Spontaneous gas-bubble nucleation in weak gas–liquid solutions has been a challenging topic in theory, experimentation, and computer simulations. In analogy with recent advances in crystallization and droplet formation studies, the diffusive-shielding stabilization and thermodynamic irreversibility of bulk nanobubble (bNB) mechanisms are revisited and deployed to characterize nucleation processes in a stochastic framework of computer experiments using the large-scale atomic/molecular massively parallel simulator code. Theoretical bases, assumptions, and limitations underlying the irreversibility hypothesis of bNBs, and their computational counterparts, are extensively described and illustrated. In essence, it is established that the irreversibility hypothesis can be numerically investigated by converging the system volume (due to the finiteness of interatomic forces) and the initial dissolved-gas concentration in the solution (due to the single-bNB limitation). Helium nucleation in liquid Pb17Li alloy is selected as a representative case study, where it exhibits typical characteristics of noble-gas/liquid-metal systems. The proposed framework lays down the bases on which the stability of gas-bNBs in weak and supersaturated gas–liquid solutions can be inferred and explained from a novel perspective. In essence, it stochastically marches toward a unique irreversible state along out-of-equilibrium nucleation/growth trajectories. Moreover, it does not attempt to characterize the interface or any interface-related properties, neither theoretically nor computationally. It was concluded that bNBs of a few tens of He-atoms are irreversible when dissolved-He concentrations in the weak gas–liquid solution are at least ∼50 and ∼105 mol m−3 at 600 and 1000 K (and ∼ 80 MPa), respectively, whereas classical molecular dynamics -estimated solubilities are at least two orders of magnitude smaller. Background: Osteoarthritis (OA) is a significant global health concern and is characterized by the degeneration of joint structures, leading to pain, stiffness, and functional limitations It is estimated that 654.1 million individuals are affected by osteoarthritis worldwide, with females, especially those older than 60 years of age, being the population most impacted. However, there remains a gap in understanding the awareness of knee osteoarthritis among specific populations, such as the Palestinian community in the West Bank, thus, highlighting the need for targeted research to address this health disparity. Methods: A cross-sectional survey was conducted in the West Bank from August to September 2024. Participants were recruited from 11 cities and a validated questionnaire adapted from a previous study was used to collect the data. The reliability of the questionnaire was assessed using Cronbach's alpha, yielding a value of standard alpha = 0.71, which is an acceptable value. The survey collected demographic data, clinical characteristics, knowledge of osteoarthritis risk factors, symptoms, and attitudes towards treatment options. R Statistical Software was used for data analysis, which included descriptive statistics, chi-square tests, and logistic regression to explore the relationships between demographic factors and osteoarthritis knowledge, whereby a p-value of ≤ 0.05 was set up as a significance level. Results: A total of 725 valid responses were included in our study. Among the participants, 569 were female, predominantly aged 25 or younger (66%, n = 479), and 62% (n = 450) were single. Knowledge assessment revealed that 80% (n = 580) recognized knee osteoarthritis as a chronic disease, while only 51% (n = 369) understood its impact on joint involvement. The majority identified high BMI (87%, n = 632), advancing age (94%, n = 683), and previous knee injury (72%, n = 520) as risk factors, although only 42% (n = 306) acknowledged genetics as one. Most participants were aware of the management strategies, including analgesics (70%, n = 510) and physiotherapy (80%, n = 579). However, 66% (n = 478) scored low for overall knowledge regarding knee osteoarthritis. Conclusion: This study emphasizes the critical importance of raising awareness and knowledge about osteoarthritis within the Palestinian community. Despite being widely known as a long-term issue, there are still noticeable gaps in the understanding of the joint impact, potential risks, and effective treatment methods for osteoarthritis. Objectives: To evaluate central quadrantectomy and nipple resection with areola preservation (CQ-NR-AP) as a new reconstructive oncoplastic technique Versus Grisotti flap mammoplasty (GFM) in central malignant tumors of the breast extending to the nipple, in terms of time procedures, breast symmetry, patient satisfaction, postoperative complications, and local recurrence. Patients and methods: The current study is a single-blind, single-center, randomized, controlled trial that was performed between May 2018 and May 2023 in the breast surgery unit of University Hospitals. This trial involved 40 individuals who had central breast lesions that extended to the nipple and were monitored for two years following surgery. Results: As regards the mean intra-operative time in minutes, in the group (I) was 80.1 with a standard deviation of ± 13.9, and ingroup (II) was 138.9 with a standard deviation of ± 14.02 (p = 0.001). The seroma was detected in zero cases in group (I) and 2(10%) cases in group II (p = 0.487) and those two cases were managed by aspiration only. Regarding, the wound infection was found in one case (5%) in group (I) and 3(15%) cases in group II (p = 0.605). Regarding patient satisfaction and breast, symmetry was much better in the group (I). Conclusion: The safety and ease of central quadrantectomy and nipple resection with areola preservation were demonstrated in a two-year follow-up, with a lower incidence of complications compared to the Grisotti flap mammoplasty technique. Furthermore, this approach was associated with higher patient satisfaction, which is a significant achievement in the management of centrally located breast tumors. Background: Oxygen is an essential drug that is commonly used in clinical practice, and its misadministration can result in severe consequences. This research sought to evaluate the knowledge and perceptions of physicians and nurses regarding acute oxygen therapy and delivery barriers. Methodology: This was a cross-sectional hospital-based survey. The study was conducted at Managil Teaching Hospital, Sudan. The study included 159 physicians and 25 nurses who were working at the hospital during the study period. The previously validated acute oxygen therapy questionnaire (AOTQ) was used to assess knowledge and barriers regarding acute oxygen therapy (AOT). The collected data were then analysed via R software. Results: A total of 184 medical professionals were included in the survey. The mean age of the study participants was 26 years, and 66.2% of all participants were male. A total of 74% of the physicians were junior physicians. Among the physicians, 15 (9.4%) had good knowledge (score > 80%), 75 (47.2%) had moderate knowledge (score > 60% & less than 80%), and 69 (43.4%) had poor knowledge about acute oxygen therapy (score < 60%). Among the nurses, 2 (8%) had good knowledge, 10 (40%) had moderate knowledge, and 13 (52%) had poor knowledge. Most of them were from medical departments, with 33% from physicians and 40% from nurses. Approximately 64% of the physicians and 68% of the nurses stated that oxygen is not a medication rather than a supportive therapy, and only 13% of the physicians and 28% of the nurses knew that oxygen should be given only after the doctor’s prescription. The availability of oxygen was reported as a barrier by 85% of the physicians and 72% of the nurses. Conclusion: A considerable percentage of physicians and nurses exhibited a limited understanding of acute oxygen therapy, demonstrated infrequent adherence to AOT guidelines due to a lack of awareness, and reported germane barriers to delivery that necessitate administrative and educational interventions. Background: The WHO defines mental health as well-being where individuals manage stress, work productively, and contribute to their community. Children's mental health is critical, especially among vulnerable groups like refugees. Research shows refugee children, including Sudanese, face PTSD, depression, and anxiety due to conflict and displacement, yet specific studies on Sudanese children are scarce. East Africa faces high prevalence due to poverty, conflict, and