clinical trials. Two-dimensional (2D) materials have brought fresh prospects for spintronics, as evidenced by the rapid scientific progress made in this frontier over the past decade. In particular, for charge perpendicular to plane vertical magnetic tunnel junctions, the 2D crystals present exclusive features such as atomic-level thickness control, near-perfect crystallography without dangling bonds, and novel electronic structure-guided interfaces with tunable hybridization and proximity effects, which lead to an entirely new group of spinterfaces. Such crystals also present new ways of integration of atomically thin barriers in magnetic tunnel junctions and an unprecedented means for developing composite barriers with atomic precision. All these new aspects have sparked interest for theoretical and experimental efforts, revealing intriguing spin-dependent transport and spin inversion effects. Here, we discuss some of the distinctive effects observed in ferromagnetic junctions with prominent 2D crystals such as graphene, hexagonal boron nitride, and transition metal dichalcogenides and how spinterface phenomena at such junctions affect the observed magnetoresistance in devices. Finally, we discuss how the recently emerged 2D ferromagnets bring upon an entirely novel category of van der Waals interfaces for efficient spin transmission and dynamic control through exotic heterostructures. This paper presents a reconfigurable architecture for Network-on-Chip (NoC) design based on configuration switches. Reconfiguration is achieved by varying connection pattern between routers, depending on the currently running application. Mapping and reconfiguration strategies have been developed for the proposed architecture. In the mapping phase, cores in the combined application set are mapped to individual routers, minimizing the overall communication cost. In the second phase, for each application, configuration information for the switches (between the routers) are generated to optimize the communication cost further to suit the corresponding application. Exact methods, based on Integer Linear Programming (ILP), have been proposed for both the phases. Since ILP takes a large amount of CPU time, Particle Swarm Optimization (PSO)-based approaches have also been developed. The architecture and mapping strategies have been evaluated against benchmarks, considering communication cost, throughput, latency and network energy before and after reconfiguration. Significant improvements could be achieved via reconfiguration. Compared to the approaches reported in the literature, communication cost, throughput, latency and energy consumption values improve by 23.4%, 4%, 3% and 7%, respectively. Background/objective Quantification of serum hepatitis B antigen (HBsAg) is an important test that marks active infection with hepatitis B and helps in the prediction of the clinical outcome and management of hepatitis B virus (HBV) infection. Correlation with HBV DNA quantitative levels may help in developing strategies for antiviral treatment. This study is aimed to evaluate HBsAg titres in various phase of HBV infection in HBsAg positive patients, and its correlation with HBV DNA viral load levels. Methods 976 HBV related patients were analysed in this retrospective cross-sectional study. Patients were categorised on the basis of the phase of HBV infection: immune tolerant phase (IT, n = 123), immune clearance phase (IC, n = 192), low-replicative phase (LR, n = 476), and HBeAg-negative hepatitis (ENH, n = 185). HBsAg titres were quantified and correlated with HBV-DNA levels and clinical parameters. Results Median HBsAg titres were different between each phases of HBV infection ( P < 0.001): (4.62 log10 IU/ml), IC (3.88 log10 IU/ml), LR (2.76 log10 IU/ml) and ENH (2.94 log10 IU/ml). HBsAg and HBV DNA levels showed significant correlation in the whole group ( r = 0.694, P < 0.001), and this was also observed in different phases of HBV infection. Strong correlation in IT phase ( r = 0.603, P < 0.001) and IC phase ( r = 0.523, P < 0.001), moderate in LR phase ( r = 0.362, P < 0.001) and weak in ENH ( r = 0.110, P = 0.04). No correlation was observed between serum HBsAg levels and biochemical parameters. Conclusion The study demonstrated significant difference in the median baseline values of serum HBsAg titres in different phases of HBV infection and provides additional information in understanding the natural history of HBV-infection. Network-on-Chip (NoC) has been proposed to replace traditional bus based System-on-Chip (SoC) architecture to address the global communication challenges in nanoscale technologies. A major challenge in NoC based system design is to select Intellectual Property (IP) cores for implementing tasks and associate the selected cores to the routers to optimize cost