Tropical perturbations have been shown theoretically and observationally to excite long‐range atmospheric responses in the form of Rossby wave teleconnections that result from the equator to pole gradient of planetary vorticity. An extreme teleconnection event occurred during March 2015 in the Southeastern Pacific. As a result, extreme high temperatures were observed in Southwestern South America and the Antarctic Peninsula simultaneously with an extreme rainfall and flood event in the hyperarid Atacama desert. We show that the origin of these seemingly disconnected extreme events can be traced to a Rossby wave response to the strongest Madden‐Julian Oscillation (MJO) on record in the tropical central Pacific. A barotropic wave number 3 to 4 perturbation with group velocity between 15 and 30 m/s is consistent with the trajectory and timing followed by the upper‐level anomalies radiating away from the tropics after the MJO episode. Plain Language Summary: Large atmospheric waves can travel from the tropical regions after being excited by deep convective systems over the oceans. Sometimes this tropical rainfall occurs associated to tropical waves that take from 30 to 60 days to travel around the planet, known as the Madden‐Julian Oscillations. The strongest of such Madden‐Julian wave was observed during March 2015. Here we show that this wave triggered a response that traveled far east and south of the original rainfall heating, producing extreme weather in Antarctica (highest temperature in continental Antarctica observed on record) as well as extreme weather in the Pacific South American coast (major flooding in the Atacama desert). We emphasize that this case illustrates the importance of tropical extremes in setting temperature and precipitation extremes far away from the tropics, including polar regions. This has major consequences for these extreme environments: for the maintenance of the ice shelves in Antarctic Peninsula and for the hyperarid character of the Atacama desert. Abstract: A record‐setting temperature of 17.5°C occurred on 24 March 2015 at the Esperanza station located near the northern tip of the Antarctic Peninsula (AP). We studied the event using surface station data, satellite imagery, reanalysis data, and numerical simulations. The Moderate Resolution Imaging Spectroradiometer Antarctic Ice Shelf Image Archive provides clear evidence for disintegration and advection of sea ice, as well as the formation of melt ponds on the ice sheet surface at the base of the AP mountain range. A deep low‐pressure center over the Amundsen‐Bellingshausen Sea and a blocking ridge over the southeast Pacific provided favorable conditions for the development of an atmospheric river with a northwest‐southeast orientation, directing warm and moist air toward the AP, and triggering a widespread foehn episode. A control simulation using a regional climate model shows the existence of local topographically induced warming along the northern tip of the AP (∼60% of the full temperature signal) and the central part of the eastern AP (>90% of the full temperature signal) with respect to a simulation without topography. These modeling results suggest that more than half of the warming experienced at Esperanza can be attributed to the foehn effect (a local process), rather than to the large‐scale advection of warm air from the midlatitudes. Nevertheless, the local foehn effect also has a large‐scale advection component, since the atmospheric river provides water vapor for orographic precipitation enhancement and latent heat release, which makes it difficult to completely disentangle the role of local versus large‐scale processes in explaining the extreme event. Abstract: We propose a new application of inexpensive light‐emitting diode (LED)‐based Sun photometers, consisting of measuring the aerosol optical thickness (AOT) with high resolution within metropolitan scales. Previously, these instruments have been used at continental scales by the GLOBE program, but this extension is already covered by more expensive and higher‐precision instruments of the AERONET global network. For this we built an open source two‐channeled LED‐based Sun photometer based on previous developments, with improvements in the hardware, software, and modifications on the calibration procedure. Among these we highlight the use of MODTRAN to characterize the effect introduced by using LED sensors in the AOT retrieval, an open design available for the scientific community and a calibration procedure that takes advantage of a CIMEL Sun photometer located within the city, enables the intercomparison of several LED