Papillary muscles, as components of the subvalvular apparatus, play a crucial role in the proper functioning of the heart valves. Although the most commonly encountered variants can be classified traditionally, it is essential to remember that there is significant variability in their number, structure, and shape. This variability is not without clinical significance – specific papillary muscle variants translate into different prognoses, therapeutic approaches, and surgical procedures. Therefore, in our study, we conducted an analysis of available literature to determine which papillary muscle variants are most common, what their vascular supply is, and what the significance of individual variants is in daily clinical practice. To achieve this, we searched popular scientific databases such as PubMed, Google Scholar, and Scopus, using keywords such as papillary muscles, clinical aspects of papillary muscles, anatomical variability of papillary muscles, and papillary muscle rupture – as one of the most common clinical situations related to papillary muscles. This paper presents the results of our review – organised and accompanied by a table.

Background and aimsDespite different etiopathogenesis, Fabry Disease cardiomyopathy (FDc) and sarcomeric hypertrophic cardiomyopathy (HCM) share a similar hypertrophic phenotype, including anomalies of the mitral valve apparatus (AMVA). Some of these anomalies have also been described in the pre-hypertrophic stage of both diseases. This cardiovascular magnetic resonance (CMR) study aimed to: (i) compare AMVA between FDc and HCM with a similar degree of left ventricular hypertrophy (LVH), to add new insights into differential diagnosis; (ii) assess whether AMVA represent an early and progressive alteration in FDc; (iii) propose simple and potentially reproducible measurements of AMVA.MethodsThis observational, retrospective study enrolled: (i) 80 Fabry patients, divided into three groups with increasing severity of cardiac phenotype (20 patients LVH-/normal T1, 20 patients LVH-/low T1 and 40 patients LVH+), and (ii) 40 patients with HCM. All patients underwent CMR. The LVH + FDc and the HCM groups were matched for age, sex, body surface area and left ventricular (LV) mass. The following AMVA were measured on cine images: papillary muscles (PMs) hypertrophy (maximal diameter (Dmax) of anterolateral (Al) and posteromedial (Pm) PM), apical displacement, anteriorization of Al PM and anterior mitral valve leaflet (AMVL) elongation. Background: Papillary muscles are structures integrated into the mitral valve apparatus, having both electrical and mechanical roles. The importance of the papillary muscles (PM) is mainly related to cardiac arrhythmias and mitral regurgitation. The aim of this review is to offer an overview of the anatomy and physiology of the papillary muscles, along with their involvement in cardiovascular pathologies, including arrhythmia development in various conditions and their contribution to secondary mitral regurgitation. Methods: A literature search was performed on PubMed using the following relevant keywords: papillary muscles, mitral valve, arrhythmia, anatomy, and physiology. Results: During the cardiac cycle, papillary muscles have continuous dimensional and pressure changes. On one hand, their synchrony or dyssynchrony impacts the process of mitral valve opening and closure, and on the other hand, the pressure changes can trigger electrical instability. There is increased awareness of papillary muscles as an arrhythmic source. Arrhythmias arising from PM were found in patients with or without structural heart disease, via Purkinje fibres, due to increased automaticity or triggered activity. Conclusions: Despite the interest in mitral valve physiology, there are still many unknowns in relation to the papillary muscles, especially with regard to their role in arrhythmogenesis and the pathogenesis of mitral regurgitation.

Abstract Midventricular hypertrophic obstructive cardiomyopathy (HOCM) is characterized by hypertrophy of the interventricular septum (IVS) and - in rare cases - of the papillary muscles (PM), which subsequently can cause dynamic left ventricular outflow tract obstruction (LVOTO) and severe heart failure symptoms. We report on a rare case of a 44-year-old patient suffering from midventricular HOCM with hypertrophic anterolateral PM and an additional chorda between the PM and the IVS. We describe a new surgical approach via right anterolateral thoracotomy in 3-dimensional (3D) video-assisted minimal-invasive technique with resection of hypertrophic PMs as well as the entire mitral valve-apparatus and valve replacement avoiding septal myectomy and potentially associated complications. After an uneventful procedure clinical symptoms improved from NYHA III-IV at baseline to NYHA 0-I postoperatively and remained stable over a follow-up period of 24 months. Therefore, the presented technique may be considered as a new and alternative approach in patients with hypertrophic PMs and hypertrophic IVS as subtype of midventricular HOCM.

Abstract Objective Most mitral valve repair techniques provide excellent surgical results by removing regurgitation, but all of these techniques simultaneously reduce posterior valve mobility. A comprehensive biometric study of the mitral valve apparatus will provide landmarks that would help improve this posterior valve mobility. Materials and methods Thirty one (31) human hearts have been studied, from 14 women and 17 men. The characteristics of the studied sample were analyzed descriptively. The difference in means of the variables between women and men were tested using a Student t test. Correlations between the different measures were determined by simple regression analysis. Mean values are shown with ± 1 standard deviation and the limit of significance was set at 0.05. Results The mean weight of the hearts was 275.3 ± 2.4 g. The anteroposterior diameter of the mitral annulus was 29.3 ± 1.22 mm, the intertrigonal distance was 25.2 ± 3.50 mm and the anterior leaflet to posterior leaflet ratio was 1.9 ± 0.10, the length of the chordae A2 = 19.4 ± 1.15 mm and P2 = 14.5 ± 0.85 mm. The length of the anterior papillary muscle averaged 30.9 ± 7.20 mm and that of the posterior one 30.0 ± 8.75 mm. The comparison of the different values measured between women and men showed no statistically significant difference (p > 0.05). There was no correlation between these different measured values (p > 0.05). Conclusion A perfect knowledge of anatomy and biometry is therefore essential to offer alternative techniques that reproduce the real anatomy and physiology with a complete reconstruction of the mitral valve.

Background: The present study aimed to identify the frequency of Fabry disease in patients with cardiac hypertrophy of unknown etiology and to evaluate demographic and clinical characteristics, enzyme activity levels, and genetic mutations at the time of diagnosis. Methods: This national, multicenter, cross-sectional, single-arm, observational registry study was conducted in adult patients with a clinical echocardiographic diagnosis of left ventricular hypertrophy and/or the presence of prominent papillary muscle. In both genders, genetic analysis was performed by DNA Sanger sequence analysis. Results: A total of 406 patients with left ventricular hypertrophy of unknown origin were included. Of the patients, 19.5% had decreased enzyme activity (≤2.5 nmol/mL/h). Although genetic analysis revealed GLA (galactosidase alpha) gene mutation in only 2 patients (0.5%), these patients were considered to have probable but not 'definite Fabry disease' due to normal lyso Gb3 levels and gene mutations categorized as variants of unknown significance. Conclusion: The prevalence of Fabry disease varies according to the characteristics of the population screened and the definition of the disease used in these trials. From cardiology perspective, left ventricular hypertrophy is the major reason to consider screening for Fabry disease. Enzyme testing, genetic analysis, substrate analysis, histopathological examination, and family screening should be performed, when necessary, for a definite diagnosis of Fabry disease. The results of this study underline the importance of the comprehensive use of these diagnostic tools to reach a definite diagnosis. The diagnosis and management of Fabry disease should not be based solely on the results of the screening tests.

