Methiopropamine is a structural analog of methamphetamine that is categorized as a novel psychoactive substance. It primarily acts as a norepinephrine-dopamine reuptake inhibitor and, secondarily, as a serotonin reuptake inhibitor. In humans, methiopropamine induces stimulation and alertness and increases focus and energy. However, significant side effects are reported, such as tachycardia, anxiety, panic attacks, perspiration, headache, and difficulty in breathing. To date, little data is available regarding its pharmacodynamic effects, thereby we aimed to investigate the acute in vivo effects induced by this drug on sensorimotor responses, body temperature, pain thresholds, motor activity, and cardiovascular and respiratory systems in CD-1 male mice. We selected a range of doses that correspond to the whole range of human reported use, in order to evaluate the threshold of adverse effects presentation. This study demonstrates that methiopropamine acts as a dopaminergic and noradrenergic stimulating drug and that the highest doses (10-30 mg/kg) impair the visual placing response, facilitate the acoustic and tactile response, induce hypothermia, increase mechanical and thermal analgesia, stimulate locomotor activity, induce motor stereotypies, and strongly affected cardiovascular and respiratory parameters, increasing heart rate, breath rate, and blood pressure but reducing oxygen saturation. On the contrary, lower doses do not show any of those effects. We hypothesize that there is a range of doses that do enhance performance but do not seem hazardous to users: this gap could induce the perception of safety and increase the abuser population.

Aims: to evaluate the prevalence of psychotropic substances and stimulants among professional athletes, submitted to antidoping controls. Methods: over 102,400 urine samples from professional athletes were tested during the period 2000-2010 in the Italian anti-doping laboratory, using specific methods developed and validated according to the ISO 17025 norm. Results were evaluated and discussed with particular attention to stimulants and psychotropic substances. Results: the drugs of abuse detected among athletes more frequently were THC, with a mean 17.6% prevalence among other adverse analytical findings (AAF), and cocaine, with a mean 6.7% prevalence among other AAFs. Only a few cases showed the presence of amphetamine or methamphetamine (0.5%), while no amphetamine-like designer drugs were found. The Adverse Analytical Findings for opiates, mainly morphine above the cut-off (1ìg/ml) were 1.3% among other AAFs; in none of the samples O-6-monoacetylmorphine was found, while 90% of them was compatible with codeine intake. The positive results obtained were compared with data on consumption of drugs from the general population in the same age groups, and with the trends in drug seizures in the same period, showing a similar trend in both cases. Conclusions: the results obtained , being related to a particular segment of the population that is supposed to be more conscious of healthy lifestyles and of the possibility to be submitted to drug testing , can be considered as a useful indicator of the diffusion of drugs of abuse in the general youth population.

A method for the toxicological screening of the new designer drug methylenedioxypyrovalerone (MDPV) is described; with an emphasis on its application for anti-doping analysis. The metabolism of MDPV was evaluated in vitro using human liver microsomes and S9 cellular fractions for CYP450 phase I and uridine 5'-diphosphoglucuronosyltransferase (UGT) and sulfotransferase (SULT) phase II metabolism studies. The resulting metabolites were subsequently liquid/liquid extracted and analyzed using gas chromatography/mass spectrometry (GC/MS) as trimethylsilyl (TMS) derivatives. The structures of the metabolites were further confirmed by accurate mass measurement using a liquid chromatography/quadrupole time-of-flight (LC/QTOF) mass spectrometer. The studies demonstrated that the main metabolites of MDPV are catechol and methyl catechol pyrovalerone, which are in turn sulfated and glucuronated. The method for the determination of MDPV in urine has been fully validated by assessing the limits of detection and quantification, linearity, repeatability, and accuracy. This validation demonstrates the suitability for screening of this stimulant substance for anti-doping and forensic toxicology purposes.