and performance. These are commonly known as the process of core selection and application mapping respectively. In this paper, integrated core selection and mapping problem has been addressed. Mesh architecture has been considered for experimentation. The integrated core selection and mapping problem takes as input the application task graph, topology graph and a core library. It outputs the selected cores for the tasks and their mapping onto the topology graph, such that, all communication requirements of the application are satisfied. The cores present in a core library may perform more than one task and have non-uniform sizes. For this, a technique based on Particle Swarm Optimization (PSO) has been proposed to select cores from the given core library and map the resultant core graph onto mesh based architectures. An efficient heuristic for mapping has also been proposed, which maps the selected cores onto mesh based architectures, considering non-uniform core sizes. Comparisons have been carried out with step-by-step core selection and mapping approach and also with mapping algorithms that exist in the literature. Significant reductions have been observed in terms of communication cost over all the cases. Area comparisons have also been made. On average, improvement of 13.05% in communication cost and 2.07% in area have been observed. The proposed approach has also been compared in dynamic environment and significant reductions in the average network latency could be observed. On average, improvement of 5.48% in average network latency and 15.68% in network throughput has been observed. Comparison of energy consumption has also been done in both the cases. Abstract: Network-on-Chip (NoC) has been proposed as a possible solution to the communication problem in nanoscale System-on-Chip (SoC) design. NoC architectures with optimized application-specific topologies have been found to be superior to the regular architectures in designing Multi-Processor System-on-Chip (MPSoC) solutions. The application specific NoC design problem takes as input the chip floorplan, library of NoC components, and communication requirements between the tasks of the application. It outputs the positions of the routers in the floorplan, such that, all communication requirements of the application are satisfied. This paper presents an Integer Linear Programming formulation of the problem, followed by a heuristic technique based on Particle Swarm Optimization (PSO) for finding the router positions from the set of available positions within the chip floorplan. The goal is to minimize the communication cost between cores, satisfying both the link length and router port constraints. The results have been shown on realistic benchmarks. Comparisons have been carried out with regular mesh and custom architectures having routers positioned at (i) the corners of the cores, (ii) the centers of the cores, and (iii) the intersections of the cores. Significant reductions in communication cost have been observed over all the cases. For smaller benchmarks, the optimum results obtained via ILP matches exactly with those reported by the PSO. Many of the existing router placement policies fail even for these small benchmarks, when restrictions are imposed on permissible link length. This establishes the merit of the PSO formulation. Link and router energy consumption of the synthesized NoC have been compared with regular mesh based architectures. The results show significant reduction in communication cost, area overhead, link energy and router energy in the synthesized NoC over regular mesh topology as well. [Copyright &y& Elsevier]

Abstract: Network-on-Chip (NoC) has emerged as a new paradigm to integrate large number of cores on a single silicon die. This paper presents a detailed study of Mesh-of-Tree (MoT) topology and explores its promise in communication infrastructure design for 2-D NoC. The performance and cost of MoT based NoC have been evaluated and compared with butterfly fat-tree (BFT) and two variants of mesh network for equal number of cores under same bisection width constraint. Simulation results under self-similar traffic show that MoT enjoys the advantage of having better performance than other topologies, whereas, it consumes lesser average packet energy than the mesh network that connects single core to each router. In the area front, MoT occupies almost similar area like mesh network connects single core to each router. The MoT network has also been evaluated under a set of real benchmark applications and compared with the above mentioned topologies. Simulation results under application specific traffic also show the competitive potential of MoT topology in NoC design. Moreover, due to lesser connectivity of the routers, synthesis result shows that MoT network can be operated