Background: The Patient Assessment of Chronic Illness Care (PACIC-5As) questionnaire has been designed to evaluate the healthcare experiences of individuals with chronic diseases such as diabetes. Older adults are at higher risk for diabetes and its associated complications. The aim of this study was transcultural adaptation and evaluation of the validity and reliability of the PACIC-5As questionnaire in older patients with diabetes residing in Qazvin City, Iran. Method: In this validation study, we recruited 306 older patients with diabetes from Comprehensive Health Centers in Qazvin, Iran. The multi-stage cluster sampling technique was used to choose a representative sample. The PACIC-5As questionnaire was translated into Persian using the World Health Organization (WHO) standardized method. The validity (face, content, and construct) and reliability (Cronbach's alpha) of the PACIC-5As were assessed. Data analysis was conducted using R software and the Lavaan package. Results: The mean age of the older patients was 69.99 ± 6.94 years old. Most older participants were female (n = 180, 58.82%) and married (n = 216, 70.59%). Regarding face validity, all items of PACIC-5As had impact scores greater than 1.5. In terms of content validity, all items had a content validity ratio > 0.49 and a content validity index > 0.79. The results of confirmatory factor analysis demonstrated that the model exhibited satisfactory fit across the expected five factors, including assess, advise, agree, assist, and arrange, for the 25 items of the PACIC-5As questionnaire. The Cronbach's alpha coefficient for the PACIC-5As questionnaire was 0.805. Conclusion: This study indicates that the Persian version of the PACIC-5As questionnaire is valid and reliable for assessing healthcare experiences in older patients with diabetes. Background and aim: The long-term impact of COVID-19 on nutrition and community health is inevitably noticeable. These effects can change the nutritional behavior and lifestyle of survivors. Due to the COVID consequential fear and anxiety, the pandemic can alter the motivations for choosing, buying, and consuming food. The relationship between nutritional behavior and COVID-19 fear is the primary purpose of this research. Materials & methods: This cross-sectional study was conducted via online and face-to-face surveys. Accessing participants was through health centers of Qazvin, Iran, and the selected centers were sampled by cluster sampling method. The study population included 331 adults aged 18 to 65. Data were collected in three sections using valid questionnaires. The Demographic Questionnaire, FCV-19S, and FCQ were used to gather demographic information, the level of fear caused by COVID-19, and food choice motivations, respectively. The statistical analyzes were performed using R software. Analysis of variance and linear regression methods were used to determine the effect of independent variables on dependent variables (p = 0.05). Results: The mean score of fear of COVID-19 in the study population was 15.25 ± 5.78. Price, Mood, Natural content, Familiarity, Convenience, and Ethical concerns were significantly and positively associated with fear of COVID-19 (p<0.05). The only food motive significantly different than before during COVID-19 was Health, which was increased (p = 0.02). Sensory appeal and Health were the most important motivations for food choices before and during COVID-19. The Ethical concern was considered the least important food motivation. Discussion and conclusion: Some food motivations were associated with fear of COVID-19, possibly due to their psychological nature. The increasing importance of the Health factor and Natural content motivations can relate to the advice of experts on the importance of eating healthy food to counteract COVID-19 and indicate people's preference for this training. Background: Responsiveness to the population's non-clinical needs encompasses various dimensions, including responsive research and an educational outreach plan at the community level. This study aims to develop a community-responsive research model in the healthcare system to ensure the connection between community-identified health priorities and research funds, as well as capacity-building efforts. Methods: A mixed-methods research study was conducted in three main phases, including a comprehensive literature review, a qualitative analysis of an expert panel's points of view, and the developing of a model using the Equation Modeling (SEM) technique. R software version 3.2.4 was used to conduct statistical analysis, considering a significance level of 0.05. Results: Based on the literature review, 41 responsiveness components were identified from sixteen relevant studies conducted between 2000 and 2022. Ten sub-themes in four major themes, including planning, implementation, monitoring and evaluation, and action, were identified through qualitative content analysis. Standardized coefficients revealed that components such as dissemination of results to all stakeholders, research prioritization aligned with community needs, commitment to implement research findings, and collaborative learning had statistically significant effects on the community-responsive research model. Conclusion: It is essential to identify community health priorities by following a community-focused, priority-setting process based on the principles of community engagement to develop a community-responsive research model. Afterward, dissemination of research findings to all stakeholders, commitment to apply the obtained results in the real world, and promotion of shared learning among research partners have been proven to facilitate collaborative investigation and mutual understanding between the community and academic partners. Background: This study examined the viral load and physical status of the human papillomavirus 16 (HPV-16) genome in non-cancerous, precancerous and cancerous cervical lesions. Methods: Quantitative real-time PCR was performed to determine HPV-16 E2 and E6 viral load in 132 cervical specimens. E2/E6 viral load ratio was used to determine the physical status of HPV-16 genome. Results: E2 gene viral load was a significant (P < 0.001) predicting biomarker in differentiating non-cancerous from precancerous and cancerous samples. E6 gene viral load was significantly different between the groups (P < 0.001). The specificity and sensitivity of E2 and E6 in distinguishing SCC samples were 100% and 95% respectively. Objective: This study aimed to evaluate the accuracy of dental calcification stages in predicting the peak pubertal stage. Methods: This retrospective study was conducted on panoramic and lateral cephalometric images of 406 female patients aged 9-14 years. The skeletal maturity and calcification stages of the mandibular canines, first premolars, second premolars, and second molars were determined using the Hassel-Farman and Demirjian (DI) methods, respectively. The prediction accuracy of the peak pubertal stage with the studied teeth was assessed using a receiver operating characteristic curve and the area under the curve (AUC). The DI stage of H was designated as the reference level, and Bayesian logistic regression analysis was used to assess the coinciding chance of each DI stage and peak pubertal stage. Results: The AUC range of studied teeth was 0.84-0.92 in predicting peak pubertal stage (all p<0.001). In the canines and first premolars, the coinciding chance of peak pubertal stage and stage H was significantly higher than that in other stages [p<0.05, odds ratio (OR) =0.14]. In the second premolars and second molars, the chance of peak pubertal stage coinciding with stages H and G did not significantly differ (p>0.05); however, the chance of stage G coinciding with peak pubertal stage in the second molars was higher than in stage H (OR=4.59). Conclusion: Stage H in all studied teeth and stage G in the second premolars and second molars predict peak pubertal stage with high accuracy. Considering that stage H is the end of tooth calcification stages and the accuracy of predicting stage G of second molar teeth is higher than the above stage, estimating the peak pubertal stage is recommended by the second case. Background: Circadian rhythms, as an integral part of daily life, govern the scheduling, management, and coordination of living organisms. Given the irregular nature of shift patterns in nurses' work schedules, investigating their implications is paramount to increasing Quality of Work Life (QWL) and productivity. The study aimed to investigate the impact of