To prevent reddening on the patient's surface during radiotherapy treatment with MV X-ray beams, the surface dose must be low. In this study, we aimed to assess the feasibility of a flat ionization chamber for surface dose measurement. We used a medical linear accelerator capable of producing 6 FF and 6 FFF beams for three collimator sizes. We used Tref and Markus chambers, slab phantoms made of both acrylic and polystyrene from PTW. The thickness of the slabs was 1 mm, 2 mm, 5 mm, and 10 mm. First, the study evaluated basic dosimetric characteristics such as the linearity, reproducibility, and dose rate dependency of the Tref chamber. Next, we measured the surface dose using the Tref chamber and compared it with the Markus chamber. We found that the linearity, reproducibility, and dose rate dependency results were all within 0.5%. In both detectors, for the FF and FFF, SDs revealed a maximum difference of 8%. We observed an average surface dose difference of 10% between both chambers. Compared to Markus Chamber’s results, Tref overestimated the surface dose. The Tref chamber, if associated correction factors are available to account for its overestimation in the buildup region, is to be an alternative solution for surface dose measurement. The hypofractionated radiotherapy modality was established to reduce treatment durations and enhance therapeutic efficiency, as compared to conventional fractionation treatment. However, this modality is challenging because of rigid dosimetric constraints. This study aimed to assess the impact of multi-leaf collimator (MLC) widths (10 mm and 5 mm) on plan quality during the treatment of prostate cancer. Additionally, this study aimed to investigate the impact of the MLC mode of energy on the Agility flattening filter (FF), MLC Agility-free flattening filter (FFF), and MLCi2 for patients receiving hypofractionated radiotherapy. Two radiotherapy techniques; Intensity Modulated Radiotherapy (IMRT) and Volumetric Modulated Arc Radiotherapy (VMAT), were used in this research. In the present study, computed tomography simulations of ten patients (six plans per patient) with localized prostate adenocarcinoma were analyzed. Various dosimetric parameters were assessed, including monitor units, treatment delivery times, conformity, and homogeneity indices. To evaluate the plan quality, dose-volume histograms (DVHs) were estimated for each technique. The results demonstrated that the determined dosimetric parameters of planning target volume (PTV)p (such as D mean, conformity, and homogeneity index) showed greater improvement with MLC Agility FF and MLC Agility FFF than with MLCi2. Additionally, the treatment delivery time was reduced in the MLC Agility FF (by 31%) and MLC Agility FFF (by 10.8%) groups compared to the MLCi2 group. It is concluded that for both the VMAT and IMRT techniques, the smaller width (5 mm) MLCs revealed better planning target volume coverage, improved the dosimetric parameters for PTV, reduced the treatment time, and met the constraints for OARs. It is therefore recommended to use 5 mm MLCs for hypofractionated prostate cancer treatment due to better target coverage and better protection of OARs. [ABSTRACT FROM AUTHOR]

Purpose: The flattening filter (FF) has traditionally been used to flatten beams or create uniform fields in conformal and intensity‐modulated radiation therapy (IMRT) but reduces the dose rate. Many studies have confirmed improvement in delivery efficiency by removing the FF, also known as flattening filter free (FFF), especially for small field stereotactic body radiation therapy (SBRT); but it is unclear if large treatment fields still favor the FFF beam. We propose a novel, unified approach to quantify delivery efficiency of the FFF and flattened beams. Methods: We modeled the FF effect by inverse conical filters and systematically studied delivery efficiency (beam‐on time, BOT) by varying the filter thickness, including the FF and FFF mode. We formulated the BOT of different beams for any arbitrary fluence map in linear programming to solve the optimal inverse conical filter that minimizes the BOT. One‐dimensional optimal filters of minimum BOT were also derived in closed form for conical fluence to gain insight for arbitrary clinical fluence maps. We evaluated the BOT of the FFF beam and flattened beam for conformal treatment fields of various dimensions ranging from 5 cm × 5 cm to 25 cm × 25 cm. We also analyzed the BOT for 698 clinical IMRT prostate fluence maps of field size 10 cm × 10 cm, 17 head‐and‐neck fluence maps of field size 15 cm × 15 cm, and additional realistic test data from 90° rotation and up to 40 cm × 40 cm enlargement of these clinical fluence maps, which were all initially generated with flattened beams. Results: The FFF beam minimized the BOT for A field size less than 20 cm in single leaf pair cases and for conformal fields of dimension less than 20 cm × 20 cm. The FFF beam also minimized the BOT for all tested prostate and head‐and‐neck cases. The median BOT ratios of the FFF beam to the flattened beam were 0.56 and 0.61 for prostate and head‐and‐neck cases, respectively. The FFF beam minimized the BOT for field size up to 30 cm × 30 cm and had similar BOTs to those of the flattened beam for field size greater than 30 cm × 30 cm in those clinically realistic test data. Conclusion: The filter modeling and BOT calculation enable us to quantify delivery efficiency of the FFF beam and flattened beam in a unified approach. The FFF beam minimized the BOT both theoretically and in simulations for all clinically relevant field sizes and fluence maps in IMRT. The results for conformal fields imply that the FFF beam requires less BOT than the flattened beam for volumetric modulated arc therapy (VMAT) treatments. The delivery efficiency consideration favors the FFF beam in intensity‐modulated treatments and may eventually lead to removal of the FF in all future linear accelerator head designs. Purpose: Addition of high Z implants in the treatment vicinity or beam path is unavoidable in certain clinical situation. In this work, we study the properties of radiation interaction parameters such as mass attenuation coefficient (MAC), x ray beam transmission factor (indirect beam attenuation), interface effects like backscatter dose perturbation factor (BSDF) and forward dose perturbation factor (FDPF) for flattened (FF) and unflattened (UF) x ray beams. Methods: MAC for stainless steel and titanium alloy was measured using CC13 chamber with appropriate buildup in narrow beam geometry. The x ray beam transmission factors were measured for stainless steel and titanium alloy for different field size, off‐axis, and depths. Profile analysis was performed using a radiation field analyzer (RFA) as a function of field size and depth to study the influence of phantom scattering and spectral variation in the beam. In addition, interface effects such as BSDF and FDPF were measured with gafchromic films at maximum BSDF peak position calculated using Acuros XB algorithm and with PPC40 chamber measured at exit side of high Z material, respectively. Results: The MAC in both cases decreases with increase in energy for stainless steel (SS) and titanium (Ti) alloy. The MAC increases with the change in x ray beam type from flattened to UF beam because of relatively lower mean energy. The x ray beam transmission factor increases with the increase in energy, field size, and depth owing to increase in penetration power phantom scatter, respectively. The measured BSDF and FDPF were found to be in good agreement with AXB algorithm. Conclusion: The dosimetric properties of x ray photon beam were studied comprehensively in the presence of high Z material like stainless steel and titanium alloy using both flattened and UF beams to understand and incorporate the findings of various parameters in clinical condition due to the variation in energy spectrum from FF to UF x ray beam.