Abstract Background Papillary muscle rupture is a rare condition. Its clinical presentation, diagnosis and management can be very challenging for the clinician. Case presentation A 73-year-old woman with hypertension presented with chest pain, ST-segment changes, and elevated serum troponin levels. Coronary angiography was normal. Echocardiography revealed normal ventricular function, flail posterior mitral leaflet, and severe mitral regurgitation. She underwent emergent mitral valve replacement. Conclusion The diagnostic and management strategies of this uncommon presentation are discussed.

Abstract Background Current indications for implantable cardioverter defibrillator (ICD) implantation for sudden cardiac death prevention rely primarily on left ventricular (LV) ejection fraction (LVEF). Currently, two different contouring methods by cardiovascular magnetic resonance (CMR) are used for LVEF calculation. We evaluated the comparative prognostic value of these two methods in the ICD population, and if measures of LV geometry added predictive value. Methods In this retrospective, 2-center observational cohort study, patients underwent CMR prior to ICD implantation for primary or secondary prevention from January 2005 to December 2018. Two readers, blinded to all clinical and outcome data assessed CMR studies by: (a) including the LV trabeculae and papillary muscles (TPM) (trabeculated endocardial contours), and (b) excluding LV TPM (rounded endocardial contours) from the total LV mass for calculation of LVEF, LV volumes and mass. LV sphericity and sphere-volume indices were also calculated. The primary outcome was a composite of appropriate ICD shocks or death. Results Of the 372 consecutive eligible patients, 129 patients (34.7%) had appropriate ICD shock, and 65 (17.5%) died over a median duration follow-up of 61 months (IQR 38–103). Abstract Background This study aimed to investigate the electrophysiological characteristics of idiopathic ventricular arrhythmias (VAs) originating from the left ventricular posterior papillary muscles (LPPM) and explore the efficiency of catheter ablation using three‐dimensional intracardiac ultrasound technology. Methods Twenty‐seven cases of premature ventricular contraction/ventricular tachycardia (PVC/VT) originating from the left ventricular posterior papillary muscles were recorded from July 2015 to June 2019 in the Central Hospital of Shengli Oil Field and the First Affiliated Hospital of Zhengzhou University. Electrophysiological mapping and radiofrequency catheter ablation (RFCA) were performed using three‐dimensional intracardiac ultrasound technology. The characteristics of the body surface and intracavity electrocardiogram were analyzed. All cases were followed up for 24 months after the operation. Results The VAs of all 27 cases were successfully eliminated by catheter ablation. QRS complexes were observed with a right bundle branch block (RBBB) pattern and a steep slope in the initial segment. Lead I appeared with an Rs pattern, and inferior leads (lead II, III, and aVF) were usually with an S wave. The lead aVR appeared with a qR pattern, while the R wave was commonly found in aVL. The main wave in leads V1‐V3 was positive but negative in V5 and V6. Conclusion Ventricular arrhythmias originating from the left ventricular posterior papillary muscles have similar electrophysiological characteristics. The origin site was accurately located using three‐dimensional intracardiac ultrasound technology. Catheter ablation effectively eliminated VAs.

A 52-year-old man presented with acute onset of chest pain and was found to have an inferolateral ST-segment elevation myocardial infarction and acute mitral regurgitation due to papillary muscle rupture. This case describes a rare, potentially fatal mechanical complication of acute myocardial infarction. (Level of Difficulty: Beginner.)

Postpartum papillary muscle rupture (PMR) is extremely uncommon and tolerated poorly with limited management options other than emergency surgical intervention. This case demonstrates the challenges of postpartum PMR in a young woman with unrecognized vascular Ehlers-Danlos syndrome and highlights the importance of preconception screening of cardiovascular disease. Abstract Background Previous studies on radiofrequency catheter ablation of premature ventricular complexes (PVCs) arising from the left ventricle (LV) papillary muscles (PM) show a modest procedural success rate with higher recurrence rate. Our study sought to explore the utility of using a multipolar mapping with a steerable linear duodecapolar catheter for ablating the PM PVCs. Methods Detailed endocardial multipolar mapping was performed using a steerable linear duodecapolar catheter in 6 consecutive PM PVCs patients with structurally normal heart. The clinical features and procedural data as well as success rate were analysed. Results LV endocardial electroanatomic mapping was performed in all patients via a retrograde aortic approach using a duodecapolar mapping catheter. All patients displayed a PVC burden with 16.2 ± 5.4%. Duodecapolar catheter mapping demonstrated highly efficiency with an average procedure time (95.8 ± 7.4 min) and fluoroscopy time (14.2 ± 1.5 min). The mean number of ablation applications points was 6.8 ± 1.9 with an average overall ablation duration of 6.1 ± 3.0 min. The values of earliest activation time during mapping using duodecapolar catheter were 37.8 ± 7.2 ms. All patients demonstrated acute successful ablation, and the PVC burden in all patients after an average follow-up of 8.5 ± 2.0 months was only 0.7%. There were no complications during the procedures and after follow-up. Conclusions Mapping and ablation of PM PVCs using a duodecapolar catheter facilitated the identification of earliest activation potentials and pace mapping, and demonstrated a high success rate during follow-up.

Abstract We compared the mortality rates of two surgical techniques for correction of atrioventricular disjunction in 10 out of 720 patients who underwent mitral valve replacement from 2005 to 2012. In group I, the mitral annulus was fixed with bovine pericardial strips; in group II, a 'patch' of bovine pericardium was sutured and extended from the base of the lateral and medial papillary muscles, covered the posterior wall of the left ventricle, went through the posterior mitral annulus, and ended in the posterior wall of the left atrium adjacent to the mitral ring. The group II technique showed a lower mortality.

