Implant-supported prosthetic rehabilitation for patients with severely atrophic jaws is challenging due to complex anatomical considerations and the limitations of conventional augmentation techniques. This study explores the potential of subperiosteal (juxta-osseous) implants as an alternative solution, using finite element analysis (FEA) to evaluate mechanical performance. Realistic jaw models, developed from radiographic data, are utilized to simulate various implant configurations and load scenarios. Results indicate that different screw placements, implant designs, and structural modifications can significantly influence stress distribution and biomechanical behavior. Upper and lower jaw models were assessed under multiple load conditions to determine optimal configurations. Findings suggest that strategic adjustments, such as adding posterior screws or altering implant connections, can enhance load distribution and reduce stress concentration, particularly in critical areas. Tensile loads in critical bone areas near cortical fixing screws exceeded 50 MPa under anterior loading, while configurations with larger load distributions reduced stress on both implant and bone. Objective: This study aimed to assess clinical efficacy of 4‐mm‐short implants in patients with posterior severe vertical bone atrophy in the medium‐ and long‐term follow‐up. Materials and Methods: Patients rehabilitated with 4‐mm‐short implants in the posterior atrophic jaws, with a minimum follow‐up of 3 years post‐loading, were included in the study. Data were collected for eligible patients, and marginal bone loss (MBL) for each implant was evaluated. The research outcomes were implant failure, MBL and complications. Results: A total of 212 patients with 496 implants were included, resulting in a mean follow‐up of 8.02 ± 2.17 years. The implant survival rate was 95.36% (95% CI: 93.12%–97.04%). More implant failures were observed in the maxilla (p =.02) and fewer failures were observed in patients undergoing more number of hygienic sessions per year (p <.001). The average MBL after 1‐year‐loading was 0.47 mm, increasing to 0.59 mm after 10 years; after 3 years no statistically significant increase in MBL was observed. Maxillary implants showed greater bone loss than mandibular ones (p <.001). More frequent professional oral hygiene sessions per year resulted being related with reduced MBL (p <.001). Conclusions: Four‐mm‐short implants showed high survival rates with an up to 10‐year follow‐up. Their use can offer a fixed prosthetic solution for patients with posterior vertical bone atrophy, minimizing surgical invasiveness, rehabilitative times and costs. [ABSTRACT FROM AUTHOR]

Objectives: To evaluate the antibacterial efficacy of two fluoride-containing (1450 ppm F) toothpastes with or without zinc-citrate (ZCT), hydroxyapatite (HAP) and potassium-citrate (KCit); to assess and compare their clinical effects in terms of tooth sensitivity, plaque accumulation and gingivitis, as well as patients’ satisfaction. Materials and methods: Healthy, adult patients were selected and randomly assigned to two groups (n = 50): Experimental: ZCT-, HAP-, KCit- and fluoride-containing toothpaste; Control: fluoride-containing toothpaste. Salivary counts of Streptococcus mutans (S. mutans), plaque and gingival index, as well as clinically diagnosed sensitivity were recorded at baseline, and after 4 weeks. A custom-made questionnaire was used to assess patients’ self-reported sensitivity (baseline and after 4 weeks) and overall satisfaction with the tested toothpastes. Data were statistically analyzed (α = 0.05). Results: After 4 weeks, a statistically significant salivary reduction of S. mutans was observed in both groups (p = 0.001). Furthermore, the percentage of S. mutans decrease was significantly higher in Experimental group (p = 0.014). There were no statistically significant differences between the groups in terms of plaque and gingival index (p > 0.05). After 4 weeks, the self-reported tooth sensitivity was lower in Experimental group (p < 0.001). Conclusions: Both toothpastes showed good antimicrobial effect after 4 weeks; however, the toothpaste containing ZCT, HAP, KCit and fluoride was found to be more effective in reducing the salivary counts of S. mutans than the product containing fluoride alone. Clinical relevance: Toothpaste containing ZCT, HAP, KCit and fluoride can be recommended for patients at risk for developing caries and may also be beneficial for individuals experiencing dental sensitivity. Background: Oral and implant surgery represent highly specialized fields within dentistry that require a deep understanding of complex anatomical structures, together with practical hands-on experience. The present review examines common trends in oral and implant surgery training, focusing on how traditional methods like donated body dissection coexist with different and modern educational tools, and highlights the pros and cons of the different approaches in order to optimize training outcomes. Methods: A systematic literature search was carried out using the databases PubMed and Cochrane Library including the last 10 years of published articles about training in oral surgery and implantology. Starting from a total of 1319 studies, 47 were included to be carefully evaluated, and 20 studies were finally selected for this narrative review. The studies utilize methodologies such as randomized controlled trials (RCTs), cross-sectional surveys, case–control studies, and systematic reviews. The results were thematically organized, highlighting key quantitative outcomes and drawing connections between the different educational approaches. Results: From the narrative review, it emerged that oral and implant surgery training requires a careful balance between traditional methods, such as donated human body dissection, and modern technological advancements like virtual simulations and synthetic models. While animal and synthetic models have specific uses, their application remains limited in replicating the full complexity of human anatomy. These last technologies offer flexibility and expanded access to education but do not substitute for the hands-on experience gained through donated human body dissection. Conclusions: As educational institutions continue to evolve their training programs, ensuring access to human body dissection remains of paramount importance. Combining the strengths of both traditional and modern approaches may help optimize oral and implant surgery education, enhancing student preparedness without overlooking the critical value of direct anatomical experience. Immediate loading implant surgery has emerged as a significant advancement in the rehabilitation of edentulous patients. This approach requires meticulous planning and precise execution to ensure successful outcomes. Transferring established intermaxillary and occlusal relationships to the definitive prostheses can be challenging. However, with a digital approach, this procedure can be standardized in cases of fully guided surgery with a flapless technique or by using disposable guides. Conversely, when extensive open flap implant surgery is required, such as in the treatment of severely atrophic patients (e.g., with zygomatic implants or simultaneous bone augmentation), the digital workflow can be demanding. The impression reference technique was proposed to enhance the digital workflow for immediate loading in zygomatic implant rehabilitation. Background: The malar bone provides an anchorage point for zygomatic implants, avoiding invasive reconstructive surgeries in the fixed rehabilitation of fully edentulous and severely atrophic maxillae. The limited bone volume, however, requires precise implant placement to prevent complications related to nearby anatomical structures. This observational cross-sectional study aims to measure the malar and zygomatic arch bones and their distances from critical anatomical landmarks to guide surgeons in safe zygomatic implant placement. Methods: Dissections were performed bilaterally on 29 heads from human donated bodies in a cross-sectional observational study. Key landmarks evaluated include the infraorbital foramen (IF), pyriform nasal aperture (PNA), infraorbital margin (IM), zygomaticofacial foramen (ZFF), anterior end (A), and the most protruding point of the zygomatic arch (B). Measurements included IF-PNA, IF-IM, IF-ZFF, ZFF-IM, A-B, and orbital floor depth (OFD). Results: Significant findings showed IF-PNA was greater in males (18.66 ± 2.63 mm, p = 0.001), and IF-ZFF varied between sides (26.72 ± 8.7 mm, p = 0.002). ZFF-IM was larger in males (7.43 ± 2.09 mm, p < 0.001). Heights and thicknesses were also assessed, with significant side differences observed. Conclusions: These findings underscore the importance of understanding precise anatomical distances for successful implant placement. The importance of social, emotional, and behavioral (SEB) skills is recognized worldwide, but their measurement has always been a challenge. The BESSI measures 32 SEB skills, divided into five domains (social engagement, cooperation, self-management, emotional resilience, and innovation), but its validity must be expanded to new languages and contexts. Across two studies (N1 = 990, N2= 824) we developed the Italian version of the BESSI, provided further support for its convergent and discriminant validity with the Big Five, and expanded its nomological network to procrastination, self-efficacy, and emotion regulation. The BESSI-I showed excellent internal reliability and satisfactory fit indices at the facet, domain, and overarching framework level. We also confirmed the correlations between the SEB skills and the Big Five personality traits and found meaningful correlations with the selected external outcomes. Overall, we confirm that the BESSI-I is a valid and useful instrument to assess SEB skills for research and clinical purposes. [ABSTRACT FROM AUTHOR]

