ABSTRACT Objective: To compare the prevalence of smokers among patients with chronic low back pain, in the presence and absence of Modic changes, also the correlation between smoking history and progression of the Modic scale. Methods: Observational study, case-control type, with the inclusion of 340 vertebral segments in a total of 68 patients, separated into groups: with Modic (case group) and without Modic (control group). The odds ratio between the groups was verified using the Chi-Square test. Degree of correlation between smoking history (packs/year) and the degree of disc degeneration using Max-Modic and Sum-Modic, using Spearman’s non-parametric test. Results: The Modic group (MG) was 54% female and 46% male, with an average smoking history of 13.84 pack-years and an average of 1.42 altered segments per patient. Conclusion: An increased risk for Modic changes was found among smoking patients (odds ratio [OR] 4.09; 95% CI, 1.26-12.31; p < 0.01) and significant correlation between Max-Modic, sum-Modic and smoking history. ABSTRACT Objective: The plantar pressure distribution can be assessed quantitatively by computerized baropodometry such as carpet or insole. An insole-type system with wireless transmission was developed and plantar pressure results were previously validated by force platform. However, the reproducibility of the system had not been determined. Our objective was to evaluate the reliability of the results in different gait cycles, clinical characteristics and in different plantar anatomical sites. Methods: 41 healthy adults (age, 34 ± 13 years; body mass index, 25 ± 5 kg/m2; 26 [63%], male, 26 [63%] practicing physical activity) were evaluated. Baropodometer evaluations were performed in 3 walking cycles with 100 m each, and the reliability between the cycles was examined. Pressure points on the heel, first metatarsal, fifth metatarsal and total plantar pressure were analyzed and compared. Results: Moderate agreement was identified between the second and third cycles (ICC, 0.66; 95% CI, 0.14-0.83). Physical activity practitioners showed higher total plantar pressure (70.8 vs 68.2 Kpa; p = 0.04) and higher pressure in the heel (70.7 vs 68.1 Kpa; p = 0.036) in relation to sedentary ones. Conclusion: The insole was able to assess plant pressure with moderate reliability from the adaptation period. Level of Evidence III, Case control study - Investigating a diagnostic test.

ABSTRACT Objective: This article aimed to evaluate the efficacy of arthroereisis using synthetic polyethylene implants to correct excessive valgus deformity in pediatric patients with flexible valgus flatfoot. Methods: Retrospective study of 20 pediatric patients with flexible valgus flatfoot, totaling 23 feet, between five and 14 years old, operated between January 2009 and July 2016. Clinical evaluations were performed by the Valenti podoscopic classification and the American Orthopaedic Foot & Ankle Society criteria, based on radiographic images and podoscopic analysis. These patients underwent surgical treatment with the introduction of a synthetic implant in the sinus tarsi. Multiple linear regression analysis with Backward selection of variables, angles of pre and postoperative of radiographic images of the patients submitted to arthroereisis were performed. Results: The arthroereisis with interposition of synthetic material was satisfactory, considering that 91% of the cases presented clinical and radiographic improvement, with correction of angles and improvement in deformity degrees. Two cases presented implant loosening. The variables of the Bordelon and Pitch angles significantly influenced (p < 0,05) the improvement of the correction of deformity degrees. Conclusion: The arthroereisis with the interposition of synthetic polyethylene material showed to be an effective technique for flexible flatfoot in symptomatic pediatric patients. ABSTRACT Objective: To evaluate the socioeconomic impact of motorcycle accidents of a metropolis with one million inhabitants treated by a university hospital in 2017. To study hospital cost and time of victims, evaluate definite and incapacitating sequelae, and analyze patient’s insertion in the labor market six months after the accident. Methods: Retrospective study of 62 victims of motorcycle accidents treated in the emergency room and referred for surgical procedure in a university hospital. Data were collected from medical records and answers via telephone six months after the trauma. Results: Injuries related to motorcycle accident resulted in high hospital costs. Average loss per patient was R$ 17,555. Of those who answered the functional impact questionnaire, 64% were on paid leave by INSS and 84% presented partial or permanent physical disabilities after the accident. Only 9 people (36%) received any kind of financial benefit from public authorities. Conclusion: High costs are directly related to hospitalization time and surgical needs. Most victims needed financial help and had their social lives aggravated. Investments in motorcyclists’ awareness of traffic laws and use of safety equipment are needed, as well as in road infrastructure. Level of evidence II, Prognostic studies.