Sun photometers with a common reference. We estimated the root‐mean‐square error in the AOT retrieved by the prototypes as 0.006 at the 564 nm and 0.009 at the 408 nm. This error is way under the magnitude of the AOT daily cycle variability measured by us in our campaigns, even for distances closer than 15 km. In addition to inner city campaigns, we also show aerosol‐tracing applications by measuring AOT variations from the city of Santiago to the Andes glaciers. Measuring AOT at high spatial resolution in urban areas can improve our understanding of urban scale aerosol circulation, providing information for solar energy planning, health policies, and climatological studies, among others. Goldblatt and Zahnle (2011) raise a number of issues related to the possibility that cirrus clouds can provide a solution to the faint young sun paradox. Here we argue that some of the criticism is not warranted. In particular, the criticism related to cirrus clouds being an "end member" case of possible clouds depends heavily on models that may have an inadequate representation of cirrus clouds. Present climate observations show that thin cirrus clouds (optical depth less than 10) can produce positive cloud radiative forcing. When this forcing is represented in models, resulting cirrus clouds are not necessarily realistic. Therefore, cirrus clouds that have a radiative forcing consistent with present climate observations, can provide a solution to the faint young sun paradox, or at least ease the amount of CO2 or other greenhouse substances needed to provide temperatures above freezing during the Archean. The pharmacokinetics of oral nicorandil 20 mg 12 hourly for 9 doses was evaluated in 21 hospitalized patients with angina pectoris due to coronary heart disease and with normal and impaired renal function. Patients were divided into 3 groups based on creatinine clearance (CL): GROUP I ( n=6) > 80 ml/min, GROUP II ( n=8) 20-80 ml/min, and GROUP III ( n=7) < 20 ml/min. After the first dose, the total clearance of nicorandil (CL) value did not change with increasing renal failure and so was not dependent on creatinine clearance. After the last dose CL was 51 l·h in Group I, 44 l·h in Group II and 56 l·h in Group III, and it was not related to creatinine clearance. The percentage of the dose excreted in the urine was 0.4%. No significant difference was noted in any of the other pharmacokinetic parameters examined in the three groups, not even on comparing values obtained on the first and last days of treatment. Slower drug absorption at night can leave residual drug from an evening dose of a sustained-release product remaining to be absorbed at the time of the next morning's dose, thereby giving higher plasma concentrations of the drug during the day than the night. When a capsule product releasing theophylline over 12 h after a morning dose was given repetitively at 8 a.m. and 8 p.m. for 4 days, daytime plasma concentrations from 4 h to 8 h after the dose were about 40% greater than corresponding night-time concentrations, and the mean steady-state concentration during the night-time interval was only 81% of that during the daytime interval. Altering the regimen to one capsule at 12 noon and one at 10 p.m. eliminated all significant differences between a.m. and corresponding p.m. plasma concentrations of theophylline and between the mean steady-state concentrations for each of the interdose intervals within a day. Cyclosporine (CsA) dosing is based on CsA plasma or blood concentrations measured 12 to 24 hours after drug administration (trough levels). This study evaluated the relationship between the timing of CsA concentrations and subsequent pharmacokinetic parameters to predict an optimal sampling period. Plasma samples were obtained from 22 patients before their morning dose of CsA and at 2,4,6,8,10, and 12 hours after the dose on the 7th and on the 21st day after heart transplantation. The plasma samples were assayed by both HPLC and FPIA. The Cmax for CsA was achieved over a period ranging from 2 to 6 hours (mean/median = 4.7/4.0) during the day 7 and the day 21 studies. The mean (±SD) half-life was 3.2 (1.0) hours on day 7 and 2.9 (1.1) hours on day 21, (P > 0.05); the mean apparent oral clearance was 276 (117) L/hr on the day 7 and 269 (209) L/hr on day 21, (P > 0.05). When CsA plasma concentration by either FPIA and HPLC was monitored, the drug concentration best correlated with AUC was found to correspond to the plasma samples taken 4 to 8 hours after drug administration. The authors conclude that trough blood sampling for therapeutic drug monitoring of CsA is not optimal, and that