Background: Growing evidence suggests that marrow adipocytes play an active role in the regulation of bone metabolism and hematopoiesis. However, research on the relationship between bone and fat in the context of hematological diseases, particularly β‐thalassemia, remains limited. Purpose: To investigate the relationship between marrow fat and cortical bone thickness in β‐thalassemia and to identify key determinants influencing these variables. Study Type: Prospective. Subjects: Thirty‐five subjects in four subject groups of increasing disease severity: 6 healthy control (25.0 ± 5.3 years, 2 male), 4 β‐thalassemia minor, 13 intermedia, and 12 major (29.1 ± 6.4 years, 15 male). Field Strength/Sequence: 3.0 T, 3D fast low angle shot sequence and T1‐weighted turbo spin echo. Assessment: Analyses on proton density fat fraction (PDFF) and R2* values in femur subregions (femoral head, greater trochanter, intertrochanteric, diaphysis, distal) and cortical thickness (CBI) of the subjects' left femur. Clinical data such as age, sex, body mass index (BMI), and disease severity were also included. Statistical Tests: One‐way analysis of variance (ANOVA), mixed ANOVA, Pearson correlation and multiple regression. P‐values <0.05 were considered significant. Results: Bone marrow PDFF significantly varied between the femur subregions, F(2.89,89.63) = 44.185 and disease severity, F(1,3) = 12.357. A significant interaction between subject groups and femur subregions on bone marrow PDFF was observed, F(8.67,89.63) = 3.723. Notably, a moderate positive correlation was observed between PDFF and CBI (r = 0.33–0.45). Multiple regression models for both PDFF (R2 = 0.476, F(13,151) = 10.547) and CBI (R2 = 0.477, F(13,151) = 10.580) were significant. Significant predictors for PDFF were disease severity (βTMi = 0.36, βTMa = 0.17), CBI (β = 0.24), R2* (β = −0.32), and height (β = −0.29) while for CBI, the significant determinants were sex (β = −0.27), BMI (β = 0.55), disease severity (βTMi = 2.15), and PDFF (β = 0.25). Data Conclusion: This study revealed a positive correlation between bone marrow fat fraction and cortical bone thickness in β‐thalassemia with varying disease severity, potentially indicating a complex interplay between bone health and marrow composition. Even though COVID-19 as a global crisis is now declared over, Catholics' desire to undertake spiritual pilgrimages has not returned to pre-pandemic levels. This is a key issue that operators of Marian tourism destinations in Indonesia currently face. This study aims to help managers of Maria Cave pilgrimage destinations develop a sustainable Marian pilgrimage, particularly in Indonesia. The findings can provide valuable insights that can inform strategies to revitalise and sustain the Marian religious tourism sector in Indonesia post-COVID. Employing the case study method, the findings indicate that long-term development is critical for creating a sustainable Marian pilgrimage climate. Some of the key elements of the long-term development include pilgrimage destination managers being aware of pilgrims' needs and desires post-pandemic. Managers of tourist destinations too can no longer ignore pilgrims' demands for a safe environment free from health risks. Further, they must also take into account and address any negative or damaging aspects of these destinations. Objectives: Accurate subtyping of breast cancer is crucial for its diagnosis, management, and prognostication. This study aimed to determine the association of magnetic resonance imaging (MRI) breast features with the molecular subtype and aggressiveness of breast cancer in a multi-ethnic population. Methods: Treatment-naive patients with invasive breast carcinoma were included in this retrospective study. Breast MRI features were recorded based on the American College of Radiology-Breast Imaging Reporting and Data System (ACR-BIRADS) criteria, with tumour size, and apparent diffusion coefficient value (ADC). The statistical association was tested with Pearson Chi-Square Test of Independence for categorical data or the Kruskal-Wallis/ Mann Whitney U test for numerical data between the MRI features and molecular subtype, receptor status, tumour grade, lymphovascular infiltration (LVI) and axillary lymph node (ALN). Multinomial logistic regression was used to test the predictive likelihood of the significant features. The breast cancer subtypes were determined via immunohistochemistry (IHC) and dual-color dual-hapten in-situ hybridization (D-DISH). The expression statuses of ER, PR, and HER-2, LVI, and ALN were obtained from the histopathology report. The ER / PR / HER-2 was evaluated according to the American Society of Clinical Oncology / College of American Pathologists. Results: The study included 194 patients; 41.8% (n = 81) Chinese, 40.7% (n = 79) Malay, and 17.5% (n = 34) Indian, involving 71.6%(n = 139) luminal-like, 12.9%(n = 25) HER-2 enriched, and 15.5%(n = 30) Triple-negative breast cancer (TNBC). TNBC was associated with rim enhancement (p = 0.002) and peritumoral oedema (p = 0.004). HER-2 enriched tumour was associated with larger tumour size (p = 0.041). Luminal-like cancer was associated with irregular shape (p = 0.005) with circumscribed margin (p = 0.003). Other associations were ER-negative tumour with circumscribed margin (p = 0.002) and PR-negative with round shape (p = 0.001). Tumour sizes were larger in ER-negative (p = 0.044) and PR-negative (p = 0.022). Rim enhancement was significantly associated with higher grade (p = 0.001), and moderate peritumoral oedema with positive axillary lymph node (p = 0.002). Conclusion: Certain MRI features can be applied to differentiate breast cancer molecular subtypes, receptor status and aggressiveness, even in a multi-ethnic population. [ABSTRACT FROM AUTHOR]