at higher frequency than others. Taking all these facts into consideration, this paper establishes that like mesh and BFT, MoT can also be applied in designing NoC based systems. This paper also focuses on the limitations of MoT and other tree based topologies in NoC design in current technology and enumerates probable solutions to make them more acceptable. Abstract: Polydispersed ZnO nanoparticles (ZnO1000 and ZnO600) with two different windows of particle size distributions (∼120 and 30nm) were synthesized using citrate gel route and different annealing treatments (1000 and 600°C, respectively). Photocatalytic efficiency of these samples was compared with TiO2 in its commercial form-P25, on two dyes, Methylene blue (MB) and Methyl orange (MO). The X-ray diffraction data showed wrutzite ZnO and anatase and rutile phases of P25. UV–visible absorbance spectra of ZnO1000 showed broad absorption range from UV-to-visible (from 382 to 700nm), as against sharp absorption peaks in UV range for both ZnO600 and P25. The microstructural morphology as seen through scanning electron micrographs showed ZnO1000 with tetrapod-like structures while the ZnO600 showed almost spherical morphologies. Upon subjecting these catalysts to dye solutions in sunlight it was found that both the dyes were completely decolorised within 20min by ZnO1000, as against partial decolorisation by ZnO600 and P25 ( 53% and 78% for MO and 77% and 88% for MB samples). The effect of catalyst loading (from 125mg to 1g) on decolorisation showed that ZnO1000 had good efficiency for all concentrations which was followed by P25 and then by ZnO600. Small perturbations are attributed to the competition between sunlight scattering-induced, reduced irradiation field and the exposed surface area offered by catalyst, which work as active sites for decolorisation. The reusability of the catalysts when studied on fresh dye samples (4 trials), the decolorisation efficiency decreased merely from 99.2% to 99.12% for ZnO1000 as compared to ZnO600 (53.3% to 19.94%) and P25 (78.3% to 31.42%), indicating the efficient reusability of ZnO1000. The effective half life of the catalysts, in terms of number of reuses, were calculated and found to be ∼3 for both ZnO600 and P25 and was >3000 for ZnO1000, which justifies its extremely high reuse. The byproduct analysis (compared with standards prescribed by World Health Organisation (WHO) and Central Pollution Control Board of India (CPCB)) showed cleavage of the chromophore and of other bonds with opening of benzene rings, indicating degradation of the dyes in concurrence with decolorisation, in the stipulated time. Further, cytotoxicity studies performed on SiHa cell lines showed non-toxicity of the byproducts with ZnO1000 as compared to ZnO600 and P25. [Copyright &y& Elsevier]

Background: Chemoprevention, which includes the use of synthetic or natural agents (alone or in combination) to block the development of cancer in human beings, is an extremely promising strategy for cancer prevention. Cinnamon is one of the most widely used herbal medicines with diverse biological activities including anti-tumor activity. In the present study, we have reported the anti-neoplastic activity of cinnamon in cervical cancer cell line, SiHa. Methods: The aqueous cinnamon extract (ACE-c) was analyzed for its cinnamaldehyde content by HPTLC analysis. The polyphenol content of ACE-c was measured by Folin-Ciocalteau method. Cytotoxicity analysis was performed by MTT assay. We studied the effect of cinnamon on growth kinetics by performing growth curve, colony formation and soft agar assays. The cells treated with ACE-c were analyzed for wound healing assay as well as for matrix metalloproteinase- 2 (MMP-2) expression at mRNA and protein level by RT-PCR and zymography, respectively. Her-2 protein expression was analyzed in the control and ACE-c treated samples by immunoblotting as well as confocal microscopy. Apoptosis studies and calcium signaling assays were analyzed by FACS. Loss of mitochondrial membrane potential (Δψm) in cinnamon treated cells was studied by JC-1 staining and analyzed by confocal microscopy as well as FACS. Results: Cinnamon alters the growth kinetics of SiHa cells in a dose-dependent manner. Cells treated with ACE-c exhibited reduced number of colonies compared to the control cells. The treated cells exhibited reduced migration potential that could be explained due to downregulation of MMP-2 expression. Interestingly, the expression of Her-2 oncoprotein was significantly reduced in the presence of ACE-c. Cinnamon extract induced apoptosis in the cervical cancer cells through increase in intracellular calcium signaling as well as loss of mitochondrial membrane potential. Bioactive compounds in plants are essential for the formation of novel chemotherapeutic drugs, which