further studies are necessary to correlate concentration monitoring during the dosing interval with pharmacologic and toxicologic parameters. The effects of short term intravenous administration of gallopamil on coronary haemodynamic variables were studied in 10 patients with stable exertional angina and angiographically confirmed coronary artery disease that affected the proximal portion of the left anterior descending artery. Blood flow in the great cardiac vein was measured by a thermodilution technique, both at rest and during ischaemia induced by atrial pacing, before and after intravenous administration of gallopamil (0.02 mg/kg as a bolus dose given over three minutes, followed by an infusion of 0.0005 mg/kg/min). Gallopamil significantly prolonged the mean (SD) duration of pacing that was tolerated (11 (2.6) vs 14.8 (2.4] min, significantly increased the mean (SD) peak heart rate attained during pacing (142 (15) vs 158 (11) beats/min), and reduced mean (SD) arterial pressure (133 (17) vs 116 (17) mm Hg). There were no changes in mean (SD) blood flow in the great cardiac vein (134.1 (57) vs 112.9 (38) ml/min, mean (SD) anterior regional coronary resistance (1.18 (0.6) vs 1.15 (0.5) mm Hg/ml/min), and mean (SD) anterior regional myocardial oxygen consumption (16.6 (6) vs 13.7 (4) ml/min). These data confirm that gallopamil is an effective antianginal agent and suggest that a reduction of myocardial oxygen demand is the predominant mechanism by which the drug exerts its beneficial effects. Here, we argue that: (1) climates having a lower than present mean surface temperature cannot be discarded as solutions to the faint young sun paradox, (2) the detrainment from deep convective clouds in the tropics is a well-established physical mechanism for the formation of high clouds that have a positive radiative forcing (even if the possible role of these clouds as a negative climate feedback remains controversial) and (3) even if some cloud properties are not mutually consistent with observations in radiative transfer parameterizations, the most relevant consistency (for the purpose of hypothesis testing) is with observations of the cloud radiative forcing. Therefore, we maintain that cirrus clouds, as observed in the current climate and covering a large region of the tropics, can provide a solution to the faint young sun paradox, or at least ease the amount of CO2 or other greenhouse substances needed to provide temperatures above freezing during the Archean. The pharmacokinetics of ofloxacin, a fluoroquinolone widely used in the treatment of bacterial infections, may be different according to age, owing to the biological differences that exist between an elderly organism and a young subject, expecially as regards the renal and the hepatic function. In our study the pharmacokinetics of ofloxacin in 12 elderly patients was found to be different from those of 12 healthy young volunteers. The elimination half-life (t) was slightly shorter in the young subjects than in the elderly: 6.2 (0.9)h against 8.5(1.2)h respectively. The oral total clearance was lower in the geriatric patients compared to the young healthy volunteers: 83.3(16.6) ml/min in the first group, and 23.3(33.3) ml/min in the second group. AUC and peak plasma concentration in elderly exceeded those noted in young healthy volunteers. The results of this study suggest that, compared with younger subjects, older patients experience delayed elimination of ofloxacin. It would be reasonable, from a pharmacokinetic point of view, to limit the dose of ofloxacin in patients more than 75 years old, at least to one half of that given to younger patients. Fifteen patients less than 2 months old with bacterial infections caused by pathogens known or presumed to be sensitive to cefoxitin were studied. Cefoxitin was administered as an i.v. bolus injection over 15 min, every 8 h for 6 to 12 days, to a total daily dosage of 90 mg/kg. In 14 patients cefoxitin therapy resulted in eradication of the pathogen and in recovery from clinical signs of infection. Only one patient did not respond to cefoxitin therapy. No adverse clinical or haematological effects definitely caused by cefoxitin were observed. Plasma and urine samples collected after the first dose were assayed for cefoxitin by HPLC. Pharmacokinetic data indicated larger apparent volume of distribution (0.51/kg), a smaller plasma clearance (0.271/h/kg) and a longer half-life (1.43 h) than in adults. The plasma half-life was inversely correlated ( p<0.05) to the postnatal age of the patients. Cefoxitin may be safely used in infants with infections caused by susceptible pathogens.