ABSTRACT Objective: To evaluate the epidemiological profile of motorcycle accident victims in a metropolis with more than one million inhabitants attended in a university hospital of reference in 2017. Methods: a retrospective study through the analysis of medical records of 105 motorcycle accident victims in Campinas (SP) attended in a university hospital of reference and who needed surgical procedure in 2017. Results: 87 patients (82.9%) were men. Multiple fractures were observed in 61 patients (58.1%) and polytrauma was found in 14 patients (13.3%). Tibial fracture was the most frequent, present in 65 cases (61.9%). Exposed fractures occurred in 68 patients (64.7%). Among the polytrauma victims, the most frequent injury was traumatic brain injury (TBI), present in seven patients (6.6%). The mean age was 29.8 years (range 6-63 years). The average length of hospital stay was 14 days (1-87). Conclusion: It is essential to investigate and evaluate the victims’ epidemiological profile, as well as the resulting injuries, in order to provide adequate support for the implementation of measures aimed at primary prevention and awareness of the most affected groups. OBJETIVO: Estudar o ombro destes indivíduos através de ressonância magnética para detectar lesões clínicas ou subclínicas para instituir programas específicos de reabilitação. MÉTODOS: Foram avaliados nove pacientes lesados medulares, com seguimento no Laboratório de Biomecânica e Reabilitação do Aparelho Locomotor HC/UNICAMP. Foram divididos em dois grupos de acordo com a presença de paraplegia ou tetraplegia e avaliados através de anamnese e exame físico para correlação com o exame de imagem. RESULTADOS: 41% dos ombros avaliados com imagens de ressonância apresentaram resultado normal. Dentre os resultados alterados, a tendinopatia do supra-espinhal e a degeneração acrômio-clavicular foram às alterações mais encontradas e 80% dos ombros com exame alterado possuiam mais de uma lesão associada. CONCLUSÃO: Pela amostra do estudo é possível identificar uma etiologia multivariada da omalgia, com particularidades para indivíduos paraplégicos e tetraplégicos. A inclusão rotineira da avaliação clínica e radiológica do ombro nos indivíduos lesados medulares pode contribuir para a evolução das técnicas de reabilitação e redução tanto das lesões músculo-articulares quanto dos sintomas álgicos. Nível de Evidência II, Desenvolvimento de critérios diagnósticos em pacientes consecutivos com padrão de referência "ouro" aplicado.OBJECTIVE: To study the shoulder of this group of patients using magnetic resonance imaging to detect clinical and subclinical disorders and establish a rehabilitation program. METHODS: Nine patients with spinal cord injury followed in the Laboratory of Biomechanics and Rehabilitation of the Locomotive System at HC/UNICAMP were divided into two groups according to the presence of paraplegia and tetraplegia and were clinically assessed for correlation with the imaging exams. RESULTS: Normal results were found in 41% of the shoulders. Most common injuries were tendinopathy of the supraspinatus and acromioclavicular joint degeneration. Eighty percent of injured shoulders had combined lesions. CONCLUSION: A great variety of causes of shoulder pain was identified in paraplegic and tetraplegic subjects. Routine clinical assessment and imaging studies of the shoulder may contribute to the evolution of rehabilitation and reduction of pain and musculoskeletal disorders. OBJETIVO: Avaliar a estratégia das transferências de paraplégicos da cadeira de rodas. MÉTODOS: Participaram do estudo doze sujeitos lesados medulares (T2 a T12), aptos a realizar independentemente a transferência da cadeira de rodas para um tablado com um metro quadrado (m²) de área por meio metro de altura. As imagens de marcadores reflexivos nos pontos anatômicos foram capturadas por seis câmeras de infravermelho ProReflex e processadas através de um software específico (QTrac). Parâmetros cinemáticos do tronco, cabeça, ombros e cotovelos foram avaliados. RESULTADOS: Os dados analisados comparam o lado preferencial do sujeito para realizar a transferência com o lado não preferencial, de acordo com as funções desempenhadas de cada segmento corporal. O deslocamento angular da cabeça no plano sagital (y-z) e ombros no plano transversal (x-y) mostraram significâncias estatísticas (pOBJECTIVE: To evaluate the transfer strategy of paraplegic subjects from their wheelchairs. METHODS: Twelve