Abstract Introduction: The number of cases for which the Batista procedure is indicated is small, but some patients with appropriate indication can achieve good therapeutic results. Objective: To avoid incorrect left ventricular incision and obtain good surgical results in patients with dilated cardiomyopathy suitable for partial left ventriculectomy, we employed intraoperative direct echocardiography to determine the exact extent and position of the myocardial incision, even for surgeons who are not very experienced with the Batista procedure. Methods: A 72-year-old man with dilated cardiomyopathy underwent the Batista procedure with the aid of epicardial echocardiography to confirm the location of both the papillary muscles and the diseased myocardium. Results: We were able to accurately perform left ventricular incision and remove the diseased lateral ventricular wall. Two years later, the patient had no symptoms of heart failure, and his brain natriuretic peptide (BNP) level decreased from 1155 to 49.3 pg/mL. Conclusions: We believe that the use of intraoperative echocardiography may have the potential to make the Batista procedure less technically demanding and more reproducible for surgeons with little experience in the procedure.

Abstract Left ventricular papillary muscles are small myocardial structures that play an important role in the functioning of mitral valve and left ventricle. Typically, there are two groups of papillary muscles, namely the anterolateral and the posteromedial groups. Cardiovascular magnetic resonance (CMR) is a valuable imaging modality in the evaluation of papillary muscles, providing both morphological and functional information. There is a remarkably wide variation in the morphology of papillary muscles. These variations can be asymptomatic or associated with symptoms related to LV outflow tract obstruction, often associated with hypertrophic cardiomyopathy. Abnormalities of the papillary muscles range from congenital disorders to neoplasms. Parachute mitral valve is the most common congenital abnormality of papillary muscles, in which all the chordae insert into a single papillary muscle. Papillary muscles can become dysfunctional, most commonly due to ischemia. Papillary muscle rupture is a major complication of acute myocardial infarction that results in mitral regurgitation and associated with high mortality rates. The most common papillary neoplasm is metastasis, but primary benign and malignant neoplasms can also be seen. In this article, we discuss the role of CMR in the evaluation of papillary muscle anatomy, function, and abnormalities.

Background: The left ventricle is built to carry out its function as a powerful pump. To detail recent findings of anatomical variants associated with the left ventricular compartment, we will first review the external and general features, followed by the internal features and subsequently, the standard variants that can exist within this chamber of the heart. Methods: This literature review seeks to collate and discuss peer-reviewed articles on the anatomy of the left ventricle. Results: The left ventricle has many unique features including walls that are thicker than those of the right ventricle, an overlap of its inlet and outlet portions, and the hinge of the leaflets of the mitral valve that are oriented cranially relative to those of the tricuspid valve. Conclusion: While the left ventricle shares many characteristics with that of the right ventricle, the unique features may contribute to cardiac pathology and can also be used to the surgeon's advantage in treating disease.

Patients with obstructive hypertrophic cardiomyopathy (HOCM) have large papillary and trabecular muscles (PTMs), which are myocardial tissue. PTMs are usually excluded from the myocardium and included in the left ventricular (LV) cavity when determining LV mass (LVM) and volumes using cardiac magnetic resonance (CMR). This conventional method may result in large distortion of LVM and other indices. We investigated 74 patients with HOCM undergoing CMR imaging. LV short-axis cine images were obtained. LV contours were drawn using two different methods: (1) the conventional method, where PTMs were included in the LV cavity; and (2) the mask method, which includes the TPMs in the LV myocardium. The LV end-diastolic volume (LV-EDV), LV end-systolic volume (LV-ESV), LV ejection fraction (LVEF), and the LVM were then calculated. Fasting NT-proBNP and CK-MB levels were measured with ELISA. In patients with HOCM, mass of PTMs (MOPTM) was 47.9 ± 18.7 g, which represented 26.9% of total LVM. Inclusion of PTMs with the mask method resulted in significantly greater LVM and LVM index (both p < 0.0001) in comparison with those measured with the conventional method. In addition, the mask method produced a significant decrease in LV-EDV and LV-ESV. LVEF was significantly increased with the mask method (64.3 ± 7.9% vs. 77.2 ± 7.1%, p < 0.0001). MOPTM was positively correlated with BMI, septal wall thickness, LVM, LV-EDV, and LV-ESV. LVEF was inversely correlated with MOPTM. In addition, MOPTM correlated positively with NT-proBNP (r = 0.265, p = 0.039) and CK-MB (r = 0.356, p = 0.002). In conclusion, inclusion of PTMs in the myocardium has a substantial impact on quantification of the LVM, LV-EDV, LV-ESV, and LVEF in patients with HOCM. The effects of the PTMs in women was greater than that in men. Furthermore, the MOPTM was positively associated with NT-proBNP and CK-MB. The PTMs might be included in the myocardium when measuring the LV volumes and mass of patients with HOCM. At present, the clinical and prognostic meaning and relevance of the PTMs is not clear and should be further studied.

The normal mitral valve is a dynamic structure that permits blood to flow from the left atrial (LA) to left ventricle (LV) during diastole and sealing of the LA from the LV during systole. The main components of the mitral apparatus are the mitral annulus (MA), the mitral leaflets, the chordae tendineae, and the papillary muscles (PM) (Figure 1). Normal valve function is dependent on the integrity and normal interplay of these components. Abnormal function of any one of the components, or their interplay can result in mitral regurgitation (MR). Understanding the anatomy and physiology of all the component of the mitral valve is important for the diagnosis, and for optimal planning of repair procedures. In this review we will focus first on normal anatomy and physiology of the different parts of the mitral valve (MA, leaflets, chordae tendineae, and PM). In the second part we will focus on the pathologic anatomic and physiologic derangements associated with different types of MR.

Aim. Evaluation of influence of the stimulated regeneration abilities of myocardium with non-cellular allogenic material (NAM) on contractility restoration in experimental post-infarction remodelling.Material and methods. The experiment was done on the Wistar rodents with postinfarction cardiosclerosis (PICS), and PICS rodents post injection of the NAM at the moment of coronary artery stenosing. PICS was being formed during 45 days after coronary occlusion. The assessments were the formation and size of postinfarction scar of the left ventricle in rodents of different groups. Myocardial contractility was assessed by inotropic reaction of isolated papillary muscles on rest periods (4-60 s) with stimulation frequency 0,5 Hz, under conditions of perfusion with oxygnated Krebs-Henseleit solution.Results. Papillary muscle rupture is a rare and life-threatening complication of acute myocardial infarction (AMI). We present a unique case of double papillary muscle rupture in a patient with three vessel disease and acute thrombotic occlusion of left circumflex obtuse marginal artery as the culprit lesion. The killer whale (Orcinus orca, Linnaeus, 1958) is the largest extant delphinid. Despite its worldwide distribution in the wild and in dolphinariums, its anatomy remains relatively poorly described. In the present study, we describe the detailed morphology of a plastinated killer whale heart. The gross description of the arteries and veins reaching the organ and its coronary vessels are reported. Additional endoscopy and CT (computed tomography) scanning were performed to provide extensive measurements of its parts. In many aspects, the killer whale heart conformed to other delphinid heart descriptions, including position, relative size and shape and specific features such as extensive papillary muscles, trabecular endocardium and trabecula septomarginalis. These characteristics are representative of the delphinid family, suggesting that its functions and capacities are similar to that of other, smaller, dolphins and help understand the conditions in which these predators exert their remarkable physical performance necessary for their survival.

