Purpose: To assess the visual impact of Diffusion Optics Technology™ 0.2 DOT lenses (SightGlass Vision Inc.) designed for myopia control on primary gaze. DOT spectacle lenses contain light scattering elements that scatter light as it passes through the lens which, in turn, reduces retinal image contrast., Methods: Fifty-one children (12.2 ± 1.3, range 10-14 years; 51% females) were randomly assigned to wear DOT spectacle (n = 27) or single vision lenses (n = 24) across six investigational sites in North America. Binocular high- and low-contrast distant visual acuities, near visual acuity, reading speed, contrast sensitivity, stereoacuity and glare were assessed in primary gaze after at least 3 years of wear, with the study 95% powered in all metrics to detect significant differences between the groups., Results: Mean binocular distance high-contrast (-0.09 ± 0.02 vs. -0.08 ± 0.02 logMAR, p = 0.81), low-contrast (0.05 ± 0.02 vs. 0.07 ± 0.02 logMAR, p = 0.52) and near visual acuity with glare sources (-0.06 ± 0.03 vs. -0.09 ± 0.03 logMAR, p = 0.32) were similar for DOT and single vision lens wearers, respectively. Contrast sensitivity was similar between children wearing DOT or single vision lenses across 11 of the 16 spatial frequencies (p > 0.05). Mean stereopsis was similar (p = 0.30) with the DOT lenses (33.2 ± 12.5″) and single vision lenses (38.1 ± 14.2″). Functional reading speed metrics were similar in both study groups, as was the objectively measured head tilt during reading (p > 0.05). The mean halo radius was 0.56° ± 0.17° with the DOT lenses compared with 0.50° ± 0.12° with single vision lenses (p = 0.02), but the statistically significant difference was smaller than the non-inferiority bound of 0.4°., Conclusion: Diffusion optics technology lenses provide a clinically equivalent visual experience to a standard single vision lens., (© 2024 SightGlass Vision. Significance: Treatment of myopic children with a dual-focus soft contact lens (DFCL; MiSight 1 day) produced sustained slowing of myopia progression over a 6-year period. Significant slowing was also observed in children switched from a single vision control to treatment lenses (3 years in each lens)., Purpose: This study aimed to evaluate the effectiveness of DFCLs in sustaining slowed progression of juvenile-onset myopia over a 6-year treatment period and assess myopia progression in children who were switched to a DFCL at the end of year 3., Methods: Part 1 was a 3-year clinical trial comparing DFCLs with a control contact lens (Proclear 1 day) at four investigational sites. In part 2, subjects completing part 1 were invited to continue for 3 additional years during which all children were treated with MiSight 1 day DFCLs (52 and 56 from the initially treated [T6] and control [T3] groups, respectively). Eighty-five subjects (45 [T3] and 40 [T6]) completed part 2. Cyclopleged spherical equivalent refractive errors (SEREs) and axial lengths (ALs) were monitored, and a linear mixed model was used to compare their adjusted change annually., Results: Average ages at part 2 baseline were 13.2 ± 1.3 and 13.0 ± 1.5 years for the T6 and T3 groups, respectively. Slowed myopia progression in the T6 group observed during part 1 was sustained throughout part 2 (mean ± standard error of the mean: change from baseline SERE [in diopters], -0.52 ± 0.076 vs. -0.51 ± 0.076; change in AL [in millimeters], 0.28 ± 0.033 vs. 0.23 ± 0.033; both P > .05). Comparing progression rates in part 2 for the T6 and T3 groups, respectively, indicates that prior treatment does not influence efficacy (SERE, -0.51 ± 0.076 vs. -0.34 ± 0.077; AL, 0.23 ± 0.03 vs. 0.18 ± 0.03; both P > .05). Within-eye comparisons of AL growth revealed a 71% slowing for the T3 group (3 years older than part 1) and further revealed a small subset of eyes (10%) that did not respond to treatment., Conclusions: Dual-focus soft contact lenses continue to slow the progression of myopia in children over a 6-year period revealing an accumulation of treatment effect. Purpose: To report on the ocular health and safety of children fit with soft hydrogel daily-disposable contact lenses, and followed for 6-years in a double-masked clinical trial investigating the performance of a dual-focus contact lens designed to control myopia progression., Methods: Children aged 8-12 years, naïve to contact lens wear, were enrolled across four international sites. During years 1-3, children were randomised to either MiSight® 1 day or Proclear® 1 day (both omafilcon A, CooperVision, Inc.). The lenses were identical in material and geometry except for the front optical zone design. At the end of year-3, all those wearing Proclear 1 day were switched to MiSight 1 day, therefore all wore MiSight 1 day in years 4-6. Subjects agreed to wear the lenses at least 10-hours/day, 6-days/week. After dispensing, study visits were at 1-week, 1-month, 6-months and every 6-months until 6-years. At each visit, ocular measurements and subjective responses were recorded. Biomicroscopy used 0-4 grading scales; grade 0 represented no findings., Results: 144 children were enrolled: 69F:75M; mean age 10.1 years; mean cycloplegic spherical-equivalent refraction -2.11D; ethnicities included 34 East-Asian, 12 West-Asian, and 79 Caucasian. 92 completed the 6-years. Only three subjects discontinued due to an ocular adverse event (AE). No contact lens related AEs were classified as serious. The incidence rate of infiltrative AEs was 0.61% (6.1/1000 wearing-years; 95%CI: 0.24%-1.57%). The most common biomicroscopy findings were limbal, bulbar and tarsal hyperaemia and tarsal roughness. 99% of all biomicroscopy findings were grade-1 or lower. After 6-years of lens wear, ocular health by biomicroscopy was similar to pre-lens wear., Conclusions: Across the 6-years, there were no contact lens related serious AEs and biomicroscopy showed no significant changes. Pollen retention on clothes, footwear, hair and body has been used to link people to localities with distinctive vegetation, or soils containing distinctive palynomorphs. Little attention has been given to human skin as a possible medium for carrying a forensically important pollen load and whether this might survive attempts to remove it. We report here the results of experiments testing the retention of pollen of 10 flowering plant species on the human skin through repeated cycles of washing and drying hands, using the WHO protocol to standardize hand-washing and drying. Between 0.36% and 2.74% (mean 0.93%) of the initial pollen load was retained through a single hand-wash. Trace amounts of some species survived multiple hand-wash cycles. It is concluded that forensic analyses can be made of the pollen load of those parts of the skin that may have been in contact with palynologically distinctive vegetation, even in cases where the person involved has washed, or been washed. Mortuary behavior (activities concerning dead conspecifics) is one of many traits that were previously widely considered to have been uniquely human, but on which perspectives have changed markedly in recent years. Theoretical approaches to hominin mortuary activity and its evolution have undergone major revision, and advances in diverse archeological and paleoanthropological methods have brought new ways of identifying behaviors such as intentional burial. Despite these advances, debates concerning the nature of hominin mortuary activity, particularly among the Neanderthals, rely heavily on the rereading of old excavations as new finds are relatively rare, limiting the extent to which such debates can benefit from advances in the field. The recent discovery of in situ articulated Neanderthal remains at Shanidar Cave offers a rare opportunity to take full advantage of these methodological and theoretical developments to understand Neanderthal mortuary activity, making a review of these advances relevant and timely., (© 2020 The Authors. Purpose: To determine the retention rate (RR) for neophyte patients fitted with contact lenses (CLs) and to identify factors associated with retention and dropout., Method: This multisite, sponsor-masked registry study evaluated the 1-year status of neophyte CL wearers fitted in a representative range of UK practices. A total of 26 UK investigational sites each recruited up to 89 neophyte patients. The participants were requested to complete surveys at 1 month (±1 week from date of fitting), 3 months (±2 weeks), and 12 months (±3 weeks) online or through phone. Participants were contacted by email (or phone) and invited to complete an online survey that asked a range of questions regarding their CL wearing experience., Results: Of the 531 participants recruited, 65.3% (347/531) responded to at least one follow-up survey and 42.3% (225/531) responded to all three surveys. One in four patients showed discontinued CL wear by 12 months. Based on evaluable subjects at 12 months, the RR was 77.6% (194/250, 95% confidence interval: 72.0-82.3). The most common reason for discontinuation was problems with vision (41%), followed by discomfort (36%) and handling problems (25%). The factors affecting first-year RRs: sex, spherical refraction, lens usage (full time or part time), and supply method (collected or posted)., Conclusions: In this prospective study, the first-year RR for neophyte CL wearers was 77.6%. This was consistent with that noted in a retrospective study: 74.0%. As with the previous study, the commonly cited reason for discontinuation was visual problems.