have been used in Ayurveda to treat a variety of illnesses. Indian medicinal herbs have been used for thousands of years to treat a variety of illnesses, such as fever, cancer, snake bites, rheumatism, skin problems, and neurodegenerative diseases. GC–MS was used to locate and categorize bioactive components in Olea dioica leaves. The results showed that presence of octanoic acid, methyl ester, decanoic acid, methyl ester, desulphosinigrin, l-gala-l-ido-octose, methyl tetradecanoate, Tetradecanoic acid, 6-benzoxazolesulfonamide, N-(2-hydroxyethyl)-2-methyl-, 10-chloro-5-methoxy-5H-dibenzo[a,d][7]annulene, pentadecanoic acid, oleic acid, n-hexadecanoic acid, hexanedioic acid, dioctyl ester, and squalene. The methanol extract of Olea dioica was effective against a wide spectrum of pathogenic bacteria at four different concentrations, with the highest activity against Escherichia coli, Klebsiella pneumoniae, Staphylococcus aureus, Xanthomonas campestris, and Salmonella typhimurium. It also showed moderate activity against Agrobacterium tumefaciens, Pseudomonas aeruginosa, Streptomyces pneumonia, and Pseudomonas syringae. The pharmacological properties of O. dioica, as well as their variety and comprehensive phytochemistry, could be exploited as a potent antimicrobial agent for future therapeutics. Sympatico–vagal balance is essential for regulating cardiac electrophysiology and plays an important role in arrhythmogenic conditions. Various noninvasive methods, including electrocardiography (ECG), have been used for clinical assessment of the sympatico–vagal balance. This study aimed to use a custom-designed wearable device to record ECG and ECG-based cardiac function biomarkers to assess sympatico–vagal balance during tonic pain in healthy controls. Nineteen healthy volunteers were included for the ECG measurements using the custom-designed amplifier based on the Texas Instruments ADS1299. The ECG-based biomarkers of the sympatico–vagal balance, (including heart rate variability, deceleration capacity of the heart rate, and periodic repolarization dynamic), were calculated and compared between resting and pain conditions (tonic pain). The custom-designed device provided technically satisfactory ECG recordings. During exposure to tonic pain, the periodic repolarization dynamics increased significantly (p = 0.02), indicating enhancement of sympathetic nervous activity. This study showed that custom-designed wearable devices can potentially be useful in healthcare as a new telemetry technology. The ECG-based novel biomarkers, including periodic repolarization dynamic and deceleration capacity of heart rate, can be used to identify the cold pressor-induced activation of sympathetic and parasympathetic systems, making it useful for future studies on pain-evoked biomarkers. Coronavirus disease-2019 (COVID-19) cases continue to increase globally. Poor outcomes in patients with COVID-19 and cirrhosis have been reported; predictors of outcome are unclear. The existing data is from the early part of the pandemic when variants of concern (VOC) were not reported. We aimed to assess the outcomes and predictors in patients with cirrhosis and COVID-19. We also compared the differences in outcomes between the first wave of pandemic and the second wave. In this retrospective analysis of a prospectively maintained database, data on consecutive cirrhosis patients (n = 221) admitted to the COVID-19 care facility of a tertiary care center in India were evaluated for presentation, the severity of liver disease, the severity of COVID-19, and outcomes. The clinical presentation included: 18 (8.1%) patients had compensated cirrhosis, 139 (62.9%) acute decompensation (AD), and 64 (29.0%) had an acute-on-chronic liver failure (ACLF). Patients with ACLF had more severe COVID-19 infection than those with compensated cirrhosis and AD (54.7% vs. 16.5% and 33.3%, P < 0.001). The overall mortality was 90 (40.7%), the highest among ACLF (72.0%). On multivariate analysis, independent predictors of mortality were high leukocyte count, alkaline phosphatase, creatinine, child class, model for end-stage liver disease (MELD) score, and COVID-19 severity. The second wave had more cases of severe COVID-19 as compared to the first wave, with a similar MELD score and Child score. The overall mortality was similar between the two waves. Patients with COVID-19 and cirrhosis have high mortality (40%), particularly those with ACLF (72%). Recognizing background information in human speech signals is a task that is extremely useful in a wide range of practical applications, and many articles on background sound classification have been published. It has not, however, been addressed with background embedded in real-world human speech signals. Thus, this work proposes a lightweight deep convolutional neural