inadequate healthcare. Sudan's situation is exacerbated by ongoing conflict and limited resources, despite efforts from NGOs and international organizations. Methodology: This cross-sectional descriptive study examines the mental health of Sudanese school-age children (6–18 years) in Ad-Damar, River Nile, Sudan, using a simple random sampling technique. The study included 246 participants. Data were collected using an author-designed questionnaire based on the Strengths and Difficulties Questionnaire (SDQ), supplemented with demographic information. Data management involved secure handling and ethical considerations, including informed consent and confidentiality. Statistical analysis was performed using SPSS V.25 to ensure accuracy and reliability of the findings. Results: The sample included children aged 6–18 years, with 54% males and 46% females. Displacement duration varied, with 34% displaced for 1–2 years. Socio-demographic findings indicated 39% lived with both parents, 56% were in urban environments, and 47% were in primary school. Mental health assessments revealed 68% frequently felt sad, 73% felt anxious, and 29% had diagnosed conditions, including PTSD and depression. Correlations showed significant positive relationships between overall mood ratings and both school support (r = 0.45, p < 0.01) and family support (r = 0.35, p < 0.05). Chi-Square tests indicated significant associations between gender and mental health diagnoses (χ²=5.67, p = 0.017) and family structure and perceived support (χ²=12.34, p < 0.05). Conclusion: The study highlights the urgent need for better mental health support for displaced children, given the high prevalence of mental health issues and inadequate support from schools and families. Future research should include longitudinal studies to evaluate long-term effects and intervention effectiveness. Background: This study compared the results of stapled hemorrhoidopexy (SH) and harmonic scalpel hemorrhoidectomy (HSH) in the management of grade III and grade IV piles regarding the time of the procedure, postoperative pain, patient satisfaction, wound infection, bleeding, incontinence, and recurrence within 1 year. Patients and methods: This was a single-blind, prospective, randomized, controlled, single-center trial conducted from January to December 2022 that included 50 (68.75%) male and 20 (31.25%) female patients with third- and fourth-degree piles. Results: The patients were divided into two groups of 35 patients each. Group I underwent SH and group II underwent HSH. The mean age of group I was 42.94 years and of group II, 42.20 years. The mean time of the procedure was 24.42 min ± 2.367 for SH and 31.48 min ± 2.21 for HSH. Postoperative pain in group I was lower than in group II during the first 2 weeks, but there was persistent mild pain in most patients in group I at the 2‑week follow-up. In group II there was significant improvement in pain after 2 weeks, with higher patient satisfaction. Wound infection was detected in 3 (5%) patients in group I and no patients in group II (p = 0.077). Postoperative bleeding occurred in 4 (11.4%) patients in group I in the form of spotting after defecation only during the first postoperative month; no bleeding was detected in group II (p = 0.039). There were 3 (15%) cases of flatus incontinence but after taking a detailed history these were found to be cases of urgency to defecate rather than incontinence. There were 7 (20%) cases of recurrence at the 1‑year follow-up in group I and 1 (2.9%) case in group II (p = 0.024). Conclusion: Compared with SH, HSH was safer, easier, and associated with a lower incidence of recurrence after 1 year and with higher patient satisfaction. [ABSTRACT FROM AUTHOR]

The study aims to study the provisional attachment of documentary letters of credit to prevent the beneficiary from liquidating the value of the credit as the beneficiary used fraudulent methods without which he would not have been able to obtain the value of the credit. The study follows the descriptive analytical comparative approach. It will discuss the legislative and judicial efforts related to the provisional attachment of documentary letters of credit. This will lead us to the comparative approach to comparing what is stated in Emirati and Saudi law and the judgments. The study concludes several findings, including that documentary letters of credit are an unquestionable means of success in international trade operations, especially since they discuss the problems of geographical distance between the seller and the buyer. However, fraudulent behaviors by sellers (beneficiaries of the documentary letters of credit) disrupt the dominance of documentary letters of credit as the most important means of payment in international trade transactions. The study concludes with several recommendations, namely: the necessity of adopting the Uniform Customs and Practice (UCP) for Documentary Letters of Credit as a rule to eliminate instances of fraud in its transactions. Fasciolosis is a zoonotic infection and is considered a developing deserted tropical illness threatening ruminant productivity and causing financial losses. Herein, we applied immunoinformatics and biophysics studies to develop an epitopes vaccine against Fasciola hepatica using glutathione transferase and Cathepsin L-like proteinase as possible vaccine candidates. Using the selected proteins, B- and Tcell epitopes were predicted. After epitopes prediction, the epitopes were clarified over immunoinformatics screening, and only five epitopes, EFGRWQQEKCTIDLD, RRNIWEKNVKHIQEH, FKAKYLTEMSRASDI, TDMTFEEFKAKYLTE, and YTAVEGQCR were selected for vaccine construction; selected epitopes were linked with the help of a GPGPG linker and attached with an adjuvant through another linker, EAAAK linker. Cholera toxin B subunit was used as an adjuvant. The ExPASy ProtParam tool server predicted 234 amino acids, 25.86257 kDa molecular weight, 8.54 theoretical pI, 36.86 instability index, and -0.424 grand average of hydropathicity. Molecular docking analysis predicted that the vaccine could activate the immune system against F. hepatica. We calculated negative binding energy values. A biophysics study, likely molecular docking molecular dynamic simulation, further validated the docking results. In molecular dynamic simulation analysis, the top hit docked compounds with the lowest binding energy values were subjected to MD simulation; the simulation analysis showed that the vaccine and immune cell receptors are stable and can activate the immune system. MMGBSA of -146.27 net energy (kcal/mol) was calculated for the vaccine-TLR2 complex, while vaccine-TLR4 of -148.11 net energy (kcal/mol) was estimated. Furthermore, the C-ImmSim bioinformatics tool predicted that the vaccine construct can activate the immune system against F. hepatica, eradicate the infection caused by F. hepatica, and reduce financial losses that need to be spent while protecting against infections of F. hepatica. The computational immune simulation unveils that the vaccine model can activate the immune system against F. hepatica; hence, the experimental scientist can validate the finding accomplished through computational approaches. Background: Anterior cruciate ligament injury poses a major effect on the quality of life of patients. Anterior cruciate ligament reconstruction (ACLR) remains the mainstay of treatment. Purposes: To (1) test for the correlation of patients' characteristics such as body mass index (BMI), age, smoking status, and postoperative infrapatellar numbness with the subjective outcome of primary ACLR and (2) investigate whether postsurgical complications can be related to the studied patients' characteristics. Study Design: Case series; Level of evidence, 4. Methods: Demographic data and operational notes from patients who underwent primary ACLR at the institute between 2011 and 2022 were extracted. Patients' surgical outcomes were evaluated subjectively using the International Knee Documentation Committee (IKDC) score and the Knee injury and Osteoarthritis Outcome Score (KOOS). Pre- and postoperative scoring were completed at postoperative follow-up in December of 2022, patients completed both the IKDC and the KOOS questionnaires. Patients were asked to rate their knee function out of 10 before and after surgery. This rating was based on the patients' self-assessment of their functional ability without the use of a formal instrument. Knee function was assigned as better, worse, or the same through a comparison of patients' reported scores before and after