Imaging photoplethysmography (iPPG) is an emerging technology used to assess microcirculation and cardiovascular signs by collecting backscattered light from illuminated tissue using optical imaging sensors. The aim of this study was to study how effective smart garment fabrics could be capturing physiological signs in a non-contact mode. The present work demonstrates a feasible approach of, instead of using conventional high-power illumination sources, integrating a grid of surface-mounted light emitting diodes (LEDs) into cotton fabric to spotlight the region of interest (ROI). The green and the red LEDs (525 and 660 nm) placed on a small cotton substrate were used to locally illuminate palm skin in a dual-wavelength iPPG setup, where the backscattered light is transmitted to a remote image sensor through the garment fabric. The results show that the illuminations from both wavelength LEDs can be used to extract heart rate (HR) reaching an accuracy of 90% compared to a contact PPG probe. Stretching the fabric over the skin surface alters the morphology of iPPG signals, demonstrating a significantly higher pulsatile amplitude in both channels of green and red illuminations. The skin compression by the fabric could be potentially utilised to enhance the penetration of illumination into cutaneous microvascular beds. The Neanderthal remains from Shanidar Cave, excavated between 1951 and 1960, have played a central role in debates concerning diverse aspects of Neanderthal morphology and behavior. In 2015 and 2016, renewed excavations at the site uncovered hominin remains from the immediate area where the partial skeleton of Shanidar 5 was found in 1960. Shanidar 5 was a robust adult male estimated to have been aged over 40 years at the time of death. Comparisons of photographs from the previous and recent excavations indicate that the old and new remains were directly adjacent to one another, while the disturbed arrangement and partial crushing of the new fossils is consistent with descriptions and photographs of the older discoveries. The newly discovered bones include fragments of several vertebrae, a left hamate, part of the proximal left femur, a heavily crushed partial pelvis, and the distal half of the right tibia and fibula and associated talus and navicular. All these elements were previously missing from Shanidar 5, and morphological and metric data are consistent with the new elements belonging to this individual. A newly discovered partial left pubic symphysis indicates an age at death of 40-50 years, also consistent with the age of Shanidar 5 estimated previously. Thus, the combined evidence strongly suggests that the new finds can be attributed to Shanidar 5. Ongoing analyses of associated samples, including for sediment morphology, palynology, and dating, will therefore offer new evidence as to how this individual was deposited in the cave and permit new analyses of the skeleton itself and broader discussion of Neanderthal morphology and variation., (Copyright © 2017 Elsevier Ltd. Significant human impacts on tropical forests have been considered the preserve of recent societies, linked to large-scale deforestation, extensive and intensive agriculture, resource mining, livestock grazing and urban settlement. Cumulative archaeological evidence now demonstrates, however, that Homo sapiens has actively manipulated tropical forest ecologies for at least 45,000 years. It is clear that these millennia of impacts need to be taken into account when studying and conserving tropical forest ecosystems today. Nevertheless, archaeology has so far provided only limited practical insight into contemporary human-tropical forest interactions. Here, we review significant archaeological evidence for the impacts of past hunter-gatherers, agriculturalists and urban settlements on global tropical forests. We compare the challenges faced, as well as the solutions adopted, by these groups with those confronting present-day societies, which also rely on tropical forests for a variety of ecosystem services. We emphasize archaeology's importance not only in promoting natural and cultural heritage in tropical forests, but also in taking an active role to inform modern conservation and policy-making.