Chronic kidney disease is a leading public health problem worldwide. The global prevalence of chronic kidney disease is nearly five hundred million people, with almost one million deaths worldwide. Estimated glomerular filtration rate, imaging such as conventional ultrasound, and histopathological findings are necessary as each technique provides specific information which, when taken together, may help to detect and arrest the development of chronic kidney disease, besides managing its adverse outcomes. However, estimated glomerular filtration rate measurements are hampered by substantial error margins while conventional ultrasound involves subjective assessment. Although histopathological assessment is the best tool for evaluating the severity of the renal pathology, it may lead to renal insufficiency and haemorrhage if complications occurred. Ultrasound shear wave elastography, an emerging imaging that quantifies tissue stiffness non-invasively has gained interest recently. This method applies acoustic force pulses to generate shear wave within the tissue that propagate perpendicular to the main ultrasound beam. By measuring the speed of shear wave propagation, the tissue stiffness is estimated. This paper reviews the literature and presents our combined experience and knowledge in renal shear wave elastography research. It discusses and highlights the confounding factors on shear wave elastography, current and future possibilities in ultrasound renal imaging and is not limited to new sophisticated techniques. Many studies have investigated the dielectric properties of human and animal tissues, particularly to differentiate between normal cells and tumors. However, these studies are invasive as tissue samples have to be excised to measure the properties. This study aims to investigate the dielectric properties of urine in relation to bladder cancer, which is safe and non-invasive to patients. 30 healthy subjects and 30 bladder cancer patients were recruited. Their urine samples were subjected to urinalysis and cytology assessment. A vector network analyzer was used to measure the dielectric constant (Ɛ′) and loss factor (Ɛ″) at microwave frequencies of between 0.2 and 50 GHz at 25 °C, 30 °C and 37 °C. Significant differences in Ɛ′ and Ɛ″ were observed between healthy subjects and patients, especially at frequencies of between 25 and 40 GHz at 25 °C. Bladder cancer patients had significant lower Ɛ′ and higher Ɛ″ compared with healthy subjects. The Ɛ′ was negatively correlated with urinary exfoliated urothelial cell number, and Ɛ″ was positively correlated. The study achieved a receiver operating characteristic area under curve (ROC–AUC) score of 0.69099 and an optimum accuracy of 75% with a sensitivity of 80% and a specificity of 70%. The number of exfoliated urothelial cell had significant effect on the dielectric properties, especially in bladder cancer patients. TMP is gradually becoming a fundamental element for quality assurance and control in ionizing and non-ionizing radiation imaging modalities as well as in the development of different techniques. This study aimed to evaluate and obtain polyvinyl chloride tissue mimicking material for dual-modality breast phantoms in mammography and ultrasound. Breast tissue equivalence was evaluated based on X-ray attenuation properties, speed of sound, attenuation, and acoustic impedance. There are six samples of PVC-plasticizer material with variations of PVC concentration and additives. The evaluation of X-ray attenuation was carried out using mammography from 23 to 35 kV, while the acoustic properties were assessed with mode A ultrasound and a transducer frequency of 5 MHz. A breast phantom was created from TMP material with tissue equivalence and was then evaluated using mammography as well as ultrasound to analyze its image quality. The results showed that samples A (PVC 5%, DOP 95%), B (PVC 7%, DOP 93%), C (PVC 10%, DOP 90%), E (PVC 7%, DOP 90%, graphite 3%), and F (PVC 7%, DOP 90%, silicone oil 3%) have the closest equivalent to the ACR breast phantom material with a different range of 0.01–1.39 in the 23–35 kV range. Based on the evaluation of the acoustic properties of ultrasound, A had high similarity to fat tissue with a difference of 0.03 (dB cm− 1 MHz− 1) and 0.07 (106 kg m− 2 s− 1), while B was close to the glandular tissue with a difference of 9.2 m s− 1. Multilayer breast phantom images' results showed gray levels in mammography and ultrasound modalities. Therefore, this study succeeded in establishing TMP material for mammography and ultrasound. Purpose: Monte Carlo (MC) simulation is an important technique that can help design advanced and challenging experimental setups. GATE (Geant4 application for tomographic emission) is a useful simulation toolkit for applications in nuclear medicine. Transarterial radioembolization is a treatment for liver cancer, where microspheres embedded with yttrium‐90 (90Y) are administered intra‐arterially to the tumor. Personalized dosimetry for this treatment may provide higher dosimetry accuracy compared to the conventional partition model (PM) calculation. However, incorporation of three‐dimensional tomographic input data into MC simulation is an intricate process. In this article, 3D Slicer, free and open‐source software, was utilized for the incorporation of patient tomographic images into GATE to demonstrate the feasibility of personalized dosimetry in hepatic radioembolization with 90Y. Methods: In this article, the steps involved in importing, segmenting, and registering tomographic images using 3D Slicer were thoroughly described, before importing them into GATE for MC simulation. The absorbed doses estimated using GATE were then compared with that of PM. SlicerRT, a 3D Slicer extension, was then used to visualize the isodose from the MC simulation. Results: A workflow diagram consisting of all the steps taken in the utilization of 3D Slicer for personalized dosimetry in 90Y radioembolization has been presented in this article. In comparison to the MC simulation, the absorbed doses to the tumor and normal liver were overestimated by PM by 105.55% and 20.23%, respectively, whereas for lungs, the absorbed dose estimated by PM was underestimated by 25.32%. These values were supported by the isodose distribution obtained via SlicerRT, suggesting the presence of beta particles outside the volumes of interest. These findings