surgery. Results: Male patients comprised 108 (98.2%) of the total cohort, the mean ± SD age at the time of surgery was 27.92 ± 7.39 years, and BMI was 26.54 ± 3.89. The mean follow-up time was 48.92 ± 27.99 months. Most patients did not need postsurgical intervention (n = 97; 88.2%), while 4 (3.6%), 7 (6.4%), and 2 (1.8%) needed revision, debridement, or both, respectively. The mean IKDC score and KOOS, respectively, were 74.37 ± 20.17 and 85.50 ± 14.83. Patients' BMI had a significant effect on IKDC score (74.68 ± 20.0; P <.001), and infrapatellar numbness after surgery also affected patients' IKDC score (P =.002) and KOOS (P =.043). The age at the time of ACLR was associated with contralateral leg surgery (P =.017). Conclusion: This study showed that higher BMI and infrapatellar numbness negatively affect the subjective outcomes after primary ACLR. Background/ Aim: Amyotrophic lateral sclerosis (ALS) is a neurodegenerative disease affecting upper and lower motor neurons, causing progressive atrophy of muscles, hypertonia, and paralysis. This study aimed to evaluate the current evidence and effectiveness of ultrasound in investigating nerve cross-sectional area (CSA) of peripheral nerves, vagus and cervical roots in those with ALS compared with healthy controls and to pool the CSA measurements. Methods: A systematic search was conducted on Cochrane, Clarivate Web of Science, PubMed, Scopus, and Embase for the mesh terms nerve, ultrasonography, and amyotrophic lateral sclerosis. A quality assessment was performed using the New-Ottawa scale. In addition, a double-arm meta-analysis using Review Manager 5 software version 5.4 was performed. Results: From the seventeen studies included in this review, the overall mean difference showed that individuals with ALS had a significantly smaller CSA in comparison to healthy controls for median, ulnar, C6 root, and phrenic nerves. However, no significant difference in the CSA was found in radial, vagal, sural, and tibial nerves. Discussion: This study confirmed results of some of the included studies regards the anatomic sites, where nerve atrophy in ALS could be detected to potentially support the diagnosis of ALS. However, we recommend further large, prospective studies to assess the diagnostic value of these anatomical sites for the diagnosis of ALS. Conclusions: Our findings confirmed specific anatomic sites to differentiate ALS patients from healthy controls through ultrasound. However, these findings cannot be used to confirm the ALS diagnosis, but rather assist in differentiating it from other diagnoses. Background: Epithelial–mesenchymal transition (EMT) is a key step in the development of colorectal cancer (CRC) that confers metastatic capabilities to cancer cells. The present study aimed to assess the immunohistochemical (IHC) expression and impact of EMT markers, including E-cadherin, Vimentin, β-catenin, and SMAD4, on the oncologic outcomes of CRC. Methods: This was a retrospective review of 118 CRC patients. Tissue slides were retrieved from the slide archive and five tissue microarray construction blocks were constructed. IHC for E-cadherin, Vimentin, β-catenin, and SMAD4 was done. The main outcome was the association between abnormal marker expression and overall survival (OS), and disease-free survival (DFS). Results: Adenocarcinomas accounted for 71.2% of tumors, whereas 25.4% and 3.4% were mucinous and signet ring cell carcinomas. The rates of lymphovascular invasion and perineural invasion were 72.9% and 20.3%, respectively. There was a positive, significant correlation, and association between the four markers. Abnormal expression of E-cadherin was associated with significantly lower OS (p < 0.0001) and similar DFS (p = 0.06). Abnormal Vimentin expression was associated with a significantly higher rate of distant metastasis (p = 0.005) and significantly lower OS and DFS (p < 0.0001). Abnormal expression of β-catenin was associated with significantly lower OS (p < 0.0001) and similar DFS (p = 0.15). Abnormal expression of SMAD4 was associated with significantly lower OS and DFS (p < 0.0001). Abnormal expression of all four markers was associated with a higher disease recurrence, lower OS, and lower DFS. This study examines the influence of risk disclosure (RD) in the annual reports on Firm Value (FV) in the UK context. Furthermore, it addresses the moderating role of the analyst information environment in shaping this relation. Our study distinguishes between favourable and unfavourable information and examines whether the different nature of risk information could affect the FV differently. Our study contributes to the existing literature by providing empirical evidence on the value relevance of the narrative risk information. In particular, using a sample of UK listed firms, we find a positive relation between risk information and FV. We also contribute to the dilemma of risk disclosure measurement by using four different RD proxies. Our study has important implications for academics, standard setters, investors and managers. This study argues that different definitions/perceptions of risk information could affect investors' decisions differently. Using a sample of 328 non‐financial UK firms and departing from existing literature, this study measures corporate risk disclosure (RD) via computerized content analysis to capture four different perspectives of defining RD. This study investigates (i) the effects of these RD measures on the Cost of Capital (COC), (ii) the influence of analysts' coverage on the relationship between RD and COC, and (iii) whether the RD nature (favourable/unfavourable) might affect COC differently. Lenders and equity holders are found not to consider any risk information expressed as a variation around a target, while only lenders consider risk information that reveals negative outcomes. However, lenders and equity holders consider risk information that expresses negative and positive outcomes together. Besides, firms that disclose extra risk information and have a large analyst following suffer from a higher Cost of Equity (COE) compared with those with fewer analysts following. Additionally, lenders impose a lower interest rate on firms with a higher unfavourable RD, while equity holders ask for lower returns from the firms with a higher favourable RD. Background: The neutrophil-to-lymphocyte ratio (NLR) proves to be a convenient and cost-effective marker with studies showing that a high NLR can serve as a mortality indicator in burn cases. We conducted a meta-analysis aiming to explore whether on-admission NLR values could serve as predictors of mortality in burn patients. Methods: PubMed, Web of Science, Scopus and Embase were searched from inception until January 2024. We included all studies investigating burn patients that contain information on the NLR value at the time of hospital admission and mortality outcomes. The studies were critically appraised using the NIH Quality Assessment Tool. Results: Nine studies fulfilled our criteria with a total population of 1837 participants, including 1526 survivor Burn patients and 311 non-survivor Burn patients. The overall mean difference measured by random model showed a significant increase in NLR by 5.06 (95% CI 3.42, 6.68) p ≤ 0.001 for the non-survivor group over the survivors group with heterogeneity I2 = 67.33%, p ≤ 0.001. A meta-regression was done to investigate the potential source of heterogeneity among studies. The results showed that age (p = 0.394), gender (p = 0.164), and sample size (p = 0.099) did not contribute to the source of heterogeneity, however, the burn surface area contributed significantly (p = 0.002). A leave-one-out meta-analysis was done, showing that omitting Le Qui et al., leads to significantly decrease the heterogeneity to be I2 = 2.73%. Meta-regression repeated to assess the burn surface area again to be found noncontributing (p = 0.404). Conclusions: Our findings support that elevated NLR values can serve as a mortality indicator in burn cases. Ventral abdominal wall incisional hernia is defined as a defect in the musculo-fascial layers of the abdominal wall in the region of the postoperative scar. There is a slight increase in the incidence of incisional hernia in the female gender. The higher percentage of incisional hernia in females might be due to laxity of abdominal wall muscles after multiple pregnancies and also an increased incidence of obesity in females. To assess incisional hernia repair using two different techniques: on-lay mesh and sub-lay mesh, as regards operative time, postoperative