Stable isotope ratios and fatty acid signature analyses were employed to examine the fine-scale population structure of a year-round resident population of 600-800 bottlenose dolphins ( Tursiops truncatus) in the Indian River Lagoon (IRL), Florida. The IRL, a 250-km-long estuary running along the central east coast of Florida (28.0°N, 80.6°W), is comprised of the northern and southern IRL, Mosquito Lagoon (ML), Banana River (BR), and St. Lucie Estuary. Samples of skin and blubber were collected from dead stranded ( n = 61, 1994-2004) and live dolphins ( n = 153, 2002-2007, 2010, 2011) from throughout the IRL and surrounding environs. Using stable isotopes (SI), dolphins could be assigned to a ML subpopulation, a St. Lucie Estuary subpopulation, and an IRL subpopulation. Fatty acid signature analysis (FASA) allowed for finer resolution, detecting ML and BR subpopulations, a separation of northern and southern IRL subpopulations, and a St. Lucie Estuary subpopulation. Differences between sexes were detectable within subpopulations using FASA, but not using SI. This may indicate that males and females are foraging in similar locations at a similar trophic level (detected using SI), but are varying in the types or proportions of specific prey (indicated by FASA). Background 'Silent' cerebral infarction and stroke are complications of transcatheter aortic valve implantation (TAVI). Objective To assess the occurrence of cerebral infarction, identify predictive risk factors and examine the impact on patient health-related quality of life (HRQoL). Methods Cerebral diffusion weighted MRI of 31 patients with aortic stenosis undergoing CoreValve TAVI was carried out. HRQoL was assessed at baseline and at 30 days by SF-12v2 and EQ5D questionnaires. Results New cerebral infarcts occurred in 24/31 patients (77%) and stroke in 2 (6%). Stroke was associated with a greater number and volume of cerebral infarcts. Age (r=0.37, p=0.042), severity of atheroma (arch and descending aorta; r=0.91, p<0.001, r=0.69, p=0.001, respectively) and catheterisation time (r=0.45, p=0.02) were predictors of the number of new cerebral infarcts. HRQoL improved overall: SF-12v2 physical component summary increased significantly (32.4±6.2 vs 36.5±7.2; p=0.03) with no significant change in mental component summary (43.5±11.7 vs 43.1±14.3; p=0.85). The EQ5D score and Visual Analogue Scale showed no significant change (0.56±0.26 vs 0.59±0.31; p=0.70, and 54.2±19 vs 58.2±24; p=0.43). Conclusion Multiple small cerebral infarcts occurred in 77% of patients with TAVI. The majority of infarcts were 'silent' with clinical stroke being associated with a both higher infarct number and volume. Increased age and the severity of aortic arch atheroma were independent risk factors for the development of new cerebral infarcts. Objectives To review the evidence for the efficacy and safety of biological agents in patients with rheumatoid arthritis (RA) to provide data to develop treatment recommendations by the European League Against Rheumatism (EULAR) Task Force. Methods Medline, Embase and Cochrane databases were searched for relevant articles on infliximab (IFX), etanercept (ETN), adalimumab (ADA), certolizumab-pegol (CZP), golimumab (GLM), anakinra (ANA), abatacept (ABT), rituximab (RTX) and tocilizumab (TCZ) published between 1962 and February 2009; published abstracts from the 2007–2008 American College of Rheumatology (ACR) and EULAR conference were obtained. Results 87 articles and 40 abstracts were identified. In methotrexate (MTX) naïve patients, biological therapy with IFX, ETN, ADA, GLM or ABT has been shown to improve clinical outcomes (level of evidence 1B). In MTX/other synthetic disease-modifying antirheumatic drug (DMARD) failures all nine biological agents confer benefit (1B), with lower efficacy noted for ANA. RTX, ABT, TCZ and GLM demonstrate efficacy in tumour necrosis factor inhibitor (TNFi) failures (1B). Less evidence exists for switching between IFX, ETN and ADA (3B). Biological and MTX combination therapy is more efficacious than a biological agent alone (1B). A safety review shows no increased malignancy risk compared with conventional DMARDs (3B). TNFi are generally associated with an increased risk of serious bacterial infection, particularly within the first 6 months of treatment initiation; increased tuberculosis (TB) rates with TNFi are highest with the monoclonal antibodies (3B). Conclusions There is good evidence for the efficacy of biological agents in patients with RA. Objective: To assess the effects of population tobacco control interventions on social inequalities in smoking. Data sources: Medical, nursing, psychological, social science and grey literature databases, bibliographies, hand-searches