(1) Background: The accuracy of the left ventricular volume (LVV) and contractility measurements with cardiac magnetic resonance imaging (CMRI) is decreased if the papillary muscles are abnormally enlarged, such as in hypertrophic cardiomyopathy in human patients or in pig models of human diseases. The purpose of this work was to establish the best method of LVV quantification with CMRI in pigs. (2) Methods: The LVV in 29 Yucatan minipig hearts was measured using two different techniques: the “standard method”, which uses smooth contouring along the endocardial surface and adds the papillary volume to the ventricular cavity volume, and the “detailed method”, which traces the papillary muscles and trabeculations and adds them to the ventricular mass. (3) Results: Papillary muscles add 21% to the LV mass in normal and infarcted hearts of Yucatan minipigs. The inclusion or exclusion of these from the CMRI analysis significantly affected the study results. In the normal pig hearts, the biggest differences were found in measurements of the LVV, ejection fraction (EF), LV mass and indices derived from the LV mass (p < 0.001). The EF measurement in the normal pig heart was 11% higher with the detailed method, and 19% higher in the infarcted pig hearts (p < 0.0001). The detailed method of endocardium tracing with CMRI closely represented the LV mass measured ex vivo. (4) Conclusions: The detailed method, which accounts for the large volume of the papillary muscles in the pig heart, provides better accuracy and interobserver consistency in the assessment of LV mass and ejection fraction, and might therefore be preferable for these analyses.

We present a rare case of severe ischaemic papillary muscle rupture in a 67-year-old male patient who was admitted to the Emergency Department of the University Hospital Bern, Switzerland, in November 2013 with acute chest pain. On admission, the patient’s blood pressure was 60/40 mm/Hg, his pulse was 110 beats per minute and his respiratory rate was 20 breaths per minute. An electrocardiogram was normal and focused assessment with sonography in trauma was negative. Transthoracic echocardiography showed possible thickening of the mitral valve leaflet with no indications of severe mitral insufficiency or wall motion abnormalities. Triple-ruleout computed tomography angiography revealed no pulmonary emboli or aortic dissection, although coronary atherosclerosis was present. Finally, severe insufficiency of the mitral valve with rupture of the papillary muscle, likely due to ischaemia, was observed via transoesophageal echocardiography. The patient underwent a successful surgical intervention and was discharged 10 days later in stable condition.

The aim of the present study is to characterize the hemodynamics of left ventricular (LV) geometries to examine the impact of trabeculae and papillary muscles (PMs) on blood flow using high performance computing (HPC). Five pairs of detailed and smoothed LV endocardium models were reconstructed from high-resolution magnetic resonance images (MRI) of ex-vivo human hearts. The detailed model of one LV pair is characterized only by the PMs and few big trabeculae, to represent state of art level of endocardial detail. The other four detailed models obtained include instead endocardial structures measuring ≥1 mm2 in cross-sectional area. The geometrical characterizations were done using computational fluid dynamics (CFD) simulations with rigid walls and both constant and transient flow inputs on the detailed and smoothed models for comparison. These simulations do not represent a clinical or physiological scenario, but a characterization of the interaction of endocardial structures with blood flow. Steady flow simulations were employed to quantify the pressure drop between the inlet and the outlet of the LVs and the wall shear stress (WSS). Coherent structures were analyzed using the Q-criterion for both constant and transient flow inputs. Our results show that trabeculae and PMs increase the intra-ventricular pressure drop, reduce the WSS and disrupt the dominant single vortex, usually present in the smoothed-endocardium models, generating secondary small vortices. Given that obtaining high resolution anatomical detail is challenging in-vivo, we propose that the effect of trabeculations can be incorporated into smoothed ventricular geometries by adding a porous layer along the LV endocardial wall. Results show that a porous layer of a thickness of 1.2·10−2 m with a porosity of 20 kg/m2 on the smoothed-endocardium ventricle models approximates the pressure drops, vorticities and WSS observed in the detailed models.

Abstract A 60-year-old male patient with blunt chest trauma was transferred to our facility because of unstable vital signs and pericardial effusion. These conditions occurred after orthopedic surgery to repair multiple left finger fractures at a local medical center. Trans-thoracic echocardiography showed severe tricuspid regurgitation and he underwent papillary muscle reimplantation and tricuspid annuloplasty open heart surgery for post-traumatic tricuspid regurgitation with anterior papillary muscle rupture. We report early surgical traumatic valve disease correction without complications.

PURPOSE: To study racemic bupivacaine, non-racemic bupivacaine and ropivacaine on myocardial contractility. METHODS: Isolated Wistar papillary muscles were submitted to 50 and 100 mM racemic bupivacaine (B50 and B100), non-racemic bupivacaine (NR50 and NR100) and ropivacaine (R50 and R100) intoxication. Isometric contraction data were obtained in basal condition (0.2 Hz), after increasing the frequency of stimulation to 1.0 Hz and after 5, 10 and 15 min of local anesthetic intoxication. Data were analyzed as relative changes of variation. RESULTS: Developed tension was higher with R100 than B100 at D1 (4.3 ± 41.1 vs -57.9 ± 48.1). Resting tension was altered with B50 (-10.6 ± 23.8 vs -4.7 ± 5.0) and R50 (-14.0 ± 20.5 vs -0.5 ± 7.1) between D1 and D3. Maximum rate of tension development was lower with B100 (-56.6 ± 38.0) than R50 (-6.3 ± 37.9) and R100 (-1.9 ± 37.2) in D1. B50, B100 and NR100 modified the maximum rate of tension decline from D1 through D2. Time to peak tension was changed with NR50 between D1 and D2. CONCLUSIONS: Racemic bupivacaine depressed myocardial contractile force more than non-racemic bupivacaine and ropivacaine. Non-racemic and racemic bupivacaine caused myocardial relaxation impairment more than ropivacaine.

