Aim: To examine the actual conditions of older patients receiving home medical care after hospitalization over a period of 2 years in Japan. Methods: The study population included 102 participants, aged ≥65 years, receiving home medical care, who consented to participate in the Osaka Home Care Registry (OHCARE) study in Japan over a period of 2 years. We investigated the actual conditions for returning home after hospitalization. Results: The median age of the 102 participants was 84 years, and 61 (59.8%) were women. In the group that returned home, 42 (55.3%) of the respondents desired to recuperate in a familiar place, as in advanced care planning (ACP). During the 2‐year follow‐up period, the group that did not return home had significantly more deaths. A multivariate analysis showed the association in the presence of ACP (odds ratio: 4.72, 95% confidence interval: 1.60–13.86) and cardiac disease (odds ratio: 0.25, 95% confidence interval: 0.08–0.76). The lack of ACP in the medical records when the patient was admitted to the hospital may have prevented the return home. Conclusion: In older patients who had difficulty returning home after hospitalization, the lack of ACP in home medical care may have been an influencing factor. We present the results of relocating 17,544 hypocenters determined from data recorded during the 2016 Kumamoto Earthquake sequence, during the interval between April 14, 2016, and August 31, 2016. For this, we used a double-difference relocation method to constrain high-resolution hypocenter locations by cross-correlation differential times as well as the NIED Hi-net catalog differential times. The sequence included two large events (on 14 April: M 6.5 and on 16 April: M 7.3) that occurred in a complicated region where the Hinagu and Futagawa faults meet. By comparing these high-resolution earthquake locations in three different periods [(P1) between 2001 and 2012; (P2) between M 6.5 and M 7.3; and (P3) between M 7.3 and August 31, 2016], we present the significant seismicity after the mainshock relative to the background seismicity. Events during the Kumamoto Earthquake sequence occurred generally within the same sites of known faults and background seismicity. For an example, the seismicity during period P2 formed a sharp linear shape along the northern part of the Hinagu fault for about 20 km. A series of linear seismicity events occurred during period P3 along the Futagawa fault to the east (for about 28 km), in the northern part of the Aso caldera, and in the Oita region around the Beppu-Haneyama fault zone. These events also extended to the mid- and southern parts of the Hinagu fault zone and were shaped only after the M7.3 event. Moreover, high-resolution hypocenter locations also allowed us to identify some clusters of events that occurred in regions where background seismicity has not been confirmed. For instance, activity on the northwestern edge of the Aso caldera and in small areas within the Beppu-Haneyama fault zone became apparent with new seismic activity. We also demonstrate herein the absence of seismicity between the northeast extension of the Futagawa fault zone and the Aso caldera region, which became clearly shown after the M7.3 event. This low-seismicity region is located at the boundary of the low- and high-velocity structures and different focal mechanisms, but is also close to the maximum slip area of the M7.3 event. We have generated a high-resolution catalog called the “Japan Unified hIgh-resolution relocated Catalog for Earthquakes” (JUICE), which can be used to evaluate the geometry and seismogenic depth of active faults in Japan. We relocated > 1.1 million hypocenters from the NIED Hi-net catalog for events which occurred between January 2001 and December 2012, to a depth of 40 km. We apply a relative hypocenter determination method to the data in each grid square, in which entire Japan is divided into 1257 grid squares to parallelize the relocation procedure. We used a double-difference method, incorporating cross-correlating differential times as well as catalog differential times. This allows us to resolve, in detail, a seismicity distribution for the entire Japanese Islands. We estimated location uncertainty by a statistical resampling method, using Jackknife samples, and show that the uncertainty can be within 0.37 km in the horizontal and 0.85 km in the vertical direction with a 90% confidence interval for areas with good station coverage. Our seismogenic depth estimate agrees with the lower limit of the hypocenter distribution for a recent earthquake on the Kamishiro fault (2014, Mj 6.7), which suggests that the new catalog should be useful for estimating the size of future earthquakes for inland active faults. In view of significant discrepancy between the slip models of the 2009 April 6 L'Aquila, (Italy) earthquake based on seismic waveforms or geodetic data, a new inversion strategy is proposed to constrain the coseismic rupture model and early post-seismic afterslip model of this earthquake. Both