Objectives: Crimean‐Congo haemorrhagic fever (CCHF) is a zoonotic viral infection which is an important public health problem in Turkey. CCHF causes fever and bleeding and can lead to severe health outcomes. The study aims to report a case of a male patient with severe CCHF, hemophagocytic lymphohistiocytosis (HLH) treated with steroids and portal vein thrombosis. Case Report: A 37‐year‐old man was admitted to the emergency department with complaints of high fever, headache, myalgia and diarrhoea. The patient travelled to the endemic region of Turkey. In laboratory findings, thrombocytopenia, abnormal liver function tests and elevated coagulation parameters were observed. Real‐time polymerase chain reaction assay was used for diagnosis of CCHF. Hypofibrinogenemia, hypertriglyceridemia, elevated ferritin and d‐dimer levels were observed in the clinical follow‐up. Prednisolone treatment was performed due to considered the diagnosis of HLH. Portal vein thrombosis was detected on abdominal computed tomography scan. He was successfully treated with ribavirin, corticosteroids, anticoagulant and supportive therapy. Conclusion: The clinical presentation of CCHF can range from self‐limiting flu‐like to severe symptoms possibly fatal. Acute portal vein embolism is a rare complication that has not been reported before to our knowledge. Corticosteroids may be a life‐saving treatment for CCHF patients presenting with HLH. This study investigates how presenteeism (PRES) influences job performance (JP), with a specific focus on how organisational attractiveness (OA) moderates this relationship. Conducted with 500 employees from a Turkish call centre in the communication sector, situated in Erzincan province, findings reveal that PRES adversely impacts JP. However, OA acts as a significant buffer, diminishing PRES's negative effects. Notably, when employees’ OA perceptions are high, PRES has less of a negative impact on their JP. Object: In patients with headache, the distinction between transverse sinus (TS) atresia/hypoplasia and TS thrombosis in cranial magnetic resonance venography (MRV) may be misleading. In this study, we aimed to distinguish TS thrombosis from atretic or severely hypoplastic TS, with the help of cranial computed tomography (CT). Method: Non-contrast cranial CT scans of 51 patients with no or severely thin signal on MRV were analyzed retrospectively using the bone window. Absence or asymmetry of the sigmoid notches on the CT predicted atretic or severe hypoplastic TS, and symmetry predicted thrombotic TS. Afterwards, it was investigated whether the patient's other imaging findings and confirmed diagnoses matched with the predictions. Findings: Of the 51 patients included in the study, 15 were diagnosed with TS thrombosis, and 36 were diagnosed with atretic/hypoplastic TS. All 36 of the congenital atresia/hypoplasia diagnoses were correctly predicted. Thrombosis was predicted correctly in 14 of 15 patients with TS thrombosis. In cranial CT, the symmetry or asymmetry of the sigmoid notch sign was examined, and it was found that the evaluation predicted with 93.3% sensitivity (95% confidence interval (CI): 68.05–99.83) and 100% specificity (95% CI: 90.26–100.00) the distinction between TS thrombosis and atretic/hypoplastic sinus. Conclusion: Symmetry or asymmetry of the sigmoid notch on CT is a reliable method that can be used to differentiate congenital atresia/hypoplasia from the TS thrombosis in patients with very thin or absent TS signal on the cranial MRV. It has been shown that there is an increased risk of cardiovascular events such as heart failure and death in nephrotic syndrome. Left ventricle global longitudinal strain (LVGLS) is a more sensitive measure of assessing myocardial dysfunction and is more reproducible than left ventricle ejection fraction (LVEF%). LVGLS can detectsubclinical deterioration in the left ventricle early. In this study, we aimed to investigate LVGLS in Primary Nephrotic Syndrome (PNS) patients with normal LVEF%. Patients with histopathologically confirmed PNS were evaluated for this prospective single-center study. Patients with similar age and gender characteristics without nephrotic syndrome were included as the control group. LVGLS measurements were performed by 2D speckle tracking echocardiography. A total of 171 patients, 57 with PNS and 114 in the control group, were included in the study. The mean age was 38±12 years in the study population, and 95(56%) of the patients were women. LVEF% was 60.2±4.2 in the PNS group and 61.1±3.2 in the control group, and there was no significant difference between the two groups (p=0.111). LVGLS was found to be significantly lower in the PNS group (-19.3±2.3% vs.