circadian rhythm on the efficiency of nurses working in hospitals in Qazvin, Iran, with QWL serving as a mediating variable. Methods: This study employed a descriptive-analytical research design, utilizing cross-sectional data collected in 2022–2023 based on the implementation of Structural Equation Modeling (SEM). The number of participants was 378 nurses. The data were obtained by administering a questionnaire and various tools, organized into four sections: demographic information, the Circadian Questionnaire, the Quality of Work Life Questionnaire, and the Nurses' Efficiency Questionnaire. The collected data were subsequently analyzed using SEM techniques within the R software. Results: The findings demonstrated statistically significant variations in mean scores about gender and efficiency (p = 0.008), marital status and efficiency (p = 0.000), and employment type and efficiency (p = 0.002) among the study participants. There was a significant association between shift patterns and QWL (p = 0.004). Expressly, the confirmed results indicated a direct impact of circadian on QWL (with a path coefficient of 0.013), as well as an indirect impact on efficiency mediated by the variable QWL (with a path coefficient of 0.037) (p < 0.05). Conclusion: Due to the critical role of nurses in the healthcare system, implementing strategies that promote their efficiency is paramount. Minced meat (MM) has a high surface-to-volume ratio, which makes it prone to the growth of pathogenic and spoilage bacteria due to increased exposure to oxygen and high nutrient levels. In this study, postbiotics were used as a natural preservative to extend the shelf life of MM. Lyophilized postbiotics from Lactobacillus rhamnosus were incorporated into bacterial nanocellulose (BNNC) to create an antimicrobial film for packaging MM. The effectiveness of the postbiotic-embedded BNNC film against Staphylococcus aureus was tested using the disk diffusion method and the optimal postbiotic concentrations were determined using the microdilution broth method. Structural changes of the BNNC film after immersion in postbiotics were examined using scanning electron microscopy, and the attachment and chemical nature of the functional groups in the sample were confirmed using FTIR. The antimicrobial efficacy of the postbiotic-embedded BNNC film against S. aureus in MM was also tested. The results showed that BNNC with 10% (P10-BNNC) and 4% (P4-BNNC) postbiotic concentrations were the optimal films, as confirmed by SEM and FTIR. The P-BNNC films significantly reduced the number of S. aureus during storage of MM for 9 days at 4°C. Background: Myocardial infarction is one of the leading causes of death in the world and accounts for 23% of mortalities. Self-care for senior patients with myocardial infarction can reduce complications, multiple hospitalizations, and financial costs. Methods: This clinical trial was performed on 128 older adults with myocardial infarction. Available sampling was done via block random sampling among patients at the Heart Clinic of Booali Hospital, Qazvin, Iran. Data were collected through interviews and demographic and self-care questionnaires regarding heart disease. Data analysis was conducted using R software, version 4.1.0, and via the mixed-effects model method and post hoc and contrast tests. Results: The mean age of the study population was 65.54±4.50 years. Before the intervention, self-care maintenance was not significantly different between the 2 groups. After the intervention, a statistically significant difference was observed between the groups (P=0.001). No statistically meaningful difference concerning self-care monitoring existed between the 2 groups at the beginning of the study (P=0.03); however, a significant difference emerged after the intervention (P=0.001). A difference existed between the groups regarding self-care confidence study commencement in that the self-care confidence level in the control group was higher (P=0.013), but no difference was observed following the intervention. Nonetheless, after 1 month, the groups were significantly statistically different (P=0.003) in that the self-care confidence level in the intervention group increased. Nowadays, antibiotic residues as a global concern pose a threat not only to public health, but also to the food industry all around the world. There have been many studies on contamination sources of bee products, which the most important of them is evaluation of honey contamination extent with various antibiotics used in colonies. Antibiotic residues have a relatively long half life and may have direct toxic effects on consumers. Considering the adverse effects of this residues, we decided to measure the residual erythromycin in different samples of honey consumes in Qazvin, Iran. The present study is a cross-sectional study which conducted in different regions of Qazvin, Iran, in 2019. 80 samples of honey were collected from different regions in Qazvin. Using semi-quantitative ELISA, honey samples were evaluated for the presence of residual erythromycin. Based on the results obtained by ELISA, residual erythromycin was detected only in 8 samples (10.66%), of which, 5 samples (6.66%) and 3 samples (4%) had between 10-120 ng g-1 erythromycin and higher than 120 ng g-1 erythromycin (higher than detection limit of kit), respectively. According to the results, more than 96 percent of our honey samples lacked any erythromycin or had erythromycin concentration lower than allowable limit. The present study demonstrated that most of the honey samples collected from different regions of Qazvin lacked erythromycin residues, but due to the importance of drug residues on community health, annual screening of produced honey for the presence of drug residues by competent authorities. Background. This study aimed to assess the effect of premolar extraction and anchorage type for orthodontic space closure on upper airway dimensions and position of hyoid bone in adults by cephalometric assessment. Methods. This retrospective study was conducted on 142 cephalograms of patients who underwent orthodontic treatment with premolar extraction in four groups of (I) 40 class I patients with bimaxillary protrusion and maximum anchorage, (II) 40 class I patients with moderate crowding and anchorage, (III) 40 class II patients with maximum anchorage, and (IV) 22 skeletal class III patients with maximum anchorage. The dimensions of the nasopharynx, velopharynx, oropharynx, and hypopharynx, and hyoid bone position were assessed on pre- and postoperative lateral cephalograms using AudaxCeph v6.1.4.3951 software. Data were analyzed by the Chi-square test, paired t-test, and Pearson's correlation test (alpha = 0.05). Results. A significant reduction in oropharyngeal, velopharyngeal, and hypopharyngeal airway dimensions was noted in groups I, III, and IV (P <0.001), which was correlated with the magnitude of retraction of upper and lower incisors (r = 0.6 - 0.8). In group II, a significant increase was observed in oropharyngeal and velopharyngeal dimensions (P < 0.001). A significant increase in nasopharyngeal dimensions occurred in all groups (P < 0.001). Also, in groups I and III, the position of hyoid bone changed downwards and backwards, which was correlated with reduction in airway dimensions (r = 0.4 - 0.6). Conclusion. According to the present results, extraction orthodontic treatment affects upper airway dimensions and hyoid bone position. Objective: This study compared the effects of intraperitoneal (i.p.) versus intravenous (i.v.) dexamethasone injection on reducing postoperative nausea and vomiting (PONV) as well as pain after gynecologic laparoscopic surgery. Materials and Methods: In this randomized, triple-blinded clinical trial, 90 women who underwent laparoscopic surgery for gynecologic indications were divided randomly into 3 groups: (1) Group IV-D, single 8-mg dose of i.v. dexamethasone intraoperatively with i.p. placebo; (2) IP-D, single 8-mg dose of i.p. dexamethasone with i.v. placebo; and (3) placebo, i.v. and i.p. distilled water. PONV and pain intensity were measured every 6 hours postoperatively in the first 24 hours with a visual analogue scale (VAS). Results: Ninety patients were in the 3 groups, with 30 patients in each group. The mean ± standard deviation age of participants was 29.38 ± 4.66 years. There was a significant difference in VAS pain at 6, 12, 18, and 24 hours after surgery between groups IV-D and IP-D (p < 0.001), groups IP-D and placebo (p < 0.001), and groups IV-D and placebo (p < 0.001). PONV VAS at 6, 12, 18 and 24 hours were