The paper presents the results of optical dating of potassium-rich feldspar grains obtained from the Haua Fteah cave in Cyrenaica, northeast Libya, focussing on the chronology of the Deep Sounding excavated by Charles McBurney in the 1950s and re-excavated recently. Samples were also collected from a 1.25 m-deep trench (Trench S) excavated during the present project below the basal level of the Deep Sounding. Optically stimulated luminescence (OSL) data sets for multi-grain, single aliquots of quartz for samples from the Middle Trench were previously published. Re-analyses of these OSL data confirm significant variation in the dose saturation levels of the quartz signal, but allow the most robust OSL ages to be determined for comparison with previous age estimates and with those obtained in this study for potassium-rich feldspars from the Deep Sounding. The latter indicate that humans may have started to visit the cave as early as ∼150 ka ago, but that major use of the cave occurred during MIS 5, with the accumulation of the Deep Sounding sediments. Correlations between optical ages and episodes of "Pre-Aurignacian" artefact discard indicate that human use of the cave during MIS 5 was highly intermittent. The earliest phases of human activity appear to have occurred during interstadial conditions (5e and 5c), with a later phase of lithic discard associated with more stadial conditions, possibly MIS 5b. We argue that the "Pre-Aurignacian" assemblage can probably be linked with modern humans, like the succeeding "Levalloiso-Mousterian" assemblage; two modern human mandibles associated with the latter are associated with a modelled age of 73-65 ka. If this attribution is correct, then the new chronology implies that modern humans using "Pre-Aurignacian" technologies were in Cyrenaica as early as modern humans equipped with "Aterian" technologies were in the Maghreb, raising new questions about variability among lithic technologies during the initial phases of modern human dispersals into North Africa., (Copyright © 2017 Elsevier Ltd. Purpose: To determine the first-year retention rate for patients fitted with contact lenses (CLs) and identify factors associated with retention and dropout., Methods: This multi-site study was a retrospective chart review of the status of neophyte CL wearers fitted in representative UK eye care practices., Results: Consecutive records for 524 patients at 29 sites were reviewed. Mean age at dispensing was 34 years (range 8-79), 68% were under 45 years and 61% female. Soft CLs were fitted to 98% of patients. After 12 months, 388 were still CL wearers, a retention rate of 74% (95% CI: 70.1-77.6). Of the 136 lapsed, 25% discontinued during the first month and 47% within 60 days. The main reasons cited for discontinuation included poor distance vision (26%; of whom, 37% were toric and 51% multifocal), poor near vision (16%), discomfort (14%) and handling problems (15%). In 32% of cases, the reasons for discontinuation were unknown. For 71% of dropouts, no alternative lens or management strategy had been tried. Significant factors associated with retention in univariate analysis were: age (younger), sphere power (higher), lens type (sphere vs multifocal) and purchase frequency (regular). Multivariate analysis showed lens sphere power, purchase frequency and lens material to be significant factors. There was a wide variation in retention rates between sites (40-100%)., Conclusions: During the first year of CL wear, the overall retention rate for neophyte CL wearers was 74% (spherical CLs 79%, torics 73%, multifocals 57%), with many lapsing during the first 2 months. Factors associated with retention and dropout in these patients include: lens power, material and type, and purchase frequency. While handling and comfort are the most commonly cited performance-related reasons for discontinuing in new spherical lens wearers, visual problems are the most common among new wearers of toric and, in particular, multifocal CLs., (Copyright © 2016 British Contact Lens Association. Purpose: To characterize and compare the prevalence of soft contact lens-related (SCL) dryness symptoms in large populations of SCL wearers in North America (NAm) and the United Kingdom (UK)., Methods: SCL wearers from NAm (n = 1443) and UK (n = 932) sites completed self-administered questionnaires on SCL symptoms and wearing experiences. A categorization for contact lens-related dry eye (CL-DE) was applied that combined Contact Lens Dry Eye Questionnaire (CLDEQ) items on dryness frequency and intensity at the end of the day (CL-DE+ = constantly/frequently/sometimes plus intensity = 3-5, and CL-DE- = never/rarely plus intensity = 0-1, Marginal = all other ratings). Data were analyzed across regions and categories, with p-values <0.05 significant., Results: Compared to UK wearers, those in NAm wore fewer toric and multifocal designs, and reported significantly longer average and comfortable wearing times (p < 0.001) but similar levels of general and end-of-day comfort. The prevalence of CL-DE+ differed between the UK and NAm: CL-DE+: 31 vs. 39%, Marginal CL-DE: 13 vs. 14%, and CL-DE-: 56 vs. 47%, p = 0.0001. Although daily disposable (DD) lenses were worn more commonly in the UK than NAm (33 vs. 10%, p < 0.0001), their use did not impact the overall prevalence of dryness symptoms. Between regions, NAm wearers reported significantly more frequent and intense end-of-day discomfort and dryness (p < 0.0001) and were significantly more likely to use treatments for CL-related symptoms yet experienced a lower degree of relief., Conclusions: SCL wearers in NAm reported longer hours of wear with significantly more symptoms of dryness and discomfort. NAm wearers used dryness treatments more often, but experienced less relief than UK wearers. In both regions, the CL-DE categorization was useful to predict poorer comfort, shorter comfortable wearing time, and increased use of treatments. These results highlight the need to benchmark SCL performance locally or regionally to understand and focus on the challenges to the lens wearing experience.

Background: The aim was to evaluate the effect of ocular topography on soft contact lens fit in Chinese and Caucasians., Method: This study evaluated 547 subjects from two ethnic groups, Caucasian (n = 250) and Chinese (n = 297), at investigational sites in three locations: Wenzhou, China, Melbourne, Australia and Jacksonville, USA. Subjects underwent measurement of a range of ocular topographic variables using identical equipment and protocols, including: apical corneal radius (CR), corneal shape factor (CSF) in the two principal meridians, horizontal visible iris diameter (HVID), vertical palpebral aperture (PA), upper lid angle (ULA) and inter-canthal angle (ICA). Subjects were fitted with a spherical lens in two base curves (BCs) (1·Day ACUVUE, 8.50, 9.00 mm) and a toric soft lens (Accelerated Stabilization Design)., Results: The steeper base curve (8.50 mm) spherical lenses gave an acceptable overall lens fit with 98 per cent or more of subjects in both groups, while the flatter lens (9.00 mm) was acceptable in a significantly higher proportion of the Chinese compared to Caucasian group (96 versus 82 per cent, p < 0.0001) . The main difference in fit between groups was for centration; there was significantly less decentration with the Chinese group (8.50 BC: 39 versus 72 per cent, p < 0.0001; 9.0 BC: 63 versus. 85 per cent, p = 0.02). The 8.5 mm base curve showed good centration (none or slight decentration) for 97 per cent of Caucasian eyes and 96 per cent of Chinese eyes. With both groups, there were some significant correlations between lens fit and ocular topographic variables, most notable between upper lid angle and toric lens orientation with the Chinese subjects. Regression analysis also showed key predictive values relating to lens fit., Conclusions: Detailed ocular measurements suggest anatomical differences between Chinese and Caucasian populations that should be considered in soft lens design. Purpose: Replacing soft contact lenses (CLs) on a daily basis brings a number of advantages, most notably, reduced exposure to deposits, disinfectants, allergens, and other contaminants. This retrospective study estimated the prevalence of problems in current wearers of reusable soft CLs and tested the effect of refitting "problem" patients with daily disposable (DD) hydrogel lenses., Methods: Prevalence was estimated from 398 current reusable CL wearers for: frequent/constant discomfort or dryness, ≥2 h of uncomfortable wear, ≥grade 2 conjunctival hyperaemia (0-4), or ≥grade 3 corneal staining (0-15). In the second part of the study, 217 reusable CL wearers classified as problem patients were randomly refitted with DD lenses manufactured from one of two materials: etafilcon A (n=96) or nelfilcon A (n=121) and reassessed 1 week later., Results: Thirty-nine percent (154/398) had some qualifying criterion: reduced comfortable wearing time (CWT), 20%; dryness, 20%; irritation, 5%; corneal staining, 8%; and hyperaemia, 7%. After refitting with DDs, the prevalence of reduced CWT was decreased from 65% to 51% (P=0.0039), dryness from 60% to 41% (P < 0.0001) and corneal staining from 28% to 21% (P=0.04). There was no significant change in the prevalence of irritation, or hyperaemia. Some differences were noted between the two lens materials., Conclusions: A high proportion of reusable soft lens wearers encounter clinically relevant signs or symptoms with their current CLs. Our limited understanding of terrestrial ecosystem responses to elevated CO2 is a major constraint on predicting the impacts of climate change. A change in botanical composition has been identified as a key factor in the CO2 response with profound implications for ecosystem services such as plant production and soil carbon storage. In temperate grasslands, there is a strong consensus that elevated CO2 will result in a greater physiological stimulus to growth in legumes and to a lesser extent forbs, compared with C3 grasses, and the presumption this will lead in turn to a greater proportion of these functional groups in the plant community. However, this view is based on data mainly collected in experiments of three or less years in duration and not in experiments where defoliation has been by grazing animals. Grazing is, however, the most common management of grasslands and known in itself to influence botanical composition. In a long-term Free Air Carbon Dioxide Enrichment (FACE) experiment in a temperate grassland managed with grazing animals (sheep), we found the response to elevated CO2 in plant community composition in the first 5 years was consistent with the expectation of increased proportions of legumes and forbs. However, in the longer term, these differences diminished so that the proportions of grasses, legumes and forbs were the same under both ambient and elevated CO2 . Analysis of vegetation before and after each grazing event showed there was a sustained disproportionately greater removal ('apparent selection') of legumes and forbs by the grazing animals. This bias in removal was greater under elevated CO2 than ambient CO2 . This is consistent with sustained faster growth rates of legumes and forbs under elevated CO2 being countered by selective defoliation, and so leading to little difference in community composition., (© 2013 John Wiley & Sons Ltd.)

