Abstract

Background: Our previous research showed that, in Alberta, Canada, a higher proportion of visits to emergency departments and urgent care centres by First Nations patients ended in the patient leaving without being seen or against medical advice, compared with visits by non-First Nations patients. We sought to analyze whether these differences persisted after controlling for patient demographic and visit characteristics, and to explore reasons for leaving care. Methods: We conducted a mixed-methods study, including a population-based retrospective cohort study for the period of April 2012 to March 2017 using provincial administrative data. We used multivariable logistic regression models to control for demographics, visit characteristics, and facility types. We evaluated models for subgroups of visits with preselected illnesses. We also conducted qualitative, in-person sharing circles, a focus group, and 1-on-1 telephone interviews with health directors, emergency care providers, and First Nations patients from 2019 to 2022, during which we reviewed the quantitative results of the cohort study and asked participants to comment on them. We descriptively categorized qualitative data related to reasons that First Nations patients leave care. Results: Our quantitative analysis included 11 686 287 emergency department visits, of which 1 099 424 (9.4%) were by First Nations patients. Visits by First Nations patients were more likely to end with them leaving without being seen or against medical advice than those by non-First Nations patients (odds ratio 1.96, 95% confidence interval 1.94-1.98). Factors such as diagnosis, visit acuity, geography, or patient demographics other than First Nations status did not explain this finding. First Nations status was associated with greater odds of leaving without being seen or against medical advice in 9 of 10 disease categories or specific diagnoses. In our qualitative analysis, 64 participants discussed First Nations patients' experiences of racism, stereotyping, communication issues, transportation barriers, long waits, and being made to wait longer than others as reasons for leaving. Interpretation: Emergency department visits by First Nations patients were more likely to end with them leaving without being seen or against medical advice than those by non-First Nations patients. Over the past ten years, the Federal Open Market Committee (FOMC) has repeatedly emphasized that future policy is data dependent. In this Economic Commentary, we investigate how financial markets expected future interest rates to change with the release of new data on inflation and labor market conditions. We find that the surprises in economic indicators have a stronger effect on the 2-year Treasury yield than on the expected federal funds rate to be set in the next FOMC meeting. This implies that markets understand that under the data-dependent approach, policy decisions do not heavily rely on the most recent data or short-run fluctuations, but, rather, rely more on the persistent trend of the economy. 5. Is the Missing Letter Effect Due Primarily to the Test Word Containing the Target Letter or to the Surrounding Words?

A simple letter detection task, in which subjects mark instances of a target letter in prose passages, has elucidated numerous cognitive processes involved in reading by examining the "missing letter" effect, in which readers' detection accuracy is especially low on frequent function words. Two experiments explore the fundamental but novel issue of whether the missing letter effect is due to the test word containing the target letter or to the words surrounding the test word. College students searched for a target letter (e in Experiment 1, o in Experiment 2) in a passage that included unrelated sentences, with each sentence containing a single instance of 1 of 2 test words (the or one in Experiment 1, of or on in Experiment 2). The sentences were intact (prose), or the words in each sentence were randomly rearranged (scrambled). The 2 test words in an experiment were surrounded by the exact same words. If the word containing the target letter is primarily responsible for the missing letter effect, the proportion of correct letter detection responses should depend on the test word, whereas if the surrounding words are primarily responsible, it should depend on the text type (prose, scrambled). In fact, in both experiments a huge effect of test word was found but no effect of text type. These results provide clear evidence for the influence of the test word but, surprisingly, no evidence for the influence of the surrounding words on the missing letter effect in the letter detection task. Tailoring text message interventions (TMIs) can help individuals with type 2 diabetes (T2D) to adhere to health behaviors. This study is part of the development of an adaptive TMI that uses participant characteristics and ongoing feedback to send text messages to participants. As a first step, 100 adults with T2D were surveyed to assess their preferences for different text message types (i.e., well-being, physical activity [PA], diet, and self-care) and identify participant characteristics associated with those preferences. Univariate analyses showed that employed individuals and those with more education preferred PA messages, those with a longer duration of T2D preferred self-care messages, and those who wished to change a health behavior preferred messages related to that behavior. This study examined 3 theoretical issues involving working memory (WM) processes, movement dimensions, and serial positions. This examination was made in the context of investigating the ability to follow navigation instructions in a situation that mimicked communication between air traffic controllers and flight crews. Subjects heard navigation instructions describing movement within a 2-dimensional (2-D) computer display of a 3-D space consisting of four 4*4 stacked matrices. They repeated the instructions and then followed them by making movements on the computer screen. The 3-D space was shown from 2 different orientations (with the matrices stacked top to bottom or front to back). The effects of movement dimension depended on orientation; the dimension that was most difficult always involved movement outside the picture plane. The movement dimension effects also depended on the response requirements (mouse or keypad); they occurred only when a spatial WM representation was needed to make the movements (mouse). Likewise, serial position effects for the immediate oral repetition responses depended on the subsequent response requirements, providing a clear and strong demonstration that response requirements of the navigation task influence memory representations and memory performance, even for immediate verbatim recall completed before making the responses. More generally, these results support and elucidate the broader proposal that spatial and verbal WM processes are interdependent. The practical implications of these findings are discussed. River ecosystems have been altered by flow regulation and species introductions. Regulated flow regimes often include releases designed to benefit certain species or restore ecosystem processes, and invasive species suppression programs may include efforts to restrict access to spawning habitat. The impacts of these management interventions are often uncertain. Here, we assess hypotheses regarding introduced brown trout (Salmo trutta) movement in a regulated river. We model mark- recapture data in a multistate framework to assess whether movement was affected by the operation of a tributary weir (restricting access to spawning habitat), experimental releases of fall-timed high flow experiments (Fall HFEs), or simply increased during the fall, spawning season. Our results suggest that the presence of the weir led to reduced tributary homing and the release of Fall HFEs stimulated upstream movement and straying. Both effects are of a similar magnitude, however the Fall HFE effect is more certain. Our results suggest the expansion of an invasive species was stimulated by management interventions, and demonstrate the potential for unanticipated outcomes of restoration in highly altered river ecosystems. Some argue that the Fed underreacted to rising inflation in 2021 after the US economy started to recover from the COVID-19 crisis. By using data from the Summary of Economic Projections (SEP), we surmise that the FOMC expected to keep the federal funds rate near zero by the end of 2021, but at the same time, the committee also expected to make the policy rate catch up to inflation over the next two years. We then argue that the Fed chose this gradual approach in response to the negative demand shock that pushed the policy rate to its effective zero lower bound.