demonstrate the importance of personalized dosimetry for a more accurate absorbed dose estimation compared to PM. Conclusion: The methodology provided in this study can assist users (especially students or researchers who are new to MC simulation) in navigating intricate steps required in the importation of tomographic data for MC simulation. Radiomics is the process of extracting useful quantitative features of high-dimensional data that allows for automated disease classification, including atherosclerotic disease. Hence, this study aimed to quantify and extract the radiomic features from Coronary Computed Tomography Angiography (CCTA) images and to evaluate the performance of automated machine learning (AutoML) model in classifying the atherosclerotic plaques. In total, 202 patients who underwent CCTA examination at Institut Jantung Negara (IJN) between September 2020 and May 2021 were selected as they met the inclusion criteria. Three primary coronary arteries were segmented on axial sectional images, yielding a total of 606 volume of interest (VOI). Subsequently, the first order, second order, and shape order of radiomic characteristics were extracted for each VOI. Model 1, Model 2, Model 3, and Model 4 were constructed using AutoML-based Tree-Pipeline Optimization Tools (TPOT). The heatmap confusion matrix, recall (sensitivity), precision (PPV), F1 score, accuracy, receiver operating characteristic (ROC), and area under the curve (AUC) were analysed. Notably, Model 1 with the first-order features showed superior performance in classifying the normal coronary arteries (F1 score: 0.88; Inverse F1 score: 0.94), as well as in classifying the calcified (F1 score: 0.78; Inverse F1 score: 0.91) and mixed plaques (F1 score: 0.76; Inverse F1 score: 0.86). Moreover, Model 2 consisting of second-order features was proved useful, specifically in classifying the non-calcified plaques (F1 score: 0.63; Inverse F1 score: 0.92) which are a key point for prediction of cardiac events. Nevertheless, Model 3 comprising the shape-based features did not contribute to the classification of atherosclerotic plaques. Purpose Accurate and early detection of breast cancer using effective imaging modalities is an active area of research in medical image analysis. Computeraided diagnosis (CAD) of breast cancer using digital mammograms may help in early diagnosis and can assist in maintaining patient health. The breast imaging reporting and data system (BIRADS) is widely used for risk assessment and classification grading in breast cancer screening. It contains seven different grading systems for breast cancer risk assessment. These range from grade 0 (incomplete) to grade 6 (proven malignancy). All other intermediate stages state the progression of risk. Methods: The current research results have shown that two‐dimensional synthesized mammogram (2DSM) imaging and conventional full‐field digital mammography (FFDM) are two important imaging modalities which can be used for screening breast cancer. To the best of our knowledge, there is no study which has yet compared the BIRADS discrimination power of 2DSM and FFDM imaging modalities. In this paper we present a novel CAD‐based comparative study, using texton and gist for the characterization of breast cancer with 2DSM and FFDM imagery. Results: The developed method achieved an average performance of 92.9% accuracy using a probabilistic neural network classifier for FFDM images with tenfold crossvalidation. Hence, our proposed model showed that FFDM images are more effective than the 2DSM imaging modality in discriminating BIRADS grades. Conclusion: The obtained results confirmed that our method performed well in the early detection of breast cancer. Consequently, it can be used as a distinct system in rural hospitals. Artificial intelligence (AI) is an innovative tool with the potential to impact medical physicists' clinical practices, research, and the profession. The relevance of AI and its impact on the clinical practice and routine of professionals in medical physics were evaluated by medical physicists and researchers in this field. An online survey questionnaire was designed for distribution to professionals and students in medical physics around the world. In addition to demographics questions, we surveyed opinions on the role of AI in medical physicists' practices, the possibility of AI threatening/disrupting the medical physicists' practices and career, the need for medical physicists to acquire knowledge on AI, and the need for teaching AI in postgraduate medical physics programmes. The level of knowledge of medical physicists on AI was also consulted. A total of 1019 respondents from 94 countries participated. More than 85% of the respondents agreed that AI would play an essential role in medical physicists' practices. AI should be taught in the postgraduate medical physics programmes, and that more applications such as quality control (QC), treatment planning would be performed by AI. Half of the respondents thought AI would not threaten/disrupt the medical physicists' practices. AI knowledge was mainly acquired through self-taught and work-related activities. Nonetheless, many (40%) reported that they have no skill in AI. The general perception of medical physicists was that AI is here to stay, influencing our practices. Summary: More than half of the world's population live in Asia‐Pacific. This region is culturally diverse, with significant disparities in terms of socio‐economic status, provision of health care and access to advanced technology. The medical use of ionising radiation is increasing worldwide and similarly within the Asia‐Pacific region. In this paper, we highlight the current status in usage of ionising radiation in medicine in the region, and review the legal framework, implementation and activities in radiation protection. Asia‐Pacific countries are active in strengthening radiation protection by promoting education and training. Various projects and activities initiated by international organisations such as the IAEA, WHO and ICRP have provided stimulation in the