-20.8±1.5 %, p<0.001). A significant relationship was observed between PNS and LVGLS in the multivariable linear regression analysis (β= 4.428, CI 95% =0.57?1.48, p<0.001). A significant relationship was observed between PNS and LVGLS, and LVGLS was found to be lower in PNS patients. Background/Aim: Diabetic macular edema (DME) is a common retinal disease and is the most common cause of vision loss due to diabetic retinopathy. During the COVID-19 pandemic, there has been a serious decrease in hospital visits due to both the measures taken by governments and the anxiety of patients fearful of contracting COVID-19. The aim of this study is to determine the problems occurring during the COVID-19 pandemic period in patients treated with intravitreal (IV) injection for diabetic retinopathy, and to provide recommendations for treatment management in these patients. Methods: Twenty-nine eyes of 17 patients who were diagnosed with DME were included. The frequency of hospital visits, treatments performed, and detailed ophthalmological examination findings, including optical coherence tomography findings and glycated haemoglobin (HbA1c) values, were recorded in the period before the COVID-19 pandemic. During the COVID-19 pandemic period, the detailed ophthalmological examination findings and HbA1c values were noted after the patients who had delayed their routine control time (>90 days) applied to the hospital. New treatments were planned according to the current situation. Results: Seventeen patients were included in the study; 10 were female (58.8%) and seven were male (41.2%). The mean time interval between following visits was 45.5 (5.9) days during the pre-pandemic period, but it increased to 110.4 (13.5) days during the COVID-19 pandemic period (P<0.001). Visual acuity (LogMAR) was 0.4 (0.4) in the pre-pandemic period and 0.8 (0.5) during the COVID-19 pandemic period (P=0.003). Central macular thickness was 300.1 (85.6) µm in the pre-pandemic period and it increased to 387.1 (144.5) µm during the COVID-19 pandemic period (P=0.007). In three patients, complications of diabetic retinopathy that could not be cured by medical treatment developed and surgical treatment was recommended. Conclusions: Delays in the treatment of diabetic retinopathy may cause permanent impairment in visual functions. The COVID-19 pandemic has caused an increase in the hospital visit intervals of patients, and this situation has resulted in disruptions in the follow-up and treatment of patients with diabetic retinopathy. Introduction: To examine the treatment success and the factors affecting the treatment success in patients with chronic viral hepatitis C. Methods: This retrospective study was conducted by scanning the files of patients with chronic viral hepatitis C, who were followed in Infectious Diseases and Gastroenterology outpatient clinics. Demographic and clinical characteristics (presence of hepatomegaly or splenomegaly, comorbidity treatment protocols, and side effects) were examined. Results: 418 patients were included in the study. The mean age of the patients was 48.4 (min: 27-max:76) years. 40.4% (n=169) of the patients were male. Fifty patients had hepatomegaly and 45 had splenomegaly. While 79.9% of the patients had no comorbid disease, 13.4% had diabetes, 5.3% had thyroid dysfunction, and 1.4% had both. Hepatocellular carcinoma was observed in 4 patients, cirrhosis was observed in 29 patients, and side effects were observed in 33 patients. Four patients received ribavirin, 32 patients received classical interferon, 13 patients received pegylated interferon, 69 patients received classical interferon+ribavirin, 297 patients received pegylated-interferon+ribavirin, and three patients received classical interferon+ ribavirin+pegylated-interferon. Of the patients, 12 resulted in "exacerbation under treatment," 14 with "partial response," 89 with "relapse," 124 with "no response," and 179 with "sustained virologic response (SVR)." Overall, the most successful results were obtained in patients treated with pegylated interferon+ribavirin. In univariate comparisons, younger age, absence of splenomegaly, and absence of cirrhosis were significant in patients with SVR. In multivariate analysis, combined therapy was found to be independently significantly successful among patients with SVR. Discussion and Conclusion: In our study, the response rate of patients to interferon-based treatment was 42.8%, while young age, female gender, and absence of organomegaly were determined as factors that increased this rate, and according to multiple analyses, it was determined that combined treatment alone was effective on SVR independently. Aim of the study: To investigate the frequency of ABO blood