recurrence, wound infection, seroma, hematoma, and flap necrosis. Pubmed, Web of Science, and Scopus were searched on 15 March 2022. The keywords incisional hernia, sub-lay mesh on-lay mesh, retromuscular mesh, and polypropylene. According to our results, there is a statistical difference between onlay and sublay regarding intra-operative time as sublay mesh is more time-consuming. Regarding postoperative complications, there is no statistical difference in recurrence, seroma, hematoma, flap necrosis, and infection but there is a statistical difference regarding in hospital stay as patients with sub-lay repair stays less than only. This research aimed to assess the hematological traits of male adults diagnosed with diabetes and investigate the relationship between blood sugar levels with hematological factors among patients. A cross-sectional comparison study was conducted at Fallujah Teaching Hospital from April 1 to July 30, 2023. The research comprised 185 volunteers, including 125 individuals with type 2 diabetes mellitus (65 with well blood sugar levels and 60 with poorly-regulated blood sugar levels) and 60 healthy individuals serving as controls. The evaluation of hematological parameters was conducted using Swelab-Alfa. An independent Ttest was used for assessment. The patients exhibited substantially decreased mean absolute lymphocyte count, Hct, MCHC, and PLT values compared to the control group. The diabetic group had significantly higher mean values for total neutrophil count, absolute basophil counts, RDWSD, RDWCV, PDW, PLCR, and MPV than the control group. Patients with poor glycemic control had substantially elevated levels of Mon, Eos, Bas, MCHC, PLT, MPV, PLCR, and PCT. In contrast, individuals with poor glycemic control had substantially lower levels of Neu, RBC count, and PDW. The findings demonstrated a statistically significant positive connection between neutrophil count, MCV, MCH, MCHC, PDW, MPV, PLCR, and PCT with FBG. Lym, RBC count, and Hct exhibited a statistically significant inverse connection with FBG in individuals with type 2 diabetes mellitus (T2DM). On a flat city road, the specificity of the suspension poses a challenge. Sport utility vehicles (SUVs) have less flexibility compared to regular cars, despite providing a firm grip on asphalt and concrete. Constant adjustments to the center of gravity of SUVs are necessary. Surprisingly, they are less reliable and stable in urban settings due to this characteristic. In this study, a mathematical model of the suspension system of SUVs based on the Newtonian approach is introduced and validated with data from experiments carried out by the MTS machine system on the mono-tube (oil/air) mixed damper element. The model accurately predicts the performance of this damper, commonly used in SUVs, across various operating conditions, including different frequencies. A coil spring element, serving as a passive suspension unit with this damper, is also tested experimentally under similar conditions. By integrating passive suspension elements with the active actuator, the proposed modified design reduces the power consumption needed for the actuator to function and ensures a certain level of reliability. The effectiveness and performance of the modified active suspension system in comparison to the traditional passive suspension system are assessed using three different strategies: hybrid PID-LQR, linear quadratic regulator (LQR), and proportional-integral-derivative (PID). The genetic algorithm is utilized to determine the optimal parameter values for each controller by minimizing a cost function, maximizing performance, and minimizing energy consumption. Simulation results demonstrate that the active suspension system controlled by the PID-LQR controller offers significantly improved ride comfort and vehicle stability compared to other systems. One of the most common cancers among women worldwide is breast cancer (BC), and early diagnosis can save lives. Early detection of BC increases the likelihood of a successful outcome by enabling treatment to start sooner. Even in areas without access to a specialist physician, machine learning (ML) aids in early BC detection. The medical imaging community is becoming more interested in using ML, and deep learning (DL) to increase the accuracy of cancer screening. Many disease-related data are sparse. However, for DL models to perform well, a large amount of data is required. Because of this, the DL models that are currently in use on medical images are not as effective as they could be. Convolutional neural network (CNN) models have recently gained popularity in the medical industry, and they perform admirably in terms of high performance and robustness at image classification. The proposed method classifies data using ensemble pre-trained models such as the dense convolutional network (DenseNet)-121 and EfficientNet-B5 feature extractor networks, as well as the support vector machine for classification. Using a modified meta-heuristic optimizer, the selected pre-trained CNN hyperparameters were optimized to improve the performance. The experimental results for the presented model on the INbreast dataset show that the EfficientNet-B5 model is effective for BC classification, with overall accuracy, sensitivity, specificity, precision, and area under the ROC curve (AUC) values of 99.9%, 99.9%, 99.8%, 99.1%, 1.0, respectively. Background: Blended learning is a key educational methodology, particularly in medical education, and involves integration of online and face-to-face interactions to enhance flexibility and engagement. Blended learning gained increased popularity during the COVID-19 pandemic due to social restrictions. Following control of the pandemic, face to face teaching and learning activities have been restored. However, some institutions continue to deliver some teaching online. This study explores the perceptions and experiences of undergraduate medical students from 15 institutions across seven countries regarding remote online learning in the post-pandemic era. Methods: This cross-sectional study utilized an online survey to gather insights into the use of learning resources, interactivity in online sessions, barriers to online learning, and preferences for learning modalities. Descriptive data were summarized by frequency, categorical comparisons were assessed with chi-squared tests, and t-tests were used for continuous data. Results: The findings of the current study show a general preference for blended learning (47.78%) over traditional face-to-face instruction (41.48%). Key benefits of blended learning reported by the participants by comfort, flexibility, reduced travel, and ability to learn at own pace. The key barriers identified were internet connectivity issues and family distractions. The study also highlighted the limitations of online learning in replacing clinical experience and practical skills acquisition, with 69.26% of respondents affirming that online learning does not sufficiently substitute for direct patient contact. Conclusion: The findings underscore the importance of integrating face to face and remote online teaching and learning frameworks to align with educational objectives, particularly in fostering interactivity and practical skill development. The study suggests that while blended learning has many benefits, its effectiveness is highly context-dependent and requires thoughtful implementation to meet the diverse needs of medical education. Background Intrusive "thoughts" represent undesirable cognitive activity that can cause distress, and occurs in individuals with and without psychological disorders. In order to deal with unwanted intrusive thoughts, individuals might consciously attempt to halt the flow of these cognitions through suppression or unconsciously avoid them automatically through repression. This study aimed to psychometrically evaluate and validate a translation of the Emotional and Behavioral Reaction to Intrusions Questionnaire (EBRIQ) in Arabic, for adults who speak the language. Methods The snowball sampling technique was used to recruit adults (n = 755) from five Arab countries (Lebanon, United Arab Emirates, Egypt, Jordan, and Kuwait), who completed the Arabic EBRIQ. A Confirmatory Factor Analysis (CFA) was conducted to examine the factor structure of the EBRIQ. Results A total of 755 participants completed the survey, with a mean age of 21.89 ± 4.18 years and 77.5% females. CFA indicated a modest fit for the one-factor model. Internal reliability was excellent (ω = 0.96; α = 0.96). No