Purpose: This study aimed to estimate the corneal keratometric index in the eyes of cataract surgery patients who received zero-power intraocular lenses (IOLs). Methodology: This retrospective study analyzed postoperative equivalent spherical refraction and axial length, mean anterior curvature radius and aqueous humor refractive index to calculate the theoretical corneal keratometric index value (nk). Data was collected from 2 centers located in France and Germany. Results: Thirty-six eyes were analyzed. The results revealed a mean corneal keratometric index of 1.329 ± 0.005 for traditional axial length (AL) and 1.331 ± 0.005 for Cooke modified axial length (CMAL). Results ranged from minimum values of 1.318/1.320 to maximum values of 1.340/1.340. Conclusion: The corneal keratometric index is a crucial parameter for ophthalmic procedures and calculations, particularly for IOL power calculation. Notably, the estimated corneal keratometric index value of 1.329/1.331 in this study is lower than the commonly used 1.3375 index. These findings align with recent research demonstrating that the theoretical corneal keratometric index should be approximately 1.329 using traditional AL and 1.331 using CMAL, based on the ratio between the mean anterior and posterior corneal curvature radii (1.22). This study considered two questions associated with intraocular lens (IOL) power and refraction: (1) Given a refraction with a particular IOL in the eye, what will be the refraction for the IOL or another IOL if located differently with regard to tilt or anterior–posterior position? (2) For a target refraction, what is the power of another IOL if located differently with regard to tilt or position? A thin lens technique was developed to address these questions. For the first question, light was traced through the initial correcting spectacle lens to the cornea, refracted at the cornea, transferred to the position of the initial IOL, refracted at this IOL, transferred to the position of a new IOL (which may be the same IOL but with a different position and/or tilt), refracted backwards through the new IOL, transferred to the cornea and refracted out of the eye to give a new correcting spectacle lens power. For the second question, light was traced through the initial correcting spectacle lens to the cornea, refracted at the cornea, transferred to the position of the initial IOL, refracted at the initial IOL and transferred to the position of a new IOL. Light was also traced through the second correcting spectacle lens, refracted at the cornea and transferred to the position of the second IOL. The difference between the reduced image vergence for the first raytrace and the reduced object vergence for the second raytrace gave the effective power of the second IOL, and from this, the power of the second IOL was determined. To evaluate prediction accuracy of pre- and post-DMEK keratometry (K) and total keratometry (TK) values for IOL power calculations in Fuchs endothelial corneal dystrophy (FECD) eyes undergoing DMEK with cataract surgery (triple DMEK). Retrospective cross-sectional multicenter study of 55 FECD eyes (44 patients) that underwent triple DMEK between 2019 and 2022 between two centers in USA and Europe. Swept-source optical coherence tomography biometry (IOLMaster 700) was used for pre- and post-DMEK measurements. K and TK values were used for power calculations with ten formulae (Barrett Universal II (BUII), Castrop, Cooke K6, EVO 2.0, Haigis, Hoffer Q, Hoffer QST, Holladay I, Kane, and SRK/T). Mean error, mean absolute error (MAE), standard deviation, and percentage of eyes within ±0.50/±1.00 diopters (D) were calculated. Studied formulae were additionally adjusted using a method published previously (IOLup1D Method), which increases the IOL power by 1D. While both eyes from the same patient were considered for descriptive statistics, we restricted to one eye per individual (44 eyes for statistical comparisons. MAEs for all formulae were lower for post-DMEK K and TK than pre-DMEK K and TK by an average of 0.24 and 0.47 D, respectively. The lowest MAE was 0.49 D for Kane using post-DMEK TK, and the highest MAE was 1.05 D for BUII using pre-DMEK TK. Most IOLup1D formulae had lower MAEs than pre-DMEK K and TK formulae. The IOLup1D Method should be used instead of pre-DMEK K and TK values for triple DMEK in FECD eyes. Using post-DMEK TK values for cataract surgery after DMEK provides better refractive accuracy than any of the three studied methods used for triple DMEK procedures. [ABSTRACT FROM AUTHOR]