significantly different between groups IV-D and IP-D (p < 0.001), groups IP-D and placebo (p < 0.001), and groups IV-D and placebo (p < 0.001). Conclusions: Dexamethasone significantly reduced incidence and severity of PONV and pain in patients after laparoscopic gynecologic surgery, more so when using i.p. injection. Introduction: In addition to the unfortunate consequences for the patient and family, abscond increases the responsibility and workload of hospital staffs. The aim of the study was to identify the prevalence and factors related to patients’ abscond from a psychiatric hospital 2011 to 2020. Material and methods: In this descriptive epidemiological study, patients’ files were reviewed over a ten-year period. Data collection tools included a questionnaire of demographic information such as patient’s characteristics. Results: The research sample consisted of 14,446 files reviewed, and 204 files related to patient escape were extracted. The escape rate was 1.61%, an average upward rate. According to the results, the incidence of patients’ escape had decreased from 3.91% in 2011 to 0.86% in 2020. The highest escape rate was observed in men (83.82%), single (41.67%) and with undergraduate education (50.98%). Also, people with a history of drug use and more extended hospital stays had higher escape rates (72.55%). The highest escape rate is in summer with an incidence of 19.87%, and the lowest rate is in winter with a prevalence of 10.18%. Conclusions: The possibility of patient escape is considered a nursing diagnosis for all patients with mental disorders, and appropriate preventive measures should be implemented. The necessary interventions by the healthcare team can reduce patients’ motivation to escape by reducing their psychological, social, and communication problems. Therefore, it is necessary to pay more particular attention to patients by identifying those at risk of escape and reducing the rate of escape by providing a high-security environment. Introduction: The social restrictions implemented during the COVID-19 pandemic likely influenced children's and adolescent's lifestyles and nutrition. This study sought to assess student dietary quality and physical activity levels during the pandemic, as well as examine their association with overweight risk. Methods: Four hundred and fifty-eight students aged 7–12 years old in Karaj, Iran, completed online self-administered questionnaires assessing diet, physical activity, and coronavirus fear. Using Kant's method and the Food Frequency Questionnaire, we obtained diversity scores for five food groups. Data were analyzed using R statistical software. Statistical significance was set at P < 0.05. Results: The mean dietary diversity score (DDS) was the lowest for the meat group and highest for the fruit group. A significant inverse relationship was observed between the meat group diversity score and student weight status (P = 0.02). The interaction of coronavirus fear and weight status with the meat group DDS was also significant in multivariate adjustment modeling (P = 0.018). Among overweight students, body mass index had a meaningful negative correlation with physical activity levels (r = −0.018, P = 0.016). In addition, a significant inverse association was detected between coronavirus fear and physical activity in univariate modeling (β = −145.47, 95% confidence interval: −248.81–−42.12, P ≤ 0.006). Conclusion: A significant association between coronavirus fear and reductions in physical activity as well as weight gain was observed. The study group also did not demonstrate sufficient dietary diversity. Therefore, we recommend promoting diet variety and physical activity for students as part of health-focused school curricula. Background: Hypertension (HTN) and diabetes mellitus (DM) as part of non-communicable diseases are among the most common causes of death worldwide, especially in the WHO’s Eastern Mediterranean Region (EMR). The family physician program (FPP) proposed by WHO is a health strategy to provide primary health care and improve the community’s awareness of non-communicable diseases. Since there was no clear focus on the causal effect of FPP on the prevalence, screening, and awareness of HTN and DM, the primary objective of this study is to determine the causal effect of FPP on these factors in Iran, which is an EMR country. Methods: We conducted a repeated cross-sectional design based on two independent surveys of 42,776 adult participants in 2011 and 2016, of which 2301 individuals were selected from two regions where the family physician program was implemented (FPP) and where it wasn't (non-FPP). We used an Inverse Probability Weighting difference-in-differences and Targeted Maximum Likelihood Estimation analysis to estimate the average treatment effects on treated (ATT) using R version 4.1.1. Results: The FPP implementation increased the screening (ATT = 36%, 95% CI: (27%, 45%), P-value < 0.001) and the control of hypertension (ATT = 26%, 95% CI: (1%, 52%), P-value = 0.03) based on 2017 ACC/AHA guidelines that these results were in keeping with JNC7. There was no causal effect in other indexes, such as prevalence, awareness, and treatment. The DM screening (ATT = 20%, 95% CI: (6%, 34%), P-value = 0.004) and awareness (ATT = 14%, 95% CI: (1%, 27%), P-value = 0.042) were significantly increased among FPP administered region. However, the treatment of HTN decreased (ATT = -32%, 95% CI: (-59%, -5%), P-value = 0.012). Conclusion: This study has identified some limitations related to the FPP in managing HTN and DM, and presented solutions to solve them in two general categories. Objectives: Diarrhoea is the most commonly related disease caused by Aeromonas. To improve knowledge on prevalence, this systematic review and meta-analysis was performed to evaluate the global prevalence of Aeromonas in children with diarrhoea worldwide. Methods: We systematically searched PubMed, Google scholar, Wiley Online Library, ScienceDirect, and Web of sciences to identify all cross-sectional published papers between 2000 and 10 July 2022. After initial scrutinizing, 31 papers reporting the prevalence of Aeromonas in children with diarrhoea were found to be adequate for meta-analysis. The statistical study was accompanied by using random effects models. Results: A total of 5660 identified papers, 31 cross-sectional studies encompassing 38,663 participants were included in the meta-analysis. The pooled prevalence of Aeromonas in children with diarrhoea worldwide was 4.2% (95% CI 3.1–5.6%). In the subgroup analysis, the highest prevalence was seen among children in Upper middle-income countries with pooled prevalence of 5.1% (95% CI 2.8–9.2%). The prevalence of Aeromonas in children with diarrhoea was higher in countries with populations of over 100 million people (9.4%; 95% CI 5.6–15.3%), and water and sanitation quality score of less than 25% (8.8%; 95% CI 5.2–14.4%). Additionally, Cumulative Forest Plot showed a decreasing trend in the prevalence of Aeromonas infection in children with diarrhoea over time (P = 0.0001). Conclusion: The results of this study showed a better comprehension of Aeromonas prevalence in children with diarrhoea on a global scale. As well as our findings showed that much work is still required to decline the burden of bacterial diarrhoea in countries with high populations, low-level income, and unsanitary water. Gender determination is the first step in forensic identification, followed by age and height determination, which are both affected by gender. This study assessed the accuracy of gender estimation using mandibular morphometric indices on panoramic radiographs of an Iranian population. This retrospective study evaluated 290 panoramic radiographs (145 males and 145 females). The maximum and minimum ramus width, coronoid height, condylar height, antegonial angle, antegonial depth, gonial angle, and the superior border of mental foramen were bilaterally measured as well as bicondylar and bigonial breadths using Scanora Lite. Correlation of parameters with gender was analyzed by univariate, multiple, and best models. All indices except for gonial angle were significantly different between males and females and can be used for gender determination according to univariate model. Condylar height, coronoid height, and superior border of mental foramen and ramus were still significantly greater in males than in females after controlling for the effect of confounders (p < 0.05). Based on the best model, a formula including five indices of bicondylar breadth, condylar height, coronoid height, minimum