The diagnosis of venous thromboembolism (VTE) is complex, and many cases of pulmonary embolism (PE) and deep vein thrombosis (DVT) go undetected despite validated diagnostic algorithms. This study evaluated the diagnostic performance of compression ultrasound (CUS) when systematically performed in patients admitted to an internal medicine department for dyspnea and/or respiratory failure. We conducted a prospective observational cohort study of consecutive adult hospitalized patients admitted for dyspnea and/or respiratory failure with at least one of the following: tachycardia (> 100 bpm), tachypnea (> 20/min), chest pain, cough, syncope, or hemoptysis. Patients with a previous diagnosis of VTE or who underwent computed tomography pulmonary angiography (CTPA) or CUS during evaluation in the emergency department were excluded. The study included 263 patients (50.2% women, average age 84 years). CUS was positive in 31 patients (11.8%); Bilateral DVT was diagnosed in two patients and unilateral DVT in 29 patients. Of these, 10 underwent CT scan, with PE confirmed in 9 cases. Using the Wells score for DVT (cut-off ≥ 2), only 8 patients (25.8%) were at high risk. The accuracy of the Wells score in identifying PE was suboptimal, as 5 of 9 patients (55.5%) with confirmed PE were in the low-risk group (three-level interpretation) and 8 (89.9%) were in the "EP unlikely" group (two-level interpretation). The systematic use of CUS as a point-of-care tool can improve the diagnostic accuracy for VTE in patients admitted to internal medicine departments with dyspnea/respiratory failure. Background: Focused extracorporeal shock wave therapy (ESWT) has been successfully used to treat musculoskeletal conditions, but ESWT stimulates nociceptors, causing pain deep in the tissue during treatment. The occurrence of pain during ESWT is a side effect, but it can help identify painful sites and assess minimum (MiTI) and maximum (MaTI) pain thresholds to ESWT pressure stimuli. This topic has received limited attention in literature. Methods: This observational study describes a specific approach to using ESWT to study pain in 71 patients. The approach proposes moving the ESWT transducer head of the device over the entire joint surface, progressively increasing the energy level until the patient experiences pain. Results: In the study, MiTI and MaTI were 0.218 ± 0.090 and 0.416 ± 0.165 mJ/mm2 in the affected joint and 0.282 ± 0.128 and 0.501 ± 0.174 mJ/mm2 in the contralateral homologous healthy joint, being significantly lower in the affected joint (MiTI: p < 0.001 and MaTI: p = 0.003, respectively). ESWT induced pain in 94.37% of the sites with the highest subjective pain and in a greater number of sites (204) than digital pressure (123) (p < 0.001). All sites with digital pressure pain also had ESWT pain. Summary: Background: Equine anoplocephalid cestodes are tapeworms that occur worldwide in horses and are known to cause pathological manifestations and clinical disease within the digestive system of horses. Objective: While these clinical manifestations and the efficacy of currently available anthelmintics against equine tapeworms have been well documented, a recent study has demonstrated treatment failure of praziquantel and pyrantel pamoate, suggesting resistance; however, this has not been heavily investigated. Study design: This case study examined 24 Thoroughbred yearlings on one Kentucky Thoroughbred horse farm to investigate the elimination of tapeworm eggs after administration of routine anticestodal anthelmintics after confirmation of positive faecal egg counts. Methods: The Mini‐FLOTAC faecal egg counting method was used to count anoplocephalid eggs in yearlings treated with pyrantel pamoate paste (13.2 mg base/kg), moxidectin‐praziquantel (0.4 and 2.5 mg/kg) and ivermectin‐praziquantel (0.2 and 1 mg/kg) over a 6‐month period in 2022. Results: Data demonstrated that tapeworm egg shedding persisted post‐deworming with all three anthelmintic products. Out of the 24 Thoroughbred enrolled, 11/24 (45.8%) had persistent infections in the month of March and 3/24 (12.5%) had persistent infections in April after deworming with ivermectin‐praziquantel. With moxidectin‐praziquantel, 2/24 (8.3%) remained egg count positive. For pyrantel pamoate, 3/24 (12.5%) were detected with tapeworm eggs post‐treatment. During July and August, selected yearlings had faecal sampling done before and 2 weeks post‐deworming with moxidectin‐praziquantel. For July 1/6 (16.7%) horses had tapeworm eggs both before and after deworming, and one additional horse was positive in the post‐treatment sample only. For the August treatment, 4/14 (28.6%) were egg count positive both before and after deworming, with one additional horse being positive in the post‐treatment sample only. Conclusions: This study demonstrated evidence of anthelmintic treatment failure against equine tapeworms by means of regular faecal egg count monitoring. Background: The antigen processing machinery (APM) plays a critical role in generating tumor-specific antigens that can be recognized and targeted by the immune system. Proper functioning of APM components is essential for presenting these antigens on the surface of tumor cells, enabling immune detection and destruction. In many cancers, defects in APM can lead to immune evasion, contributing to tumor progression and poor clinical outcomes. However, the status of the APM in sarcomas is not well characterized, limiting the development of effective immunotherapeutic strategies for these patients. Methods: We investigated 126 patients with 8 types of bone and soft tissue sarcoma operated between 2001–2021. Tissue microarrays mapped 11 specific areas in each case. The presence/absence of APM protein was determined through immunohistochemistry. Bayesian networks were used. Results: All investigated sarcomas had some defects in APM. The least damaged component was HLA Class I subunit β2-microglobulin and HLA Class II. The proteasome LMP10 subunit was defective in leiomyosarcoma (LMS), myxoid liposarcoma (MLPS), and dedifferentiated liposarcoma (DDLPS), while MHC I transporting unit TAP2 was altered in undifferentiated pleomorphic sarcoma (UPS), gastrointestinal stromal tumor (GIST), and chordoma (CH). Among different neoplastic areas, high-grade