Lung cancer has the highest incidence and cancer-related mortality worldwide. In Portugal, it ranks as the fourth most common cancer, with nearly 6000 new cases being diagnosed every year. Lung cancer is the main cause of cancer-related death among males and the third cause of cancer-related death in females. Despite the globally accepted guidelines and recommendations for what would be the ideal path for a lung cancer patient, several challenges occur in real clinical management across the world. The recommendations emphasize the importance of adequate screening of high-risk individuals, a precise tumour biopsy, and an accurate final diagnosis to confirm the neoplastic nature of the nodule. A detailed histological classification of the lung tumour type and a comprehensive molecular characterization are of utmost importance for the selection of an efficacious and patient-directed therapeutic approach. However, in the context of the Portuguese clinical organization and the national healthcare system, there are still several gaps in the ideal pathway for a lung cancer patient, involving aspects ranging from the absence of a national lung cancer screening programme through difficulties in histological diagnosis and molecular characterization to challenges in therapeutic approaches. In this manuscript, we address the most relevant weaknesses, presenting several proposals for potential solutions to improve the management of lung cancer patients, helping to decisively improve their overall survival and quality of life.

Purpose: To determine near and intermediate vision areas of progressive addition lenses by means of a deflectometer., Methods: Twenty-two progressive addition lenses with +1.00 SD far power and two different additions (add 1.00 and 2.00; eleven subjects in each addition) were studied. Near and intermediate vision areas within 0.50 CD isoastigmatic lines were determined., Results: There are significant differences between near and intermediate vision areas of the studied lenses. There is also an inverse correlation between the addition and intermediate areas as well as direct relation between the vertical length of the corridor and its area., Conclusion: Based on those findings, progressive addition lenses can be selected to suit the wearer's visual requirements.

Purpose: To analyze visual acuity without correction and rotational stability outcomes following toric IOL implantation., Methods: Prospective study of 20 eyes of 13 patients that underwent phacoemulsification surgery indicated for cataract associated with regular keratomeric astigmatism, symmetrical, ranging from 1 to 4 diopters. Best corrected visual acuity, refraction, keratometry and computed topography were performed preoperatively. The calculation of cylindrical lens power and its placement were determined by the manufacturer. All lenses were implanted in the capsular bag by the same surgeon. The patients were examined by a second independent observer, at 1st, 10th, 20th, 30th, and 60th postoperative day., Results: Visual acuity without correction ranged between 20/15 and 20/40. One eye achieved 20/15 (5%), 4 eyes 20/20 (20%), 6 eyes 20/25 (30%), 7 eyes 20/30 (35%) and 2 eyes 20/40 (10%). Best corrected visual acuity ranged between 20/15 and 20/40; two eyes with 20/15 (10%), 9 eyes 20/20 (45%), 7 eyes 20/25 (35%), 1 eye 20/30 (5%) and 1 eye 20/40 (5%). It is important to remember that the average spherical refraction was -0.05 SD (ranging from -0.50 to +0.75 SD). The mean cylindrical refraction was -0.63 CD ranging from -0.50 to -1.25 CD. The IOL rotation in this study had an average of 3.2 masculine to 30 masculine, ranging from 0 masculine of rotation to a maximum of 13 masculine; 7 lenses (35%) suffered no rotation, 9 lenses (45%) suffered rotation between 1 masculine to 5 masculine, 3 lenses (15%) had rotation between 6 masculine to 10 masculine, and ultimately 1 lens (5%) had rotation between 11 masculine to 15 masculine. There was no significant rotation after the 30th postoperative day., Discussion: The average of rotation of the IOL was 3.2 masculine, where 95% of IOLs presented rotation less than or equal to 10 masculine what means a very good rotational stability. In daily practice, a good visual acuity is directly related to IOL rotational stability and refractive predictability.

Ocular adnexal lymphoid tumors are slow-growing neoplasms that affect predominantly the elderly. The purpose of this study is to report a non-Hodgkin lymphoma from mantle zone cells, a rare ocular annexal B-cell lymphoma subtype., Case Report: A 62-year-old male patient with superior eyelid, maxillary and oral tumours, had an initial diagnosis of Mikulics disease, developed lesion enlargement, despite immunosuppressant therapy. Left superior eyelid and bone marrow biopsy revealed non-Hodgkin lymphoma the mantle zone. In spite of dissemination (stage IV), chemotherapy and bone marrow transplant led to disease remission., Conclusive Commentaries: Despite the availability of advanced complementary diagnostic methods, like immunophenotyping and molecular genetic analysis, lymphoma represents, for physicians and pathologists, a challenge regarding diagnosis and prognosis.