The 1950s excavations by Charles McBurney in the Haua Fteah, a large karstic cave on the coast of northeast Libya, revealed a deep sequence of human occupation. Most subsequent research on North African prehistory refers to his discoveries and interpretations, but the chronology of its archaeological and geological sequences has been based on very early age determinations. This paper reports on the initial results of a comprehensive multi-method dating program undertaken as part of new work at the site, involving radiocarbon dating of charcoal, land snails and marine shell, cryptotephra investigations, optically stimulated luminescence (OSL) dating of sediments, and electron spin resonance (ESR) dating of tooth enamel. The dating samples were collected from the newly exposed and cleaned faces of the upper 7.5 m of the ∼14.0 m-deep McBurney trench, which contain six of the seven major cultural phases that he identified. Despite problems of sediment transport and reworking, using a Bayesian statistical model the new dating program establishes a robust framework for the five major lithostratigraphic units identified in the stratigraphic succession, and for the major cultural units. The age of two anatomically modern human mandibles found by McBurney in Layer XXXIII near the base of his Levalloiso-Mousterian phase can now be estimated to between 73 and 65 ka (thousands of years ago) at the 95.4% confidence level, within Marine Isotope Stage (MIS) 4. McBurney's Layer XXV, associated with Upper Palaeolithic Dabban blade industries, has a clear stratigraphic relationship with Campanian Ignimbrite tephra. Microlithic Oranian technologies developed following the climax of the Last Glacial Maximum and the more microlithic Capsian in the Younger Dryas. Neolithic pottery and perhaps domestic livestock were used in the cave from the mid Holocene but there is no certain evidence for plant cultivation until the Graeco-Roman period., (Copyright © 2013 Elsevier Ltd. Purpose: To evaluate factors affecting corneoscleral profile (CSP) using anterior segment optical coherence tomography (AS-OCT) in combination with conventional videokeratoscopy., Methods: OCT DATA WERE COLLECTED FROM 204 SUBJECTS OF MEAN AGE 34.9 YEARS (SD: ±15.2 years, range 18-65) using the Zeiss Visante AS-OCT and Medmont M300 corneal topographer. Measurements of corneal diameter (CD), corneal sagittal height (CS), iris diameter (ID), corneoscleral junction angle (CSJ), and scleral radius (SR) were extracted from multiple OCT images. Horizontal visible iris diameter (HVID) and vertical palpebral aperture (PA) were measured using a slit lamp graticule. Subject body height was also measured. Associations were then sought between CSP variables and age, height, ethnicity, sex, and refractive error., Results: Significant correlations were found between age and ocular topography variables of HVID, PA, CSJ, SR, and ID (P < 0.0001), while height correlated with HVID, CD, and ID, and power vector terms with vertical plane keratometry, CD, and CS. Significant differences were noted between ethnicities with respect to CD (P = 0.0046), horizontal and vertical CS (P = 0.0068 and P = 0.0095), and horizontal ID (P = 0.0010). The same variables, with the exception of vertical CS, also varied with sex; horizontal CD (P = 0.0018), horizontal CS (P = 0.0018), and ID (P = 0.0012). Age accounted for the greatest variance in topography variables (36%)., Conclusions: Age is the main factor influencing CSP; this should be taken into consideration in contact lens design, IOL selection, and in the optimization of surgical procedures. Ocular topography also varied with height, sex, ethnicity, and refractive error.

Purpose: To evaluate the ease with which astigmats from three groups of current non-wearers of toric contact lenses can be fitted with current toric soft lenses, and to determine toric lens-wearing success., Methods: This was a 1-month, randomized, bilateral, open-label, daily wear study involving 200 subjects and 16 investigational sites in the UK. Three groups of astigmatic subjects aged from 16 to 60 and with a mean age of 36 years (67 spherical soft contact lens users, SW; 72 contact lens drop-outs, DO; and 61 neophytes, Neo) were fitted with one of two toric soft lens types: a daily disposable toric soft lens (1 Day Acuvue Moist for Astigmatism, Johnson & Johnson Vision Care) or a 2-weekly replacement, re-usable toric soft lens (Acuvue Oasys for Astigmatism, Johnson & Johnson Vision Care). Subjects were evaluated after 1 month of wear and success rates calculated according to pre-set criteria for acceptable fit, orientation stability, visual acuity, quality of vision, and overall comfort., Results: Eighty-eight percent of lenses were fitted at the first attempt. Of 198 subjects dispensed with lenses, 182 (92%) completed the study. When analysed by subject group, the success rates according to the pre-set criteria were 80% (53/66), 74% (52/70), and 70% (39/56) for the SW, DO and Neo groups respectively. Comfort was the most common reason for failure with all three groups; however, visual problems were more common in the DO and Neo groups (13% vs 6%). VA with the study lenses was significantly better in the SW group compared to their habitual lenses. For the DO and Neo groups combined (i.e. spectacle wearers), visual acuity was comparable to their habitual spectacles., Conclusions: A high proportion of astigmats who are not using toric soft contact lenses can be fitted with current lenses. Previous contact lens drop-outs and neophyte wearers achieve visual acuity comparable to spectacles when fitted with toric soft lenses. Spherical contact lens wearing astigmats achieve superior visual acuity when refitted with toric soft lenses. These findings suggest that many astigmats who are not currently using toric soft contact lenses could do so successfully., (Ophthalmic & Physiological Optics © 2013 The College of Optometrists.)

Purpose: To estimate the prevalence of self-reported "sensitive eyes" (SEs) in soft contact lens (CL) wearers, evaluate the clinical characteristics of patients with SEs, and examine the effect of refitting them with silicone hydrogel lenses., Methods: After self-assessment, 2154 CL wearers were separated into SE and non-SE patients. Demographics, biometric data, wearing time, symptoms, and signs were compared between the two populations. Sixty-three SE patients were randomized into senofilcon A (senA) lenses and 65 into a non-senA arm (lotrafilcon B, omafilcon A, and balafilcon A lenses). The performance of senA lenses was compared against habitual and non-senA lenses 2 weeks later., Results: A total of 12.2% of CL wearers reported SEs with their habitual CLs. No significant differences were noticed between SE and non-SE patients in sex, age, or refraction. The prevalence of dryness (43 vs 19%, p < 0.0001), irritation (25 vs 11%, p < 0.0001), redness (20 vs 6%, p < 0.0001), and stinging (6 vs 1%, p < 0.0001) was higher in SE patients. Average wearing time (13.0 vs 14.1 hours, p < 0.0001) was lower in this group. Limbal/bulbar hyperemia and corneal/conjunctival staining were not significantly different between the two populations. Senofilcon A increased the number of patients reporting no dryness (habitual vs senA, 20 vs 44%, p < 0.0003), irritation (22 vs 37%, p = 0.015), redness (52 vs 76%, p =0.009) and stinging (58 vs 77%, p = 0.012) but did not significantly affect clinical signs. Senofilcon A was significantly more efficient than non-senA lenses in improving dryness (scale of 0 to 3: senA vs non-senA, 0.64 vs 1.02, p = 0.0056), irritation (0.72 vs 1.16, p = 0.0015), and stinging (0.18 vs 0.53, p = 0.0049)., Conclusions: A substantial proportion of CL wearers report SEs with their habitual lenses. These patients are characterized by a high prevalence of additional symptoms, which are not reflected in clinical signs. Senofilcon A, or lenses with similar properties, may help reduce these symptoms in SE patients.