significant difference was found in terms of EBRIQ scores between males (M = 10.37, SD = 7.80) and females (M = 10.52, SD = 7.99) in the total sample, t(753) = − 0.22, p =.830. The highest EBRIQ scores were found in Jordanian participants (12.55 ± 6.94), followed by Emirati (12.23 ± 8.20), Lebanese (11.12 ± 7.69), Egyptian (8.96 ± 8.05) and Kuwaiti (8.20 ± 7.75) participants, F(4, 750) = 10.36, p <.001. Conclusion This study suggests that our Arabic translation of the EBRIQ is psychometrically proven to be reliable for use in Lebanon, United Arab Emirates, Egypt, Jordan, and Kuwait. Background: Minimally invasive mitral valve surgery has been introduced as a safe alternative to full sternotomy through smaller chest wall incisions to reduce the potential sternal complications in addition to the favorable aesthetic effect. This observational study has been designed to evaluate and compare operative and short-term postoperative outcomes of video-assisted versus right anterior thoracotomy mitral valve replacement. The study was conducted on 40 cases and divided into two groups: group 1, comprised of 20 patients who underwent video-assisted mitral valve replacement (VAMVR), and group 2, comprised of 20 patients who underwent right anterior mini-thoracotomy mitral valve replacement (ATMVR). Preoperative data, intraoperative procedures, and early postoperative results have been compared between both techniques. Results: Intraoperative parameters showed no statistical difference between the two groups. Wound length in the VAMVR group was statistically smaller than the ATMVR group, 5.33 ± 0.96 cm versus 7.08 ± 1.24 cm, respectively (P < 0.001). The intraoperative assessment by TEE showed a well-suited, well-functioning prosthetic valve, and no paravalvular leak in the two groups. In both groups, there were statistically significant reductions in left ventricular end diastole, left atrial diameter, and pulmonary artery systolic pressure in the 3-month postoperative ECHO in comparison to the preoperative ECHO (P < 0.001). There was a statistically significant decrease in left ventricular end-systole in the VAMVR group in the 3-month postoperative ECHO in comparison to the preoperative ECHO (P < 0.001). ICU visual analog scale (VAS) was significantly lower in the VAMVR group than in the ATMVR group (P = 0.049). There was a gradual reduction in VAS in both groups along the period of 3 months postoperatively without significant differences between the two groups in 2 weeks and 3 months postoperatively. Conclusions: VAMVR and ATMVR could be considered safe, feasible approaches with favorable perioperative surgical and echocardiographic results. VAMVR showed statistically significant smaller wounds and less postoperative pain than ATMVR, which may offer potential advantages in terms of reduced postoperative discomfort and cosmetic appearance for the thoracoscopic technique. Background: Few studies have investigated the effects of wood dust on carpenters, who are frequently exposed to this hazardous material. This study aimed to determine the effects of wood dust exposure on carpenters pulmonary function test (PFT) parameters. Methods: This comparative cross-sectional community-based study included 130 participants; a group of carpenters and a matched cohort. Standard techniques were used to measure each participant's force vital capacity (FVC), force expiratory volume in one second (FEV1), FEV1/FVC ratio, and peak expiratory flow rate (PEFR). Statistical tests were employed to investigate different variables. A P value of < 0.05 is considered significant. Results: Carpenters had a mean age of 51.6 ± 19.9 years, whereas the matched cohort had a mean age of 49.8 ± 13.9 years. Approximately 23 (35.4%) carpenters experienced respiratory problems. Carpenters had significantly lower FEV1, FVC, FEV1/FVC%, and PEFR values than the matched cohort (P < 0.001). Additionally, the association between the carpenters duration of exposure to wood dust and their PFT values showed a significant statistical difference (P < 0.05). Likewise, the correlation between the duration of exposure to wood dust and their PFT values was significant (< 0.001). Conclusions: Wood Dust's Effects on carpenters' respiratory systems are worth further discussion. The dynamic, complex process of cutaneous wound healing is required to restore skin integrity following an injury. This intricate process consists of four sequential and overlapping phases: hemostasis, inflammation, proliferation, and remodeling. Hemostasis immediately begins to function in response to vascular injury, forming a clot that stops the bleeding. To fight infection and remove debris, immune cells are enlisted during the inflammatory phase. Angiogenesis, re-epithelialization, and the creation of new tissue are all components of proliferation, whereas tissue maturation and scarring are the outcomes of remodeling. Chronic wounds, like those found in diabetic ulcers, frequently stay in a state of chronic inflammation because they are unable to go through these stages in a coordinated manner. The important regulatory roles that microRNAs (miRNAs) play in both normal and pathological wound healing have been highlighted by recent investigations. The miRNAs, small non-coding RNAs, modulate gene expression post-transcriptionally, profoundly impacting cellular functions. During the inflammatory phase, miRNAs control pro- and anti-inflammatory cytokines, as well as the activity of immune cells such as neutrophils and macrophages. Additionally, miRNAs are essential components of signaling networks related to inflammation, such as the toll-like receptor (TLR), nuclear factor kappa B (NF-kB), and Janus kinase/signal transducers and activators of transcription (JAK/STAT) pathways. Some miRNAs have been discovered to either increase or alleviate inflammatory reactions, indicating their potential as therapeutic targets. Other miRNAs aid in angiogenesis by promoting the development of new blood vessels, which are essential for providing oxygen and nutrients to the healing tissue. They also affect keratinocyte migration and proliferation during the re-epithelialization phase, which involves growing new epithelial cells over the lesion. Another function of miRNAs is that they control the deposition of extracellular matrix (ECM) and the creation of scars during the remodeling phase. The abnormal expression of miRNAs in chronic wounds has led to the exploration of miRNA-based treatments. With a focus on resistant instances such as diabetic wounds, these therapeutic techniques seek to improve wound healing results by correcting the dysregulated miRNA expression. Background: Nurses are the cornerstone of healthcare delivery, playing a pivotal role in ensuring safe, high-quality, and patient-centered care. However, the significant demands on their time and resources often lead to the rationing or omission of essential nursing care activities, undermining patient outcomes. As frontline caregivers, nurses' ability to collaborate effectively within teams is critical to overcoming these challenges. The Big Five Teamwork Model, which emphasizes trust, communication, and leadership, offers a transformative approach to strengthening teamwork and addressing barriers to comprehensive nursing care. Aim: This study aimed to assess the impact of the Big Five Teamwork Awareness Sessions (Big 5TWAS) on the rationing of nursing care and patient-centered care. Methods: A quasi-experimental pre-post-test design was conducted in critical care units at an Egyptian university hospital. A convenience sample of 60 nurses and 31 admitted patients participated in the study. Big 5TWAS interventions were implemented for nurses, and study variables were measured using nursing teamwork surveys, the Rationing of Nursing Care Observational Checklist (RONCO), and patient-centered care questionnaires before and after the intervention. Data were analyzed using descriptive and inferential statistics. Results: The Big 5 Teamwork Awareness Sessions (Big 5TWAS) demonstrated a significant impact on the study variables. Post-sessions, there was a significant decrease in rationing of nursing care scores and significant increases in nursing teamwork and patient-centered care scores (p ≤ 0.001). In addition to the significant correlation values, regression analysis revealed that overall teamwork accounted for approximately 22% of the variance in rationing of nursing care (R² = 0.224, p = 0.015), with trust as the significant predictor. Furthermore, teamwork explained approximately 80% of the variance in patient-centered