Objectives: We developed the first German evidence- and consensus-based clinical guideline on diagnosis, treatment, and follow-up of germ cell tumours (GCT) of the testes in adult patients. We present the guideline content in 2 separate publications. The present second part summarizes therecommendations for the treatment of advanced disease stages and for the management of follow-up and late effects. Materials and Methods: An interdisciplinary panel of 42 experts including 1 patient representative developed the guideline content. Clinical recommendations and statements were based on scientific evidence and expert consensus. For this purpose, evidence tables for several review questions, which were based on systematic literature searches (last search in March 2018), were provided. Thirty-one experts, who were entitled to vote, rated the final clinical recommendations and statements. Results: Here we present the treatment recommendations separately for patients with metastatic seminoma and non-seminomatous GCT (stages IIA/B and IIC/III), for restaging and treatment of residual masses, and for relapsed and refractory disease stages. The recommendations also cover extragonadal and sex cord/stromal tumours, the management of follow-up and toxicity, quality-of-life aspects, palliative care, and supportive therapy. Conclusion: Physicians and other medical service providers who are involved in the diagnostics, treatment, and follow-up of GCT (all stages, outpatient and inpatient care as well as rehabilitation) are the users of the present guideline. The guideline also comprises quality indicators for measuring the implementation of the guideline recommendations in routine clinical care; these data will be presented in a future publication. Part I covers the topic's background, methods, epidemiology, classification systems, diagnostics, prognosis, and treatment recommendations for the localized stages. Methods: An interdisciplinary panel of 42 experts including 1 patient representative developed the guideline content. Clinical recommendations and statements were based on scientific evidence and expert consensus. For this purpose, evidence tables for several review questions, which were based on systematic literature searches (last search was in March 2018) were provided. Thirty-one experts entitled to vote, rated the final clinical recommendations and statements. Results: We provide 161 clinical recommendations and statements. We present information on the quality of cancer care and epidemiology and give recommendations for staging and classification as well as for diagnostic procedures. The diagnostic recommendations encompass measures for assessing the primary tumour as well as procedures for the detection of metastases. One chapter addresses prognostic factors. In part I, we separately present the treatment recommendations for germ cell neoplasia in situ, and the organ-confined stages (clinical stage I) of both seminoma and nonseminoma. Conclusion: Although GCT is a rare tumour entity with excellent survival rates for the localized stages, its management requires an interdisciplinary approach, including several clinical experts. Quality of care is highly related to institutional expertise and can be reassured by established online-based second-opinion boards. There are very few studies on diagnostics with good level of evidence. Treatment of metastatic GCTs must be tailored to the risk according to the International Germ Cell Cancer Collaboration Group classification after careful diagnostic evaluation. An interdisciplinary approach as well as the referral of selected patients to centres with proven experience can help achieve favourable clinical outcomes. [ABSTRACT FROM AUTHOR]

The article relates Paul Hinlicky's project of critical dogmatics to the constructive work of two Reformed theologians, Jürgen Moltmann's The Crucified God (1972) and Mark Lewis Taylor's The Executed God (2001/2015). After highlighting central tenets of Hinlicky's paradigm I discuss and compare the monographs by Moltmann and Taylor, weighing their similarities and differences. The article discusses Richard Wagner's last music-drama, which today is the traditional Good Friday "opera" in New York, Vienna, and other venues around the globe. I argue that Parsifal utilizes traditional Christian symbols and thereby transforms them, in order to help transform the world of the audience. The first part of the article summarizes the dramatic conflict and analyzes how the work appropriates the Christian symbolism of the Lord's Supper. I also look at Wagner's essay "Religion and Art," which was written during the composition of Parsifal and presents an ethical critique of Christianity in the name of "true religion." The second part of the article presents two assessments of Parsifal, both