region, but more work is needed to continue to improve protection practices. In 2020 the world is facing unprecedented challenges due to COVID‐19. To address these challenges, many digital tools are being explored and developed to contain the spread of the disease. With the lack of availability of vaccines, there is an urgent need to avert resurgence of infections by putting some measures, such as contact tracing, in place. While digital tools, such as phone applications are advantageous, they also pose challenges and have limitations (eg, wireless coverage could be an issue in some cases). On the other hand, wearable devices, when coupled with the Internet of Things (IoT), are expected to influence lifestyle and healthcare directly, and they may be useful for health monitoring during the global pandemic and beyond. In this work, we conduct a literature review of contact tracing methods and applications. Based on the literature review, we found limitations in gathering health data, such as insufficient network coverage. To address these shortcomings, we propose a novel intelligent tool that will be useful for contact tracing and prediction of COVID‐19 clusters. The solution comprises a phone application combined with a wearable device, infused with unique intelligent IoT features (complex data analysis and intelligent data visualization) embedded within the system to aid in COVID‐19 analysis. Contact tracing applications must establish data collection and data interpretation. Intelligent data interpretation can assist epidemiological scientists in anticipating clusters, and can enable them to take necessary action in improving public health management. Featured Application: We evaluate and propose a method for evaluating optimized CT protocol for pediatric-based figure of merit and image performance metrics. The aim of this study is to investigate the impact of CT acquisition parameter setting on organ dose and its influence on image quality metrics in pediatric phantom during CT examination. The study was performed on 64-slice multidetector CT scanner (MDCT) Siemens Definition AS (Siemens Sector Healthcare, Forchheim, Germany) using various CT CAP protocols (P1–P9). Tube potential for P1, P2, and P3 protocols were fixed at 100 kVp while P4, P5, and P6 were fixed at 80 kVp with used of various reference noise values. P7, P8, and P9 were the modification of P1 with changes on slice collimation, pitch factor, and tube current modulation (TCM), respectively. TLD-100 chips were inserted into the phantom slab number 7, 9, 10, 12, 13, and 14 to represent thyroid, lung, liver, stomach, gonads, and skin, respectively. The image quality metrics, signal to noise ratio (SNR) and contrast to noise ratio (CNR) values were obtained from the CT console. As a result, this study indicates a potential reduction in the absorbed dose up to 20% to 50% along with reducing tube voltage, tube current, and increasing the slice collimation. Papillary breast lesions include a wide spectrum of pathologies ranging from benign to malignant. The word papillary originates from finger-like projections, or papules, which are seen when these lesions are projected under a microscope. Papillary breast lesions have an array of radiological features at presentations; hence differentiation between benign and malignant based on imaging features is challenging. Histopathological diagnosis is crucial for the distinction and further management of the lesions. Traditionally, tumor and ductal excision is the treatment of choice for malignant and atypical or benign papilloma with imaging discordance. However, current clinical practice guidance advocates complete surgical excision, even for asymptomatic and purely benign papillomas diagnosed on core needle biopsy, as they are highly associated with atypia and malignant upstage on subsequent surgery. Computer aided diagnosis (CAD) is a non-invasive method of diagnosing medical signals/images using advanced image processing followed by soft computing techniques. In this study, we have developed a non-invasive CAD system for differentiating benign versus malignant papillary breast lesions using bi-dimensional empirical mode decomposition (BEMD) and the discrete cosine transform (DCT) followed by locality sensitive discriminant analysis (LSDA). The developed model is validated using a large collection of ultrasound images of papillary breast lesions, and achieved a maximum performance of 98.63% accuracy. We have also developed a breast papillary index, which may in the future be used as a substitute for the conventional soft computing techniques. This study objectively evaluates the similarity between standard full-field digital mammograms and two-dimensional synthesized digital mammograms (2DSM) in a cohort of women undergoing mammography. Under an institutional review board–approved data collection protocol, we retrospectively analyzed 407 women with digital breast tomosynthesis (DBT) and full-field digital mammography (FFDM) examinations performed from September 1, 2014, through February 29, 2016. Both FFDM and 2DSM images were used for the analysis, and 3216 available craniocaudal (CC) and mediolateral oblique (MLO) view mammograms altogether were included in the dataset. We analyzed the mammograms using a fully automated algorithm that computes 152 structural similarity, texture, and mammographic density–based features. We trained and developed two different global mammographic image feature analysis–based breast cancer detection schemes for 2DSM and FFDM images, respectively. The highest structural similarity features were obtained on the coarse Weber Local Descriptor differential excitation texture feature component computed on the CC view images (0.8770) and MLO view images (0.8889). Although the coarse structures are similar, the global mammographic image feature–based cancer detection scheme trained on 2DSM images outperformed the corresponding scheme trained on FFDM images, with area under a receiver operating characteristic curve (AUC) = 0.878 ± 0.034 and 0.756 ± 0.052, respectively. Consequently, further investigation