models were jointly inverted simultaneously from the near-source strong ground motion recordings, teleseismic P waves, long-period Rayleigh waves, GPS displacement vectors and InSAR line-of-sight displacement image. The deformations are mapped onto the fault with strike 140° and dip 50° to the southwest. The coseismic period here represents the first 10 s after the rupture initiation, when over 99 per cent of our total seismic moment (3.07 × 1018 Nm) is already released in our preferred coseismic model. Most coseismic slip occurs in a small rupture area of approximately 16 km along strike and in a depth range of 3–10 km. The average slip is 0.45 m; peak slip reaches about 0.90 m. Our result reveals that L'Aquila rupture has a lag of 0.6 s before it first propagates updip to break an asperity equivalent to an Mw 5.7. And then the rupture propagates along strike to break a large patch located about 8.0 km along strike and 4.0 km updip from the hypocentre. The weighted average of the rise time and slip velocity are 0.71 s and 0.77 m s−1, respectively. Our estimate of radiated seismic energy is 1.67 × 1013 J, yielding an energy to seismic moment ratio of 0.55 × 10−5, considerably smaller than the global average for shallow earthquakes. The early post-seismic period here represents the time window from 10 s to about first day. During this period, our preferred post-seismic model has an accumulated seismic moment of 6.0 × 1017 Nm, equivalent to an Mw of 5.8. The afterslip distribution features two separated high slip fault patches, locating either near the hypocentre or the region with peak coseismic slip. Their locations only partially overlap with the fault patches with significant afterslip after the first day. In particular, the slip distribution of the shallower patch is quantitatively consistent with the Amoruso and Crescentini’ diffusive afterslip model. We present an approach for seismic noise which improves Rayleigh-wave ellipticity estimates by reducing the influences from body waves and Love waves. The method requires three-component seismographs and uses the phase-shift information between the vertical and horizontal components. We select data that show the phase-shift of about 90° in order to separate Rayleigh waves from body waves and Love waves. In comparison to the usual H/V approach, the estimates for Rayleigh-wave ellipticity are systematically different by about 20 per cent because the existence of S waves and Love waves systematically increases horizontal amplitudes. The differences in the inverted S-wave velocity structure reach up to about 20 per cent in the upper 20 km. The method is limited to a relatively low-frequency range, below 0.3 Hz, as the 90° phase-shift peaks become difficult to identify above 0.3 Hz. Despite this limitation in the frequency range, this approach can improve the S-wave velocity structure in the upper 5–10 km, which should lead to a better prediction of long-period ground motions for periods of about 5–10 s. An improved prediction of such long-period ground motion is important for hazard mitigation in the world's metropolitan areas with high-rise buildings. This approach is applied to the structure in the broader Los Angeles basin in Southern California and improvement of the reference velocity structure in the upper 5–10 km is demonstrated. The particle motion of surface waves, in addition to phase and group velocities, can provide useful information for S-wave velocity structure in the crust and upper mantle. In this study, we applied a new method to retrieve velocity structure using the ZH ratio, the ratio between vertical and horizontal surface amplitudes of Rayleigh waves. Analysing data from the GEOSCOPE network, we measured the ZH ratios for frequencies between 0.004 and 0.05 Hz (period between 20 and 250 s) and inverted them for S-wave velocity structure beneath each station. Our analysis showed that the resolving power of the ZH ratio is limited and final solutions display dependence on starting models; in particular, the depth of the Moho in the starting model is important in order to get reliable results. Thus, initial models for the inversion need to be carefully constructed. We chose PREM and CRUST2.0 in this study as a starting model for all but one station (ECH). The eigenvalue analysis of the least-squares problem that arises for each step of the iterative process shows a few dominant eigenvalues which explains the cause of the inversion's initial-model dependence. However, the ZH ratio is unique in having high sensitivity to near-surface structure and thus provides complementary information to phase and group velocities. Application of this method to GEOSCOPE data suggest that low velocity zones may exist beneath some stations near hotspots. Our tests with different starting models show that the models with low-velocity anomalies fit better to the ZH ratio data. Such low velocity zones are seen near Hawaii (station KIP), Crozet Island (CRZF) and Djibuti (ATD) but not near Reunion Island (RER). It is also