We report an initial investigation of a subtraction-based method to estimate right ventricle ejection fraction (RVEF) from ECG-gated planar equilibrium radionuclide angiography (ERNA) data. Twenty-six consecutive patients referred for scintigraphic evaluation of cardiac function prior to chemotherapy had ECG-gated first-pass (FP) imaging and ERNA imaging performed following the same radiotracer injection. RVEF was computed from FP images (RVEFFP) and separately from ERNA images (RVEFERNA). Standard methods for computing ejection fractions were used to obtain RVEFFP values. RVEFERNA values were obtained using harmonic subtraction of the left ventricular contribution from a biventricular region of interest contoured on the equilibrium images acquired in the shallow right anterior oblique projection. Clinically acquired chest CT data were used to derive information regarding the relative position of the left and right ventricle and about the presence of pulmonary artery enlargement. Computation of RVEFERNA was successful for each of the 26 patients. Computation of RVEFFP failed for four patients. For the 22 patients for which RVEF was computed using both methods, the average RVEFFP was 49% and the average RVEFERNA was 51%, with coefficients of variation of 11 and 7.5%, respectively. Low RVEFERNA values were associated with pulmonary artery dilation. Estimation of RVEFERNA, using a harmonic subtraction-based method of computation is clinically feasible and accurate in the patient population studied. Prompted by clinical concerns for false-negative tests, we implemented a clinical intervention consisting of a training session and an image-based verification procedure to document homogeneous radioactivity distribution in the radiolabeled meal (egg substitute per the guideline). Methods: A technologist training session emphasized the importance of thorough mixing of 99mTc-sulfur colloid in the egg meal. For 6 mo after training, an image of the prepared mixed egg was acquired before patient ingestion. Consecutive gastric-emptying studies performed 6 mo before and after training were reviewed by 2 experienced physicians. Results: There were 7 abnormal and 44 normal studies before and 15 abnormal and 29 normal studies after training (P < 0.05). Subjective evaluations of images for meal-mixing quality by 2 readers correlated with each other and with an objective measure of expected gastric-emptying physiology (correlation coefficients, 0.54 and 0.38, respectively). Purpose: The accurate harvesting of a sentinel node in breast cancer includes a sequence of procedures with components from different medical specialities, including nuclear medicine, radiology, surgical oncology and pathology. The aim of this document is to provide general information about sentinel lymph node detection in breast cancer patients. Methods: The Society of Nuclear Medicine and Molecular Imaging (SNMMI) and the European Association of Nuclear Medicine (EANM) have written and approved these guidelines to promote the use of nuclear medicine procedures with high quality. The final result has been discussed by distinguished experts from the EANM Oncology Committee, the SNMMI and the European Society of Surgical Oncology (ESSO). Conclusion: The present guidelines for nuclear medicine practitioners offer assistance in optimizing the diagnostic information from the SLN procedure. These guidelines describe protocols currently used routinely, but do not include all existing procedures. They should therefore not be taken as exclusive of other nuclear medicine modalities that can be used to obtain comparable results. The quality of a Teboroxime single-photon emission computed tomography (SPECT) study can be degraded by Teboroxime's rapid myocardial washout and high liver uptake. Described is the use of fast-fanning acquisition and nonlinear regression with distance penalty (NLRDP) to compensate for the two effects. Tl-201/conventional single-rotation Sestamibi (MIBI) and Tl-201/dynamic fast-fanning Teboroxime (TEBO) rest/stress acquisitions were conducted on 18 subjects. TEBO reconstructions were processed by NLRDP using group-averaged kinetics to separate images into liver, myocardium, and background. Reprojections of these compartments were registered with the measured projections. This made possible the extraction of patient-specific kinetics. NLRDP identified defects (abnormal myocardium with different kinetics from normal tissue) by differences in the kinetics of the defect pixels. Enhanced-TEBO (E-TEBO) images were produced by using the kinetic curves to calculate the Teboroxime concentration in each pixel at the time of expected peak myocardial uptake and by omitting the liver compartment. Stress and reversibility total severity scores and total impaired and reversible myocardial mass were computed for the MIBI, TEBO, and E-TEBO images using the Emory Cardiac Toolbox. Regression and correlation analyses of these quantitative indices indicate that Teboroxime SPECT can produce results similar to those of Sestamibi SPECT if E-TEBO images are generated. Background: The sentinel lymph node (SLN) biopsy is revolutionizing the surgical management of primary malignant melanoma. It allows accurate nodal staging, and targets patients who may benefit from regional lymphadenectomy and systemic therapy; however, its use in the management of head and neck melanoma has not been widely accepted. Methods: A retrospective review of patients treated for clinical stages I and II malignant melanoma of the head and neck with dynamic lymphoscintigraphy and gamma probe–guided SLN biopsy. Results: Fifty-eight patients (47 male and 11 female) were identified. Primary melanoma sites included the scalp (21), ear (8), face (13), neck (15), and eyelid (1). Primary tumor staging was T2 (11), T3 (24), and T4 (23). Dynamic lymphoscintigraphy visualized SLNs in 57 patients (98.3%). In 43 cases (75%) a single draining nodal basin was identified, and in 14 cases there were multiple draining nodal basins. Sentinel lymph nodes were successfully identified in 72 (96%) of 75 nodal basins. Positive SLNs were identified in 10 patients (17.5%). Sentinal lymph node positivity by tumor staging was T3, 16.7% and T4, 27.3%. Completion lymphadenectomy revealed residual disease in 3 patients (30%). Relapse occurred in 10 (21.3%) of the 47 patients with negative SLN biopsy results and 7 (70%) of those with positive results. Conclusions: Gamma probe–guided SLN localization in the head and neck region was successful in 96% of draining nodal basins. An experiment was designed to determine whether or not acute pneumococcal pneumonia in dogs is associated with intravascular thrombosis, or with angiographic features distinguishable from pulmonary embolism. In dogs with normal baseline chest radiographs and perfusion scans, pneumonia was produced by transbronchial instillation of type III pneumococcus. After 2 days, perfusion scans demonstrated discrete appropriate defects. In vivo magnification pulmonary arteriography, postmortem pulmonary arteriography, and histologic examination disclosed no evidence of thrombi. [ABSTRACT FROM AUTHOR]

Tc-mercaptoacetyltriglycine (MAG) is a new renal radiopharmaceutical which has been shown to have biological properties similar toI-hippurate (OIH) in animals and volunteers.Tc-MAG has now been compared withI-orthoiodohippurate (OIH) in a group of patients with varying degrees of renal impairment. In all cases, theTc-MAG images were superior regardless of serum creatinine. Selected examples are illustrated including scans and renogram curves in a normal volunteer, transplant patients with creatinines of 9.8 mg/dl and 2.6 mg/dl respectively, and a furosemide study in a patient with questionable obstruction. Our preliminary results suggest thatTc-MAG performs well in patients with impaired renal function and may well provide an acceptable replacement for OIH. In 37 patients with suspected portal hypertension, the bone marrow and splenic uptake on 99mTc-sulfur colloid liver spleen scans correlated with increased portal pressures. The degree of increase was not possible to predict from the scans.