is required to examine whether DBT can replace FFDM as a standalone technique, especially for the development of automated objective-based methods. Objectives: To propose a novel robust radiogenomics approach to the identification of epidermal growth factor receptor (EGFR) mutations among patients with non-small cell lung cancer (NSCLC) using Betti numbers (BNs). Materials and methods: Contrast enhanced computed tomography (CT) images of 194 multi-racial NSCLC patients (79 EGFR mutants and 115 wildtypes) were collected from three different countries using 5 manufacturers' scanners with a variety of scanning parameters. Ninety-nine cases obtained from the University of Malaya Medical Centre (UMMC) in Malaysia were used for training and validation procedures. Forty-one cases collected from the Kyushu University Hospital (KUH) in Japan and fifty-four cases obtained from The Cancer Imaging Archive (TCIA) in America were used for a test procedure. Radiomic features were obtained from BN maps, which represent topologically invariant heterogeneous characteristics of lung cancer on CT images, by applying histogram- and texture-based feature computations. A BN-based signature was determined using support vector machine (SVM) models with the best combination of features that maximized a robustness index (RI) which defined a higher total area under receiver operating characteristics curves (AUCs) and lower difference of AUCs between the training and the validation. The SVM model was built using the signature and optimized in a five-fold cross validation. The BN-based model was compared to conventional original image (OI)- and wavelet-decomposition (WD)-based models with respect to the RI between the validation and the test. Results: The BN-based model showed a higher RI of 1.51 compared with the models based on the OI (RI: 1.33) and the WD (RI: 1.29). Conclusion: The proposed model showed higher robustness than the conventional models in the identification of EGFR mutations among NSCLC patients. Background: β‐thalassemia is a genetic disease that causes abnormal production of red blood cells (ineffective erythropoiesis, IE). IE is a condition known to change bone marrow composition. Purpose: To evaluate the effect of IE on the marrow fat content and fat unsaturation levels in the proximal femur using 1H‐MRS. Study Type: Prospective. Subjects: Twenty‐three subjects were included in this study, seven control and 16 β‐thalassemia subjects. Field Strength/Sequence: 3.0T; stimulated echo acquisition Mode (STEAM); magnetic resonance spectroscopy (MRS) sequence. Assessment: Multiecho MRS scans were performed in four regions of the proximal left femur of each subject, that is, diaphysis, femoral neck, femoral head, and greater trochanter. The examined regions were grouped into red (diaphysis and femoral neck) and yellow marrow regions (femoral head and greater trochanter). Statistical Tests: The Jonckheere–Terpstra test was used to evaluate the impact of increasing disease severity on bone marrow fat fraction (BMFF), marrow conversion index, and fat unsaturation index (UI). Pairwise comparison analysis was performed when a significant trend (P < 0.05) was found. K‐means clustering analysis was used to examine the clusters observed when BMFF in the red and yellow regions were studied (diaphysis against greater trochanter). Results: BMFF showed a significant decreasing trend with increasing disease severity in both red (TJT = 109.00, z = –4.414, P < 0.05) and yellow marrow regions (TJT = 108.00, z = –4.438, P < 0.05). The opposite trend was observed in UI in both bone marrow regions (red marrow: TJT = 180.5, z = 3.515, P < 0.05; yellow marrow: TJT = 155.0, z = 2.282, P = 0.05). Three distinct forms of marrow adipogenesis were found when plotting BMFF diaphysis against BMFF greater trochanter: 1) normal (centroid: 80.4%, 66.6%), 2) partial disruption (centroid: 51.1%, 16.6%), and 3) total disruption (centroid: 2.6%, 1.6%). Aim: Renal biopsy is the gold standard for the histological characterization of chronic kidney disease (CKD), of which renal fibrosis is a dominant component, affecting its stiffness. The aim of this study was to investigate the correlation between kidney stiffness obtained by shear wave elastography (SWE) and renal histological fibrosis. Methods: Shear wave elastography assessments were performed in 75 CKD patients who underwent renal biopsy. The SWE‐derived estimates of the tissue Young's modulus (YM), given as kilopascals (kPa), were measured. YM was correlated to patients' renal histological scores, broadly categorized into glomerular, tubulointerstitial and vascular scores. Results: Young's modulus correlates significantly with tubulointerstitial score (ρ = 0.442, P <.001) and glomerular score (ρ = 0.375, P =.001). Patients with no glomerular sclerosis showed lower mean YM measurements compared to those with glomerular sclerosis. The mean YM increased as the percentage of interstitial fibrosis and tubular atrophy increased. The area under the receiver operating characteristic curve (ROC) for SWE in differentiating between mildly and moderately impaired kidneys was 0.702. Conclusion: Shear wave elastography accurately detects chronic renal damage resulting from glomerular sclerosis, interstitial fibrosis and tubular atrophy, using the optimal cut‐off YM value of ≥5.81 kPa. Oncoplastic breast surgery (OBS) improves margin clearance and produces good esthetic outcome in breast cancer treatment. This study evaluates the complications and outcome of OBS in a multiracial patient cohort. Data of 421 patients between 2011 and 2018 were analyzed. The majority were Malays (41.8%), followed by Chinese (39.7%) and Indians (16.8%). Low local complications were noted, with no significant differences in disease‐free survival (P =.927) and overall survival (P =.719) between low and high OBS levels. Chronic kidney disease (CKD) is a continuing loss of kidney function, and early detection of this disease is fundamental to halting its progression to end-stage disease. Numerous methods have been proposed to detect CKD, mainly focusing