Background: The pathogenesis of articular- and bursal-sided partial-thickness rotator cuff tears (PTRCTs) is considered to be different, and associated lesions with PTRCTs need to be examined., Methods: The current study consisted of 76 shoulders of 73 patients (27 men, 46 women, 64.0 ± 8.3 years old) who underwent mini-open repair after conversion to full-thickness tears for either articular side (group A; n = 56) or bursal side (group B; n = 20) PTRCT with at least a 2-year follow-up. Clinical outcomes and their images were compared between the groups. Plain radiographs were used with the arm passively elevated in maximum elevation to assess restriction of glenohumeral motion., Results: The retear rate was not significantly different between the groups. Both groups showed significant improvement in functional scores at the final follow-up. Patients in group A showed a higher incidence of preoperative fibrosis in the rotator interval (69.6% vs. 35.0%, respectively; P  = .006) and a lower incidence of an acromial spur (7.1% vs. 35.0%, respectively; P  = .008) compared with group B. Plain radiographs with arm elevation showed restriction of glenohumeral movement in 49 of 76 shoulders (64.5%) preoperatively, including 36 of the 46 shoulders with fibrosis and 13 of the other 30 shoulders., Conclusion: Both articular- and bursal-sided PTRCTs showed significant functional improvements after surgery. The articular-sided tears had a lower incidence of an acromial spur, but had a higher incidence of fibrosis in the rotator interval, which led to a limitation in glenohumeral motion., (© 2020 The Author(s).)

We proposed a simple technique for reduction of cardiac-related motion artifacts on contrast-enhanced images in the breast by using cylindrical regional-suppression technique (CREST) that can directly suppress the heart signals. The purpose of this study was to select the optimal scan parameters and to evaluate the feasibility in the breast. We demonstrated that the optimized CREST could dramatically reduce the cardiac-related flow artifacts without any penalty to the acquisition time, signal-to-noise ratio and contrast-enhanced lesion-to-parenchyma contrast.

Purpose: To demonstrate the usefulness of hyperecho and PROPELLER (HEP) for carotid arterial vessel wall imaging by using a quantitative comparison with conventional methods. PROPELLER is a motion-insensitive turbo spin-echo (TSE) sequence and has recently been utilized in magnetic resonance (MR) plaque imaging instead of double inversion recovery TSE (DIR-TSE). Wider blade-width, higher k-space density, and an improved blood suppression effect result in better image quality. In this study we introduce a new combination of HEP., Materials and Methods: A total of 17 subjects were examined on a 3.0T system. We conducted quantitative comparisons for signal-to-noise ratio (SNR), contrast-to-noise-ratio, and image sharpness among HEP, DIR-TSE, and conventional PROPELLER (c-PROPELLER). Subsequently, images obtained with DIR-TSE, c-PROPELLER, and HEP were visually evaluated using a three-point scale by two board-certified radiologists., Results: HEP showed high SNR similar to c-PROPELLER, good T 2 contrast approximating DIR-TSE, and better blood suppression compared with the other two methods (P < 0.05). The image sharpness of HEP (2.55 ± 0.53) was higher than that of DIR-TSE (1.89 ± 0.33) and the absence of ghost or streak artifacts in HEP (2.89 ± 0.33) was better than that in both other methods (2.22 ± 0.83 for DIR-TSE and 2.00 ± 0.50 for c-PROPELLER) (P < 0.05). Furthermore, the degree of blood suppression, particularly in cases of slow or turbulent flow close to the atherosclerotic plaque, was identical for HEP (2.80 ± 0.45) and DIR-TSE (2.80 ± 0.45) but was significantly better than for c-PROPELLER (1.60 ± 0.55) (P < 0.05)., Conclusion: This study demonstrates the usefulness of HEP in the carotid arteries. Unlabelled: The denture bases made by using polymethyl methacrylates of Acrylic resin family have excellent physical properties, simple to process and easy to reline, rebase and repair. One of the inherent disadvantages of this material is the liability to break during function. The strength assessment of acrylic resins have been made generally by transverse defection tests., Aim: To evaluate the impact strength valves of certain brands of commercially available denture base resins and suggest their suitability., Materials and Methods: The denture bases we made using polymethyl methacrylates of acrylic resin family because they have excellent physical properties, simple to process and easy to reline and rebase. Six commercial brands of polymethyl meth- acrylate, namely Stellon (DPI-India), Acralyn-H (Asian Acrylate, India), Trevalon (Dentsply-England), Lucitone 199 (Dentsply/ York division), Acralyn-H (Super Unbreakable), Trevalon HI (Dentsply, Detray division, England) were tested by breaking them using Analog Pendulum (ASTM D 256)., Results: From the entire study the maximum impact strength was reported for Acralyn-H super unbreakable (Asian Acrylates, India) 62.19 joules., Conclusion: All the analysis led to conclusion that there is basic change in material composition within and among the different groups of denture base resins., Clinical Implications: The complete dentures made using denture base resins with high impact strength valves (e.g. Acralyn-H super unbreakable) will be more durable and can be used by the patient for considerable period of time, i.e. beyond 4 to 5 years.