areas showed different patterns of expression compared to high lymphocytic infiltrate areas. Heterogeneity at the patient level was also observed. Loss of any APM component was prognostic of distant metastasis (DM) for LMS and DDLPS and of overall survival (OS) for LMS. Conclusion: Sarcomas exhibit a high degree of defects in APM components, with differences among histotypes and tumoral areas. The most commonly altered APM components were HLA Class I subunit β2-microglobulin, HLA Class I subunit α (HC10), and MHC I transporting unit TAP2. The loss of APM components was prognostic of DM and OS and clinically relevant for LMS and DDLPS. This study explores sarcoma molecular mechanisms, enriching personalized therapeutic approaches. Active wearable devices such as protective smart belts have been proposed to reduce hip impact in the event of a fall. This study primarily evaluated the feasibility and acceptance of a specific protective belt among selected patients identified as being at risk of falling who were admitted to an ICS Maugeri Neurorehabilitation Unit from September 2022 to April 2023. According to previous institutional observations, the device was worn between the 6th and 21st days of recovery. Out of 435 admitted patients, 118 were considered eligible, but 101 declined to participate (about 50% refused to wear the belt without first trying it on; the other 50% found it too heavy or difficult to manage). Among the 17 patients who accepted (users), 9 used the belt correctly. The remaining eight patients refused to wear it after 24 h, due to discomfort. Out of 435 patients admitted, we observed at least one fall in 49 patients, of whom 5 were eligible patients; 1 was a user who had quickly refused to use the belt and fell with mild damage. Two non-eligible patients and one eligible non-user patient experienced falls resulting in hip fractures; only in the latter case could the use of the belt have limited the damage to the hip. Diagnosis of myeloid neoplasm is currently performed according to the presence of a predetermined set of clinical, morphological, and molecular diagnostic criteria agreed upon by a consensus of experts. Even strictly adhering to these criteria, it is possible to encounter patients who present features that are not easily ascribable to a single disease category. This is the case, e.g., of patients with de novo myeloid neoplasms with features intermediate between primary myelofibrosis (PMF) and chronic myelomonocytic leukemia (CMML). In this study, we retrospectively searched the pathological database of IRCCS Humanitas Research Hospital to identify cases of chronic myeloid neoplasm with monocytosis with a driver mutation of classic myeloproliferative neoplasms (MPN) and showing morphological MPN features. For each case, we assessed all epidemiological, clinical, histopathological, and molecular data. Then, we carried out a literature review, searching for cases with features similar to those of our patients. We retrieved a total of 13 cases presenting such criteria (9 from the literature review and 4 from our institution); in all of them, there was a coexistence of clinical, histopathological, and molecular myelodysplastic and myeloproliferative features. To date, according to current classifications (World Health Organization and International Consensus Classification), given the presence/absence of essential features for PMF or CMML, these patients should be formally diagnosed as myelodysplastic/myeloproliferative neoplasm unclassified/not otherwise specified (U/NOS). This review aims to summarize the features of these difficult cases and discuss their differential diagnosis and their classification according to the novel classifications and the existing literature on overlapping myeloid neoplasms. Vacuoles, E1-enzyme, X-linked, Autoinflammatory, Somatic (VEXAS) syndrome is caused by mutations in the UBA1 gene in myeloid precursors, leading to systemic inflammatory manifestations. We present the case of a 75-year-old man presenting with fever, panniculitis, and macrocytic anemia testing repeatedly negative for UBA1 mutations in peripheral blood samples, but ultimately found positive on bone marrow mononuclear cell DNA. Immersive virtual reality (VR) simulations are increasingly being used in diverse educational and training contexts to supplement traditional learning methods. The high versatility of virtual laboratories allows students to take advantage of many benefits, like experiencing dangerous reactions, time-consuming protocols, or expensive equipment without the necessity of a real science laboratory. However, little research is currently available to support the efficacy and efficiency of this new learning tool. In this context, the main objective of this study was to assess the influence of biotechnology training by using immersive VR technologies on the student’s motivational and learning outcomes as compared with learning with conventional methods only. To this aim we tested two diverse strategies, respectively VR simulations were used in place of or in addition to the teacher’s introductory lesson of a hands-on laboratory experience. Aligned questionaries were administered before and after the proposed activities to assess theoretical knowledge, self-efficacy, interest in biotechnology, and engagement of the participants. We found that when the introductory lesson to a biotechnology hands-on laboratory is replaced with an equivalent immersive VR simulation, the student’s learning outcomes are lower with respect to the traditional approach. On the contrary, when VR simulations are integrated as an additional tool to the existing learning methods, higher learning outcomes were observed demonstrating a deeper understanding of the learning contents. Furthermore, our study showed that learning with immersive VR simulations motivates students more than the traditional methods, thus, using this new technology in addition to the existing educational methods in biotechnology could be considered as a win-win strategy to raise the attention of the students while increasing the learning outcomes. In post-stroke patients, the disabling motor deficit mainly affects the upper limb. The focus of rehabilitation is improving upper limb function and reducing long-term disability. This study