Stimulants are banned by the World Anti-Doping Agency (WADA) if used “in competition”. Being the analysis of stimulants presently carried out on urine samples only, it might be useful, for a better interpretation of analytical data, to discriminate between an early intake of the substance and an administration specifically aimed to improve the sport performance. The purpose of the study was to investigate the differences, in terms of excretion/disappearance of drugs, between urine and oral fluid, a sample that can reflect plasmatic concentrations. Oral fluid and urine samples were collected following oral administration of the following stimulants: modafinil (100 mg), selegiline (10 mg), crotetamide/cropropamide (50mg each), pentetrazol (100 mg), ephedrine (12 mg), sibutramine (10 mg), mate de coca (a dose containing about 3mg of cocaine); analysis of drugs/metabolites was carried out by gas chromatography/mass spectrometry (GC/MS) in both body fluids. Our results showthat both the absolute concentrations and their variation as a function of time, in urine and in oral fluid, are generally markedly different, being the drugs eliminated from urinemuch more slowly than from oral fluid. Our results also suggest that the analysis of oral fluid could be used to successfully complement the data obtained from urine for “in competition” anti-doping tests; in all those cases in which the metabolite(s) concentration of a substance in urine is very low and the parent compound is not detected, it is indeed impossible, relying on urinary data only, to discriminate between recent administrations of small doses and remote administrations of higher doses.

A gas chromatographic/mass spectrometric (GC/MS) study aimed at identifying the metabolites of sibutramine (1-(4-chlorophenyl)-N,N-dimethyl-a-(2-methylpropyl)cyclobutanemethanamine) in urine is described. Urinary excretion of sibutramine metabolites following the oral administration of a single dose of sibutramine was followed by GC/MS analysis. After identification of the chromatographic signals corresponding to the six main urinary metabolites, the fragmentation pattern was studied in electron ionization (EI) mode after derivatization to the corresponding methyl and trimethylsilyl derivatives. Urine samples were pretreated according to a reference procedure (liquid/liquid separation, enzymatic hydrolysis, pre-concentration under a stream of nitrogen and derivatization, either under thermal incubation and by microwave irradiation). All sibutramine metabolites were excreted as glucuroconjugates, and retain the chiral carbon present in the sibutramine skeleton. The metabolites identified included mono-desmethylsibutramine (norsibutramine), bi-desmethylsibutramine (nor-nor-sibutramine), and the corresponding hydroxylated compounds, the hydroxylation taking place either on the cyclobutane or on the isopropyl chain. The excretion profiles of the different metabolites were also evaluated. From an analytical point of view, the method can be applied to different fields of forensic analytical toxicology, including anti-doping analysis. Although the lack of certified reference materials does not allow a precise determination of the limits of detection (LODs) of all the sibutramine metabolites, an estimation taking into account the response factor of similar compounds ensures that all metabolites are still clearly detectable in a range of concentrations between 10 and 50 ng/mL, thus satisfying the minimum required performance limits (MRPLs) of the World Anti-Doping Agency (WADA).

Under current IOC (International Olympic Committee) regulations an athletes dope test is only described as positive if the actual presence (in some cases in concentrations above a certain threshold limit) can be identified of drugs and/or their metabolites which appear on the IOC's own list of prohibited substances and methods. The present paper describes the use of keratin matrices for antidoping test purposes and discusses their potential and limitations. Particular attention is paid to analysing hair samples for traces of drugs and/or metabolites with reference to the specific classes of prohibited substances. The possibility of using hair sample analysis as a means of distinguishing between the occasional and regular intake of given drugs is examined in particular detail in the belief that it may be possible to obtain experimental data of a pharmacokinetic nature which would make it possible to differentiate between the illegal use of a prohibited substance for performance-enhancing purpose and its ingestion for legitimate medical reasons. Finally the paper examines aspects relating to the applicability of hair tests in the framework of the IOC's anti-doping surveillance with emphasis on the importance of guaranteeing at least the same level of confidentiality, reliability and reproducibility as currently apply to urine tests