Background: Rugby is a collision sport with a high risk of shoulder injury. Although traumatic anterior shoulder instability is common, the long-term effects of rugby and joint instability on the shoulder have not been described; thus, this study assessed the effects of rugby itself, and joint instability, on the glenoid cavity., Materials and Methods: Both sides of the shoulders from 25 rugby players and 17 control patients with unilateral shoulder instability were prospectively evaluated by means of computed tomography osteoabsorptiometry, which represents the distribution of mineralization in subchondral bone plate (DMSB) as a marker of the long-term loading history of a joint. For the quantitative analysis, intergroup differences of maximum Hounsfield unit (HU) values in 7 areas on the glenoid were assessed in the uninjured intact shoulder to characterize the influence of rugby. Side-to-side differences of the HUs in each area were assessed in each participant to characterize the effects of shoulder instability. For the qualitative analysis, associations between the patterns of each DMSB and each group were assessed by means of correspondence analysis., Results: All examined areas on the glenoid had a significantly higher HUs in rugby players. Shoulder instability affected the HUs in both groups. A qualitative analysis demonstrated that the maximum HU tended to be shifted more inferiorly in rugby players and in the unstable shoulders., Conclusions: Rugby affects the shoulder joint, regardless of any history of instability, suggesting that "rugby shoulder" tends to involve degenerative changes, such as osteoarthritis or labral tears., (Copyright © 2013 Journal of Shoulder and Elbow Surgery Board of Trustees. Background: We analyzed the mechanical environment of the supraspinatus tendon using a three-dimensional finite element model with the software programs MENTAT and MARC., Methods: The supraspinatus tendon that attaches to the superior facet was extracted and modeled. The geometric shape of the humeral head was determined from computed tomography images, and the shape of the supraspinatus tendon was determined from magnetic resonance images of the shoulder at 0 degrees of abduction in a healthy 27-year-old man. The distal portion of the humeral head was fixed, and 10 N of tensile force was applied to the proximal end of the tendon. The tensile stress was calculated., Results: The tensile stress was 1.8 MPa for the bursal side and 15.0 MPa for the articular side of the anterior portion of the supraspinatus tendon. The intensity was 0 MPa for the bursal side and 4.5 MPa for the articular side of the middle portion of the tendon. The intensity was 0.1 MPa for the bursal side and 5.2 MPa for the posterior edge of the tendon., Conclusions: Based on the three-dimensional finite element method, the maximal tensile stress was observed on the articular side of the anterior edge of the supraspinatus tendon. Our result may explain the frequent occurrence of rotator cuff tears at this site.

Objective: To evaluate magnetic resonance (MR) lymphography with submucosal injection of superparamagnetic iron oxide (SPIO) for imaging lymphatic pathways from thoracic esophageal cancer., Methods: In 24 patients with esophageal cancer, SPIO was injected into the submucosal layer of the peritumoral region endoscopically and MR lymphography was conducted. In study 1, fast spoiled gradient-recalled acquisition using a steady-state (FSPGR) sequence was performed from the neck to the upper abdomen before and at 20, 40, and 60 minutes after injection in 10 patients. In study 2, FSPGR and spin echo T1-weighted images were obtained after injection in 14 patients. Areas scanned were the neck to the upper mediastinum and the upper abdomen., Results: In study 1, at 20 minutes after injection, the signal of each lymph node appeared attenuated when compared with precontrast images. The signal-to-noise ratio in lymph nodes exhibiting influx of SPIO was significantly lower than that found on precontrast images (P < 0.0005). In study 2, influx to the neck lymph nodes was detected in 8 patients (64.3%), whereas influx to the upper abdominal lymph nodes was detected in 13 (92.9%)., Conclusions: Magnetic resonance lymphography with SPIO could visualize the lymphatic pathways draining from the injection site and the location of lymph nodes exhibiting influx of SPIO in patients with thoracic esophageal cancer.