care (R² = 0.801, p < 0.001), with trust, backup, and team leadership emerging as significant predictors. Conclusion and recommendations: The study underscores the critical role of teamwork in reducing the rationing of nursing care and enhancing patient-centered care in critical care units. The Big 5TWAS was effective in fostering improved team dynamics, trust, and leadership, which translated into better care delivery. These findings highlight the need for ongoing inter-professional training and teamwork-enhancing strategies led by nurse managers to sustain and amplify these improvements. Objective: Epidural scar fibrosis commonly leads to functional disability and pain following spinal surgery and is a prevalent manifestation of Failed Back Surgery Syndrome (FBSS). This study aimed to evaluate the use of silver nano-articles (AgNPs) loaded on chitosan (Chi/Ag-NPs) with platelets-rich fibrin (PRF) gel for the reduction of post-laminectomy epidural scar adhesions. Methods: A total of 90 male Sprague Dawley rats (255 ± 55gm) were randomized in-to six groups, each group with 15 rats: control group, laminectomy group, PRF group, Chi/Ag-NPs group, combined treatment group (PRF + Chi/Ag-NPs), and a group to prepare PRF. Lumbar laminectomy procedures were performed between L3-L5 in all rats except the control group. After a 30-days follow-up, macroscopic examination, histological studies, and mRNA evaluation for TGFβ-1and IL-6, were conducted. Results: Data revealed that epidural scar adhesion, scaring, arachnoid involvement, dural thickness, as well as inflammation and TGFβ-1and IL-6 coding genes expression were significantly reduced in PRF group, Chi/Ag-NPs group, and combined group compared to the laminectomy group. Combined treatment showed more significant better outcomes. Conclusion: The use of PRF with Chi/Ag-NPs as nano biomaterials could be considered a combination therapy for the reduction of EF post-laminectomy in a rat model. [ABSTRACT FROM AUTHOR]

Background: There has been an increasing comprehension and recognition of endocrine dysfunction among both pediatric and adult patients with sickle cell disease (SCD). Thyroid disorders can have significant clinical consequences, including growth retardation and impaired cognitive function. However, there is a disparity in the available data concerning the magnitude and spectrum of thyroid abnormalities in this population. This review aimed to provide a systematic summary and analyses on the status of thyroid function abnormalities in individuals with SCD. Methods: Following the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines, a comprehensive search was conducted across Medline/PubMed, Google Scholar, World Health Organization Virtual Health Library Regional Portal, and ScienceDirect. Pooled prevalence and standardized mean difference (SMD) estimates with 95% confidence intervals (CIs) were calculated using Comprehensive Meta-Analysis Software version 3.3. Results: Nineteen studies met the inclusion criteria and were incorporated into the analyses. Serum thyroid-stimulating hormone (TSH) levels were significantly higher in SCD patients compared to controls (SMD = 1.184; 95% CI, 0.269–2.099; p = 0.011). While non-significant, there was a trend towards lower levels of triiodothyronine (T3), thyroxin (T4), free T3, and free T4 in the SCD group (T3: SMD = -1.746; 95% CI, -3.561–0.070; p = 0.059; T4: SMD = -1.365; 95% CI, -3.030–0.300; p = 0.108; free T3: SMD = -0.384; 95% CI, -1.128–0.356; p = 0.311; free T4: SMD = -1.205; 95% CI, -2.522–0.111; p = 0.073). The pooled prevalence of hypothyroidism and subclinical hypothyroidism among SCD patients was found to be 4.9% and 8.7%, respectively. Conclusion: Individuals with SCD exhibit a tendency towards elevated TSH levels compared to the general population, with a subset potentially developing thyroid abnormalities, particularly subclinical hypothyroidism. Although not highly prevalent in the SCD population, monitoring thyroid function remains essential due to the potential for progression to overt hypothyroidism and its associated adverse health outcomes. Background and Objectives: Understanding the knowledge and dietary patterns of type 2 diabetes mellitus (T2DM) patients is essential to identify gaps and design tailored health education strategies to improve self-management and clinical outcomes. We assessed the diabetes-related dietary patterns, knowledge, and associated factors of T2DM patients. Methods: The study utilized a cross-sectional design, surveying 363 T2DM patients using a validated and pretested questionnaire. Knowledge levels were categorized as low (<50%), medium (50–75%), and high (>75%), and dietary patterns were classified as unhealthy (<34%), moderately healthy (34–67%), and healthy (>67%). We analyzed the data using the Statistical Package for the Social Sciences (SPSS, version 23.0). The authors tested the relationship between diabetes-related dietary patterns and knowledge with Spearman's analysis. The multivariate regression approach established the factors associated with these two components. Results: The findings revealed that 36.4% of participants had low knowledge, 34.4% had medium knowledge, and only 29.2% demonstrated high knowledge. Regarding dietary patterns, 34.4% were categorized as unhealthy, 33.3% as moderately healthy, and 32.3% as healthy. The authors found that these two domains were positively correlated (rho = 0.649, p = 0.001). Diabetes-related knowledge was significantly associated with gender (p = 0.018), duration of diabetes (p = 0.001), and patients with a family history of T2DM (p = 0.007). The dietary pattern was significantly associated with age (p = 0.001), duration of diabetes (p = 0.032), and presence of other chronic diseases (p = 0.028). Conclusions: The findings underscore the need for targeted health education strategies that address gaps in dietary knowledge and promote healthier eating behaviors among T2DM patients in Central Saudi Arabia. Purpose: The thyroid gland is one of the most vital endocrine organs. It is responsible for the synthesis and secretion of hormones principally triiodothyronine (T3) and thyroxine (T4). These hormones play a significant role in the functions and the metabolism of the body. The thyroid gland could be affected by several infections, among them Toxoplasma gondii. Congenital toxoplasmosis took place when the parasite reached the developing fetus and infected any nucleated cells within it. This study assessed the effect of experimental congenital toxoplasmosis on the thyroid gland structure and function. Methods: We used 34 Wistar rats and allocated them into two groups: normal control group (17 rats) and congenital toxoplasmosis group (17 rats). After euthanasia, the brain and the thyroid gland was assessed through histopathological examination. Thyroid functions examination was performed through measuring the serum levels of T3, thyroxine T4, and thyroid stimulating hormone (TSH). Moreover, serum levels of thyroid antibodies [thyroid-peroxidase antibody (TPO-Ab) and anti-thyroglobulin (TG-Ab)] were examined. Results: The examination of thyroid tissues of the congenital toxoplasmosis group showed decreased or absent colloid secretion. About 47.1% of follicles showed degeneration with different grades. Parafollicular cells hyperplasia were observed in 23.6% of specimens. The serum concentrations of T3, T4, and TSH were significantly higher in congenital toxoplasmosis group than the control group. The congenital toxoplasmosis group had lower serum concentrations of TPO-Abs than the control group. Conclusion: These results indicated that congenital Toxoplasma infection could result in a central hyperthyroidism state with alteration of thyroid gland structure in offspring. Purpose: Tongue base mucosectomy (TBM) is a well-established procedure in investigating cervical squamous cell carcinoma of occult primary. However, its risks have not been balanced against its benefits with validated tools. Methods: A systematic literature review was conducted for reported complications and quality-of-life outcomes following TBM. The complications and quality-of-life outcomes following TBM at our institution are then reported using objective metrics and validated assessment tools, including Performance Status Scale for Head and Neck Cancer Patients (PSS-HNS), University of Washington Quality-of-life Questionnaire (UW-QOL) and M. D. Anderson Dysphagia Inventory (MDADI). Results: Eighteen studies met the criteria for inclusion in the systematic review. Of these, 9 addressed swallowing outcomes described in text, without using validated assessment tools. No studies reported taste, speech and pain outcomes after TBM. Post-operative bleeding was not consistently reported. 