Purpose: To evaluate predisposing factors in subjects susceptible to solution-induced corneal staining (SICS)., Methods: Four hundred forty-six existing soft contact lens wearers were randomly fitted with one of two study lenses (lotrafilcon B, CIBA Vision, or omafilcon A, CooperVision) and used a hydrogen peroxide care system (Clear Care, CIBAVision) for a 1-month period. Each subject was then challenged with the same lenses presoaked in a multipurpose solution (ReNu MultiPlus, Bausch & Lomb) and assessed after approximately 2 h to be categorized as either "stainers" or "non-stainers" based on whether they showed a classic SICS pattern (diffuse punctate corneal staining in three or more peripheral quadrants)., Results: Overall, 36% (155 of 425) of eligible subjects were classified as stainers and included a higher proportion of omafilcon wearers: 44% (91 of 205) vs. 29% (64 of 220). After using the peroxide system, the stainers were more likely to show poorer wetting (odds ratio [OR] = 1.76, p = 0.02) compared with the non-stainers. Lens wetting showed a significant negative correlation with film deposits (r = -0.63, p < 0.0001, n = 425). Furthermore, on slit lamp examination, the stainers were more likely to show higher levels of upper- and lower-palpebral hyperemia (OR = 2.18, p = 0.02; OR = 1.93, p = 0.04), tarsal roughness (OR = 1.71, p = 0.01), corneal staining (OR = 17.1, p < 0.0001), and conjunctival staining (OR = 1.94, p = 0.004). Although not exposed to preservatives, twice as many eyes in the stainers group had some level of corneal staining: 51% vs. 25%. There were no significant differences between the stainers and non-stainers with respect to age, sex, refraction, or keratometry (p > 0.05)., Conclusions: This study highlighted some notable trends in patients susceptible to SICS. The findings point toward poor tear quality as a predisposing factor for SICS.

Purpose: To report demographics, wearing patterns, and symptoms from soft contact lens (SCL) wearers with significant SCL-related dryness symptoms with and without significant ocular signs of dryness., Methods: In a multicenter, prospective observational clinical trial, symptomatic SCL wearers reported significant SCL-related dryness via self-administered questionnaire of frequency and intensity of dryness after a dry eye (DE) examination. DE etiology was assigned post hoc by an expert panel, and those with and without significant DE-related signs were analyzed by univariate logistic regression. Possible DE etiologies were aqueous tear deficiency, SCL-induced tear instability, meibomian gland dysfunction, or "other." Wearers without signs that qualified for any DE etiology were designated as No DE Signs (NDES)., Results: Of the 226 SCL symptomatic wearers examined, 23% were without signs, 30% had aqueous tear deficiency, 25% had SCL-induced tear instability, 14% had meibomian gland dysfunction, and 8% had "other" diagnoses. The NDES wearers had significantly longer pre-lens break-up time (9.8 vs. 6.6 s, p < 0.0001), better lens wetting (3.4 vs. 2.4 0 to 4 scale, p < 0.0001), lower levels of film deposits on lenses (0.45 vs. 0.92, 0 to 4 scale, p < 0.0001), and of most slit lamp signs. The NDES wearers were significantly more likely to be male (36% vs.19%, p = 0.013), were less likely to have deteriorating comfort during the day (81% vs. 97%, p = 0.001), reported longer average hours of comfortable wear (11 ± 3 vs. 9 ± 4 h, p = 0.014), had older contact lenses (18 ± 14 vs. 13 ± 12 days, p = 0.029), and greater intensity of photophobia early and late in the day (p = 0.043 and 0.021)., Conclusions: Symptoms of dryness in SCL wearers stem from a variety of underlying causes. However, nearly one-quarter of these symptomatic SCL wearers appear to be free of signs of dryness. The effective management of CL-related dryness requires a comprehensive range of clinical assessments and the use of a diverse range of management strategies.

Purpose: To estimate the prevalence of and factors associated with contact lens-related dryness symptoms in a large population of soft contact lens patients in the UK., Methods: Soft contact lens (CL) wearers (n=932) from 12 UK clinical sites were queried regarding CL history and wearing experience by a self-administered questionnaire. A new score using Contact Lens Dry Eye Questionnaire (CLDEQ) items was used where CL-related dry eye (CL-DE)=frequency of 'sometimes', 'frequently', or 'constantly' plus intensity 3-5; NoCL-DE=frequency of 'never' or 'rarely' and intensity 0-1; Marginal CL-DE, all other categories. Data were analyzed across categories, P-values <0.05 significant., Results: The prevalence of CL-DE was: CL-DE=31%, Marginal CL-DE=13% and NoCL-DE=56%. Wearers with CL-DE had significantly lower typical comfort (CL-DE=7.0 ± 1.9; Marginal=7.6 ± 1.9; NoCL-Dry=8.7 ± 1.8, P<0.001), end-of-day comfort (CL-DE=4.9 ± 2.5; Marginal=6.1 ± 2.0; NoCL-DE=7.8 ± 2.1, P<0.001) and shorter comfortable wearing times in hours (CL-DE=9.1 ± 4.4; Marginal=9.8 ± 3.6; NoCL-DE=12.4 ± 5.1, P<0.001). Positive CL-DE status was significantly more likely among toric lens wearers (n=129) compared to spherical lens wearers (43% vs. 30%, P=0.04). Lens material class, lens care system and gender were not significantly related to CL-DE status. Of the CL-DE group, only 38% had been diagnosed with dry eye and 47% self-assessed as dry eye. Use of artificial tears, CLs for dryness, ointments/gels and warm compresses differed between groups according to CL-DE status (P<0.001, all comparisons)., Conclusions: The survey provided useful data regarding the current status symptoms among UK soft CL wearers. Purpose: To estimate the proportion of potential soft contact lens wearers requiring an astigmatic correction and to estimate the proportion of astigmats who can be accommodated with toric soft lenses of varying prescription range., Method: A database of 11,624 spectacle prescriptions was used to calculate the prevalence of astigmatism for various thresholds (0.50-2.00 DC) by eye and by patient. The coverage of various prescription ranges was estimated using a subset of the database comprising those patients with at least 0.75 D of astigmatism in at least one eye (n = 5,444)., Results: The prevalence of patients showing astigmatism of 0.75 and 1.00 D or greater in at least one eye was 47.4% and 31.8% and, in both eyes, 24.1% and 15.0%, respectively. The proportion of eyes showing astigmatism greater than or equal to 0.75, 1.00, 1.50, and 2.00 D was 35.7%, 23.4%, 10.8%, 5.6%, respectively. The prevalence of astigmatism of 0.75 D or greater was almost double in myopes compared with hyperopes: 31.7% vs. 15.7%. The prevalence of with-the-rule (WTR) astigmatism was higher than against-the-rule (32.9% vs. 29.1%); the proportion was also higher for WTR in eyes with astigmatism ≥0.75 D (15.3% vs. 14.5%). We estimate that approximately one third of potential contact lens wearers require astigmatic correction. A stock range of toric soft lenses in sphere powers +6.00 to -9.00 D, three cylinder powers, and 18 axes requires nearly 3,000 prescriptions and provides coverage for 90% of astigmats., Conclusion: These findings provide an estimate of the proportion of soft contact lens patients requiring an astigmatic correction and some useful insights into the proportion of astigmats covered by toric soft lens stocks of varying range.