Purpose: To compare indirect magnetic resonance (MR) arthrography findings in patients with rotator cuff tears with and without symptoms., Materials and Methods: Indirect MR arthrography results of 15 asymptomatic and 23 symptomatic rotator cuff tears were retrospectively reviewed, comparing the type and size of tears and amount of subacrominal-subdeltoid bursal fluid., Results: There were 14 partial-thickness tears and one full-thickness tear in the asymptomatic group, with 4 small and 11 medium tears. In the symptomatic group, there were 16 partial-thickness and 7 full-thickness tears, with 14 small, 5 medium, and 4 large tears. In the asymptomatic group, the amount of subacrominal-subdeltoid bursal fluid was grade 0, 6 patients; grade 1, 6 patients; grade 2, 2 patients; and grade 3, 1 patient. In the symptomatic group, the amount of subacrominal-subdeltoid bursal fluid was grade 0, 4 patients; grade 1, 3 patients; grade 2, 8 patients; and grade 3, 8 patients. We found no statistically significant difference between symptomatic and asymptomatic tears in the kind and size of tears, and in the amount of subacrominal-subdeltoid bursal fluid., Conclusion: We found no difference in the MR findings between symptomatic and asymptomatic rotator cuff tears.

Purpose: The purpose of this study was to evaluate the use of radionuclide angiography and single photon emission computed tomography (SPECT) using Tc-99m hexakis-2-methoxyisobutylisonitrile (Tc-99m MIBI) for analyzing musculoskeletal lesions., Materials and Methods: Radionuclide angiography and SPECT using Tc-99m MIBI were performed in 36 patients with various soft tissue and bone pathologies (18 benign and 18 malignant lesions). The ratio of the counts of the lesion to that of the contralateral normal area was calculated from the region of interest drawn on the MIBI scan. The MIBI-uptake ratio was calculated using SPECT and the MIBI-perfusion index was calculated using radionuclide angiography., Results: The MIBI-uptake ratio in malignant lesions (4.80 +/- 4.43) was significantly higher (P < 0.05) than that in benign lesions (1.83+/-2.48). The sensitivity, specificity, accuracy, positive predictive value, and negative predictive value of the MIBI-uptake ratio for malignant lesions were 72%, 83%, 78%, 81%, and 75%, respectively. The MIBI-perfusion index in malignant lesions (17.68 +/- 21.19) was significantly higher (P < 0.05) than that in benign lesions (2.25 +/- 2.56). The sensitivity, specificity, accuracy, positive predictive value, and negative predictive value of the MIBI-perfusion index for malignant lesions were 87%, 75%, 81%, 77%, and 86%, respectively., Conclusions: The MIBI-uptake ratio and MIBI-perfusion index using Tc-99m MIBI can yield important additional information with which to differentiate musculoskeletal lesions.

Objective: The purpose of this study was to detect lymphatic drainage into the superior mediastinum and neck in thoracic esophageal cancer patients using ferumoxides-enhanced magnetic resonance imaging (MRI), and to have this information assist in determining the appropriate extent of lymphadenectomy., Methods: Nine male patients with T2-T3 mid- and lower-thoracic esophageal cancer with lymph node metastasis were examined. The day before surgery, ferumoxides was endoscopically injected into the submucosal layer of the peritumoral lesion. Thereafter, lymph nodes in the superior mediastinum and neck, which were shown to be ferumoxides-enhanced on MRI, were harvested and evaluated; magnetic force from all harvested lymph nodes was measured ex vivo., Results: MRI of the superior mediastinum and neck revealed 1(median) ferumoxides-enhanced lymph nodes in eight (89%) patients, and there was laterality in the lymphatic mapping in both areas. Of the 15 lymph nodes into which drainage was detected by enhanced MRI, 12 (80%) were magnetite-positive. In six patients (67%), magnetic resonance enhanced lymph nodes corresponded completely with the ex vivo magnetite examination, and in 3 patients (33%) there was partial agreement. In 3 (60%) of the 5 patients that showed paratracheal and/or supraclavicular lymph node metastases, all of the affected nodes were detected by MRI; in one patient some of the affected nodes were detected., Conclusion: Ferumoxides-enhanced MRI is useful for visualizing lymphatic drainage to the superior mediastinum and neck in thoracic esophageal cancer. It is an adequate procedure to form an estimate on the appropriate extent of lymphadenectomy.