thoracic spinal cord injured subjects participated in this study (T2 to T12). The subjects were able to independently transfer from a wheelchair to a one square meter (m2) table, half a meter in height. Images of reflexive anatomic markers were captured by six ProReflex infrared cameras and processed using a QTRac Capture software. Kinematic parameters of the trunk, head, shoulders and elbows were evaluated. RESULTS: The data analyzed compared the subjects' preferential side for performing transfers, according to the functions performed by each body segment. O objetivo desta pesquisa foi investigar as variáveis cardio-respiratórias (Pa, FC, VO2, VCO2 e Ve) durante a Estimulação Elétrica Neuromuscular (EENM) do quadríceps em portadores de lesão medular. Participaram da pesquisa dez pacientes (cinco paraplégicos e cinco tetraplégicos). O protocolo do teste consistiu em 10 minutos de repouso, 20 minutos de EENM dos quadriceps e 10 minutos de recuperação. Durante a EENM foram constatados baixos valores de VO2 e VCO2. Os paraplégicos apresentaram rápida cinética dos gases e os tetraplégicos lenta cinética dos gases. Houve o aumento da Pa sistólica e da FC. Ainda, os valores das variáveis cardio-respiratórias foram inversamente relatadas para o nível de lesão, ou seja, quanto maior o nível de lesão, menor os valores. Portanto, a maioria dos pacientes apresentaram algumas limitações nas respostas cardio-respiratórias, indicando realização de exercício exaustivo, mas apresentaram capacidade de realização de exercício induzido artificialmente, possivelmente devido aos benefícios da EENM.The objective of this study was to investigate cardiorespiratory responses (Heart Rate, Blood Pressure, VO2, VCO2 e Ve) to Neuromuscular Electrical Stimulation (NMES) of the quadriceps in patients with spinal cord injury. Ten patients (five paraplegics and five tetraplegics) participated in this study. The protocol of the test consisted of ten minutes of rest, twenty minutes of NMES of the quadriceps and ten minutes of recovery. The findings in this study indicated that, during NMES, the patients demonstrated low levels of VO2 and VCO2 and slow gas kinetics for tetraplegic individuals, and a fast gas kinetics for paraplegic individuals. Moreover, there were increases in blood pressure and heart rate. Cardiorespiratory responses increased with descending spinal cord injury level, meaning that the more severe the lesion, the lower the values. Therefore, most of the patients presented some limitations in cardiorespiratory responses, indicating the performance of exhaustive exercise, but the use of NMES can elicit improvements in exercise tolerance due to its benefits.

A Biomecânica, atualmente, é caracterizada por novos procedimentos para análise do movimento humano e por novas técnicas de medição, armazenamento e processamento de dados, contribuindo para os avanços na prática ortopédica. A temática proposta no presente trabalho baseia-se na aplicação da biomecânica ortopédica em diferentes quadros clínicos, através de avaliações quantitativas do movimento dos membros superiores e inferiores, além da criação de novas técnicas de medição. Três exemplos são mostrados neste artigo: (I) análise tridimensional do movimento de abdução do ombro; (II) análise da marcha de indivíduos com lesão no ligamento cruzado anterior; e (III) o desenvolvimento de instrumentação eletrônica para dispositivos de auxílio, ou seja, bengalas e muletas instrumentalizadas para o estudo das relações de força entre os membros superiores e inferiores, durante a marcha de pacientes com lesão ortopédica e neurológica.Biomechanics is nowadays characterized by new procedures for human movement analysis and by modern measurement techniques, data processing and storage, contributing to the advances in the orthopedic practice. The proposed thematic in this work is based on the application of orthopedic biomechanics to different clinical cases, through quantitative evaluation of the upper and lower limb movement and the development of new assessment techniques. Three examples are shown in this paper: (I) a tridimensional analysis of the abduction movement of the shoulder; (II) a gait analysis in individuals with anterior cruciate ligament injury; and (III) the development of electronic instrumentation for walking aids i.e., instrumented canes and crutches for the study of the relationship between the upper and lower limb loads during gait of orthopedic and neurologic patients.