groups and the relationship between nephrotic syndrome (NS) and blood groups in patients with NS. Material and methods: 199 patients with a diagnosis of NS and 200 healthy individuals, who applied to Dicle University Faculty of Medicine, Department of Nephrology between 2016 and 2020, and whose blood group information was in the records, were included in the study as the control group. This study was carried out retrospectively using data from the file registration system in our hospital. Results: In the study, there were 199 patients: 52.8% (n = 105) male and 47.2% (n = 94) female and 200 healthy individuals: 47.5% male (n = 95) 52.5% female (n = 105). The mean age of the patients was 35.79 ± 12.21 (min-max: 17-71), and the mean proteinuria was 7.08 ± 4.33 (min-max: 3.50-25.50). There was no difference between sick and healthy individuals in terms of age or sex (p > 0.05). It was determined that 43.2% of the patients had blood group A, 26.1% blood group O, 25.1% blood group B, and 5.5% blood group AB. There was no significant difference between blood groups of patients with NS and individuals in the healthy group (p > 0.05). Membranous glomerulonephritis (MGN) (27.9%) was most common in blood group A; FSGS was detected most frequently in blood groups B (46.0%), O (28.8%), and AB (36.4%). The most common cause of NS was focal segmental glomerulosclerosis (FSGS) in 31.2% (n = 62), and MGN was the second common cause in 19.1% (n = 38), blood group B (37.1%) in FSGS, blood group O (46.7%) in systemic lupus erythematosus, MGN (63.2%), IgA nephropathy (50%), and MPGN (52.2%) was found most frequently in blood group A. Background: A good glycemic regulation should be provided to reduce mortality and morbidity in patients with end-stage renal failure due to diabetes mellitus. However, the use of insulin therapies in patients with renal failure is troublesome due to the increased rate of side effects. In our study, we investigated the frequency of hypoglycemia and its symptoms in patient groups receiving intensive and mixed insulin therapies. Materials and methods: This prospective study included 89 hemodialysis patients with DM-related stage 5 chronic kidney disease who were followed up in a nephrology clinic of a university hospital between January 2013 and August 2015. Our study group was divided into two groups as those receiving intensive insulin therapy and mixed insulin therapy. Group 1 and group 2 consisted of 46 patients and 43 patients, respectively. Hypoglycemia (glucose < 70 mg/dl) was investigated in patients with HbA1c levels that decreased below 7% after treatment, and the symptoms were evaluated according to the hypoglycemic scale. The results between the groups were evaluated using chi-square and Student's t test Results: A total of 89 patients were included in the study. HbA1c was 6.83% in the group receiving intensive insulin therapy and 6.95% in the group receiving mixed insulin therapy (p > 0.05). Hypoglycemia was detected in 27 patients (58.7%) in the intensive insulin therapy group and 14 patients (32.6%) in the mixed insulin therapy group. There was a significant level of hypoglycemia in the group receiving intensive insulin therapy (p < 0.05). In our study, the group receiving intensive insulin therapy had higher frequency and severity of common hypoglycemia symptoms such as confusion, sweating, weakness, dysphasia, palpitations, blurred vision, and feeling hungry. Conclusion: There was a higher frequency of hypoglycemia under intensive insulin therapy in patients undergoing dialysis due to chronic kidney failure, which suggests that mixed insulin therapy is the ideal treatment to avoid hypoglycemia in this group of patients. Graves' disease has a multitude of effects on the cardiovascular system. In the detection of subclinical left ventricular dysfunction, speckle-tracking echocardiography is more useful than conventional echocardiography. The aim of the present study was to compare the longitudinal global strain values and venous blood concentration of (tissue inhibitor of metalloproteinase-1) TIMP-1, a regulator of the extracellular matrix, among hyperthyroid patients with Graves' disease, euthyroid patients with Graves' disease and healthy control subjects. The study enrolled 40 hyperthyroid patients with newly diagnosed Graves' disease, 40 patients with Graves' disease who were euthyroid for at least 6 months and 40 control subjects with normal thyroid function. Participants underwent conventional echocardiography and speckle-tracking echocardiography to obtain segmental and global longitudinal strain values. In addition, the serum TIMP-1 value was assessed in the venous blood samples of the participants. The hyperthyroid Graves' patients