ramus width, and superior border of mental foramen was used for gender determination. Values higher than 56% indicate male gender, while lower values indicate female gender, with 81.38% specificity for correct detection of females and 88.97% sensitivity for correct detection of males. Background: A further understanding of the mechanisms linking inflammation to T2D and related complications can help prevent or control this silent but dangerous disease. This study was conducted to determine the association between paraoxonase 1 (PON1) activity and interleukin-6 (IL-6) in type 2 diabetes (T2D). Furthermore, we have evaluated the role of age and gender in the relationship between the PON1 activity and IL-6. Methods: A total of 105 people with T2D were enrolled in this study. IL-6 levels were determined using ELISA. For the PON1 activity assay, the hydrolysis rate of the substrate phenylacetate was spectrophotometrically assayed in serum at 270 nm. The determined velocities were the initial velocities of substrate hydrolysis. Results: PON1 activity was negatively correlated with IL-6 in total data (r = -0.34, P = 0.001). In both groups with age ≥50 and <50 years, PON1 activity was negatively correlated with IL-6, but the correlation was significant in patients aged 50 years and above (r = -0.358, P = 0.005) compared with patients with age <50 years. In both women and men, PON1 activity was negatively correlated with IL-6, but the correlation was significant in women (r = -0.318, P = 0.006) in comparison with men. Conclusions: Inverse association between PON1 activity and IL-6 in T2D may represent the oxidative-inflammatory interaction in this disease. Our findings highlight that at older ages and in women, the associations between lower PON1 activity and higher IL-6 concentrations are more evident, and this should be considered in patients with T2D. This study aimed to compare the accuracy of three types of apex locators versus digital radiography for working length (WL) determination. This experimental study was conducted on 58 extracted maxillary premolars. The teeth were decoronated, the access cavity was prepared, and WL was determined using a #15 K-file to serve as reference. The WL was then measured by Woodpex V, Woodpex III, and Root ZX apex locators in the presence of 0.9% saline, and also on a photostimulable phosphor plate (PSP) digital radiograph taken by the parallel technique. The values were compared with the actual WL using the paired t-test (alpha = 0.05). Digital radiography, Root ZX, Woodpex V, and Woodpex III determined the WL within ±0.5 mm from the actual value in 84.48%, 100%, 89.66%, and 87.93% of the cases, respectively. Woodpex V (p = 0.039), Woodpex III (p = 0.001), and Root ZX (p = 0.001) significantly over-estimated the WL. The WL measured on digital radiographs was not significantly different from the actual WL (p = 0.213). The position of the apical foramen (central/lateral) had no significant effect on the accuracy of WL determination by different techniques (p >0.05). Within the limitations of this in vitro study, all the tested modalities showed acceptable accuracy for WL determination in maxillary premolars. [ABSTRACT FROM AUTHOR]

This study was aimed to evaluate the antibacterial and biofilm inhibitory activity of Penicillium chrysogenum-derived silver nanoparticles (AgNPs) against the standard and pathogenic Acinetobacter baumannii using a 96-well microtiter plate-based method. The AgNPs were characterized by using UV–Vis, TEM, AFM, XRD, DLS, Zeta potential, and FT-IR. The nanoparticles (NPs) were fabricated with a spherical shape and an average hydrodynamic diameter of 48.2 nm. The minimum inhibitory concentration (MIC) and minimum bactericidal concentration (MBC) of AgNPs were found to be 4 and 32 µg/mL respectively, whereas the MIC and MBC of tetracycline were found to be 1024 and 8192 µg/mL against A. baumannii (ATCC 19606). The AgNPs and tetracycline represented considerable biofilm inhibitory activity against both the standard and pathogenic A. baumannii at the studied concentrations. However, the AgNPs depicted higher potency to inhibit the process of biofilm formation of some pathogenic A. baumannii species compared to tetracycline. The AgNPs at the concentration of 0.5*MIC (2 µg/mL) inhibited above 90% biofilm inhibition, whereas tetracycline reached 90% biofilm inhibition at the concentration of 4*MIC (4096 µg/mL) against A. baumannii (ATCC 19606). Tomato is one of the popular crops in the world and tomato paste is a product of it. Due to its wide application, quality control of this product is important. The goal of our study was to the assessment of potassium sorbate and sodium benzoate preservatives and artificial color in bulk tomato paste samples in Qazvin, Iran. In this study, 45 samples were analyzed by high-performance liquid chromatography (HPLC) to determine the preservatives. Benzoic acid was detected in 17 (37.78%) samples, ranging from nd not detected to 1896 mg kg-1 and Potassium sorbate was not detected in any of the samples. The detection of color in samples was done by the thin-layer chromatography method (TLC). Three samples (6.67%) had Pansio 4R artificial color. The mean and standard deviation of sodium benzoate in samples were 990.62 mg kg-1 and 396.07 mg kg-1, respectively. According to the national standard of Iran for canned tomato paste, adding any kind of preservative and color to tomato paste is prohibited. The results show the importance monitoring of sodium benzoate and artificial colors in bulk tomato paste by health authorities. [ABSTRACT FROM AUTHOR]

Honey is a unique food product containing bioactive compounds derived from bees and plants Nowadays, animal food products that may contain antibiotic residues have caused a lot of concern for the consumers. The presence of antibiotic residues in honey may be harmful to human health. One of the most important risks of antibiotic residues in food products is the occurrence of drug resistance in pathogenic bacteria in the body. Regarding the complications caused by these residues, the present research has investigated sulfonamide antibiotic residues in the honey samples consumed in Qazvin province. The present research is a cross-sectional study performed in different regions of Qazvin province in 2019. For this purpose, 80 honey samples were collected from different regions of Qazvin. The samples were transferred to the food safety laboratory of the Health Faculty under proper conditions. First, the samples were tested to check the sulfonamide antibiotic residue by the semi-quantitative ELISA assay. The data were recorded in SPSS 23, and data analysis was done by one-way analysis of variance (ANOVA) and (post hoc) Tukey test at the significant level of P 0.05. According to the results of the ELISA assay, 10-120 ng of sulfonamide antibiotic residue was found in 23.75% (19 samples) of the samples. The findings showed that the highest and the lowest sulfonamide concentrations were respectively reported as 30.81 and 6.26 ppb, and the average sulfonamide residue was obtained as 14.50 ppb. According to the results, more than 75% of the honey samples collected from Qazvin province are free of sulfonamide or contain a little sulfonamide concentration. The research showed that most of the honey samples collected from different regions of Qazvin province are free of sulfonamide or contain a little sulfonamide concentration. Regarding the important role of honey consumption in health and the wide use of antibiotics in beehives, it is necessary to take the necessary actions to control the quality of this product. Background: The consumption of uncooked or undercooked food from infected intermediate hosts can result in Toxoplasma gondii infection in humans. However, few studies have investigated the genetic diversity of this protozoan parasite in Iran. The aim of the present study was to genetically characterize isolates of T. gondii from intermediate host animals in Mazandaran Province, Iran. Methods: Blood and heart tissue samples were collected from 204 ruminants, and brain tissue was collected from 335 birds. The prevalence of T. gondii infection in these samples was determined serologically using the modified agglutination test and by conventional PCR assays. Those PCR samples positive for T. gondii DNA and 13 DNA samples extracted from aborted fetuses in a previous study were genotyped with 12 genetic markers using the multilocus-nested PCR-restriction fragment length polymorphism (Mn-PCR–RFLP) technique. Results: Antibodies for parasites were found in 35.7% of the ruminant (39.1% of sheep and 26.4% of goats) samples and in 51.3% of the bird (100% of geese, 52.3% of free-range chickens and 46% of ducks) samples. Molecular detection by PCR of the repetitive 529-bp DNA fragment revealed contamination of 13.2% of ruminant (14.6% of sheep and 9.4% of goats) samples and of 9.6% of bird (11.1% of chickens, 5.7% of ducks and 0% of geese samples). The results from 30 DNA samples revealed five ToxoDB (genome database for the genus Toxoplasma) PCR–RFLP genotypes, including #1 (Type II), #2 (Type III), #10 (Type I), #27 and #48, with genotype #1 the most predominant. Conclusions: As evidenced by the results of this study, ruminants and birds are infected with T. gondii in the region, suggesting that they are likely to be involved in the transmission of T. gondii to humans through meat consumption. The identification of different genotypes may suggest a higher genetic diversity of this parasite in Mazandaran, reflecting local environmental contamination. Background: Retinopathy of Prematurity (ROP) is a pathologic condition in the retina characterized by abnormal vasoproliferation. We aim to investigate the correlation of different birth- and hospitalizationrelated factors in the progression of ROP. Methods: This historical cohort study performed in the ophthalmology center of Ayatollah Rouhani Hospital in Babol (Babol University of Medical Sciences, Babol, Iran), included 828 infants (gestational age <35 weeks and birth weight <2500 g). Also, data were collected from the history of hospitalized premature infants. Results: The lower gestational weight of infants has been observed in ROP infants (1418.05±547.09 g) compared to non-ROP infants (1917.31±486.01 g) (p<0.001). In addition, the average gestational age of ROP infants (30.28±2.34 weeks) was younger than non-ROP infants (33.26±1.00 weeks) (p=0.042). Increase in the duration of hospitalization increased 6% the chance of ROP in infants (OR=1.06; 95%CI: [1.05,1.08]) (p<0.001). Furthermore, increase in the count of blood replacement decreased 29% the chance of complete remission of ROP (OR=0.71; 95%CI: [0.51,0.95]) (p=0.029). The blood group does not have an impact on ROP development or remission. Conclusion: More birth weight of premature infants is associated with less chance of ROP incidence. Background: Retinopathy of prematurity (ROP) is a vasoproliferative retinal disease in premature infants that causes lifetime visual impairment and blindness in the early ages. In this study, we investigated the differences in the values of clinical laboratory parameters between different ROP and its remission/progression statuses regarding stages and zones. Methods: This historical cohort study includes 828 infants divided into two groups after the first examination containing ROP infants and controls. The biochemical and hematological parameters of the two groups have been collected from the patient's history. Results: In infants with ROP, the hematopoiesis-related parameters, including the mean level of hemoglobin, total bilirubin, potassium, calcium were significantly less than controls (P=0.039, P=0.001, P=0.001, and P=0.046, respectively). The percentages of reticulocyte and the levels of BUN in ROP patients were significantly higher than in normal infants (P=0.015 and p <0.001, respectively). Moreover, the levels of hemoglobin and BUN were significantly different in the different zones of ROP (P=0.017 and P=0.001, respectively). Also, higher hemoglobin levels, total bilirubin, and CRP were observed in the reduced stages of ROP (P=0.041, P=0.045, and P=0.039, respectively). Gastrointestinal (GI) cancers are responsible for approximately half of cancer-related deaths, highlighting the need for the identification of distinct and common features in their clinicopathological characteristics. Long ncRNA (lncRNAs), which are involved in competitive endogenous RNA (ceRNA) networks with critical roles in biological processes, constitute a substantial number of non-coding RNAs. Therefore, our study aimed to investigate the similarities and differences in the ceRNA networks of The Cancer Genome Atlas (TCGA)-GI cancers. We performed a comprehensive bioinformatics analysis of ceRNA networks for TCGA-GI cancers in terms of the deferential mRNA, lncRNA, and miRNA expression levels, ceRNA networks, overall survival analysis, correlation analysis, pathological cancer stages, and gene set enrichment analysis. Our study revealed several common and distinct mRNAs and lncRNAs with prognostic values in these networks. It was specifically noteworthy that MAGI2-AS3 lncRNA was found to be shared in almost all GI cancers. Moreover, the most common shared mRNAs between GI cancers were MEIS1, PPP1R3C, ADAMTSL3, RIPOR2, and MYLK. For each cancer ceRNA network, we found that the expression level of a number of lncRNAs and mRNAs was specific. Furthermore, our study provided compelling evidence that several genes, most notably KDELC1 , can act as novel proto-oncogenes in cancers. This, in turn, can highlight their role as new prognostic and therapeutic targets. Moreover, we found cell cycle and extracellular matrix structural constituent as the top shared KEGG and molecular function, respectively, among GI cancers. Background: Identifying appropriate biomarkers for predicting type 2 diabetes (T2D) patients with increased HbA1c may prove helpful in preventing increased risk of cardiovascular disease (CVD). The present study was conducted to analyze the diagnostic performance of the atherogenic index log (TG/HDL-C) in T2D patients with increased HbA1c. Methods: Patients with T2D were classified into two groups according to having an HbA1c <8% or =8%. Atherogenic index was calculated from the logarithmic transformation of TG/HDL-C. Receiver operating characteristic curve was used to evaluate the diagnostic performance of log (TG/HDL-C). Insulin and fasting glucose concentrations were used to determine homeostasis model assessment for insulin resistance (HOMA-IR). Results: Compared with the patients with HbA1c <8%, log (TG/HDL-C) was significantly higher in the patients with HbA1c =8% (p = 0.025). The atherogenic index was a biomarker for the prediction of T2D patients with HbA1c =8% versus patients with HbA1c <8%, as shown in the area under the curve (AUC = 0.61, P = 0.013). The best cut-off point of log (TG/HDL-C) for the discrimination between patients with HbA1c =8% versus patients with HbA1c <8% determined to be 0.44. Atherogenic index was significantly and positively correlated with HOMA-IR in female patients (r = 0.313, P = 0.003) and in patients with an age =5o (r = 0.253, P = 0.021). Conclusion: The log (TG/HDL-C) in addition to its known association with enhanced CVD risk could be considered as a biomarker to predict T2D patients with poor glycemic control. Therefore, the increased ratio may provide a simple and useful way of identifying poor glycemic T2D patients who are possibly to be at elevated risk of CVD. [ABSTRACT FROM AUTHOR]

Background: Musculoskeletal disorders have been a major cause of disability in pregnant women. The aim of this study is to determine the effect of exercise in an aquatic environment on the musculoskeletal pain among pregnant women. Material and methods: This randomized clinical trial was conducted on 54 pregnant women aged 20–35 years. The subjects were divided into the intervention group and control groups. Women who were in 20 to 26 weeks of gestation were included in the study. The intervention was Water Exercise in Pregnancy program conducted from week 20 to week 34. The control group received standard prenatal care. Musculoskeletal complications were assessed using the Nordic questionnaires at the baseline and, 4th and 8th week of pregnancy. The intensity of pain was assessed using a visual analog scale in the two groups. Data were analyzed using descriptive and inferential statistics via the SPSS v.16. The lsmeans package was used to examine comparisons. Results: Results of the study showed that level of discomfort and musculoskeletal complications in the intervention group was significantly reduced during the 4th and 8th week of the study compared to the control group (p < 0. 0 0 1). After the intervention, intensity of musculoskeletal pain in the intervention group showed a significant decrease compared to the control group (p < 0. 