This study evaluates the accuracy of a newly developed intraocular lens (IOL) power calculation method that applies four different IOL power calculation formulas according to 768 biometric subgroups based on keratometry, anterior chamber depth, and axial length. This retrospective cross-sectional study was conducted in at Korea University Ansan Hospital. A total of 1600 eyes from 1600 patients who underwent phacoemulsification and a ZCB00 IOL in-the-bag implantation were divided into two datasets: a reference dataset (1200 eyes) and a validation dataset (400 eyes). Using the reference dataset and the results of previous studies, the Eom IOL power calculator was developed using 768 biometric subgroups. The median absolute errors (MedAEs) and IOL Formula Performance Indexes (FPIs) of the Barrett Universal II, Haigis, Hoffer Q, Holladay 1, Ladas Super, SRK/T, and Eom formulas using the 400-eye validation dataset were compared. The MedAE of the Eom formula (0.22 D) was significantly smaller than that of the other four formulas, except for the Barrett Universal II and Ladas Super formulas (0.24 D and 0.23 D, respectively). The IOL FPI of the Eom formula was 0.553, which ranked first, followed by the Ladas Super (0.474), Barrett Universal II (0.470), Holladay 1 (0.444), Hoffer Q (0.396), Haigis (0.392), and SRK/T (0.361) formulas. In conclusion, the Eom IOL power calculator developed in this study demonstrated similar or slightly better accuracy than the Barrett Universal II and Ladas Super formulas and was superior to the four traditional IOL power calculation formulas. Purpose: The proper selection of an intraocular lens power calculation formula is an essential aspect of cataract surgery. This study evaluated the accuracy of artificial intelligence-based formulas. Design: Systematic review. Methods: This review comprises articles evaluating the exactness of artificial intelligence-based formulas published from 2017 to July 2023. The papers were identified by a literature search of various databases (Pubmed/MEDLINE, Google Scholar, Crossref, Cochrane Library, Web of Science, and SciELO) using the terms "IOL formulas", "FullMonte", "Ladas", "Hill-RBF", "PEARL-DGS", "Kane", "Karmona", "Hoffer QST", and "Nallasamy". In total, 25 peer-reviewed articles in English with the maximum sample and the largest number of compared formulas were examined. Results: The scores of the mean absolute error and percentage of patients within ±0.5 D and ±1.0 D were used to estimate the exactness of the formulas. In most studies the Kane formula obtained the smallest mean absolute error and the highest percentage of patients within ±0.5 D and ±1.0 D. Second place was typically achieved by the PEARL DGS formula. The limitations of the studies were also discussed. Conclusions: Kane seems to be the most accurate artificial intelligence-based formula. PEARL DGS also gives very good results. A thin lens technique was developed to determine how the effective powers of toric monofocal intraocular lenses (IOLs) are influenced by tilt and the refractive errors associated with the tilt. A series of steps determined the effective power of the cornea at the IOL, the IOL power, the effective power of the tilted IOL, the correction required at the front of the eye and the power of an IOL that would compensate for the tilt. The correction was determined by starting at the ideal reduced image vergence at the IOL, backwards raytracing to obtain a reduced image vergence at the cornea, and subtracting the cornea power from this reduced image vergence. Examples are presented for different situations where the IOL is either tilted about the vertical or an oblique axis. Raytracing with a thick lens verified the accuracy of the technique. Background: To evaluate the feasibility of creating flanges using an optic piercing technique with a 6 − 0 polypropylene monofilament for scleral fixation of dislocated one-piece diffractive multifocal intraocular lenses (IOLs). Study Design: Experimental study and case series. Subjects: Optical bench test and eyes with IOL dislocation. Methods: Two separate 6 − 0 polypropylenes were penetrated twice at the opposite peripheral optic of the TECNIS Synergy IOL (Johnson & Johnson Vision). The root mean square of the modulation transfer function (MTFRMS), at between + 1.00 and − 4.00 D of defocus, was measured in the TECNIS Synergy IOL both with and without optic piercing in the optical bench study. This case series included three eyes from two patients who underwent scleral-fixation of multifocal IOLs using the four-flanged polypropylene optic piercing technique. The postoperative corrected distance visual acuity (CDVA) at 4 m, the uncorrected near visual acuity (UNVA) at 40 cm, and IOL centration were evaluated. Results: The optical bench test showed no differences in MTFRMS values measured in the TECNIS Synergy IOL, either with or without optic piercing at all defocuses. In all three case series, the postoperative CDVA at 4 m was 20/20 and UNVA at 40 cm was J1. Postoperative anterior segment photographs showed good centration of IOLs in all cases. Conclusion: The four-flanged polypropylene optic piercing technique for multifocal