We performed 2-dimensional finite element model analysis to estimate the mechanical environment of the supraspinatus tendon. The geometric shape of the finite element model was determined by magnetic resonance imaging of a normal human shoulder obtained at 0 degrees, 30 degrees, and 60 degrees of abduction, whereas the histologic location of noncalcified and calcified fibrocartilage was determined from a cadaveric specimen. The supraspinatus tendon was pulled proximally with the force of 10 N at 0 degrees, 53 N at 30 degrees, and 115 N at 60 degrees of abduction. The area of high principal stress maximum was observed on the articular side of the supraspinatus tendon, which shifted toward the insertion as the arm was abducted. High stress concentration on the articular side of the supraspinatus tendon near its insertion during arm elevation may explain the frequent occurrence of rotator cuff tears at this site.

Background: Elongation of the shoulder capsule is often noticed on arthrograms or during surgery in shoulders of patients who have experienced recurrent anterior dislocations., Hypothesis: We can quantify the elongation of the capsule in shoulders with recurrent anterior dislocations by using magnetic resonance arthrography., Study Design: Retrospective review of prospectively collected data., Methods: Twelve patients with unilateral recurrent anterior shoulder dislocations were enrolled in this study. Magnetic resonance images in the axial and coronal oblique planes were obtained from both shoulders (involved and uninvolved sides) after 10 ml of gadolinium/saline solution was injected into the glenohumeral joint. The length of the anteroinferior, inferior, and posteroinferior portions of the capsule was measured by using image analyzing software and normalized to the humeral head diameter., Results: The anteroinferior capsule was significantly elongated in the involved shoulder at 4 mm (16% elongation) and 10 mm (19% elongation) superior to the inferior margin of the glenoid. The inferior capsule was also significantly elongated in the involved side both at the center (12% elongation) and at 4 mm anterior to the center of the glenoid (29% elongation). The posteroinferior capsule did not show any significant elongation., Conclusions: The anteroinferior and inferior portions of the shoulder capsule are elongated an average of 19% in shoulders with recurrent anterior dislocation., (Copyright 2003 American Orthopaedic Society for Sports Medicine)

The purpose of this study was to evaluate enhancement (signal loss) of the axillary lymph nodes on MR lymphography after intramammary injection of superparamagnetic iron oxide (Ferumoxides) for detection of the sentinel lymph node. MR lymphography was performed in a total of 11 patients with breast cancer without palpable axillary lymph node swelling before operation. Coronal and axial images were obtained before and after intramammary injection of 1.5 ml Ferumoxides adjacent to the breast tumor. In all patients, decreased intensity was recognized in the axillary lymph nodes. MR lymphography could detect the sentinel lymph node with its decreased signal intensity 20 minutes after the intramammary injection of Ferumoxides.

We studied visualization of the subcoracoid bursa by conducting a retrospective analysis of the MR arthrograms of 101 shoulders with surgical confirmation. MR arthrography showed the subcoracoid bursa in 28 shoulders, among which the bursa was revealed by inadvertent direct injection of contrast material into the bursa in 10 shoulders. Of the remaining 18 shoulders, 16 shoulders had rotator cuff tears, one had shoulder instability, and one had dislocation of the long head of the biceps tendon. Both the subcoracoid and subscapularis bursae are located in the subcoracoid space. The subcoracoid bursa does not communicate with the subscapularis bursa, but occasionally communicates with the subacromial-subdeltoid bursa. On MR arthrography, contrast material in the subcoracoid bursa indicates a pathologic condition such as rotator cuff tear. Careful examination of the subcoracoid bursa on MR arthrography helps to achieve better assessment of the rotator cuff.