showed greater serum TIMP-1 levels versus the control group and the euthyroid Graves' group. Compared to the control group, the TIMP-1 level was also significantly higher in the euthyroid Graves' group. Lower GLS (global longitudinal strain) average and GLS2C, GLS3C, GLS4C values were observed in both the hyperthyroid and euthyroid Graves' patients compared to the control group. The euthyroid Graves' patients and hyperthyroid Graves' patients had similar GLS values. The serum TIMP-1 level was negatively correlated with global strain values. Graves' disease coexists with impaired segmental and global longitudinal strain and increased TIMP-1 levels. This coexistence seems to be independent of serum thyroid hormone levels. This report illustrates a patient presenting with a unilateral headache at whom brain magnetic resonance imaging showed temporal lobe parenchyma herniation. In our patient, there were no clinical or neuroimaging findings of increased intracranial pressure (ICP) that might lead to a headache. On the other hand, considering the dynamic nature of headache in our patient, we cannot exclude a mechanism of possible episodic ICP increments that might be related to encephalocele. Objective: Myocardial infarction (MI) with non-obstructive coronary arteries (MINOCA) is a relatively new term that is characterized by clinical evidence of MI with normal or near-normal coronary arteries on coronary angiography (QCA). To date, there have been no population-based studies on the prevalence of MINOCA in Turkey. The aim of this nationwide study was to document the prevalence and demographics of MINOCA in a Turkish population. Methods: MINOCA-TR is national, multi-center, prospective, all-comer study that was conducted in 32 hospitals. All consecutive patients who were =18 years old, diagnosed with MI according to the Third Universal Definition of Myocardial Infarction, and had undergone QCA were included in the study. Patients with stable coronary artery disease, unstable angina pectoris, a history of revascularization, and type 4/5 MI were excluded. Results: A total of 1793 patients who were diagnosed with MI and had undergone QCA were screened between March 2018 and October 2018, of whom 1626 (mean age: 61.5±12.5 years, 70.7% male) were enrolled from 32 centers. The prevalence of MINOCA was 6.7% (n=109) in the overall study population. Compared with non-MINOCA patients, those with MINOCA were younger, had a higher prevalence of the female gender, and had a history of flu. The percentages of current smokers, ST-segment elevated myocardial infarction patients, and those with a history of hypertension, diabetes mellitus, and hyperlipidemia were significantly lower in MINOCA patients (p<0.05, for all). Also, the median left ventricular ejection fraction as seen on echocardiography and the ratio of Killip Class I status at presentation was significantly higher in MINOCA patients than in non-MINOCA patients (p<0.001). Patients with MINOCA received a preload dose of P2Y12 antagonist before QCA less often than non- MINOCA patients (p<0.001). Conclusion: The prevalence of MINOCA in Turkey is 6.7% in patients who were admitted with MI. Background: We aimed to assess the factors influencing primary care physicians' (PCPs) approach to adult vaccination in specific risk groups and evaluate the compliance to adult immunization guidelines. Methods: This cross-sectional study performed between January 2016 and April 2016 in İstanbul, Turkey. A questionnaire designed to obtain physicians' demographical data, experience, immunization status, and attitude on prescribing or recommending vaccines for adults in the risk group. Healthy individuals older than 65 and patients suffer from chronic diseases or had splenectomy before are considered as a risk group. The questionnaire was sent via email to a randomly selected group of 1,500 PCPs. The data of 221 physicians who responded emails were recorded for statistical analysis. Results: Of the 221 participants (123 women, 98 men), the majority were aged 31-40 years. Their vaccination rates were 74.2% for hepatitis B, 54.3% for seasonal influenza, and 47.1% for tetanus. Among participants, the highest recommendation and prescription rate of adult vaccines was recorded in PCPs aged 31-40 years. In addition, PCPs with <10 years occupational experience were found to prescribe adult vaccines more frequently than PCPs with longer occupational experience. Conclusions: Primary care physicians with lower age and relatively less experience are more intent to prescribe adult vaccines to patients that are in risk groups. This result may be due to increased awareness of adult immunization among PCPs who had more recent medical