aims to evaluate the feasibility of using the Gloreha Aria (R-Lead), a sensor-based upper limb in-hospital rehabilitation, compared with conventional physiotherapist-led training in subacute hemiplegic patients. Twenty-one patients were recruited and randomised 1:1 to a sensor-based group (treatment group TG) or a conventional group (control group, CG). All patients performed 30 sessions of 30 min each of dedicated upper limb rehabilitation. The Fugl–Meyer Assessment for Upper Extremity (FMA-UE) was the primary evaluation., both as a motor score and as individual items. Secondary evaluations were Functional Independence Measure; global disability assessed with the Modified Barthel Index; Motor Evaluation Scale for UE in stroke; power grip; and arm, shoulder, and hand disability. All the enrolled patients, 10 in the TG and 11 in the CG, completed all hand rehabilitation sessions during their hospital stay without experiencing any adverse events. FMA-UE scores in upper limb motor function improved in both groups [delta change CG (11.8 ± 9.2) vs. TG (12.7 ± 8.6)]. The score at T1 for FMA joint pain (21.8 vs. 24 best score) suggests the use of the Gloreha Aria (R-Lead) as feasible in improving arm function abilities in post-stroke patients. The diffusion of brake-shoe system has made it necessary to study brake–wheel–rail interactions. This study aims to elucidate this topic with an innovative small-scale test rig, the '4-contact machine'. A test procedure is designed in which an ER7 steel wheel is coupled with a R260 steel rail and two different brake-shoe materials (cast iron and sintered material). The results show that the 4-contact machine reproduces the effects of wheel–rail–brake shoe contact and damage similar to those observed on a railway wheel at the end of its life, highlighting marked differences in behaviour between the two brake materials. Histiocytoses patients present with highly heterogeneous clinical and histopathological pictures requiring multidisciplinary management. Mixed histiocytosis is a recently described group of syndromes defined by the histological overlap of Langerhans cell histiocytosis and other histiocytic proliferations that include three clinically and prognostically different conditions (i.e., Type-1–3 mixed histiocytosis). We describe a 10 year-old boy with unifocal (type-3) mixed histiocytosis – Langerhans cell histiocytosis combined with a lesion with features intermediate between Rosai-Dorfman disease and reticulohistiocytosis. Sixty months after excision, the child is disease free. Cutaneous type-3 mixed histiocytosis (Langerhans cell histiocytosis with Rosai-Dorfman disease/reticulohistiocytosis) may occur in older childhood, be unifocal, and be cured by surgical excision. Migmatites and granitic-gneisses exhumed in Archean to Phanerozoic segments are former partially molten crustal roots, display typical domes structures ranging in size from kilometres to decakilometres, and are often interpreted as resulting from the development of diapiric or convective gravitational instabilities. In previous work (part I), we determined various regimes of gravity-driven segregation, by considering a thick continental crust heated from below and containing melt related heterogeneities. These heterogeneities, represented by inclusions of distinct densities and viscosities with respect to the ambient partially molten material, can be entrained into convection cells (in the 'suspension' and 'layering' regimes) and/or accumulate as clusters (in the 'layering' and 'diapirism' regimes). Here we further investigate the specific conditions that allow for the formation and preservation of domes resulting from diapirism at the top of convective cells. We show that both the cessation of basal heating and the freezing of the buoyant inclusions density favour their stacking and preservation at ca. 15 km depth, within about 10 Myr. The buoyant inclusions form domes, 5–20 km in size, that also record several convective cycles at velocities ranging from 0.5–4 cm yr−1. 3-D models demonstrate their radial geometrical nature. The influence of the size and concentration of the inclusions is also assessed, complementing the characteristics of crustal heterogeneity in driving its differentiation and the formation of migmatite domes. This article discusses a case study of a 47-year-old woman with cutaneous-onset Erdheim-Chester disease (ECD), a rare multisystem non-Langerhans cell histiocytosis. The patient initially presented with periocular nodular xanthomas, which were diagnosed as xanthoma disseminatum (XD) after an unsuccessful treatment with hydroxychloroquine and cyclophosphamide. However, over time, the patient developed systemic manifestations and was eventually diagnosed with ECD. The patient was successfully treated with a combination of anakinra and cobimetinib, which resulted in a reduction in skin lesions and improvement in other symptoms. The case highlights the challenges in diagnosing and treating ECD and suggests that prolonged follow-up is necessary for patients with atypical presentations. [Extracted from the article]

Myeloid neoplasms may metastasize to the skin, presenting a wide range of clinical‐pathological features that often lead to a reduction in patients' survival. The presentation varies depending on the category of myeloid neoplasm and its prognostic significance. The literature has specifically focused on the features of acute myeloid leukemia (AML), myelodysplastic syndrome (MDS), and chronic myelomonocytic leukemia (CMML). In this article, we aimed to uncover the peculiarities of clonal skin proliferations in the course of Ph‐negative myeloproliferative neoplasms (MPNs). We conducted a systematic review and statistical analysis of the literature data. MPN patients mainly exhibited cutaneous extramedullary hematopoiesis, while a minority displayed cutaneous histiocytic lesions. Furthermore, these patients showed lower survival rates compared to the median survival of MPN patients, especially when calculating survival from the appearance of cutaneous lesions. Our work highlights, for the first time, the prognostic relevance and histological heterogeneity of cutaneous lesions in MPN. Chronic myelomonocytic leukemia (CMML) is a myelodysplastic syndrome/myeloproliferative neoplasm (MDS/MPN) chacaterized by persistent peripheral blood monocytosis, hypercellular bone marrow and dysplasia at least in one myeloid lineage. CMML shares much of its molecular landscape with other myeloid neoplasms, while differs from others such as chronic neutrophilic leukemia (CNL), given the high frequency of CSF3R mutations in the latter. In this article, we report a case of CSF3R-mutated CMML and dissect this rare entity by reviewing the medical literature, with the intent to understand how this rare mutation shapes CMML's clinical and morphological phenotype. CSF3R-mutated CMML emerges as a rare entity meeting the ICC/WHO diagnostic criteria for CMML and simultaneously showing clinical-pathological and molecular traits of CNL and atypical chronic myeloid leukemia, rising an important and difficult diagnostic and therapeutical issue. Indeterminate cell histiocytosis (ICH) is a very rare histiocytic disorder, primarily involving the skin. It affects more frequently adults, often presenting with a generalized papular eruption, and needs to be differentiated from other neoplastic, paraneoplastic, and infectious diseases through clinical and histological examination. The knowledge on ICH is limited to case reports and small series. Thus, the lack of larger multicentric studies has prevented recognizing and addressing the specific clinical need of the entity. In this systematic review, we comprehensively analysed the medical literature describing histologically‐confirmed cases of ICH and divided the patients into epidemiologically and clinically different groups. We demonstrate that ICH in adulthood is strongly associated with the development of haematological (and especially myeloid) neoplasms. In this subset of patients, we identify blastic morphology of neoplastic cells as a novel independent prognostic factor and an early histopathological predictor of an associated myeloid neoplasm. Moreover, we highlight that even though ICH may also present in childhood, these patients often show indolent behaviour. Genetically, ICH emerges as a heterogeneous condition. While patients with associated myeloid neoplasms are enriched in pERK pathway gene mutations, in others a specific ETV3::NCOA2 rearrangement is described. We finally reviewe the nosology of ICH since its first description, its possible cell of origin, and summarize the therapeutic options reported for each different clinical subgroup. We compute the flow induced by the steady translation of a rigid sphere along the axis of a large cylindrical container filled with a low-viscosity fluid set in rigid-body rotation, the sphere being constrained to spin at the same rate as the undisturbed fluid. The parameter range covered by the simulations is similar to that explored experimentally by Maxworthy (J. Fluid Mech. , vol. 40, 1970, pp. 453–479). We describe the salient features of the flow, especially the internal characteristics of the Taylor columns that form ahead of and behind the body and the inertial wave pattern, and determine the drag and torque acting on the sphere. Torque variations are found to obey two markedly different laws under rapid- and slow-rotation conditions. The corresponding scaling laws are predicted by examining the dominant balances governing the axial vorticity distribution in the body vicinity. Results for the drag agree well with the semi-empirical law proposed for inertialess regimes by Tanzosh & Stone (J. Fluid Mech. , vol. 275, 1994, pp. 225–256). This law is found to apply even in regimes where inertial effects are large, provided that rotation effects are also large enough. Influence of axial confinement is shown to increase dramatically the drag in rapidly rotating configurations, and the container length has to be approximately a thousand times larger than the sphere for this influence to become negligibly small. The reported simulations establish that this confinement effect is at the origin of the long-standing discrepancy existing between Maxworthy's results and theoretical predictions. The main purpose of this study was to characterize Freisa red wines from Piedmont (northern Italy) according to their volatile organic compounds (VOCs) and elemental composition. Moreover, the authors investigated whether it was possible to distinguish among the five different DOCs produced in Piedmont on the basis of these chemical parameters. The VOCs profile of Freisa wines was very similar, and the most abundant species detected were isoamyl alcohol, phenylethyl alcohol, ethyl octanoate, and diethyl succinate. Conversely, elemental composition allowed us to partially distinguish one Freisa DOC with respect to the others. Multivariate statistical analysis applied to elemental composition revealed differences among Freisa wines and other red wines from different regions of Italy. In particular, Freisa wines featured higher concentrations of Cr and Ni metals, which are strongly correlated with the composition of the soil of Piedmont. Background: The characterization of a typical red wine from Piedmont (north-west Italy), Pelaverga, was performed by means of headspace solid phase microextraction coupled to gas chromatography–mass spectrometry. Two different kinds of Pelaverga are known, hystorical and Verduno, which differ with the area of production. The main volatile com- pounds of the wine were identified and semi-quantified. Principal components analysis (PCA) was performed to identify possible clusters among the different samples analyzed. Results: Forty-eight volatile compounds were unambiguously identified in the wines sampled. The two types of Pelaverga analyzed displayed a very similar volatile profile. The most abundant volatile compounds were two esters, ethyl octanoate and diethyl succinate, and two alcohols, isoamyl alcohol and ethyl phenyl alcohol. Few varietal com- pounds were detected, probably due to their low concentration; among these, we were not able to detect rotundone, which is reported to impart characteristic spicy notes to this kind of wine. Application of PCA did not allow to differentiate between the two types of Pelaverga on the basis of their volatile profile. Conclusion: The present work is, to the best of our knowledge, the first complete report about the volatile profile of Pelaverga wines and represents the first step