Purpose: To compare a multishot echo-planar fluid-attenuated inversion-recovery (EPI-FLAIR) sequence with fast spin-echo FLAIR (F-FLAIR) and fast spin-echo T2-weighted (FSE-T2W) sequences in depiction of white matter lesions., Methods: Thirty-five patients with various white matter lesions were included in this prospective study. Two independent readers for lesion detection (lesion size, >2 mm) compared sequences quantitatively. In 22 patients, contrast was calculated between periventricular hyperintensity (PVH) and the cerebrospinal fluid (CSF)., Results: EPI-FLAIR revealed more lesions than FSE-T2W (p < 0.01). However, F-FLAIR revealed more lesions than EPI-FLAIR (p < 0.01). For PVH-to-CSF contrast, EPI-FLAIR demonstrated significantly higher contrast than FSE-T2W. There were no differences in PVH-to-CSF contrast between EPI-FLAIR and F-FLAIR., Conclusions: This study shows that EPI-FLAIR has distinct advantages over FSE-T2W in the depiction of white matter lesions. Although EPI-FLAIR reduces imaging time by more than 60% relative to F-FLAIR, it cannot replace F-FLAIR for the detection of lesions in the cerebral white matter.

Multi-shot echo-planar fluid-attenuated inversion-recovery (EPI-Flair) was compared with spin-echo T1-weighted (SE-T1W), fast SE T2-weighted (FSE-T2W), and fast Flair (F-Flair) in imaging brain tumors. In 32 patients with various different brain tumors, three reviewers independently evaluated image quality. Two reviewers evaluated the image quality of precontrast EPI-Flair to be significantly better than that of precontrast SE-T1W. Two reviewers evaluated the image quality of postcontrast EPI-Flair as superior to that of postcontrast SE-T1W. Artifacts on postcontrast EPI-Flair were significantly more prominent than those on postcontrast F-Flair. Multi-shot EPI-Flair appeared to be superior to SE-T1W, and almost equivalent to FSE-T2W in terms of image quality.

A 4-month-old Japanese girl with a developmental dislocation of the right hip was treated with manual reduction under general anesthesia followed by cast immobilization. Serial magnetic resonance imaging (MRI) revealed that a large interposition of fat tissue in the acetabulum, observed immediately after cast immobilization, became remarkably small in size as the congruity improved after 3 weeks. These MRI findings demonstrate the mechanism of a "squeezing phenomenon" after reduction of the hip.

Background: Glenohumeral dislocations often recur, probably because a Bankart lesion does not heal sufficiently during the period of immobilization. Using magnetic resonance imaging, we assessed the position of the Bankart lesion, with the arm in internal and external rotation, in shoulders that had had a dislocation., Methods: Coaptation of a Bankart lesion was examined with use of magnetic resonance imaging, with the arm held at the side of the trunk and positioned first in internal rotation (mean, 29 degrees) and then in external rotation (mean, 35 degrees), in nineteen shoulders. Six shoulders (six patients) had had an initial anterior dislocation, and thirteen shoulders (twelve patients) had had recurrent anterior dislocation. Fast-spin-echo T2-weighted axial images were made when the dislocation had occurred less than two weeks earlier, and spin-echo T1-weighted axial images after intra-articular injection of gadolinium-diethylenetriamine pentaacetic acid were made when the dislocation had occurred more than two weeks earlier. Separation and displacement of the anteroinferior portion of the labrum from the glenoid rim were measured on the axial images, and coaptation of the anterior part of the capsule to the glenoid neck was assessed by measurement of the detached area, opening angle, and detached length., Results: Separation and displacement of the labrum were both significantly less (p = 0.0047 and p = 0.0017, respectively) when the arm was in external rotation than when it was in internal rotation. The detached area and the opening angle of the anteroinferior portion of the capsule were both significantly smaller (p = 0.0003 and p < 0.0001, respectively), and the detached length was significantly shorter (p < 0.0001) with the arm in external rotation., Conclusion: Immobilization of the arm in external rotation better approximates the Bankart lesion to the glenoid neck than does the conventional position of internal rotation.