network (CNN) in conjunction with spectrograms for an efficient background sound classification with practical human speech signals. The proposed model classifies 11 different background sounds such as airplane, airport, babble, car, drone, exhibition, helicopter, restaurant, station, street, and train sounds embedded in human speech signals. The proposed deep CNN model consists of four convolution layers, four max-pooling layers, and one fully connected layer. The model is tested on human speech signals with varying signal-to-noise ratios (SNRs). Based on the results, the proposed deep CNN model utilizing spectrograms achieves an overall background sound classification accuracy of 95.2% using the human speech signals with a wide range of SNRs. Traffic flow prediction is an essential part of the intelligent transport system. This is the accurate estimation of traffic flow in a given region at a particular interval of time in the future. The study of traffic forecasting is useful in mitigating congestion and make safer and cost-efficient travel. While traditional models use shallow networks, there has been an exponential growth in the number of vehicles in recent times and these traditional machine learning models fail to work in current scenarios. In our paper, we review some of the latest works in deep learning for traffic flow prediction. Many deep learning architectures include Convolutional Neural Network (CNN), Recurrent Neural Network (RNN), Long ShortTerm Memory (LSTM), Restricted Boltzmann Machines (RBM), and Stacked Auto Encoder (SAE). These deep learning models use multiple layers to extract higher level of features from raw input progressively. The latest deep learning models developed to tackle this very problem are reviewed and due to the complexity of transport networks, this review gives the reader information about how various factors influence these models and what models work best in different scenarios. Background: The simultaneous or intermittent use of alternative treatments and prescription medications for hypertension and type 2 diabetes mellitus can have adverse health effects. Objectives: To identify beliefs and practices associated with the use of alternative treatments for hypertension and type 2 diabetes mellitus among patients. Methods: A mixed-methods study including an investigator-administered survey and focus group discussion sessions using convenience sampling was conducted among patients aged ≥18 years during May to August 2018. Descriptive statistics were used to describe and compare demographic characteristics among groups of survey participants using JMP Pro 14.0. Thematic analysis was conducted to analyze the qualitative data using NVivo. Results: Most study participants (87–90%) were on prescription medication for their condition. Of survey participants, 69% reported taking their medication as prescribed and 70% felt that prescription medicine was controlling their condition. Almost all participants (98%) reported using alternative treatments, mainly herbal medications, and 73–80% felt that herbal medicines controlled their conditions. One-third believed that herbal medicines are the most effective form of treatment and should always be used instead of prescription medication. However, most participants (85%) did not believe that prescription and herbal treatments should be used simultaneously. Most (76–90%) did not discuss herbal treatments with their healthcare providers. Four themes emerged from the focus group sessions: 1) Simultaneous use of herbal and prescription medicine was perceived to be harmful, 2) Patients did not divulge their use of herbal medicine to healthcare providers, 3) Alternative medicines were perceived to be highly effective, and 4) Religiosity and family elders played key roles in herbal use. Conclusions: This study provides useful insights into perceptions and use of alternative treatments by patients that can be used by healthcare providers in developing appropriate interventions to encourage proper use of prescription medicines and alternative medicines resulting in improved management of these chronic diseases. Objective: Shared decision‐making (SDM) occurs when physicians and patients jointly select treatment that aligns with patient care goals. Incorporating patient preferences into the decision‐making process is integral to successful decision‐making. This study explores factors influencing treatment selection in older patients with early‐stage breast cancer (EBC). Methods: This qualitative study included women age ≥65 years with EBC. To understand role preferences, patients completed the Control Preferences Scale. Semi‐structured interviews were conducted to explore patients' treatment selection rationale. Interview transcripts were analyzed using a constant comparative method identifying