Background: The causes of death for acromegaly are three times more commonly respiratory causes than in general population. Macroglosia and hypertrophic soft tissues have been identified in several reports as implications in airway obstruction. Material and methods: Case report about a 43-year old woman diagnosed with a hypophysis adenoma. Physical examination includes macrocephaly, coarse facial features, frontal bossing, prognatism; predictors for a difficult airway such as macroglossia, Mallampati IV, Bellhouse Dore III, jaw protrusion II. Discussion: It is possible to secure the airway using alternative devices other than laryngoscopy for a better visualization and reduction of time for intubation as well as post-intubation complications. In this case we used Truview EVO™. Conclusion: The anesthesiologists must be ready to secure an airway with different devices and nonsurgical techniques whether it is an anticipated difficult airway or not. [ABSTRACT FROM AUTHOR]