Purpose: Zygomatic implants are considered one of the last options for the rehabilitation of severe maxillary atrophy when standard implants cannot be placed. They offer several advantages but can also present complications. This study aimed to investigate the long-term clinical and radiographic outcomes of zygomatic implant placement. Materials and methods: A retrospective chart review was conducted, and the inclusion criteria consisted of patients previously treated with zygomatic implants who had Class V or VI maxillary bone atrophy according to Cawood and Howell, and with a minimum follow-up period of 2 years after prosthetic loading. Outcome measures included implant and prosthesis survival rate, biological and biomechanical complications, and Lund-Mackay staging score before and after implant placement. Results: The study included 78 patients who received a total of 274 zygomatic implants. The mean follow-up period was 90.4 ± 26.0 months. Seventeen implant failures occurred, resulting in a survival rate of 93.8%, with a statistically significant negative correlation with smoking habits (P = 0.049), anchorage to the two zygomatic bone cortices (bicorticality) (P < 0.001) and soft tissue complications (P < 0.001). The prosthetic success rate was 92.3%. A statistically significant increase in maxillary sinus radiopacity was recorded when comparing the situation before and after surgery (P < 0.001), and the intrasinus pathway had a statistically significant influence on that increase (P = 0.003). Conclusions: Zygomatic implants utilised for rehabilitating patients with severe maxillary atrophy have shown favourable outcomes. Nonetheless, owing to potential complications, strict case selection is necessary, combined with regular recall visits and proper oral hygiene maintenance. Objective: To assess peri‐implant soft tissues condition, comparing clinical parameters of implants placed in patients with atrophic upper jaws and patients who underwent maxillary resection for oncological reasons. Background: Zygomatic implants (ZIs) in oncologic patients could be affected by more complications compared to implants placed in atrophic maxillae. The soft tissue condition is an essential requirement for implant success, but few studies have investigated this topic. Methods: A nested monocentric prospective parallel cohort (atrophic vs. oncological patients) study was performed. Clinical visits and professional hygiene sessions were performed every three months, and bleeding on probing (BOP), probing pocket depth (PPD), gingival index (GI), plaque index (PI) and implant mobility were recorded by a blind outcome assessor. Results: In total, 77 ZIs placed in 21 patients were evaluated: 54 (70.1%) ZIs were inserted in patients belonging to the atrophic cohort (PAM) and 23 (29.9%) ZIs in the oncologic cohort (OP). The probability of having BOP at the considered mean follow‐up (27 months) was 24.8% (95% CI 19.0–31.9) for PAM and 22.9% (95% CI 15.1–33.9) OP. The mean PPD values were 2.78 ± 1.28 (range 1–8) in PAM and 2.91 ± 1.98 (range 0–10) in OP. None of the implants showed mobility. No associations between group belongingness and the entity of PPD, PI, GI and the risk of BOP were found, adjusting for the considered confounding factors (age, smoking and implant position). Conclusions: Under a strict supportive hygiene therapy protocol ZIs in oncologic patients showed similar peri‐implant tissue conditions to that of patients with maxillary atrophy. Purpose: To compare the clinical effectiveness of three different devices used in guided bone regeneration procedures for partially atrophic arches. Materials and methods: A randomised controlled trial with three parallel arms was conducted. The study evaluated titanium-reinforced polytetrafluoroethylene membrane (PTFE group), semiocclusive CAD/CAM titanium mesh (mesh group) and occlusive CAD/CAM titanium foil (foil group) in terms of surgical outcomes and complications as well as surgical times and surgeon satisfaction in 27 guided bone regeneration procedures, presenting results from 1 year post-implant placement. Results: Complications occurred in seven patients. No significant difference was found between the groups in terms of the occurrence of complications (P = 0.51), device exposure (P = 0.12) and implant failure (P = 0.650). Surgeon satisfaction varied significantly, with the PTFE group differing from the mesh (P = 0.003) and foil groups (P < 0.001), but not between meshes and foils (P = 0.172). Surgical times also differed significantly, with longer times for PTFE membranes compared to meshes (P < 0.001) and foils (P = 0.006), but with no difference between meshes and foils (P = 0.308). The mean reconstructed bone volume was 1269.55 ± 561.08 mm3, with no significant difference observed between the three groups (P = 0.815). There was also no significant difference for mean maximum height (6.72 mm, P = 0.867) and width (7.69 mm, P = 0.998). The mean marginal bone loss at 1 year after implant placement was 0.59 ± 0.27 mm. Conclusions: Although this study provides valuable insights into the potential benefits of using different types of CAD/CAM devices, further research with larger sample sizes and longer follow-up periods is warranted to validate these findings. Purpose: The aim of this study was accuracy assessment of placed implants in full-arch cases using specific software and hardware to perform static computer-assisted implantology and immediately loaded prostheses. The degree of deviation existing between planned and achieved implants was carried out by a new noninvasive measurement procedure of the implant position performed on stone casts. Materials and Methods: Fourteen stone casts retrieved from 14 full-arch fully guided implant treatments were selected to perform the study. Each cast, manufactured for the surgical treatment by using a specific laboratory kit, was obtained from the respective surgical guide. A sleeve for each implant was embedded into the guide, which helped the examiners to manufacture a stone cast per guide containing the implant analogs, which was used to recover the final position of the planned implants. A total sample of 60 implants were assessed. The postoperative casts, poured to produce the immediate prostheses, were then processed by a contact (or tactile) scanner, and the generated standard tessellation language (STL) files were overlapped (best-fit alignment) using engineering software that revealed all the measured discrepancies. In terms of accuracy, differences relating to arch, assessed bone quality, implant length, and drill length (prolongation short or long) were reported. Results: The use of a noninvasive tactile scanner revealed mean entry point horizontal deviations of 0.30 mm (SD: 0.39 mm), mean entry point vertical deviations of 0.20 mm (SD: 0.25 mm), mean apical horizontal deviations of 0.50 mm (SD: 0.73 mm), and mean apical vertical deviations of 0.24 mm (SD: 0.28 mm). The frontal and lateral angular deviations were investigated, and corresponding mean values of 1.99 degrees (SD: 2.30 degrees) and 1.80 degrees (SD: 2.44 degrees) were detected. Purpose: The reverse guided bone regeneration protocol is a digital workflow that has been introduced to reduce the complexity of guided bone regeneration and promote prosthetically guided bone reconstruction with a view to achieving optimal implant placement and prosthetic finalisation. The aim of the present study was to investigate the accuracy of this digital protocol. Materials and methods: Sixteen patients with partial edentulism in the maxilla or mandible and with vertical or horizontal bone defects were treated using the reverse guided bone regeneration protocol to achieve fixed implant rehabilitations. For each patient, a digital wax-up of the future rehabilitation was created and implant planning was carried out, then the necessary bone reconstruction was simulated virtually and the CAD/CAM titanium mesh was designed and used to perform guided bone regeneration. The computed tomography datasets from before and after guided bone regeneration were converted into 3D models and aligned digitally. The actual position of the mesh was compared to the virtual position to assess the accuracy of the digital project. Surgical and healing complications were also recorded. A descriptive analysis was conducted and a one-sample t test and Wilcoxon test were utilised to assess the statistical significance of the accuracy. The level of significance was set at 0.05. Results: A total of 16 patients with 16 treated sites were enrolled. Comparing the virtually planned mesh position with the actual position, an overall mean discrepancy between the two of 0.487 ± 0.218 mm was achieved. No statistically significant difference was observed when comparing this to a predefined minimum tolerance (P = 0.06). No surgical complications occurred, but two healing complications were recorded (12.5%). Conclusion: Within the limitations of the present study, the reverse guided bone regeneration digital protocol seems to be able to achieve good accuracy in reproducing the content of the virtual plan. Rehabilitating thin jaws without reconstructive surgery