0 0 1). Conclusion: Exercise in water during pregnancy can be effective to reduce musculoskeletal complications. Objective: The use of assisted reproductive technology (ART) has been associated with an increased risk of gestational diabetes mellitus (GDM) in previous studies, but its risk has not been consistent. Therefore, we aimed to estimate the risk of GDM in women who conceived with ART via a systematic review and meta-analysis of cohort studies. Methods: ISI Web of Knowledge, Medline/PubMed, Scopus, and Embase databases were searched to identify studies that evaluated the risk of GDM through May 2017 using the relevant keywords. Two reviewers independently performed the screening, data extraction, and quality assessment. Meta-analysis was performed with a random effects model. Results: The search yielded 957 records relating to GDM and use of ART, from which 59 eligible cohorts were selected for meta-analysis (n=96,785). There was evidence of substantial heterogeneity among these studies (v(58) 2=3072.34, p<.001; I2=98.1%). The pooled estimate of GDM risk using the random effects model was 9.00% (95% CI: 7.90-10.20). Visual inspection of the funnel plot indicated the presence of low publication bias, but Egger's test did not reveal publication bias. Conclusions: The findings revealed that the risk of GDM was very high among women who conceived with ART treatment. GDM screening, management, and improved care are vital in ART pregnancy. Background: To assess the potential impact of a tax-induced cigarette price increase on financial and health outcomes by different socioeconomic groups. Methods: In a modeled condition using pooled cross-section data from Household Income and Expenditure Survey (2002–2017) and Iran 2019 population data, a methodology of an extended cost effectiveness analysis (ECEA) was applied to model the impact on cigarette consumption of hypothetically increased cigarette tax. The methodology was employed to evaluate: [1] health benefits (premature deaths averted); [2] health expenditures regarding smoking-related disease treatment averted; [3] additional tax revenues raised; [4] change in household expenditures on cigarettes; and [5] financial risk protection among male Iranian smokers in a time span of 60 years following a one-time increase in cigarette price of 75%. The Stata version 15.1 (StataCorp., College Station, TX, USA) was used to perform the relevant analysis and estimate regression models. Results: A 75% increase in cigarettes price through taxation would reduce the number of smokers by more than half a million, 11% of them in the poorest quintile; save about 1.9 million years of life (11% of which would be gained in the lowest quintile compared to 20% in the highest one); eliminate a total of US$196.4 million of health expenditures (9% of which would benefit the bottom quintile). Such a policy could raise the additional annual tax revenues by roughly US$ 1 billion, where the top two quintiles bear around 46% of the total tax burden. We estimated that the tax increase would avert an estimated 56,287 cases of catastrophic expenditure that wholly concentrated among the bottom two expenditure quintiles. Conclusion: Increasing cigarette tax can provide health and financial benefits, and would be pro-poor in terms of health gains, Out-of-Pocket (OOP) savings, and financial risk protection against smoking-related diseases. Background: The objective of this study was to investigate serum levels of anti-Müllerian hormone (AMH) in normal-ovulatory infertile women with polycystic ovarian morphology (PCOM) and their association with ovarian hyper-response. Materials and Methods: This prospective cohort study was carried out on 100 infertile women with PCOM who were treated with an antagonist/agonist triggered stimulation protocol at Shahid Akbar-Abadi Hospital IVF Centre, Tehran, Iran. Serum AMH levels were measured before starting the assisted reproductive technology (ART) cycle and the ovarian hyper-response was evaluated by retrieved oocyte numbers, ooestradiol levels on the triggering day, and the incidence of ovarian hyper-stimulation syndrome (OHSS) clinical signs and symptoms. Logistic regression and the area under the curve (AUC) were used to estimate the effects of AMH and the accuracy of the test. Results: Receiver operating characteristic (ROC) curve analysis showed that AMH could significantly predict ovarian hyper-response in PCOM patients (AUC=0.73). The estimated threshold value was 4.95 ng/ml, with a specificity of 74.58% (95% confidence interval [CI]: 50.85, 93.22) and sensitivity of 73.17% (95% CI: 48.78, 92.68). Logistic regression results showed a significant interaction between AMH and body mass index (BMI, P=0.008), which indicated that BMI had a moderation effect. Conclusion: Individualized stimulation protocols for patients with isolated PCOM and AMH greater than 4.95 ng/ml may significantly reduce the chances of developing OHSS. However, the AMH cut-off values to predict ovarian hyper-response differ for different BMI categories among PCOM patients; thus, it becomes a more precise predictive marker with increasing BMI. Upper gastrointestinal (UGI) cancer, including esophageal and gastric, is one of the most common cancers in the world. Hence, the determination of risk factors of UGI helps to reduce the economic and social burden of this cancer in communities. In Iran, the consumption of opium because of its neighborhood with Afghanistan are considerable. In this study, we examine the causal effect of opium use on the time to UGI cancer death. Based on the Golestan Cohort Study (GCS) in northeastern of Iran, about 50000 adults were enrolled to the study for four years (2004–2008) and followed annually until July 2018. We used "parametric g-formula" to study the causal effect of opium use on the time to death due to UGI. In this study, the information of 49946 individuals due to missingness were analyzed. So the median of follow-up time was 144 months and the prevalence of opium use was 17% (about 8489 persons). During the follow-up period, 593 (1.2%) death from upper gastrointestinal cancer were reported. The study showed that the effect of opium use on the time to UGI death was statistically significant (adjusted risk-ratio based on parametric g-formula = 1.31, 95% CI: [1.04, 1.65]). Additionally, the Population Attributable Fraction (PAF) in UGI cancer deaths of opium use was estimated 5.3% (95% CI: [0.6%, 11.3%]). Background and Objective: We conducted this study to assess the prevalence of plagiarism and to shed light on some dark aspects of this issue. The main objectives included to find out the etiology, prevalence, and detection of various forms plagiarism. Methods: In this Cross-sectional study we used a questionnaire, face-to-face interview, analyzing the present notifications and codes, websites, and literature review. The current study was conducted throughout Iran from 2017-2018. Those associated with scientific journalism, academic staffs, and authors were interviewed or asked to fill out a prepared questionnaire. Results: Nine hundred seventy nine questionnaires were circulated. Out of this 706 (72.1%) were completed and returned. Those with a master degree were most cooperative in filling out the questionnaires (36.4%); followed by Assistant Professors (29.6%). About 74.1% of respondents, had not participated in any educational workshops on plagiarism (P<0.001) while 10.8% had not heard anything about plagiarism (P<0.001). As regards correct reply as for definition and detecting plagiarism; 91.1%, 40.8%, 48.4% and 57.9% could reply correctly (P<0.001). Forty-one-point one percent of the participants believed that reprimand would be the best punishment. The percentage of plagiarism as per people associated in journal administration, was 22.9%; based on experts' opinions, it was 30.0%; and based on analysis of some journals published in Iran, it was 25.5%. Conclusion: We found a noticeable prevalence of plagiarism in Iran. Many factors are involved in this misconduct; most important being the need for academic staff and students to publish e more papers regardless of their quality to meet some of the academic requirements. Considering the high rank of Iran in terms of scientific growth worldwide, it is expected from the regulatory authorities to monitor all aspects of scientific misconducts in medical journalism. Noise exposure is one of the most important physical agents in the workplace which can induce job stress in several ways. The aim of this study was to model the interactions between independent and mediating variables and job stress using structural equation modeling. In this study, Weinstein's noise sensitivity scale, noise annoyance questionnaire, Health and Safety Executive (HSE) job stress questionnaire and job satisfaction scale were used. To assess worker's noise exposure, the 