found near Echery (ECH) which is in a geothermal area. However, this method has a tendency to produce spurious low velocity zones and resolution of the low velocity zones requires further careful study. We also performed simultaneous inversions for volumetric perturbation and discontinuity-depth perturbation. While its formulation and inversion were straightforward, there seemed to be a difficult trade-off problem between volumetric perturbation and discontinuity-depth perturbation. The 2018, Hokkaido Eastern Iburi, Japan, earthquake is an event characterized by complexity of the rupture process and slip pattern, which may involve both reverse and strike-slip motion depending on the locations on the fault surface. We perform dynamic rupture simulations based on simple physical laws, conditions for stressing and fault friction, and the non-planar fault geometry constrained by the aftershock observation. The complex fault geometry is numerically treated by the boundary integral equation method accelerated by the fast domain portioning method. The fault geometry is characterized primarily by the combination of six fault planes. As a result, we are able to explain several observed features of the event, including the spatial variation of the final fault slip and rupture velocity, which are inferred from the kinematic slip inversion. We also succeed in refining the constraint of the regional stress field in the focal area based on the simulation. Our results show that the overall patterns of the complex rupture event can be reproduced by a relatively simple model of the regional stress and the fault friction, if the geometrical complexity of the fault is properly taken into account. Sichuan pepper (Zanthoxylum bungeanum, HJ), a spice widely used in China, has antioxidative, anti-inflammatory, antibacterial, and anti-obesity properties. In this study, to confirm the value of HJ as a functional food, the in vitro antioxidant and bile acid-lowering capacities, as well as the effects on caecal microbiota, were compared with those of cumin (Cuminum cyminum, CM) and coriander (Coriandrum sativum, CR) seeds in Institute of Cancer Research (ICR) mice fed a high-sucrose and low-dietary fibre diet. The total phenolic content, superoxide anion radical-scavenging capacity, and Fe-reducing power of the HJ aqueous solution were higher than those of CM and CR (p < 0.05). The bile acid (taurocholic, glycocholic, and deoxycholic acids)-lowering capacity of the HJ suspension was also higher than those of CM and CR. Compared with mice fed a control diet (no fibre, NF), caecal Lactobacillus gasseri- and Muribaculum intestinale-like bacteria were higher in mice fed a diet containing 5% (w/w) of CM, CR, or HJ for 14 days. Bifidobacterium pseudolongum-, Lactobacillus murinus/animalis-, and Faecalibaculum rodentium-like bacteria were significantly increased, while Desulfovibrio-like bacteria were significantly decreased in the HJ group. In addition, CM and HJ may benefit specific metabolic functions of gut microbiota, such as starch, sucrose, and tyrosine metabolism. The tumour necrosis factor (TNF-α) concentration in the spleen tissue of ICR mice was decreased by the intake of spices. However, there were no changes in interleukin-2 (IL-2) and IL-10 levels in HJ fed mice. These results suggested that HJ has potential as a functional food related to gut microbiota. Abstract: A π-expanded BODIPY dye with an intramolecular boronate skeleton was synthesized by retro Diels–Alder reaction of the bicyclo[2.2.2]octadiene (BCOD)-fused BODIPY and the subsequent O-chelation. The photophysical and electrochemical properties were examined by UV–vis, fluorescence, CV measurements, and DFT calculation. This BODIPY exhibited the absorption and emission over a visible–NIR region at 600–850 nm. O-chelated BODIPY showed a bathocromic shift compared to F-BODIPYs. This dye showed a bright fluorescence emission at 733 nm with the high Φ value of 0.58. [Copyright &y& Elsevier]

Background: Receptor tyrosine kinases (RTKs) such as epidermal growth factor receptor (EGFR) and platelet-derived growth factor receptor (PDGFR) are capable of eliciting kinase activity after ligand binding. In several cells, RTKs are activated via the G-protein-coupled receptor independent of the ligand-RTK interaction. We have previously found that EGFR is transactivated via CC chemokine receptor 3 in bronchial epithelial cells and that this pathway is important for mitogen-activated protein (MAP) kinase activation and cytokine production. It has recently been suggested that hypereosinophilic syndrome results from the fusion tyrosine kinase FIP1L1-PDGFRA. Although it is possible that the PDGFR signal is involved in eosinophil function, the details are still unclear. Methods: Blood eosinophils were purified using Percoll and anti-CD16 antibody-coated magnetic beads. Expression of PDGFR mRNA was examined by RT-PCR. After stimulating eosinophils with eotaxin, the phosphorylation of MAP kinases was examined by Western blotting with the antiphosphospecific