entails using narrow implants. The proposed treatment adopted an innovative implant system, allowing the mini-screws to be parallel and immediately loaded. A mandible, wearing an overdenture, was functionalized contextually to the residual dental extraction and the placement of six 2.4-mm thick one-piece implants. Low-profile intermediate abutments, the LEMs, able to rotate over the spherical heads of the fixtures, were connected after suturing, oriented, and blocked in a mutual parallel position. The copings, engaging with a tapered juncture of the LEMs, resulted in their alignment to be intraorally wedged together. The provisional superstructure enclosed the copings and was immediately connected to the implants, and the definitive prosthesis was delivered after three months. No clinical signs of peri-implantitis or radiographically evident bone loss were recorded after a two-year follow-up without any prosthetic complication. Understanding the individual qualities sustaining students with and without specific learning disabilities (SLDs) is key to supporting their academic achievement and well-being. In this study, we investigated the differences between students with and without SLDs in terms of intraindividual factors (soft skills and study-related factors), academic and nonacademic outcomes (achievement, academic and life satisfaction), and the relationships between such intraindividual factors and the three outcomes. A total of 318 students (79 males; M age = 22.7; SD = 3.56; age range = 19–45 years; 147 with SLDs) completed self-reported questionnaires and a measure of fluid intelligence. The results showed that students with SLDs reported higher creativity but lower academic self-efficacy, study resilience, and academic achievement, with small-to-medium effect sizes. In both groups, achievement significantly positively related to academic self-efficacy and negatively to creativity. Life satisfaction was positively related to study resilience; and academic satisfaction was related to critical thinking, curiosity, and academic self-efficacy. Purpose: Dynamic navigation is a technique that allows for the placement of dental implants using a computer-guided approach according to preoperative planning. Its accuracy has been assessed in several previous studies. The purpose of this study was to summarize data on implant placement accuracy using dynamic navigation, to synthesize the frequency of intraoperative complications and implant failures, and to compare this technique with static computer-guided surgery and a freehand approach. Materials and Methods: Electronic and manual literature searches until December 2019 were performed. The outcome variables were implant placement accuracy using dynamic navigation, accuracy differences between dynamic and static techniques and between dynamic and freehand techniques, intraoperative complications, and implant failures. Random-effects meta-analyses were performed. Results: A total of 32 studies were included; 29 reported accuracy values (2,756 implants), and 10 focused on complications and implant failures (1,039 implants). The pooled mean implant placement errors were 0.81 (95% CI: 0.677 to 0.943) mm at the entry point and 0.910 (95% CI: 0.770 to 1.049) mm at the apical point. The pooled mean vertical and angular deviations were 0.899 (95% CI: 0.721 to 1.078) mm and 3.807 (95% CI: 3.083 to 4.530) degrees. The navigation group showed significantly lower implant placement errors with respect to the freehand technique (P < .01) and similar accuracy values (P = .05) compared with the static technique. The pooled prevalence of failures was 1% (95% CI: 0.00% to 2%). Conclusion: Dynamic navigation provided small implant placement errors, comparable with those obtained using static computer-guided surgery, and can be considered a more accurate technique than conventional freehand surgery. In recent years, an increasing number of students with specific learning disabilities (SLDs) have enrolled in universities. The present exploratory study examined the frequency of use and appreciation of universal (open to every student) and specific services (offered to students with SLDs) and their relation to age, academic achievement, satisfaction, self‐efficacy, and use of self‐regulated learning (SRL) strategies. Participants were 147 Italian university students with SLD diagnoses (42 males; mean age: 22.49, SD = 3.29). Results showed that, overall, the frequency of use and appreciation of specific services were positively related to academic satisfaction, self‐efficacy, and SRL strategies. Furthermore, frequency of use of compensatory tools and dispensatory measures was positively associated with academic achievement. These findings suggest that universities play an important role in supporting students with SLDs during their academic years by providing them with useful services and accommodations. Purpose: Guided bone regeneration is a widely used technique for the treatment of atrophic arches. A broad range of devices have been employed to achieve bone regeneration. The present study aimed to investigate the clinical and histological findings for a new titanium CAD/CAM device for guided bone regeneration, namely semi-occlusive titanium mesh. Materials and methods: Nine partially edentulous patients with vertical and/or horizontal bone defects underwent a guided bone regeneration procedure to enable implant placement. The device used as a barrierwas a semi-occlusive CAD/CAM titanium mesh with a lasersintered microperforated scaffold with a pore size of 0.3 mm, grafted with autogenous and xenogeneic bone in a ratio of 80:20. Eight months after guided bone regeneration, surgical and healing complications were evaluated and histological analyses of the regenerated bone were performed. Results: A total of 9 patients with 11 treated sites were enrolled. Two healing complications were recorded: one late exposure of the device and one early infection (18.18%). At 8 months, well-structured new regenerated trabecular bone with marrow spaces was mostly present. The percentage of newly formed bone was 30.37% ± 4.64%, that of marrow spaces was 56.43% ± 4.62%, that of residual xenogeneic material was 12.16% ± 0.49% and that of residual autogenous bone chips was 1.02% ± 0.14%. Conclusion: Within the limitations of the present study, the results show that semi-occlusive titanium mesh could be used for vertical and horizontal ridge augmentation. Zygomatic implant-supported rehabilitation has grown in popularity for use in clinical practice. Although many studies have been carried out into the surgical procedure, the prosthetic workflow is not clearly defined and standard techniques are not readily applied; thus, a digital approach may ultimately streamline the procedure. In the present study, the authors examined a digital workflow for immediately loaded prostheses supported by zygomatic implants. Background and Aim: It is unclear if fibrous dysplasia (FD) represents a contraindication for implant borne rehabilitation, and only two successful cases are reported in the literature with a 4–2‐year follow‐up. The present paper discusses this issue reporting a full‐arch maxillary rehabilitation with a >5‐year follow‐up. Materials and Methods: A 79‐year‐old woman complained of a progressive asymmetry of the medial facial third with inter‐arch occlusal alterations, reduced mandibular movements, upper right dental dislocation and mobility and toothache in the upper posterior jaw. The X‐ray supported monostotic FD diagnosis was followed by a remodelling intervention in general anaesthesia, with the extraction of the hopeless teeth. After that and the failed rehabilitation with a removable prosthetic device, the patient underwent implant placement procedure for a fixed 'Toronto‐bridge' prosthesis. Results: After 6 years from implant loading, a posterior vestibular swelling of the affected maxilla was recorded, and one implant in 1.1 position had to be removed for peri‐implantitis, without compromising the rehabilitation. Conclusions: The acceptable results obtained in this case should promote the dental implantology practice in FD affected jaws, evaluating the proper clinical situations and the more adequate technological solutions. Zygomatic implant site preparation could be considered a challenging procedure because of the use of a very long twist drill that could sometimes be difficult to control due to the need for drilling the malar bone on an oblique surface. Ultrasound was recently suggested to achieve better control, but the specific long tips required are not readily available, and the elongated tip also tends to reduce the efficacy. This technical note describes a proposal of a novel computer-aided technique to simplify the procedure of zygomatic implant site preparation. This method uses a standard-length ultrasonic tip to prepare the crestal bone and the zygomatic bone in two individual steps. The desired implant trajectory can be achieved during preparation using a real-time tracking navigation system. The combined use of the navigation system and the ultrasonic tips could aid the surgeon, during the first steps, in achieving optimal control of the instruments employed for the implant site preparation and keeping the planned zygomatic implant position. If the efficacy of the procedure is confirmed through clinical trials, this technique could also contribute to reducing the invasiveness of the procedure, promoting a smaller flap, and reducing the soft tissue damage. Featured Application: The proposed protocol, starting