major themes related to treatment selection. Results: Of 33 patients, the majority (48%) desired shared responsibility in treatment decision‐making. Interviews revealed that EBC treatment incorporated three domains: Intrinsic and extrinsic influences, clinical characteristics, and patient values. Patients considered 19 treatment selection themes, the most prioritized including physician trust and physical side effects. Conclusions: Because preferences and approach to treatment selection varied widely in this sample of older, EBC patients, more research is needed to determine best practices for preference incorporation to optimize SDM at the time of treatment decisions. Amylase inhibitors, also known as starch blockers, contain substances that prevent dietary starches from being absorbed by the body via inhibiting breakdown of complex sugars to simpler ones. In this sense, these materials are projected as having potential applications in diabetes control. In this context, we report on zinc oxide nanoparticles as possible alpha-amylase inhibitors. Zinc oxide nanoparticles have been synthesized using soft-chemistry approach and 1-thioglycerol was used as a surfactant to yield polycrystalline nanoparticles of size ∼18 nm, stabilized in wurtzite structure. Conjugation study and structural characterization have been done using x-ray diffraction technique, Fourier transform infrared spectroscopy, UV-visible spectroscopy, and transmission electron microscopy. Cytotoxicity studies on human fibrosarcoma (HT-1080) and skin carcinoma (A-431) cell lines as well as mouse primary fibroblast cells demonstrate that up to a dose of 20 μg/ml, ZnO nanoparticles are nontoxic to the cells. We report for the first time the alpha-amylase inhibitory activity of ZnO nanoparticles wherein an optimum dose of 20 μg/ml was sufficient to exhibit 49% glucose inhibition at neutral pH and 35 °C temperature. This inhibitory activity was similar to that obtained with acarbose (a standard alpha-amylase inhibitor), thereby projecting ZnO nanoparticles as novel alpha-amylase inhibitors. Background and aim Progression of hepatitis B virus infection (HBV) might be affected by host genetic factors. The present study was undertaken to study the role of glutathione S-transferases (GST)-M1 and T1 gene polymorphisms in different stages of HBV infection: HBV inactive carrier, chronic hepatitis B and cirrhosis, and cryptogenic cirrhosis. Methods The study population comprised of 170 subjects; 120 cases (HBV inactive carrier, n = 30; HBV related chronic hepatitis, n = 30; HBV related cirrhosis, n = 30; cryptogenic cirrhosis, n = 30) and 50 unrelated healthy adults without liver disease as controls. Analysis of GSTM1 and GSTT1 gene polymorphisms was done by multiplex polymerase chain reaction. Results The GSTM1 null genotype was seen more commonly in hepatitis B cirrhosis ( n = 21; 70%), chronic hepatitis B ( n = 19; 63.33%) and cryptogenic cirrhosis ( n = 17; 56.67%) as compared with inactive carrier ( n = 9; 30%) and controls ( n = 13; 26%). The GSTT1 null genotype was seen less frequently in all the groups, the observed frequencies were controls ( n = 7; 14%), inactive carrier ( n = 5; 16.67%), chronic hepatitis B ( n = 8; 26.67%) and hepatitis B cirrhosis ( n = 7; 23.33%). The difference of GSTM1 null genotype frequencies was statistically significant for hepatitis B cirrhosis vs. controls ( P = 0.0002), chronic hepatitis B vs. controls ( P = 0.002) and cryptogenic cirrhosis vs. controls ( P = 0.01). The GSTT1 null genotype was not found to vary significantly between the groups. Conclusion The patients with GSTM1 null genotype are at risk of progression of liver disease as the frequency of GSTM1 null genotype was found to be significantly higher in chronic hepatitis B, hepatitis B cirrhosis and cryptogenic cirrhosis as compared with controls. Context: Advance care planning (ACP) improves alignment between patient preferences for life-sustaining treatment and care received at end of life (EOL).Objectives: To evaluate implementation of lay navigator-led ACP.Methods: A convergent, parallel mixed-methods design was used to evaluate implementation of navigator-led ACP across 12 cancer centers. Data collection included 1) electronic navigation records, 2) navigator surveys (n = 45), 3) claims-based patient outcomes (n = 820), and 4) semistructured navigator interviews (n = 26). Outcomes of interest included 1) the number of ACP conversations completed, 2) navigator self-efficacy, 3) patient resource utilization, hospice use, and chemotherapy at EOL, and 4) navigator-perceived barriers and facilitators to ACP.Results: From June 1, 2014 to December 31, 2015, 50 navigators completed Respecting Choices® First Steps ACP Facilitator training. Navigators approached 18% of patients (1319/8704); 481 completed; 472 