Objectives: The chordae tendinae (CT) are strong, fibrous connections between the valve leaflets and the papillary muscles. Dysfunction of the papillary muscles and chordae is frequent. Mitral valve replacement with preservation of CT and papillary muscles may preserve postoperative left ventricular function better than conventional mitral valve replacement in patients with chronic mitral regurgitation. Methods: The study was carried out on 116 human cadaveric hearts. The heart was opened through the atrioventricular valve to view the constituents of the complex. Origin, attachments, insertions, distribution, branching pattern and gross structure of CT were observed and studied in detail. Results: In the present study more than 21 terminologies of CT were defined by classifying it into six different types. Classification is done according to the origin, attachments, insertion, distribution, branching pattern and gross structure. Terminologies defined are as follows. Apical pillar chordae, Basal pillar chordae, True chordae, False chordae, Interpillar chordae, Pillar wall chordae, Cusp chordae, Cleft chordae, Commissural chordae, First order chordae, Second order chordae, Free zone chordae, Marginal chordae, Rough zone chordae, Straight chordae, Branched-fan shaped chordae, Spiral chordae, Irregular-web chordae, Tendinous chordae, Muscular chordae, Membranous chordae. Basal pillar chordae are found in 9.48%. Mean number of chordae taking origin from apical half of a single papillary muscle or single head of papillary muscle was 9.09 with the range of 3-18. Mean number of the marginal chordae attached to a single cusp was 22.63 ranging from 11 to 35. Strut chordae showed interesting insertion with broad aponeurosis in 38.79% and large muscular flaps in 13.79%. Chordae muscularis were found in 14% and membranous chordae were found in 6%. Conclusions: This knowledge may prove useful for cardiologists and cardiac surgeons.

Background. The mitral valve complex of the human heart consists of: the fibrous annulus, valve leaflets, chordae tendineae and papillary muscles. Deficiency or degeneration one of these structures may result in the dysfunction of the valvular apparatus. Topography and structure of the chordae tendineae effect to the normal functioning of heart valve complex and its hemodynamics, due to the aim of our research was to examine the peculiarities of the structural organization normally located mitral valve chordae tendineae of the human heart. Methods. The investigation was made on sectional material. We used light microscopy and immunohistochemistry methods. Results. The chordae tendineae are cord-like tendons that connect the papillary muscles to the valve leaflets in the heart. The chordeae tendineae were covered by endocardium which contained superficial layer of endothelium. Subendocardial layer includes loosely located collagen and elastic fibers which were interwoven with each other. Cor of the chordae are composed of bundles of densely packed collagen fibrils with few elastic fibers. Conclusion. The investigations allowed to receive a new objective data on some structural peculiarities of chordae tendineae of the human mitral valve that later can become the basis for the differential diagnosis of disease, including valvular heart pathology. OBJETIVO: Avaliar, clinicamente e pelo ecodopplercardiograma, o funcionamento da valva mitral em 22 pacientes submetidos à correção do refluxo valvar com substituição das cordas tendíneas nativas por cordas padronizadas de pericárdio bovino. MÉTODOS: Os pacientes apresentavam insuficiência mitral degenerativa. Quatorze (63,6%) pacientes eram do gênero masculino e a idade variou de 19 a 76 anos (média 56,8±13,8 anos). As cordas de pericárdio bovino foram tratadas com glutaraldeído, com reforço de suas extremidades transversais formando um trapézio. RESULTADOS: Um (4,5%) paciente faleceu no pós-operatório imediato em síndrome de baixo débito cardíaco e três (13,6%) no pós-operatório tardio. Uma (4,5%) paciente foi reoperada. As curvas atuariais de sobrevivência livre de óbitos por causa cardiovascular e livres de reoperações para os pacientes que deixaram o hospital (21) demonstraram taxas de 82,0±9,8% e 83,9±10,4%, aos 70 meses de pós-operatório, respectivamente. Dezessete (77,3%) pacientes estão vivos com a própria valva. Dos 17 pacientes vivos com a própria valva 16 (94,1%) estão em classe funcional I. O ecodoppler pós-operatório (média de 41 meses; 4 a 70 meses) demonstrou ausência de regurgitação mitral em 11 (64,7%) pacientes e regurgitação discreta em cinco (29,4%). CONCLUSÃO: A técnica de implante de cordas padronizadas de pericárdio bovino para substituição de cordas tendíneas da valva mitral em pacientes com insuficiência mitral degenerativa demonstrou resultados bastante satisfatórios.OBJECTIVE: To evaluate clinically and by Doppler Echocardiography 22 patients submitted to mitral valve repair after valvular regurgitation using standardized bovine pericardium chordae. METHODS: The patients had degenerative mitral regurgitation. Fourteen (63.6%) patients were male and the age ranged from 19 to 76 years (mean 56.8 ± 13.8 years). The strings of bovine pericardium treated with glutaraldehyde were reinforced in its transverse ends forming a trapezoid. RESULTS: One patient (4.5%) died in the immediate postoperative period with in low cardiac output syndrome and three (13.6%) in the late postoperative period. One patient (4.5%) was reoperated. The actuarial curves for survival free of death from cardiovascular causes and free from reoperation for patients who left the hospital (21), showed rates of 82.0 ± 9.8% and 83.9 ± 10.4% at 70 months postoperatively, respectively. Seventeen patients (77.3%) are alive with native valves. Of the 17 patients alive with native valves 16 (94.1%) were in functional class I. The Doppler Echocardiography postoperatively (mean 41 months, 4-70 months), showed no mitral regurgitation in 11 (64.7%) patients and mild regurgitation in five (29.4%). CONCLUSION: The technique of standard cords of bovine pericardium implantation to replace chordae tendineae of the mitral valve in patients with degenerative mitral regurgitation showed satisfactory results.

Background. Despite the high prevalence of heart defects due attention is not given for investigation of morphometric parameters of papillary muscles and respective regions of ventricle wall at tetralogy of Fallot. Objective. To study the morphometric parameters of papillary muscles of atrioventricular valves, and the thickness of the ventricle walls at the base of papillary muscles in tetralogy of Fallot comparing with normal hearts. Methods. 27 samples of abnormal hearts with tetralogy of Fallot from 20 weeks of prenatal period up to 1 year after birth were investigated with methods of anatomical dissection and morphometry, in particular height, width and thickness of each papillary muscle were measured. Results. Tetralogy of Fallot was accompanied by increase in the thickness of right ventricle wall at the base of papillary muscles (р0,05). It was established that the height of papillary muscles did not depend on thickening of ventricles walls of heart. The thickening of the right ventricle wall of the heart in tetralogy of Fallot was accompanied by increase in width and thickness of papillary muscles of the right atrio-ventricular valve. Conclusion. Major changes in tetralogy of Fallot were exhibited in the thickening of papillary muscles and adjacent region of ventricle wall but were not in relation with the height of papillary muscles.

