This paper explores how health professional education and scholarship is likely to evolve over the next 50 years. Recognising the ambitious nature of this goal, we have adopted an approach of "looking back to predict the future", drawing on past trends and lessons learnt to offer some predictions and recommendations. In doing so, we are mindful that recent advances in technology, especially related to generative artificial intelligence (AI), have added a further layer of complexity. The rapid rate of maturation of these technologies and the accompanying array of AI-based software solutions suggest that the educational landscape is likely to change dramatically--and continuously--over the foreseeable future. Within this context, it may seem premature to predict the future or make recommendations. However, in this paper we argue that urgent action is needed by educators to engage with the evolving landscape and to proactively develop models to harness the full potential of emerging technologies. Six themes are addressed, including: 1) the evolving role of educators and education providers, 2) innovative educational models and ethical frameworks, 3) future-focused competencies, 4) authentic assessment, 5) interprofessional education and 6) educational scholarship. This paper proposes a set of recommendations with the intention of stimulating critical discourse that can potentially lead to ongoing exploration of relevant issues. [ABSTRACT FROM AUTHOR]

Dopaminergic mechanisms regulating cognitive and motor control were evaluated comparing visuoperceptual and perceptuomotor functions in Parkinson's disease (PD). The performance of PD patients (n = 40) was contrasted with healthy controls (n = 42) across two separate visits (on and off dopaminergic medications) on computerized tasks of perception and aiming to a target at variable stimulus lengths (4, 8, 12 cm). Novel visuoperceptual tasks of length equivalence and width interval estimations without motor demands were compared with tasks estimating spatial deviation in movement termination. The findings support the presence of spatial deficits in early PD, more pronounced with increased discrimination difficulty, and with shorter stimulus lengths of 4 cm for both visuoperceptual and perceptumotor functions. Dopaminergic medication had an adverse impact on visuoperceptual accuracy in particular for length equivalence estimations, in contrast with dopaminergic modulation of perceptuomotor functions that reduced angular displacements toward the target. The differential outcomes for spatial accuracy in perception versus movement termination in PD are consistent with involvement of the direct pathway and models of progressive loss of dopamine through corticostriatal loops. Future research should develop validated and sensitive standardized tests of perception and explore dopaminergic selective deficits in PD to optimize medication titration for motor and cognitive symptoms of the disease. Purpose: The aim of this research was to determine the long-term prosthodontic maintenance requirements of mandibular two-implant overdentures using different loading protocols and attachment systems. Materials and Methods: A total of 106 participants were allocated randomly to one of four different implant systems (Steri-Oss, Southern, Straumann, or Branemark). Three different loading protocols (2, 6, and 12 weeks) were used with six different ball abutment patrices and their respective matrices (Steri-Oss rubber, Straumann gold, Straumann titanium, Branemark gold, Southern plastic, and Southern gold/platinum). Prosthodontic maintenance events were documented prospectively from baseline until the 8-year recall according to predefined categories. Results: After 6 years, 90 participants attended recall and, thereafter, 68 participants were followed for 8 years. No significant differences were found between the number of prosthodontic maintenance events and the loading protocol used. Steri-Oss rubber matrices had the highest mean number of maintenance events at 32.2 ± 14.5 events, followed by the Branemark gold matrices at 28.8 ± 12.6 events. The Southern plastic matrices had a significantly lower mean number of maintenance events (8.7 ± 4.2) when compared with all other groups. Over a 6-year period, the matrices with the best longevity were Straumann gold at 3.9 ± 2.1 years. Straumann gold matrices also lasted significantly longer than all other matrices (P < .05). Southern gold/platinum, Branemark gold, and Southern plastic matrices all lasted significantly longer than the Straumann titanium and Steri-Oss matrices (P < .05). The mean time to reline for overdentures was 3.37 ± 2.06 years; remaking of overdentures peaked by year 7, with a mean time to remake of 5.81 ± 2.04 years. Conclusion: Early loading protocols do not influence long-term prosthodontic maintenance requirements of unsplinted mandibular two-implant overdentures. Purpose: The aim of this study was to review the published literature investigating the accuracy of fit of fixed implant frameworks fabricated using different materials and methods. Materials and Methods: A comprehensive electronic search was performed through PubMed (MEDLINE) using Boolean operators to combine key words. The search was limited to articles written in English and published through May 2010. In addition, a manual search through articles and reference lists retrieved from the electronic search and peer-reviewed journals was also conducted. Results: A total of 248 articles were retrieved, and 26 met the specified