and contact with authors. Study selection: Studies were included (n = 84) if they reported the effects of any population-level tobacco control intervention on smoking behaviour or attitudes in individuals or groups with different demographic or socioeconomic characteristics. Data extraction: Data extraction and quality assessment for each study were conducted by one reviewer and checked by a second. Data synthesis: Data were synthesised using graphical ("harvest plot") and narrative methods. No strong evidence of differential effects was found for smoking restrictions in workplaces and public places, although those in higher occupational groups may be more likely to change their attitudes or behaviour. Smoking restrictions in schools may be more effective in girls. Restrictions on sales to minors may be more effective in girls and younger children. Increasing the price of tobacco products may be more effective in reducing smoking among lower-income adults and those in manual occupations, although there was also some evidence to suggest that adults with higher levels of education may be more price-sensitive. Young people aged under 25 are also affected by price increases, with some evidence that boys and non-white young people may be more sensitive to price. Conclusions: Population-level tobacco control interventions have the potential to benefit more disadvantaged groups and thereby contribute to reducing health inequalities. [ABSTRACT FROM AUTHOR]

The dissociative chemisorption of H2(D2) at a Fe(110) surface has been studied as a function of translational energy ET, internal energy Ei, incident angle θi, and surface temperature Ts. Adsorption is activated, the sticking probability increasing steadily with translational energy with no evidence of a threshold for dissociation. Within experimental error there is no isotope effect or surface temperature dependence (180 Ks<400 K). Using seeded beams at constant translational energy, sticking on a clean surface is insensitive to the internal state distribution of the incident molecules, consistent with a barrier to dissociative chemisorption in the entrance channel. For translational energies below 0.2 eV sticking deviates from normal energy scaling, momentum parallel to the surface strongly inhibiting dissociative chemisorption. The initial sticking coefficient for dissociative chemisorption of 2 on Fe(110) has been measured as a function of translational energy, incidence angle, and surface temperature. Dissociation is activated at translational energies above 100 meV, with the sticking probability rising from 0.26 at low energy to >0.9 for energies above 400 meV. At low energies the sticking probability increases as the surface temperature and the normal component of the momentum are reduced, indicating dissociation proceeds via trapping into a molecular precursor state. The initial saturation coverage at room temperature, calibrated from the ordered LEED phases, is 2.2 monolayers with both the saturation coverage and the uptake kinetics being sensitive to the surface temperature. We analyzed tissue samples from bottlenose dolphins (Tursiops truncatus) that had stranded on beaches in Texas and Florida over a 1-year period starting in September 1991. The concentrations of 10 elements plus methyl mercury (MeHg) were determined in brain, kidney, and liver, and we examined these results for differences based upon age, site, sex, and tissue type. A strong inverse relationship between total mercury (Hg) and the percentage that was MeHg was found in liver, kidney, and brain tissue, presumably due to demethylation of MeHg. A threshold concentration was found for total Hg in brain tissue, indicating that most Hg was present as MeHg up to about 8 years of age. Increases in total Hg after this age were accompanied by an increase in theratio of total Hg to MeHg, indicating demethylation. Strong relationships were found between total Hg in liver and age and between total Hg and selenium in liver, which have been observed before in many fish- and squid-eating marine mammals. The only difference based on sex of the animals was observed for MeHg, which was higher in females andcontrary to the pattern often observed for organic contaminants. Several elements (copper, Hg, lead, zinc) exhibited intersite differences, which were not consistent. Bottlenose dolphin from Florida exhibited the highest levels of MeHg and total Hg, while animals from Texas exhibited the highest levels of lead, copper, and zinc. The essentialelements copper and zinc were expected to be the same for the Texas and Florida animals; however, observed differences may indicate population differences in basic physiological levels, dietary intake, or health status.