from a careful prediction of the final result with the ideal relationship between the prosthesis and soft tissues, aims to improve the GBR approach, particularly for critical atrophies.Background: Treating extended alveolar defects is challenging for their irregular shape and lack of hard and soft tissues. Virtual planned guided bone regeneration (GBR) with customized meshes aims to optimize the treatment by reducing the risk of dehiscence. The mucosa characteristics are crucial in preserving the bone graft covering and vitality. Methods: Two three-dimensional and extended defects, a mandibular posterior and anterior maxillary atrophy were reconstructed with a particulate graft and a digitally customized scaffold. The workflow entailed merging the pre-operatory clinical related data from intra-oral scanning with the radiologic ones from cone beam-CT. A final ideal prediction of the soft tissue relationship with the implant-borne prosthesis was virtually elaborated, conditioning the design of the titanium membrane fitting the bone defects. Results: A good matching between the scaffold and the bone surface was intra-operatory noted; no complications were registered in the first months of healing with complete integrity of the soft tissues above the graft. Conclusions: A careful evaluation of the soft tissues and a forecast of their final relationship with the implant and prosthesis can improve digital mesh/membrane manufacturing with a suitable healing process up to implant placement and loading, favoring peri-implant tissue stability over time. The present study aimed to evaluate the influence of manual expertise on static computer-aided implantology (s-CAI) in terms of accuracy and operative timings. After the cone-beam CT (CBCT) scanning of eleven mandibular models, a full-arch rehabilitation was planned, and two different skilled operators performed s-CAI. The distances between the virtual and actual implant positions were calculated considering the three spatial vectorial axes and the three-dimensional Euclidean value for the entry (E) and apical (A) points, along with the axis orientation differences (Ax). These values emerged from the overlapping of the pre-op CBCT to post-op CBCT data (method 1), from scanning the data from the laboratory scanner (method 2), and from the intra-oral scanner (method 3) and were correlated with the operators' expertise and operative timings. The mean values for accuracy from the three methods were: E = 0.57 (0.8, 0.45, 0.47) mm, A = 0.6 (0.8, 0.48, 0.49) mm, and Ax 1.04 (1.05,1.03,1.05) ° for the expert operator; and E = 0.8 (0.9, 0.87, 0.77), A = 0.95 (1.02, 0.95, 0.89), and Ax =1.64 (1.78, 1.58, 1.58) for the novice. The mean value of the operative timings was statistically inferior for the expert operator (p < 0.05), with an improved accuracy over time for both operators. A significant difference (p < 0.05) emerged between method 1 and methods 2 and 3 for seven of the nine variables, without differences between the evaluations from the two scanners. The support from digital surgical guides does not eliminate the importance of manual expertise for the reliability and the shortening of the surgical procedure, and it requires a learning pathway over time. This case report describes new implant site preparation techniques joining the benefits of using an intraoral navigation system to optimize three-dimensional implant site positioning in combination with an ultrasonic osteotomy. A report of five patients is presented, and the implant positions as planned in the navigation software with the postoperative scan image were compared. Objectives: To evaluate the outcomes of bone regeneration using a customized titanium mesh scaffold to cover a bone graft for reconstruction of complex defects of the jaws. Materials and Methods: 19 large defects were digitally reconstructed using CT scans according to the prosthetic requirements. A titanium mesh scaffold was designed to cover the bone (autologous/bovine bone particulate) graft. At least 6 months after surgery, a new cone‐beam CT was taken. The pre‐ and postoperative CT datasets were then converted into three‐dimensional models and digitally aligned. The actual mesh position was compared to the virtual position to assess the reliability of the digital project. The reconstructed bone volumes (RBVs) were calculated according to the planned bone volumes (PBVs), outlining the areas under the mesh. These values were then correlated with the number of exposures, locations of atrophy, and virtually planned bone volume. Results: The mean matching value between the planned position of the mesh and the actual one was 82 ± 13.4%. 52.3% (40% early and 60% late) exposures were observed, with 15.8% exhibiting infection. 26.3% resulted as failures. The amount of reconstructed bone volume (RBV) in respect to PBV was 65 ± 40.5%, including failures, and 88.2 ± 8.32% without considering the failures. The results of the exposure event were statistically significant (p =.006) in conditioning the bone volume regenerated. Conclusions: This study obtained up to 88% of bone regeneration in 74% of the cases. Purpose: To compare the efficacy of 1-stage versus 2-stage lateral maxillary sinus lift procedures. Materials and methods: Sixty partially edentulous patients requiring 1 to 3 implants and having 1 to 3 mm of residual bone height and at least 5 mm of bone width below the maxillary sinus, as measured on CT scans, were randomised into two equal groups to receive either a 1-stage lateral window sinus lift with simultaneous implant placement or a 2-stage procedure with implant placement delayed by 4 months using a bone substitute in 3 different centres. Implants were submerged for 4 months and loaded with reinforced provisional prostheses, which were replaced, after 4 months, by definitive prostheses. Outcome measures were augmentation procedure failures, prosthesis failures, implant failures, complications and marginal peri-implant bone loss assessed by a blinded outcome assessor. Patients were followed up to 4 months after loading. Only data of implants placed in 1 to 3 mm of bone height were reported. Results: Two patients dropped out from the 1-stage group and none from the 2-stage group. No sinus lift procedure failed in the 1-stage group but 1 failed in the 2-stage group, the difference was not statistically significant (P = 1.00). Two prostheses failed or could not be placed in the planned time in the 1-stage group and 1 in the 2-stage group, the difference was not statistically significant (P = 0.51). Three implants failed in 3 patients of the 1-stage group versus 1 implant in the 2-stage group, the difference was not statistically significant (P = 0.28). Two complications occurred in the 1-stage group and 1 in the 2-stage group, the difference was not statistically significant (P = 0.61). There were no statistically significant differences in bone loss between groups at loading (0.05mm). Sites treated in 1 stage lost an average of 0.56mm (SD: 0.36; 95% CI: -0.70 to -0.42; P < 0.001) of peri-implant bone and 2-stage sites approximately 0.61 mm (SD: 0.34; 95% CI: -0.74 to -0.48; P < 0.001). Conclusions: No statistically significant differences were observed between implants placed according to 1- or 2-stage sinus lift procedures. However, this study may suggest that in patients having a residual bone height between 1 and 3 mm below the maxillary sinus there might be a slightly higher risk for implant failures when performing a 1-stage lateral sinus lift procedure. Objectives: The aim of this study was to evaluate hard and soft tissues around implants placed in posterior mandible 1 year after vertical ridge augmentation (VRA). Materials and Methods: 40 patients with vertical defects were enrolled and treated according to the study protocol. Patients were randomly divided into two study groups: reinforced PTFE membranes (group A) and titanium meshes plus collagen membranes (group B). All patients received simultaneous implants which were evaluated after prosthetic restoration at baseline and after 1 year, using the following parameters: peri‐implant bone levels (PBLs), interproximal bone peaks (IBPs), pocket probing depth (PPD), bleeding on probing (BoP), plaque index (mPI), gingival index (mGI), keratinized tissue thickness/width (tKT and wKT), and fornix depth (FD). Statistical analysis was performed to investigate any statistically significant differences and/or correlations (p =.05). Results: 30 patients were completely followed up according to the study protocol. After 1 year, implants showed a change in PBL from 0.12 to 0.76 mm, with marginal bone loss of 0.67 and 0.61 mm for group A and group B, respectively, without significant differences (p >.9337). The estimated difference between treatments for the change from baseline in PBL was −0.05 (95% CI −0.27 to 0.16). Statistical analysis revealed strong correlations between PBL and IBP (p <.0001). However, no significant differences were observed for PPD, mPI, mGI, tKT, wKT, and FD (p >.05). Conclusion: The results indicate that GBR treatment with titanium meshes plus collagen membranes (Group B) compared to reinforced PTFE membranes does not appear to be inferior or superior in terms of PBL change. In both groups, hard and soft tissues were stable after 1 year of follow‐up, with a peri‐implant bone loss less than 1.0 mm in the first year (study registered at ClinicalTrials.gov NCT04332679). Purpose: To evaluate clinical safety and effectiveness of a novel calcium-incorporated titanium implant (Xpeed, MegaGen Implant