MAP kinase antibody. The eotaxin-induced eosinophil chemotaxis was studied using Boyden chambers. Results: Eosinophils expressed PDGFRβ  mRNA in 4 out of 8 donors, while PDGFRα mRNA was expressed in only 1 donor. Protein expression of PDGFR was also detectable in eosinophils from some donors. AG1295, a specific inhibitor of PDGFR, showed dose-dependent inhibition of eotaxin-induced MAP kinase phosphorylation in the eosinophils expressing PDGFRβ mRNA. The chemotaxis of these eosinophils was significantly inhibited by AG1295 (n = 3). Conclusions: Our results suggest that PDGFR modifies the CCR3-MAP kinase signaling pathway and chemotactic response in some donors. The photostabilities of three novel bicyclo-boron dipyrromethene difluoride dyes (bicyclo-BODIPY) in solution were investigated by measuring the fluorescence intensity as a function of accumulated 532 nm laser pulses. At 10−6 M concentration 2-butanone and methanol solutions of the novel bicyclo-BODIPY dyes 1 and 2 exhibited photodegradation half-lives within 25% of the photodegradation half-life for 1,3,5,7,8-pentamethyl-2,6,-di-t-butyl pyrromethene difluoroborate (PM-597). At 10−7 M the photostability of a 2-butanone solution of bicyclo-BODIPY 2 is only half that of PM-597. Bicyclo-BODIPY dyes 1 and 2 demonstrated maxima in the fluorescence vs. accumulated pulses plots, attributed to a fluorescent benzo intermediate produced by a retro Diels–Alder reaction. Background: β-agonists are frequently used as bronchodilators for asthma as not only a reliever but also a controller, and their utility has increased with the development of long-acting β2 selective drugs. Although anti-inflammatory effects of β2 selective-agonists have been reported in vitro, side effects on augmentation of airway hyperresponsiveness by chronic use of β2 selective-agonists have been described in several reports. In this study, we investigated the effects of procaterol, a second-generation β2-agonist, on airway inflammation in vivo using an antigen-specific murine model of asthma. Methods: Mice immunized with ovalbumin (OVA) + alum and challenged with inhaled ovalbumin were orally administered procaterol during the challenge. After inhalation, the mice were tracheostomized and placed in a body box under controlled ventilation to measure airway resistance before and after acetylcholine inhalation. Results: Administration of procaterol at a clinical dose equivalent did not augment airway hyperresponsiveness, inflammation of the airway wall, or subsequent airway wall thickening induced by OVA inhalation. BALF cell analysis revealed that the eosinophil number in the BALF was significantly reduced in procaterol-treated mice compared to untreated mice. Conclusions: Oral administration of procaterol at a clinical dose did not augment airway responsiveness, but did reduce eosinophil inflammation. [ABSTRACT FROM AUTHOR]

Abstract: Background: The pathogenesis of reflux esophagitis is not well understood and remains controversial. Distal gastrectomy serves as a model to assess the role of duodenal reflux with low gastric acidity in the development of reflux esophagitis. We investigated the clinical usefulness and antitrypsin activity after treatment with a trypsin inhibitor, camostat mesilate, against the reflux esophagitis after distal gastrectomy reconstructed with Billroth-I anastomosis. Methods: Twenty-eight patients with gastroesophageal reflux disease after distal gastrectomy were prescreened according to esophageal pH level and trypsin activity, and consequently 11 patients were enrolled in the present clinical study. Esophageal and duodenal washings were aspirated for the evaluation of the pretreatment trypsin activity. Then 100 mg of camostat mesilate was administered orally. At 30 and 120 minutes after the administration, duodenal washings were aspirated for the evaluation of posttreatment trypsin activity. Thereafter, 300 mg of camostat mesilate was administered orally 3 times daily for a 4-week period. On the 28th day of administration, the grade of reflux esophagitis (Los Angeles classification) was re-evaluated under endoscopy and the esophageal washings were aspirated for the evaluation of trypsin activity. Results: The trypsin activities in the duodenum both at 30 and 120 minutes after oral ingestion of camostat mesilate were decreased significantly in comparison with those in the pretreatment period in each patient (P &#60; .001). In 6 of 7 patients with detectable trypsin activity in the esophagus, the activities after the 28th day of treatment were lower than those in the pretreatment period, and the symptoms were milder than those before treatment (P &#60; .05). Furthermore, endoscopic evaluation showed that 40% of patients were grade B, C, and D after treatment (28th day), whereas 70% of patients were grade B, C, and D before the treatment. Conclusions: Oral administration of trypsin inhibitor can improve reflux esophagitis after distal gastrectomy concomitant with decreased trypsin activity. [Copyright &y& Elsevier]