Purpose: To determine which ocular topography variables affect soft contact lens fit., Methods: Fifty subjects each wore three pairs of soft lenses in random succession (Vistakon Acuvue 2, Vistakon Acuvue Advance, Ciba Vision Night & Day), and various aspects of lens fit were evaluated. The steeper base curves of each type were worn in one eye and the flatter base curves in the other eye. Corneal topography data were collected using a Medmont E300 corneal topographer (Camberwell, Australia). Corneal curvature, shape factor (SF), and corneal height were measured over a 10 mm chord and also over the maximum measurable diameter. These were measured in the horizontal, vertical, steepest, and flattest meridians., Results: With each lens type, the steeper base curve provided the best fit on the greatest proportion of eyes and the significant differences in various aspects of fit were noted between base curves. For each lens type, there was no significant difference in mean K-reading between those eyes best fit with the steeper base curve and those eyes best fit with the flatter base curve. Two of the lenses showed a positive correlation between centration and horizontal corneal height (maximum), whereas one lens showed a negative correlation between centration and horizontal SF (SF = e). Several lenses showed a positive correlation between post-blink movement and horizontal or vertical corneal SF., Conclusions: The measurement of corneal topography using current Placido disc instrumentation allows a better prediction of soft lens fit than by keratometry, but it is not reliable enough to enable accurate selection of the best fitting base curve. Some correlations are evident between corneal measurements; however, trial fitting remains the method of choice for selection of soft lens base curve.

Clinical studies have revealed that aseptic loosening is the dominant cause of failure in total hip arthroplasty, particularly for the acetabular component. For a cemented polyethylene cup, failure is generally accompanied by the formation of fibrous tissue at the cement-bone interface. A variety of reasons for the formation of this tissue have been suggested, including osteolysis and mechanical overload at the cement-bone interface. In this study, a computational cement damage accumulation method was used to investigate the effect of polyethylene cup penetration, cement mantle thickness, and cement porosity on the number of cycles required to achieve mechanical fatigue failure of the cement mantle. Cup penetration was found to increase cement mantle stresses, resulting in a reduction in cement mantle fatigue life of 9% to 11% for a high cup penetration rate. The effect of using a thin (2 mm) over a thick (4 mm) cement mantle also reduced cement mantle fatigue life between 9% and 11%, and greatly raised cancellous bone stresses. Cement porosity was found to have very little effect on cement mantle fatigue life. Failure modes and cement stresses involved suggest that only extreme combinations of a thin cement mantle and high cup penetration may lead to mechanical failure of the cement mantle, thereby allowing wear debris access to the cement-bone interface. A thin cement mantle may also lead to the mechanical overload of the cement-bone interface. In this manner, the authors suggest that the mechanical factors may contribute to the failure mode of cemented polyethylene cups., (Copyright (c) 2009 Orthopaedic Research Society.)

Purpose: To evaluate and compare the effect of gravity and gaze direction on toric lens orientation and visual acuity (VA)., Method: This was a 14 subject, randomised, unmasked, non-dispensing study, relating to the effect of gaze direction and posture on toric lens orientation and VA. Four lens types were assessed: Acuvue Oasys for Astigmatism (AOfA), Purevision Toric (PVT), Air Optix for Astigmatism (AOT) and Proclear Toric (PCT). In the first part of the study, subjects were positioned on their side and once lenses had settled, VA was measured and photographs taken of the lens orientation position. In the second part, the subjects were positioned at a slit-lamp and video-recordings taken as they changed from the primary gaze position to the eight cardinal directions of gaze., Results: In Part 1, all lenses rotated as a result of change in posture and head position. With subjects in a recumbent position mean rotation ranged from 11.0 degrees with AOfA to 29.1 degrees with PCT. The consequent mean reduction in VA ranged from 0.05logMAR for AOfA to 0.15logMAR for PVT and was significantly worse with PVT and PCT compared with AOfA (P<0.05). In Part 2, lenses tended to show inferior-nasal rotation on upgaze and inferior-nasal rotation on downgaze. The AOfA lenses showed less rotation on inferior-nasal version than each of the other designs (P<0.005). The AOT lenses showed significantly less rotation on superior-temporal version than PVT (P=0.01)., Conclusion: Toric soft contact lens stability in extreme versions and postural positions can affect orientation and VA.

Purpose: To evaluate the comparative orientation characteristics of currently available silicone hydrogel torics., Methods: The re-orientation characteristics of four toric lenses were assessed: three prism-ballasted (PB) designs, Purevision Toric (PVT), Air Optix Toric (AOT), Proclear Toric (PCT), and an "Accelerated Stabilization Design" (ASD) ACUVUE ADVANCE for Astigmatism (AAfA). In the first part of the study, the toric lenses were rotated, approximately 45 degrees in the infero-temporal direction and a video recording of lens reorientation was made from which blink-by-blink measurements of lens orientation were taken. In the second part of the study, subjects were positioned with their head orientated 90 degrees to the vertical and lens orientation photographed once it had settled., Results: In Part 1, the speed of re-orientation from oblique mis location was similar for the four designs tested and ranged from 22°/min (PVT) to25°/min (AOT). Lenses showed more rotation during rather than between the blink (p<0.0001). Between blinks, AAfA and AOT showed significantly more rotation than PCT and PVT (p0.04). There were no significant differences between lenses for rotation during the blink. The PB lenses showed faster re-orientation further away from the zero position.AAfA showed no significant difference in rotation speed with varying orientation position. In Part 2, all the PB lenses rotated significantly more under the effect of gravity than the ASD lenses.The mean total rotation of the AAfA lenses,from their settled position, was 11.4° compared with 25.9° for PVT (p0.01), 18.4° with AOT (p0.02) and 37.4° with PCT (p0.0002) [corrected]. The mean total rotation of the AAfA lenses, from their settled position, was 11.2 degrees compared with 32.8 degrees for PVT (p < 0.001), 16.1 degrees with AOT and 33.1 degrees with PCT (p < 0.001)., Conclusions: The four lens designs show similar re-orientation speeds. In the recumbent position, PB lenses rotate more from their normal settled position than ASD lenses.