8-hours equivalent continuous A-weighted sound pressure level (LAeq;8 h), was measured based on ISO 9612 (2009). To achieve the aims of study, the structural equation model was run using R software 3.4.1 and Cytoscape software 3.6.0. Based on the results, while there was a direct positive correlation of noise exposure on total job stress, there were also indirect positive effects through job satisfaction and noise sensitivity as mediator variables. Using hearing protective devices negatively affected total job stress through a direct pathway and an indirect pathway when job satisfaction was a mediator variable. Regarding the total effect of noise exposure and using hearing protection devices on job stress subscales, it can be concluded that noise exposure and using hearing protection devices had greatest effect on colleagues support and demand, respectively. It can be concluded that noise exposure and lack of hearing protective devices have a significant positive effect on job stress among workers of a textile industry. In addition to the direct effect, this factor can induce job stress through noise sensitivity, job satisfaction and noise annoyance. Objective: Previous studies have indicated that women with assisted reproductive technology (ART) pregnancy have an increased risk of preeclampsia. The aim of this current study was to estimate, through a systematic review and meta-analysis of cohort studies, the risk of preeclampsia in women who conceived with ART.Materials and methods: We searched ISI Web of Knowledge, Medline/PubMed, Scopus and Embase up to August 2017 for English-language articles pertaining to risk of preeclampsia in ART pregnancy using standard keywords. Data extraction was conducted by two authors and quality of the studies was assessed using the Newcastle-Ottawa Scale. A random-effects model was used for the meta-analysis.Results: In total, 72 cohort studies (n = 164 870) were included. The results of Cochran test and I2 statistic indicated considerable heterogeneity among studies (Q = 15 415.61, df = 71, p < .001, I2=99.5%). The pooled estimate of preeclampsia risk using the random effects model was 10.8% (95% CI: 9.10-12.5). Furthermore, the funnel plot and Begg's test showed evidence of publication bias.Conclusions: We found that the risk of preeclampsia was very high among women who conceived with ART. Background: Prostate specific antigen (PSA) is an important biomarker to monitor patients after treated with radiation therapy (RT). The aim of this study is to evaluate the relationship between the PSA data and prostate cancer recurrence using the joint modeling. Methods: This historical cohort study was performed on 422 prostate cancer patients. Inclusion criteria included: patients with localized prostate cancer referring to Cancer Institute in Tehran (Iran) from 2007 to 2012, and under radiation therapy. Joint model has two components or sub-models. We showed the results by parameter estimating the longitudinal sub-model and survival sub-model. EM algorithm, Newton-Gauss and Gauss-Hermit law were used for final model parameters. R software version 3.2 was used for statistical analysis. Results: In this study, considering the inclusion and exclusion criteria, out of 422 patients, the data on 314 cases were selected for analysis and the main result of joint model was obtained. PSA directly and significantly was associated with recurrence risk, therefore increasing 2.6 ml/lit PSA (one unit in transformed PSA) increases 39% recurrence risk (95% CI for RR: 1.09-1.77). Also, slope of PSA trend has significant association with prostate cancer recurrence risk (95% CI for RR: 1.05-1.41). Conclusion: This study showed a significant relationship between PSA, and its slope with the recurrence risk by joint model, with regard to the pathological, demographic and clinical features in the Iranian population. Prostate cancer has emerged as the common malignancy in men and is considered as a global issue. Among different strategies to combat prostate cancer, nanobiotechnology appears as a novel approach for prostate cancer theranostics. The current systematic review discussing the recent development in the prostate anticancer activity of biologically synthesized silver nanoparticles (AgNPs) and gold nanoparticles (AuNPs) through laboratory investigations. The databases including Cochrane, Web of Science, PubMed, Scopus, Science Direct, ProQuest, and Embase were used to identify the records. It was found that most of the studies used herbal extracts for preparation of AgNPs and/or AuNPs. Majority of biosynthesized nanoparticles (NPs) were formed with spherical morphology. Besides, all studies reported the size of NPs less than 100 nm. Moreover, PC-3 prostate cancer cell was the predominant cell line that was evaluated for prostate anticancer activity of NPs. In the present study, we provided valuable information suggesting the significant prostate anticancer potential of biogenic AgNPs and/or AuNPs. Objective: The use of assisted reproductive technology (ART) has been associated with an increased risk of gestational diabetes mellitus (GDM) in previous studies, but its risk has not been consistent. Therefore, we aimed to estimate the risk of GDM in women who conceived with ART via a systematic review and meta-analysis of cohort studies.Methods: ISI Web of Knowledge, Medline/PubMed, Scopus, and Embase databases were searched to identify studies that evaluated the risk of GDM through May 2017 using the relevant keywords. Two reviewers independently performed the screening, data extraction, and quality assessment. Meta-analysis was performed with a random effects model.Results: The search yielded 957 records relating to GDM and use of ART, from which 59 eligible cohorts were selected for meta-analysis (n = 96,785). There was evidence of substantial heterogeneity among these studies (χ(58)2 = 3072.34, p < .001; I2=98.1%). The pooled estimate of GDM risk using the random effects model was 9.00% (95% CI: 7.90-10.20). Visual inspection of the funnel plot indicated the presence of low publication bias, but Egger's test did not reveal publication bias.Conclusions: The findings revealed that the risk of GDM was very high among women who conceived with ART treatment. Purpose. One of the characteristics of Prostate-Specific Antigen (PSA) is PSA slope. It is the rate of diminishing PSA marker over time after radiotherapy (RT) in prostate cancer (PC) patients. The purpose of this study was to evaluate the relationship between increasing RT doses and PSA slope as a potential surrogate for PC recurrence. Patients and Methods. This retrospective study was conducted on PC patients who were treated by radiotherapy in the Cancer Institute of Iran during 2007–2012. By reviewing the records of these patients, the baseline PSA measurement before treatment (iPSA), Gleason score (GS), clinical T stage (T. stage), and periodic PSA measurements after RT and the total radiation dose received were extracted for each patient separately. We used a Bayesian dose-response model, analysis of variance, Kruskal–Wallis test, Kaplan–Meier product-limit method for analysis. Probability values less 0.05 were considered statistically significant. Results. Based on the D’Amico risk assessment system, 13.34% of patients were classified as “Low Risk”, 51.79% were “Intermediate Risk”, and 34.87% were “High Risk”. In terms of radiation doses, 12.31% of the patients received fewer than 50 Gy, 15.38% received 50 to 69 Gy, 61.03% received 70 Gy, and 11.28% received more than 70 Gy. The PSA values decreased after RT for all dose levels. The slope of PSA changes was negative for 176 of 195 patients. By increasing the dosage of radiation, the PSA decreased but these changes were not statistically significant (p = 0.701) and PSA slope as a surrogate end point cannot met the Prentice’s criteria for PC recurrence. Conclusion. Significant changes in the dose-response relationship were not observed when the PSA slope was considered as the response criterion. Therefore, although the absolute value of the PSA decreased with increasing doses of RT, the relationship between PSA slope changes and increasing doses was not clear and cannot be used as a reliable response surrogate endpoint. Purpose: The aim of the present study was to evaluate the effect of myo-Inositol administration on oocyte quality, fertilization rate and embryo quality in patients with PCOS during assisted reproductive technology (ART) cycles.Methods: Fifty infertile PCOS patients were randomly designated in two groups. In the study group, patients received daily