CT and MR findings were retrospectively reviewed in 12 patients with germ cell tumors originating from the non-pineal regions. Cystic or necrotic components were seen in 6 patients. Of 4 germinomas, 2 showed mixed density on the CT. The MR signal intensity of the tumor was non-specific. Of 8 germinomas, 4 were inhomogeneously enhanced on postcontrast CT and T1-weighted MR images. CT and MR features of germinomas originating from non-pineal regions frequently differ from those of germinomas originating from the pineal region. The mass of the tumor often appeared cystic and inhomogeneously enhanced following contrast infusion.

Bursitis or large bursa formation associated with osteochondroma has rarely been reported. A 33-year-old male presented with upper back pain, a rapidly developing mass beside the lateral border of his right scapula and snapping elicited by movement of the scapula. Plain radiograms and CT revealed osteochondroma on the ventral surface of the scapula without any unmineralized component and a huge cystic lesion around the osteochondroma. Aspiration of the cystic lesion showed the presence of sero-sanguineous fluid. MRI following the aspiration showed a thin cartilaginous cap with distinct outer margin and no soft tissue mass around the cap. Pathological examinations confirmed the diagnosis of osteochondroma with the large bursa formation. Clinical examination 19 months postoperatively showed an uneventful clinical course.

The computed tomography (CT) and magnetic resonance imaging (MRI) results of 30 consecutive patients with tumours in the skull base, and who had abnormalities of the pterygopalatine fossa (PPF) on CT or MRI, were retrospectively compared with respect to visualization of tumour infiltration into the PPF. CT did not depict the abnormalities in the PPF in five patients (17%), while unenhanced T1-weighted MR images depicted tumour infiltration in all patients. Obliteration of PPF fat was better visualized on CT than T2-weighted and proton density weighted MR images, as were bony abnormalities. On MRI, intracranial extension was seen in eight of 25 patients with extracranial tumour. MRI is a sensitive method of demonstrating both tumour infiltration of the PPF and perineural tumour spread.

We compared MR imaging with CT in revealing tumor infiltration of bone and bone marrow in the skull base of 54 patients. MR imaging had no advantages over when tumor involved the anterior compartment. However, precontrast T1-weighted MR images were more efficient than CT in 37.5% of tumors involving the middle compartment and in 54.5% of tumors involving the posterior compartment, respectively. Precontrast T1-weighted images were more accurate than other pulse sequences in revealing bone and bone marrow that were replaced by tumors.

The purpose of this paper is to retrospectively evaluate CT and MR findings of central nervous system (CNS) involvement of leukemia and systemic lymphoma in children. Over a 12-year period, sixty-five patients with leukemia and fifteen patients with systemic lymphoma underwent cerebral CT and/or MR imaging. Nine patients (11.3%) were diagnosed as CNS involvement of leukemia and lymphoma. The diagnostic criteria of CNS involvement were as follows; 1) Histological proof was confirmed by surgery, 2) Tumor cells were found in the cerebrospinal fluid examinations, 3) Increase in size of the lesion during observation without specific treatment, and 4) Response to the treatment for leukemia or lymphoma. All of nine patients fulfilled more than two criteria of 1)-4). The CT and MR abnormalities in these patients were correlated with the findings of histology, cerebrospinal fluid cytology, and/or treatment. The age of the patients ranged from 0 to 15 years old. They consisted of 6 boys and 3 girls. The CT examinations were performed before and after contrast administration. MR examinations were performed on a 1.5-T unit, and T1-weighted, T2-weighted, and proton density-weighted images were obtained using spin-echo or fast spin-echo sequences. Tumor masses were present in seven with leukemia (acute lymphoblastic leukemia 4; acute myeloblastic leukemia 1; acute promyelocytic leukemia 1; acute monocytic leukemia 1), and in two with malignant lymphoma. On the CT scan, tumor masses were hyperdense with contrast enhancement. On the MR images, their signals were variable. In all of nine patients, tumor masses were contiguous with a meningeal surface. Postcontrast T1 weighted images were valuable in demonstrating meningeal infiltration. Tumoral hemorrhage was found in two patients. In a patient with tumor at the superior sagittal sinus, venous infarct was observed. CNS leukemic and lymphomatous masses are almost hyperdense on the CT and they are characteristically contiguous with a meningeal surface. MR imaging was valuable in demonstrating meningeal infiltration. Findings of CT and MR imaging, cerebrospinal fluid examinations, and response to the treatment are useful in the differentiation of CNS involvement of leukemia and lymphoma from other lesions such as infectious diseases and leukoencephalopathy.