training. Objective: Endothelial dysfunction (ED) is a condition that involves increased oxidative stress and decreased total antioxidant status (TAS) levels. Systemic lupus erythematosus (SLE) is also associated with ED. We aimed to determine the association between serum TAS and ED as assessed by flow-mediated dilation (FMD) in patients with SLE Methods: Thirty-four patients with stable SLE who were not undergoing any treatment and 39 healthy volunteers without any overt cardiovascular disease were included in this cross-sectional study. Doppler ultrasound was used to measure FMD to assess ED in the study groups. Serum TAS levels were measured using a TAS kit. High-sensitivity C-reactive protein (hs-CRP) and anticardiolipin antibody (aCLA) levels were also measured to assess the inflammatory state. The SLE group further was divided into 2 groups according to presence or absence of aCLA. SLE disease activity was assessed using the SLE disease activity index (SLEDAI). Regression analysis was used to define independent predictors Results: The mean TAS levels were significantly lower in patients with SLE than in controls (1.60±0.11 versus 1.73±0.15 mmol/L, p<0.001). hs-CRP levels were significantly higher in patients with SLE than in controls (8.2±6.0 vs. 2.9±4.0 mg/L; p<0.001), particularly in SLE patients with positive aCLA when compared with SLE patients with negative aCLA (13.8±4.3 vs. 5.6±4.8 mg/L, p<0.001). The FMD percent was significantly lower in patients with SLE than in controls (8.1±4.9 vs. 10.6±4.7, p=0.04). There was a significant positive correlation between FMD and TAS in the SLE group (r=0.448, p=0.008) and the control group (r=0.367, p=0.03) and a significant negative correlation between FMD and serum hs-CRP (r=-0.368, p=0.04) in only the SLE group. In multiple linear regression analysis, TAS, hs-CRP, and SLEDAI were independently correlated with FMD (β=0.50, p=0.003; β=-0.33, p=0.03; and β=-0.36, p=0.03; respectively) Conclusion: Patients with SLE who have no overt cardiovascular disease are at increased risk for ED and this may be associated with underlying inflammation and impairment of TAS. Microbubbles have been presumed as gaseous emboli, which originate during mechanical heart valve closure, but are not seen in bioprosthetic valves. In this report, we presented a cluster of microbubbles mimicking mobile thrombus in a patient with mechanical mitral valve prosthesis. A 30-year-old female with a history of implanted mechanical valve at the mitral position underwent a routine examination. She was asymptomatic and her physical examination was unremarkable. Transthoracic echocardiography showed a mobile thrombus-like mass on the ventricular side of the prosthetic mitral valve moving into the left ventricular outflow tract. However, close examination of images indicated that the mass was in fact intense microbubbles mimicking thrombus. Intense mobile microbubbles can be misdiagnosed as a mobile thrombus. We recommend and underscore the importance of detailed echocardiographic examination in case of mobile mass to avoid misdiagnosis in patients with mechanical heart valves. [ABSTRACT FROM AUTHOR]

Magnetic poly(ethylene glycol dimethacrylate‐N‐methacryloyl‐(L)‐histidine methyl ester) [m‐poly(EGDMA‐MAH)] beads were prepared by suspension polymerization for the affinity depletion of immunoglobulin‐G (IgG) from human serum in a batch system. Elemental analysis of the magnetic beads for nitrogen was estimated as 70 μmol MAH/g polymer. IgG adsorption onto the m‐poly(EGDMA) was negligible. Higher adsorption value (up to 46.8 mg/g) was obtained in which the m‐poly(EGDMA‐MAH) beads were used. IgG adsorption capacity of the magnetic beads increased with an increase in the concentration of IgG. The maximum IgG adsorption was observed at pH 6.5 for MOPS buffer. IgG molecules could be repeatedly adsorbed and eluted with these adsorbents, without noticeable loss in their IgG adsorption capacity. Adsorption capacity decreased for both increasing salt concentration and temperature. In this study, we show that m‐poly(EGDMA‐MAH) beads (wherein IgG molecules bind directly with the matrix) can be used directly for affinity depletion without further modification. Higher adsorption value was obtained from human serum (up to 85.7 mg/g). The elution results demonstrated that the adsorption of IgG to the adsorbent was reversible. The depletion efficiencies for IgG were above 85% for all studied concentrations. Eluted portion was analyzed for testing the IgG removal efficiency by two dimensional gel electrophoresis. Eluted proteins include mainly IgG, and a small number of nonalbumin proteins such as apolipoprotein A1, serotransferrin, haptoglobulin, and α1‐antitrypsin.