doses of 4 g myo-Inositol combined with 400 mg folic acid and in the control group patients received only 400 mg folic acid from 1 month before starting the antagonist cycle until the day of ovum pick up. Oocyte and embryo qualities were assessed according to European Society of Human Reproduction and Embryology (ESHRE) guidelines. The gene expression of PGK1, RGS2 and CDC42 as a factor of oocyte quality in granulosa cells was analyzed using real-time RT-PCR. Levels of total antioxidant capacity (TAC) and reactive oxygen species (ROS) were evaluated by chemiluminescence assay in follicular fluid.Results: The percentage of metaphase II oocyte, fertilization rate and embryo quality significantly improved in the study group (p < 0.05), but the number of retrieved oocytes and follicle count were not statistically different between groups. Furthermore, the gene expression of PGK1, RGS2 and CDC42 was significantly higher in the study group (p < 0.05) but no differences were found between two groups in terms of TAC and ROS levels.Conclusions: The present study findings suggest that myo-Inositol alters the gene expression in granulosa cells and improves oocyte and embryo quality among PCOS patients undergoing ART. The objective of current study was to optimize the average diameter size of biosynthesized Silver Nanoparticles (AgNPs) induced by Penicillium citrinum. The AgNPs were prepared with a spherical shape and good monodispersity, determined by scanning electron microscopy (SEM), atomic force microscopy (AFM) and photon correlation spectroscopy (PCS). Besides, response surface methodology (RSM) was conducted to optimize the average diameter size of AgNPs by employing Box-Behnken design (BBD). The effective factors in this process were concentration of AgNO3 (mM) (X1), pH of solution (X2), temperature of shaker incubator (°C) (X3), rate of shaker incubator (rpm) (X4) and time of incubation (hour) (X5). The R2 (0.8894) value of the obtained model indicated a good correlation between the observed and predicted values. Overall, it could be concluded that multivariate analysis facilitated to find out the optimum conditions for the biosynthesis of AgNPs induced by P. citrinum with the reduced time and cost. Background: Fluid balance and oliguria influence outcome in critically ill patients. Although, osmotic dieresis with hypernatraemia is a predictor of mortality in critically ill patients, the purpose of this study was to demonstrate the effect of polyuria as an independent predictor on weaning outcome in mechanically ventilated patients. Materials and Methods: This retrospective, single center, cohort study was carried out at Imam Teaching Hospital Intensive Care Unit (ICU) on 263 adult mechanically ventilated patients. We collected data of these patients during the mean seven consecutive days before weaning from mechanical ventilator. Patients with polyuria (sustained urine output greater than 3000 ml/day) were compared with patients without polyuria. The primary endpoint was successful weaning and the secondary endpoints were the mechanical ventilation duration, post weaning length of ICU stay, post weaning length of hospitalization and rate of mortality. Results: In 93 patients with polyuria, the mean age was 45.14±19.47 years in comparison of 170 patients without polyuria with mean age of 52.9±21.37 years (P=0.004). Fluid intake, urine output and temperature were significantly higher in patients with polyuria, but there were no statistical differences in systolic and diastolic blood pressure, serum electrolytes, urea and creatinine. No significant differences were found in primary and secondary endpoints including successful weaning, post weaning length of ICU stay, post weaning hospital duration and mortality, except for duration of mechanical ventilation (P=0.014). The area under the ROC curve for variables showed only seven days mean creatinine level before weaning which may act as a predictor of successful weaning (ROCAUC=0.67, 95% CI 0.61-0.73, P=0.0002). Serum creatinine level of 0.8 provided best overall combination of sensitivity and specificity for successful weaning (sensitivity 72.22%, 95% CI 54.8-85.8; specificity 61.19%, 95% CI 54.1-68.0). Conclusion: Polyuria cannot predict weaning outcome but maybe considered as a predictor of longer duration of mechanical ventilation and is probably associated with a subclinical renal dysfunction. Background: The aim of this study was to determine whether insulin resistance, beta-cell function, and their associations with alanine aminotransferase (ALT) are affected by the functional variants of paraoxonase-2 (PON2) as an intracellular antioxidant in patients with type 2 diabetes (T2D). Materials and Methods: Quantitative insulin sensitivity check index (QUICKI) and homeostasis model assessment for beta-cell function (HOMA-BCF) were assessed in T2D patients. Insulin levels were determined using ELISA. The variants PON2-A148G and PON2-S311C were genotyped using polymerase chain reaction-based restriction fragment length polymorphism. Results: According to the PON2-G148A variant, ALT was found to be significantly correlated with QUICKI (r = -0.616, P = 0.005) and HOMA-BCF (r = 0.573, P = 0.01) in the GA + GG group; however, the correlations were not statistically significant in the AA genotypes. Based on the genotypes of PON2-S311C, there was a significant correlation between ALT with QUICKI (r = -0.540, P = 0.031) and HOMA-BCF (r = 0.567, P = 0.022) in the SC + CC group. In the multiple adjusted logistic regression analyses, considering the variants PON2-G148A and PON2-C311S as independent variables and QUICKI and HOMA-BCF as the dependent variables, both variants were significantly associated with the QUICKI (P = 0.019 for PON2-G148A and P = 0.041 for PON2-C311S). Furthermore, PON2-C311S remained significantly associated with HOMA-BCF (P = 0.03). Conclusion: These data implicate a role for the functional variants of PON2 in insulin resistance and beta-cell function as well as underscore the effective role of these variants in the associations between them and ALT. Autosomal recessive deficiency in the caspase recruitment domain containing protein 9 (CARD9) results in susceptibility to fungal infections. In the last decade, infections associated with CARD9 deficiency are more reported due to the advent of genome sequencing. The aim of this study was to evaluate the frequency, geographic distribution and nature of mutations in patients with CARD9 deficiency. We identified 60 patients with 24 mutations and different fungal infections. The presence of the homozygous (HMZ) p.Q295X (c.883C > T) and HMZ p.Q289X (c.865C > T) mutations were associated with an elevated risk of candidiasis (OR: 1.6; 95% CI: 1.18–2.15; p = 0.004) and dermatophytosis (OR: 1.85; 95% CI: 1.47–2.37; p < 0.001), respectively. The geographical distribution differed, showing that the main mutations in African patients were different Asian patients; HMZ p.Q289X (c.865C > T) and HMZ p.Q295X (c.865C > T) accounted for 75% and 37.9% of the African and Asian cases, respectively. The spectrum of CARD9 mutations in Asian patients was higher than in African. Asia is the most populous continent in the world and may have a greater genetic burden resulting in more patients with severe fungal infections. The presence of a high diversity of mutations revealing 24 distinct variations among 60 patients emphasize that the unique genetic alteration in CARD9 gene may be associated with certain geographical areas. This study aimed to perform a systematic review and meta‐analysis of papers discussing the efficacy of microbial synthesised metallic nanoparticles (MNPs) against cancerous and normal cell lines by exploiting Bayesian generalised linear (BGL) model. Data was systematically collected from published papers via Cochrane library, Web of Science, PubMed, Science Direct, ProQuest, Scopus, and Embase. Impressively, most of the studies were carried out on HeLa and A549 cancer cell lines. Specifically, a hefty 65.67% of studies employed bacteria to biofabricate MNPs. Significantly, BGL meta‐analysis represented highly valuable information. Hence, based on adjusted analysis, the MNPs with the size of 25–50 nm were found to be far less cytotoxic than the MNPs with the size of ≤25 nm (OR = 0.233, P ˂ 0.05) against either cancerous or normal cell lines. Interestingly, it was found that the odds of cytotoxicity in cancerous cell lines were practically nine times more than normal cell lines, representing the substantially more cytotoxicity of MNPs in cancerous cell lines (OR = 9.004, P ˂ 0.001).