20 patients underwent robotic TBM at our institution between 2017 and 2023. The primary tumour was identified in 50% (10/20) of cases. The median time to commencing soft diet and median time of NG feeding was 0 days. The median return to normalcy of diet score was 95. Median post-treatment UW-QOL pain and swallowing scores were 100 and 70 respectively. The median speech score was 100, saliva 70, and taste 70. The median normalised MDADI scores were: global 80; emotional 67; functional 80 and physical 65. Conclusions: Validated assessment tools better inform patients about treatment options and can help compare post-TBM results across institutions. Our data demonstrates that TBM patients have a functional post-operative swallow, are pain and gastrostomy free, even after adjuvant treatment. Discoidin Domain Receptor 1 (DDR1) is a receptor tyrosine kinase that binds to and is activated by collagen(s), including collagen type I. Ddr1 deletion in osteoblasts and chondrocytes has previously demonstrated the importance of this receptor in bone development. In this study, we examined the effect of DDR1 ablation on bone architecture and mechanics as a function of aging. Femurs were collected from female global Ddr1 knockout (KO) and wild-type (WT) mice at 2, 6, and 12 mo of age and analyzed by high-resolution micro-computed tomography (μCT), mechanical testing, and histology. Primary monocytes were collected for in vitro osteoclastogenesis assays. Our studies on younger (2 mo) mice revealed no significant differences between the two genotypes and the microarchitectural and mechanical features had a similar trend as those reported earlier for osteoblast or chondrocyte specific Ddr1 knockdown. At an advanced age (12 mo), significant differences were noted across the two genotypes. μCT analysis showed a decrease in medullary cavity area as well as increased trabeculation in cortical and trabecular bone in the Ddr1 KO vs. WT mice. In addition, Ddr1 KO mouse bones exhibited reduced mechanical properties (lower peak load, yield load, and energy to yield) at 12 mo. Histological analysis revealed reduced osteoclast count in Ddr1 KO femurs at 12 mo with no significant difference in osteocyte count between the genotypes. In vitro, osteoclastogenesis was impaired in Ddr1 KO bone marrow derived cells. These results suggest that DDR1 deficiency adversely impacts osteoclast differentiation and bone remodeling in an age-dependent manner. [ABSTRACT FROM AUTHOR]

Autism spectrum disorder (ASD) affects 1 in 100 children globally. Early detection and intervention can enhance life quality for individuals diagnosed with ASD. This research utilizes the support vector machine-recursive feature elimination (SVM-RFE) method in its approach for ASD classification using the phenotypic and Automated Anatomical Labeling (AAL) Brain Atlas datasets of the Autism Brain Imaging Data Exchange preprocessed dataset. The functional connectivity matrix (FCM) is computed for the AAL data, generating 6670 features representing pair-wise brain region activity. The SVM-RFE feature selection method was applied five times to the FCM data, thus determining the optimal number of features to be 750 for the best performing support vector machine (SVM) model, corresponding to a dimensionality reduction of 88.76%. Pertinent phenotypic data features were manually selected and processed. Subsequently, five experiments were conducted, each representing a different combination of the features used for training and testing the linear SVM, deep neural networks, one-dimensional convolutional neural networks, and random forest machine learning models. These models are fine-tuned using grid search cross-validation (CV). The models are evaluated on various metrics using 5-fold CV. The most relevant brain regions from the optimal feature set are identified by ranking the SVM-RFE feature weights. The SVM-RFE approach achieved a state-of-the-art accuracy of 90.33% on the linear SVM model using the Data Processing Assistant for Resting-State Functional Magnetic Resonance Imaging pipeline. The SVM model's ability to rank the features used based on their importance provides clarity into the factors contributing to the diagnosis. The thalamus right, rectus right, and temporal middle left AAL brain regions, among others, were identified as having the highest number of connections to other brain regions. These results highlight the importance of using traditional ML models for improved interpretability and provide insight into which features to target for ASD patient rehabilitation. Purpose: The present study aims to overcome the poor oral bioavailability of ezetimibe (EZ), a selective Biopharmaceutics Classification System (BCS) Class II cholesterol absorption inhibitor drug. EZ-loaded nanostructured lipid carriers (EZ-NLCs) were dried by lyophilization and incorporated in a convenient oral solid dosage form to enhance its dissolution, and absorption and increase patient compliance. Response surface methodology (RSM) was employed to systematically optimize formulation variables, improving the efficiency of disintegration and drug release characteristics. Methods: RSM was adopted to study the effects of (A) increasing the amount of the super-disintegrant, crosscarmelose sodium, (CCS), and (B) varying the ratio between the used drying excipients Avicel and mannitol (A: M) on the disintegration time (R1), and the percentage drug released after 24 h (R2). Thirteen EZ-NLCs tablets were prepared and subjected to pre-compression and post-compression evaluation. Furthermore, a bioequivalence study was conducted by administering EZ-NLCs and ezetrol® tablets to Sprague Dawley male rats. Results: The optimized EZ-NLCs tablet (prepared with the ratio of Avicel: mannitol (7.5:0) using 30 mg CCS), revealed a disintegration time of 3.85 ± 0.03 min, and 98 ± 3.09% of the drug were released at the end of the 24 h. EZ-NLCs tablet displayed a maximum concentration (Cmax) of 3.57 ± 0.27 ng/mL and an area under the curve (AUC0− 24) of 22.44 ± 2.68 ng.hr/mL, while those of ezetrol® were 2.79 ± 0.15 ng/mL and 15.36 ± 0.86 ng.hr/mL, respectively. Conclusion: The assessed relative bioavailability demonstrated the superiority of EZ-NLCs tablet over ezetrol® with 1.5 fold improvement which proves that EZ-NLCs tablet could be a good candidate to enhance the oral bioavailability of EZ. Many research studies on photodegradation, energy storage applications, and biosensing have focused on single and mixed Cu and Mn divanadates (MV2O6, M2+ = Cu2+ and/or Mn2+). In this work, MV2O6 was synthesized, and its electrical conductivity and dielectric properties were studied. The electrical and dielectric properties were investigated in a frequency range of 0.1-300 kHz and a temperature range of 298 K to 573 K. The results indicate that the DC conductivity, AC conductivity, dielectric constant, dielectric loss factor, and loss tangent of the investigated materials increase with temperature, which support the contribution of grains and grain boundaries to the electrical properties. The materials exhibit non-Debye type relaxation, as indicated by the progression of the relaxation frequency. As frequency increases, the activation energy of AC conduction decreases, indicating that hopping conduction is the dominant mechanism. In all samples, the DC conduction activation energy is higher than the AC conduction activation energy. The circuit model was used to estimate the relaxation times. The results revealed remarkable dielectric characteristics, notably higher dielectric loss tangent values at frequencies below 1 kHz. In this study, the structural, optical, and magnetic properties of the Zn0.5Ni0.5Fe2O4 soft ferrite nanoparticle, the Ba0.4Sr0.4Ca0.2Fe12O19 hard ferrite nanoparticle, and their nanocomposite (Zn0.5Ni0.5Fe2O4)0.5(Ba0.4Sr0.4Ca0.2Fe12O19)0.5 have been investigated. The nanoparticles and the nanocomposite were synthesized via co-precipitation and ball-milling routes, respectively. The formation, purity, and crystallinity of the samples were verified from the Rietveld analysis of X-ray diffraction (XRD) data as well as the essential absorption bands in the Fourier transform infrared (FTIR). The morphology and lattice spacing were investigated using transmission electron microscopy (TEM), high-resolution TEM (HRTEM), and selected area electron diffraction (SAED). TEM micrographs