The fast spin-echo (FSE) technique has been a successful alternative to conventional spin-echo (CSE) imaging in the brain, spine, and pelvis, but not in the knee. This study evaluated the performance of the FSE technique in comparison with the established CSE technique as a reference standard. Oblique coronal images of 30 shoulders were obtained by both FSE and CSE techniques. The FSE images were compared with CSE images in terms of blurring and motion artifact, fat signal intensity, structural conspicuity, and visualization of joint effusion and rotator cuff tears. We imaged 30 consecutive patients with suspected rotator cuff tears who were referred for MR examination. FSE images often were worse in blurring and motion artifact than CSE images. FSE images showed higher fat signal intensity, but visualized structural conspicuity, joint effusion, and rotator cuff tear as well as CSE images. Our results suggest that the FSE technique is not equal to the CSE technique but can be used for the diagnosis of rotator cuff tears.

Thyroid tumors affect adjacent structures and reveal their nature as they grow. Differences in benign and malignant tumor growth patterns were studied by CT. Twenty-nine benign (follicular adenoma) and 29 malignant (papillary carcinoma) tumors with their findings were arranged according to size to speculate on how they grew. Benign tumors grew expansively and made a beak-like appearance in the marginal thyroid tissue (beak sign). They displaced vessels but kept a fat plane or deep sulcus between them (sulcus sign). Malignant tumors, on the other hand, grew invasively and reached the trachea rapidly without the beak sign. They destroyed the fat plane and contacted the vessels without the sulcus sign. Expansive and invasive growth forms of benign and malignant thyroid tumors could be represented schematically.

We present a case of thrombophlebitis of the inferior vena cava (IVC) with Crohn's disease after intestinal perforation and prolonged indwelling of a catheter in the IVC. Magnetic resonance imaging demonstrated abnormal thickening and enhancement of the IVC wall. In addition, IVC thrombus formation was shown. The abnormal enhancement extended from the wall in the pericaval tissue and into the retroperitoneum, and regressed as the Crohn's disease subsided.

We connected two 3-inch coils to a dual phased array receiver system and sandwiched the shoulder between the two coils. To obtain a quantitative assessment of the improved SNR, we imaged a phantom with both the dual phased array coils and the standard shoulder coil. SNR as a function of distance from the coils was computed by the National Electrical Manufacturers Association (NEMA) standard. At the expected depth of the humeral head, the SNR of the phased array coils on single excitation was 1.42 times that of the standard shoulder coil on two excitations. The higher quality of the images on single excitation shortened the imaging procedure. The dual phased array coils realized the minimum FOV: 8 cm with a pixel of 0.31 x 0.41 mm. The dual phased array coils achieved high spatial resolution images of the shoulder with significantly shorter imaging times.

Infantile and maternal choriocarcinoma is a very rare disease. We report a case with the characteristic clinical features of infantile choriocarcinoma: developing anemia, hemorrhagic liver tumors, rapid progression to death and maternal choriocarcinoma. Bone scintigraphy showed increased uptake by the liver tumors. In this case there were two possible primary sites: the placenta of this pregnancy and a hydatidiform mole that had been present 2 years previously.

