Background and Objectives: Evidence and specific interventions after carpal tunnel release are limited. The main purpose of this study was to elucidate the current practice patterns of professionals from around the world after carpal tunnel release., Material and Methods: A 15-item English survey was designed and distributed via email and through social networks to professionals from different countries. Introduction: Pronator syndrome has classically required open surgery that leaves a large scar; initial endoscopic techniques required approaches of an average 4cm without achieving release of all structures. The purpose of this study was to describe a new endoscopic approach that allows the median nerve to be safely and completely decompressed, leaving a smaller and less visible scar., Methods: Description of a new approach for decompression of the median nerve in the proximal third of the forearm with minimal incision and endoscopic technique in cryopreserved cadaveric specimens, describing incision, endoscopic anatomy, safe corridors and decompression sites., Results: In 20 elbows of cadavers, an endoscopic approach of the median nerve in the proximal forearm with a 4mm endoscope and 0° of angulation was performed. The advantages and limitations of the technique and surgical details are presented for release in the most common compression points of the nerve in the forearm. We performed this technique in 3 patients with good results without complications., Discussion: Release of the median nerve and section of potential aponeurotic compression structures by endoscopy is possible. The ulnar head of the pronator and the aponeurotic arch of the flexor digitorum superficialis are frequently implicated in the syndrome. The scar is aesthetically good. It is a relatively new technique, with lower morbidity that allows faster recovery of patients., Conclusions: It is possible to perform a complete decompression of the median nerve in the forearm using an endoscopic approach, safely with lower comorbidity for the patient., (Copyright © 2019 SECOT. Introduction: To recognise the relationship between the needle tip and the median nerve during peripheral nerve block is of interest to avoid neural damage. However, signs of intraneural injection are not clearly established. The aim of this study was to define the changes observed in the peripheral nerve after the intraneural or perineural administration of 1ml of solution., Material and Methods: Ultrasound guided median nerve blocks were performed in the forearm of 10 fresh cadavers on 60 occasions (3 per forearm). They were randomised into the intraneural (n=30) or perineural (n=30) location of the needle tip, after the consensus of location by 7 specialists. After 1ml of solution was injected an evaluation was made of the changes in the cross-sectional area of the nerve, as well as the displacement along the nerve., Results: The cross-sectional area of the median nerve was increased in both groups, however, the increase was significantly higher in the intraneural group (perineural 0.007±0.013cm2 vs. intraneural 0.032±0.021cm2, P<.0001). An increase of more than 27% of the area ensures an intraneural injection in the median nerve according to the ROC curve analysis. Both proximal and distal diffusion were observed more frequently in the intraneural group (proximal: 86% vs 14%, P<.0001, Distal: 43% vs 4%, P<.0001)., Conclusions: Based on this experimental study, it is concluded that the injection of a small volume (1ml) allows to discriminate the disposition of the intraneural vs perineural needle in a high percentage of cases. Objectives: To describe the distribution of the terminal branches of the brachial plexus at the axillary level and define distribution patterns after ultrasound evaluation., Material and Method: Fifty volunteers underwent ultrasound bilateral axillary brachial plexus scanning exploration. Nerve distribution around the humeral artery was described and the distance between each nerve and the center of the artery was measured. The distance and relationship between the ulnar nerve and the humeral vein were also recorded., Results: The median nerve was located in the anterolateral quadrant (-29±40°) and at a mean distance of 2.1±0.9mm from the artery (85%). The ulnar nerve was found at 53±26° and at 4.2±2.1mm from the artery in the anteromedial quadrant (90%), anterolateral to the vein in 46% of cases, and deep to it in 54%. The radial nerve was at 122±38° and at 3.3±1.7mm from the artery in the posteromedial quadrant (86%). The musculocutaneous nerve was found at -103±22° and 9.3±5.6mm from the artery in the posterolateral quadrant (90%) and in the anterolateral quadrant (-55±16°) at 4.8±2.7mm (10%). There were no differences regarding laterality, gender or overweight patients., Conclusions: Our results allow defining four different anatomical patterns, two based in the position of the musculocutaneous nerve and two based on the disposition of the ulnar nerve with respect to the humeral vein. Introduction: The lipofibrohamartoma is a rare entity of unknown origin that can affect any peripheral nerves, but mainly being found in the median nerve within the carpal tunnel. The lipofibrohamartoma is frequently associated with other conditions such as macrodactyly, the Proteus and Klippel-Trenaunay-Weber syndromes and multiple exostosis, among others., Clinical Cases: Two cases of lipofibrohamartoma in the carpal tunnel with associated median nerve palsy are described in the present article. They were treated by simple decompression of the median nerve by releasing the transverse carpal ligament and a palmaris longus tendon transfer to improve the thumb abduction (Camitz procedure). In one of the cases (a previously multi-operated median nerve entrapment at the carpal tunnel), a posterior interosseous skin flap was employed to improve the quality of the soft tissues on the anterior side of the wrist., Discussion: A review of the literature is also presented on lipofibrohamartoma of the median nerve, covering articles from 1964 to 2010. The literature suggests that the most recommended treatment to manage this condition is simple release of the carpal tunnel, which should be associated with a tendon transfer when a median nerve palsy is noticed., (Copyright © 2013 SECOT.