of a research project aiming at characterizing food products from Piedmont on the basis of chemical parameters. In this study, we wanted to depict the complete clinical–pathological profile of a series of patients affected by primary ITCL involving the small bowel, diagnosed and treated at a referral center for celiac disease and lymphomas. Furthermore, we tried to define clinical and histopathological hallmarks of these lymphomas to widen the knowledge on these rare and still partially characterized neoplasms. The gastrointestinal (GI) tract is the most common extranodal site of occurrence of non-Hodgkin lymphomas. Most GI lymphomas are of B-cell lineage, while T-cell lymphomas are less frequent. The aim of our retrospective study was to depict the clinical–pathological profile of a series of patients affected by intestinal T-cell lymphomas (ITCL) and possibly define hallmarks of these neoplasms. A total of 28 patients were included: 17 enteropathy-associated T-cell lymphomas (EATL), 5 monomorphic epitheliotropic T-cell lymphomas (MEITL), 3 indolent T-cell lymphoproliferative disorders of the gastrointestinal tract (ITCLDGT), and 3 intestinal T-cell lymphomas not otherwise specified (ITCL-NOS). Celiac disease (CD) was diagnosed in around 70% of cases. Diagnosis of EATL showed a significant correlation with CD30 expression, whereas MEITL with angiotropism and CD56 positivity. ITCLDGT cases showed plasma cells infiltration. Peripheral lymphocytosis, the absence of a previous diagnosis of CD, an advanced Lugano clinical stage, and the histological subtype ITCL-NOS were significantly associated with worse survival at multivariate analysis. Our findings about the epidemiological, clinical, and histopathological features of ITCL were in line with the current knowledge. Reliable prognostic tools for these neoplasms are still lacking but according to our results lymphocytosis, diagnosis of CD, Lugano clinical stage, and histological subtype should be considered for patient stratification. Background: The COVID-19 outbreak led to the diffusion of several telemedicine solutions. The choice of the correct platform is crucial for ensuring the release of effective assistance. However, there is a lack of an objective method for the assessment of technical features. Objective: This study proposes a methodology for the evaluation of functional requirements of telemedicine platforms. This approach also permits the comparison of solutions in the Italian market by means of defined parameters, thus directing the choice of health care professionals. Methods: The study is divided into three phases. First, a mapping of the telemedicine platforms operating in Italy is performed. Then, the available platforms are selected based on the offered telemedicine activity. Finally, a method for evaluating the investigated platforms is defined. Results: Thirty-three (n = 33) technological systems were identified through an accurate investigation on the web and interviews with IT companies. Fifteen parameters were defined and organized into three categories: (1) usability of the telemedicine platform, (2) security, and (3) technological and organizational aspects. A score between 1 and 4 was assigned to each parameter, proportionally to the completeness of the platform. In particular, 62.96% of platforms reached an average score between 3.01 and 4 points; 33.33% of them had scores between 2.01 and 3, while the remaining 3.70% of solutions obtained a result between 1.01 and 2. Conclusions: The study provides an evaluation approach that is easily usable by health professionals to select the most suitable platform. Introduction: Philadelphia‐negative myeloproliferative neoplasms (MPN) are clonal myeloid proliferative disorders characterized by sustained systemic inflammation. Despite its renowned importance, the knowledge concerning the inflammatory pathophysiology of these conditions is currently limited to studies on serum cytokines, while cellular immunity has rarely been investigated. Methods: In the present study, we targeted Arginase‐1 immunosuppressive myeloid cells in the bone marrow of MPN patients and healthy controls and investigated their clinical and prognostic significance. We demonstrated that MPN are characterized by a significant reduction of bone marrow immunosuppressive cells and that the number of these cells significantly correlates with several clinical and histopathological features of diagnostic and prognostic importance. Moreover, we identified an unreported correlation between a reduction of Arginase‐1+ bone marrow cells and the presence of CALR mutations, linking tumor‐promoting immunity and molecular drivers. Finally, we postulate that the reduction of bone marrow Arginase‐1+ immunosuppressive cells may be due to the migration of these cells to the spleen, where they may exert systemic immunomodulatory function. Conclusion: Altogether, this study preliminary investigated the contribution of cellular immunity in the pathogenesis of myeloproliferative neoplasms and identified a possible interesting therapeutic target as well as a set of new links that may contribute to unraveling the biological mechanisms behind these interesting hematological neoplasms. Background: Minimal residual disease (MRD) monitoring is an important tool to optimally address post-transplant management of acute myeloid leukemia (AML) patients. Methods: We retrospectively analyzed the impact of bone marrow CD34+ molecular chimerism and WT1 on the outcome of a consecutive series of 168 AML patients submitted to allogeneic stem cell transplantation. Results: The cumulative incidence of relapse (CIR)was significantly lower in patients with donor chimerism on CD34+ cells = 97.5% and WT1 < 213 copies/ABL x 10-4 both at 1st month (p=0.008 and p<0.001) and at 3rd month (p<0.001 for both). By combining chimerism and WT1 at 3rd month, 13 patients with chimerism < 97.5% or WT1 > 213 showed intermediate prognosis. 