FUNDAMENTO: O estresse crônico está associado à remodelação cardíaca; entretanto, os mecanismos permanecem a ser descobertos. OBJETIVO: A proposta deste estudo foi testar a hipótese de que o estresse crônico promove disfunção cardíaca associada a depressão da atividade do canal-L para Ca2+. M MÉTODOS: Ratos Wistar machos com 30 dias de idade (70 - 100 g) foram distribuídos dentro de dois grupos: controle (C) e estresse crônico (St). O estresse consistiu na imobilização durante 15 semanas, cinco vezes por semana, 1 h por dia. A função cardíaca foi avaliada pela performance do ventrículo esquerdo por meio do ecocardiograma e pelo músculo papilar ventricular isolado. A função do músculo papilar foi avaliada em condição basal e com manobras inotrópicas, como: pós-pausa e elevação na concentração extracelular de Ca2+, na presença ou ausência de um bloqueador específico de canal-L para Ca2+. RESULTADOS: O estresse ficou caracterizado por hipertrofia das glândulas adrenais, aumento nos níveis de corticosterona circulante e por hipertensão arterial. Ainda, o estresse crônico gerou hipertrofia ventricular esquerda. O estresse crônico foi capaz de melhorar a resposta no músculo papilar para manobras inotrópicas positivas. A melhora de função não esteve associada com o canal-L para Ca2+. CONCLUSÃO: O estresse produziu hipertrofia cardíaca; entretanto, nos estudos de músculo papilar isolado, as manobras inotrópicas positivas potencializaram a função cardíaca em ratos estressados, sem o envolvimento do canal-L para Ca2+. Assim os mecanismos responsáveis permanecem incertos para alterações no influxo de Ca2+.BACKGROUND: Chronic stress is associated with cardiac remodeling; however the mechanisms have yet to be clarified. OBJECTIVE: The purpose of this study was test the hypothesis that chronic stress promotes cardiac dysfunction associated to L-type calcium Ca2+ channel activity depression. METHODS: Thirty-day-old male Wistar rats (70 - 100 g) were distributed into two groups: control (C) and chronic stress (St). The stress was consistently maintained at immobilization during 15 weeks, 5 times per week, 1h per day. The cardiac function was evaluated by left ventricular performance through echocardiography and by ventricular isolated papillary muscle. The myocardial papillary muscle activity was assessed at baseline conditions and with inotropic maneuvers such as: post-rest contraction and increases in extracellular Ca2+ concentration, in presence or absence of specific blockers L-type calcium channels. RESULTS: The stress was characterized for adrenal glands hypertrophy, increase of systemic corticosterone level and arterial hypertension. The chronic stress provided left ventricular hypertrophy. The left ventricular and baseline myocardial function did not change with chronic stress. However, it improved the response of the papillary muscle in relation to positive inotropic stimulation. This function improvement was not associated with the L-type Ca2+ channel. CONCLUSION: Chronic stress produced cardiac hypertrophy; however, in the study of papillary muscle, the positive inotropic maneuvers potentiated cardiac function in stressed rats, without involvement of L-type Ca2+ channel. Thus, the responsible mechanisms remain unclear with respect to Ca2+ influx alterations.

OBJECTIVES: Tension cost, the ratio of myosin ATPase activity to tension, reflects the economy of tension development in the myocardium. To evaluate the mechanical advantage represented by the tension cost, we studied papillary muscle contractility and the activity of myosin ATPase in the left ventricles in normal and pathophysiological conditions. METHODS: Experimental protocols were performed using rat left ventricles from: (1) streptozotocin-induced diabetic and control Wistar rats; (2) N-nitro-L-arginine methyl ester (L-NAME) hypertensive and untreated Wistar rats; (3) deoxycorticosterone acetate (DOCA) salt-treated, nephrectomized and salt- and DOCA-treated rats; (4) spontaneous hypertensive rats (SHR) and Wistar Kyoto (WKY) rats; (5) rats with myocardial infarction and shamoperated rats. The isometric force, tetanic tension, and the activity of myosin ATPase were measured. RESULTS: The results obtained from infarcted, diabetic, and deoxycorticosterone acetate-salt-treated rats showed reductions in twitch and tetanic tension compared to the control and sham-operated groups. Twitch and tetanic tension increased in the N-nitro-L-arginine methyl ester-treated rats compared with the Wistar rats. Myosin ATPase activity was depressed in the infarcted, diabetic, and deoxycorticosterone acetate salt-treated rats compared with control and sham-operated rats and was increased in N-nitro-L-arginine methyl ester-treated rats. These parameters did not differ between SHR and WKY rats. In the studied conditions (e.g., post-myocardial infarction, deoxycorticosterone acetate salt-induced hypertension, chronic N-nitro-L-arginine methyl ester treatment, and streptozotocin-induced diabetes), a positive correlation between force or plateau tetanic tension and myosin ATPase activity was observed. CONCLUSION: Our results suggest that the myocardium adapts to force generation by increasing or reducing the tension cost to maintain myocardial contractility with a better mechanical advantage.

For the purpose of the definition of the features of anatomic reorganization of anterior ventricular heart walls inthe place of arise of papillary muscles at children with the full form of the transposition of the great vessels 31 specimenswith the given pathology from a birth till 11 months and 31 specimens of usually formed hearts of the same age group havebeen investigated. The distinguish features of abnormal hearts were: atrial and ventricular septal defects, patent ductus arteriosus.For reception of quantitative parameters of the object of the research - thickness of forward walls ventricles made themeasurement. The statistical analysis of the received data depending on age, type and quantity of communications is made.At a combination of a transposition with ventricular septal defect the thickening of right ventricular walls in the place of ariseof papillary muscles more often came to light. The transposition was accompanied by atrial septal defect, thinning of a leftventricular wall after 1 month was established. The transposition with patent ductus arteriosus and septal defects characterizedby thickening of wall of the right ventricle since 3 week. The combination of the transposition with ventricular and atrialseptal defects leads to the thickening of the wall of both ventricles.