inclusion criteria for the review. The selected articles assessed the fit of fixed implant frameworks fabricated by different techniques. The investigated fabrication approaches were one-piece casting, sectioning and reconnection, spark erosion with an electric discharge machine, computer-aided design/computer-assisted manufacturing (CAD/CAM), and framework bonding to prefabricated abutment cylinders. Conclusions: Cast noble metal frameworks have a predictable fit, and additional fit refinement treatment is not indicated in well-controlled conditions. Base metal castings do not provide a satisfactory level of fit unless additional refinement treatment is performed, such as sectioning and laser welding or spark erosion. Spark erosion, framework bonding to prefabricated abutment cylinders, and CAD/CAM have the potential to provide implant frameworks with an excellent fit; CAD/CAM is the most consistent and least technique-sensitive of these methods. Purpose: This review critically compares the available clinical and laboratory methods for assessing the fit of an implant prosthesis. Materials and Methods: MEDLINE and PubMed databases were searched for studies related to the fit of implant prostheses or the effect of misfit. Different combinations of key words were used. The screening procedure was performed in two stages. In the first stage, the relevant articles were selected. In the second stage, from those articles, specific inclusion and exclusion criteria were applied. Results: Fifty-four articles were selected. Two different techniques were identified: in vitro and in vivo. The advantages, disadvantages, and limitations of each technique are discussed. There are clear limitations for the in vivo techniques. The in vitro techniques provide a more accurate measure of prosthesis misfit. Conclusions: Because of the variety of techniques and the different parameters assessed by each, it is useful to combine several techniques to assess the accuracy of fit, quantify the effect of misfit, and subsequently determine an acceptable level of fit. A novel silver nanoparticle (AgNP) formulation was developed as a targeted application for the disinfection of carious dentine. Silver nitrate (AgNO3) was chemically reduced using sodium borohydrate (NaBH4) in the presence of sodium dodecyl sulfate (SDS) to form micelle aggregate structures containing monodisperse 6.7- to 9.2-nm stabilized AgNPs. AgNPs were characterized by measurement of electrical conductivity and dynamic light scattering, scanning electron microscopy, transmission electron microscopy, and inductively coupled plasma mass spectrometry. Antimicrobial activity of AgNPs was tested against planktonic cultures of representative grampositive and gram-negative oral bacteria using well diffusion assays on tryptic soy broth media and monoculture biofilms grown with brain heart infusion ± sucrose anaerobically at 37°C on microtiter plates. Biofilm mass was measured by crystal violet assay. Effects were compared to silver diamine fluoride and chlorhexidine (negative controls) and 70% isopropanol (positive control) exposed cultures. In the presence of AgNPs, triplicate testing against Streptococcus gordonii DL1, C219, G102, and ATCC10558 strains; Streptococcus mutans UA159; Streptococcus mitis 118; and Enterococcus faecalis JH22 for planktonic bacteria, the minimum inhibitory concentrations were as low as 7.6 µg mL-1 and the minimum bacteriocidal concentrations as low as 19.2 µg mL-1 silver concentration. Microplate readings detecting crystal violet light absorption at 590 nm showed statistically significant differences between AgNP-exposed biofilms and where no antimicrobial agents were used. The presence of sucrose did not influence the sensitivity of any of the bacteria. By preventing in vitro biofilm formation for several Streptococcus spp. and E. faecalis, this AgNP formulation demonstrates potential for clinical application inhibiting biofilms. Introduction Joint haemorrhage is the principal clinical manifestation of haemophilia frequently leading to advanced arthropathy and arthrofibrosis, resulting in severe disability. The degree and prevalence of arthrofibrosis in hemophilic arthropathy is more severe than in other forms of arthropathy. Expression of connective tissue growth factor ( CTGF) has been linked to many fibrotic diseases, but has not been studied in the context of haemophilic arthropathy. Aim We aim to compare synovial tissues histologically from haemophilia and osteoarthritis patients with advanced arthropathy in order to compare expression of proteins that are possibly aetiologic in the development of arthrofibrosis. Methods Human synovial tissues were obtained from 10 haemophilia and 10 osteoarthritis patients undergoing joint surgery and processed for histology and immunohistochemistry. Results All samples from haemophilia patients had synovitis with hypertrophy and hyperplasia of synovial villi. Histologically, synovial tissues contained hyperplastic villi with increased cellularity and abundant haemosiderin- and ferritin-pigmented macrophage-like cells ( HMCs), with a perivascular localization in the sub-surface layer. CTGF staining was observed in the surface layer and sub-surface layer in all haemophilia patients, exclusively co-localizing with HMCs. Quantification showed that the extent of CTGF-positive areas was correlated with the degree of detection of HMCs. CTGF was not observed in any of the samples from osteoarthritis patients. Conclusion Using histological analysis, we showed that CTGF expression