Bakgrund: Hinnsvepning är en icke-farmakologisk induktionsmetod som används inom förlossningsvården med önskan om att initiera en förlossningsstart. Svenska föreningen för obstetrik och gynekologi [SFOG] rekommenderar hinnsvepning i fullgången graviditet. Totalt inkluderades 100 kvinnors förlossningsjournaler. Resultat: Av de 100 kvinnorna som hinnsveptes inducerades 41 med annan induktionsmetod och 33 kvinnor blev överburna. Paritet hade inte några statistiskt signifikanta samband med varken induktion, tid till aktiv förlossningsfas eller överburenhet, till skillnad från Bishop Score som hade ett statistiskt signifikant samband med både induktion, tid till aktiv förlossningsfas och överburenhet. Ett statistiskt signifikant samband kunde även ses mellan BMI och överburenhet samt tid till aktiv förlossningsfas. Bishop Score var den variabeln med starkast samband till induktion, tid till aktiv förlossningsfas och överburenhet. Konklusion: Kvinnans Bishop Score och BMI bör inte negligeras i samband att hinnsvepning genomförs då det kan påverka utfallet. BACKGROUNDWomen eligible for vaginal birth after cesarean section (VBAC) have lower morbidity rates than women who undergo subsequent elective cesarean sections.Objective:To identify the obstetric parameters those influence the success of vaginal delivery in women with previous cesarean section.METHODOLOGYThis descriptive cross sectional study was conducted at Gynaecology and Obstetrics Department of Khyber Teaching Hospital, Peshawar from 1st May 2015 to 31st April 2016.Inclusion criteria was all women of any age or parity presenting at >36weeks gestational age, with singleton pregnancy, vertex presentation, estimated fetal weight of 2.5 -3.5kg and documented previous lower uterine segment cesarean section for a non recurrent cause. Five predictors of success of vaginal birth after previous cesarean including maternal age, gestational age, and history of vaginal delivery, onset of labor and bishop score were evaluated in each patient.RESULTSOut of 100 women, 64 had VBAC and 36 had cesarean section. Fifty five were booked and 45 were non booked. Total women with history of VBAC were 24, out of these 16(66%) had VBAC and 8(33%) had repeat cesarean section. Ninety three had spontaneous onset of labor whereas 7 were induced,88 patients had Bishop score >5,out of these 61(69%) ended in VBAC and 27(30.6%) had cesarean section. The most common age group where VBAC occurred in majority was 25- 35 years. Thirty nine (61%) women had period of gestation between 38weeks to 39weeks+6days. Mean maternal age was 29.42 + 3.54 yearsCONCLUSIONHistory of vaginal delivery, spontaneous sonset of labor and Bishop score >5 are the factors which are associated with more chances of vaginal delivery after cesarean section.

Methods: Cohort study of twin pregnancies >= 34 weeks, planned for vaginal delivery, from two University Hospitals in Sweden. Data were collected from medical records during the periods 1994 (Orebro) and 2004 (Uppsala) to 2013. During the study period there were 78,180 live born births and 1,282 were twin births. Women with previous caesarean section were excluded. Induction methods were categorized into amniotomy, oxytocin and cervical ripening (intra cervical Foley catheter or prostaglandin). Adjusted odds ratios (AOR) with 95 % confidence interval (CI) for caesarean section were calculated by logistic regression and were adjusted for parity, maternal age, gestational length, complications to the pregnancy, infant birth weight and year of birth. Spontaneous labour onsets were used as the reference group. The main outcome measure was caesarean section. Results: In 462 twin pregnancies, 220 (48 %) had induction of labour and 242 (52 %) a spontaneous labour onset. Amniotomy was performed in 149 (68 %) of these inductions, oxytocin was administered in 11 (5 %) and cervical ripening was used in 60 (27 %). The rate of caesarean sections was 21 % in induced and 12 % in spontaneous labours (p 0.01). The absolute risk of caesarean section following induction was: 15 % with amniotomy; 36 % with oxytocin and 37 % with Foley/prostaglandin. Induction of labour increased the risk of caesarean section by 90 % compared with spontaneous labour onset (AOR 1.9, 95 % CI 1.1-3.5) and, when cervical ripening was used, the risk increased more than two fold (AOR 2.5, 95 % CI 1.2-5.3). Background: Induction of labour is associated with increased risk for caesarean delivery among nulliparous women. The aims of this study were to evaluate the risk factors for caesarean delivery and to investigate the risk of maternal and neonatal infections in nulliparous women undergoing induction of labour by Foley