Co. Limited, Gyeongbuk, South Korea). Materials and methods: Sixty patients were randomised to receive either 1 to 6 calcium-incorporated or control titanium implants in the maxilla according to a parallel group design at 2 different centres. Implants were submerged and exposed at 3 different endpoints in equal groups of 20 patients each at 12, 10 and 8 weeks, respectively. Within 2 weeks, implants were functionally loaded with provisional or definitive prostheses. Outcome measures were prosthesis failures, implant failures, any complications and peri-implant marginal bone level changes. Results: Thirty patients received 45 calcium-incorporated implants and 30 patients received 42 control titanium implants. One year after loading, no drop-outs and no prosthesis or implant failures occurred. There were no statistically significant differences between groups for complications (P = 0.61; difference in proportions = -0.27; 95% CI -0.71 to 0.18) and mean marginal bone level changes (P = 0.64; mean difference -0.04 mm; 95% CI -0.22 to 0.13). Conclusions: Nanostructured calcium-incorporated titanium implants seem to be at least as effective and safe as conventional titanium implants. Objectives: To evaluate whether the adjunctive use of a bone substitute at immediate single implants placed in fresh extraction sockets with a residual buccal bone-to-implant gap of at least 1 mm could improve the aesthetic outcome of guided bone regeneration (GBR). Materials and methods: Eighty patients requiring bone augmentation at a single immediate post-extractive implant to improve the aesthetic outcome were randomly allocated to an augmentation procedure using a resorbable barrier alone (GBR group; 40 patients) or a bone substitute plus a resorbable barrier (GBR + BS group; 40 patients) according to a parallel group design at four different centres. Three to 4 months after implant placement/augmentation, implants were loaded with provisional or definitive single crowns. Outcome measures were implant failures, complications, aesthetics assessed using the pink esthetic score (PES), patient satisfaction and peri-implant marginal bone levels, recorded by blinded assessors. All patients were followed up to 1 year after loading.Results: One patient dropped out from the GBR group. Seven (9%) implants failed: 2 (5%) in the GBR + BS group and 5 (12.5%) in the GBR group. Six minor complications occurred in the GBR +BS group versus 2 in the GBR group. These differences were not statistically significant. Just after implant placement/augmentation, mean bone levels were -0.21 mm at GBR + BS implants and -1.92mm at GBR implants whereas at 1 year after loading they were -1.04 and -1.76, respectively. When comparing the two groups, GBR + BS implants had 0.70 mm more peri-implant marginal bone than GBR implants. Aesthetics was scored by a blinded assessor as statistically significantly better for the GBR + BS group. Patients were equally satisfied. Objectives: To evaluate whether 6.3 mm-long implants could be a suitable alternative to longer implants placed in vertically augmented atrophic posterior mandibles.Materials and methods: Sixty partially edentulous patients having 7 to 8 mm of residual crestal height and at least 5.5 mm thickness measured on CT scans above the mandibular canal were randomised according to a parallel group design either to receive 1 to 3 submerged 6.3 mm-long implants or 9.3 mm or longer implants (30 patients per group) placed in vertically augmented bone. Bone was augmented with interpositional an organic bovine bone blocks covered by resorbable barriers. Grafts were left to heal for 5 months before implant placement. Four months later, provisional acrylic prostheses were delivered, and were then replaced after another 4 months by definitive metal-ceramic prostheses.Outcome measures were prosthesis and implant failures, complications, and radiographic peri-implant marginal bone level changes. All patients were followed up to 3 years after loading.Results: Four patients dropped out, two from each group. The augmentation procedure failed in two patients and only 6.3 mm-long implants could be inserted. There were no statistically significant differences for prosthesis and implant failures. Three prostheses could not be placed or had to be remade in the short implant group versus 4 prostheses in the augmented group. Two short implants failed versus 3 long implants, all in different patients. There were statistically significantly more complications in augmented patients (22 complications in 20 augmented patients versus 5 complications in 5 patients of the short implant group). Both groups gradually lost peri-implant bone in astatistically significant way at 4 months, and 1 and 3 years after loading. Three years after loading,patients of the short implant group lost an average of 1.24 mm of peri-implant bone compared with 1.76 mm in the long implant group. Short implants experienced statistically significantly less bone loss (0.52 mm; CI 95% 0.20 to 0.83, P = 0.002) than long implants.Conclusions: When residual bone height over the mandibular canal is between 7 and 8 mm, 6.3 mm short implants could be an interesting alternative to vertical augmentation since the treatment is faster,cheaper and associated with less morbidity. Purpose: To evaluate whether short (5 to 8.5 mm) dental implants could be a suitable alternative to longer (>11.5 mm) implants placed in atrophic maxillae augmented with autogenous bone for supporting dental prostheses. Materials and methods: Twenty-eight patients with fully edentulous atrophic maxillae having 5 to 9 mm of residual crestal bone height at least 5 mm thick, as measured on computerised tomography scans, were randomised into two groups either to receive 4 to 8 short (5 to 8.5 mm) implants (15 patients) or autogenous bone from the iliac crest to allow the placement of at least 11.5 mm long implants (13 patients). Bone blocks and the windows at maxillary sinuses were covered with rigid resorbable barriers. Grafts were left to heal for 4 months before placing implants, which were submerged. After 4 months, provisional reinforced acrylic prostheses or bar-retained overdentures were delivered. Provisional prostheses were replaced, after 4 months, by definitive screw-retained metal-resin cross-arch fixed dental prostheses. Outcome measures were: prosthesis and implant failures, any complications (including prolonged postoperative pain) and patient satisfaction. All patients were followed for 5 months after loading. Results: All patients could be rehabilitated with implant-supported prostheses and none dropped out. One bilateral sinus lift procedure failed due to infection, though short implants could be placed. One implant failed in the augmented group versus 2 short implants in 2 patients. All failures occurred before loading. Significantly more complications occurred in augmented patients: 8 complications occurred in 5 augmented patients (all complained of pain 1 month after bone harvesting from the iliac crest). No complications occurred in the short implant group. All patients were fully satisfied with the treatment and would do it again. Conclusions: This pilot study suggests that short implants may be a suitable, cheaper and faster alternative to longer implants placed in bone augmented with autogenous bone for rehabilitating edentulous atrophic maxillae. Purpose: To evaluate whether 5 mm short dental implants could be an alternative to augmentation with anorganic bovine bone and placement of at least 10 mm long implants in posterior atrophic jaws. Materials and methods: Fifteen patients with bilateral atrophic mandibles (5-7 mm bone height above the mandibular canal), and 15 patients with bilateral atrophic maxillae (4-6 mm bone height below the maxillary sinus) and bone thickness of at least 8 mm, were randomised according to a split-mouth design to receive one to three 5 mm short implants or at least 10 mm long implants in augmented bone. Mandibles were vertically augmented with interpositional bone blocks and maxillary sinuses with particulated bone via a lateral window. Implants were placed after 4 months, submerged and loaded, after 4 months, with provisional prostheses. Four months later, definitive provisionally cemented prostheses were delivered. Outcome measures were: prosthesis and implant failures, any complication and peri-implant marginal bone level changes. Results: In 5 augmented mandibles, the planned 10 mm long implants could not be placed and shorter implants (7 and 8.5 mm) had to be used instead. One year after loading no patient dropped out. Two long (8.5 mm in the mandible and 13 mm in the maxilla) implants and one 5 mm short maxillary implant failed. There were no statistically significant differences in failures or complications. Patients with short implants lost on average 1 mm of peri-implant bone and patients with longer implants lost 1.2 mm. This difference was statistically significant. Conclusions: This pilot study suggests that 1 year after loading, 5 mm short implants achieve similar if not better results than longer implants placed in augmented bone. Short implants might be a preferable choice to bone augmentation since the treatment is faster, cheaper and associated with less morbidity, however their long-term prognosis is unknown. Objective: To test if using a CAD/CAM system might reduce the necessity of bone augmentation in patients with atrophic maxillary arches before implant therapy. Method and Materials: Twenty male and female patients consecutively scheduled for bone augmentation of the jaw before