is elevated in haemophilia patients with arthrofibrosis and absent in patients with osteoarthritis. Additionally, we found that CTGF is always associated with haemosiderin-pigmented macrophage-like cells, which suggests that CTGF is produced by synovial A cells following the uptake of blood breakdown products. To measure and compare the intraoral pH and temperature of individuals during sleep with and without mouth breathing. Ten healthy participants [mean age = 25·8 (± 4·3)] wore a custom-made appliance fitted with a pH probe and thermocouple for two sets of 48 h. Continuous pH and temperature measurements were taken from the palatal aspect of the upper central incisors. To simulate mouth breathing during sleep, participants wore a nose clip for two nights of the four, with the first group ( n = 5) wearing the nose clip during the first night and the rest ( n = 5) wearing the nose clip during the second night of sleep to balance any potential bias from the wearing sequence. Both qualitative and quantitative analyses were conducted. The mean intraoral pH during daytime was 7·3 (± 0·4) and during sleep was 7·0 (± 0·5). The mean intraoral pH during sleep with mouth breathing was 6·6 (± 0·5), which was statistically significant compared with the normal sleep condition ( P < 0·01). The intraoral pH decreased slowly over the hours of sleep in all participants. When sleeping with forced mouth breathing, intraoral pH showed a greater fall over a longer period of time. The mean intraoral temperature was 33·1 °C (± 5·2) during daytime and 33·3 °C (± 6·1) during sleep, with no statistical significance between sleep with and without mouth breathing ( P > 0·05). The results suggest that mouth breathing during sleep is related to a decrease in intraoral pH compared with normal breathing during sleep, and this has been proposed as a causal factor for dental erosion and caries. The Marine Institute, Ireland, runs a suite of operational regional and coastal ocean models. Recent developments include several tailored products that focus on the key needs of the Irish aquaculture sector. In this article, an overview of the products and services derived from the models are presented. The authors give an overview of a shellfish model developed in-house and that was designed to predict the growth, the physiological interactions with the ecosystem, and the level of coliform contamination of the blue mussel. As such, this model is applicable in studies on the carrying capacity of embayments, assessment of the impacts of pollution on aquaculture grounds, and the determination of shellfish water classes. Further services include the assimilation of the model-predicted shelf water movement into a new harmful algal bloom alert system used to inform end users of potential toxic shellfish events and high biomass blooms that include fish-killing species. Models are also used to identify potential sites for offshore aquaculture, to inform studies of potential cross-contamination in farms from the dispersal of planktonic sea lice larvae and other pathogens that can infect finfish, and to provide modelled products that underpin the assessment and advisory services on the sustainable exploitation of the resources of marine fisheries. To describe a novel approach for continuous measurement of intra-oral p H and temperature in individuals carrying out normal daily activities over 24 h. We designed, validated and constructed a custom-made appliance fitted with a p H probe and a thermocouple. Six subjects wore the appliance over a 24-h period for two non-consecutive days, while the intra-oral p H and temperature were measured continuously and recorded. Intra-oral p H and temperature were very similar across different recording days, the difference being not statistically significant ( P ≥ 0·14). There was a noticeable difference in the pattern of variation of p H between day and night. During the day, the mean p H was 7·3 (±0·4) and dropped markedly only after consumption of acidic food and drinks. The intra-oral p H decreased slowly during sleep with an average p H of 6·6 (±0·4) being recorded. The difference between day and night was statistically significant ( P = 0·002). The mean intra-oral temperature was 33·9 °C (±0·9) during daytime and 35·9 °C (±0·5) during sleep ( P = 0·013) with minor fluctuations occurring over 24 h. The continuous and simultaneous intra-oral p H and temperature measurement system described in this report is reliable, easy to construct, able to measure variables over a sustained period and may serve as a future diagnostic tool in a number of applications. In June 2013, a record-breaking female Isurus oxyrinchus (total length 373 cm, mass 600 kg) was captured by rod and reel off Huntington Beach, California, where it was subsequently donated to research and provided a rare opportunity to collect the first data for a female I. oxyrinchus of this size. Counts of vertebral band pairs estimate the shark to have been c. 22 years old, depending upon assumptions of band-pair deposition rates, and the distended uteri and spent ovaries indicated that this shark had recently given birth. The stomach contained a c. 4 year-old female California sea lion Zalophus californianus that confirmed the high trophic position of this large I. oxyrinchus, which was corroborated with the high levels of measured contaminants and tissue isotope analyses. The Marine Institute, Ireland, runs a suite of operational regional and coastal ocean models. Recent developments include several tailored products that focus