catheter. Methods: This clinical retrospective study of 432 nulliparous women with singleton pregnancy and intact amniotic membranes at or beyond 37 gestational weeks scheduled for induction of labour by Foley catheter was conducted over the course of one year, between January 2012 and January 2013, in Helsinki University Hospital. The main outcome measures were caesarean section rate and maternal and neonatal infections. Univariate and multivariate logistic regressions were used to estimate relative risks by odds ratios with 95 % confidence intervals. Results: The caesarean section rate was 39.1 % (n = 169). In multivariate regression analysis, the factors associated with caesarean section were the need for oxytocin for labour induction [OR 2.9 (95 % CI 1.8-4.5) p <0.001] and early epidural analgesia [OR 9.9 (95 % CI 2.1-47.5), p = 0.004]. The maternal intrapartum infection rate was 6.3 %, and the clinical neonatal infection rate was 2.8 %. In multivariate analysis, gestational diabetes was associated with maternal intrapartum infection [OR 4.3 (95 % CI 1.7-11.0, p = 0.002] and early epidural analgesia with neonatal clinical sepsis [OR 10.5 (95 % CI 1.4-76), p = 0.02]. Conclusions: Oxytocin induction and early epidural analgesia were associated with caesarean delivery. Gestational diabetes and early epidural analgesia were associated with infectious morbidity. Since the first caesarean delivery has a major impact on subsequent pregnancies, optimising labour induction among nulliparous women is important.

Background: Induction of labour is a common obstetric procedure. At present, different methods are used for induction of labour in women with an unfavourable cervix. Recently, we showed that in term women with an unfavorable cervix the use of a Foley catheter in comparison with vaginal Prostaglandin E2 gel, results in a comparable vaginal delivery rate. A meta-analysis on the subject indicated lower rates of hyperstimulation, and probably as a sequel fewer cases of postpartum haemorrhage. Misoprostol (PgE1) is another type of prostaglandin frequently used for labour induction, recommended by the international federation of gynaecology and obstetrics (FIGO). Misoprostol can be administered by vaginal, rectal and oral route. There is evidence that oral administration results in less asphyxia and hyperstimulation than vaginal administration. At present, valid comparisons between oral misoprostol and Foley catheter are lacking. Therefore, we propose a randomised controlled trial comparing Foley catheter to oral misoprostol in order to assess safety and cost-effectiveness.Methods/Design: We plan a multicentre, randomised, controlled, open-label clinical trial among term pregnant women with a vital singleton in cephalic presentation, unfavorable cervix, intact membranes and an indication for induction of labour. Objective: The natural distribution and predictive accuracy of Bishop scores was evaluated to predict cesarean delivery (CD) in nulliparas between 37 and 42 weeks gestation. Study design: Subjects underwent serial digital cervical examinations. The Bishop score was evaluated as a binary and continuous factor to predict CD at each gestational week beginning at 37 weeks. Bishop scores were categorized as 5 or 45, and CD rates were compared across Bishop score categories using chi-square or Fisher exact tests at each gestational week beginning at 37 weeks. Results: In all, 171 patients were prospectively followed. The overall CD rate was 27.5%. The prevalence of unfavorable Bishop scores, categorized as 5, decreased with increasing gestation age until 41 weeks. CD rates for the cohort with unfavorable Bishop scores was higher than those with favorable scores at each week. The likelihood ratio for CD was 1.35-2.00, depending on gestational age. The Bishop score that best predicted subsequent vaginal delivery following expectant management was 43 at 37 weeks and 45 at 39 weeks. Conclusion. Bakgrund Att inducera en förlossning innebär att försöka manipulera igång ett förlossningsarbete i syfte att åstadkomma en vaginal förlossning. Nyttan med induktionen måste alltid överväga riskerna för förlossningskomplikationer. Riksgenomsnittet för inducerade förlossningar var år 2007, 13,2 %. Syfte Kartlägga inducerade förlossningar på en förlossningsklinik i södra Sverige under året 2009. Kartläggningen inbegrep induktionsindikation, använda induktionsmetoder samt förlossningsförlopp och utfall. Metod Retrospektiv observationsstudie där samtliga inducerade förlossningar med levande foster vid induktionsstarten under 2009 på förlossningskliniken inkluderades. Detta gav ett underlag på 306 inducerade förlossningar. Chi-två test och Fisher´s