implant surgery were included in this study, with a total of 29 jaws (maxillary and mandibular) to analyze for the implant-supported fixed prosthesis group and 19 maxillary arches for the implant-supported removable prosthesis group. NobelGuide System (Nobel Biocare), Autocad System (Autodesk), and routine manual CT measurements of available bone were used in this study. Results: The total results of the mean values of the fixed prosthesis group plus the mean values of the removable prosthesis group showed a statistically significant difference between the NobelGuide intervention score and both manual (P = .004) and Autocad (P = .001) measurements. Conclusions: The NobelGuide System represents a viable diagnostic device to reduce the entity or avoid bone reconstructive surgery before implant placements in the atrophic maxilla and mandible. Purpose: To compare the effectiveness of two different techniques to lift the maxillary sinus via a crestal approach: the Summers versus the Cosci technique. Materials and methods: Fifteen partially edentulous patients missing bilaterally maxillary molars and/ or premolars having 4 to 7 mm of residual crestal height and at least 5 mm thickness below the maxillary sinuses measured on computed tomography scans were randomised to have implants placed in sinuses crestally lifted according to the Cosci or Summers technique with bone substitutes according to a split-mouth design. Implants were left to heal submerged for 6 months. Implants were loaded with acrylic provisional crowns/prostheses. Screw-retained definitive metal-ceramic prostheses were delivered 4 months after provisional loading. Outcome measures were prosthesis and implant failures, any complications, operation time, operator preference, and patient preference assessed 1 month after surgery and 1 month after delivery of the final prostheses by a blinded outcome assessor. All patients were followed up to 5 months after loading (1 year after implant placement). Results: Nineteen study implants were placed according to each technique. No patient dropped out and no implant failed. No discomfort/complications occurred at sites treated with the Cosci technique whereas 12 patients reported discomfort during the augmentation procedure at the side treated with the Summers technique, this was statistically significant, and in one of these patients a perforation of the sinus membrane occurred. Postoperatively, headache was reported by nine patients and swelling occurred in three of these patients at the Summers treated sides. Statistically significantly less time was required to place implants according to the Cosci technique (33 versus 24 minutes, on average). The two operators and 14 out of 15 patients preferred the Cosci technique. Conclusions: Both crestal sinus lift techniques were successful but the Cosci technique required less surgical time, produced less intra- and postoperative morbidity and was preferred by patients. Purpose: To compare the efficacy of two different techniques to augment maxillary sinuses using a lateral window approach: rigid synthetic resorbable barriers (Inion) versus granular anorganic bovine bone (Bio-Oss). Materials and methods: Ten partially edentulous patients having bilaterally 1 to 5 mm of residual bone height and at least 5 mm bone width below the maxillary sinuses, as measured on computed tomography (CT) scans, were randomised to receive two different 2-stage sinus lift procedures using the lateral window approach. In one side, the sinus lining was raised by placing a resorbable rigid Inion barrier without any bone substitute whereas the contralateral side was loosely packed with 100% granular Bio-Oss. After 6 months, 2 to 3 implants were inserted at each side and submerged for 4 months. Implants were loaded with provisional acrylic prostheses and replaced after 4 months, by definitive screwretained metal-ceramic prostheses. Outcome measures were: time necessary to complete the augmentation procedure, bone gain on CT scans, histomorphometry, any complication, implant and prosthetic failures, and clinician and patient preference assessed by a blinded outcome assessor. All patients were followed up to 5 months after loading. Results: No patient dropped out. There was no significant difference in time to complete the augmentation procedure (19.8 minutes for Inion versus 20.5 for Bio-Oss). After 6 months, both interventions gained bone in a highly statistically significant way (14.4 mm for Inion versus 14.1 mm for Bio-Oss) with no significant differences between the procedures. Histologically, more new bone formed at Bio-Oss treated sites (36.1% versus 24.2%), the difference being highly statistically significant (P = 0.002). There were no differences in complications between groups (2 perforations of the maxillary lining at Inion treated sites versus 1 at a Bio-Oss site), however, in one of the patients where a perforation occurred at the Inion site, at implant placement, the sinus was two-thirds filled with soft tissue and the site was successfully retreated with Bio-Oss. No implant failed. The clinician preferred Bio-Oss because it was simpler to handle. There were no statistically significant differences in patient preference 1 month after surgery and 1 month after delivery of definitive prostheses: 8 patients had no preference while 2 preferred the Bio-Oss treated side. Purpose: To evaluate whether short (5 mm) dental implants could be a suitable alternative to augmentation and placement of longer implants (10 mm) in posterior atrophic jaws. Materials and methods: Thirty partially edentulous patients with bilateral posterior edentulism were included: 15 patients having 5 to 7 mm of residual crestal height above the mandibular canal, and 15 patients having 4 to 6 mm of residual crestal height below the maxillary sinus and bone thickness of at least 8 mm measured on a CT scan. The patients were randomised either to receive one to three submerged 5-mm-long Rescue implants (Megagen) or 10-mm-long implants placed in augmented bone according to a split-mouth design. Mandibles were augmented with interpositional anorganic bovine bone blocks (Bio-Oss) and maxillae with granular Bio-Oss placed through a lateral window under the lifted sinus membrane. Resorbable barriers were used to cover the grafted sites. Grafts were left to heal for 4 months before placing the implants using a submerged technique. Four months after implant placement, provisional reinforced acrylic prostheses were delivered and replaced 4 months later by definitive screw-retained metal-ceramic prostheses. Outcome measures were: prosthesis and implant failures, any complications, time needed to fully recover mental nerve function (only for mandibular implants) and patient preference assessed 1 month after loading. All patients were followed up to delivery of the final restorations (4 months after loading). Results: A systematic deviation from the research protocol occurred: the operator used another implant system (EZ Plus, Megagen) for implants 10 mm or longer with a diameter of 4 mm at the augmented sites. No patients dropped out. In 5 patients of the augmented group (all mandibles), there was not enough height to place 10-mm-long implants as planned and shorter implants (7 and 8.5 mm) were used instead. In each group, one prosthesis could not be placed when planned because an implant was found to be mobile at abutment connection: one 5 mm maxillary implant and one 8.5 mm mandibular implant in the augmented group. Five complications occurred: two in the augmented group (one maxillary sinus perforation and one mandibular wound dehiscence after implant placement possibly associated with the failure of one implant) versus three maxillary sinus perforations in the 5-mm-long implant group. The difference was not statistically significant. No patient suffered from permanent disruption of alveolar inferior nerve function, however, significantly more patients had paraesthesia for up to 3 days in the augmented group. There was no statistically significant difference in patient preference with the majority of patients expressing no preference for which treatment they received, finding both of them acceptable. Purpose: To compare the efficacy of two different techniques to augment maxillary sinuses using a lateral window approach: rigid synthetic resorbable barriers (Inion) versus granular anorganic bovine bone (Bio-Oss). Materials and methods: Ten partially edentulous patients having bilaterally 1 to 5 mm of residual bone height and at least 5 mm bone width below the maxillary sinuses, as measured on computed tomography (CT) scans, were randomised to receive two different 2-stage sinus lift procedures using the lateral window approach. In one side, the sinus lining was raised by placing a resorbable rigid Inion barrier without any bone substitute whereas the contralateral side was loosely packed with 100% granular Bio-Oss. After 6 months, 2 to 3 implants were inserted at each side and submerged for 4 months. Implants were loaded with provisional acrylic prostheses and replaced after 4 months, by definitive screwretained metal-ceramic prostheses. Outcome measures were: time necessary to complete the augmentation procedure, bone gain on CT scans, histomorphometry, any complication, implant and prosthetic failures, and clinician and patient preference assessed by a blinded outcome assessor. All patients were followed up to 5 months after loading. Results: No patient dropped out. There was no significant difference in time to complete the augmentation procedure (19.8 