IOL scleral fixation can provide excellent clinical outcomes and IOL stability after surgery without diminishing the performance of the multifocal IOLs. Purpose: To scrutinize the accuracy of 24 intraocular lens (IOL) power calculation formulas in unoperated eyes. Methods: In a series of consecutive patients undergoing phacoemulsification and implantation of the Tecnis 1 ZCB00 IOL (Johnson & Johnson Vision), the following formulas were evaluated: Barrett Universal II, Castrop, EVO 2.0, Haigis, Hoffer Q, Hoffer QST, Holladay 1, Holladay 2, Holladay 2 (AL Adjusted), K6 (Cooke), Kane, Karmona, LSF AI, Naeser 2, OKULIX, Olsen (OLCR), Olsen (standalone), Panacea, PEARL-DGS, RBF 3.0, SRK/T, T2, VRF, and VRF-G. The IOLMaster 700 (Carl Zeiss Meditec AG) was used for biometric measurements. With optimized lens constants, the mean prediction error (PE) and its standard deviation (SD), the median absolute error (MedAE), the mean absolute error (MAE), and the percentage of eyes with prediction erros within ±0.25, ±0.50, ±0.75, ±1.00, and ±2.00 D were analyzed. Results: Three hundred eyes of 300 patients were enrolled. The heteroscedastic method revealed statistically significant differences (P <.05) among formulas. Newly developed methods such as the VRF-G (standard deviation [SD] ±0.387 D), Kane (SD ±0.395 D), Hoffer QST (SD ±0.404 D), and Barrett Universal II (SD ±0.405) were more accurate than older formulas (P <.05). These formulas also yielded the highest percentage of eyes with a PE within ±0.50 D (84.33%, 82.33%, 83.33%, and 81.33%, respectively). Conclusions: Newer formulas (Barrett Universal II, Hoffer QST, K6, Kane, Karmona, RBF 3.0, PEARL-DGS, and VRF-G) were the most accurate predictors of postoperative refractions. Collisionless flows expanding into a vacuum from a circular slit are investigated using the gaskinetic theory. Both accurate and approximate expressions to the plume fields along a centerline and a constant radius are obtained, including the density, velocity components, and translational temperature. Special non-dimensional parameters such as the exit semi-opening angle, gas speed ratio at the exit, and positions are included in these expressions. Particle simulations with the direct simulation Monte Carlo method are performed to validate the above results. These expressions are complex and may need computers for evaluations; however, the cost is minor when compared to particle simulations. With the assumptions of a high or low exit speed ratio and small exit angle, simple explicit expressions for properties along the centerline and a constant radius are obtained. The investigations also find out that the cosine law relations can provide fast and crude estimations, but it is not recommended to use the cosine law relation to compute collisionless plume flowfields with a large exit speed. Ionospheric storm enhanced density (SED) has been extensively investigated using total electron content deduced from GPS ground and satellite‐borne receivers. However, dayside in situ electron density measurements have not been analyzed in detail for SEDs yet. We report in situ electron density measurements of a SED event in the Northern Hemisphere (NH) at the noon meridian plane measured by the Challenging Minisatellite Payload (CHAMP) polar‐orbiting satellite at about 390 km altitude during the 20 November 2003 magnetic storm. The CHAMP satellite measurements render rare documentation about the dayside SED's life cycle at a fixed magnetic local time (MLT) through multiple passes. Solar wind drivers triggered the SED onset and controlled its lifecycle through its growth and retreat phases. The SED electron density enhancement extended from the equatorial ionization anomaly to the noon cusp. The midlatitude electron density increased to a maximum at the end of the growth phase. Afterward, the dayside SED region retreated gradually to lower magnetic latitudes. The observations showed a hemisphere asymmetry, with the NH electron density exhibiting a more significant enhancement. The simulations using the Thermosphere Ionosphere Electrodynamic General Circulation model show a good agreement with the CHAMP observations. The simulations indicate that the dayside midlatitude electron density enhancement has a complicated dependence on vertical ion drift, neutral wind, magnetic latitude, MLT, and the height of the F2 layer. Finally, we discuss the notion of using the mean cross‐polar cap electric field as a proxy for assessing the effects of solar wind drivers on producing midlatitude electron density enhancement. Plain Language Summary: Ground radar and GPS stations have frequently detected enhancement of ionospheric electron density at midlatitudes and in the polar cap during magnetic storms. However, the storm enhanced density (SED) structure onset has not been observed before. We report in situ satellite