exakta test användes för att testa skillnader i resultatet. Resultat Indikationerna till förlossningsinduktion fördelade sig så att 250 (82 %) utgjorde medicinsk indikation, där vattenavgång utgjorde den främsta anledningen, och 56 (18 %) på humanitär indikation. Det var 220 kvinnor som krävde minst ett upprepat induktionsförsök innan de kom i aktivt förlossningsarbete. Det fanns en signifikant skillnad som talade för att ju högre Bishop score det var vid första induktionsförsöket, desto kortare tid till partus. Förlossningsutfallet blev 71% partus normalis, 18% sectio, 6% vakuum extraktion och 5% partus cum haem. Slutsats Studien visade att det kan finnas anledning för förlossningskliniken att förbättra journalföringen och öka följsamheten av PM vid förlossningsinduktioner., Background To induce labour means trying to manipulate labour to start in order to achieve vaginal childbirth. The benefits of induction must always outweigh the risk of labour complications. The national average of induced labour in 2007 was 13, 2%. Aim To map induced labours at a childbirth clinic in Southern Sweden during 2009. The mapping included induction indication, used methods of induction, childbirth processes and outcome. Method Retrospective observation study which included all induced labours with living foetuses at the start of induction, at the childbirth clinic during 2009. This gave a basis of 306 induced labours. Chi-squared testing and Fisher’s exact testing was used to test significant differences in the result. Result Indications of labour induction were divided where 250 (82%) constituted medical indication, the water breaking constituting the main reason, and 56 (18%) humanitarian indication. 220 women required at least one repeated induction try before they were in active labour. There was a significant difference indicating that the higher the Bishop Score was at the first induction try, the shorter the time until delivery. Labour outcome was 71% partum normalis, 18% caesarean sections, 6% vacuum extractions and 5% post partum haemorrhages. Conclusion The study showed that there might be reasons to improve the journal keeping and follow the PMs regarding labour induction at the childbirth clinic.

Metod Retrospektiv observationsstudie där samtliga inducerade förlossningar med levande foster vid induktionsstarten under 2009 på förlossningskliniken inkluderades. Detta gav ett underlag på 306 inducerade förlossningar. Chi-två test och Fisher´s exakta test användes för att testa skillnader i resultatet. Resultat Indikationerna till förlossningsinduktion fördelade sig så att 250 (82 %) utgjorde medicinsk indikation, där vattenavgång utgjorde den främsta anledningen, och 56 (18 %) på humanitär indikation. Det var 220 kvinnor som krävde minst ett upprepat induktionsförsök innan de kom i aktivt förlossningsarbete. Det fanns en signifikant skillnad som talade för att ju högre Bishop score det var vid första induktionsförsöket, desto kortare tid till partus. Förlossningsutfallet blev 71% partus normalis, 18% sectio, 6% vakuum extraktion och 5% partus cum haem. Slutsats Studien visade att det kan finnas anledning för förlossningskliniken att förbättra journalföringen och öka följsamheten av PM vid förlossningsinduktioner., Background To induce labour means trying to manipulate labour to start in order to achieve vaginal childbirth. The benefits of induction must always outweigh the risk of labour complications. The national average of induced labour in 2007 was 13, 2%. Aim To map induced labours at a childbirth clinic in Southern Sweden during 2009. The mapping included induction indication, used methods of induction, childbirth processes and outcome. Method Retrospective observation study which included all induced labours with living foetuses at the start of induction, at the childbirth clinic during 2009. This gave a basis of 306 induced labours. Chi-squared testing and Fisher’s exact testing was used to test significant differences in the result. Result Indications of labour induction were divided where 250 (82%) constituted medical indication, the water breaking constituting the main reason, and 56 (18%) humanitarian indication. 220 women required at least one repeated induction try before they were in active labour. There was a significant difference indicating that the higher the Bishop Score was at the first induction try, the shorter the time until delivery. Labour outcome was 71% partum normalis, 18% caesarean sections, 6% vacuum extractions and 5% post partum haemorrhages. Conclusion The study showed that there might be reasons to improve the journal keeping and follow the PMs regarding labour induction at the childbirth clinic.