visualized the hexagonal morphology of Ba0.4Sr0.4Ca0.2Fe12O19 phase and the spherical-like shape of Zn0.5Ni0.5Fe2O4 phase. A scanning electron microscope (SEM) revealed well-distributed grains in the composite. Furthermore, energy-dispersive X-ray analysis (EDX) analysis validated the chemical compositions and the coexistence of Zn0.5Ni0.5Fe2O4 and Ba0.4Sr0.4Ca0.2Fe12O19 phases in one matrix. Additionally, the optical behavior of the samples was tested via ultraviolet–visible and photoluminescence spectroscopies. Tauc plots were used to estimate the direct energy gap using UV spectroscopy. The direct and indirect energy band gaps of (Zn0.5Ni0.5Fe2O4)0.5(Ba0.4Sr0.4Ca0.2Fe12O19)0.5 nanocomposite are in between that of the pure phases, due to the increase in energy states with particle size. Also, near-band edge emissions and deep-level emissions have been observed in PL studies. X-ray photoelectric spectroscopy (XPS) studies were performed to investigate the elemental composition and cationic distribution. A good exchange coupling between both nanoparticles was verified via a single-phase behavior in the magnetic hysteresis loop. (Zn0.5Ni0.5Fe2O4)0.5(Ba0.4Sr0.4Ca0.2Fe12O19)0.5 nanocomposite had a higher saturation magnetization ( M s ) compared to Ba0.4Sr0.4Ca0.2Fe12O19 phase and a squareness ratio (SQR) of 0.207 value ( < 0.5) , making it suitable for use in magnetic hyperthermia, the production of permanent magnets, and extremely sensitive for magnetic recording media. Purpose: Equal level trauma centers in the same country might have significant differences regarding their demographics and types of trauma. Understanding geographic variations in injury patterns are essential for optimal care. Here we describe the differences in injury patterns and associated outcomes of thoracic trauma patients between rural and urban level-II trauma centers in a single country. Method: A retrospective analysis of patients with thoracic trauma over a three-year period in a rural level-II and an urban level-II in Israel were compared. Demographics, mechanism of injury, prehospital care, transport, hospitalization course, discharge disposition, and outcomes were all analyzed. Results: There were significant differences between the demography, mechanisms of injury, and sustained injuries. The urban population was older, with more pedestrians hit by motor vehicles, falls from standing or sitting, and penetrating injuries. The rural population suffered from more motor vehicle crashes and falls from heights, with injuries related to higher velocity mechanisms such as lung contusions, sternal fractures, and liver injuries. There was no significant difference in transportation time or injury severity scores. More advanced life support ambulances were utilized in the rural setting together with more transfers to a higher level of care, while more patients in the urban center were lost to follow-up. Conclusion: Understanding the differences between geographic locations is crucial towards optimizing trauma care. The complexities of thoracic trauma patients are an important example of the variations between rural and urban trauma centers. For over a decade, developing nations have used foreign direct investment and trade to access advanced economies' technology and knowledge. However, the role of emigrants in promoting the productive capacity of their home countries through the diffusion of knowledge and novel technologies has been relatively overlooked. The present study investigated the relationship between emigration stock and productive knowledge embedded in their origin economy, as measured by the Economic Complexity Index (ECI). The study applied a GMM Panel Vector Autoregressive (PVAR) technique to 86 non-high-income countries between 1995 and 2020. In principle, the PVAR model findings indicated that international emigrants promoted the level of technology and knowledge in their home countries. Further causality analyses revealed that within all countries, there was unidirectional causality from emigration to the ECI. However, a bidirectional relationship existed between the two variables in countries with low education levels. Nanomaterials such as selenium nanoparticles (SeNPs) are commonly used in everyday products and offer numerous health benefits to aquatic animals. Vitamin C (Vit C) is an essential micronutrient for fish because of its strong antioxidant properties. However, the combined effects of SeNPs and Vit C on enhancing disease resistance in Nile tilapia (Oreochromis niloticus) have not been thoroughly explored. This study investigates the impacts of SeNPs and/or Vit C on the performance, blood parameters, histology, gene expression, and resistance to Saprolegnia ferax in Nile tilapia (O. niloticus). The fish fingerlings (n = 240, averaging 32 ± 3.0 g) were divided equally into four experimental groups, with 60 fish in each group. The fish were fed a basal diet as a control group (CON group) or supplemented with 100 mg of Vit C (VC100 group), 1 mg of SeNPs/kg (SeNP group), or a combination of SeNPs and vitamin C (SeNPs + VC100 group) for 70 days. At the end of the study, a challenge trial with S. ferax was conducted. The growth and feed efficiency results indicated a significant improvement in the combined group (SeNPs + VC100 group) compared with the other groups (P < 0.05). Significant reduction in fat and moisture contents and the crude protein and ash significantly improved in the combined group (P < 0.05). Liver enzymes were reduced, with the lowest values observed in the SeNPs + VC group, and total protein and globulin contents were improved in all supplemented groups (P < 0.05). All supplements resulted in significant improvements in digestive enzymes (lipase, protease, and amylase) and immune response (lysozyme and phagocytic activities) compared to the control group (P < 0.05). The serum levels of catalase and superoxide dismutase significantly increased in fish of the SeNPs and SeNPs + VC100 groups, while lipid peroxidation (MDA) significantly reduced by the treatments with the best reduction in the combined group. The liver of fish in the supplemented groups showed improved structural integrity of hepatocytes, hepatic sinusoids, and pancreatic ducts, with reduced inflammatory cell presence and fibrocytes compared to the control fish. The intestinal structure of fish fed supplemented diets exhibited normal histomorphometry with an increase in villus length, height, and layer thickness. Diets fortified with SeNPs, VC100, or their mixture significantly boosted the growth (GH and IGF-1) and immune-antioxidant-related genes (IL-1β, IL-8, CAT, and SOD) compared with the untreated groups. Fish treated with a combination of nutrients exhibited higher survival rates and increased resistance to S. ferax. This study revealed that a dietary combination of SeNPs and Vit C synergistically enhanced tilapia fish’s antioxidant capacity, immune response, organ health, growth, and disease resistance. Purpose: To present a simple transconjunctival technique for the excision of intraconal orbital hydatid cysts. Methods: This retrospective, non-comparative, clinical intervention case study was conducted between April 2018 and October 2023. The study included five patients presented to the Orbital unit of Assiut University Hospital with an intraconal orbital cyst, which histologically proved to be a hydatid cyst. In all cases, a conjunctival incision near the fornix was made depending on the cyst location as revealed by computed tomography (CT) or magnetic resonance imaging (MRI). A traction suture was applied to the two relevant recti muscles to guide the globe toward the desired direction. Blunt orbital dissection was made toward the cyst until exposing its anterior surface. A 20-gauge needle was introduced into the cyst and followed by aspiration of its content. The collapsed cyst was then removed by non-toothed forceps and followed by copious irrigation of the field. The follow-up period ranged from 11 to 58 months. Results: The age of patients ranged from 11 to 44 years. Three were males and two were females. The cyst was iso-dense to the vitreous on CT and iso-intense to the vitreous on MRI. In all cases after aspiration of the content, the collapsed cyst was easily removed. None of the five patients developed recurrence during the follow-up period. Conclusion: The removal of the collapsed orbital hydatid cyst in the intraconal space after the aspiration of its content via transconjunctival anterior orbitotomy is a simple, fast technique with early recovery and maximum cosmesis.