minutes for Inion versus 20.5 for Bio-Oss). After 6 months, both interventions gained bone in a highly statistically significant way (14.4 mm for Inion versus 14.1 mm for Bio- Oss) with no significant differences between the procedures. Histologically, more new bone formed at Bio-Oss treated sites (36.1% versus 24.2%), the difference being highly statistically significant (P = 0.002). There were no differences in complications between groups (2 perforations of the maxillary lining at Inion treated sites versus 1 at a Bio-Oss site), however, in one of the patients where a perforation occurred at the Inion site, at implant placement, the sinus was two-thirds filled with soft tissue and the site was successfully retreated with Bio-Oss. No implant failed. The clinician preferred Bio-Oss because it was simpler to handle. There were no statistically significant differences in patient preference 1 month after surgery and 1 month after delivery of definitive prostheses: 8 patients had no preference while 2 preferred the Bio-Oss treated side. Purpose: To evaluate whether short (5 mm) dental implants could be a suitable alternative to augmentation and placement of longer implants (10 mm) in posterior atrophic jaws. Materials and methods: Thirty partially edentulous patients with bilateral posterior edentulism were included: 15 patients having 5 to 7 mm of residual crestal height above the mandibular canal, and 15 patients having 4 to 6 mm of residual crestal height below the maxillary sinus and bone thickness of at least 8 mm measured on a CT scan. The patients were randomised either to receive one to three submerged 5-mm-long Rescue implants (Megagen) or 10-mm-long implants placed in augmented bone according to a split-mouth design. Mandibles were augmented with interpositional anorganic bovine bone blocks (Bio-Oss) and maxillae with granular Bio-Oss placed through a lateral window under the lifted sinus membrane. Resorbable barriers were used to cover the grafted sites. Grafts were left to heal for 4 months before placing the implants using a submerged technique. Four months after implant placement, provisional reinforced acrylic prostheses were delivered and replaced 4 months later by definitive screw-retained metal-ceramic prostheses. Outcome measures were: prosthesis and implant failures, any complications, time needed to fully recover mental nerve function (only for mandibular implants) and patient preference assessed 1 month after loading. All patients were followed up to delivery of the final restorations (4 months after loading). Results: A systematic deviation from the research protocol occurred: the operator used another implant system (EZ Plus, Megagen) for implants 10 mm or longer with a diameter of 4 mm at the augmented sites. No patients dropped out. In 5 patients of the augmented group (all mandibles), there was not enough height to place 10-mm-long implants as planned and shorter implants (7 and 8.5 mm) were used instead. In each group, one prosthesis could not be placed when planned because an implant was found to be mobile at abutment connection: one 5 mm maxillary implant and one 8.5 mm mandibular implant in the augmented group. Five complications occurred: two in the augmented group (one maxillary sinus perforation and one mandibular wound dehiscence after implant placement possibly associated with the failure of one implant) versus three maxillary sinus perforations in the 5-mm-long implant group. The difference was not statistically significant. No patient suffered from permanent disruption of alveolar inferior nerve function, however, significantly more patients had paraesthesia for up to 3 days in the augmented group. There was no statistically significant difference in patient preference with the majority of patients expressing no preference for which treatment they received, finding both of them acceptable. Purpose: To evaluate whether 7-mm-long implants could be a suitable alternative to longer implants placed in vertically augmented bone for the treatment of atrophic posterior mandibles. Materials and methods: Sixty partially edentulous patients having 7 to 8 mm of residual crestal height and at least 5.5 mm thickness measured on a computed tomography scan above the mandibular canal were randomised to receive either two to three submerged 7-mm-long NanoTite External Hex implants (Biomet 3i) or 10-mm or longer implants (30 patients per group) placed in vertically augmented bone. Bone was augmented with anorganic bovine bone blocks (Bio-Oss) using a sandwich technique and resorbable barriers. The grafts were left healing for 5 months before placing the implants, which were submerged. Four months after implant placement, provisional acrylic prostheses were delivered. Definitive screw-retained metal-ceramic prostheses were delivered 4 months later. Outcome measures were: prosthesis and implant failures, any complications, and time needed to fully recover mental nerve sensitivity. All patients were followed up to the delivery of the final restorations (4 months after loading). Results: No patient dropped out. In two patients of the augmented group, there was not enough space to place 10-mm or longer implants as planned and 7-mm-long implants were used instead. The most likely reason for this is that the Bio-Oss blocks fractured in many pieces at placement. One prosthesis could not be placed when planned in the 7-mm group versus three prostheses in the augmented group, because of failure of one implant in each patient. The difference was not statistically significant. All implants were successfully replaced and final prostheses delivered. Four complications (wound dehiscence) occurred during graft healing in the augmented group (one possibly associated with the failure of one implant) versus none in the 7-mm-long implant group. The difference was not statistically significant. No patient suffered from permanent paraesthesia of the alveolar inferior nerve; however, sensitivity was recovered significantly faster in the short implant group. Conclusions: The early results of this study suggest that, when the residual bone height over the mandibular canal is between 7 and 8 mm, 7-mm short implants might be a preferable choice since the treatment is faster, cheaper and associated with less morbidity than vertical bone augmentation. Purpose: To compare the efficacy, complications and patient preference of two different techniques for vertical bone augmentation of the posterior mandible: bone blocks harvested from the iliac crest versus anorganic bovine bone blocks (Bio-Oss®) were used as inlays. Materials and methods: Ten partially edentulous patients, requiring bilateral and vertical bone augmentation of the posterior mandible (having 5 to 7 mm of residual crestal height and at least 5 mm thickness above the mandibular canal to allow for implant placement) had their posterior mandibles randomly allocated to both interventions. Resorbable barriers were used to cover the grafts. After 4 months, implants were inserted, and after a further 4 months, provisional prostheses were inserted. Definitive prostheses were delivered 4 months later. Prosthesis and implant failures, the amount of vertically regenerated bone measured on computerised tomography (CT) scans, any complications, the time needed to fully recover mental nerve sensitivity and patient preference were all recorded. All patients were followed up for up to 1 month after the delivery of the final restorations. Results: Up to 5 months post-loading no patients dropped out or were excluded. Both procedures obtained significant bone gain and achieved the desired results. Four months after grafting, autogenous bone loss was on average 1.1 mm (P=0.088) and Bio-Oss 0.6 mm (P=0.001). There were no statistically significant differences in bone gain and maintenance among the two procedures. The sides treated with Bio-Oss recovered their full mental nerve sensitivity significantly faster than those treated with autogenous bone (4 versus 6.3 days). Three complications occurred during graft healing; two in the autogenous bone group and one determining the complete failure of the augmentation procedure. No implants or prosthesis could be placed in the planed area. There was no statistically significant differences in the occurrence of complications between the procedures. After implant placement one complication occurred in the autogenous bone group (probably as a consequence of a previous complication). Patients significantly preferred the treatment with Bio-Oss: 3 weeks after augmentation seven patients preferred Bio-Oss and three patients found the treatments to be 'equally good' (odds ratio 0.045 [5% confidence interval (CI) 0.00 to 0.87], P=0.04). One month after delivery of the final prostheses, eight patients preferred Bio-Oss and two patients found the treatments to be 'equally good' (odds ratio 0.03 [95% CI 0.00 to 0.64], P=0.02). Conclusions: This pilot study suggests that it might be sensible to use Bio-Oss blocks rather than bone harvested from the iliac crest as the interpositional graft in the treatment of resorbed posterior mandible, as patient discomfort is reduced. Background: Dental rehabilitation with conventional prostheses after microvascular jaw reconstruction is often unsuccessful because of the altered oral anatomy and the lack of adequate dentition for fixation. Purpose This retrospective study evaluated the long‐term outcomes of patients who underwent jaws reconstruction with a fibula free flap and subsequent rehabilitation with implant‐supported prostheses. Materials