OBJETIVO: Os pacientes lesado-medulares passam a se submeter a uma nova conformação de forças sobre as articulações. O quadril é uma das mais afetadas, por utilizarem a cadeira de rodas como meio de locomoção. Alterações osteoarticulares, como ossificação heterotópica, podem ser encontradas nesses pacientes, sendo evidenciadas por estudos radiográficos. Este estudo visa identificar a incidência das alterações radiográficas em quadris de paciente lesados medulares. MÉTODOS: Foram avaliados 15 pacientes (30 quadris) acompanhados no Laboratório de Reabilitação Biomecânica do Aparelho Locomotor do HC-Unicamp, analisando-se radiografias da bacia em posições antero-posterior e lowenstein. RESULTADOS: Dos quadris avaliados, apenas sete (23%) não possuíam alguma evidência de dano à superfície articular. A prevalência de ossificação heterotópica encontrada (16,6%) aproximou-se a da literatura. CONCLUSÃO: Devido à prevalência de alterações articulares encontradas, justifica-se o acompanhamento radiográfico dos quadris destes pacientes. Nível de Evidência II. (com padrão de referência "ouro" aplicado).OBJECTIVE: Patients with spinal injury patients are submitted to a new kind of force on their joints. The hip is one of the most affected joints, because these usually use wheelchairs to move around. Osteoarticular changes, like heterotopic ossification, can be seen in these patients, as shown in radiographic studies. This study aims to identify radiographic changes in hips of spinal injured patients. METHODS: 15 patients (30 hips) were analyzed, who were in follow-up at the Biomechanic Reabilitation of Osteoarticular System Laboratory of HC-Unicamp. Their hip x-rays were analyzed in two positions (anteroposterior and Lowenstein). RESULTS: Of the total hips, only seven (23%) had no evidence of articular damage. The prevalence of heterotopic ossification found (16.6%) was similar to the literature. CONCLUSION: The radiographic assessment of these patient's hips is justified by the prevalence of joint changes found. OBJETIVO: Analisar e identificar possíveis adaptações da marcha em indivíduos com diagnóstico de instabilidade patelofemoral objetiva, durante a atividade de subida e descida de escada. MÉTODOS: Foram analisados um grupo controle (grupo A), composto por 9 mulheres com média de idade de 25 anos (±1,87), média de altura de 1,62m (±0,05) e média de peso de 56,20kg (±7,34); e, um grupo de 9 mulheres com instabilidade patelofemoral objetiva (grupo B), média de idade de 24 anos (±6,02), média de altura de 1,62m (±0,06) e média de peso de 60,33kg (±10,31). Os grupos foram submetidos a uma análise cinemática, onde as voluntárias subiram e desceram degraus, em uma área previamente selecionada. As imagens foram obtidas por seis câmeras (Qualysis) e a análise dos dados foi realizada através do programa Q gait. RESULTADOS: O grupo B apresentou, no período de apoio, menor flexão do joelho durante a subida (p=0,0268), além de menores velocidade (p=0,0076/ p=0,0243) e cadência (p=0,0027/ p=0,0165) na subida e na descida, respectivamente. CONCLUSÃO: Sugere-se que o grupo B utilizou adaptações funcionais como redução da flexão do joelho, da velocidade e da cadência, durante a subida e a descida de degraus.OBJECTIVE: To analyze and to identify possible gait adaptations by individuals with objective patellofemoral instability when climbing up/down stairs. METHODS: A control group (group A) composed by nine women with mean age = 25 years (±1.87), height = 1.62 m (±0.05) and weight = 56.20 kg (±7.34), and; nine women with objective patellofemoral instability (group B) with mean age = 24 years (±6.02), height = 1.62 m (±0.06) and weight = 60.33 kg (±10.31) were analyzed. The groups underwent kinematic analysis while climbing up/down stairs, in a previously determined area. Images were obtained by six cameras (Qualysis) and data analysis utilized the Q gait software program. RESULTS: Group B presented, in the support phase, less knee flexion when climbing up (p = 0.0268), and lower speed (p = 0.0076/ p =0.0243) and pace (p = 0.0027/ p = 0.0165) when climbing up and down stairs, respectively. CONCLUSION: It is suggested that group B used functional changes such as reduced knee flexion, speed and pace when climbing up and down stairs.