on classification based upon peripheral clinical parameters and quantitative ultrasound parameters that must be manually calculated, or on shear wave elastography. No studies have been found that detect the presence or absence of CKD based solely from one B-mode ultrasound image. In this work, we propose an automated system to detect chronic kidney disease utilizing only the automatic extraction of features from a B-mode ultrasound image of the kidney, with a database of 405 images. Higher-order bispectrum and cumulants, and elongated quinary patterns, are extracted from each image to provide a final total of 24,480 features per image. These features were subjected to a locality sensitive discriminant analysis (LSDA) technique, which provides 30 LSDA coefficients. The coefficients were arranged according to their t value and inserted into various classifiers, to yield the best diagnostic accuracy using the least number of features. The best performance was obtained using a support vector machine and a radial basis function, utilizing only five features, resulting in an accuracy of 99.75%, a sensitivity of 100%, and a specificity of 99.57%. Purpose: Lung nodules have very diverse shapes and sizes, which makes classifying them as benign/malignant a challenging problem. In this paper, we propose a novel method to predict the malignancy of nodules that have the capability to analyze the shape and size of a nodule using a global feature extractor, as well as the density and structure of the nodule using a local feature extractor. Methods: We propose to use Residual Blocks with a 3 × 3 kernel size for local feature extraction and Non-Local Blocks to extract the global features. The Non-Local Block has the ability to extract global features without using a huge number of parameters. The key idea behind the Non-Local Block is to apply matrix multiplications between features on the same feature maps. Results: We trained and validated the proposed method on the LIDC-IDRI dataset which contains 1018 computed tomography scans. We followed a rigorous procedure for experimental setup, namely tenfold cross-validation, and ignored the nodules that had been annotated by < 3 radiologists. The proposed method achieved state-of-the-art results with AUC = 95.62%, while significantly outperforming other baseline methods. Conclusions: Our proposed deep Local–Global network has the capability to accurately extract both local and global features. A disaster is a crisis situation that causes significant harm to humans, the environment, the economy, and impacts upon society's ability to cope both during the event and with the aftermath. A disaster may escalate rapidly to reach unpredicted consequences. They can be naturally occurring, or man-made, or a combination of the two. In this study we have focused on the Fukushima Daiichi nuclear power plant accident of 2011 that subsequently unleashed a floodgate of scientific activities, including basic and applied research, technology development, policy reform, inter-disciplinary collaboration, citizen science and re-strategization of research networks. In this context, a bibliometric study has been conducted on 5455 publications on the Fukushima disaster indexed by Scopus for the 2011–2017 period. The aim is to identify key papers in the field, the most substantive contributors to the literature, and to begin to map out the emergence of a body of knowledge resulting from the disaster. The findings present main national and international collaborators on Fukushima disaster related research, as well as an indication of the major research themes evolving over the immediate years following the event. This article investigated and compared the dielectric properties of urine between pregnant women and non-pregnant women. Urine samples were collected from 30 pregnant women and 30 non-pregnant women. Dielectric properties of the urine were measured between 0.2 and 50 GHz at temperatures of 25, 30 and 37 °C. The composition of urine and human chorionic gonadotropin (hCG) were also determined accordingly. The urine of pregnant women was observed to have statistically significant lower dielectric constant than that of non-pregnant women at three temperatures and at certain frequencies (p < .05, mean relative error <0.48%). Loss factor (p < .05, mean relative error < .56%) and loss tangent (p < .05, mean relative error <0.71%) were statistically significant higher in pregnant women than that in non-pregnant women at three temperatures and at certain frequencies. The dielectric properties decreased with temperature before the crossing point while they increased with temperature after the crossing point. In general, the dielectric properties correlated positively with hCG at low frequencies but they correlated negatively with hCG at high frequencies. The correlation coefficients of more than ±0.85 indicated strong correlation, which suggested potential development of a diagnostic tool for pregnancy using the dielectric properties of urine. Automation of diagnosis process in medical imaging using various computer-aided techniques is a leading topic of research. Among many computer-aided methods, nonlinear entropies are widely applied in the development of automated algorithms to diagnose abnormalities present in medical images. The use of entropy features in development of Computer-Aided Diagnosis (CAD) may enhance the accuracy of the system. Entropy features depict the nonlinearity of images and thereby the presence of complexity in the images. Various types of entropies have been employed in medical image analysis for automated diagnosis of abnormalities present in the images. This paper focuses on the diverse types of entropies employed in the development of CAD systems for the diagnosis of abnormalities in the medical images. Objectives: To investigate: (1) the variability of mammographic compression parameters amongst Asian women; and (2) the effects of reducing compression force on image quality and mean glandular dose (MGD) in Asian women based on phantom study. Methods: We retrospectively collected 15818 raw