and Methods: In total, 21 patients were enrolled; 108 osseointegrated dental implants, positioned on 21 fibular free flaps were assessed. Follow‐up examinations were performed according to a standardized protocol, including clinical and radiological evaluations. The mean follow‐up after implant insertion was 90.2 (range, 20‐204) months. Results: The overall implant survival rate was 97.2% at the 12‐month follow‐up, 86.5% at 60 months, and 79.3% at 120 months. The rate of overall implant success was 95.4% at the 12‐month follow‐up, 73.5% at 60 months, and 64.7% at 120 months. Implant failure was more common in patients that had implants placed after they underwent radiation therapy. Peri‐implantitis and mucositis were identified in 14.8% and 20.3% of surviving implants, respectively, at the 5‐ and 10‐year follow‐up. The risk of developing peri‐implantitis was estimated to be higher (odds ratio = 1.5) for patients without connective tissue or skin grafts versus patients with one of them (18.2% vs 9.5%). Conclusions: Long‐term implant‐related outcomes show that the survival rate of implants placed into the fibula is acceptable. Radiotherapy negatively impacts survival and success, in particular in the short and medium‐term follow‐up. Moreover, a relevant peri‐implant bone resorption does occur over time and ultimately influences implants success, and it is mainly related to peri‐implant gingival mucositis. Purpose: The aim of this study was to describe fully digital workflows for the prosthetically guided reconstructive surgery (PGRS) of edentulous patients affected by atrophic maxillae. Case Report: The preoperative provisional removable full denture was used as starting point to determine esthetic parameters, occlusion of the final prosthetic rehabilitation, as well as the respective maxillary implant positions using conventional software for the diagnosis of available bone and the design of the full-arch prosthesis. Four patients were included. The regenerative option (2 patients; bone augmentation using customized titanium mesh) or the long-implant option (2 patients; navigated zygomatic implants) were both digitally designed depending on the patient needs. After implant healing, two sets of digital impressions were acquired: set 1 was the digital impression of implant positions; set 2 was the digitalization of the provisional full denture together with the impression of the rear side. The two sets were digitally combined into a single file. The metal framework of the final prosthesis was designed and manufactured using digital hybrid manufacturing technology and clinically evaluated using the Sheffield test and radiographs. At the same time, a resin prototype was produced for clinical try-in of esthetic and functional parameters. Results: Esthetic parameters and the occlusion of the provisional removable full-denture were digitally transferred to the provisional and definitive full-arch fixed prosthesis. The digital models were 3D-printed in occlusion to finalize the fullarch prosthesis in three steps. Conclusion: The workflows of PGRS allowed realization of the full process of the prosthetic and surgical rehabilitation of edentulous patients with atrophic maxilla using completely digital CAD/CAM technology. Background: This study evaluates implant-borne prosthetic rehabilitation of 10 totally edentulous atrophic maxillae after bone reconstruction with a titanium-mesh technique and particulate bone graft.Methods: Ten atrophic maxillae were reconstructed with 19 titanium meshes and particulate autologous-heterologous bone. Maxillae were rehabilitated at least 5 months, with placement of 67 implants connected to the prostheses after an additional 4 months of rehabilitation. Cases were evaluated retrospectively in terms of complication rates, particularly on the amount of mesh exposure, implant survival, and success rates at the end of follow-up.Results: In seven cases, two meshes were prematurely exposed (within 4 to 6 weeks), and five were exposed later (after 4 to 6 weeks). Only two of the later exposures extended >1 cm(2). Nevertheless, reconstructions allowed implant placement and prosthetic rehabilitation in all cases. Two implants were lost before loading. After mean follow-up at 39.3 (20 to 56) months since prosthetic loading, all 65 implants were functional (100% implant survival), but 15 implants demonstrated a peri-implant mean bone resorption of 2.96 mm increased bone loss, yielding a cumulative implant success rate of 76.9%. No prosthetic problem was observed.Conclusions: Use of titanium mesh with particulate bone can be considered a valid option in reconstructing atrophic maxillae to allow for implant-borne prosthetic rehabilitation. Background: Various sinus-lift techniques have been described in the literature. The aim of this retrospective study is to evaluate the efficacy of a transalveolar sinus-lift technique in terms of implant survival, marginal bone loss, and complications. Methods: A total of 538 patient records were examined, and after applying exclusion and inclusion criteria, a sample of patients was included with a mean follow-up of 5 years. Implants with a smooth or tapered surface were considered. Therefore, only sinus lifts with inorganic bovine bone matrix or demineralized bone matrix were included, and 1,536 periapical radiographs were analyzed. Outcome measures were implant success, implant failure (peri-implantitis and loss of osseointegration), marginal bone resorption, and biologic complications (membrane perforations, sinusitis, and intraoperative and/or postoperative hemorrhage). Periapical radiographs were evaluated before surgery, post-surgery, and after 6 months and 1, 3, and 5 years. We analyzed the residual crestal bone height under the sinus, the amount (mm) of height increase after surgery, and values of implant marginal bone re-sorption for considered follow-ups. Results: Two hundred eighty-two (282) patients were excluded. Therefore, 256 patients treated with the transalveolar sinus-lift technique were included. A total of 376 dental implants and 323 sinus lifts were analyzed. The overall rates of implant success and failure were 94.9% and 5.1%, respectively. The mean bone loss around implants was 1.98 mm (mean follow-up of 5 years). Patients treated with inorganic bovine bone matrix showed a better implant success rate (P = 0.03) than did patients treated with demineralized human matrix. Three Schneiderian membrane perforations occurred in the 323 sinus lifts. In these cases, the surgeon performed another surgical operation after 3 months. Post-operative complications were peri-implantitis (six cases) and osseointegration losses (13 cases). Conclusions: The transalveolar sinus-lift technique was a safe, minimally invasive technique with an implant success rate comparable to traditional implantology. Four complications (wound dehiscence) occurred during graft healing in the augmented group vs. none in the 7 mm group. No complications occurred after implant placement. These differences were not statistically significant. One year after loading, patients of both groups lost an average of 1 mm of peri-implant bone. Objectives: To compare two different techniques for vertical bone augmentation of the posterior mandible: bone blocs from the iliac crest vs. anorganic bovine bone blocs used as inlays. Histomorphometry of samples trephined at implant placement, prosthesis and implant failures, any complication after loading and peri-implant marginal bone-level changes were assessed by masked assessors. All patients were followed up to 1 year after loading. Results: Four months after bone augmentation, there was statistically significant more residual graft (between 10% and 13%) in the Bio-Oss group. There were no statistically significant differences in failures and complications. Two implants could not be placed in one patient augmented with autogenous bone because the graft failed whereas one implant and its prosthesis of the Bio-Oss group failed after loading. After implant loading only one complication (peri-implantitis) occurred at one implant of the autogenous bone group. In 16 months (from implant placement to 1 year after loading), both groups lost statistically significant amounts of peri-implant marginal bone: 0.82 mm in the autogenous bone group and 0.59 mm in the Bio-Oss group; however, there were no statistically significant differences between the groups. Purpose: To compare the efficacy of inlay and onlay bone grafting techniques in terms of vertical bone formation and implant outcomes for correcting atrophic posterior mandibles. Materials and Methods: Twenty surgical sites were assigned to two treatment groups, inlay and onlay, with iliac crest as donor site. After 3 to 4 months, 43 implants were placed and loaded 4 months later. The median follow up after loading was 18 months. Results: For the inlay versus onlay group, median bone gain was 4.9 versus 6.5 mm ( p = .019), median bone resorption was 0.5 versus 2.75 mm ( p < .001), and median final vertical augmentation was 4.1 versus 4 mm ( p = .190). The implant survival rate was 100% in both groups, while the implant success rate was 90% versus 86.9% ( p = .190, not significant). A minor and major complication rate of 20% and 10%, respectively, for both groups was encountered. Conclusions: Inlay results in less bone resorption and more predictable outcomes, but requires an experienced surgeon. In contrast, onlay results in greater bone resorption and requires a bone block graft oversized in height, but involves a shorter learning curve.