in process; 366 declined. Navigators were more likely to approach African American patients than Caucasian patients (20% vs. 14%, P < 0.001). Significant increases in ACP self-efficacy were observed after training. The mean score for feeling prepared to conduct ACP conversations increased from 5.6/10 to 7.5/10 (P < 0.001). In comparison with patients declining ACP participation (n = 171), decedents in their final 30 days of life who engaged in ACP (n = 437) had fewer hospitalizations (46% vs. 56%, P = 0.02). Key facilitators of successful implementation included physician buy-in, patient readiness, and prior ACP experience; barriers included space limitations, identifying the "right" time to start conversations, and personal discomfort discussing EOL.Conclusion: A navigator-led ACP program was feasible and may be associated with lower rates of resource utilization near EOL. Myrrh (guggulu) oleoresin from the Commiphora mukul tree is an important component of antiarthritic drugs in Ayurvedic medicine. Clinical data suggest that elevated levels of hyaluronidase and collagenase type 2 enzymes contribute significantly to cartilage degradation. Triphala guggulu (TG) is a guggulu-based formulation used for the treatment of arthritis. We assessed the chondroprotective potential of TG by examining its effects on the activities of pure hyaluronidase and collagenase type 2 enzymes. Triphala shodith guggulu (TSG), an intermediate in the production of TG, was also examined. A spectrophotometric method was used to assay Hyaluronidase activity, and to detect potential Hyaluronidase inhibitors. Aqueous and hydro-alcoholic extracts of TSG showed weak but dosedependent inhibition of hyaluronidase activity. In contrast, the TG formulation was 50 times more potent than the TSG extract with respect to hyaluronidase inhibitory activity. A validated X-ray film-based assay was used to measure the gelatinase activity of pure collagenase type 2. Hydro-alcoholic extracts of the TG formulation were 4 times more potent than TSG with respect to collagenase inhibitory activity. Components of Triphala were also evaluated for their inhibitory activities on hyaluronidase and collagenase. This is the first report to show that the T2 component of Triphala (T. chebula) is a highly potent hyaluronidase and collagenase inhibitor. Thus, the TG formulation inhibits two major enzymes that can degrade cartilage matrix. Our study provides the first in vitro preclinical evidence of the chondroprotective properties of TG. This is the first report describing two novel chondroprotective activities of aqueous extracts of Withania somnifera root powder. First, these extracts had a statistically significant, short-term chondroprotective effect on damaged human osteoarthritic cartilage matrix in 50% of the patients tested. Background: Chemoprevention, which includes the use of synthetic or natural agents (alone or in combination) to block the development of cancer in human beings, is an extremely promising strategy for cancer prevention. Cinnamon is one of the most widely used herbal medicines with diverse biological activities including anti-tumor activity. In the present study, we have reported the anti-neoplastic activity of cinnamon in cervical cancer cell line, SiHa.Methods: The aqueous cinnamon extract (ACE-c) was analyzed for its cinnamaldehyde content by HPTLC analysis. The polyphenol content of ACE-c was measured by Folin-Ciocalteau method. Cytotoxicity analysis was performed by MTT assay. We studied the effect of cinnamon on growth kinetics by performing growth curve, colony formation and soft agar assays. The cells treated with ACE-c were analyzed for wound healing assay as well as for matrix metalloproteinase-2 (MMP-2) expression at mRNA and protein level by RT-PCR and zymography, respectively. Her-2 protein expression was analyzed in the control and ACE-c treated samples by immunoblotting as well as confocal microscopy. Apoptosis studies and calcium signaling assays were analyzed by FACS. Loss of mitochondrial membrane potential (Deltapsim) in cinnamon treated cells was studied by JC-1 staining and analyzed by confocal microscopy as well as FACS.Results: Cinnamon alters the growth kinetics of SiHa cells in a dose-dependent manner. Cells treated with ACE-c exhibited reduced number of colonies compared to the control cells. The treated cells exhibited reduced migration potential that could be explained due to downregulation of MMP-2 expression. Interestingly, the expression of Her-2 oncoprotein was significantly reduced in the presence of ACE-c. Cinnamon extract induced apoptosis in the cervical cancer cells through increase in intracellular calcium signaling as well as loss of mitochondrial membrane potential.Conclusion: Cinnamon could be used as a potent chemopreventive drug in cervical cancer.