FUNDAMENTO: As técnicas de substituição mitral com preservação do aparelho subvalvar têm reafirmado sua superioridade, sendo a papilopexia cruzada uma nova opção técnica que além de permitir preservação de estruturas anatômicas, oferece suporte na contração e protege o miocárdio na diástole ventricular, necessitando de estudos que documentem seus resultados. OBJETIVO: Avaliar as funções atrial e ventricular esquerdas, através de ecodopplercardiografia, em pacientes submetidos à substituição da valva mitral com papilopexia cruzada. MÉTODOS: Foram submetidos à substituição valvar mitral 15 pacientes, sendo nove (60%) do sexo masculino, com idade média de 45,7 anos. Quanto à etiologia, nove (60%) casos eram degenerativos, três (20%) reumáticos, dois (13,3%) isquêmicos e um (6,7%) com endocardite infecciosa. Após atriotomia e avaliação anatômica do aparelho valvar o folheto anterior foi desinserido do anel e centralmente dividido, sendo cada metade com seu complexo de cordas tendíneas fixada à comissura oposta por sua extremidade medial. Foi Implantada a prótese valvar biológica (em 13 casos) ou mecânica, fixada por pontos separados, com redução do anel valvar nos casos com miocardiopatia dilatada. Foram tomadas avaliações clinicas e ecodopplercardiográficas no pré-operatório, e sexto mês de pós-operatório. RESULTADOS: Todos os pacientes receberam alta em condições clínicas estáveis. Foi demonstrada redução significativa dos diâmetros ventriculares e dos diâmetros atriais (p< 0,001) sem comprometimento das vias de entrada e saída do ventrículo esquerdo. CONCLUSÃO: As substituições da valva mitral realizadas com a técnica de papilopexia cruzada apresentaram resultados favoráveis, com efeito positivo na recuperação morfológica atrial e ventricular esquerdas.FUNDAMENTO: Las técnicas de sustitución mitral con preservación del aparato subvalvular han estado reafirmando su superioridad, siendo la resuspensión de los músculos papilares una nueva opción técnica que además de permitir la preservación de estructuras anatómicas, ofrecer soporte en la contracción y protege el miocardio en la diástole ventricular, necesitando estudios que documenten sus resultados. OBJETIVO: Evaluar las funciones atrial y ventricular izquierdas, mediante ecocardiografía Doppler, en pacientes sometidos a sustitución de la válvula mitral con resuspensión de los músculos papilares. MÉTODOS: Se sometieron a sustitución valvular mitral 15 pacientes, siendo nueve (60%) del sexo masculino, con una edad promedio de 45,7 años. En cuanto a la etiología, nueve (60%) casos eran degenerativos, tres (20%) reumáticos, dos (13,3%) isquémicos y uno (6,7%) con endocarditis infecciosa. Después de la atriotomía y evaluación anatómica del aparato valvular, el velo anterior se retiró del anillo y se dividió centralmente, siendo cada mitad con su complejo de cuerdas tendinosas fijadas a la comisura opuesta por su extremidad medial. Se implantó prótesis valvular biológica (en 13 casos) o mecánica, fijada por puntos separados, con reducción del anillo valvular en los casos con miocardiopatía dilatada. Se realizaron evaluaciones clínicas y por ecocardiografía Doppler en el preoperatorio, y sexto mes de postoperatorio. RESULTADOS: Todos los pacientes recibieron alta en condiciones clínicas estables. Se demostró reducción significativa de los diámetros ventriculares y de los diámetros atriales (p< 0,001) sin compromiso de las vías de entrada y salida del ventrículo izquierdo. CONCLUSIÓN: Las sustituciones de la válvula mitral realizadas con la técnica de resuspensión de los músculos papilares presentaron resultados favorables, con efecto positivo en la recuperación morfológica atrial y ventricular izquierdas.BACKGROUND: Techniques for mitral valve replacement with preservation of the subvalvular apparatus have proven their superiority, and crossed papillopexy is a new technical option which, besides allowing Preservation of the anatomical structures, provides contractility support and protection to the myocardium during ventricular diastole. The technique requires further studies that document its results. OBJECTIVE: To evaluate the left atrial and ventricular function by Doppler echocardiography in patients who have undergone mitral valve replacement with crossed papillopexy. METHODS: Fifteen patients underwent mitral valve replacement, 9 (60%) of them male, and the mean age was 45.7 years. As to the etiology of aortic valve disease, nine (60%) cases were degenerative, three (20%) were rheumatic, two (13.3%) were ischemic, and one patient (6.7%) had infectious endocarditis. After closure of the atriotomy and anatomical evaluation of the valvar apparatus, the anterior leaflet was detached from the annulus and centrally split in halves, each one with its complex tendinous chords attached to the opposing commissure by its medial extremity. Biological (13 cases) or mechanical prosthetic valves were implanted and secured with separate stitches. Reduction of the valvar annulus was performed in patients with dilated cardiomyopathy. Patients underwent clinical and Doppler echocardiographic examinations before surgery and six months after the procedure. RESULTS: All patients were clinically stable at discharge. A significant reduction in ventricular and atrial diameters was demonstrated (p< 0.001) without impairment of the entry and exit points of the left ventricle. CONCLUSION: Mitral valve replacements performed with the crossed papillopexy technique showed favorable results with a positive effect on the recovery of left atrial and ventricular morphology.