on the key needs of the Irish aquaculture sector. In this article, an overview of the products and services derived from the models are presented. A shellfish model that includes growth and physiological interactions of mussels with the ecosystem and is fully embedded in the 3-D numerical modelling framework has been developed at the Marine Institute. This shellfish model has a microbial module designed to predict levels of coliform contamination in mussels. This model can also be used to estimate the carrying capacity of embayments, assess impacts of pollution on aquaculture grounds and help to classify shellfish waters. The physical coastal model of southwest Ireland provides a three day forecast of shelf water movement in the region. The aim of this study was to examine the synthesis of sulfated glycosaminoglycans during normal healing of experimental acetic acid-induced gastric ulcer in rats and to investigate the effect of indomethacin, a drug known to delay ulcer healing, on this synthesis using an in vivo labelling system. Analysis revealed the presence of two major sulfated species in control tissue; a population of sulfated mucins and glycosaminoglycans, predominantly galactosaminoglycans. The incorporation of [35S]sulfate label into glycosaminoglycans synthesized in the granulation tissue of healing ulcers increased significantly (P < 0.05) as compared to day 0 and control levels at day 14. Treatment of animals with indomethacin (1 mg/kg daily) resulted in a further significant (P < 0.01) rise in sulfated glycosaminoglycan synthesis in indomethacin-treated ulcer tissue compared to that found in healing ulcers at day 14. The increased glycosaminoglycan synthesis was due to increased levels of chondroitin sulfate and dermatan sulfate. Glycosaminoglycan synthesis is elevated at the ulcer site during healing of experimental gastric ulcers; however, indomethacin treatment, which delays ulcer healing, significantly increases the synthesis of glycosaminoglycans above that seen in healing ulcers. Changes in the sulfated glycosaminoglycan content of the ulcer may play a role in the healing process and may give further insight into the mechanisms by which indomethacin delays ulcer healing. Interactions between Candida albicans, saliva and saliva-coated oral surfaces are initial events in the colonization of the oral cavity by this commensal yeast, which can cause oral diseases such as candidiasis and denture stomatitis. Candida albicans also colonizes silicone voice prostheses, and the microbial biofilm formed can impair valve function, necessitating frequent prosthesis replacement. We have previously shown that saliva promoted binding of C. albicans cells to silicone in vitro, and that the selective binding of specific salivary proteins to voice prosthesis silicone mediated attachment of C. albicans cells. The C. albicans cells adhered to a polypeptide (or polypeptides) of ~36 kDa eluted from saliva-treated silicone. We show here that a protein of similar size was identified in replicate blots of the eluate from saliva-treated silicone when the blots were probed with antibodies to human SPLUNC2, a salivary protein with reported microbial agglutination properties. In addition, SPLUNC2 was depleted from saliva that had been incubated with silicone coupons. To determine whether SPLUNC2 is a yeast-binding protein, SPLUNC2 cDNA was expressed in Escherichia coli. Purified recombinant His-tagged protein (SPLUNC2r) bound to silicone as demonstrated by immunoblot analysis of an eluate from SPLUNC2r-treated silicone coupons and (35) S-radiolabelled C. albicans cells adhered in a dose-dependent manner to SPLUNC2r-coated silicone. We conclude that SPLUNC2 binds to silicone and acts as a receptor for C. albicans adherence to, and subsequent colonization of, voice prosthesis silicone. [ABSTRACT FROM AUTHOR]

ENSURF (Ensemble SURge Forecast) is a multimodel application for sea level forecast that makes use of several storm surge or circulation models and near-real time tide gauge data in the region, with the following main goals: 1. providing easy access to existing forecasts, as well as to its performance and model validation, by means of an adequate visualization tool; 2. generation of better forecasts of sea level, including confidence intervals, by means of the Bayesian Model Average technique (BMA). The Bayesian Model Average technique generates an overall forecast probability density function (PDF) by making a weighted average of the individual forecasts PDF's; the weights represent the Bayesian likelihood that a model will give the correct forecast and are continuously updated based on the performance of the models during a recent training period. This implies the technique needs the availability of sea level data from tide gauges in near-real time. The system was implemented for the European Atlantic facade (IBIROOS region) and Western Mediterranean coast based on the MATROOS visualization tool developed by Deltares. Results of validation of the different models and BMA implementation for the main harbours are presented for these regions where this kind of activity is performed for the first time. The system is currently operational at Puertos del Estado and has proved to be useful in the detection of calibration problems in some of the circulation models, in the identification of the systematic differences between baroclinic and barotropic models for sea level forecasts and to demonstrate the feasibility of providing an overall probabilistic forecast, based on the BMA method.