12 of these patients fell in this category because of molecular chimerism < 97.5% at a time-point in which WT1 was < 213. Conclusions: Our results confirm that lineage-specific molecular chimerism and WT1 after allo-SCT (1st and 3rd month) are useful MRD markers. When considered together at 3rd month, CD34+ molecular chimerism could represent an earlier predictor of relapse compared to WT1. Primary large B-cell lymphoma of immune-privileged sites (IP-LBCL) is a rare malignant hematological neoplasm. Involvement of the cerebellum is even rarer and its diagnosis is often difficult to make due to its non-specific clinical and radiological presentation. Methods: We reported 3 cases of cerebellar IP-LBCL followed at our hospital and reviewed the medical literature to unravel the peculiarities of this poorly studied entity. Outcomes: Analyzing our cases and reviewing the literature, we could collect and study 26 cases of cerebellar IP-LBCL. To the best of our knowledge, this is the largest cohort of such patients currently published. Conclusion: Cerebellar IP-LBCL presents more often in adult females with cerebellum-related focal neurological signs such as ataxia, headache, and nausea. Histological confirmation is mandatory for a correct diagnosis and treatment and all cases feature diffuse large B-cell lymphoma histopathology. Compared to other encephalic IP-LBCL, cerebellar cases seem to include a higher number of cases with germinal center B-cell phenotype and better survival. These differences may be related to a different immune microenvironment and especially immunoregulation that distinguishes the cerebellum from other areas of the CNS. [ABSTRACT FROM AUTHOR]

Understanding how humans master complex skills has the potential for wide-reaching societal benefit. Research has shown that one important aspect of effective skill learning is the temporal distribution of practice episodes (i.e., distributed practice). Using a large observational sample of players (n = 162,417) drawn from a competitive and popular online game (League of Legends), we analysed the relationship between practice distribution and performance through time. We compared groups of players who exhibited different play schedules using data slicing and machine learning techniques, to show that players who cluster gameplay into shorter time frames ultimately achieve lower performance levels than those who space their games across longer time windows. Additionally, we found that the timing of intensive play periods does not affect final performance—it is the overall amount of spacing that matters. These results extend some of the key findings in the literature on practice and learning to an ecologically valid environment with huge n. Gastrointestinal (GI) involvement of plasma cell neoplasms is extremely rare. Herein, we describe the case of a 74-year-old Caucasian woman who came to our attention with abdominal pain, food vomiting, and weight loss of 10 kg over 1 year. A computed tomography scan of the abdomen revealed circumferential thickening of terminal ileum, for which the patient underwent an urgent 20-cm-long ileal resection. Histopathological and immunophenotypic analysis revealed a plasma cell neoplasm of the ileum. Subsequent investigations found a serum monoclonal immunoglobulin A component, an osteolytic lesion of the left jaw, and a clonal bone marrow plasma cell infiltrate carrying 1q21 amplification. Given the final diagnosis of plasma cell myeloma (PCM), the patient underwent a VMD (bortezomib, melphalan, and dexamethasone) chemotherapy regimen, achieving a complete remission after a 12-month treatment. For disease relapse, two further chemotherapy regimens were later attempted. At the last follow-up 4 years after the diagnosis, the patient is still alive. This case draws attention to the extramedullary presentation of plasma cell neoplasms, even if rare, as a prompt diagnosis seems to result in a better prognosis. In addition, it highlights the relevance of a multidisciplinary approach, involving gastroenterologists, hematologists, and pathologists, to the diagnosis and management of these neoplasms. Background: The characterization of five different types of Italian unifloral honeys was performed by means of headspace solid‐phase microextraction coupled to gas chromatography–mass spectrometry. The main volatile compounds of the honeys were identified and semi‐quantified. Pattern recognition methods such as principal component analysis (PCA) and k‐nearest neighbors (k‐NN) were performed to classify honey samples with different botanical origins, and identify the most discriminating parameters. Results: More than 100 of compounds were unambiguously identified in the honeys sampled. Methyl salicylate was present in honeydew honeys only, and this permitted to differentiate this kind of honeys from the others. Dandelion honeys featured high levels of nitriles, while lime tree honeys displayed the presence of carvacrol and of the terpenes cis and trans‐p‐mentha‐2,8‐dien‐1‐ol. Acacia honeys did not display potential chemical markers but were characterized by low levels of benzaldehyde and high levels of cis‐linalool oxide. Chestnut and rhododendron honeys did not show a volatile profile markedly different from those of other honeys. Application of PCA and k‐NN methods did not allow a complete classification of honeys of different botanical origin on the basis of their volatile profile. Conclusion: Unifloral honeys from Piedmont (Italy) displays a rich and variegate profile of volatile compounds, some of which are candidates as markers. Nonetheless, to classify honeys, further studies are necessary likely comprising a larger set of samples and the determination of different chemical parameters in conjunction with volatile compounds. We investigate numerically some thermomechanical conditions for the development of crustal scale diapirism and convection in a heterogeneous continental crust independently from the action of regional tectonics. Here, we consider a hot crust, with unmolten and partially molten domains of specific temperature and strain-rate dependent power-law rheologies. We take advantage of the volume of fluid (VOF) method to capture the coalescence and separation of deformable inclusions in the partially molten domain. The inclusions, of several hundred metres in size, are more or less dense and viscous with respect to the ambient medium (they also behave with a power-law rheology). We restrict our study to a 20 Myr time scale, during which gravitational dynamics may dominate over lateral tectonics and lithospheric thermal re-equilibration. The motion of these inclusions during the