FUNDAMENTO: Os efeitos do envelhecimento no músculo papilar têm sido amplamente demonstrados, mas não há dados disponíveis sobre os efeitos do exercício nas alterações relacionadas à idade. OBJETIVO: Analisar os efeitos do envelhecimento nas propriedades morfológicas e quantitativas do músculo papilar e investigar se um programa contínuo de exercícios moderados pode exercer um efeito protetor contra as conseqüências do envelhecimento. MÉTODOS: Microscopia eletrônica foi utilizada para estudar a densidade dos miócitos, capilares e tecido conectivo e área transversal dos miócitos do músculo papilar no ventrículo esquerdo de ratos Wistar de 6 e 13 meses, não-treinados e submetidos a exercícios. OBJETIVO: Analisar os resultados em curto e médio prazo de pacientes portadores de insuficiência cardíaca e insuficiência mitral moderada/grave submetidos a substituição valvar mitral com a técnica da papilopexia cruzada e constrição anular. MÉTODOS: Treze pacientes em classe funcional III ou IV (NYHA), idade média de 54,1 anos, etiologia idiopática, foram submetidos a substituição valvar mitral com constrição do anel e papilopexia cruzada. Foram analisados os parâmetros ecocardiográficos, classe funcional e curva atuarial de sobrevivência. RESULTADOS: Não houve mortes no período trans e pósoperatório imediato. A média dos diâmetros diastólicos e sistólicos ventricular esquerdo reduziu de 71 ± 8,6 mm para 65,3 ± 8,6 mm (p=0,049) e de 59,1 ± 8,5 mm para 50,4 ± 11,1 mm (p=0,002), respectivamente. Os diâmetros atriais variaram de 49,4 ± 6,4 mm para 44 ± 5,9 mm (p=0,017); o percentual de encurtamento sistólico do ventrículo esquerdo foi de 17 ± 4% para 24 ± 8,3% (p=0,014), a fração de ejeção variou de 34 ± 9% para 45 ± 14% (p=0,008), todos estatisticamente significativos. Onze (84,6%) pacientes se encontram em classe funcional I e II. A sobrevivência foi de 100%, 82,6%, 71,6%, respectivamente para 1, 6 e 12 meses após a cirurgia de seguimento, mantendo-se 71,6% em período superior a 36 meses. CONCLUSÃO: Os resultados obtidos, em pacientes com insuficiência cardíaca e insuficiência mitral moderada/grave submetidos a substituição valvar mitral com papilopexia cruzada e constrição anular, apresentaram evidências de remodelamento cardíaco favorável e significativa recuperação funcional ventricular esquerda.OBJECTIVE: To analyze the short-term and mid-term follow-ups of patients with heart failure and moderate to severe mitral valve insufficiency and who have undergone mitral valve replacement with crossed papillopexy and annular constriction. METHODS: Thirteen patients in NYHA functional class III or IV, with a mean age of 54.1 years and with idiopathic etiology, underwent mitral valve replacement with ring constriction and crossed papillopexy. Echocardiograph parameters, functional class and survival actuarial curve were analyzed. RESULTS: There were no deaths during surgery or in the postoperative period. The mean left ventricular diastolic diameter was reduced from 71 ± 8.6 mm to 65.3 ± 8.6 mm (p=0.049) and the mean left ventricular systolic diameter was reduced from .1 ± 8.5 mm to 50.4 ± 11.1 mm (p=0.002). The atrial diameters varied from 49.4 ± 6.4 mm to 44 ± 5.9 mm (p=0.017); the percentage of the left ventricular shortening was 17 ± 4 % to 24 ± 8.3% (p=0.014); the ejection fraction varied statistically and significantly from 34 ± 9% to 45 ± 14% (p=0.008). Eleven (84.6%) patients were in FC I and II. At 1, 6 and 12 months after follow-up surgery, the survival rate was 100%, 82.6%, 71.6%, respectively. This rate was maintained at 7.6% for more than 36 months. CONCLUSION: The results obtained from patients with heart failure and moderate to severe mitral valve insufficiency and who underwent mitral valve replacement with crossed papillopexy and annular constriction presented evidence of improved heart remodeling and significant improvement in left ventricular performance.

O objetivo deste artigo é mostrar a técnica da papilopexia cruzada com o uso de enxerto de pericárdio bovino na substituição mitral por fibrose e calcificação intensa valvar e subvalvar. Esta técnica permite a preservação funcional do aparelho subvalvar mitral e foi aplicada pela primeira vez, na literatura alcançada, em paciente portador de estenose mitral reumática calcificada, submetido a troca valvar mitral. O paciente evoluiu sem complicações, com alta hospitalar em seis dias. Controles ecocardiográficos mostraram função ventricular esquerda preservada sem interferência na dinâmica da prótese mitral.The aim of this study is to present the crossed papillopexy technique using bovine pericardial graft in mitral replacement caused by intense valvar and subvalvar fibrosis and calcification. This technique allows the functional preservation of the mitral subvalvar apparatus and was successfully applied, by the first time as far as we known, in a patient with calcified rheumatic mitral stenosis, who underwent mitral valve replacement surgery. The patient had an uneventful recovery and was discharged home on postoperative day 6. Echocardiography controls showed normal left ventricular function without any interference on the mitral prosthesis dynamics.

OBJETIVO: Apresentar a técnica de papilopexia cruzada e seus resultados iniciais na preservação dos músculos papilares em operações de substituição valvar mitral e remodelamento ventricular na insuficiência cardíaca (ICC). MÉTODO: Foram estudados dez pacientes submetidos à cirurgia de troca valvar mitral, sendo sete (70%) do sexo masculino, com idade variável entre 15 e 75 anos (média de 44,4 ± 18,7 anos) portadores de disfunção valvar mitral reumática (50%), prolapso valvar mitral (10%) ou miocardiopatia dilatada e ICC (40%). Após atriotomia e adequada exposição da valva mitral, a cúspide anterior foi desinserida do anel e centralmente dividida, sendo cada metade, com seu complexo de cordas tendíneas, fixada à comissura oposta por sua extremidade medial. Em seguida, foi implantada a prótese valvar mecânica (sete casos) ou bioprótese porcina (três casos) fixada ao anel valvar por meio de pontos separados, com redução do anel valvar mitral, para melhor remodelamento ventricular nos casos com ICC. RESULTADOS: Todos os pacientes receberam alta hospitalar em condições clínicas estáveis, apresentando melhora do desempenho cardíaco no final do primeiro mês de pós-operatório, com redução significativa (pOBJECTIVE: To present the crossed papillopexy technique and its initial results in the preservation of papillary muscles in mitral valve replacement and ventricular remodeling surgeries for heart failure (CHF). METHOD: Ten patients, 70% male, with ages between 15 and 75 years old (mean 44.4 ± 18.7 years old), suffering from rheumatic mitral valve disease (50%), mitral valve prolapse (10%) or dilated cardiomyopathy (40%), were studied. After opening the left atrium and adequate exposure of the mitral valve, the anterior leaflet already free of its annulus fixation was centrally divided and each half, with its cordae tendineae complex fixed to the commissure on the opposite side. Following this, mechanical (seven cases) or biological (three cases) prostheses were implanted using single sutures in the valve annuli, with reduction of the mitral valve annulus for better ventricular remodeling in CHF cases. Abstract This paper presents a case study of a 30-year-old male patient with dyspnea on exertion had echocardiographic diagnosis of aortic subvalvar stenosis. Discrete mitral regurgitation and aortic valve dysplasia with mild to moderate insufficiency and hypertrophic cardiomyopathy were also noted. During surgery, a rare condition was identified: presence of papillary muscle anomaly associated with the subaortic membrane as a cause of obstruction of the left ventricular outflow tract. With the resection of these structures and a mitral valve annuloplasty, the patient evolved with a significant improvement of clinical condition and heart failure, with no residual mitral insufficiency.
