As the diagnosis determines therapy, it is important that the tumor is identified correctly. Dear Editor, Fibroepithelioma of Pinkus (FeP) is an uncommon subtype of basal cell carcinoma (BCC) with a particular growth pattern, first described by Hermann Pinkus in 1953.1 This tumor is slow growing and commonly found on the trunk or extremities of persons in the fifth and sixth decades of life.2 Although its etiopathogenesis is not fully understood, chronic ultraviolet damage seems to be a lesser factor than it is in BCC.3,4 Mutations in the PTCH gene of the Hedgehog signaling pathway could be involved in its development.5 FeP arising after radiation therapy has been reported, although a direct link is yet to be defined.6 FeP is a rare neoplasm that is often difficult to diagnose, as it simulates other skin tumors. The authors aim to present the butterfly effect, a concept based on the theory that small changes might have a powerful effect, as an example of the important connection between diet and acne. Western diet is currently a well‐known environmental factor which, mainly via the overstimulation of mTORC1 (mammalian target of rapamycin complex 1), is responsible for the development and aggravation of acne and other age‐related diseases of civilization. From the authors' point of view, "the butterfly effect" extrapolated to acne and diet depicts the importance of dietary interventions in acne so as to prevent more serious mTORC1‐driven diseases of civilization like obesity, diabetes, and cancer. Laser hair removal became a very popular intervention, with a permanently increasing number of procedures being performed worldwide. The procedure is often performed by nonmedical personnel, and even by untrained personnel, that could be associated with different kind of adverse events, from mild to serious. The complications associated could be lower if the operators understand the basic of laser physics and the working principles of devices. Dear Editor, We have read with interest the article written by Yalici-Armagan and Elcin.1 We want to thank the authors for their exciting observation leading to future debates about the role of I Demodex folliculorum i (DF) in rosacea and the control of its proliferation.1 A classic and pertinent dilemma is related to the fact that vascular changes and inflammation constitute a perfect medium for DF to multiply or already multiplied DF represents a starter for inflammatory and/or vascular changes in rosacea or rosaceiform conditions. The Food and Drug Administration (FDA) has defined permanent hair reduction as " I a significant reduction in the number of terminal hairs after a given treatment, which is stable for a period of time longer than the complete growth cycle of hair follicles at the given body site i ,"4 and this statement must be discussed at the patient's first visit, before the beginning of the procedure. Therefore, the FDA amended the permanent hair reduction definition, stating that " I the number of hairs regrowing must be stable over a time greater than the duration of the complete growth cycle of hair follicles, which varies from 4 to 12 months according to body location i ." LHR is generally a safe and effective method for long time reduction of unwanted hair, with minor and temporary side effects, but serious complications may rarely arise, especially when laser devices are used unproperly by untrained personnel.6 " I Primum non nocere i " meaning "First, do no harm" must be applied in every aspect of our practice, including in LHR, and the most practical way is to prevent complications. Dear Editor, We read with great interest the article of Gaspari, Orioni, Viviani et al speculating on the potential sexual route of transmission of the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) virus.1 Our team recently published our conclusions on the potential for sexual contact to act as vector through which infection is possible in which we underline the importance of several other possible mechanisms of viral transmission.2 It is true that angiotensin-converting enzyme 2 (ACE2) receptor presence in Leydig cells has been documented, whereas there is currently a lack of evidence for viral DNA within seminal fluid either due to an insufficient concentration of ACE2 receptors at this level or a reduced viremia that does not permit the virus to cross the blood-testis barrier.3 This makes viral transmission through contact with seminal fluid unlikely, but leaves the door open for other types of sexual contact to function as a route of viral transmission. Oro-anal contact is one such mechanism as the SARS-CoV-2 virus continues to be shed in the gastrointestinal tract even after its presence disappears in the nasopharyngeal tract.2,4 As such, hygiene measures continue to be the most important factor in preventing viral transmission. 4 Patrì A, Gallo L, Guarino M, Fabbrocini G.