During a 3-year period, 6 of 50 children with systemic malignacies developed neurologic complications such as hemiparesis, convulsions and loss of consciousness. The children consisted of 1 boy and 5 girls, from 3 to 12 years old, 3 with acute lymphoblastic leukemia and 3 with malignant lymphoma. Four patients received induction treatment that included intravenous administration of L-asparaginase and/or intrathecal administration of methotrexate. One patient received induction treatment and consolidation treatment that included intravenous administration of L-asparaginase. One patient received induction and consolidation treatment, and the protocol for peripheral blood stem cell transplantation. Laboratory examinations revealed coagulation dysfunction in 3 patients treated with L-asparaginase and 1 patient with disseminated intravascular coagulation (DIC). Magnetic resonance imaging (MRI) was performed on a 1.5-T unit, using spin-echo or fast spin-echo sequences. T1-weighted, T2-weighted, and proton density-weighted images were obtained in the axial and/or coronal plane (section thickness, 4 mm; inter-section gap, 2 mm). MRI was initially performed within 36 hours after the onset in all patients, and follow-up MRIs were performed for 6 months. MRI showed lesions involving the cortex and subcortex in 4 patients with coagulation dysfunction. In 2 of these 4 patients, Gd-enhanced T1-weighted images showed contrast enhancement in the surface of the gyrus, suggesting focal vascular stasis. Serial MRI revealed nearly complete resolution of the lesions. Symptoms were relieved in every case. The lesions on MRI were presumed to be due to venous thrombosis related to the coagulation dysfunction caused by L-asparaginase or DIC. On the other hand, in 2 patients with onset after intrathecal administration of high-dose methotrexate and cytarabine, MRI revealed multiple lesions involving the centrum semiovale and periventricular white matter. No Gd-enhancement of the lesion was detected. This MRI finding was consistent with leukoencephalopathy. As time passed, the symptoms improved completely, and the lesions became better demarcated. MRI is useful for differentiating lesions related to coagulation dysfunction from leukoencephalopathy.

Purpose: To determine the efficacy of superselective angio-CT in the diagnosis of astrocytoma., Methods: Nineteen patients with astrocytoma had superselective angio-CT before chemotherapeutic agents were administered via superselective intraarterial infusion. CT was performed after contrast material was delivered through a microcatheter, which had been advanced into the feeding arteries of the tumor. Superselective angio-CT scans were compared with digital subtraction angiograms, conventional contrast-enhanced CT scans, and contrast-enhanced T1-weighted MR images., Results: Superselective angio-CT scans depicted contrast enhancement of the tumor in all of the patients and the medullary veins of the tumors in 32% of the patients. Digital subtraction angiograms showed tumor stains in 68% of the patients and the medullary veins in only 5%. Conventional CT scans and MR images showed contrast enhancement of the tumor in 89% of the patients. Superselective angio-CT scans confirmed the proper position of the catheter tip for the infusion of a chemotherapeutic agent., Conclusions: Superselective angio-CT can be used to depict contrast enhancement of tumors and the vascular structures that are characteristic of astrocytomas.

The increasing use of interventional angiography makes it important to know the anatomy of the maxillary artery. The course and ramification pattern of the first and second segments of this artery were studied angiographically in 100 cases. The branches of the maxillary artery were identified on lateral stereoscopic images. The maxillary artery has three segments divided by the external pterygoid muscle, and the second of these segments divides into superficial and deep types. Of the 100 Japanese angiograms, 93% were of the superficial type and 7% of the deep type. The 93% was a much higher incidence than in Western populations. In the superficial types, the middle deep temporal artery and inferior alveolar artery arose separately from the maxillary artery, while in the deep types, they had a common trunk. This made it easy to differentiate the two types. The maxillary artery was classified into four subgroups (Ia, Ib, IIa and IIb) by the ramification patterns of the middle meningeal and accessory meningeal arteries from the first segment.

We evaluated normal variations of the temporal bone on high-resolution computed tomograms (HR-CTs) and investigated their incidence. HR-CTs of the temporal bones of 325 patients were retrospectively reviewed. Six groups of variants, which were considered important for presurgical planning, were evaluated on HR-CTs. These included: (1) an incomplete bony covering of a high-positioned jugular bulb; (2) severe asymmetry of the jugular foramen; (3) an anteriorly located sigmoid sinus; (4) a deep sinus tympani; (5) a large internal auditory canal; and (6) a large cochlear aqueduct. The frequency of the variations were as follows: (1) 2.4%; (2) 4.0%; (3) 1.6%; (4) 5.9%; (5) 2.3%; and (6) 3.0%. Bilateral involvement with variation (4), (5) and (6) was frequently seen. Normal anatomical variations of the temporal bone are therefore not rare and awareness of the possible variants is necessary before surgery of the inner ear, middle ear and posterior fossa.