digital mammograms from 3772 Asian women aged 35–80 years who underwent screening or diagnostic mammography between Jan 2012 and Dec 2014 at our center. The mammograms were processed using a volumetric breast density (VBD) measurement software (Volpara) to assess compression force, compression pressure, compressed breast thickness (CBT), breast volume, VBD and MGD against breast contact area. The effects of reducing compression force on image quality and MGD were also evaluated based on measurement obtained from 105 Asian women, as well as using the RMI156 Mammographic Accreditation Phantom and polymethyl methacrylate (PMMA) slabs. Results: Compression force, compression pressure, CBT, breast volume, VBD and MGD correlated significantly with breast contact area (p<0.0001). Compression parameters including compression force, compression pressure, CBT and breast contact area were widely variable between [relative standard deviation (RSD)≥21.0%] and within (p<0.0001) Asian women. The median compression force should be about 8.1 daN compared to the current 12.0 daN. Decreasing compression force from 12.0 daN to 9.0 daN increased CBT by 3.3±1.4 mm, MGD by 6.2–11.0%, and caused no significant effects on image quality (p>0.05). Conclusions: Force-standardized protocol led to widely variable compression parameters in Asian women. Objectives: This study aimed to measure the absorbed doses in selected organs for prospectively ECG-triggered coronary computed tomography angiography (CCTA) using five different generations CT scanners in a female adult anthropomorphic phantom and to estimate the effective dose (HE). Materials and Methods: Prospectively ECG-triggered CCTA was performed using five commercially available CT scanners: 64-detector-row single source CT (SSCT), 2 × 32-detector-row-dual source CT (DSCT), 2 × 64-detector-row DSCT and 320-detector-row SSCT scanners. Absorbed doses were measured in 34 organs using pre-calibrated optically stimulated luminescence dosimeters (OSLDs) placed inside a standard female adult anthropomorphic phantom. HE was calculated from the measured organ doses and compared to the HE derived from the air kerma-length product (PKL) using the conversion coefficient of 0.014 mSv∙mGy-1∙cm-1 for the chest region. Results: Both breasts and lungs received the highest radiation dose during CCTA examination. The highest HE was received from 2 × 32-detector-row DSCT scanner (6.06 ± 0.72 mSv), followed by 64-detector-row SSCT (5.60 ± 0.68 and 5.02 ± 0.73 mSv), 2 × 64-detector-row DSCT (1.88 ± 0.25 mSv) and 320-detector-row SSCT (1.34 ± 0.48 mSv) scanners. HE calculated from the measured organ doses were about 38 to 53% higher than the HE derived from the PKL-to-HE conversion factor. Conclusion: The radiation doses received from a prospectively ECG-triggered CCTA are relatively small and are depending on the scanner technology and imaging protocols. HE as low as 1.34 and 1.88 mSv can be achieved in prospectively ECG-triggered CCTA using 320-detector-row SSCT and 2 × 64-detector-row DSCT scanners. [ABSTRACT FROM AUTHOR]

Alleviating acute pain and providing pain relief are central to caring for surgical patients as pain can lead to many adverse medical consequences. This study aimed to explore patients' experience of pain and satisfaction with postoperative pain control. A cross-sectional survey was carried out among 107 respondents who had undergone abdominal surgery in the surgical ward of an urban hospital using the Revised American Pain Society's Patient Outcome and Satisfaction Survey Questionnaires ( APS- POQ- R). Data were analysed using descriptive statistics and chi-square test. Chi-square test showed significant association between race ( P = 0.038), education level ( P ≤ 0.001), previous operation status ( P = 0.032) and operation status ( P ≤ 0.001). Further analysis on nominal regression, association between dissatisfaction with factors of operation status (46.09 (95% CI 7.456, 284.947)) and previous operation status (13.38 (95% CI 1.39, 128.74)) was found to be significant. Moderate to high levels of pain intensity in the last 24 h after surgery, as well as moderate to high rates of pain-related interference with care activities were most reported. Pain still remains an issue among surgical patients, and effective pain management and health education are needed to manage pain more effectively after surgery. [ABSTRACT FROM AUTHOR]

Introduction: Inadequate oral care has been implicated in the development of aspiration pneumonia in frail geriatric patients and is a major cause of mortality, due to the colonisation of microbes in vulnerable patients. This type of pneumonia has been associated with an increase in respiratory pathogens in the oral cavity. The aim of this study was to evaluate the effects of chlorhexidine compared to routine oral care in edentulous geriatric inpatients.Methods: A double-blind, parallel-group randomised controlled trial was carried out. The intervention group received oral care with chlorhexidine 0.2%, while the control group received routine oral care with thymol. Nurses provided oral care with assigned solutions of 20 mL once daily over seven days. Oral cavity assessment using the Brief Oral Health Status Examination form was performed before each oral care procedure. Data on medication received and the subsequent development of aspiration pneumonia was recorded. An oral swab was performed on Day 7 to obtain specimens to test for colonisation.Results: The final sample consisted of 35 (control) and 43 (intervention) patients. Chlorhexidine was effective in reducing oral colonisation compared to routine oral care with thymol (p < 0.001). The risk of oral bacterial colonisation was nearly three times higher in the thymol group compared to the chlorhexidine group.Conclusion: The use of chlorhexidine 0.2% significantly reduced oral colonisation and is recommended as an easier and more cost-effective alternative for oral hygiene. Radio frequency (RF) exposure from the proliferation of telecommunication structures in Malaysia triggers concerns as growing numbers of people claim