The physician's perspective on clinical assessment skills by nurses in psychiatry: A case study Abstract: Background: People with a mental and physical illness receive less good somatic care compared to the general population. Patients in psychiatry receive better medical care when nurses have in-depth medical expertise. Therefore, in a psychiatric institution in Switzerland in 2017, the Clinical Assessment and Decision Making (CADM) by nurses was implemented. Aim: The aim of the study was to investigate how physicians and senior psychologists experience the implementation of CADM by nurses in order to derive recommendations on how to optimize the collaboration and promote a sustainable establishment. Methods: An embedded single case study was used, and the data analysis was based on grounded theory strategies by Charmaz. Semi-structured expert interviews (n = 11) and open unstructured observations were conducted in a psychiatric institution in Switzerland. Results: The results showed nine relevant aspects regarding collaboration and CADM by nurses: "Strengths", "Weaknesses", "Risks", "Opportunities", "Expectations", "Challenges", "Benefits", "Competencies of CADM nurses" and "Wishes for the future". Conclusions: The physicians and senior psychologists considered the CADM by nurses as an added value for the interprofessional team and for the patients. Lack of clarity regarding the scope of responsibilities, role definitions and possible applications made the implementation of CADM difficult.

Inpatient Schema Therapy for School Avoidance - A Case Study Abstract. After a short overview of the basic concepts and methods of schema therapy, we report a case study on the inpatient treatment of a 16-year-old youth with depression and chronic school avoidance. The scores on the Schema inventories and Depression questionnaires are compared before vs. after the treatment, and we then discuss the opportunities and limitations of schema therapy with school-avoiding youths.

Family-Based Trauma-Focused Cognitive Behavioral Therapy with Three Siblings of a Refugee Family The possibility and relevance of a joint trauma-therapy with siblings has yet received little attention in research and clinical practice. The following case study presents a joint family-based trauma-focused therapy process with a refugee family. All three siblings suffered from post-traumatic stress disorder (PTSD) before treatment. The treatment followed the manual of Trauma Focused Cognitive Behavioral Therapy (TF-CBT; Cohen, Mannarino, Deblinger, 2009). Measures were the short version of the Child and Adolescent Trauma Screen (CATS 7-17), as well as the Teacher's Report Form (TRF). After 18 treatment sessions together with the mother, all three children did no longer meet PTSD criteria. Benefits of the joint therapy were for all three siblings to be sharing and imitating each other's coping strategies. Furthermore, the protective factor of social support after experiencing a traumatic event became evident. The apprehension of the therapist not being sufficiently neutral towards all three siblings was not observed.

This article highlights the feasibility of a dyadic prolonged exposure (DPE) intervention (L. Rachamim, I. Mirochnik, L. Helpman, N. Nacasch, & E. Yadin, ) in a 3-year-old preschooler and in a 6-year-old kindergartener immediately following the traumatic death of their younger sibling. It presents a detailed case description of the DPE treatment addressing traumatic grief and includes transcribed treatment dialogue. At the time of treatment termination, both children and caregivers resumed normal functioning. The results suggest that DPE intervention may ameliorate posttraumatic grief symptoms in young children. Through commentary on four clinical vignettes, this article focuses on the anthropological transformations taking place in contemporary society, underlining their differences from the anthropologies of reference of the founding fathers of psychoanalysis. Hybridization between man and machine and the speeding up and alteration of communications which the new technologies promote are now crucial issues facing psychoanalysis. Social media and a 24/7 internet connection have produced deep changes in the way people live and perceive relationships. Analytical practice is not exempt from such issues, which can be particularly insidious, often subtle and difficult to recognize, or even underestimated or ignored by psychoanalysts outright, in order to preserve the illusion of a complete understanding of what unfolds in the analytical space. The authors suggest that such transformations, by (partially) rendering inadequate the theoretical and technical corpus on which the various depth psychologies are founded, require personal engagement on the part of psychoanalysts in the search for new strategies to treat their patients, with the consequent abandonment of the 'certainties' offered by sclerotic models of clinical procedure., (© 2017, The Society of Analytical Psychology.)

Background: The purpose of periodic health examinations is to prevent disease through early intervention and through stratifying individual patients' health risk. The German health examination as defined in section 25 of the Social Code Book V (Sozialgesetzbuch V) seems to be outdated in many respects. In order to develop an alternative to the current German system an international comparison of periodic health examinations in other healthcare systems is useful., Objective: The study investigates the measures taken for primary prevention of cardiovascular disease in seven countries. Content, implementation and organisation of the various programmes are analysed in the present paper., Methods: The present study was designed as a qualitative multiple case study. The collection of data on cardiovascular screening programmes in the seven different healthcare systems in this study relies on information publicly available on the Internet. The data were entered into a matrix which had been created prior to the data collection and which covers the relevant points of screening. Finally, the data were triangulated using guided telephone interviews with key informants. One general practitioner (GP) was interviewed per country. The measures taken in the respective healthcare systems were then compared and analysed., Results: The measures taken in the countries studied for the primary prevention of cardiovascular disease are heterogeneous. The structures range from guideline recommendations (Castilla y León, Switzerland, Norway), and incentive payments for doctors (British Columbia), to opportunistic and population-based programmes (Germany and England, and Group Health in the US). The American health maintenance organisation Group Health offers an established, evidence-based programme whereas the German health examination dates back to a time when evidence-based medicine was not yet established and is therefore relatively outdated., Discussion: Despite scientific evidence of primary prevention of cardiovascular disease, evidence-based measures cannot be found in all the healthcare systems analysed.