development of gravitational instabilities displays distinct flow regimes that depend on two Rayleigh numbers denoted Ra UM and RaPM , for the unmolten and partially molten rock properties, respectively. A 'suspension' regime occurs at high Ra UM and high Ra PM, in which most of the light compositional heterogeneities remain entrained in the convective cells. In contrast at low Ra UM and high Ra PM, a 'layering' regime is characterized by merging of the light inclusions as floating clusters below the rigid upper crustal lid. This regime occurs in association with a sharp viscosity gradient at upper-to-middle crust transitional depths. In these two regimes, the dense inclusions accumulate at the bottom of the partially molten zone. Finally at moderate Ra PM, a 'diapiric' regime reflects the segregation of the heavy and the light inclusions, respectively downward and upward, without global convection. These numerical experiments lead to a first order evaluation of the physical parameters required for the segregation of deformable inclusions of variable densities and convection, in a partially molten crust, and provide insights on the conditions for the development of migmatite domes. Geological data indicate that these processes likely occur in a large number of settings from Archean to Phanerozoic times, and contribute to the differentiation of the continental crust. Quinoidal oligothiophenes have received considerable attention as interesting platforms with remarkable amphoteric redox behavior associated with their diradical character increasing with the conjugation lengths. In this work, we considered a family of quinoidal oligothiophenes bearing cyano-ester terminal groups and characterized them by UV-Vis-NIR absorption and Raman spectroscopy measurements at different excitation wavelengths. The experimental investigation is complemented by quantum-chemical studies to assess the quality of computed density functional theory (DFT) ground state structures and their influence on predicted Raman intensities. In addition, resonance conditions with the optically active HOMO→LUMO transition as well as with the more elusive state dominated by the doubly excited HOMO,HOMO→LUMO,LUMO configuration, are determined with DFT-MRCI calculations and their contributions to Raman activity enhancement are discussed in terms of computed vibrational Huang–Rhys (HR) factors. Background:In the first few months of 2020, Lombardy was the hardest-hit region in Italy for COVID-19 cases. Our Rehabilitation Institute offered a telemedicine service to COVID-19 patients discharged after hospitalization. Methods:Patients transferred from Emergency, Intensive Care, and Pulmonology departments of the principal regional hospital hubs had an average stay in our hospital of 2–3 weeks. On discharge, at home, they underwent a telecare nursing and specialist teleconsultation program for 3 months, including monitoring of vital signs and symptoms. Patients completed the SF-12 questionnaire at the start and end of the program and rated their satisfaction with it. Results:The program involved 130 patients (51%). During the period, there were 14 ± 2 (1,800 in total) telenursing support phone calls per patient made, and 12.5 ± 3.4 oxygen saturation readings per patient (1,631 in total). Persisting symptoms, frequently in combination, were present at the start of the program in 124 (94%) patients. There was a significant reduction of symptoms (p < 0.0000) after the telecare program. The physical component of SF-12 significantly improved at the end [Δ(t1–t0) = 6.7 ± 9.3, p < 0.0001]. On the contrary, the mental component of SF-12 remained unchanged or decreased slightly in patients ≤70 years of age [Δ(t1–t0) = −2.7 ± 12.3, ns], while it decreased significantly [Δ(t1–t0) = −5.4 ± 12.4, p = 0.0367] in older patients (although remaining mild). Patient satisfaction with the program was very high in all 130 patients. Conclusions:Our Telehealth and Telecare Service offers an example of rapid scaling and adaptation of an existing program to meet the needs of COVID-19 patients. Our findings indicate that telemedicine can be an integral part of clinical practice if supported by the institution with training and IT support provided to patients, nurses, and clinicians. Our report highlights the importance of necroscopy of stillbirths for the recognition of DS and diagnosis of TAM/AMkL, even in apparently normal pregnancies. Transient abnormal myelopoiesis (TAM), which develops in 10% to 20% of DS infants, is recognized as an obligate step toward AMkL in DS [[1]]. BACKGROUND: Walking independently after a stroke can be difficult or impossible, and walking reeducation is vital. But the approach used is often arbitrary, relying on the devices available and subjective evaluations by the doctor/physiotherapist. Objective decision making tools could be useful. OBJECTIVES: To develop a decision making algorithm able to select for post-stroke patients, based on their motor skills, an appropriate mode of treadmill training (TT), including type of physiotherapist support/supervision required and safety conditions necessary. METHODS: We retrospectively analyzed data from 97 post-stroke inpatients admitted to a NeuroRehabilitation unit. Patients attended TT with body weight support (BWSTT group) or without support (FreeTT group), depending on clinical judgment. Patients' sociodemographic and clinical characteristics, including the Cumulative Illness Rating Scale (CIRS) plus measures of walking ability (Functional Ambulation Classification [FAC], total Functional Independence Measure [FIM] and Tinetti Performance-Oriented Mobility Assessment [Tinetti]) and fall risk profile (Morse and Stratify) were retrieved from institutional database. RESULTS: No significant differences emerged between the two groups regarding sociodemographic and clinical characteristics. Regarding walking ability, FAC, total FIM and its Motor component and the Tinetti scale differed significantly between groups (for all, p < 0.001). FAC and Tinetti scores were used to elaborate a decision making algorithm classifying patients into 4 risk/safety (RS) classes. As expected, a strong association (Pearson chi-squared, p < 0.0001) was found between RS classes and the initial BWSTT/FreeTT classification. CONCLUSION: This