of which acknowledge its inherent religious symbolism but come to different conclusions regarding its significance (Christian versus atheistic). Der Aufsatz bietet eine kritische Würdigung des lutherischen Theologen Hermann Cremer (1834-1903), der mit Recht als ›Haupt der Greifswalder Schule‹ an der Wende vom 19. zum 20. Jahrhundert bezeichnet wird. Im Mittelpunkt steht Cremers Abhandlung »Die christliche Lehre von Gottes Eigenschaften« aus dem Jahr 1897, die im Kontext seiner Soteriologie und theologischen Epistemologie behandelt wird. Cremers Neuansatz in der Eigenschaftslehre intendiert eine gesamtbiblische Entfaltung des Begriffs der Gerechtigkeit, die betont, dass Gott gerade in seinem erlösendem Handeln gerecht richtet: Vergebung heißt »nicht Rettung von seiner Hand, sondern Rettung durch seine Hand«. Anschließend kommt Cremers in der bisherigen Forschung kaum beachtete Debatte mit seinem Rostocker Kollegen K. F. Nösgen über zentrale Fragen der Rechtfertigungslehre zur Sprache, bevor wir einen genaueren Blick auf die Rezeption von Cremers Schrift im 20. Jahrhundert werfen. The article offers a critical appreciation of the Lutheran theologian Hermann Cremer (1834-1903), the leading figure of the so-called Greifswald School at the turn from the 19th to the 20th century. In particular, I consider Cremer's essay on the doctrine of God's attributes from 1897 in the context of his soteriology and theological epistemology, with a particular emphasis on his creative revision of the concept of God's righteousness. According to Cremer, God's grace is efficient precisely in and through God's judgment. I also look at Cremer's debate with Karl Friedrich Nösgen on the 'righteousness of faith' and the idea of justification by faith alone. Cremer's thinking is then brought into conversation with several interpreters of his work in the 20th century. I conclude by raising some questions for further discussion, especially in regard to Cremer's concept of conscience. [ABSTRACT FROM AUTHOR]

The article compares the doctrine of election in the theology of Friedrich Schleiermacher, particularly his magisterial essay on the topic from 1819, and the theology of Karl Barth between 1920 and 1925. It argues that both positions are strikingly similar, in regard to both their critical evaluation of the tradition and their constructive proposals for a new foundation. Both theologians offer a theocentric reassessment that shuns the particularism of previous approaches and affirms the unity of the divine will. Schleiermacher defends the Augustinian-Calvinist view of election as a bulwark against Manichaeism and Pelagianism. Still, he criticizes the idea of eternal damnation and argues that the concept of reprobation instead should be understood as a temporal 'passing over'. The kingdom of God is realized gradually, not in one instant. Similarly, for Barth predestination is not a pre-temporal decree that divides human beings into two separate groups of persons but is actualized ever anew in history, when God's address entails the miracle of faith through the work of the Holy Spirit. The twofold possibility of faith and unbelief is constitutive for the human encounter with God, and it concerns believers and unbelievers alike, since no person is forever exclusively elect or reprobate. Barth also insists that the relation between reprobation and election is non-dualistic and teleological: predestination is a movement from reprobation to election, and never the other way around. I discuss two recent books by Ingolf U. Dalferth, which consider the problem of suffering and evil. Dalferth argues that evil is defined by events where someone experiences ills, not by an act of the will or an evil intention, as Kant thought. Suffering is seen in relation to evil, evil in relation to God, and God in relation to the history of Jesus Christ, in whom God created new life through the overcoming of evil. Christianity shall proclaim the end of evil, not the end of suffering. Dalferth contends that nothing connects human beings as strongly as the common experience of evil. This article analyses Schleiermacher's essay on election, which has not received the attention it deserves. I argue that it presents a significant advance in the debate about unconditional grace and the relation between redemption and predestination. While Schleiermacher defends the Augustinian-Calvinist view of election, he criticizes the ideas of double predestination and eternal damnation (for the unbelievers). Thus, he wants to provide a new basis for Lutherans and Calvinists to settle or, at least, reconsider their longstanding controversy over the matter. After the publication of the essay in 1819, Schleiermacher's doctrine of election remained unchanged, except for minor modifications. [ABSTRACT FROM AUTHOR]