A ossificação heterotópica é uma complicação freqüente após a lesão medular. Os avanços graduais no campo da fisiopatologia, reabilitação e novos métodos de tratamento são uma esperança para a reversão do quadro clínico do lesado medular num futuro próximo. O objetivo desse estudo é avaliar a resposta da ossificação heterotópica das articulações coxo femorais à estimulação elétrica neuromuscular em pacientes tetraplégicos após trauma raquimedular. Seis pacientes foram submetidos à estimulação elétrica neuromuscular por um período médio de 16,6 meses, sendo avaliados radiologicamente. Foi identificada a melhora radiológica em dois pacientes e a não progressão do quadro nos demais. A estimulação elétrica neuromuscular em lesados medulares pode ser um método válido no tratamento da ossificação heterotópica e prevenção de sua progressão.Heterotropic ossification is a common complication after spinal cord injury. Gradual advancements in the physiopathology and rehabilitation fields, and new treatment methods are a hope for the recovery of the clinical picture of injured individuals in the near future. The objective of this study was to evaluate the heterotropic ossification response of the thigh-femoral joints to neuromuscular electric stimulation in tetraplegic patients after rachial-medullar trauma. Six patients were submitted to neuromuscular electric stimulation for an average period of 16.6 months, being evaluated for X-ray imaging. Improvements on X-ray images were identified in two patients and the non-progression of the picture was observed in the remaining patients. Neuromuscular electric stimulation in spinal cord-injured individuals could be a useful method for treating heterotropic ossification and related progression prevention.

ABSTRACT Objective: To assess the effect of pulsed electromagnetic field (PEMF) on the consolidation of instrumented lumbar posterolateral arthrodeses in patients who have been surgically treated for degenerative spine disease. Methods: Forty cases were recruited from 163 consecutive patients undergoing lumbar arthrodesis at the same center. The patients were randomized into two groups of 20 patients: Active Group, who were exposed to PEMF for 4 hours a day for 90 days after surgery, and Inactive Group, who received an identical device, with the same instructions for use but without the ability to generate PEMF. The patients underwent computed tomography scans at 45, 90, 180 and 360 days after surgery to check for the occurrence of arthrodesis at each operated spinal level. Results: In the course of the study, two patients were excluded from each group. There were no significant differences between the groups with respect to age, gender, smoking habit, or the number of vertebral levels included in the arthrodesis. ABSTRACT Objective To compare the sagittal alignment (SA) parameters in individuals with LCS and surgical indication with a control group and to study the correlations between SA parameters and ODI, VAS and EQ-5D in individuals with LCS and surgical indication. Methods In this multicenter cross-sectional case-control study, the individuals were allocated as follows. A stenosis group (SG) composed by patients with LCS confirmed by magnetic resonance imaging with surgical indication, treated between July 2010 and August 2016 and a control group (CG), without LCS. All subjects underwent anamnesis, completed the Health-related Quality of Life (HRQoL) and total spine radiographs were taken. Clinical data, HRQoL and radiographic parameters were correlated. Results Sixty-four individuals formed the SG and 14 the CG. ABSTRACT Objective: This study evaluated the intraobserver and interobserver reliability of the AO classification for standard radiographs of wrist fractures. Methods: Thirty observers, divided into three groups (orthopedic surgery senior residents, orthopedic surgeons, and hand surgeons) classified 52 wrist fractures, using only simple radiographs. After a period of four weeks, the same observers evaluated the initial 52 radiographs, in a randomized order. The agreement among the observers, the groups, and intraobserver was obtained using the Kappa index. Kappa-values were interpreted as proposed by Landis and Koch. Results: The global interobserver agreement level of the AO classification was considered fair (0.30). The three groups presented fair global interobserver agreement (residents, 0.27; orthopedic surgeons, 0.30; hand surgeons, 0.33). The global intraobserver agreement level was moderated. The hand surgeon group obtained the higher intraobserver agreement level, although only moderate (0.50). The residents group obtained fair levels (0.30), as did the orthopedics surgeon group (0.33). Conclusion: The data obtained suggests fair levels of interobserver agreement and moderate levels of intraobserver agreement for the AO classification for wrist fractures.

ABSTRACT Objective: This study aims to review, identify and study the determinations of the main orthopedic aspects in SCI patients. Methods: A total of 80 articles from PUBMED and three theses (MSc. /DSc.) were examined. Results: The results refer to the most essential joints. There is a chronic overload on the shoulder girdle due to the use of the upper limb as a supporting joint. The elbow presents osteoarthritis, subclinical, acute and chronic pain, mainly in quadriplegic patients. In the hand and wrist joints there are cases of paralysis, osteoporosis and osteoarthritis. Hips are the main weight-bearing joints while sitting which leads to a substantial degenerative process of this joint. Lastly, on the knee, feet and ankles, spasticity, contractures, osteoporosis and deformities can arise. Conclusion: Along with the increase in cases and research that analyze the alterations that spinal cord-injured individuals suffer, it is necessary to recognize the orthopedic changes to understand their limits and identify the relevance of the rehabilitation program to improve the muscle performance.