decision making algorithm provides an objective tool to direct post-stroke patients, on admission to the rehabilitation facility, to the most appropriate form of TT. Background: The visualization and analysis of brain data such as white matter diffusion tractography and magnetic resonance imaging (MRI) volumes is commonly used by neuro-specialist and researchers to help the understanding of brain structure, functionality and connectivity. As mobile devices are widely used among users and their technology shows a continuous improvement in performance, different types of applications have been designed to help users in different work areas.Results: We present, ABrainVis, an Android mobile tool that allows users to visualize different types of brain images, such as white matter diffusion tractographies, represented as fibers in 3D, segmented fiber bundles, MRI 3D images as rendered volumes and slices, and meshes. The tool enables users to choose and combine different types of brain imaging data to provide visual anatomical context for specific visualization needs. ABrainVis provides high performance over a wide range of Android devices, including tablets and cell phones using medium and large tractography datasets. Interesting visualizations including brain tumors and arteries, along with fiber, are given as examples of case studies using ABrainVis.Conclusions: The functionality, flexibility and performance of ABrainVis tool introduce an improvement in user experience enabling neurophysicians and neuroscientists fast visualization of large tractography datasets, as well as the ability to incorporate other brain imaging data such as MRI volumes and meshes, adding anatomical contextual information. Systemic mastocytosis (SM) is a clinically heterogeneous disorder, characterized by the infiltration of various organs by morphologically, immunophenotypically, and genetically abnormal mast-cells (MC). SM is currently sub-classified into six different categories according to clinical features and aggressiveness (i.e. indolent SM, bone marrow SM, smoldering SM, SM with an associated hematological neoplasm (AHN), aggressive SM, and mast-cell sarcoma) [[1]]. The restriction of SM proliferation to the BM induced us to initially consider a diagnosis of bone marrow mastocytosis (i.e. mastocytosis with isolated involvement of the bone marrow). However, giving the coexistence of lymphoma and SM, we found that the patients actually fulfilled the current WHO criteria for SM-AHN, the second most common subgroup of SM [[1]]. Currently, few evidences have shown the possible involvement of autoimmunity in patients affected by coronavirus disease 2019 (COVID‐19). In this study, we elucidate whether severe acute respiratory syndrome coronavirus disease 2 (SARS‐CoV‐2) stimulates autoantibody production and contributes to autoimmunity activation. We enrolled 40 adult patients (66.8 years mean age) admitted to Alessandria Hospital between March and April 2020. All the patients had a confirmed COVID‐19 diagnosis and no previously clinical record of autoimmune disease. Forty blood donors were analyzed for the same markers and considered as healthy controls. Our patients had high levels of common inflammatory markers, such as C reactive protein, lactate dehydrogenase, ferritin, and creatinine. Interleukin‐6 concentrations were also increased, supporting the major role of this interleukin during COVID‐19 infection. Lymphocyte numbers were generally lower compared with healthy individuals. All the patients were also screened for the most common autoantibodies. We found a significant prevalence of antinuclear antibodies, antineutrophil cytoplasmic antibodies, and ASCA immunoglobulin A antibodies. We observed that patients having a de novo autoimmune response had the worst acute viral disease prognosis and outcome. Our results sustain the hypothesis that COVID‐19 infection correlates with the autoimmunity markers. Our study might help clinicians to: (a) better understand the heterogeneity of this pathology and (b) correctly evaluate COVID‐19 clinical manifestations. Our data explained why drugs used to treat autoimmune diseases may also be useful for SARS‐CoV‐2 infection. In addition, we highly recommend checking patients with COVID‐19 for autoimmunity markers, mainly when deciding on whether to treat them with plasma transfer therapy. Mixed histiocytoses are a rare and recently recognized subset of histiocytic disorders that may involve the skin, characterized by the synchronous or metachronous development of lesions with Langerhans and/or non‐Langerhans cell histiocytosis histopathological features. Around 10% of patients diagnosed with histiocytosis may develop a hematological malignancy, often with dramatic prognostic consequences. We hereby describe the exceptional case of a patient developing a MAP2K1‐driven mixed histiocytosis with Langerhans cell histiocytosis, Rosai‐Dorfman‐Destombes disease, and Erdheim‐Chester disease features and cutaneous involvement, progressing to a fatal and clonally‐related acute myeloid leukemia. Background: Histiocytoses are haematological disorders of bone marrow origin that share many biological and clinical features with haematological neoplasms. The association between histiocytoses of the cutaneous‐group and myeloid malignancies is a poorly investigated topic of high biological and clinical impact. Methods: We performed a systematic review of the scientific literature, compliant with PRISMA guidelines, to unravel the clinical and pathological features of this intriguing association. Findings: We gathered and analysed 102 patients. Most were children with generalised cutaneous eruptions and displayed risk organ involvement (i.e. bone marrow, spleen, liver). Interestingly, all these features are uncommonly encountered in C‐group histiocytosis not associated with haematological neoplasms. Conclusions: Our review shows that generalised eruptions and risk organ involvement in cutaneous‐group histiocytosis should raise a suspicion for a concomitant myeloid neoplasm both in children and in adults and warrant further investigations. A rapid recognition of this association is required to start a prompt and effective therapeutic management given the aggressive behaviour of the associated myeloid neoplasm in most instances.