Purpose: To report the symptoms and experiences associated with habitual hydrogel (Hyd) and silicone hydrogel (SiHy) contact lenses (CLs) among young adults and measure their association with age., Methods: Questionnaire responses from 699 Hyd and 183 SiHy wearers were analyzed to test the association of age with features indicating struggle with CL wear. Prevalence by age was compared between groups with Pearson's chi-square and within groups with Spearman's correlation coefficient., Results: Hyd and SiHy groups were similar for sex and age distribution. Diagnosis of dry eye increased with age in the Hyd wearers from 10.6% to 21.1% (18-24 vs. 30-35 years), but was approximately 19% in the SiHy wearers across age. Adverse environments caused more discomfort with Hyd than SiHy lenses and Hyd wearers' discomfort increased with age (smoky (p=0.0001), dry air environments (p=0.002), after napping or sleeping (p=0.004). More Hyd wearers considered discontinuation of CL wear with age, from 4.6% to 14.2% (18-24 vs. 30-35 years, p<0.04), but the proportion remained steady at 9% across age for SiHy wearers (p=0.46)., Conclusions: From 18 to 35 years, hydrogel CL wearers reported increasing struggle with CL wear and more negative ratings compared to SiHy wearers. Older hydrogel CL wearers were also more likely to consider discontinuing CL wear compared with younger hydrogel or silicone hydrogel CL wearers in general. Young adult CL wearers should be routinely queried directly about specific aspects of CL wear in order to determine whether different lens types should be prescribed to avoid abandonment of CL wear.

Purpose: To clinically evaluate long-term users of two different contact lens care preservative systems and to investigate whether prolonged use is associated with an increase in the prevalence of dry eye., Methods: Eighty-nine wearers of group IV hydrogel or silicone hydrogel lenses participated in this one-visit, investigator-masked study. Subjects were required to have consistently used a polyhexamethylene biguanide (PHMB) or polyquaternium-1 (PQT) based solution for 2 years. Consistent use was defined as 80% for the past 2 years and 100% for the past year. Clinical assessments included: average and comfortable wear time; overall and end-of-day comfort; signs of dryness, discomfort, burning or stinging, grittiness or scratchiness and visual changes; non-invasive and fluorescein break-up-time; pre-ocular tear film lipids, tear meniscus height, Schirmer and fluorescein clearance tests; limbal and bulbar hyperemia; palpebral roughness; corneal and conjunctival staining; lens front surface wetting; and lens film deposits., Results: Significantly more grittiness or scratchiness was reported by subjects using a PHMB-containing system (67% vs. 44%; P = 0.02). Palpebral roughness and hyperemia were significantly greater in the PHMB group wearing group IV lenses (P = 0.01 and P = 0.05, respectively). Corneal staining was significantly higher in the PHMB users in all four peripheral sectors (P < 0.01). Nasal and temporal conjunctival staining was also significantly higher for users of PHMB-containing systems (P < 0.05). Front surface lens wettability was significantly better for group IV PQT users compared to PHMB users (P = 0.008), with 84% vs. 72%, respectively, with lenses graded by the investigator as having "good" or "excellent" wettability. Significantly higher levels of lens front surface film deposits were noted with PHMB users (P = 0.007), with 58% of group IV lenses treated with PHMB compared with 38% of group IV lenses treated with PQT showing some lens front surface film deposition. No significant differences between the two preservative system groups were noted for the range of dry eye evaluations nor the remaining clinical assessments., Conclusions: Differences in both ocular and lens characteristic were observed between long-term users of two preservative systems used in many contact lens multi-purpose solutions. The findings from this study did not support the hypothesis that prolonged use of PHMB-containing solutions leads to dry eye. Additional studies including a larger sample size and perhaps longer use of the systems could help to further elucidate differences in clinical performance between systems.

Purpose: To evaluate the clinical performance of two daily disposable contact lenses: 1-DAY ACUVUE (1DA) (etafilcon A, Johnson & Johnson Vision Care) and FOCUS DAILIES with AquaComfort (FD) (nelfilcon A, CIBA Vision, Inc.), which contains a quickly released moisture enhancing agent, polyvinyl alcohol (PVA)., Methods: This was a 1-week, daily wear, subject-masked, bilateral, parallel group study with subjects randomly assigned to one of two daily disposable soft contact lenses. Subjects were existing soft contact lens wearers in the age range 18-39 years with a spherical refraction between -0.50 and -6.00 D. Subjects were assessed at baseline and after 1 week. Assessments included both subjective (symptoms, wearing time, vision) and objective (lens fit and ocular health) outcomes., Results: Twenty clinical sites enrolled 282 subjects (74% female) of whom 276 (98%) successfully completed the study. Significantly more 1DA wearers reported higher mean comfort scores than with FD (3.95 versus 3.41, respectively, P<0.0001). End-of-day comfort means were also significantly higher with the 1DA group compared to the FD group (3.3 versus 3.0, P=0.03). The 1DA group reported significantly longer mean overall wearing time (1DA was worn on average 0.66+/-0.30 h longer than FD, P=0.03) and longer mean comfortable wearing time (1DA was worn on average 1.73+/-0.35 h longer than FD, P<0.0001). The 1DA lens generally showed significantly better on-eye fitting in terms of movement in primary (P<0.0001) and upgaze (P<0.0001) and ease of removal from the eye (92% versus 63%, P<0.0001) compared to FD. There was significantly less corneal staining observed with the 1DA wearing eyes compared with the FD wearing eyes (23% versus 45%, respectively, P<0.0001). Finally, there was a weak correlation between corneal staining and comfort for FD (r=0.27, P=0.002, n=136), but not for 1DA (r=-0.11, P=0.18, n=140)., Conclusion: Subjective and objective clinical performance differences between two commonly used daily disposable lenses highlight that, although both lenses may be considered as clinically acceptable, these lenses should not be regarded as interchangeable.

Purpose: The purpose of this analysis was to measure the proportion of soft contact lens wearers who wear lenses in challenging environments and to evaluate their resulting comfort in those conditions. A second objective was to determine whether refitting with silicone hydrogel lenses affects lens comfort among subjects who wear lenses in these environments., Methods: Baseline data were collected from 496 hydrogel soft lens wearers on the frequency of use of contact lenses and the associated comfort in 12 challenging environments. Subjects were then refit into second generation silicone hydrogel lenses: senofilcon A (n = 228), galyfilcon A (n = 132), or lotrafilcon B (n = 136). Comfort responses were compared for subjects who "always" or "frequently" used lenses in the queried environments after 2 weeks of silicone hydrogel daily lens wear., Results: More than 80% of subjects used habitual lenses "always" or "frequently" while reading, sitting in an air-conditioned or heated car, using a computer and while driving at night. Less than 40% used lenses "always" or "frequently" while riding in an airplane, at high altitudes, or while napping or sleeping. The least comfortable environments were in dusty, polluted or smoky environments (30% comfortable), and the most comfortable were while reading and using a computer. After 2 weeks use of senofilcon A and galyfilcon A lenses, comfort in all environments improved significantly (Wilcoxon signed rank) except for using a computer with galyfilcon A. After 2 weeks with lotrafilcon B lenses, there was improved comfort while using lenses in all environments except driving at night, using a computer and sitting in an air-conditioned or heated car (p < 0.04, Wilcoxon signed rank)., Conclusions: Daily wear contact lens wearers limit use of lenses during air travel or while napping. Use of daily wear silicone hydrogel lenses improved comfort significantly in most environments, allowing subjects to wear them in challenging environments.

