1. Investigation, findings and recommendations into shaft related incidents at President Steyn mine.

- Abstract
Reports of incidents between April 1993 and March 1995 at the mine in South Africa were analysed and the root causes identified. These could be divided into three categories: lack of self-discipline and/or supervision, poor previous mining practices and inadequate system provided. 72% of accidents had lack of self-discipline and/or supervision as their root cause. These were found to be related to poor training. 56% of incidents occurred between 6 pm and 2 am. Current practices, procedures and special instructions were also investigated to prevent potential shaft incidents. It is recommended that training courses should be undertaken, poor maintenance practices be detected, a monthly system meeting be held at each shaft and supervision be increased on afternoon and night shifts.