observations of the SED onset near 400 km at the noon meridian plane during an intense magnetic storm. It provides clear evidence about the life cycle of ionospheric electron density enhancement, starting from its onset at noon midlatitudes, building up to a maximum value, and retreating to lower latitudes. The midlatitude ionospheric electron density was mainly enhanced in the Northern Hemisphere, triggered by the passage of a solar wind dynamic pressure shock front. Global circulation modeling suggests that ion drift and meridional neutral wind are crucial in transporting midlatitude plasmas during a magnetic storm. As a result, the plasma transport produces the observed dayside enhanced density structure. Given the lack of current solutions, we aim to describe a method for correction of AL measurements in patients who need combined cataract surgery and removal of PFCL tamponade. The refractive considerations of cataract surgery with silicone oil in situ have been well documented.3 The refractive index of silicone oil (1.404) is higher than that of vitreous (1.336) which means that if not corrected for, the optically measured axial length (AL) becomes inaccurate. This paper reports the recent development of a full-scale particle-in-cell (PIC) simulation package highlighting an efficient electro-statics (ES)-PIC implementation for spacecraft charging problems. Numerical simulations are crucial in studying plasma flows because analytical solutions are rare, and experiments are expensive. There are many types of plasma flow that need various numerical methods for efficient and accurate simulations; as such, how to implement and organize those different methods into one comprehensive simulation package is challenging. This work adopted several modern software design patterns and developed a versatile package that includes various PIC schemes. This package has an open architecture, clean interfaces with both serial and parallel simulation capabilities. Two benchmark test cases are included to demonstrate the capabilities of this package. Then two plasma flows around a positively charged probe are simulated, and the results are discussed. The simulation results are consistent with past simulation results, and new insights are obtained. Purpose: To compare axial length measurements based on multiple specific refractive indices for each segment of the eye to those obtained using a single refractive index for the entire eye and to evaluate the subsequent effects on IOL power calculation. Setting: One site in Lynwood, CA. Design: Single-arm, non-interventional, non-randomized retrospective chart review. Methods: Eyes undergoing cataract surgery where biometry and IOL power calculations were based on axial length calculated with multiple specific refractive indices (multiple) were evaluated. A simulated axial length based on using a single refractive index was calculated for each case (single). The expected residual refractions based on different IOL formulas were calculated for both single and multiple groups. Formulas were then optimized, and the mean prediction errors (MPE) and mean absolute prediction errors (MAE) were calculated, based on the difference between the (optimized) expected value and the actual refractive outcome. Results: A total of 595 eligible eyes were evaluated. Differences between the axial lengths determined in the single and multiple groups ranged from +0.28 mm to − 0.14 mm, with a significant correlation between the difference in AL and average AL (r2 = 0.73, p < 0.001). AL differences between groups were statistically significant in long and short eyes (p < 0.001) but not in average eyes or overall (p > 0.25). In nearly all cases, the average MPE in the multiple group was lower than that for the single group across all axial lengths and formulas. When larger differences in MAE were present, the multiple group results were more often lower (better). Conclusion: Differences were found between axial lengths calculated using a single refractive index and multiple refractive indices, mainly in the short and long eyes. We extend past research on the economic lot scheduling problem to address some of the limitations of the earlier work. In particular we develop mixed integer programming formulations with the assumption of a production precedence sequence. When evaluated over a variety of problems from the literature, it is clear that finding a schedule that minimizes costs is not trivial. In this short note, we present some work on investigating electron temperatures and potentials in steady dilute plasma flows. The analysis is based on the detailed fluid model for electrons. Ionizations, normalized electron number density gradients, and magnetic fields are neglected. The transport properties are assumed as local constants. With these treatments, the partial differential equation for electron