that RF emission is a health hazard. Effective risk communication is needed to address this misperception but most risk communication programmes fail because of public distrust. This is worrying as trust is a vital ingredient in communicating risk. As such, this study uses critical discourse studies and Candlin and Crichton's focal theme of trust framework to examine how the politician's discourse on RF exposure affects trust. The data is from four (4) face to- face semi-structured interviews with politicians as they are answerable to their constituents' protests. The findings reveal that politicians use blame avoidance and legitimization tactics to counter or reject the views of the 'other' in order to build trustworthy relationships with the local communities. Breast density is a strong predictor of the failure of mammography screening to detect breast cancer and is a strong predictor of the risk of developing breast cancer. The many imaging options that are now available for imaging dense breasts show great promise, but there is still the question of determining which women are "dense" and what imaging modality is suitable for individual women. To date, mammographic breast density has been classified according to the Breast Imaging-Reporting and Data System (BI-RADS) categories from visual assessment, but this is known to be very subjective. Despite many research reports, the authors believe there has been a lack of physics-led and evidence-based arguments about what breast density actually is, how it should be measured, and how it should be used. In this paper, the authors attempt to start correcting this situation by reviewing the history of breast density research and the debates generated by the advocacy movement. The authors review the development of breast density estimation from pattern analysis to area-based analysis, and the current automated volumetric breast density (VBD) analysis. This is followed by a discussion on seeking the ground truth of VBD and mapping volumetric methods to BI-RADS density categories. The authors expect great improvement in VBD measurements that will satisfy the needs of radiologists, epidemiologists, surgeons, and physicists. The authors believe that they are now witnessing a paradigm shift toward personalized breast screening, which is going to see many more cancers being detected early, with the use of automated density measurement tools as an important component. Introduction: Samarium-153 (153Sm) styrene divinylbenzene microparticles were developed as a surrogate for Yttrium-90 (90Y) microspheres in liver radioembolization therapy. Unlike the pure beta emitter 90Y, 153Sm possess both therapeutic beta and diagnostic gamma radiations, making it possible for post-procedure imaging following therapy. Methods: The microparticles were prepared using commercially available cation exchange resin, Amberlite IR-120 H+ (620–830 μm), which were reduced to 20–40 μm via ball mill grinding and sieve separation. The microparticles were labelled with 152Sm via ion exchange process with 152SmCl3, prior to neutron activation to produce radioactive 153Sm through 152Sm(n,γ)153Sm reaction. Therapeutic activity of 3 GBq was referred based on the recommended activity used in 90Y-microspheres therapy. The samples were irradiated in 1.494 x 1012 n.cm-2.s-1 neutron flux for 6 h to achieve the nominal activity of 3.1 GBq.g-1. Physicochemical characterisation of the microparticles, gamma spectrometry, and in vitro radiolabelling studies were carried out to study the performance and stability of the microparticles. Results: Fourier Transform Infrared (FTIR) spectroscopy of the Amberlite IR-120 resins showed unaffected functional groups, following size reduction of the beads. However, as shown by the electron microscope, the microparticles were irregular in shape. The radioactivity achieved after 6 h neutron activation was 3.104 ± 0.029 GBq. The specific activity per microparticle was 53.855 ± 0.503 Bq. Gamma spectrometry and elemental analysis showed no radioactive impurities in the samples. Radiolabelling efficiencies of 153Sm-Amberlite in distilled water and blood plasma over 48 h were excellent and higher than 95%. Conclusion: The laboratory work revealed that the 153Sm-Amberlite microparticles demonstrated superior characteristics for potential use in hepatic radioembolization. [ABSTRACT FROM AUTHOR]

This study was carried out to investigate the suitability of using the optically stimulated luminescence dosimeter (OSLD) in measuring surface dose during radiotherapy. The water equivalent depth (WED) of the OSLD was first determined by comparing the surface dose measured using the OSLD with the percentage depth dose at the buildup region measured using a Markus ionization chamber. Surface doses were measured on a solid water phantom using the OSLD and compared against the Markus ionization chamber and Gafchromic EBT3 film measurements. The effect of incident beam angles on surface dose was also studied. The OSLD was subsequently used to measure surface dose during tangential breast radiotherapy treatments in a phantom study and in the clinical measurement of 10 patients. Surface dose to the treated breast or chest wall, and on the contralateral breast were measured. The WED of the OSLD was found to be at 0.4 mm. For surface dose measurement on a solid water phantom, the Markus ionization chamber measured 15.95% for 6 MV photon beam and 12.64% for 10 MV photon beam followed by EBT3 film (23.79% and 17.14%) and OSLD (37.77% and 25.38%). Surface dose increased with the increase of the incident beam angle. For phantom and patient breast surface dose measurement, the response of the OSLD was higher than EBT3 film. The in-vivo measurements were also compared with the treatment planning system predicted dose. The OSLD measured higher dose values compared to dose at the surface (Hp(0.0)) by a factor of 2.37 for 6 MV and 2.01 for 10 MV photon beams, respectively. The measurement of absorbed dose at the skin depth of 0.4 mm by the OSLD can still be a useful tool to assess radiation effects on the skin dermis layer.
