A fatality involving verapamil, a calcium channel blocker agent, is presented. A 51-year old male ingested 7200 mg of sustained-release (SR) verapamil at T[sub 0] and died 40 hours later of refractory, mixed shock and multiorgan failure. The symptoms displayed during hospitalization were quite typical and involved altered consciousness, hypotension, bradycardia, atrioventricular block, metabolic acidosis and renal failure. Verapamil and its primary metabolite, norverapamil, were assayed on eight plasma and two urine samples, successively taken between the admission to the ICU (T[sub 0] + 4 hours) and time of death, using an original high-performance liquid chromatography/mass spectrometry (HPLC/MS) procedure with verapamil-d3 as internal standard. Plasma verapamil and norverapamil levels on admission were 0.94 and 1.36 μg/mL, respectively, then verapamil remained practically unchanged throughout the hospitalization (0.85 μg/mL at T[sub 0] + 40 hours). The discussion focuses on the detrimental role of SR formulations in overdose, with special emphasis on the risk of pharmacobezoar development already reported with SR-verapamil. To our knowledge, this is the first report of a verapamil fatality documented by repeated plasma measurements of the drug during the antemortem period. [ABSTRACT FROM AUTHOR]

Dans le traitement des anévrismes situés au niveau de la partie cervicale haute de l'ACI, la technique de pontage cervico-pétreux par greffon veineux “ in situ” (télescopage du greffon veineux dans l'ACI) peut être une alternative aux pontages cervico-pétreux extra-anatomiques (avec passage sous-cutané du greffon veineux), en ayant les avantages d'un pontage anatomique sans nécessité d'une résection extensive de la base du crâne pour positionner en continuité le greffon veineux. Si la technique des pontages extra-intracrâniens prophylactiques implantés sur le territoire de l'a. cérébrale moyenne est actuellement bien codifiée, la réalisation des pontages veineux extra-intracrâniens caroti-do-carotidiens posent de nombreuses difficultés techniques qui n'ont pas toutes été résolues. En particulier, le taux de perméabilité des greffons veineux diminue de façon significative après 10 ans et ce en rapport avec le développement d'une hyperplasie intimale par surcharge hémodynamique [11]. Un des intérêts potentiels du pontage “ in situ” serait de protéger le greffon veineux, la contrainte hémodynamique étant prise en charge par la paroi artérielle receveuse. [ABSTRACT FROM AUTHOR]

Purpose Post-operative endophthalmitis remains the most severe complication of cataract surgery. The purpose of this study was to determine the incidence of post-operative endophthalmitis in Languedoc Roussillon between 2010 and 2014. Secondary objective was to assess the correlation between the use of intracamerular injection of cefuroxime and post-operative endophthalmitis incidence. Methods All patients >40 years old who underwent a primary cataract surgery in Languedoc Roussillon (France) between January 2010 and December 2014 were collected from the regional administrative database of medical care. Incidence rate of post-operative endophthalmitis was estimated. Spearman coefficient was used to assess the correlation between the incidence of post-operative endophthalmitis and the use of intracamerular cefuroxime at the end of cataract surgery. Results In 5 years, 179 515 eyes underwent a cataract surgery in Languedoc Roussillon. A total of 182 patients had a post-operative endophthalmitis owing to a 1 year incidence of 0.10% after cataract surgery. The incidence of endophthalmitis decreased over the study: 0.16%, 0.12%, 0.11%, 0.07% and 0.05% in 2010, 2011, 2012, 2013 and 2014, respectively. The use of cefuroxime prophylactic injection at the end of cataract surgery increased over the study: 3.27%, 7.65%, 32.04%, 75.67% and 85.36% in 2010, 2011, 2012, 2013 and 2014, respectively. The incidence of post-operative endophthalmitis was negatively correlated with the use of intracamerular injection of cefuroxime (r = −0.96, p = 0.008). Conclusions This study provides information on the decrease of post-operative endophthalmitis incidence and its correlation with the increased use of cefuroxime intracamerular injection to prevent the risk of infection in the real world. Purpose The aim of this study was to assess the relation between retinal vascular caliber and renal function in normotensive (systolic/diastolic blood pressure <140/90 mmHg) and never‐treated uncomplicated hypertensive subjects. Methods Eighty subjects (51 normotensive and 29 hypertensive , mean age 47 yrs and 51% female) with serum creatinine < 1.2 mg/dL and without diabetes were recruited. Retinal vascular calibers were measured from fundus photographs and expressed as central retinal artery and venular equivalent (CRAE and CRVE, respectively). Renal function was assessed by measurement of glomerular filtration rate (GFR, urinary clearance of 99mTc‐DTPA) and urinary albumin/creatinine ratio (UACR). Results Mean GFR was 117 ml/min/1.73m². The adjusted mean CRAE was smaller in the hypertensive group as compared to the normotensive group (mean±standard error: 135±24µm vs. 142±18µm; P=0.03); whereas adjusted mean CRVE was similar. In all participants, CRAE and CRVE were positively correlated to GFR (r²=0.09, P=0.005 and r²=0.09, P=0.006, respectively). In addition, CRAE was negatively correlated to UACR (r²=0.12, P=0.02) and no significant relationship between CRVE and UACR was found (r²=0.01, P=0.31). The observed relations between retinal vascular calibers (CRAE and CRVE) and renal function parameters (GFR and UACR) remained significant after adjusting for age, gender, mean blood pressure, smoking, glycemia, body mass index and lipid profile. Conclusion In normotensive and never‐treated hypertensive subjects with normal renal function, the decrease in retinal arteriolar and venular calibers was associated with reduced kidney function, suggesting common determinants of these preclinical target organ damages.