temperature degenerates as an ordinary differential equation. Along an electron streamline, two simple formulas for electron temperature and plasma potential are obtained. These formulas offer some insights, e.g., the electron temperature and plasma potential distributions along an electron streamline include two exponential functions, and the one for plasma potential includes an extra linear distribution function. Inappropriate administration of radiation for cancer treatment can result in severe consequences such as premature death or appreciably impaired quality of life. There has been little study of vulnerable treatment process components and their contribution to the risk of radiation treatment (RT). In this article, we describe the application of probabilistic fault tree methods to assess the probability of radiation misadministration to patients at a large cancer treatment center. We conducted a systematic analysis of the RT process that identified four process domains: Assessment, Preparation, Treatment, and Follow-up. For the Preparation domain, we analyzed possible incident scenarios via fault trees. For each task, we also identified existing quality control measures. To populate the fault trees we used subjective probabilities from experts and compared results with incident report data. Both the fault tree and the incident report analysis revealed simulation tasks to be most prone to incidents, and the treatment prescription task to be least prone to incidents. The probability of a Preparation domain incident was estimated to be in the range of 0.1–0.7% based on incident reports, which is comparable to the mean value of 0.4% from the fault tree analysis using probabilities from the expert elicitation exercise. In conclusion, an analysis of part of the RT system using a fault tree populated with subjective probabilities from experts was useful in identifying vulnerable components of the system, and provided quantitative data for risk management. We present an analysis of the charging interactivity between surrounding surface materials aboard a spacecraft at geosynchronous altitudes. In particular, bootstrap charging of a small surface may occur if is surrounded by a large negatively charged surface. Here, a negative potential barrier forms above the small surface, resulting in suppression of photo- and secondary electron emission from that surface. Additionally, the small sur- face experiences an enhancement of the collection of the photo- and secondary electrons emitted from the surrounding surface. This mechanism results in the charging of the small surface to higher levels than that of the patch in isolation, and in many cases the final potential will reach that of the potential of the larger surrounding surface. With this study we examine bootstrap charging behavior with model data and with data collected on orbit. We have modeled the DSCS-III B7 geosynchronous satellite with realistic geometry and spacecraft materials. Additionally, a previous study has shown that bootstrap charging has been observed on the DSCS-III B7 geosynchronous spacecraft. Both Astroquartz and Kapton cloth patches charged up to the frame potential of the satellite during periods of severe frame charging. The results of modeling bootstrap charging of a small Kapton patch floating relative to the DSCS-HI frame fixed at a potential of -1,000 V show that the patch will indeed charge up negatively to match the frame potential, with the temporal increase in negative potential following an exponential time characteristic. Nascap-2k is a modern spacecraft charging code, replacing the older codes NASA Charging Analyzer Program for GEosynchronous Orbit (NASCAP/GEO), NASA Charging Analyzer Program for Low-Earth Orbit (NASCAP/LEO), Potentials Of Large objects in the Auroral Region (POLAR), and Dynamic Plasma Analysis Code (DynaPAC). The code builds on the physical principles, mathematical algorithms, and user experience developed over three decades of spacecraft charging research. Capabilities include surface charging in geosynchronous and interplanetary orbits, sheath, and wake structure, and current collection in low-Earth orbits, and auroral charging. External potential structure and particle trajectories are computed using a finite element method on a nested grid structure and may be visualized within the Nascap-2k interface. Space charge can be treated either analytically, self-consistently with particle trajectories, or consistent with imported plume densities. Particle-in-cell (PIC) capabilities are available to study dynamic plasma effects. Auxiliary programs to Nascap-2k include Object Toolkit (for developing spacecraft surface models) and GridTool (for constructing nested grid structures around spacecraft models). The capabilities of the code are illustrated by way of four examples: charging of a geostationary