Recent research in Europe, Africa, and Southeast Asia suggests that we can no longer assume a direct and exclusive link between anatomically modern humans and behavioral modernity (the 'human revolution'), and assume that the presence of either one implies the presence of the other: discussions of the emergence of cultural complexity have to proceed with greater scrutiny of the evidence on a site-by-site basis to establish secure associations between the archaeology present there and the hominins who created it. This paper presents one such case study: Niah Cave in Sarawak on the island of Borneo, famous for the discovery in 1958 in the West Mouth of the Great Cave of a modern human skull, the 'Deep Skull,' controversially associated with radiocarbon dates of ca. 40,000 years before the present. A new chronostratigraphy has been developed through a re-investigation of the lithostratigraphy left by the earlier excavations, AMS-dating using three different comparative pre-treatments including ABOX of charcoal, and U-series using the Diffusion-Absorption model applied to fragments of bones from the Deep Skull itself. Stratigraphic reasons for earlier uncertainties about the antiquity of the skull are examined, and it is shown not to be an 'intrusive' artifact. It was probably excavated from fluvial-pond-desiccation deposits that accumulated episodically in a shallow basin immediately behind the cave entrance lip, in a climate that ranged from times of comparative aridity with complete desiccation, to episodes of greater surface wetness, changes attributed to regional climatic fluctuations. Vegetation outside the cave varied significantly over time, including wet lowland forest, montane forest, savannah, and grassland. The new dates and the lithostratigraphy relate the Deep Skull to evidence of episodes of human activity that range in date from ca. 46,000 to ca. 34,000 years ago. Initial investigations of sediment scorching, pollen, palynomorphs, phytoliths, plant macrofossils, and starch grains recovered from existing exposures, and of vertebrates from the current and the earlier excavations, suggest that human foraging during these times was marked by habitat-tailored hunting technologies, the collection and processing of toxic plants for consumption, and, perhaps, the use of fire at some forest-edges. The Niah evidence demonstrates the sophisticated nature of the subsistence behavior developed by modern humans to exploit the tropical environments that they encountered in Southeast Asia, including rainforest.

This reconnaissance study of radon concentrations in the Great Cave of Niah in Sarawak shows that in relatively deep pits and trenches in surficial deposits largely covered by protective shelters with poor ventilation, excavators are working in a micro-environment in which radon concentrations at the ground surface can exceed those of the surrounding area by a factor of > x 2. Although radon concentrations in this famous cave are low by world standards (alpha track-etch results ranging from 100 to 3075 Bq m(-3)), they still may pose a health risk to both excavators (personal dosemeter readings varied from 0.368 to 0.857 mSv for 60 days of work) and cave occupants (1 yr exposure at 15 h per day with an average radon level of 608 Bq m(-3) giving a dose of 26.42 mSv). The data here presented also demonstrate that there is considerable local variation in radon levels in such environments as these.

Qatar has serious water resource problems, following rapid socioeconomic development and massive population increase. Municipal water provision depends on costly and unsustainable desalination. There is little regulation. Native Qataris do not pay a water tariff and migrants pay a subsidized price--approximately one third of the cost of production--so there is little awareness of the true cost of water and use is profligate. This paper discusses trends in water use and identifies issues underlying sustainable water use in Qatar. A questionnaire of respondents chosen to represent Qatari social groups measured awareness and attitudes to water. The results show that previous efforts to control water demand in Qatar, using awareness campaigns, legal restrictions and tariffs, have been ineffectual. The questionnaire evaluated reactions to possible measures to limit uses by raising awareness, using legal restrictions and raising tariffs. From this, a number of policy changes can be suggested, to bring Qatar's water industry towards sustainability.

An objective, digital-imaging method of measuring corneal staining was evaluated in 24 subjects wearing soft contact lenses. The method was used to compare the clinical performance of common multipurpose care systems (MPS) for soft contact lens care. Subjects used three different MPS, one containing polyquaternium-1 (PQ) and two containing polyhexanide (PX1 and PX2), for 2 weeks in a randomised, single-masked (investigator) crossover study. Corneal staining induced with the three MPS was analysed using an image-processing program (ImageTool, UTHSCSA Version 2, University of Texas, USA). Conjunctival hyperaemia and papillae were also evaluated. The intraclass correlation coefficient was similar with image analysis to that of investigator grading (0.876, 0.879, respectively). Significant differences in staining response were detected using the objective method. There was significantly less staining area with polyquaternium-1 (PQ) than polyhexanide (PQ: 0.12 mm(2), PX2: 0.91 mm(2)). Inferior palpebral papillae were significantly greater with PX2 than with PQ (1.0, 0.7 (0-4), respectively). The technique was shown to be an effective method of evaluating different corneal staining responses. Bilateral corneal staining in three or more quadrants is useful in the diagnosis of MPS-related staining.

Patients with acute leukaemia or undergoing allogenic bone marrow transplantation at University College London Hospital Trust were screened for the presence of aspergillosis by polymerase chain reaction (PCR). Aspergillus DNA, from whole blood samples, was amplified by nested PCR to detect a 135 bp fragment in the mitochondrial region of Aspergillus fumigatus or A. flavus (121 bp). One colony-forming unit (CFU) per 2 ml of blood or 1-10 fg DNA could be detected. Patients at risk of aspergillosis were classified as probable or possible based on the European Organization for Research and Treatment of Cancer definitions. Antifungal drugs given were recorded. In four of 17 patients studied, infection was not suspected and the PCR was negative. Four patients were considered to have possible aspergillosis infection and were PCR positive on at least one occasion. Of the three patients in the probable group, four of the nine samples tested PCR positive from one patient and in another patient only one of nine samples tested positive. The remaining six patients were not suspected of having fungal infection but each had one or two PCR-positive results. In summary, six of seven patients thought to have clinical evidence of infection were PCR positive on at least one occasion and treatment with antifungals may have reduced infection below detectable levels.

Purpose: Some knowledge of the orientation position of toric soft lenses is required as part of the fitting process. Although it is known that factors such as lid anatomy, lens design, and fit are important, there is little detailed understanding of how these relate to toric lens orientation. The purpose of this study was to evaluate various patient and lens factors that might influence toric soft lens fit and to determine the predictor variables for lens orientation., Methods: This was a clinical study in which 45 subjects were fitted with a prism-stabilized, mid-water, cast-molded, back surface toric soft lens. The lens was representative of commonly used designs for frequent replacement toric soft lenses. Patient and lens fit variables were assessed, including a range of lid topography features, which were measured using a digital photographic technique., Results: The main patient factors associated with lens orientation and stability were degree of myopia, palpebral aperture, and several angles relating to lid anatomy. Greater upward incline of the temporal lids was associated with increasing inferior-temporal lens orientation; lower myopia and smaller palpebral aperture size were associated with more stable lens orientation. The principal lens fit characteristics affecting lens orientation were postblink movement and lens tightness. Less movement was associated with more stable lens orientation, and increased tightness was associated with slower reorientation speed. Weaker but significant correlations were also shown between a number of other patient and lens variables., Conclusions: A number of patient factors and lens fit characteristics influencing toric soft lens orientation were identified. The findings fall short of allowing practitioners to accurately predict toric lens orientation; however, they do provide some factors to consider when fitting toric soft lenses.