satellite, self-consistent potentials for a negative probe in a low-Earth orbit spacecraft wake, potentials associated with thruster plumes, and PIC calculations of plasma effects on a very low frequency (about 1 to 20 kHz) antenna. Many disasters have occurred because organizations have ignored the warning signs of precursor incidents or have failed to learn from the lessons of the past. Normal accident theory suggests that disasters are the unwanted, but inevitable output of complex socio-technical systems, while high-reliability theory sees disasters as preventable by certain characteristics or response systems of the organization. We develop an organizational response system called incident learning in which normal precursor incidents are used in a learning process to combat complacency and avoid disasters. We build a model of a safety and incident learning system and explore its dynamics. We use the model to motivate managers to implement incident learning systems as a way of moving safety performance from normal accidents to high reliability. Abstract: Managing production and inventory for large-scale continuous processing plants is a key to success in the chemical process industry, particularly in the production of commodity polymers such as polyethylene and polypropylene. When setting up a production schedule, planners must consider the effects of off-grade production and production rate penalties, which are typically sequence-dependent as products are transitioned from one to another, as well as inventory holding costs and capacity constraints. We call this the continuous economic lot sizing and scheduling problem (CELSP) to distinguish it from closely related problems found in discrete part production industries. We present a formulation that addresses the particular aspects of the CELSP and apply the proposed mathematical modeling approach to several plants of a chemical processing company. This company was concerned with ensuring the plants were carrying proper amounts of inventory and with evaluating the number of products produced at each plant. Our results show actual overall inventory levels were close to the levels suggested by our model and therefore company plans for further inventory reductions would not be appropriate. However, the company should carefully consider the addition of new products to current plant product slates. Due to the effects of product transitions and inventory levels, even adding products with significant contribution margins may negatively affect the financial performance of the plant. An analysis of differential charging between dielectric surface materials and the frame of a DSCS-III geosynchronous spacecraft is presented. Charging levels measured by surface potential monitors (SPMs) covered with samples of Kapton and Astroquartz have been recorded for one half of a solar cycle. Both seasonal and solar cycle effects are seen in the daily peak levels of the SPM voltages, with local maxima occurring near the equinoxes and a general trend increasing as solar max is approached. Charge neutralization by an onboard Xe plasma contactor was demonstrated to be effective throughout the mission, with a mean voltage reduction of 86% for Astroquartz and 74% for Kapton. Though a statistical analysis shows a general correlation between the fluence of charging electrons with SPM voltages, the event-specific correlation contains enough variance to cast doubt on the usefulness of an electron sensor as a differential charging alarm. This article describes a system dynamics analysis of the 1992 Westray mine disaster in Nova Scotia, Canada. It examines the causal structure of the Westray system, including relationships that could have led to conditions that caused the fatal explosion at the mine. The value of simulation is its ability to capture a “mental model” of the safety system, which can stimulate discussion among safety experts as to the systemic causes of a disaster. This short note presents concise semi-analytical expressions for electron temperature and potential in unsteady dilute cold plasma flows. The analysis is based on the detailed fluid model for electrons. Ionizations and normalized electron number density gradients are neglected, and the transport properties are assumed as local constants. Flow is unsteady and Maxwell's equations are adopted in the analysis. With these treatments, the partial differential equations for unsteady electron temperature and potential degenerate as ordinary differential equations. Along an electron streamline, two simple formulas for unsteady electron temperature and plasma potential are obtained. These formulas offer some insights, for example, the electron temperature and plasma potential distributions along an electron streamline include two exponential functions: one for spatial distance along a streamline and the other for time.
